We Are Community!

“I have been clean from meth for 14 months, and getting my teeth removed is the last thing I need to do to move forward with my life,” my patient stated during her triage screening at OkMOM this past February. It was a bittersweet moment, for while she was crying when making this statement, she was also smiling through the tears. I heard many similar stories from the hundreds of patients who traveled from all over Oklahoma and beyond the borders. Each warmed my soul, as I knew they were getting the care they desperately needed. After a two-year hiatus, it was gratifying to be back in the community providing treatment at OkMOM in Shawnee. Community involvement is one of the many reasons people choose the field of dentistry. The Oklahoma Dental Association provides many opportunities for our members to help, to care for, and to improve the lives of those around us.

is a personal favorite of mine! Since it’s a reporting year, for all the procrastinators, Drs. Braly and Jones have you covered with our required opioids and ethics courses. I know my residents will be interested in Dr. Susan McMahon’s CE on “A New Generation for Cosmetic Dentistry: Delivering the Selfie-Ready Smile,” as well as the keynote speaker, Kate Germano, sharing her leadership journey and experiences. With so many options, remember to save time for the vendors and chatting with classmates and colleagues come together as a community.

Since 2010, through OkMOM has provided more than 103,597 procedures and our community treated 17,381 patients, totaling 14.5 million in donated dental treatment, which breaks down to about $863 per patient! The legacy of OkMOM is broad and impressive. The whole of the dental community coming together and contributing is life changing for the patients and the practitioners. The ODA is currently working to secure continuing financial support for OkMOM for years to come through the OkMOM Legacy Fund. By reaching our $448,000 goal (which is doubled by a matching gift!), we are ensuring that communities around the state of Oklahoma are supported and cared for indefinitely. If attending is out of reach, think about donating! Together, we can ensure the services provided at OkMOM are not a given but a guarantee.

The premier dental meeting in the state of Oklahoma is happening in Tulsa on April 28th and 29th. I enjoy this time for the dental community to come together and support each other. This year, we are featuring many topics at the forefront of dental education. Dr. David Clark will discuss injection molded composite dentistry, and Dr. Rick Cardoza is discussing laser-assisted dentistry. If you want to enjoy the classics, then check out Dr. Bill Robbins' “Global Diagnosis for the 21st Century” or Dr. Kathleen Higgins' review on “What’s That White Lesion?”. The ODA provides something for everyone, including hygiene. Nancy Miller will discuss many topics including “Power off Biofilm” and the business side of hygiene. Donna Miller will discuss the inflammatory side of dentistry, which

The ODA also advocates for our members and the whole dental community. The tireless members of the ODA are working to pass a bill that would address the medical loss ratio (MLR). The MLR is the minimum percentage of all premiums collected by an insurer each year that must be spent on actual patient care rather than overhead costs. If passed by the legislature, this will set the MLR at 80% of premiums for patient care or rebate it back to the premium payers (our patients). This regulation is present in medical plans and the ACA, but not in dental plans, contributing to our consistent and suffocating fight for coverage of our patients' treatment. This bill was passed in Massachusetts, with a 72% approval rating from voters. With our help, by reaching out to our state representatives, we can make this a reality in Oklahoma, as well.

Community involvement and improvement are a core attributes of the ODA. Helping people and improving their lives is as much a part of the practice of dentistry as radiographs. We cannot do one without the other, and the ODA is here to support both the practice of dentistry and its impact on the communities we serve. Please check out the many opportunities to get involved, or let us know if there is a need that we are not meeting. The ODA is here to impact the community of dentistry.

FROM THE ODA PRESIDENT

I am looking forward to seeing all of you at the upcoming annual meeting! The ODA Annual Meeting promises to be full of great times, wonderful CE, and opportunities to connect with a wide variety of vendors. The meeting has really come together nicely, and I’m confident you will get a great CE line-up to go along with the fun times at our many events.

We recently wrapped the Mission of Mercy in Shawnee and it was a rousing success. After two years of trying and rescheduling, the community was exceedingly welcoming, and we were able to provide dental care to many patients in need. I was blown away at the outpouring of volunteers—dentists and their teammates alike. I’d like to send a huge shout-out to all the tear-down volunteers who really helped expedite the process on Saturday night. Dr. Brandon Loeser did a marvelous job after several years of planning! Drs. Rieger Wood and Chris Fagan and the other Leads helped us treat 765 patients and provide $1 million in free dental care. I would also like to recognize the ODA and ODF central office staff, along with our co-producer, Delta Dental of Oklahoma Foundation, for their contributions to making the return of OkMOM such a resounding success. Next year’s event will be in Lawton, February 9 & 10 so make your plans now!

I was particularly touched to see the Mission of Mercy, which Dr. Paul Mullasseril originally planned over three years ago, assisted by Dr. Fagan, finally come to fruition. I remain absolutely astounded at the generosity shown by Dr. Tim and Mrs. Pamela Fagan in establishing our OkMOM Legacy Fund. I invite all of you to be generous in this campaign. The leadership and example shown to us by the Fagans is a beacon of selflessness, and I hope you all join me in doing our part to ensure that legacy continues.

The legislative session is well underway, and I assure you that your ODA is very busy representing our patients and our profession across a variety of different legislative issues. Several bills could impact dentistry; specifically, we are most engaged on a bill regarding medical loss ratios. You have probably heard by now that we are pushing for dental insurance carriers and dental benefit providers to spend a minimum of 80% of their premium dollars on claims for dental care for insured patients. We will continue to stay engaged on this and many other issues and will reach out to you through legislative alerts. We need your help to be successful!

This will be my final journal letter, and I am extremely honored to have had the chance to work with many of Oklahoma’s finest dentists and other professionals over the last year. I

am always so proud of the contributions that our members and our staff make in every aspect of their careers. As a whole, our profession has done so much to take care of Oklahomans, and I continue to hear story after story of the great things our members do. I am excited and optimistic about the coming year under the leadership of Dr. Paul Wood, along with an excellent slate of officers. I was blessed to have so much momentum from the executive committees of the past several years who so expertly guided our ODA through a variety of challenging issues and times, including COVID-19. It’s been so rewarding to see the projects they initiated carried out, and their mark endures.

GET TO KNOW YOUR INCOMING ODA PRESIDENT PAUL WOOD, DDS

It is my great honor to be your incoming president of the Oklahoma Dental Association. I expect a very eventful term with all the things that are planned to benefit the association. The first thing you need to know is that all the work is done by Lynn Means and her staff. Without them, nothing of value for the ODA would be possible. The Oklahoma Dental Association represents all dental professionals in Oklahoma, regardless of who, where, or how you provide dentistry to your patients. We are all in this together.

I grew up in rural Vernon County, Missouri and lived in a small town life that was very enjoyable. I was truly fortunate. I joined the U.S. Army at the age of 17 and proceeded to Fort Dix for basic training, and on to Fort Lee for Advanced Individual Training. I spent five years enlisted and then enrolled in ROTC at Missouri Southern State University. I was commissioned as an O-1 Second Lieutenant U.S. Army Field Artillery. I attended the Officer Basic Course in Field Artillery and Military Intelligence and was stationed at Fort Huachuca, Arizona. I was accepted into the University of Missouri at Kansas City School of Dentistry and transferred to the Missouri Army National Guard. While in dental school, I served as a U.S. Army Captain Field Artillery and was Commander of Service Battery 1st of the 129th Field Artillery.

Upon graduation from dental school, I accepted a commission in the Commission Corp of the United States Public Health Service and was stationed at the WW Hastings Indian Hospital in Tahlequah, Oklahoma. I worked as a staff dentist, spent time in the OR with the pediatric dentist, and traveled to Ada to work with the oral surgeon. I then applied for a long-term training opportunity in Oral and Maxillofacial Surgery. I was selected for long-term training, interviewed at five programs, and was offered a position at The Ohio State University Medical Center in Columbus, Ohio. We moved to Columbus and had a wonderful experience in central Ohio. The training I received was outstanding and I am very happy I got the opportunity. I then returned to the Oklahoma Area Indian Health Service and served as Area/Regional Consultant in Oral and Maxillofacial Surgery. I worked at all the IHS facilities in Oklahoma, but my home base was the Lawton Indian Hospital. Over the years, I was promoted three times and was named the National Consultant in Oral and Maxillofacial Surgery for the United States Public Health Service. I traveled to many states and was repeatedly sent on temporary duty to Cape May, New Jersey, to perform extractions on United States Coast Guard recruits.

I retired from active duty on February 1, 2010 with 33 years and 11 days, at the rank of Captain O-6. Once retired, I joined Joplin Oral Surgery in Joplin, Missouri, and worked there until an F5 tornado destroyed the practice and town in May 2011. I then opened Lawton Oral Surgery and worked out of Dr. Todd Bridges' office in Lawton until it was discovered that I had a pituitary

macroadenoma. I had skull base robotic brain tumor removal at UT Southwestern in Dallas. My recovery lasted 6 months, and I then joined Live Oak Oral and Maxillofacial Surgery as an associate, which led to partnership. We were bought out by the corporate entity Paradigm Oral Health. I now work as a corporate Oral Surgery employee at Oklahoma Dental Implants and Oral Surgery.

My son, Austin, and daughter, Katherine, are both first-year dental students at the OU College of Dentistry, and I could not be prouder of them. They are enjoying the experience very much and are grateful to be there.

I hope that my tenure as President of the ODA sees the successful passage of medical loss ratio legislation in Oklahoma for dental insurance. Many people have already worked toward that goal, and we need all of you to contact your state senators to support this effort.

Dentistry has been very good to me and I think the future is very bright. I have high hopes for my children and expect they will have wonderful careers. Everyone has different goals in the profession and there is room for everyone. My dream since graduation has been to have my own practice and office, and I know that there are still infinite possibilities within dentistry for that to happen. The future is bright, especially if we all work together for our profession.

As the incoming ODA President,

1. My vision for the ODA is to emphasize that organized dentistry includes all dental professionals, regardless of who, where, or when. We need to organize as dental professionals to protect our profession from outside influence that seeks only to profit from our efforts, while they could not care less about the patient.

2. Organized dentistry is the only way the profession that I love will ever be able to represent itself at the national level. I am very proud to be part of organized dentistry.

3. The biggest challenges to dentistry today are government attempts to socialize dentistry and third-party payers interfering with the doctor–patient relationship.

Twana Duncan, DDS Eastern District

Twana Duncan, DDS is a practicing general dentist in Southeast Oklahoma. Dr. Duncan was born to Joe and Patsy Farley and lived in several states while her father served in the military. From a young age, Dr. Duncan has had a passion for making a difference in the world. After graduating Cum Laude from Southeastern Oklahoma State University with a double major in biology and chemistry, Dr. Duncan attained her Doctor of Dental Surgery from the University of Oklahoma College of Dentistry in 1991. Upon graduation, she entered the United Staes Navy, serving on active duty from 19911994. Dr Duncan was stationed in Kingsville, Texas as the Junior Dental Officer in charge, achieving the rank of LDCR.

After the Navy, Dr. Duncan saw a need to help rural communities and chose to return close to her roots, starting her office in Antlers, Oklahoma in June of 1994. Dr. Duncan is a member of numerous organizations including President of the Southeast Region for the ODA, the Chair on Budget & Finance for the ODA, The J. Dean Robertson Society, The AAPD Southeast Region Affiliate Advisory Board of Directors, The AAPD’s Special Needs Committee, Past President, Diplomate and examiner for the American Orthodontic Society, the American Dental Association, the American Association of Women Dentists, the American Dental Society of Anesthesiology, and is a member of the American College of Dentistry.

Dr. Duncan makes it her mission to maintain the access to care that she has provided for her patients in a rural community. Her practice, Antlers Family Dentist, PC is a dental home to patients of all ages, backgrounds, and special needs. Dr. Duncan and her team are passionate about serving patients and their home, often volunteering in different areas of the community. Dr. Duncan has volunteered for the Oklahoma Mission of Mercy, is a Special Olympics Bronze Sponsor, and is also active in The Pay It Forward Foundation, Relay For Life, Children With Hair Loss, The House of Joy Women’s Shelter, D-Dent Program, and was awarded the Little Dixie National Award for The Friend of Little Dixie.

A practitioner and general dentist for over 30 years, Dr. Duncan prides herself on the treatment and care of her patients. She has maintained a passion for learning the newest and most innovative dental and orthodontic technologies, while bringing affordable diagnostic treatment practices to the area. Dr. Duncan strives to immerse her practice and herself in the betterment of the communities she serves and has continuously proven that dentistry is a craft that can always be improved and perfected, and that kindness and love are the best tools of all.

Dr. Duncan is married to Jack Duncan, a retired agent for the Oklahoma Bureau of Narcotics, and lives in Antlers, Oklahoma. Today, Dr. Duncan practices alongside her son, Dr. Caleb Smith, and Dr. Haley Harrington, both of whom graduated in 2022 from the University of Oklahoma College of Dentistry. Her daughter, Kaitlyn Werner, MD is an internal medicine physician in Paris, Texas. Dr. Duncan has two bonus children: Jack, an engineer with BNSF Railroad, and Sarah, BSRN at Hilcrest Hospital in Tulsa. Dr. Duncan enjoys traveling and has visited all 50 states & numerous foreign countries, but the greater adventure yet has been being Nonna to her five amazing grandchildren, as well as numerous grand cats and dogs.

ADA DELEGATE LINDSAY SMITH, DDS Tulsa County

Dr. Lindsay Smith grew up in Oologah, Oklahoma, and received his bachelor’s degree in 2000 from the University of Central Oklahoma after receiving a fouryear President’s Leadership Council Scholarship. In 1999, he received the John and Eleanor Kirkpatrick Student Leadership and Public Service Award. Smith was accepted into the University of Oklahoma College of Dentistry’s class of 2004. During his student tenure, he was active in the Christian Medical Dental Association’s medical mission trips to Bolivia (2002) and Guatemala (2003). He also participated in the Good Shepherd Mission, Give Kids a Smile Day, and numerous Staples Society activities. He received a J. Dean Robertson Society scholarship in 2003, and at his 2004 graduation, he was recognized with the American Association of Oral and Maxillofacial Surgeons Dental Implant Student Award, the Southwest Academy of Restorative Dentistry Award, and the Academy of General Dentistry Senior Dental Student Award. He was a member of the Dean’s Honor Roll from 2000 to 2004.

After graduating from OUCOD, Smith began a career in public health, working for the Cherokee Nation and Creek Nation tribal health systems as a general dentist. In September 2007, he opened his private general dental office in Bixby. In 2015, he moved his practice to Tulsa.

Since graduation, Smith has held active memberships in the American Dental Association, Oklahoma Dental Association, Tulsa County Dental Society, and Academy of General Dentistry. He served as Chair of the ODA’s Council on Membership and Membership Services (2009-2013) and Chair of the Access to Care Committee for the Oklahoma Dental Foundation (2010-2013), and he sat on the Board of Trustees for the Oklahoma Dental Foundation for three years. He has been a member of the ODA House of

Delegates since 2009. He served on the Tulsa County Dental Society Executive Committee from 2009 to 2012, and held the offices of Secretary/Treasurer, President-Elect, and President. He served on the Oklahoma Dental Association Executive Committee from 2013 to 2017, holding the offices of Vice President, President-Elect, President and Immediate Past President. He has served as Trustee of the Oklahoma Dental Association since 2019 and as Chair of the Council on Governmental Affairs for the Oklahoma Dental Association since 2018. Dr. Smith currently serves on the Council of Dental Practice for the American Dental Association, representing the 12th District. He has been inducted into the American College of Dentists (2013), the Pierre Fauchard Academy (2014), and the International College of Dentists (2015). He is an avid volunteer of the Oklahoma Mission of Mercy and served as Co-Chair of OkMOM 2016. Smith enjoys volunteering his time at the Tulsa Catholic Charities’ Mother Theresa Dental Clinic where he supervises the fourth-year dental students externship program.

Smith and his wife, Brooke, are the proud parents of two beautiful daughters, Ainsley and Marin. The Smith family currently resides in Tulsa.

ADA ALTERNATE DELEGATE CHRIS FAGAN, DDS NORTHWEST DISTRICT

Dr. Chris Fagan graduated from the University of Oklahoma College of Dentistry in 2011. He is currently a member of the American Dental Association, the Oklahoma Dental Association and the Northwest District Dental Society. Dr. Fagan served as the president of the Oklahoma Dental Association in 2021-2022. He is a fellow in the Pierre Fauchard Academy, a member American College of Dentists, and a fellow in the International College of Dentists. He has served as the President of the Northwest District of the ODA and as the President of the Garfield County Dental Society. He is a Dean’s Circle member of the J. Dean Robertson Society and has served as a trustee on that board. He has served on the OUCOD Alumni committee, the ODA Council on Membership and Membership Services, the ODA House of Delegates and the ODA Student Mentoring Program. Dr. Fagan is the current DENPAC chair and Immediate Past President of the ODA. He has also been a volunteer with the Oklahoma Mission of Mercy, serving as a Restorative Lead for three years and as the Co-Chair for the event in 2019. He is an advisor to the dental assisting program at the Autry Technology Center in Enid. In 2014, Dr. Fagan was recognized as the Young Dental Alumnus of the Year by the OUCOD Alumni Association. In 2017, he was honored as one of the top 10 Young Professionals under age 40 in Enid, Oklahoma. Dr. Fagan also represents the George Miksch Sutton Avian Research Center as a board member. Dr. Fagan lives just outside of Enid with his wife, Heather, and their two boys, Cooper and Caleb. He enjoys traveling, cooking, golfing, hunting, and spending time with friends and family.

Finding and Hiring the Right Candidate for Your Dental Practice

Growing your dental practice team is both an exciting and a stressful time. Finding someone who has the skills you need and who will mesh well with your team may seem like finding a needle in a haystack. But it doesn’t have to be.

Having a clear idea of the type of person and skills you want your next team member to have is key to making the hiring process flow as smoothly as possible. It’s good to have a full list of hard and soft skills your ideal candidate should have and then prioritize what’s most important to your team. Here are some examples to get you started:

• Hard Skills – Technical skills (how to do x-rays, prepare supplies, disinfect rooms, etc.), computer knowledge, hand-eye coordination, motor skills

• Soft Skills – Team player, attention to detail, compassion, dependability

Once you have a clearer picture of the type of person you’d like to add to your team, it’s time to start actively recruiting. According to the National Association of Colleges and Employers, it costs businesses, on average, $7,645 to hire a new employee. Making sure you find the right team member the first time will save you not only time but also money. Here are some things to consider when posting a job opening:

• Create a compelling job title and description. LinkedIn reports that job seekers lose interest after 14 seconds, so you want to make sure your first impression is the best.

• Make sure your posting isn’t too long (or too short). Between 300 and 700 words is ideal.

• Post on multiple sites. You never know where the right candidate will come from, so it’s best to post your job openings in more than one place. Aside from large national sites like LinkedIn and Indeed, consider posting on local job boards, Facebook groups, and your website. If you don’t have a website, or if you need help with your website, ProSites can help!

When it's time to interview candidates, asking the right interview questions will ensure that you find the best team member for your practice. Here are several examples:

TELL ME WHY…

An open-ended introductory question requires a candidate to go beyond just sharing facts and stock answers and think deeper to share some of their motivations and personality. This will help you gauge how

a candidate will gel with your team and how they align with how your practice serves patients.

A few open-ended introductory questions:

• Why do you want to work here?

• What made you choose a career in the dental field?

• What do you find most rewarding about your work?

TELL ME HOW…

Asking candidates technical interview questions, such as how they prepare for patients or how they perform specific procedures, provides dental assistants and hygienists with the opportunity to share their knowledge of processes that would likely be part of their daily duties and demonstrate that they have learned and performed them correctly. Some sample questions that lead to “tell me how”:

• Do you have experience assisting with fillings, crowns, or extractions?

• Do you have experience making dental impressions?

• How do you help keep patients calm during procedures?

HOW DO YOU EXPLAIN THE NEED FOR A DENTAL PROCEDURE TO A PATIENT WHO DOESN’T UNDERSTAND THE IMPORTANCE OF THE TREATMENT?

This open-ended question can tell you a lot about the candidate. It gives them the chance to share their technical knowledge of procedures, as well as their communication and compassion for patients.

WHAT EXPERIENCE DO YOU HAVE WITH PRACTICE EFFICIENCY?

Your practice may be like others. Where practices are commonly understaffed and busy with appointments, dental teams with a collective knowledge of important tools and systems to save time and improve practice efficiency can leanly navigate toward the better tomorrow that so many practices now seek.

Asking candidates about what they bring to the table beyond traditional dental skills invites them to share what they have to offer that other candidates perhaps don’t. In doing so, you have the chance to identify a potential team member whose experience can benefit many or all aspects of your practice, including operational efficiency, time-savings patient relations, profitability, and team well-being.

Make sure candidates know that practice efficiency experience is not a prerequisite for the job (unless it is). You’re simply letting them know that you value the whole of their experience, and you’re inviting them to share it.

Example questions:

• Do you have experience with practice management software?

• Do you have experience with automated patient communication systems?

If a candidate doesn’t have direct experience with a particular system, ask them how they think a particular system might improve practice efficiency. This tells candidates that you’re a forward-thinking practice (which may differentiate your practice if a candidate is shopping around for such a practice) and informs them that you value their input, which is a retention factor.

WHAT KINDS OF CHALLENGES ARE YOU SEEKING IN THIS POSITION?

With this question, candidates can share specifics about what they hope to gain from being hired, and you gain further insight into retaining them if you hire them. It could be something missing from a previous position that a practice didn’t offer (for example, a more active role in leadership or decision-making) or something you provide in your practice, which becomes an attractant and a retention factor for a candidate.

WHAT ARE YOUR INTERESTS OUTSIDE WORK?

In addition to shedding additional light on a candidate’s personality, this question shows that you care about work-life balance, and you’re interested in what they do to help maintain it.

WHAT DO YOU SEE AS SOME OF THE BIGGEST CHALLENGES FACING OUR INDUSTRY?

As difficult as it can be for some dentists and management to hear how dental assistants, hygienists, and office staff really feel about the state of the dental industry, some of these candidates believe that practices are unaware of (or choose to ignore) workplace issues that can cause team members to quit, either their job or the profession entirely. Asking what they see as some of the biggest challenges facing the dental industry informs candidates that you care about a healthy workplace, that you value their voice, and that you can see the big picture.

Procedures are restorations, amalgam and composite. Extractions are simple and surgical, based on the comfort level of the dentist. Exams and digital x rays are offered. For complex extractions and molar endo we have both an endodontist (in Durant, OK) and the ability to refer out root canals to outside providers. This also applies to oral surgery for complex cases, third molars, full mouth extractions or complex extractions where anatomical considerations may impact the case. Also, we do have one pediatric dentist (in Durant, OK). Dentures and partials are handled by outside providers, and the patient is given contact instructions.

New DEA Guidelines for Prescribing Controlled Substances

A bill passed by Congress in December 2022 requires prescribers of controlled substances to complete eight hours of one-time training on safe controlled substance prescribing to receive or renew their registration with the U.S. Drug Enforcement Administration (DEA). The statutory language was taken from a separate bill called the Medication Access and Training Expansion Act (or MATE Act). The ADA successfully advocated for Congress to amend certain provisions of the MATE Act to adhere with ADA policy. The federal requirement applies to any licensed DDS or DMD who needs a DEA registration to prescribe controlled substances (Schedules II, III, IV, and V). Those who do not need a DEA registration are not affected. A full mouth rehabilitation with FP1 prosthesis using advanced prosthodontics and periodontics approach: A case report (Part

INTRODUCTION

There has been tremendous change in treatment approaches for patients with terminal dentition since Dr. Branemark's scientific breakthrough discovery of osseointegration in 1952. Dental implants offer patients more treatment solutions, including fixed prosthesis options for edentulous areas, and maintaining the bone structure and soft tissue health. Virtual implant planning using cone-beam computed tomography provides high accuracy for planning implant placement. Integration of CAD/CAM software allows for the simultaneous designing of an interim prosthesis in conjunction with surgical planning. However, a direct pick-up of an interim FP1 prosthesis can be challenging, especially in the mandible. The incorporation of existing teeth as a seating guide can facilitate proper positioning of the interim prosthesis. This case report delineates a workflow of restorative-driven implant rehabilitation from virtual planning to implant placement between two specialties, prosthodontics and periodontics, to achieve the optimal outcomes for the patient, using the most up-to-date technologies.

CASE REPORT

A 53 year old female business owner presented to the prosthodontics clinic at the University of Buffalo with the chief complaint of “I have bad teeth with multiple failing restorations, periodic swelling, and pain.” She did not like her smile and had concerns regarding both esthetics and function. The recurring pain was in the maxillary and mandibular anterior regions. The patient was classified as ASA III, due to malignant lung cancer that had metastasized to the lung, brain, bone, and lymph nodes. Previous chemotherapy was done to treat her brain and lung cancer. The brain tumor was treated 5 years ago with gamma ionizing radiation. In addition, she had a history of chronic sinusitis. At the time of surgery, the patient was taking prochlorperazine (Compazine, PBM Pharmaceuticals), rivaroxaban (Xarelto, Bayer’s Wuppertal laboratories), pembrolizumab (Keytruda, Merck), pemetrexed (Alimta, Eli Lilly and Company) folic acid (Zydus Pharmaceuticals), vitamin D3 (cholecalciferol, Superior Supplement Manufacturing) (Table 1). Upon examination, #9-11 bridge was present, with #9 exhibiting positive pain on palpation, but no pain on percussion. Teeth #27-29 was an existing bridge with marginal leakage on both abutments. Tooth #26 was previously root canal treated, and was negative to percussion and palpation (Figure 1). A problem list was developed, diagnosis confirmed, and treatment plan was produced (Tables 2, 3).

MATERIAL AND METHODS

From a periodontal perspective, one of the most important processes for complex cases is assessing the patient’s motivation for oral hygiene.

The patient quit smoking five years ago and had been brushing twice daily with an electric toothbrush since 2020. She also used an antiseptic mouth rinse every day. She stated that she clenches and grinds while sleeping and is interested in wearing a nightguard. No primary or secondary trauma from occlusion was noted. Her last scaling was completed at a private practice one year ago. The patient’s periodontal diagnosis was stage III localized periodontitis grade B, because the patient has tooth #1 site with a probing depth of more than ≥ 6mm, vertical bone loss ≥3 mm, and a moderate ridge defect. Additionally, radiographic bone loss extended to the middle third of the root and beyond. The rest of the sites showed coronal third bone loss of less than 15%. The patient’s treatment prognosis was discussed before the surgery. Emphasis on the patient’s oral hygiene was a must, especially if it involved multiple implant placements. The university did not agree to pay additional treatment fees for possible peri-implantitis in the future - the patient understood and wished to proceed. The patient’s surgery was scheduled with moderate intravenous sedation to ease the patient’s anxiety.

As for the disease control phase, scaling and root planing was completed. Tooth #1 was ultimately planned for extraction; however, the treating prosthodontist planned to use it in the interim to hold occlusal vertical dimension on the right side. Endodontic retreatment of #9 was performed to eliminate the ongoing periapical abscess.

The treatment plan involved extraction of teeth #19, 21, 22, 23, 24, 25, 26, and 28 due to extensive caries and multiple failing restorations. After extractions, the lower arch was restored with implant-supported fixed restorations. The goal of the restorative plan for the maxillary arch was to restore her failing tooth (#9) and improve the appearance of her smile, according to her chief complaint. Therefore, the plan was to replace the bridge #9-X-11 and restore #3, 4, 5, 6, 7, 8, 12, 13, and 14 with bonded restorations.

Conventional diagnostic workup was performed on stone casts to prepare the interim prosthesis and plan for implant surgery. The casts were then digitized using a desktop scanner (Rainbow scanner, Dentium USA) and aligned to the CBCT data. Prosthetically-driven virtual implant planning was completed based on an ideal diagnostic wax-up on commercially available implant planning software (Blue Sky Plan, Blue Sky Bio). Implant positions were virtually planned on the sites of teeth #18, 20, 22, 27, 29, and 30. The interim prosthesis was designed using CAD software (Exocad dental CAD, USA). The design of this interim prosthesis had acrylic extensions on the occlusal surfaces of the lower molars to facilitate the direct pick-up process after implant placement. Relief holes were also digitally created in the

regions of planned implants (Figure 2).

Prior to the surgery, the existing bridge was sectioned between #28 and 29 pontic for the ease of extraction. Next, extraction of #21, 22, 25, 26, 29 and implant placement at #19, 20, 22, 27, 29, 30 was completed, along with socket grafting. All teeth were extracted using elevators and forceps, and special care was taken not to damage crestal bone height (Figure 3a). A surgical curette and small round bur with a handpiece were used to remove granulation tissue at site #29 and ensured that sound bone was exposed (Figure 3b). After the surgical guide was positioned, 4.3mm x 11.5 mm implants were placed (Nobel Active TiUltra, Nobel Biocare) (Figures 3c, 3d, 3e). Osteotomy preparation was completed with the medium bone drilling protocol and mandatory tapping. In this case, the bone around the site of #30 was dense. Therefore, a dense bone drilling protocol was used and tapping was done twice. All implants were hand-torqued with a wrench at more than 35Ncm. After all implants were placed, alveoloplasty was completed using a mock surgical template for final restoration (Figure 3f). Then, the patient was transferred to another room with the completion of sedation. Implant stability quotient (ISQ) values were checked with a transducer attached to an implant fixture (Osstell, Osstell AB) (table). Temporary prosthetic cylinders were placed, and seating was verified with radiographs. The interim prosthesis was picked-up intraorally using a dual-cure bisacryl resin. During the removal of the temporary cylinder for implant #29, the implant lost stability and began spinning. The decision was made to submerge #29, and a cover screw was placed. All other implants received healing abutments during the completion of grafting and closure. Bone graft (AlloOss cortico-cancellous 250-1000 Microns, Ace Surgical Supply) was mixed with platelet-rich-plasma (PRP), while another 1cc was prepared separately. The bone defect on #29 and extraction sockets were filled with graft (Figure 3g). For sites #22 and 27, a collagen membrane (Biogide, Geistlich) was used to cover the graft (Figure 3h). Then, platelet-rich-fibrin (PRF) membranes were placed on the sites of #22, 27, and 29 (Figure 3i). Periosteal releasing incisions were made lingually and buccally. Subsequently, a combination of horizontal mattress suture and simple interrupted sutures were placed using resorbable sutures (Glycolon 4-0, Osteogenics). Complete wound closure and positive hemostasis was achieved (Figure 3j). The patient was accompanied by her father and left the office healthy. After 4 months, 2nd stage uncovery surgery was completed on #29.

The restorative phase continued on the maxillary arch. A prefabricated post and core was placed on #9. Subsequently, maxillary teeth were prepared for crowns and a bridge. For the mandibular arch, a conventional impression was made to create a master cast. The implants on the canine areas were verified using a pattern resin verification jig for fabrication of an implantsupported bridge #22-27, and the verified master cast was subsequently scanned with and without the interim prosthesis in occlusion. The mandibular PMMA prototype was designed using CAD software (3Shape Dental System, 3Shape), milled, assembled, and prepared for delivery. This second set of provisionals served as a blueprint for the final prosthesis fabrication (Figure 4).

The final restorations for the mandibular arch included #19, 20, 29, 30 implant-supported zirconia crowns, and #22-27 implantsupported zirconia bridge. The natural dentition on the maxilla was restored with lithium disilicate crowns (Figures 5, 6). A night guard was fabricated to protect the restorations from parafunctional jaw movement. Post-operative 1 year follow-up radiographs and

periodontal charting showed stable bone levels around implants, as well as clean restorative margins (Figures 7, 8).

MyHealth Access Network Identified as the State Designated Entity (SDE)

Research indicates that patient care can be fragmented when clinical information comes only from a single clinic’s or hospital’s electronic health record (EHR). Oklahoma’s Health Information Exchange, MyHealth Access Network, reports that, on average, Oklahoma patients have medical records in more than three healthcare organizations, with some—those with the most complicated conditions—having important health records in as many as 25 places. Interoperability allows providers to access data available from the Health Information Exchange (HIE), which includes multiple sources and allows immediate access to a longitudinal patient record. This synergistic approach to information sharing improves the quality of care, avoids mistakes, enables clinical decision support and care coordination, and enhances the patient experience (Journal of the American Medical Informatics Association, 2021).

The exchange allows providers, nurses, case managers, and other care team members involved in a patient’s care to appropriately access and securely share a patient’s vital medical information electronically. Nurses in every specialty can relate to the challenges of acquiring patient health information when it is needed most—at the point of care. Whether a direct admit to a hospital, a required transfer for level of care, or in an emergent situation, having even the most basic information, such as allergies and a medication list, improves the speed, quality, safety and reduces the cost of patient care. The exchange complies with state and federal programs such as Merit-based Incentive Payment System (MIPS), Office of National Coordinator for Health Information Technology (ONC), and Centers for Medicare and Medicaid Services (CMS) interoperability rules, and other requirements.

MyHealth Access Network, having served as an HIE in Oklahoma since 2010, and currently connecting more than 80% of all healthcare data in the state, has been identified as the state designated entity (SDE) for Health Information Exchange and is here to help you meet the requirements of SB1369.

In March 2022, the State of Oklahoma passed SB 1369, and the subsequent rules, requiring all health care providers to report data to and utilize the State Designated Entity (SDE) for Health Information Exchange beginning July 1, 2023.

All health care providers shall establish a direct, secure connection to the state-designated entity for HIE and submit data in the form and format as defined on the Office of the State Coordinator for Health Information Exchange website. Providers should actively use HIE services to securely access records during and/or in support of treatment or healthcare operations.

MyHealth Access Network currently links more than 4,000 Oklahoma healthcare providers and more than 4 million patients within and beyond Oklahoma, who now have their health records available wherever and whenever they seek care. The data found in MyHealth include diagnoses, visits, medications, allergies, vital signs, lab results, immunizations, procedures, clinical notes, social history, and much more. By using the MyHealth Provider Portal, care team members can access the complete health history of each patient any time that it is needed for decisions about the patient’s care and plan.

Dangers of Medication Errors and How ePrescribing Can Help

In 2019, the pharmaceutical industry spent $83 billion on research and development and on the introduction of new drugs for sale. These medications are intended to be prescribed to help patients manage and negotiate medical and dental conditions.
