New companies are continually being added and less effective ones removed. Managing Marketing | Other Advertising
TRADITIONAL ADVERTISING While the focus on online marketing may have you thinking that print marketing is dead, rest assured that’s not the case. When done selectively, traditional marketing can be a very strategic and effective aspect of a comprehensive marketing campaign. Your approach will need to be more especially compelling since it’s competing for attention in a very crowded environment. Traditional marketing, especially in localized communities, can make you a neighbor, someone who’s part of the neighborhood and accessible and convenient to local patients. In fact, it’s even possible that the results of your patient survey revealed that your best patients initially came to your practice because of traditional marketing. Another plus to this approach is that it can help differentiate you in a crowded marketplace. More information is available in The Patient Survey section of this module; the article "Collecting and Using the Survey Data" will provide more useful information. Also consider reviewing the American Dental Association’s (ADA) Advertising Basics for Dentists and Dental Associations: A Guide to Federal and State Rules and Standards. This publication offers many tools to help dentists understand issues related to advertising and includes an overview of federal advertising rules, additional resource links, information on state advertising regulations and information on codes of ethics in dentistry. DIRECT MAIL Dental care is built on creating and nurturing a lasting and trusting doctor-patient relationship. Direct mail gets the attention of prospective patients who may only come across your website if they narrow their search by geographic location. It establishes you as part of the community, which can be an important factor for patients who want a dentist close to their home or office or who want the dentist to be someone they know. Many businesses like direct mail because they believe it hits the target every time. Like social media advertising, direct mail campaigns can help ensure that you reach prospective patients in a specific area. Direct mail campaigns also allow you to time when mailers drop. Direct mail is most effective when the printed piece, which can be a postcard or a brochure, has an eye-catching design and features an appealing message. Some practices opt to partner with an external marketing consultant or professional who knows which marketing strategies, offers and design are likely
to have the greatest impact. Oftentimes they can also provide data detailing how many new patients contacted the practice as a result of a particular mailer. PRINT ADVERTISING When used in the right way, print advertising can be very effective, especially if your market research and your patient surveys reveal that your preferred patients are a mature population or those who read local newspapers or specific magazines. The articles in The Patient Survey section of this module, "Why Survey Patients?", "Who and What to Survey", "Fielding the Survey", and "Collecting and Using the Survey Data", will recap why it’s important to survey patients and provide suggestions on how to do that. Unlike broadcast media and billboards, print ads can tell a compelling and inspiring story and their visuals can have a very strong impact. Print ads should highlight an offer that’s too good to miss and that connects with prospective patients. Whenever possible, use high-resolution photos of real patients rather than stock images in your ads. Make sure you have the patient’s written permission to use his/her image before developing the campaign. A sample Photography/Image Release Form is available in this module as well in the Case Presentations article in the Managing Patients module of the ADA’s Guidelines for Practice Success™. In addition, dental practices covered under the Health Insurance Portability and Accountability Act (HIPAA) must also have the patient’s written valid authorization before using full-face photos and comparable images that could be used to identify the patient, even if the patient is not named. More information on the U.S. Department of Health and Human Services’ (HHS) regulations on covered entities and business associates is available through the hyperlink below. The ADA offers many helpful resources, including the ADA Complete HIPAA Compliance Kit to help dentists understand and comply with the regulations. Finally, repetition matters: print ads need to run a minimum of 10 times just to be remembered. Make sure your ads feature the practice’s name, phone number and website in a way that’s both easy to see and read. NEWSLETTERS The digital age has caused many dentists to abandon newsletters so this traditional marketing approach can actually be an effective way to differentiate your practice from others. The purpose of a practice newsletter is to educate and inform, Continued on next page

not to promote, and it should always highlight the benefits of treatment rather than discussing specific equipment. For example, it’s fine to include an article that describes how your patient management and clinical systems respect patients’ time. Articles that focus on the latest piece of dental equipment installed in the practice will not be likely to attract patients into the practice. Newsletter content should inspire confidence by addressing and resolving patients’ obstacles and fears about getting dental treatment. You should look at doing a newsletter as a value-added proposition – a thank you – to a person for being your patient. Effective newsletters contain content that’s relevant, educational and beneficial to all patients. Send it to current patients only since they’re the right audience for its messages about the services you provide. TELEPHONE MARKETING While telephone marketing might not be considered traditional marketing, it’s more aligned with that approach than online marketing, even if it involves contacting patients via other electronic methods, such as email, or is mainly used with patients of record. In addition to being both relatively low-cost and highly effective, most patients respond favorably to phone marketing by keeping their regularly scheduled appointments. This approach can be managed by staff or outsourced to a third-party company. Of course, you should always make sure that you have the necessary permissions before calling, emailing or texting patients, and you must comply with laws such as the Telephone Sales Rule, Telephone Consumer Protection
Act, and CAN-SPAM law, as well as HIPAA restrictions on using patient information for marketing purposes. If you use a vendor to communicate with patients, you may need to have a HIPAA business associate agreement with the vendor before disclosing information such as patients’ names, addresses and phone numbers. You may not realize it, but you’re actually using phone marketing when you or someone on staff contacts patients who’ve had major procedures done to see how they’re doing. Consistently taking this extra step to contact patients reinforces how much you care about them and strengthens the doctor-patient relationship by letting them know they matter. That type of interaction can strengthen the doctorpatient relationship and boost word of mouth referrals as current patients tell their family and friends about what a great and caring dentist you are. Dental insurance is more like a prepaid gift card or phone card – your dental insurance has a maximum allowable per year and when it is spent, it is spent. So it is very important to familiarize yourself with all of the dental insurance options available to you in order to get the most “bang for your buck”. LOOK CLOSELY AT ALL OF YOUR OPTIONS! Sometimes, for just a few dollars more, you can get $500 or $1,000 more in benefits per year. Ask questions and make an informed decision!

ADA Advocacy News DOC Access Act Introduced
Ensuring Lasting Smiles Act
On July 15th, Rep. David Loebsack of Iowa and Rep. Earl “Buddy” Carter of Georgia introduced the Dental and Optometric (DOC) Access Act, H.R. 3762, in the House of Representatives. The act would prohibit certain dental and vision plans from setting fees for non-covered services. The bill would also prohibit these plans from imposing restrictions on medical plan participation and setting limits on a doctor's choice of lab. Currently, 39 states have passed laws limiting the ways that insurance companies can interfere in terms of fee-setting and the doctor-patient relationship. The DOC Access Act is narrowly drawn to apply only to certain employer-funded dental and vision plans regulated by the federal government. This legislation would not interfere with the states’ abilities to maintain and enforce their own insurance regulations and laws, as well as complement the work already done by most state legislatures across the country. The ADA believes that the DOC Access Act would bring needed balance to contract negotiations between providers, who are often small business owners, and large dental insurance companies. Several dentist members of Congress, including Rep. Mike Simpson of Idaho, Rep. Paul Gosar of Arizona, Rep. Drew Ferguson of Georgia, and Rep. Jeff Van Drew of New Jersey, also served as original cosponsors of this bill. The ADA encourages the passage of the DOC Access Act, as it is time to curb anti-patient and anti-competitive practices of dental insurance plans.
The Ensuring Lasting Smiles Act, (S. 560/H.R. 1379) or ELSA, would help patients with craniofacial anomalies. ELSA would require all private group and individual health plans to cover medically necessary services resulting from a congenital anomaly or birth defect. This legislation is crucial to ensure that children suffering from congenital anomalies and birth defects are able to receive the treatment they need.
ADA Contacts Federal Agencies About Business Practices & Patient Safety The American Dental Association sent a complaint letter to the Federal Trade Commission’s (FTC) Bureau of Consumer Protection raising concerns over aspects of SmileDirect Club, L.L.C.’s (SDC) marketing and direct-to-consumer sales of plastic teeth aligners. The letter was sent approximately two months after the ADA filed a citizen’s petition with the U.S. Food and Drug Administration (FDA) in April stating that SmileDirectClub is placing the public at risk by knowingly evading the FDA’s “by prescription only” restriction the agency has placed on teeth aligning materials. The ADA took these actions out of concern for consumer safety and customer recourse when negative outcomes from this direct-to- consumer dentistry product occur.

DR. RAY COHLMIA: THE TIP OF THE PYRAMID By Dr. Frank J. Miranda
Dynasty: a succession of people from the same family who play a prominent role in business, politics, or another field. In the realms of politics, stage/screen and old-money wealth, the word “dynasty” would likely conjure up the family names Kennedy, Barrymore and Vanderbilt. On a much smaller but no less significant scale, Oklahoma dentistry has its own dynastic family name – Cohlmia. With no less than 26 members of the extended clan having earned their places in the dental profession (plus another three currently in dental school), few can reasonably dispute that the Cohlmias have earned the title of “First Family of Oklahoma Dentistry.” And at the

tip of that expanding pyramid is its 90-yearold patriarch, Dr. Ray Cohlmia. We are proud to briefly spotlight this true giant of the dental profession. “It was all because of my mother,” Cohlmia reminisced about why he chose dentistry for a career. “She suggested I go back to college after my stint in the army during the Korean War. ‘What should I do?’ I asked her. She said, ‘You’d be a great doctor. No, actually being a dentist would be better for you.’ So, there it is! I’ve never wanted to do anything else but if I had to choose, I guess my second choice would have been medicine.
“I remember the dean of Southwestern University looking closely at my transcript when I was trying to go back to school. Let’s just say my school experiences before joining the army weren’t very good! Anyway, he looked at me and said, ‘Why don’t you take an aptitude test to find out what you’re good for.’ He actually said that! Looking back, though, that comment made me a stronger person. I made up my mind then and there that nothing would keep me from becoming a dentist.” Cohlmia attended Baylor College of Dentistry, graduating in 1959 and beginning a 57-year career practicing dentistry in NW Oklahoma City. He and

his younger brother Woody purchased and shared an office building on Drexel Avenue until Woody’s untimely passing in 1992. “Practicing with Woody is a wonderful memory. But my greatest days were when my sons, Raymond and Matthew, joined me in the practice. I’m also proud that I’ve been able to white-coat four of my grandchildren at the OU College of Dentistry. I have been blessed to have so many of my family be a part of the profession I love.” Space limitations prevent a complete review of Cohlmia’s contributions to the profession over his lifetime (nor, given his humility, would he want the attention). Nevertheless, a few are in order: past president of both the Oklahoma County Dental Society and the Oklahoma Dental Association, 1988 recipient of the Oklahoma Dentist of the Year Award, and the third-ever recipient (2004) of the James A. Saddoris Lifetime of Leadership Award. This last one was particularly gratifying since Cohlmia counts Jim, the first Oklahoman to serve as ADA President, as his main dental hero. Despite his many impressive credentials, when asked about his most rewarding philanthropic activity Cohlmia points to being in charge of the greeters for the St. Elijah Orthodox Christian Church’s annual bake sale for over 30 years. With the hindsight of experience, Cohlmia has one lesson he would have loved to give his earlier self. “I would have studied much more in school. Back then, my forte and main love was basketball. In fact, I wanted to be a basketball player when I grew up. I played at OU and at Wichita [State] University. I played semipro ball for Cessna. I even played against the Harlem Globetrotters in 1948, back when the exhibition games weren’t staged. They whipped us royally!

NO ONE ELSE CAN TAKE OUR PLACE By: Angela Craig, RDH Did you ever think you would make Rockstar status with people standing in line or on waiting lists just to see you? The skills, knowledge and time of dental professionals are gifts that people are willing to go to great lengths to access, especially those who cannot afford regular dental visits. Dental nonprofits like D-DENT, Good Shepherd and Catholic Charities (not to mention ten years of OkMOM) demonstrate the great value of volunteer dental hygienists, dentists and dental assistants. If you’ve ever felt burned out or a little bored with the daily routine of work, I encourage you to find a charity to volunteer with or to create a project of your own that assists individuals or groups with improving their oral health. When you see the relief and appreciation in the eyes of the patients you help it can restore your sense of mission: To help people reach better oral health. Teaching someone the art of toothbrushing seems trivial until the only thing one can do for their teeth is to brush them. The medical community has become more enlightened on oral health, but we know that only dental professionals can solve most of the problems pertaining to the mouth. And every member of the dental team can contribute. You could volunteer to see prescreened patients at your office or make time to help at one of the nonprofit

clinics. If all you can handle right now is to donate toothpaste and toothbrushes monthly to a public school pantry or a senior center, then do it. Even small steps are greatly appreciated. The Delta Dental of Oklahoma Oral Health Foundation has a list called “Resource for Dental Care - A Guide to Free and Low-Cost Dental Care” on their website with several clinics and nonprofits that could use volunteers. It is impressive to see dental and dental hygiene student applicants giving their time at various dental opportunities leading up to their application; it is even more impressive when you see those students in their first year of dental school still volunteering several times a year. Millennials have a reputation for being a more socially conscious group, which is good news because the need for volunteers with dental skills is great. At the other end of the career path is a licensure category called “retired volunteer,” which enables retiring professionals to keep their licenses active, solely to volunteer their valuable skills without pay. Details can be found on the Board of Dentistry website. The dental world has always appreciated the necessity of good oral health and its effects on overall health. As dental professionals we know the importance of controlling biofilm, not only for healthier teeth and the periodontium but also for better health including such things as high A1c levels and the complications of diabetes. The medical world has become more involved in having their patients seek oral care, but that care is not as readily accessed as a medical
doctor is. Until this disparity is solved, the dental community must fill the gaps. Thank you for being a part of the dental safety net for these individuals. No one else can take our place. Angela Craig has been active in the Oklahoma Dental Hygienists’ Association for over 40 years, holding numerous positions including ODHA President. She has also served as the dental hygiene member on the Oklahoma Board of Dentistry. Angela is a Community Oral Health Educator for D-DENT (Dentists for the Disabled and Elderly in Need of Treatment) and has been frequently recognized for her volunteer service activities, including her selection as OUCOD’s 2013 Dental Hygiene Alumna of the Year. The Difference Between Being Busy and Being Profitable: Contract Provisions
By: Craig Dreiling, Founder & CEO, Solutions-101 | Part 3 of a three-part series We have talked about leased networks, absorption of an existing patient base, and some hidden costs that are being thrown into the management of dental practices. Lastly, we need to explore some of the contract provisions to fully understand why they can reduce profitability. First, and most importantly, can you walk into your office or practice and pull a copy of every in-network contract you actively participate in? If you personally can’t, is there someone in your office that can? If not, you need to put this at the top of your to-do list. Understanding your limitations and exposure to an in-network contract is critical when trying to understand how and where you’re getting paid for a leased network. If you need to renegotiate, replace, or terminate a contract, terms governing this are more than likely already specified in the agreement. Other arrangements in such contracts are dealing more and more with patient communications. During negotiation and recredentialing, there are updated sections that allow insurance companies to “direct” and/or “influence” patients’ choice of provider. Such clauses say that you have no problem with these carriers telling patients which providers to choose for dental services. Solutions-101 has helped practices across the United States realize profitability in an ever-changing market environment. Their proven method has allowed them to expand to meet client needs, resulting in a dedicated team to ensure success. They have been referred to offices from multiple sources, allowing them to create networks that can continue to help offices once their services have been completed.
The first question you should ask is where would they be directing patients? I’d guess it would be to a lower-reimbursed provider. Since saving expenses generates more income, paying a lower-reimbursed provider for the same treatment retains premiums paid and benefits paid out. All of your hard work to negotiate a better reimbursement could very well be lost during the negotiation and recredentialing process. Another provision that gets embedded into agreements deals with companies making no “guarantees” or their willingness to “identify” participants or income. As a provider you are agreeing to take a reduced fee to see patients in network, but you will have no access to that patient and/or a company’s employer base. We’ve heard everything from “it’s proprietary” to “it’s a HIPAA violation to tell you which companies participate with ABC Insurance.” Either way, remember that it is up to you to attract and retain the patients. Also, you should know that you could be listed on some carriers’ websites. [Although not all carriers or their leased or rented networks will list your office.] We always recommend looking at the networks you contract with and see if and exactly where you are listed. How easy is it to find your practice? How likely is it that patients are finding you through that portal? Ask new patients where they are finding you -- you might be surprised by the answer. Once you review your contracts (since you have a copy of all of them in your office) make sure you understand what the process is to remove, renegotiate, or request any terms or reimbursement schedules. It is crucial to know what your waiting period is if you need to terminate a contract. It is also critical that you keep a record of that process and request. It is not uncommon for requests (even one as serious as trying to terminate a contract) to be submitted but not completed or executed.
Finally, have a general understanding of your state’s statutes governing these entities and relationships. It is important that you fully grasp the various provisions and amendments. Request any documents referenced in the contracts and who to contact with questions about who leased access is granted to. Ask all relevant questions and thoroughly review any contract before signing. Understanding your contracts, relevant state statutes, and your state’s Dental Practice Act will help in fully understanding what you can and cannot do as a provider. This knowledge will also guide carriers to limit or enhance their interactions with you as a provider accordingly. Be vigilant in knowing these documents or having someone who does. As networks expand, your office is vulnerable to those changes. Your complacency will be to the benefit of the marketplace rather than to your practice.

YEAR-END PLANNING – NOW IS THE TIME By Joe Hendrix, Senior Wealth Advisor for Mariner Wealth Advisors While it may seem a bit premature to be thinking about wrapping up 2019, it’s really not too early to make sure you’re on track to achieve your annual wealth planning goals. As we move into the fall and winter, consider the following tips to help end the year on a positive note. YEAR-END ASSESSMENT Begin your year-end assessment with an accountability review. What goals did you set for yourself at the beginning of the year? Were you successful in achieving them? If not, why? What steps will you take to accomplish any unmet goals before the end of the year? Should any of the goals change based on your evolving circumstances? Once you’ve conducted an honest evaluation of your status, it’s time to schedule meetings with your financial professionals to check in on your progress from their perspective. Set up meetings or calls with your wealth advisor, CPA, attorney, banker, and any other professionals who can provide insight. Ask them to help with accountability and a realistic measure of your financial plan. PRACTICE PLANNING If you are planning to buy or sell your practice, the end of the year is a great time to reevaluate your transition plan to help ensure it remains in line with your objectives. For those who are nearing retirement and anticipating some form of upcoming transition, it is paramount to start planning several years in advance. Steps like cleaning up the practice’s financials (collecting old accounts receivable, reducing personal expenses paid by the practice, upgrading equipment/technology, etc.) can help the practice’s valuation and the ease of the overall transition. Further, if you have not identified a successor to your practice, consider increasing awareness to market the opportunity for this transition. Also, the Tax Cuts and Jobs Act (TCJA) of 2017 included some potential tax benefits for business owners. There are certain provisions such as the qualified business income deduction that may eliminate the deduction for certain types of businesses. Since dental professionals are subject to 36 journal | Sept/Oct 2019
certain income limitations for purposes of calculating this deduction, it may be beneficial to time your equipment purchases/upgrades so as to lower your taxable income, which may in turn provide a better QBI (qualified business income) deduction. Before you act, be sure to consult with your tax advisor to verify how your individual tax situation may be impacted as this calculation is complicated and subject to multiple variables. DEBT MANAGEMENT Take a look at any outstanding loans to confirm that your payment plans still make sense given your current financial situation. Consider the interest rates of each loan and try to tackle the higher-rate loans first. By building a debt management ladder, you can view which loans have the highest rates and consider paying those down first. Second, be sure to consider the tax deductibility of certain loans; there are tax benefits to some forms of interest expense that can be useful in your overall tax management plan. SAVINGS Year’s end is also a great opportunity to review emergency funds, outside investments and your overall liquidity. Consider any changes undergone in your practice or personal life during the year, and how those changes may impact your savings amounts. If you are a younger practitioner, determine if your current cash flow allows you the flexibility to begin making more substantial contributions to your retirement account so as to maximize the potential benefits of compounding interest over the length of your career. If you haven’t established a practice retirement plan and your cash flow allows for additional savings, consider a SIMPLE IRA, a SEP (Simplified Employee Pension) IRA or a 401(k) for the practice. SIMPLE (Savings Incentive Match Plan for Employees) IRAs are great starter retirement plans because they typically require the least administrative efforts and lowest upfront costs. Finally, if your income levels allow for it, consider maximizing Roth IRA contributions as much as possible. Once your income reaches a certain level, these
contributions are phased out, so if it is a good fit for your situation, maximizing these as much as possible is beneficial due to the tax-free growth on these accounts. Another tax-deductible savings opportunity to consider is making contributions to a health savings account (HSA). If you have a high-deductible healthcare plan (an annual deductible of no less than $1,350 for single coverage and $2,700 for family coverage1), you may be eligible to make pre-tax contributions that can accrue over time. In addition, the earnings on these contributions are tax-free, making this a beneficial savings vehicle for many individuals with multiple different tax benefits so long as distributions are only made towards qualified medical expenses. Also, consider your progress toward college funding for children or grandchildren. A 529 plan is another tax-advantaged option to help save for educational expenses. Contributions to an Oklahoma plan can be tax-deductible, and the future growth on these accounts are tax-free at both the federal and state levels so long as distributions are used for qualified education expenses. In certain cases, distributions are also allowed for private school K-12 expenses. Consult with your tax advisor to apply these rules for your individual circumstances as limitations apply. INSURANCE As a business owner and medical professional, having adequate insurance coverage is vital to protecting your future. Begin your insurance review by assessing your property and casualty coverage to ensure that it remains adequate. Also, you may consider implementing an umbrella policy if you have not already done so. This type of policy provides an additional layer of protection to those who are at risk for being sued for damages to other people’s property or injuries caused to others in an accident. It goes beyond the limits of home or auto insurance. This coverage is especially important to individuals who have accumulated a lot of assets and are at a heightened risk for being sued.
If you have recently welcomed children into your home, you may consider increasing your life and disability coverage to ensure it remains adequate to support your growing family. On the flip side, if you are nearing retirement, evaluate any old life and disability insurance policies to determine if they are still current and/or necessary. ESTATE PLANNING It’s never too early to implement an estate plan, especially when you own a practice and/or have children. If you haven’t yet created an estate plan, consider developing a basic plan that fits your overall needs. You can always add to the plan as your life and business become more complex. If you have children under the age of 18 and no estate plan, this is perhaps the most important decision to make, as you can appoint a legal guardian for your children should an unforeseen accident occur. If you have an estate plan, the end of the year is a great time to review your existing documents to make sure they remain current and up to date. Specifically, evaluate your current appointments and distribution language in light of any life events that may have occurred. The birth of children or grandchildren, marriages, divorces and health changes are all events that may prompt you to make some revisions to your plan. If gifting to your children or grandchildren is part of your estate plan, you may wish to consider making those gifts during your lifetime to take advantage of the annual gift exclusion, which is $15,000 in 2019.2 If you have an irrevocable life insurance trust (ILIT), your named beneficiaries may have the right to withdraw their share of the gift through a “Crummey letter.” These are physical letters sent to the beneficiaries of an ILIT to inform them of the gift and their right to withdraw the funds. In essence, this changes a future gift into a present gift by allowing the beneficiaries the right to withdraw the funds for a certain period of time.3 To help ensure everyone in your family is on the same page regarding your estate planning wishes, year’s end is a great time to have a family meeting to discuss your plans. TAX PLANNING Some year-end tax planning items to consider include: • Roth IRA conversions – Today’s federal tax rates may well be the lowest many of us will see in our lifetimes. And, while TCJA (Tax Cuts & Jobs Act) rate cuts are scheduled to last through 2025, it’s possible they may end sooner based on political events. While a Roth conversion will trigger a taxable distribution from your traditional IRA, it may make sense in the long term; Roth IRAs have the benefits of tax-free growth/ distributions and are exempt from required minimum distributions at retirement.4 This can be a complicated decision so you should seek counsel from your wealth and tax advisor to ensure that all variables are carefully considered. • Charitable giving – Using donoradvised funds, bunching multiple years of contributions into a single tax year to maximize their impact, and donating appreciated stock are all charitable giving strategies that can lower your tax bill when implemented prior to year’s end. If you have children at mature ages, including them in charitable gifting decisions can be a great first step in building financial discipline and learning about the responsibility of wealth management and the positive impact wealth has on our communities. Again, your tax advisor should be consulted to help determine if a giving strategy is right for you. • Tax loss harvesting – It’s also important to review your potential capital gains for the year to determine if there are any tax losses that can be harvested to offset your taxes by year’s end. Managing your capital gains tax is an important tax planning measure that is often overlooked. If you complete any tax loss harvesting sales prior to year’s end, be sure to discuss with your tax advisor to help avoid a wash sale that could disallow the recognized loss. In general, a wash sale occurs if you purchase or re-purchase a security sold at a loss within 30 days of the date of sale. • Business planning – There are several business-related events that also have the potential to lower your taxes. Equipment and supply purchases, compensation adjustments to key staff and technology updates can all count as tax-deductible improvements to your practice.

But a closer reading of the reasons for creating the ODF may lead the Foundation to take a different route to assisting with “access-to-care” issues in the state. “We have,” said John Wilguess, ODF Executive Director, “been focused on going into underserved communities, working with non-profit site partners who understand the greatest unmet needs in their areas, and providing as much dental care as we can in the limited time we have. That usually means treating somewhere between 10 -12 patients each day. While the numbers have been good, we consistently ask ourselves if we’re accomplishing what we set out to do. How do we know? What are the measures of ‘successful’ charitable outreach? That is a much more difficult outcome to determine.” In 2018, ODF’s MobileSmiles Program provided treatment to 2,388 patients over a period of 220 days with a total treatment value of $1,086,752. According to Wilguess these numbers reflect a significant amount of good being done in the name of dentistry, but it also comes at a price. “The MobileSmiles program has been successful in treating thousands of patients every year. Yet, we remain troubled by the fact that the program only reaches those patients who are actually treated. That leaves thousands more who are not being given a hand in improving their oral health even though MobileSmiles staff and resources are within easy reach. In many cases, we are literally on the other side of a door for someone to get help.” Wilguess notes that in most situations of bringing treatment to access-to-care-challenged areas “it’s as if ODF staff is trying to put out an inferno with a water balloon.” While Mobilesmiles is accomplishing the goals and the mission of the ODF, it’s only helping a few and not changing the dominant culture in any community so that dental disease is seen as preventable. “When the ODF was founded in 1959, its primary role was seen as providing education. We are now exploring what the

work of the ODF might look like if we ‘flipped’ MobileSmiles to replace treatment with education in proper oral health care as a first step. At the conclusion of this education outreach we would identify those ‘students’ needing dental care and provide follow-up treatment.” The purpose of MobileSmiles has been to address Oklahoma's dismal health outcomes and lack of oral health opportunities, a direct result of overwhelming "access-to-care" challenges. This proposed hands-on education program, conducted by ODA dentists and the ODF dental team (dentist, dental assistants, dental students, community partner) is designed to provide students and their families an easy understanding of proper dental care, what care means to their overall health, tools to care for their dental needs, and an introduction to dentists as caring health professionals. Wilguess encourages the input of ODA members to assist in working through curriculum development, educational design and identification of those locations that can be candidates for this type of project to improve oral health outcomes in Oklahoma.
