Ophthalmology Services For My Enlightening 30-Year Journey as an Ophthalmologist
W
BY ALAN D. MENDELSOHN, MD, FACS
hen I completed my fellowship training at Bascom Palmer Eye Institute in 1987 and began practicing ophthalmology in Hollywood July 1987, I was part of a new wave of ophthalmologists who brought the exciting technique of out-patient cataract surgery to Broward County. Our predecessors would admit patients to the hospital the night prior to surgery and keep them hospitalized for an additional night following surgery. Avoiding the hassles and expenses of hospitalization was attractive, but even more advantageous was avoiding the risks of hospital –induced infections. Out-patient cataract surgery became an almost instant success.
Outpatient Cateract Surgery
For the next decade, those of us on the “cutting edge” of cataract surgery in Hollywood were doing cataract surgery by a procedure called Extracapsular Cataract Extraction whereby we removed the nucleus of the cataract in its entirety. This necessitated that we make a large incision in the eye. The cataract was removed, followed by insertion of an intraocular lens (IOL). At the completion of surgery, five to seven sutures were required to close and secure the surgical incision. The eye was pressure patched. For the next three weeks, we encouraged our post cataract surgery patients with the following refrain: “Do not bend, do not lift, do not exercise, do not exert.” Finally, after three weeks our patients could become “free agents” again and resumed their normal lives. We began suture removal in the office, and by the sixth post-operative week the patient received a final new eyeglass prescription. The IOLs utilized in that era provided fairly good distance vision, but eyeglasses were needed to see near and intermediate distances, with fine tuning of distance vision.
“No Stich” Cateract Surgery
While moving cataract surgery to an out-patient basis was considered revolutionary at the time, an even bigger milestone transpired with the transformation of cataract surgery in the mid to late 1990’s to a highly improved technology called Phacoemulcification (phaco). Instead of removing the cataract nucleus in its entirety, with a hightech ultrasound we could break up the nucleus into tiny fragments and suck them out. This paved the way to tiny incisions in the eye and the advent of “no stitch” cataract surgery. Phaco brought quicker, safer, and better visual outcomes to the cataract surgery arena. With great pleasure, on the very first post-operative day, I was now able to encouragingly tell my patients, “You can now bend, lift, exercise and resume 100% of your activities!” Even more importantly, post-operative edema, infections, and inflammations plummeted with the new technology. Cataract surgeries were now routinely scheduled two weeks apart instead of two months apart. Concurrently, there have been remarkable enhancements in the instruments that we utilize pre-operatively in the office to ascertain the optimal custom fit intraocular lens for any given patient. The quality of these high tech ophthalmic devices and ultrasounds utilized pre-operatively for Iens determinations, as well as the expertise of the staff actually making the measurements, are of paramount importance for excellent visual outcomes.
30th Anniversary
From the onset of my career in 1987, through to the present time, we have been blessed with newer generations of intraocular lenses (IOLs) which provide superior vision. Another career milestone occurred within the past ten years with the arrival of the multifocal ReStor IOL. With the ReStor IOL, we can generally correct most patients’ cataracts
and enable them to see near, far, and everywhere in between without the aid of glasses or contact lenses. The ability to read, drive, work on the computer, and see without glasses is an incredible freedom that was never possible with traditional monofocal lenses. My journey in the world of cataract surgery in Hollywood has now hit three decades as I had my 30-year anniversary of practicing ophthalmology on July 1st! Perhaps the most incredible landmark and noteworthy step on this journey has been the landmark transformation in cataract surgery to the new era of laser-assisted cataract surgery that has commenced in earnest in the past five years. Femtosecond laser cataract surgery is highly reproducible, far more accurate, and far safer than older techniques of cataract surgery with a scalpel. The laser offers excellent precision thereby producing the absolute best visual outcomes in current cataract surgery. Furthermore, patients with low or moderate levels of astigmatism can have this astigmatism eliminated or greatly reduced at the same time as the cataract procedure, resulting in a markedly enhanced quality of vision and overall outcome. In essence, this is customized laser cataract surgery utilizing high-tech 3D imaging and tailoring the ultra-precise treatment to the unique size and shape of your eye.
My Own Cataract Surgery
On a personal note, even though I am a cataract surgeon, naturally I am not exempt from developing cataracts myself. In fact, in 2014, at age 56, I began to notice the litany of complaints that I have heard from my patients. First, I noticed that while driving at night, I was not as comfortable or confident as I was driving during the day. Second, oncoming headlights became bothersome, and eventually so did the rising or setting sunlight in my face
Dr. Alan D. Mendelsohn
while driving. Third, I had always been an avid reader, able to read small print journals or novels for hours at a time prior to the inception of my cataracts. But in 2013 and 2014, I noted that reading tiny print became more of a chore and challenge, curtailing my reading duration. Finally, and most importantly by far, I needed to be positive that my vision was excellent to examine my patients and perform surgical procedures. I decided that it was time to proceed. In July 2014, I was the personal beneficiary of the wonderful transformations of modern ophthalmology that I described. I had advanced laser cataract surgery with the insertion of an ReStor lens in my right eye, with the same procedures to my left eye the following week. My vision is now phenomenal! I wore eyeglasses starting at six years of age and then began with contact lenses around age 13. Until age 56, I needed glasses or contact lenses for everything! Now, for the past three years, I wear no glasses, no contact lenses, nothing. Of course, I practice what I preach and always wear sunglasses with UV400 blocker and polarization outdoors during daylight hours for ocular protection. My quality of vision and depth perception are the absolute best of my entire adult life. I am delighted! Now at age 59 and feeling like the bionic man, I look forward to my next decade serving the community as an ophthalmologist on this thoroughly enlightening journey!

Celebrating Hispanic Heritage Month
E
xplorer Ponce de Leon discovered Florida in 1513, claiming the land for Spain. Five hundred years later, settlers from Spanish-speaking countries continue to call Florida home. In fact, almost 30% of the population in Broward County and nearly 70% in Miami Dade is Hispanic. That is why National Hispanic Heritage Month is especially meaningful here in South Florida. Observed from September 15th to October 15th, National Hispanic Heritage Month celebrates the histories, cultures and contributions of American citizens whose ancestors came from Spain, Mexico, the Caribbean and Central and South America. Throughout the month, there will be numerous events and celebrations taking place, such as the Hispanic Heritage Festival at Miramar Regional Park on September 16th, a Hispanic Heritage Concert at the NSU Miniaci Performing Arts Center on October 8th, or the Hispanic Heritage Month Festival on October 14th at the Pembroke Pines Branch Library featuring food, music, crafts, and storytelling. Experience all that the region has to offer by taking advantage of the calendar section in Our City magazine. Each month we include a curated list of diverse community activities and events, which appeal to a wide range of people and interests.

he ancient India physical, mental, and spiritual practice of yoga is one of the most popular across the country right now, and the City of Aventura has proudly been offering complimentary classes in one of their most pristine parks. There’s no better place to do a sun salutation and a tree pose than in Aventura’s Founders Park. Since late last year, “Yoga in the Park” started in the northwest area of the park under the trees, and is now in the northeast corner where a neighboring condo provides additional shade. “Yoga at Founders Park is a great way to create community through people, parks and programs,” says Kimberly S. Merchant, Community Services Director of City of Aventura. Practice is open to everyone of all levels and abilities. However, residents are encouraged to obtain a park ID prior to attending class to expedite access to the park. Non-residents will be required to pay the park entrance fee ($5 per adult and $3 per child under the age of 18, if they are accompanied by an adult) upon arrival to Founders Park. Some Lucky Dogs SECTION HEAD COMMUNITY NEWS
BY BEVERLY PERKINS
T
he Humane Society of Greater Miami hosted an Adopt-O-Thon this summer for the homeless animals at the Soffer and Fine Adoption Center. The successful event was sponsored by Miami Model Citizens and Phenom Foundation. Both philanthropic organizations have been known to have a soft spot for homeless animals. Additionally, NFL superstar Nick Moody, came out to promote his “starting line-up of big dogs”. The linebacker is the new spokesperson for the “Big Dogs Snuggle More” campaign. The big dog himself autographed bandanas for all the lucky dogs that were adopted that day. The morning began with plenty of pet pampering before hundreds of people came to see the fashionable pooches and possible furry companions in the Adopt-O-Thon. The volunteers and sponsors walked alongside the pups as they strutted their stuff, and showed off their true and beautiful personalities in hopes of finding their forever families. Attendees enjoyed tasty treats and activities during the afternoon as they visited the shelter meeting the animals, and searching for their purr-fect match. Over 20 dogs and cats were adopted during the Adopt-O-Thon proving that everyone worked together with a common goal to find their four-legged friends
forever homes. Adoption fees were reduced in honor of the event; however, it was still important for adopters to have a regular background check and screening. Phenom Foundation and Miami Model Citizens made a very generous donation to help the dogs and cats at the Humane Society of Greater Miami. This donation will go towards sponsoring many of the long-time residents at the Humane Society’s Soffer and Fine Adoption Center, as well as the adoption of one of the dogs rescued by the organization’s Second Chance Fund, Regina. “It was a truly magical event,” said Laurie Hoffman, Executive Director at the Humane Society of Greater Miami. “We are very grateful to have the members of these two amazing organizations show their love for animals by volunteering hands-on with our shelter’s pets.” The Humane Society of Greater Miami, Soffer and Fine Adoption Center is located at 16101 West Dixie Highway, North Miami Beach, FL 33160. Bank of America Awards Miami-Dade Teen with Summer Internship BY BEVERLY PERKINS
CAN DO KID
N
orth Miami resident and Miami Edison Senior High School graduate Amanita Pierre was selected as a Miami-Dade Bank of America Student Leader. The Bank of America Charitable Foundation selects only five exceptional high school juniors and seniors for summer internship. The choices are based on the students’ commitment to serving the community and making Broward a better place to live along with their outstanding academic achievements. “Since I was very young I’ve always wanted to give back to my community but I never really had the opportunity to do so back home in Haiti,” said Amanita. “My main inspiration for giving back comes from all the hardships my family and I went through. I’ve experienced everything from domestic violence to one of the biggest natural disasters back in Haiti. But the one experience that has impacted me the most was having to go through a domestic violence situation at home. That experience has driven me to get involved with organizations that advocate against domestic violence.” The coveted position for the Bank of America Student Leaders® Program was one at the Boys and Girls Club of Miami-Dade County. The eight-week paid internship opportunity gave junior and senior level students the opportunity to develop and apply leadership skills through hands-on community work experience at local nonprofit organizations as part of the Bank of America’s Student Leaders® program. At the Boys and Girls Club, they shadowed executive staff to learn how nonprofits work, served as counselors to elementary school students in first through fifth grades, and worked
on a special project for the club that focused on alumni. In addition to the internship, these select individuals from Broward County had the opportunity to travel to Washington D.C. to join more than 200 other Bank of America Student Leaders from around the country for a week-long Leadership Summit. The students met with members of Congress, learned about diversity and nonprofit work, and built and applied leadership skills through hands-on community work experience. “D.C. Summit was a surreal experience, I was able to meet incredible people of different backgrounds who have overcome hardships throughout their lives much like me,” said Amanita Pierre. “By far, meeting other fellow Student Leaders, getting to know them and their stories was my favorite part of the summit.” “Through Student Leaders®, we’re building youth pathways to success and giving voice to the next generation, who will help develop a more diverse and inclusive society,” said Gene Schaefer, Miami market president, Bank of America. “By engaging young people in meaningful and relevant experiences such as building a resume and managing a paycheck, we’re advancing economic mobility in Miami as we work to create a thriving community.” Since 2004, the program has supported 2,200 young people nationwide. s part of the Kids and The Power of Work (KAPOW) program, one hundred 2nd graders from Fulford Elementary in North Miami Beach (NMB) were able to take a special field trip at the end of the 2016-2017 school year to NMB City Hall and Police Department. The special field trip was presented by Learning for Success, Inc. and The City of North Miami Beach in conjunction with Miami-Dade County Schools. KAPOW, now in its 25th year, serves 6,000 students annually. The unique collaboration between the school district, 100 area businesses and community organizations, volunteers, and 64 South Florida schools introduces and exposes students to career awareness through professionally designed lessons taught by volunteers in the classroom and on work sites. John Casbarro, President of Learning for Success, Inc. which manages KAPOW in South Florida states, “Young people need to understand that there is a reason for studying beyond doing so for the sake of learning. The KAPOW program addresses this need by helping young people understand early in life that reading, writing, and math are vital to success in school, work, and life.” Volunteers are assigned to one class at local elementary and middle schools,

THE DONOWAY CENTER NEW HOPE FOR ADVANCED PANCREATIC AND LIVER CANCER PATIENTS Patient’s once diagnosed with locally advanced Pancreatic and Liver Cancers were given little hope and often offered limited treatment options all with certain short life expectancies despite the treatment they were receiving. However, a new procedure called Irreversible Electroporation, also known as IRE or NANO-KNIFE for short, has changed all that. Patients are seeing a doubling and tripling of their life expectancies based on recently published reports. Though currently used in patients with known locally advanced disease it has now found its place in the operating room and used when patients are “unexpectedly” found to have more advanced disease than pre-operative imaging shows.
Robert B. Donoway, M.D., F.A.C.S., F.S.S.O. Diplomate American Board of Surgery Diplomate, Society of Surgical Oncology Former Medical Director Memorial Cancer Center Medical Director of Institute for Breast Cancer Prevention
Dr. Robert Donoway, a surgical oncologist trained at SloanKettering, is the only surgeon performing these procedures. During the procedure, in the operating room, after it has been determined that the patient’s tumor is non-resectable, fine needle probes are placed through the tumor using ultrasound guidance to ensure there is no injury to critical structures such as the Bile Ducts and Portal Vein. High voltage current up to 3000 volts is delivered through the probes in 90 microsecond pulses synchronized with the patient’s heart rhythm to avoid any cardiac problems. The procedure lasts between 1 & 2 hours using imaging to follow the ablative process. After the procedure is completed the probes are removed and the patient is taken to ICU step down for a 24-hour observation period. Most patients are discharged in 2 or 3 days following surgery and because there is no major surgery to recover from they are able to start postoperative neo-adjuvant type chemotherapy and radiation sooner. After 3 to 6 months of combination treatment patients are re-evaluated and restaged. A number have been converted to Resection candidates, which is still deemed the standard of care. Preliminary results have been very encouraging and have more than doubled many patients' life expectancy. Clinical trials are ongoing to use new novel agents including monoclonal antibodies and vaccines as the next line of treatment in this highly selected & heavily pre-treated group. Dr. Donoway is the only surgeon performing this procedure in Florida. For more information about Dr. Donoway visit www.AtlasOncology.com

Sunny Isles Beach Mayor George “Bud” Scholl wished he could have lunch with his grandmother, who died when he was 18. “She had a profound influence on me,” he said. “It would be very meaningful for her to see how we’ve developed over the years. Family is near and dear to my heart.” Summing up the “Meet the Mayors” theme, Gibbons noted, “The nice thing about this panel is something I think is really important for our constituents - not just to know us by what we do, but to know us by who we are.” For more information, contact the Aventura Marketing Council / World-Class Chamber of Commerce at 305-932-5334 or visit aventuramarketingcouncil.com.

Traditional Bullying: Safety and Prevention BY NORTH MIAMI BEACH CHIEF OF POLICE WILLIAM “BILL” HERNANDEZ, MSM
T
raditional bullying is defined as unwanted, aggressive behavior among school-aged children that involves a real and perceived difference in power. Bullying is when one child picks on another child again and again. Evidence gathered over decades supports that (4) out of five (5) students admitted to some form of bullying behavior at least once a month, ranging from name-calling to verbal threats to physical violence. Both boys and girls can be bullies where they target children who are either weaker or smaller, are shy and generally feel helpless. Bullying is different from fighting or teasing; a bully has power over another child, bullies try to control other children by scaring them, being picked on over and over can make the child a victim and bullying happens when other children are watching. There are three (3) types of bullying: physical (i.e., hitting, kicking, pushing, choking, and punching); verbal (i.e., threatening, taunting, teasing, hate speech); social (i.e., excluding victims from activities or starting rumors about them). In general, bullying is not OK! Even if you do not think your child is bullied, a bully, or is acting as a bystander, you will be helping to protect your child just by asking these questions: ▸▸ “How are things going at school?” ▸▸ “What do you think of the other kids in your class?” ▸▸ “Does anyone get picked on or bullied?” When your child is bullied: ▸▸ Alert school officials to the problems and work with them on solutions. ▸▸ Teach your child when and how to ask a trusted adult for help. ▸▸ Recognize the serious nature of bullying and acknowledge your child’s feeling about being bullied. ▸▸ Help your child learn how to respond by teaching your child how to: ▸▸ Look the bully in the eye. ▸▸ Stand tall and stay calm in a difficult situation. ▸▸ Walk away. ▸▸ Encourage your child to make friends with other children. ▸▸ Support activities that interest your child... When your child is the bully: ▸▸ Be sure your child knows bullying is never OK. ▸▸ Set firm and consistent limits on your child’s aggressive behavior. ▸▸ Use effective, non-physical discipline, such as loss of privileges. When your child is a bystander: ▸▸ Encourage your child to tell a trusted adult about the bullying. Encourage your child to join with others in telling bullies to stop. ▸▸ Help your child support other children who may be bullied. Encourage your child to include these children in activities. In general, even if your child is bullied, a bully or a bystander monitor social media or texting interactions so you can identify problems before they get out of hand. ASK THE EXPERT
T
he vertebral column (backbone) is made up of 33 vertebrae. These vertebrae are then grouped into divisions called the cervical (neck), thoracic (upper back), and lumbar (lower back). A fibrous disc (intervertebral disc) connects each pair of the vertebrae. Healthy spines have discs that cushion the vertebrae allowing for normal flexibility and movement of the spine. Degenerative Disc Disease (DDD) is the weakening of one or more of these intervertebral discs. Unfortunately, the condition develops as a natural part of the aging process. These discs between the bones of the spine are made up of cartilage, fibrous tissue, and water. With age, the discs weaken, flatten, bulge, or break down. Sometimes there may be a genetic component in some individuals who suffer from DDD, but the true cause can be a combination of things, but usually it’s simple “wear and tear” or the result of an injury. Degenerative Disc Disease in the lower back, or lumbar spine, occurs when a disc is compromised and causes pain. The disc itself does not have a blood supply, so if an injury occurs it is unable to repair itself the way other tissues in the body can. DDD is so common that an estimated 30% of individuals ages 30-50 years old have some degree of disc space degeneration. Many may not ever even experience pain or have it diagnosed. After a person reaches the age of 60, some level of disc degeneration is considered normal and nothing to be alarmed about. ▸▸ Symptoms. Symptoms vary with each individual depending on the location and the type of disc degeneration. However, a primary symptom of the disease includes sharp and/or chronic pain in the back and neck. Other signs are weakness or numbness. As a disc undergoes change, the body may react and develop bone spurs as well. These bony growths may take up space, therefore compressing the nerves, and causing weakness in the arms and/or legs. ▸▸ Tests. If a patient presents any of the above symptoms, their doctor may order X-rays, an MRI (Magnetic Resonance Imaging), or a CT (Computed Tomography) scan. ▸▸ Treatment. The severity of the condition will determine the best approach for treatment before considering surgery. Nonoperative measures may include: heat and/or ice therapy, activity modification, medications, physical therapy to strengthen the muscles, and epidural injections. These measures are often effective in providing pain relief. Surgery, such as a discectomy, may be required for those patients not responding to any of the nonoperative measures, whose disc changes have resulted in spinal cord compression, who have a structural abnormality, or those who have such severe chronic pain. The procedure removes the disc and any associated bone spurs which are compressing the spinal cord and/or surrounding nerves. The removal makes room for the spinal cord and nerves to move freely again, and the pressure is relieved.
