REFLECTIONS ON 35 YEARS WITH THE JOURNAL Dr. Miranda has announced his resignation as Associate Editor of the ODA Journal effective December 31, 2021, bringing to a close a 35-year tenure with the Journal. The following are his reflections on his time with the editorial board and his acknowledgement of all those with whom he has worked with over the years.. In 1974, I was recruited from California to join the charter faculty at the new OU College of Dentistry. One of my first actions upon arriving was to join the Oklahoma Dental Association. Many valuable perks have come with my 47+ years of membership, but the most enjoyable has been my official association with the ODA Journal. Since first joining its Editorial Board in 1983, I’ve had the privilege to be intimately involved in the development and publication of the Journal for 35 years. During that time no experience has been more gratifying than the sheer number of personal and professional relationships I’ve been blessed to cultivate.

The one individual most responsible for my formal involvement in Journal activities was Dr. Thomas J. Murdoch. I first met Tom in the late 1970s when he joined the OUCOD as part-time faculty in the Department of Operative Dentistry. In 1975, he and Dr. Earl Mabry had begun a 15-year tenure as alternating editors of the ODA Journal. (Truth be told, they were essentially coeditors. Pretty much joined at the hip, they were a seamless and well-oiled team.) As my friendship with Tom deepened, I became intrigued with his journalistic activities and jumped at his invitation to join the Editorial Board in 1983. For the next seven years, I served first as reporting liaison for the College of Dentistry to ensure that OUCOD news made it into the Journal, and then alternating with either Tom or Earl as associate or advisory editor when the other was serving as editor. We jokingly referred to ourselves as the “3-M Company” (Murdoch, Mabry, Miranda), borrowing the moniker from the morefamous dental product supplier. When Tom and Earl decided to hang it up in 1990, I

stayed on the Journal staff as a scientific consultant and OUCOD “roving reporter,” serving with four different editors over the next four years. In addition to Tom’s talents as a writer and editor, he was also a superb and avid photographer; hundreds of his photographs have graced the pages (and covers) of the Journal over the years. I owe an enormous debt of gratitude to him (and Earl) for introducing me to the art of journalism. In 1995, I assumed editorship of OU Dentistry, OUCOD’s in-house alumni magazine. While Tom and Earl had provided valuable preparation for this role, the more intensive hands-on nature of dealing with the school’s publication made it very difficult to adequately attend to both projects. So I reluctantly took a four-year hiatus from the Journal. (Fortunately, three of the four ODA editors during this time were OUCOD faculty, so the pipeline for school news remained intact.) When I resigned my first stint as OU Dentistry editor in 2000 I resumed my formal association with the ODA Journal. For the past 21 years that relationship has been continuous, involving service as either Advisory or Associate Editor (in addition to my own stint as Editor in 2002-03). Throughout my time with the Journal, I’ve had the privilege of working with some of the true giants of Oklahoma Dentistry, like Dr. Ray Cohlmia (who was ODA president when Tom and Earl brought me on board and who became a dear and lifelong friend and mentor). I’ve worked with the ODA’s last three executive directors, Bob Berry, Dana Davis and the tireless Lynn Means. (Incidentally, Lynn, the ODA’s seventh executive director, will soon complete her


13th year in the position, which will make her tenure the secondlongest in ODA history, trailing only Bob Berry, who served an astounding 34 years!) I’ve served under 34 ODA presidents (including 16 of my former OUCOD students), 13 different editors (five of whom are also former students), numerous editorial board members who have toiled tirelessly to turn the Journal into the outstanding publication it is today, and a truly dedicated cadre of ODA staff that just keeps getting better and better through the years. (Thanks, Lynn!) A number of innovative changes have marked the Journal’s progress during the last four decades. (As a historical aside, the Bulletin of the Oklahoma State Dental Association was born in 1911 as one of the country’s earliest state dental publications; it was renamed the Journal of the Oklahoma State Dental Association in 1951.) When I first joined the editorial staff, the Journal was being published quarterly, with issues identified as Spring, Summer, Fall and Winter. In 2006, publication was increased to ten issues per year. While this vastly expanded the workload for the editorial staff, more news and articles were made available in a more timely fashion. In 2015 issue frequency was reduced to six issues per year and has remained so to the present day (much to the relief of the editorial staff, I’d wager!). Some significant content changes have also made the Journal more inclusive. Beginning in 2008, a concerted attempt was made to devote one issue each year primarily to the affairs of the OU College of Dentistry, creating an even closer bond between the school and the professional community. In the last few years, the Hygiene Hotspot has become a regular feature of nearly every issue, providing a crucial link between dentists and dental

hygienists (two groups who have sporadically been at odds in the past), and giving hygienists an opportunity to share their thoughts and ideas with the larger Journal readership. These are a few examples of the progress that has made the ODA Journal one of the premier dental publications in the country. The past 15 years have been particularly satisfying as I’ve had the distinct pleasure to work alongside four of my former students: Drs. Raymond Cohlmia, Randy White, Phoebe Vaughan, and Mary Hamburg. Nothing gives a retired teacher more joy than to witness the positive impact his students make on the dental profession. The editorial management of these four men and women has been nothing short of stellar. It has been a true honor to work with each of them. I could devote numerous paragraphs singing the praises of so many more people. In lieu of that, let me close with this observation: Every single year of my time with the Journal is full of pleasant memories that I will revisit often and cherish forever. To all those who dwell in and have contributed to those memories (and you are legion) -- you have my eternal gratitude. AUTHOR’S NOTE: I’d love to ascribe much of the foregoing historical information to a razor-sharp memory, but I must credit Ms. Chris Sutton’s excellent article on the ODA’s first 100 years as published in the ODA Journal’s Centennial issue (July/August 2007, Vol. 98, No. 10). Much of the information about the ODA’s past presidents, editors, executive directors and, of course, the Journal, has been culled from that source. If you haven’t read this superb chronicle, I recommend it highly.

MESSAGE FROM THE ODA PRESIDENT

Emerge Several ODA past presidents told me early on that my year of presidency would fly by. They weren’t wrong. It’s hard to believe it’s been seven months since I was installed. My goal for the year was very simple: I wanted our organization to emerge from the weight of COVID that has been hanging over us for the past 18 months. The ODA offers so many benefits but, in my opinion, the greatest benefit is the camaraderie and fellowship we share as members of this profession. My goal was for us to safely get back together and return to some semblance of normal. I’m pleased to report that the Oklahoma Dental Association is well on its way to accomplishing that goal.

July 30 marked the first time in many months that a meeting was held at the ODA building. The Annual Meeting Planning Committee met to prepare for the upcoming ODA Annual Meeting being held in OKC next April. More on that later... It was so refreshing to be face to face with the ODA staff and the members of that committee. I don’t think we’ll ever get away from Zoom calls, but conducting the business of the ODA is so much more enjoyable and productive in person! We have hosted two happy-hour events recently in Oklahoma City and Tulsa, and it was great to see everyone. We are also hosting a day at the Tulsa Zoo for you and your families on November 13, 9am-5pm. This is a free event to members. If you would like to participate, please contact Madison at the ODA office. Out of an abundance of caution, we decided to cancel the Oklahoma Mission of Mercy last year. I’m happy to announce that OkMOM will be held in Shawnee on Feb 4-5, 2022. Preparations are currently under way to hold a safe and productive event for everyone involved. Volunteer registration opened October 15 and I hope you’ll sign up for a shift or two at okmom. org. The ODA Annual Meeting will be held next April 1-2 at the new Oklahoma City Convention Center and Omni hotel. Dr. Blaire Bowers Erstenuik, annual meeting chair, and Dr. David Wong have arranged a phenomenal lineup of speakers. The meeting will offer over 40 hours of CE including ethics and opioid training.

There will also be several opportunities for hands-on training. Look for your registration packet soon and be sure to sign up after January 1. The Oklahoma Dental Association and 20 other state medical associations are currently involved in the fight of our lives. As most of you know, Governor Stitt is attempting to push the Oklahoma Health Care Authority into a managed-care system. To put it simply, he wants to turn over OHCA to insurance companies instead of maintaining the state-run system we currently have. This move would negatively affect the quality of health care patients would receive and would significantly increase the cost of this care to the state of Oklahoma. The governor was able to push his agenda through the Legislature at the end of last session. Less than one month later the Oklahoma Supreme Court ruled that OHCA did not have the authority to enter into contracts with these managed care companies without the Legislature’s approval. This fight is not over; this is likely just the beginning. Recently the governor removed the only two medical professionals from the OHCA Board because they were not in line with his agenda. I wrote an article that was published in The Oklahoman that explained our disdain with his actions. The fact that a board with no representation from the medical community is managing the health and dental care of close to one million Oklahomans is simply unacceptable. Governor Stitt is continuing his efforts to implement managed care and we will be looking for your support in the upcoming year as this fight continues. Looking forward to the remaining five months of my term, I can’t help but be excited and optimistic. We should all be very proud that our own Dr. Raymond Cohlmia has taken over the leadership of the American Dental Association as its new executive director. His vision for the future of the ADA, and dentistry in general, is refreshing. The future of dentistry in Oklahoma is also in great hands as our immediate past president, Dr. Paul Mullasseril, has been named Dean of the OU College of Dentistry. I look forward to seeing all of you in person in the very near future.

This year, the Oklahoma Dental Association accomplished some major goals to benefit you, our ODA members, and your patients. Take a look through the ODA’s top five accomplishments of 2021! #1. The Fight Against Managed Medicare. On June 1, the Oklahoma Supreme Court ruled 6-3 that the Oklahoma Health Care Authority does not have the authority to move forward with any part of managed care Medicaid. It was a VERY busy and productive legislative session as the ODA joined forces with every other healthcare association to successfully fight this threat. #2. First-Ever ODA Virtual Annual Meeting. On April 9-10, 2021, more than 700 dental professionals joined the ODA for its 2021 Annual Meeting on a new virtual platform. The virtual attendees earned a total of more than 5,100 hours of CE! Because it was a virtual meeting, the ODA was able to host speakers from around the world and even host attendees from across the country. Dr. Dean Licenblat joined from Australia, and Drs. Bhide, Albatish and Shah joined from Canada. The virtual meeting was an outstanding success, but we are looking forward to seeing everyone in person at the 2022 meeting in Oklahoma City! #3. Inaugural Veteran’s Dental Day Event. Rescheduled because of COVID and then an ice storm, the inaugural Veteran’s Dental Day event finally took place on March 27, 2021. The ODA partnered with the Oklahoma Dental Foundation, the OU College of Dentistry, and Henry Schein to provide free dental care to more than 50 Oklahoma veterans. Patients received free cleanings, extractions

and more. OUCOD dental students and faculty, plus a handful of ODA volunteers, provided the care. As a former Army Reserve member, ODA immediate past president Dr. Paul Mullasseril made it his mission during his presidency to plan a free dental care day for metro-area veterans that would continue annually into the future. The second annual event was held November 6th. #4. The ODA Welcomed Dental Graduates to Organized Dentistry. On August 19, 2021, the ODA’s collaborative recruitment efforts were recognized by the American Dental Association for successfully encouraging graduating dental students to become tripartite members of the ADA, ODA and local dental societies. With more than 80% joining, the ODA, the OU College of Dentistry and chapter leaders of the American Dental Student Association (ASDA) were recognized nationally for helping to fill the pipeline and help transition students to practicing dentists. #5. ODA Members Saved Through Two NEW Rewards Partners.* On February 5, 2021, the ODA Rewards Committee voted to include Black Talon Security and Enviro-Mist as ODA Rewards Partners. Maximizing Referrals You will always have a small group of patients who will consistently refer other patients to you. Treat your patients as if they are the most important person and assure them that their business is appreciated. Your respect will be rewarded and you may be sent one or many referrals. • Start with the phone call. First impressions are key. Make sure your phones are answered promptly and with a friendly voice. • Prepare for the visit. Is your office welcoming? Are directions and parking easily accessible? • Create a referral plan. Does your staff know the importance of referrals? Consider developing a short script that staff can modify and/or promotional material that patients can take with them.

• Thank your referrers. Send a thank you note for referrals. Most people like to be recognized and this is a simple way to continue building loyalty. Referrals are a low-cost, high-return marketing tool. Understanding what patients respond positively to is a main factor in improving referrals. Auto-Renew Plan This plan allows your dues to be renewed automatically and paid with your credit card or an ACH/ICA payment each year. You will be allowed to update/ change your credit card, recognizing it will expire at some point, or change to a different card for future charges. VOLUNTEER INFORMATION OKMOM 2022 The OkMOM leadership team and public health officials are collaborating to safely move forward with OkMOM 2022. Strict safety protocols will be implemented to maintain the health and safety of all volunteers, dental professionals and patients. We are confident in our ability to provide a safe and successful event, and we appreciate the cooperation of all participants.

OkMOM leadership encourages all volunteers and patients to get vaccinated for the health and safety of all participants. For the safety and success of the 2022 OkMOM dental clinic, it is imperative that all volunteers understand and comply with the following safety measures: • All volunteers on the treatment floor will be required to wear the PPE recommended by the Centers for Disease Control and Prevention (CDC), Occupational Safety and Health Administration (OSHA), Oklahoma State Department of Health (OSDH), the Oklahoma Board of Dentistry and the American Dental Association (ADA) as of Feb. 4-5, 2022. • All volunteers are required to wear a mask while in the building. Masks will be provided at volunteer check-in. • All volunteers are required to complete a COVID-19 questionnaire upon arrival. Any individuals unwilling to comply with the safety protocols will not be eligible to volunteer. We thank you for your cooperation. After registering to volunteer, registrants will receive a confirmation email with full details regarding OkMOM 2022: Shawnee details and protocols. OkMOM 2022 Location Heart of Oklahoma Expo Center 1700 W Independence St Shawnee, OK 74804 Parking Parking is free, but we encourage ride sharing as parking may be somewhat limited. Registration Register online at okmom.org. Volunteers must register individually. Online registration closes January 25. There will be no on-site registration. All volunteers must pre-register online. Continuing Education Dental personnel can earn continuing education credit by volunteering at OkMOM 2022. Dentists can earn up to 10 hours of credit and hygienists can earn up to five hours of credit during each reporting period. CE verification forms will be provided at volunteer check-in.

OkMOM Shifts For the 2022 event, the volunteer shifts will be shorter. Most volunteer categories will have three shift options - a morning, midday and afternoon option. Volunteers can register for more than one shift per day. Volunteer Hospitality To help in the mitigation of COVID-19, we will be serving prepackaged snacks for those who volunteer partial-day shifts. For those who volunteer for a full day, we will provide a meal in a separate venue. We ask that those volunteering for one or two shifts eat before arriving to the clinic.

The objective of this course is to raise awareness of the scope of the problem in the US, provide attendees with studies showing that other therapies work just as well as opioids (if not better), and to provide attendees with statutes and rules they should be abiding by when prescribing. CRACK AND FRACTURE MANAGEMENT Vertical root fractures (VRF) and cracked tooth syndrome represent some of the most difficult clinical problems to diagnose and manage. CBCT, in conjunction with radiographic and clinical findings, is extremely helpful in the detection and treatment planning of fractures. Additionally, with more options of endodontic and restorative materials, proper management of cracked teeth can allow patients to maintain their natural dentition more predictably than ever before.

This lecture will review the current concepts of restoratively-driven endodontics and provide a hands-on experience to learn the techniques of managing endodontic treatment from access to restoration. With a short lecture and longer hands-on portion, this course will discuss the changes toward a modern and restoratively-driven approach to endodontic treatment, and it will introduce the concept of contemporary instrumentation and obturation using the heat-treated endodontic instruments and bioceramic obturation.

Newly Elected ADA Officers Dr. Cesar R. Sabates Installed as ADA President During his address to the House of Delegates, Dr. Sabates told members that he stood before them as an example of the American Dream.

ADA President Cesar R. Sabates, D.D.S., addresses the House of Delegates during the American Dental Association meeting in Las Vegas.

Cesar R. Sabates, D.D.S., of Coral Gables, Florida, took office Oct. 16 as the 158th president of the ADA — becoming the Association’s first Cuban-American president. Dr. Sabates and the new and returning officers were installed in their offices before proceedings of the ADA House of Delegates during SmileCon in Las Vegas.

“As you may know, I was a child of Castroera Cuba,” he said. “At seven years old my family boarded a freedom flight to the United States. My parents sacrificed everything to ensure that their children would have the chance to be free and educated here in the land of opportunities.” And as ADA president, Dr. Sabates said he pledges to give the Association his best to help it succeed. “You can count on me to represent dentistry and to be steadfast in my advocacy, support, and defense of our profession,” he said. “I want to unite the ADA in protecting dentistry from excessive regulations and other outside influences that threaten the very principles of our work. I also want to unify the ADA for the sake of strengthening our professional family and our future.”

Dr. Sabates received his dental degree from the University of Missouri-Kansas City School of Dentistry. He has been actively involved at the ADA for many years, previously serving as the 17th District trustee for the ADA Board of Trustees from 2016-20 and as a delegate in the ADA House of Delegates from 2000 to 2016. He is past president of the Florida Dental Association and South Florida District Dental Association. Dr. Sabates is a member of the Academy of General Dentistry, Hispanic Dental Association, American College of Dentists, International College of Dentists and Pierre Fauchard Academy.

Dr. George R. Shepley elected ADA President-elect Dr. Shepley is the immediate past trustee of the ADA’s 4th District. He previously served as president of the Maryland State Dental Association from 2013-14, and chaired the ADA Budget and Finance Committee, Standing Committee on International Accreditation and the ADA Council on Communications. He is a member of the Academy of General Dentistry, American College of Dentists, International College of Dentists and Pierre Fauchard Academy. Dr. Shepley received his dental degree from West Virginia University School of Dentistry, and has been a member of the ADA for 41 years.

Second Vice President, Dr. Mark Bronson Dr. Bronson previously served as president of the Ohio Dental Association and chair of the ADA Council on Government Affairs and ADA State Public Affairs Committee. He received his dental degree from the Ohio State University College of Dentistry and is a member of Pierre Fauchard Academy, American College of Dentists, International College of Dentists and the National Dental Association.

LEGISLATIVE LOOP Legislative Overview & Political Update

CE & Golf Scramble for DENPAC A CE and golf tournament was held to benefit the ODA’s political action committee, DENPAC, on August 27 at Karsten Creek Golf Club in Stillwater, raising over $17,000 to help support dentistry-friendly candidates. Players received two hours of CE from Dr. David Rice who presented the course, Make it Stick!, sponsored by Ivoclar Vivadent and igniteDDS. Following the CE, twelve teams competed in the golf scramble and prizes were given to the 1st, 2nd and 3rd place teams, as well as the players with the longest drive and closest to the pin. DR. JAMES YEATS By: Makenzie Dean, ODA Programs & Operations Manager Dr. James (Jim) Yeats earned his bachelor’s degree from Oklahoma State University and originally planned to pursue a career in medicine or veterinary medicine. But when a friend decided to apply to dental school, Yeats decided to follow suit. In 1973 he was accepted as a member of the new OU College of Dentistry’s second class, eventually graduating in 1977. For the next 35 years until his retirement in 2012, he practiced in Catoosa, a suburb of Tulsa. When Yeats entered dental school nearly 50 years ago, there were only 24 students in his class. He and his classmates had the unique experience of receiving most of their instruction from the new heads of the departments, including the founding dean of OUCOD, Dr. William Brown. Yeats went on to name other faculty members who impacted him while he was in school and noted that they achieved a status among the profession that makes him proud to have attended OUCOD. Some of these influences include Drs. Walt Dilts, Earl Collard, Stewart Shapiro, Kevin Avery, Herb Shillingburg, Dean Robertson, and Tom Glass. When asked who his role models have been, he mentioned some of the above and then went on to describe a faculty member who came to OUCOD just after Yeats had completed his first year of school. According to Yeats, this dentist had a southern California accent and “still looked like a kid.” When he saw this new faculty member’s work he realized he wanted to develop hands like that. Yeats recalled, “Dr. Frank Miranda had a friendly, calm and caring personality and built a highly respected career over his many years of teaching.” 40 journal | Nov/Dec 2021

Even all these years later, Yeats would advise dental students at OUCOD to start their careers with confidence knowing that they have received an excellent education. “Even though you may be done with school, never stop learning. And maybe add business to your education list!” Yeats met his wife, Martha, in 1985 and the couple later married in 1987. During their many years together, they have enjoyed traveling and finding time to give back to their community. Philanthropy plays a vital role in their lives. He and Martha have worked with the Multiple Sclerosis Society of America for twenty years and work on the annual “Walk MS: Tulsa” sponsored by the National Multiple Sclerosis Society. They have also worked with St. Jude Hospital for many years. Since 2012, Yeats has also worn

several hats with Meals on Wheels, but notes that his favorite is delivering the meals to the elderly and disadvantaged. They always greet him with a smile and some conversation or a simple request, like bringing in the garbage cans, which Yeats is always happy to help them with. The couple also does work with the Tulsa Community Foundation. According to Yeats, giving back is the best joy of all! Yeats has been a loyal member of the ODA for 44 years and is now enjoying his lifetime membership. As a consistently active member for so long, he now gets to appreciate all the benefits of ODA membership for $0 dues. He believes membership is a “must” for young

EDMOND ORAL SURGEON DR. ROBERT LAMB RECEIVES NATIONAL HONOR In September, the American Association of Oral and Maxillofacial Surgeons held its Annual Meeting in Nashville, Tennessee. During Opening Ceremonies, more than 30 award recipients were honored for their many accomplishments. Among them was Dr. Robert Lamb of Edmond who received the AAOMS Humanitarian of the Year Award. A retired Colonel (USAF), Lamb received his OMS training at the University of Oklahoma and was subsequently assigned to Tinker AFB. Following his military commitment, he practiced for many years in Edmond before his recent and well-deserved retirement. He was coaxed out of retirement to join the OMS faculty at OUCOD where he currently teaches predoctoral students and OMS residents. Lamb’s selection was based largely on his numerous dental mission trips to Central America. He ultimately established a permanent clinic there, and then organized other dentists to staff it on a rotating basis. Each year since the clinic’s establishment, Lamb has spent multiple weekly rotations there. As if that wasn’t enough, he has also served for many years as the Oral Surgery Lead for OkMOM. Even after turning the reins over to other surgeons, he continued to mentor the new OMS Leads.

Lamb has done all of this completely “below the radar,” never seeking personal recognition. For his years of dedication and selfless service to others, this AAOMS award is extremely well-deserved and quite an honor for a man who counts being a husband, father and grandfather as his most important accomplishments. During the coming year, Lamb will serve as President of the American College of Dentists after years of service as a District Regent. The ACD’s principal focus is the promotion of ethics and diversity in dentistry. There is no person more qualified to fill this role than Dr. Robert Lamb. It was a true joy to join his wife, Donna, and their children who were in Nashville to witness the award presentation. Congratulations, Dr. Lamb! Following his recognition, Dr. Lamb was approached about what this award has meant to him personally. The following are his reflections. WHAT DOES THIS AWARD MEAN TO YOU? There is no greater honor than being recognized by your peers, but I did not do the work to be recognized or to get an award. I’ve gone to Guatemala for nearly 30 years because I enjoy serving people less fortunate. Many people don’t realize how fortunate we are to live in the United States and have the advantage of a great healthcare system. HOW IS THIS AWARD SELECTED? I was nominated by a fellow oral and maxillofacial surgeon. The American Association of Oral and Maxillofacial Surgeons Board of Trustees reviewed the award packet. The Humanitarian of the Year Award was established in 1997 to honor an AAOMS Fellow or Member for participation in community leadership or volunteer projects, including efforts on an international basis. Two oral and maxillofacial surgeons are selected per year for this award. I was particularly honored to have this award presented by a friend, Dr. B. D. Tiner, President of AAOMS. TELL US A LITTLE BIT ABOUT YOUR INVOLVEMENT IN THE AAOMS. I have not been involved much at the national level of AAOMS, but I was President of the Oklahoma Society of Oral and Maxillofacial Surgeons in 1995-96. Then I served as the Legislative Chair of the Oklahoma Society of Oral and Maxillofacial Surgeons for about 20 years. ALTERNATIVE OPTIONS FOR PROVISIONAL PROSTHESES

By: Michael Steffen, DMD; David Wong, DDS; Terry Philbin, DDS Immediate dental implants have become an attractive alternative to conventional implants. While there are many advantages of immediate implants, there are situations where the extraction site does not possess adequate bone or soft tissue thickness to support an implant immediately. In these situations repair of the volumetric defect is required for a more ideal implant placement. When immediate loading is not possible provisionalization of the implant area becomes very important and can be challenging (for example, when a patient is informed of the prospect of wearing a provisional removable prosthesis). It is important that the hard and soft tissues of the implant area drive the entire treatment process rather than just the provisional restoration.

not advisable, the soft and hard tissues are best supported by a provisional resin-bonded bridge. A custom-fabricated healing abutment may also be delivered once the implant is placed in order to help guide the soft tissue and assist in shaping the emergence profile of the prosthesis.

When determining if an immediate provisional should be considered, the primary stability of the implant is often assessed with either a torque test or an implant stability quotient (ISQ). When primary stability is inadequate, a custom healing abutment may be placed instead of a provisional restoration until the implant has properly healed.

CASE #1: EXTRACTION AND RESTORATION OF COMPROMISED ANTERIOR LEFT CENTRAL AND LATERAL INCISORS (#9 AND #10). To allow the area to heal without immediate loading, provisionalization was achieved with a Maryland bridge. After an adequate period of healing, the Maryland bridge was removed, the implant and custom abutment were placed, and the Maryland bridge reattached until full stability of the implant was achieved.

CASE #2: REPLACEMENT OF MULTIPLE TEETH IN THE ANTERIOR ESTHETIC ZONE LOST DUE TO FAILING ENDODONTIC TREATMENT AND PERIODONTAL DISEASE. Planned final treatment involved an implant-supported six-unit fixed bridge spanning from #6 to #11. Because of the need for ridge augmentation, immediate implant placement was contraindicated. Provisionalization with a temporary fixed bridge provided somewhat pleasing and functional esthetics without unduly compressing the delicate post-surgical bone grafting sites. Since the space will shrink as the site heals, a second provisional bridge may be indicated following placement of the implants.

CASE #3: EXTRACTION AND IMPLANT RESTORATION OF MAXILLARY LEFT FIRST MOLAR (#14). In the first two cases, the focus was on balancing the patient’s desire for functionality while restoring both the hard and soft tissues for an ideal final prosthesis. In this case, since primary stability of the implant could not be immediately achieved the Anatotemp SC system, which combines both a prefabricated anatomic healing abutment and a digital impression body, was used. With such a system, several appointments can be saved; once the implant is placed and the healing cap attached it can be scanned all in one appointment. There is no need to remove the healing abutment and place an impression post or scan body once the implant is integrated. This can also be done at the implant surgeon’s office as well, saving several appointments at the restorative dentist’s office.

HYGIENE HOTSPOT It is NOT “Just a Cleaning” By: Shelly Short, RDH If you ask anyone in the dental business office what frequent comment a patient might make when cancelling a dental hygiene appointment, they’ll probably say “It’s just a cleaning.” We know that dental patients are often notorious procrastinators when it comes to scheduling oral health care visits, but they are also experts at finding barriers to receiving dental care. Why do our patients have this perception that time spent with a dental hygienist is “just a cleaning” so it can simply be postponed? For one thing, we in the hygiene profession have created this devalued mindset for our patients because we refer to the next “cleaning” appointment. A second major challenge is the often silent and asymptomatic nature of diseases of the oral cavity. Patients do not “feel” dental decay spreading and destroying another tooth surface. The same is true with periodontal disease; patients do not realize that the supporting structures of their teeth are compromised due to bacteria and inflammation. Sadly, oral cancer is often undiagnosed because patients do not feel lumps, bumps, dots and spots that are pre-cancerous or cancerous. Unlike most medical issues that have a pain factor that triggers one to call for a medical appointment, many dental issues often lack such a pain-driving trigger. Without pain or discomfort, it is not hard to understand why people procrastinate with dental appointments. As a profession we must teach our patients that hygiene procedures are either on the preventive or therapeutic treatment continuum. When scheduling a future hygiene appointment it is essential that we explain why the prescribed time interval is in the patient’s best interest. It is equally essential that the patient understand that their preventive care appointment will include as necessary a head and neck oral cancer screening, radiographs, an assessment of the integrity of dental restorations, and an assessment of the health of the supporting tooth structures. To counter the challenge of oral diseases being silent in nature, dentistry has a strong and uniquely preventive nature! When patients understand the “value” of preventive dental hygiene care with the goal of preventing disease or early detection, they have a reason to return for future hygiene care. When the entire dental team educates and communicates value regarding the hygiene visit dental practices will have fewer cancelled hygiene appointments, and ultimately healthier patients. The pandemic has certainly shed light on the importance of optimal health and immunity as research continues to unpack the impact of oral health on systemic health. Reducing the inflammatory burden in the mouth has a positive influence on other systemic medical concerns. Our dental community has an opportunity to elevate oral health care by jettisoning the reference to a “cleaning” appointment. Dental hygienists are preventive specialists and oral health care educators that provide preventive and/or therapeutic dental procedures.

Upon completion of the hygiene visit it is imperative that the clinician summarize dental concerns, services provided, and the purpose of continued dental hygiene care and how it benefits the patient. Also, when it is understood that delaying preventive care may necessitate therapeutic treatment the patient’s motivation for keeping a hygiene visit is increased. When patients are committed to their next preventive or therapeutic treatment procedure with their dental hygienist, the business team can then direct efforts to promoting the dental practice rather than perpetually rebuilding the hygiene schedule! As the pandemic has brought center stage the need to send a stronger educational message about the importance of hygiene retention we are, at the same time, experiencing a shortage of dental hygienists to meet treatment demands. With an abundance of research revealing the vital role of oral health on systemic health, it is distressing that so many last-minute cancellations continue to occur in the hygiene department. As a health care profession, we have a responsibility to educate our patients about the importance of dental hygiene care. Until we deprogram ourselves from using the devalued “cleaning” terminology patients will continue to delay oral health care. We must reflect the change we want to see in our patients by giving them a reason to return for preventive or therapeutic hygiene care. As a bonus, with the appropriate appointment our patients will receive a “professional cosmetic polish!” ABOUT THE AUTHOR: Working in the dental field for more than 35 years, Shelly Short has consulted and coached in dentistry since 1996. Prior to owning a dental practice management consulting business, she was a senior practice management business and clinical efficiency consultant for an international practice management company. She has expertise in practice analysis, business management, clinical efficiency, non-surgical periodontal programs, health education and health promotion, team building, interpersonal communications including case presentation, and the effect of clinician/patient rapport following the health-belief model all wrapped around a holistic approach to professional and personal well-being – particularly work-life balance. Short practiced clinical dental hygiene in Zurich, Switzerland for six years. While working overseas, she gained extensive experience treating periodontally compromised patients. She was an active member of the American Dental Hygienists’ Organization of Switzerland, which emphasized continuing education. Currently, at the University of Oklahoma College of Dentistry Short is a Clinical Assistant Professor, Division of Community Dentistry, and Course Director for Practice Management, and a former


Adjunct Clinical Faculty Member in the Department of Dental Hygiene. She serves on the Pediatric Dental Assistants Association Advisory Committee and is on the Board of Trustees for the Oklahoma Dental Foundation. Additionally, she was the 2019 Oklahoma College of Dentistry Dental Hygiene Alumna of the Year recipient and is an accomplished speaker and writer. Short consults across the United States, and has also consulted in the U.K., Denmark, Switzerland, and Italy. Her bachelor’s degree is in biology sciences dental hygiene, and her master’s degree in nutritional sciences with an emphasis in health promotion. She has also earned an alternative correspondence doctorate of philosophy in holistic nutrition.

OU College of Dentistry Announces Director for New Tulsa Clinic From Official Press Release on October 25, 2021 The University of Oklahoma College of Dentistry has named Chalmers Rieger Wood III, D.D.S., to direct its new clinical operation in Tulsa. The expansion of the College of Dentistry’s educational programs and clinical services in Tulsa was approved last week by the Oklahoma State Regents for Higher Education. The new dental clinic will be housed within the OU Health Physicians Schusterman Center Clinic building on the OU-Tulsa campus, with construction of a 5,000-square-foot, stateof-the-art dental clinic beginning soon. An OU College of Dentistry faculty member since 2006, Wood was named clinical assistant professor in 2014 and currently serves as a group practice director in the Division of Comprehensive Care. Wood also served on the college’s COVID-19 Task Force, COVID-19 Re-engagement Committee, and strategic workgroups engaged in the planning and implementation of the forthcoming dental program expansion in Tulsa. Wood earned his Doctor of Dental Surgery degree at the OU College of Dentistry in 1981. That same year, he established a private dental practice in Tulsa, which he still owns and operates to this day. Wood also completed the Advanced Education in General Education residency program at the College of Dentistry in 2015.

Care will be provided by thirdand fourth-year dental students, under the supervision of Wood and other licensed dental faculty members. The clinic is the first of several OU College of Dentistry clinical extensions anticipated throughout the state.
