ODA Rewards Program Offers You Savings in Practice Expenses and Reduced Dues Your ODA Rewards Program is still alive and kickin’! Did you know that as an ODA member you can save an average of $14,254 in one year if you used every product/service the ODA endorses? In 2016, the ODA received more than $143,000 in non-dues revenue because our members used ODA Rewards products and services, saving each member the equivalent of $133 in membership dues! The ODA Rewards Program seeks to endorse companies with a broad-based appeal to its members and their staff. It is comprised of a number of products and services that provide real savings for you both personal and professional - like office services, insurance, financial services and awesome travel discounts. Every partnership is evaluated on a case-bycase basis and represents a win-win-win relationship: a win for the company, a win for the ODA and, most importantly, a win for you, the ODA member! The companies chosen by the ODA Board of Trustees are thoroughly researched, must meet a list of criteria, and must pass an extensive evaluation-of-success process before ultimately being selected as an ODA Rewards partner. The Board of Trustees’ Standing Committee on ODA Rewards Partners compares each company’s proposal to a minimum of two of their biggest competitors before ultimately referring the company to the Board, which makes the final decision to name and endorse an “ODA Rewards Partner.” These relationships are re-evaluated twice a year. New companies are added and less effective companies are removed. 6

Dr. David Birdwell

The rewards committee is continuing to seek out new relationships that can help provide revenues to minimize increases in our membership dues. This past year, we have added a great partner by collaborating with the University of Oklahoma College of Dentistry. The College has a significant budget for clinical supplies and is continually working hard to keep these costs low but not compromise the professional clinical experience. The College, in support of your ODA, has agreed to purchase their gloves through the Association Gloves. Our thanks to Dr. Paul Mullasseril and Dean Raymond Cohlmia for working on this project. They are looking at other ways the school can help partner with your ODA. For this issue of the ODA Journal, we are once again spotlighting all of the Partners in our Rewards Program. We feel it is vital that our members trust and know the companies we ultimately endorse and promote, and we encourage you to take advantage of the great, members-only savings these Partners have to offer us. There are a wide variety of products available -- everything from insurance coverage to medical waste to office supplies to a new Mercedes! Please take a moment to review them all. I also ask that you please pass this information on to your staff responsible for ordering products, as well as to your family who might take advantage of the personal services offered. Beginning on page 34 you will have the opportunity to look at each company endorsed by the ODA and learn more

about what they have to offer. We are confident in our promotion of each company – in what they can and will do for you – and we hope you will take advantage of the great opportunity to work with them to save you and your practice money. If you have any problems or questions about any of the ODA Rewards Partners, please don’t hesitate to contact Britney Morris, ODA Operations Manager, at bmorris@okda.org or 405.848.8873. Cyber Liability Insurance Covers More Than You Know Who is going to pay for your defense and indemnity costs if a data breach occurs in your business and you are held liable?

A violation of Oklahoma Title 24 Sec. 161-166 may result in a penalty up to $150K. Who is going to defend you during an investigation or pay potential fines?

In most cases you are required to notify your clients if a data breach occurs. Who is going to pay for this notification process and possibly credit monitoring?

If your system is damaged the costs to hire outside experts and consultants and potential loss of business income to rebuild/restore your data will be significant.

Extortion, Identity Theft, Telephone Hacking, and Social Engineering (phishing) are the most costly and common claims to occur to a small business.

Allegations against you for what is on your website or social media postings are a very large exposure. You may not be able to control what is inadvertently said in social media by one of your employees which might cause harm to your clients.

f

Typically a public relations consultant will be needed to preserve your reputation if a Cyber Liability claim occurs.

Important Information for Your Public Employee Patients The Oklahoma Dental Association recommends that you display the flyer on the opposite page in your waiting area or at your front-desk check-out window to help better inform your patients who are public employees about dental insurance and options that may be available to them and their families. Below are some “talking points” that may assist you and your office staff when discussing dental insurance with your public employee patients: •

DENTAL INSURANCE IS NOT LIKE MEDICAL INSURANCE. Dental insurance is more like a prepaid gift card or phone card – your dental insurance has a maximum allowable per year and when it is spent, it is spent. So it is very important to familiarize yourself with all of the dental insurance options available to you in order to get the most “bang for your buck”.

MAKE SURE YOU EVALUATE YOUR COVERAGE EVERY YEAR. Your enrollment/renewal period is in October and you can make changes during that time. Benefits and coverage within the same plan may change from year to year, and if you don’t stay informed, your benefits could decrease or even change so much that they no longer meet your and your family’s dental needs.

LOOK CLOSELY AT ALL OF YOUR OPTIONS! Sometimes, for just a few dollars more, you can get $500 or $1,000 more in benefits per year. Ask questions and make an informed decision!

BE SURE AND MAKE THE RIGHT CHOICE REGARDING COVERAGE FOR YOU AND YOUR FAMILY. For example, regarding referrals to specialists for orthodontics or root canals, some dental insurance plans don’t cover these services and some do.

STAY INFORMED ABOUT HOW MUCH OF YOUR DENTAL INSURANCE YOU HAVE USED. Don’t let your dental benefits go to waste by not using it all before the year ends. Your oral health is just as important as your overall health. Dental insurance is a benefit to you as a public employee and you should use 100% of it every year in order to maintain your and your family’s oral health.

10 journal | Sept/Oct 2017

Please cut out or copy the following page and place at your front desk. This information will help your patients make educated decisions about their health insurance.

D-DENT’s 21st Veterans Golf Tournament Scheduled for October 23 Each year, supporters of D-DENT’s Golf Tournament come together to celebrate, network, and raise funds for veterans' oral health. This year’s tournament will take place on October 23 at River Oaks Golf Club in Edmond, OK. D-DENT is a non-profit organization that provides free comprehensive dental care through a network of volunteer dental professionals, for individuals with I/DD, the elderly, and veterans residing in Oklahoma. D-DENT volunteer dentists work out of their private clinics, donating their time, equipment, staff, and supplies. D-DENT pays any dental laboratory fees the dentists may incur during the patient’s dental restoration. This partnership with dentists allows D-DENT to complete a patient’s full mouth restoration, absolutely free. D-DENT is the only non-profit dental program that offers free comprehensive dental restoration statewide. D-DENT has been in operation for 30 years and has been recognized with numerous awards throughout its history. Since 1986 over 600 volunteer dental professionals statewide have donated comprehensive dental services valued at over $20 million.

ODA MARKETING COACH

Marketing Your Dental Practice: What it Means Marketing matters more than ever before. A recent report from the American Dental Association’s Health Policy Institute (HPI) revealed that the percentage of Americans aged 19-64 who saw a dentist dropped from 41 percent in 2003 to 35.5 percent in 2013. As a result, many practices had excess capacity. When done correctly, marketing can be an effective tactic for bringing in new patients and is becoming more commonplace in all types of communities. It’s an important element for any practice and especially critical for those opening or purchasing practices.

Recognize the differences and similarities in marketing and branding.

Identify your brand.

Determine which marketing channels are best for your practice.

Decide who should manage the marketing of your practice.

Create and implement a formal marketing plan.

Deciding how and where to market your practice can seem overwhelming. The ADA’s Center for Professional Success includes a Managing Marketing module that will help guide you through the different aspects of marketing, including branding, internal marketing, website development, print marketing, referral generating techniques and advertising. Articles will provide information and resources that will help you:

Create and manage the practice’s social media footprint.

Understand Search Engine Optimization (SEO) and Pay Per Click (PPC).

Ensure that your website is accessible, effective and easy to navigate on multiple platforms.

Understand what marketing is – and what it isn’t.

Assess your current marketing efforts.

Identify the type of patients you want to attract to the practice.

The content contained in the Managing Marketing module of the ADA’s Guidelines for Practice Success TM (GPSTM) details aspects of marketing via nine major topics that offer more detailed information about the options available. The module also includes original resources, such as checklists and tip sheets, which were

created specifically for this project and designed to help ADA members manage the marketing process. Future issues of the ODA Journal will include individuals articles from this module, but you can see the module in its entirety at http://success.ada.org/en/ practice-management/marketing. ODA COUNCIL MEETING BRIEFS ODA Council on Dental Education and Public Information Dr. David Wong, Chair

The Council on Dental Education and Public Information met on June 16, 2017 in Oklahoma City. Members discussed the current ODA website and the opportunity to build a new site using the ADA-branded template. The Council approved the motion to remain with the current website provider and design and to offer website advertising opportunities as a new nondues revenue source. The Council requested practice management articles in the ODA Journal that provide advice and checklists to help members with their digital presence and online reputation. The articles will then be placed on the ODA’s member-only website for constant reference (see page 14 for more information). Dr. Juan Lopez will lead a task force designed to create a CE course that can be presented at study clubs around the state, but specifically in rural areas. The purpose is to provide practice management CE while using that opportunity to talk about the value and benefits of an ODA membership. The next Council on Dental Education and Public Information will be held at the Oklahoma Dental Association on January 19, 2018 at 12:00 p.m.

ODA Council on Governmental Affairs Dr. Matt Cohlmia, Chair

The ODA Governmental Affairs Council met on July 14 and after reviewing the 2017 legislative session, began work on the ODA’s 2018 legislative priorities. The Council will be forming a coalition of representatives from all the other Medicaid providers to pursue legislation to make changes to the Medicaid Integrity Audit process. The coalition’s first meeting is scheduled for September 29th. The next Governmental Affairs Council meeting will be November 3, 2017 at 9:00 a.m.

ODA Council on Membership and Membership Services Dr. Edward Harroz III, Chair

The Council on Membership and Membership Services met for the first time with its new chair, Dr. Edward Harroz III on July 21, 2017. The Council discussed plans for membership activities with new programs being introduced for new dentists who have been practicing ten years or less, and dental students at the University of Oklahoma College of Dentistry. The Council discussed the Resolution to add four dental student seats to the House of Delegates as well as created task forces to explore a hygiene membership category, and how we can better reach dentists employed by dental service organizations. The next Council on Membership and Membership Services meeting will be held at the Oklahoma Dental Association on January 26, 2018 at 1:00 p.m.

Want more information? All ODA Council meeting minutes are posted online. Log in to the Members Only website at okda.org and click on Your Association on the left-hand side for the complete list of Councils.

OKLAHOMA DENTAL FOUNDATION

Supporting Rural Oklahoma By Kaylan Goodwin

Traveling across the state of Oklahoma, MobileSmiles works to deliver dental care to those who need it most by partnering with local agencies that agree to host the Mobile unit in their towns. Site partners are a vital part of MobileSmiles’ success at any given location. MobileSmiles is thrilled to welcome Salina Area Chamber of Commerce (SACC) as a new site partner. SACC is a volunteer organization primarily focused on the economic advancement and well being of Salina, a rural Oklahoma town of about 1,600 people. “In most parts of Oklahoma, especially the more rural areas, access to dental care is a serious problem,” said John Wilguess, Executive Director of the Oklahoma Dental Foundation. “Partnering with local sites as our boots on the ground before we arrive makes all the difference in the success of our visit. We are thrilled to have Salina Area Chamber of Commerce on board as an exceptional new partner.” Sanders Barnes, SACC President, first reached out to MobileSmiles at the suggestion of a colleague at NeoHealth, a federally qualified health center in Salina serving the town’s insured, uninsured and under-insured population. Barnes presented the opportunity to SACC, which immediately approved the muchneeded service for the community. “When Kimberly Dry of NeoHealth informed me of MobileSmiles’ services,” Barnes said, “I knew it was something we needed to consider. Compassion runs deep here and we recognize that dental problems often cause other health-related issues.” MobileSmiles always relies on local partners to schedule patients ahead of time from the

24 journal | Sept/Oct 2017

existing client base. NeoHealth served as a supporting agency and helped SACC screen and pre-qualify patients from the Salina area for treatment in the MobileSmiles unit. To date, MobileSmiles has been to Salina twice, treating a total of 24 patients. Barnes is quick to point out the impact the visits have had, not only for those who received treatment, but also on the entire community. “I have visited with folks who received care from MobileSmiles and their praises ring loud and clear,” Barnes said. “I had the pleasure of sharing a meal with a gentleman who received extensive care from the team and he was just so grateful to be able to chew his food without discomfort or pain.” For those who live in rural Oklahoma, a dental office isn’t always readily accessible. Often, the nearest dentist requires a trip out of town, and not everyone is able to give dental care the priority it needs. “When patients can picture the mobile unit here in town and know that they can receive treatment – that is priceless,” Barnes said. “Needs get met, health improves and quality of life is better. Partnering with MobileSmiles is a win-win.”

LEGISLATIVE LOOP Legislative Overview & Political Update

Celebrating Bipartisan Health Care Victories If you open a newspaper or turn on a cable news network program these days, you’ll hear stories about the crippling partisan divide in Washington when it comes to nearly any political issue, but most especially when it comes to reforming our nation’s health care system. Reporters have subscribed to the saying that ‘if it bleeds it leads,’ and Americans are left with the most divisive and explosive political stories to read and watch, with no mention of the wins Congress is delivering to the American people in a bipartisan way. Not many reporters are interested in writing about two members of the opposite party working together to reauthorize major health programs, or pass legislation to improve preventive health care measures. The truth is Congress is working better than the public thinks, and some of the biggest bipartisan accomplishments so far are changes in health care policy. Improving our nation’s health care system isn’t a one-hit-wonder when it comes to a single piece of legislation. It takes several small steps at a time to move closer to a better health care system. Examples include the RACE for Children Act (H.R. 1231) which helps deliver lifesaving treatments for pediatric cancer patients and the FDA Reauthorization Act (FDARA) of 2017 (H.R. 2430) which reauthorizes the FDA’s user fees program which is crucial to allow the FDA to effectively and efficiently bring safe devices, drugs, and treatments to patients. Both passed the House with unanimous bipartisan support. Another piece of health care legislation particularly important to the dental community is the Action for Dental Health Act of 2017 (H.R. 2422). H.R 2422 passed out of the House Energy and Commerce Committee in late July. As a member of the Energy and

26 journal | Sept/Oct 2017

Commerce Committee, I was glad to see this legislation pass out of committee with support from both sides of the aisle. In the coming months, we’ll hope to see this piece of legislation come to the House Floor for a full vote.

I’ll continue to work with my colleagues to pass reforms that improve health care for all Oklahomans. If my office can ever do anything to assist you, I hope you won’t hesitate to let me know by calling one of my offices or visiting Mullin.House.Gov.

The Action for Dental Health Act of 2017 reauthorizes oral health promotion and disease prevention programs through Fiscal Year 2022 and allows the Centers for Disease Control and Prevention (CDC) to award grants for portable or mobile dental equipment and assist in providing dental services free of charge to underserved populations. This bill aims to deliver high quality care to rural parts of our country, so that getting a checkup or a teeth cleaning is easier and closer to home than ever before. As the representative of Oklahoma’s Second District with several rural communities in my district, I know firsthand the difficulties of delivering health care to America’s rural communities. Last August, I visited the Sallisaw Health and Wellness Center and was able to see their new telemedicine unit and their mobile dental unit. These kinds of technological advancements in medicine allow for patients in even the most rural areas of our country to access the care they need in order to stay healthy. This coming fall, Congress will be working across the aisle to reauthorize the Children’s Health Insurance Program (CHIP), which provides coverage for dental services for children. The Energy and Commerce Committee has already started their work on reauthorizing this important piece of legislation by holding hearings and engaging stakeholders like the American Dental Association (ADA). CHIP has historically been a bipartisan bill, and I look forward to working with my colleagues on both sides of the aisle in its reauthorization this year.

Congressman Markwayne Mullin was first elected to serve the people of Oklahoma’s Second Congressional District in November 2012. He is currently serving his third term in office. Mullin serves on the House Energy and Commerce Committee and sits on three subcommittees: the Subcommittee on Energy, the Subcommittee on Health, and the Subcommittee on Digital Commerce and Consumer Protection. Mullin is a member of a number of caucuses, and is a co-chair of the Men’s Health Caucus and of the Regulatory Review Caucus.

ODA MEMBER SPOTLIGHT

Edward Harroz III, DDS Midwest City, OK

Sometimes, life can get hectic. Our jobs can be overwhelming. School activity schedules begin to feel like a part-time job. Family time can easily fall to the bottom of the list, with little or no priority left for involvement with organized dentistry. This is a trend that the Oklahoma Dental Association and many other professional associations seem to be facing. With membership being the lifeblood of the ODA, how do we change this trend to make involvement a priority again? Enter Dr. Edward Harroz III, the current chair of the ODA’s Council on Membership and Membership Services. A 2007 graduate of the University of Oklahoma College of Dentistry, Dr. Harroz accepting the 2017 ODA Young Dentist Award.

28 journal | Sept/Oct 2017

Harroz appreciates the importance of membership to the success of the ODA. “My goals are simple. I want people to look forward to being involved in our different activities, and I want the ODA to be all-inclusive and more active.” With that in mind, Harroz has embarked upon an exciting plan for the Membership Council during his tenure as chair.

Dr. Harroz talking to OUCOD graduates about organized dentistry during the 2017 ODA Senior Signing Day.

Harroz feels that a major key to success is inclusivity. “I think we need to treat everyone as equals. It is important for the older dentists to engage with the younger dentists, the males to interact with the females – for each group to identify with everyone else.” He plans to create activities that will engage all ODA members, active and inactive. Overall, he hopes to accomplish a sense of camaraderie, which he personally feels is the most important aspect of ODA membership. “The one thing we have in common is that we are all dentists in the state of Oklahoma. Being able to bounce different ideas off my peers and mentors is priceless to me.” Another aspect that Harroz finds important is an evolving leadership. Recruiting new people to become involved on different committees will create new ideas with the same mission in mind. Harroz’s advice to those looking to become involved? Talk with past or present ODA leaders and to our ODA Executive Director, Lynn Means. Then, find your passion! “Our members need to find an understanding of when, where and how to enjoy their time with the ODA. I want membership to mean more than a sticker each year.

I want members to understand their purpose. If leadership isn’t quite what you’re looking for, don’t sweat it. By just retaining your membership, you make a vital contribution to the association. Your membership experience is what you make of it. The legislative fight is important and our voice as ONE is important. There is power in numbers – both of dollars and of warm bodies. The ODA needs you to be a part of organized dentistry.” Above all, Harroz feels that the relationships he has made during his time as an ODA member have been more than rewarding. “An ODA dentist is someone that I truly respect and appreciate. Our members have wonderful hearts and take great care of the people of our state. I am so proud to be a small part of the fantastic voice that the ODA has.” Harroz and his wife, Jessica, have been married for ten years and have three children, Gia [5], Nyssa [3] and Edison [2]. When he isn’t at his practice in Midwest City, you can most likely find Harroz playing golf, making travel plans, or working to make the ODA membership even better. To learn more about leadership opportunities, events and ways to become involved, please visit the ODA website at okda.org or contact Madison Huffines, Membership Communications Manager, at mhuffines@okda.org or 405.848.8873.

Celebrating Dr. J. Dean Robertson On June 23, the ODA and the OU College of Dentistry hosted a celebration for Dr. J. Dean Robertson's 100th birthday. Many dentists, young and seasoned, came to shake his hand, give him a hug, and wish him a happy birthday. Numerous guests spoke about how Dr. Robertson influenced them on a personal and professional level. Thank you to everyone who joined in this wonderful celebration! 1. Dr. Robertson was surrounded by friends, family, and colleagues of all ages. 2. A few of the recipients of the JDR scholarships from the OUCOD attended and had an opportunity to show their gratitude. 3. ODA President Dr. Juan Lopez with Dr. Robertson. 4. Oklahoma City pediatric dentist Dr. Martha Garzon was greatly influenced by Dr. Robertson. 5. Former OUCOD Deans, Dr. Russell Stratton and Dr. Stephen Young, spoke about how Dr. Robertson paved the way for the future of OUCOD. 6. Drs. Ray Cohlmia, Raymond Cohlmia, and Frank Miranda had many fond memories of their time working with Dr. Robertson. 7. Dr. Robertson with Dr. Fräns Currier, a faculty member at OUCOD. 8. Pediatric dentist Dr. Tim Fagan had Dr. Robertson autograph his copy of Fundamentals of Pediatric Dentistry, a textbook co-authored by Dr. Robertson, and was used in the education of many dental students in the 1980s.

ODA FEATURE

Know How to Treat Your Patients in Recovery By Robert Westcott, M.D. Oklahoma is in the middle of an addiction epidemic. This is especially true of opiate addiction. It is important for all health care providers to educate themselves about this disease, for we may unknowingly play a role in it, especially those of us who prescribe controlled substances. This article will provide an overview of addiction and how we not only contribute to the problem, but also how we can help address it. The Prescription Monitoring Program (PMP) is operated by the Oklahoma Bureau of Narcotics and Dangerous Drugs and is designed to deter the abuse of prescription drugs. Oklahoma was one of the first states to create such a program. Since its development, we now have access to a report of all the controlled substances prescribed for any given patient. You can access prescription records by your own DEA number to see what drugs and medications you have prescribed. This will help you determine if someone has illegally obtained your DEA number and written prescriptions in your name without your knowledge. Currently 49 states, Washington D.C. and Guam have similar programs. Recently provisions were established that allow you to look up data on patients in neighboring states. Oklahoma law requires mandatory PMP checking for all new patients or after 180 days has elapsed on established patients prior to prescribing any of the following:

30 journal | Sept/Oct 2017

opiates, synthetic opiates, semi-synthetic opiates, benzodiazepine or carisoprodol (Soma). There are exclusions for hospice or end-of-life, or patients residing in nursing facilities. Since the creation of the PMP, doctor shopping has nearly been eliminated for those healthcare providers who use it. In addition to being required by law, checking the PMP regularly can be an effective control mechanism. If a patient just got a prescription filled for a large quantity of narcotics, you would know not to duplicate that prescription or give them something similar. (Addicts will not usually volunteer this information.) You can also see what other controlled substances the patient is currently being prescribed, so as not to add to the problems of polypharmacy. The leading cause of death from drug overdoses occurs with mixing medications. The trifecta of mixing opioids, benzodiazepines and Soma is notorious as the most lethal combination. Dentists might not prescribe a lot of benzodiazepines or Soma, but if they have patients taking these medications it could be dangerous to also prescribe opiates. Drug addiction, like alcoholism, is a disease. It is not a respecter of persons; dentists and physicians are not immune. In fact, such professionals have a slightly higher addiction rate than the general population. According to the American Society of Addiction Medicine addiction is a “primary, chronic disease of brain reward, motivation, memory and related circuitry...Addiction is characterized by the inability to consistently abstain; impairment in behavioral control; craving; diminished recognition of significant problems with one’s behaviors and interpersonal relationships; and a dysfunctional emotional response.” Like other chronic diseases, addiction often involves cycles of relapse and remission. It is a very complicated process and our understanding of this disease has really progressed as a result of research in the neurobiology of the brain. But in layman’s terms, it is because of the cravings that people have such a hard time abstaining from drug use, and relapse often occurs.

When the addict has intense cravings, the rational thought of the prefrontal cortex is hijacked, and the midbrain’s neurotransmitters create intense feelings that do not make sense to the normal person. They feel like if they don’t use, they might die. When a drug addict is struggling to get their life back or they are in early recovery (i.e., abstinence plus a state of mind that has embraced a lifestyle based on honesty and spirituality), they can be very vulnerable to the medications prescribed for them. When the brain is exposed to something like an opiate, it can sometimes reignite the addiction, and the phenomenon of cravings begins again. Once this starts, these individuals are at risk for relapse, which can possibly be followed by years of abuse, legal consequences, destroyed families and even death. As practitioners we need to be aware of the patient in recovery. Hopefully, such patients will tell you that they are in recovery, and you can address the issue with them. Sometimes due to fear or shame they may not reveal the problem. If prescribing an opiate to someone in recovery, one option would be to have somebody else (e.g., a spouse) manage their medications for them. Perhaps they can get by with just NSAIDs and Tylenol.

Definitely do not prescribe more than they need. (This is true for all patients, not only those in recovery.) Addiction often starts with teenagers taking their mother's hydrocodone out of the medicine cabinet and may end with them on heroin before they graduate from high school. The next issue to be addressed is Medication-Assisted Treatment (MAT) using medications such as buprenorphine or methadone. MAT is a treatment method that, while controversial, gives an opiatedependent person a drug that limits their cravings and lets them feel normal. A MAT regimen, along with counseling and some type of 12-step program, can hopefully lead to recovery. At the very least, it lessens the prospects of prison or death, and allows the patient to have a fairly normal life. While both drugs are in the same category, they are very different

from each other. The following discussion focuses on buprenorphine products (Suboxone, Subutex, Butrans, Bunavail, Buprenorphine-Naloxone and Zubsolv). Patients on any of these medications will hopefully reveal this to you either directly or by disclosing that they are being treated for opiate addiction or chronic pain. This information can also be found by looking at the PMP. If a patient discloses that they’re taking Suboxone, what are you going to do differently? You should not prescribe any narcotics for pain without first talking to their physician. Buprenorphine (like methadone) is a very good analgesic. Maybe the patient will not need anything else for pain except Tylenol. Most patients can just slightly increase their Suboxone dosage. It is the patient’s responsibility to call the physician who prescribed the

Suboxone to ask for guidance regarding additional treatment of pain. For increased pain, the prescribing physician should be the primary pain management doctor as well as addictionologist. Many patients who are on buprenorphine or chronic pain medication (which would be obvious from consulting the PMP) have signed a contract with their doctor saying they cannot get a prescription for controlled substances from anybody but them. If you do have concerns, you should call the doctor and talk to them personally. Remember, the first tenet of medicine is “Do No Harm.” Dr. Robert Westcott is the Director of the Oklahoma Health Professionals Program.

National Recovery Month September is National Recovery Month. This observance highlights individuals who have reclaimed their lives in long-term recovery, but also honors the prevention, treatment, and recovery services that providers are making possible. Recovery Month promotes the message that recovery, in all its forms, is possible and also encourages citizens to take action to help expand and improve the availability of effective prevention, treatment, and recovery services for those in need. Millions of Americans’ lives have been transformed through recovery programs, including more than 1,000 health care providers in Oklahoma through the Oklahoma Health Professionals Program (OHPP). Since 1983, OHPP has provided services to health care providers who suffer with alcohol and chemical dependence.

If you or a colleague needs help from OHPP, contact them today at okhpp.org or 405.601.2536.

While in Jamaica, they performed extractions, fillings and cleanings on over 350 patients. “It’s been so awesome. The patients are so extremely thankful for any little time we spend with them and any way we can help them out. It’s been an awesome experience so far.” – Kenzie Karnish, OUCOD Class of 2019

On June 17, ASDA hosted its first-ever Summer Pre-Dental Simulation Course. Dean Raymond Cohlmia and Assistant Dean Kevin Haney spoke to the pre-dental students who then got to get their hands dirty in our preclinic lab waxing up a tooth, taking impressions of typodonts, pouring up casts, prepping a plastic tooth with a high-speed handpiece, and restoring it with amalgam and composite. According to ASDA President-Elect Brittany Hand (2020), “This new addition was a hit and we plan to keep it in the future!”

Real Estate

The Second Highest Expense in Your Practice When it comes to managing expenses in your practice, there are dozens of categories to evaluate: equipment, technology, loan costs and interest rates, marketing, and so on. Many practice owners are quick to shop out what they believe are the most obvious expenses, but few understand the true impact of two of the largest expenses, payroll and real estate, and how they can be reduced dramatically to increase profitability. Real estate encompasses your monthly rent or mortgage payments, along with the property’s operating expenses, utilities, maintenance fees, and janitorial costs. Of these top two expenses, only one of them is truly negotiable. You can either pay people their value or they will find another job that will. You can cut staff, but for those people you truly need you must pay them what they deserve or they will eventually leave. Real estate, however, is 100 percent negotiable. You have the choice of leasing or owning, as well as of being in a stand-alone building, a multi-office building, a retail center, or a large medical complex. You can choose the size of your space, its design, and the landlord you wish to work with -- or you can be your own landlord. And if you do own, you can decide whether to buy an existing building or an office condo, or develop your own building from the ground up. When negotiating the economic terms of a lease, you can negotiate the length of lease and such desired concessions as build-out period, tenant improvement allowance, free rent, lease rates, annual rate increases, etc.

With so many choices to evaluate and understanding that each one affects the final economic outcome, why is it that so many practices fail to capitalize on their real estate opportunities? The short answer is that most practice owners and administrators simply don’t have the knowledge and expertise in commercial real estate to understand how to make the most of these opportunities. They view real estate as a necessary evil instead of an incredible opportunity to improve profitability, reduce expenses, and improve the quality of their patients’ experiences. When the correct approach is taken, you may actually look forward to your real estate negotiation instead of dreading it. Let’s take a look at three key ideas that will help you make the most of your next real estate transaction. Timing Every type of transaction has an ideal timeframe to start the process. When you start too early or too late, you communicate to the landlord or seller that you don’t really know what you’re doing. Such a message hurts your ability to receive the best possible terms. For example, don’t wait for your landlord to approach you on a lease renewal negotiation. Consult with a professional so you can understand the ideal time frame to start your transaction and come up with a specific game plan for what you want to achieve. You can then be the one to approach your landlord with renewal terms. Representation Landlords and sellers prey on unrepresented tenants who don’t really know the market or what their options are. If the tenant was a Fortune 500 company, the landlord would approach them with a high level of respect, expecting that they either have a real estate broker hired to represent them or an internal team of professionals well equipped to handle the transaction. In contrast, when a landlord or seller starts speaking with a tenant who isn’t represented, and whom they suspect doesn’t know the market as well as the landlord/seller does, that tenant is not going to get the same level of respect throughout the process. The landlord will sense an opportunity to take advantage of the tenant that doesn’t

have the requisite real estate knowledge and skills, and that doesn’t have adequate representation. When you understand that commissions are paid in commercial real estate just like they are in residential real estate (set aside in advance for two parties, not just one), then you can appreciate that there aren’t any savings by not having a broker. Showing up without one only further detracts from your credibility. Leverage and Posture It is nearly impossible to emerge victorious from a negotiation without leverage and posture which are created by having multiple options in the market. If you limit yourself to one property, you are at the mercy of that property’s owner. Since most landlords and sellers negotiate professionally, it is easy for them to know when you don’t have other viable options. Simply telling a landlord that you have a proposal from another landlord won’t give you a strong enough posture. Most landlords look at unrepresented tenants and assume they don’t know the market, don’t understand all their options, and aren’t really serious about making the landlord compete for their business. Leverage and posture are created when you have the right timing, professional representation, an understanding of all your available options, and a detailed game plan of what you want to accomplish in order to capitalize on the market. These three key ideas are the first of many factors that allow healthcare tenants and buyers to reduce their second highest expense which will dramatically impact their profitability and cash flow. Carr Healthcare Realty is the nation’s leading provider of commercial real estate services for healthcare tenants and buyers. Every year, thousands of medical, dental, veterinary and other healthcare practices trust Carr to achieve the most favorable terms on lease and purchase negotiations. Carr’s team of experts can assist with start-ups, lease renewals, expansions, relocations, additional offices, purchases, and practice transitions. Healthcare practices choose Carr to save them a substantial amount of time and money, while ensuring that their interests always come first.

It's Time for Some R and R

seeing them. Have each person in the practice send at least five cards a week to patients. - Send a package with a toothbrush and floss (I guarantee they will open it). •

Send birthday, anniversary, holiday, congratulations and other thoughtful cards.

Use email to send patients seasonal communications. Some great topics to consider are “Spring Cleaning,” “Get Ready for Back-to-School,” and “Fall Use It or Lose It” reminding them of their dental insurance benefits.

By Sandy Pardue

Success lies in the combination of being able to attract new patients and convert them into long-term, loyal dental advocates. Unfortunately, I have found many practices have a revolving front door, with patient retention averaging between 40 and 50 percent. This means that 50 to 60 percent of the patients who came into a practice last year may go right through the revolving door and not return this year. I believe all practices already have the resources and patients they need to grow – from within. They just need time for a little R and R and R – Retention, Recall and Reactivation. The Most Important R: Retention Wouldn’t it be nice if dental practices were filled with patients who can’t wait for their next appointment? In reality, to improve patient retention, teams should start by adding more value to visits and improving patient education. Work on building stronger relationships with patients. Communicate on a consistent basis, and be interested in them as humans first, patients second. And of course, take the time to educate them. The public, in general, may have a very low dental IQ, especially when it comes to the value of hygiene and the role of dental insurance benefits. It makes our jobs challenging, but we must educate patients and add value to their lives every single time they come into the practice. Retaining patients is all about building good patient relationships. Because patients are most likely to visit the practice at most two times a year, building relationships takes a bit more work. Here are a few ideas: •

Send handwritten notes to overdue patients. Let patients know you miss

Place a phone call reminding them of incomplete dentistry. If cost was a barrier to care, it would be important to review payment options with them, including a financing option through a third-party company.

requires advanced notice, since the time has been reserved for them. For example: “We are confirming your appointment. If you are unable to keep this appointment, please call the office at (phone number with area code) as soon as possible so that the time reserved can be utilized. 4. Send a reminder text three days prior to the appointment. 5. If the patient did not confirm from the text, call the patient one day prior to the appointment. It’s important to have human-to-human contact because the patient’s tone of voice and reaction are key. Avoid sending several texts and emails in a short period in regards to one appointment.

The key is to be proactive, timely and consistent. These activities take a little time but truly have a big impact on patient retention.

For patients without an appointment that are due for recall, send out a recall card, but start the first paragraph with, “We are letting you know that it is time for your dental checkup.”

Hygiene’s Favorite R: Recall

Production’s Favorite R: Reactivation

Most practices may not be doing enough to keep patients coming back. One major problem may be the lack of a true, proven recall process. There are two rules to an effective recall system. First, every patient should leave the practice with a future recall date or a future appointment. Second, be consistent and predictable. When dental teams implement a recall system, it’s best to put it in writing so that it can be followed exactly, month after month. Only consistency brings predictable results.

Practices typically start thinking about reactivation when they are desperate for patients. A Reactivation Project should be done each January.

There are ways to structure an effective recall system:

Step 3: Mail a recall card to them - not a letter but a postcard. It should stand out in the mail. Put “Return Service Requested” on the card under the return address, so that any undeliverable cards are returned to the practice with the new address. If Mrs. Jones moved out of state, she will probably not need to be contacted again.

1. Find a recall card that is professional looking and represents the office well. 2. Utilize email and text. 3. For patients that pre-scheduled their appointment, send the card out on a designated day each week for patients scheduled three weeks away. Include the appointment date and time and let patients know that if they need to change the appointment, the practice

Here’s the process: Step 1: Assign a champion. This project is too valuable to the practice not to have someone accountable for its completion. Step 2: Print a list of patients that have not been in for six months. Go back at least three years.

Step 4: Include the following message: “Dear (name): We miss seeing you in our office. As you know, when dental decay, gingivitis, Continued on page 49

ODA annual meeting 2018 Michael DiTolla, DDS, FAGD Modern Crown & Bridge Techniques and Materials Six (6) CE Hours Saturday, April 21, 2018 9:00 AM – 12:00 PM & 2:00 PM – 5:00 PM Lecture Format for Dentists, Assistants and CAD Techs Advances in several new all-ceramic restorations has resulted in significant and consistent reductions in the number of PFM (porcelain-fused-to-metal) restorations being fabricated annually. CAD/CAM technology in the dental laboratory has helped make these newer restorations more consistent in terms of fit, contact and occlusion. Full-contour zirconia restorations are becoming more popular in the industry; proper cementation/bonding protocols for these restorations will be discussed in this course. Digital shading has improved to the point where not only dentists but also many labs quality-control their restorations with it. A prep technique will also be demonstrated that ensures adequate reduction and nearly perfect margins with relatively little effort.

Course objectives After completing this program, the dental professional should be able to:

1.

Recognize the shift from PFMs to monolithic (singlematerial) restorations that allow the clinician to use highstrength materials while satisfying patients’ esthetic desires.

2.

Understand the use of a prep technique that will impress your lab by improving the quality of your preps and impressions.

3.

Be familiar with full-contour zirconia and the pros and cons of using this structural ceramic material as a crown and bridge material, and be familiar with the cementation/bonding protocol.

4.

Be familiar with the new class of cementable all-ceramic restorations such as lithium disilicate, and understand how the CAD/CAM milling process can improve consistency and quality.

5.

Understand why the shade guides many dentists use today are inadequate, and what shade guide can cut your shade remakes in half.

Meeting Registration Opens in February!
