ASK THE EXPERT

Flat Feet In Children, Teens And Adults BY ROBERT H. SHEINBERG, D.P.M., D.A.B.P.S., F.A.C.F.A.S.

F

lat feet are caused by loose ligaments in the arch as well as trauma or weakening of tendons (Posterior Tibial) and ligaments (Spring) in the foot. Flatfooted children usually become flatfooted adults. Tight calf muscles can severely aggravate the condition as can tight hip and ITB muscles. Rheumatoid or Psoriatic arthritis can also cause the foot to flatten. Flat feet can cause you to have pain in the heel, arch, ankles, legs, knees and back. This is aggravated by standing and walking. Cramping in the legs, especially at night, premature fatigue during any type of activity, or a general lack of interest in participating in sports or even walking activities generally follow. Signs of flat feet include flattening of the arch when standing and the foot pointing excessively to the outside during gait. Flat feet are often associated with knock-knees and abnormal shoe wear. Bulging of the bones on the inside of the ankle and foot are usually seen and there could be swelling on the inside of the foot and arch as the problem progresses. Problems related to flat feet include hammertoes, heel spurs and bunions. Lower back and knee problems can also be seen. Also, inactivity causing weight gain and poor cardiovascular fitness may result, as well as arthritis in the foot, ankle, knees, hips and back. Adults that are flatfooted generally are in poor physical shape and will almost always worsen over time. Their lack of activity because of pain or fatigue is the problem.

It is important to treat flatfooted people early, especially those with tight muscles whose feet also point out (Duck footed). Treatment includes proper shoe gear and custom molded orthotics to support the foot and leg. Also, activity modification and weight reduction are critical. Physical therapy will improve flexibility, strength, balance and coordination. Lastly, surgery to correct the deformity and return the child, teen or adult to full or near function may be needed. This should NOT be a last resort in many cases. Prognosis is excellent in most cases if caught early. If seen late, the deformity can still be corrected allowing a return to activities of daily living without pain. Many flat feet are destructive over time which will have an adverse effect on one’s life and livelihood. n Dr. Robert Sheinberg is a Board-Certified Foot, Ankle and Leg Specialist practicing in Weston and Pembroke Pines since 1988. It will be held oon March 21, from 6:30pm -9pm, at Studio 18 in the Pines. This is the first time the poetry workshop is being offered. The event begins with guests sipping on wine and enjoying light refreshments, as they view the artwork in the gallery. Next, Shannon Adams will recite poetry and then give a question and answer

PPTOPA Presents “Addams Family” on Stage BY DEBBY TEICH

I

f you have always dreamed of seeing a Broadway show, the Pembroke Pines Theatre of the Performing Arts (PPTOPA) can give you a taste of what it is like. It offers top-notch community theatre at an affordable price. From March 15 to April 7, “Addams Family” will be on stage for several performances. Show times are Friday and Saturday nights at 8pm, as well as Saturday and Sunday afternoons at 2pm, at the Susan B. Katz Theatre at the River of Grass Arts Park. “This macabre comedy, featuring catchy songs and a host of one-liners, is fun for the whole family,” said Wendy Marrero, vice president. Based on the popular television show, in the eccentric, kooky, upside down world of the Addams family, sad is happy and pain is joy. While celebrating their yearly ritual of summoning their ancestors from the dead, Wednesday Addams has grown up and fallen in love with a sweet, smart young man from a respectable family. Everything will change for the family when they host a dinner for her boyfriend and his parents. The production team includes Michael Biggs (Director), Anthony Campisi (Musical Director) and Del Marrero (Choreographer). Principle roles are played by Steven Fuentes (Gomez), Myria Jean

Baum (Morticia) and Daniela Dugan/Jessie Hoffman (Wednesday). PPTOPA is the oldest continuously operating community theater in Broward County. It was founded in 1993 by a group of dedicated individuals who wanted to provide low cost, high quality live theater for the community, as well as opportunities for anyone interested in assisting in the creation of theater. While technically an “amateur” company, PPTOPA productions routinely feature some of the finest actors and actresses in South Florida. They are teachers, attorneys, doctors, business owners and others who volunteer their time to immerse themselves in something they love. Recent casts have included performers who have headlined productions at Stage Door, Curtain Call, Tamarac, and Actors Theaters. They have been on national Broadway tours, starred in Hollywood films, and written and produced their own plays. Many have been nominated and selected as recipients of South Florida’s Carbonell awards. PPTOPA presents four shows annually. The nonprofit organization is known for showcasing exceptional regional talent and its professional sets and cosPerformances are held at the Susan Katz Theatre, 17195 tumes. The theatre company raises money through Sheridan Street, in Pembroke Pines. For more information, call donations and ticket sales.

Tee Off with the Mayor at the 18th Annual PPCS Golf Tournament BY DEBBY TEICH

O

n March 22, Pembroke Pines Mayor Frank C. Ortis and Waste Pro will present the 18th Annual Pembroke Pines Charter School Golf Tournament at Pembroke Lakes Country Club, located at 10500 Taft Street. It features contests, lunch, dinner, prizes, awards and more. Held from 10:30am to 6pm, the popular event usually sells out quickly. All funds raised will benefit the Pembroke Pines Charter School Foundation and will be used to enhance each student’s educational experience. Last year’s tournament raised $80,000. The Pembroke Pines Charter School System was created and built by the City of Pembroke Pines. It is the largest municipally run charter school system in the country and has been recognized for its innovation and academic excellence. It is home to four elementary schools, three middle schools and one high school housed on five educational campuses throughout the City. Since Mayor Ortis established the tournament, it has raised over $1 million dollars for the school system. Registration for the golf tournament begins at 10:30am at the Pembroke Lakes Country Club. Following registration, golfers can participate in a putting contest and enjoy some lunch. The shot-gun tee off is at 12:30pm. The driving range and practice tee will also be open. This year organizers are adding a ball launcher, allowing players to shoot a ball

approximately 300 yards. The fun continues at 5:30pm with a cocktail reception that includes open bar, followed by a deluxe buffet, prize presentations and a live auction. To raise additional funds, PPCSS recently held the Pines “Light UP the Night” 5K, which began and ended at the City of Pembroke Pines Charles F. Dodge City Center. This year’s theme was “Peace, Love & Sole.” Each participant got “lit up” as they completed the course. They were given a RUN/LED bracelet, which changed colors and light up at each mile marker. As runners finished the race, their bracelets showed a variety colors when they passed under lighted arches and disco balls on the plaza of the City Center. After the run, there were plenty of festivities, including a DJ, free food samples from local restaurants, raffles and much more. Last month, area residents attended the ten-year celebration of the City of Pembroke Pines’ premier tasting event - Taste of Pines. Food lovers enjoyed selections of wine and sampled cuisine and desserts from more than 30 local restaurants and vendors. Tickets also included a concert and live musical entertainment. All of the proceeds benefited the Pembroke Pines Charter School Foundation. n Pembroke Lakes Country Club is located at 10500 Taft Street. For information on sponsorship levels and golf registration, send an email to trichards@ ppines.com or call 954-435-6759. Do Your Eyes Make You Look Tired?

T E S CE ! S T VI N FA ER OW S T IN

ASK THE EXPERT

SHOE & LUGGAGE REPAIRS

BY CHARLES A. MESSA, III, M.D., F.A.C.S. he eyes are the first thing we notice when we meet someone. Beautiful eyes can make up for other features that are less than ideal. Likewise, eyes that are tired, droopy, or puffy may not reflect a vibrant inner self. It is often said that the earliest signs of aging are around the eyes. Perhaps people are always questioning whether or not you had a good night’s sleep, because your eyes are swollen with bags. Fortunately, at the Weston Cosmetic Surgery Center, we can give you dramatically younger looking eyes through “eyelid lifts,” also known as blepharoplasty. A primary reason for droopy eyes is simply due to age. It can also be hereditary. As everything droops, so do the eyebrows. As the outer part of the eyebrows droop, the skin of the upper eyelids will look saggy, as if you had developed extra skin that was not there several years before. Removing this “extra” skin can drastically improve that droopy look. Another reason for “tired eyes” is because of droopy or migrating eyelid fat. This fat is held back by a thin wall of fibrous tissue called the orbital septum, which weakens over time. The fat, held firmly back by this wall in youth can bulge with age, giving you “baggy” upper and lower eyelids. Blepharoplasty removes this extra fat and tightens the wall, restoring a youthful contour. For those patients with puffiness under the eyes or baggy upper and lower eyelids, laser eyelid surgery can reduce and often eliminate these problems. The end results are a more alert and youthful look around the eyes. The prominent fat pockets can be removed from a hidden incision along the inside surface of the eyelid. This “no stitch”, no external scar technique is utilized when patients have fat prominence of “eyelid bags” alone without excess or sagging eyelid skin. If minimal excess skin is present, a skin pinch technique is combined with transconjunctival fat removal. In this case, a “pinch” of lower eyelid skin is removed. These techniques typically result in less swelling, less discomfort, and shorter recovery time. I will assess which technique is ideal for your eyes during your personal consultation. Recovery from eyelid surgery usually takes seven to ten days. Sunglasses allow you to go out in public immediately after surgery, and make-up can be worn within about one week. Normal activity can be resumed within a few days, and exercise can be resumed within 14 days after surgery. If the eyes looking back at you in the mirror are not reflecting how you are feeling, it might be time to consider a blepharoplasty. Dr. Messa is board certified by both the American Board of Plastic Surgery and the American Board of Surgery, an active member of the American Society of Aesthetic Plastic Surgery, American Society of Plastic Surgeons, and a Fellow of the American College of Surgeons. He is a recognized expert in cosmetic plastic surgery and has achieved Allergan’s Black Diamond Award as one of the top 1% of plastic surgery practices in the country by volume. Dr. Messa has also achieved Chairman status with Medicis and Mentor Corporation. This award honors distinguished leaders in cosmetic surgery for demonstrating expertise and excellence through a high volume of aesthetic product procedures. The certification is awarded to leaders in the field of medical aesthetics and demonstrates their commitment to innovation, excellence, and the highest level of patient care.

ASK THE MAYOR

Pembroke Pine Update BY MAYOR FRANK C. ORTIS he safety of our children in and out of school remains a top priority for everyone in the city. That’s why we have been busy in sharing information with school administrators, parents, students and community leaders via school presentations and special gatherings that target such critical issues from online and school safety to drug concerns. Most recently, I attended a presentation by our Pembroke Pines Police Department Threat Assessment Team that was made to West Broward High School and Silver Trail Middle School parents and students, as well as at a principals’ breakfast. Information regarding online and gaming safety risks, which included the topics of social media awareness, cyberbullying, sexting, online predators, exposure to inappropriate content etc., and the dangers and consequences of vaping and THC Oil, were discussed. Suggestions were provided to parents about what they can do to protect their children and how to address these hidden dangers with their children. And, of course, school security is always a topic that we address with all of our school principals.

Once again, we also cannot stress enough that false threats of violence against schools are not jokes. There will be severe consequences for such behavior that will affect the student far into the future and for the rest of their lives. On a lighter note, I’d like to invite everyone to the 18th Annual Mayor’s Golf Tournament being held on Friday, March 22, 2019 at the Pembroke Lakes Country Club, located at 10500 Taft Street, from 10:30 a.m. to 6 p.m. This Golf Tournament benefits the Pembroke Pines Charter School Foundation. Over the past 18 years, this event has raised over one million dollars thanks to the generosity of sponsors and golfers. It includes an 18-hole tournament, a putting contest, lunch, refreshments on ‘The Green,’ deluxe buffet dinner, prizes, a live auction and awards. I hope to see you there! And, be sure to check out our city calendar on the city’s website at www. ppines.com (scroll down and hit View All Events) to make sure you don’t miss a lot of exciting programs and activities coming up, such as the 21st ArtFest in the Pines, Relay for Life and all of the many fun events happening at the Carl Shechter Southwest Focal Point Community Center like the Spring Tea. As always, if you have any questions or concerns, please do not hesitate to contact my office at 954-450-1020, or by e-mail at fortis@ppines.com.

LEGALLY SPEAKING

Are You Single? More than a law firm. We’re your neighbors.

BY BRUCE IDEN, ESQUIRE f you are single you make up over 45 percent of the US population. Whether unmarried, divorced or widowed, single people have important estate planning issues to think about. If you have children, your decisions are a bit more complicated. Consider the following: ► Single with minor children: It is vital to designate whom you wish to serve as your child’s guardian. Think about this. Even if there is a surviving parent, they may not be your choice to serve as guardian. What about leaving assets to your minor children? Realistically, leaving an inheritance to minor children can be fraught with risks. Who manages the assets for your minor children? Should you select your ex-spouse or perhaps another family member? Often parents select one person to serve as guardian for minor children, name another individual to manage and invest assets for their children, and possibly choose a third to determine asset distributions. ► Single with adult children: When a child turns 18, legally he or she is fully capable of receiving their inheritance and making life decisions, such as selecting a college or managing and spending their own money. Do 401(k) and IRA accounts constitute a significant portion of your estate? Severe and irreversible tax consequences can result from careless withdrawals from these inherited assets by “young” beneficiaries. Do you have any concerns about your child inheriting a large amount of life insurance money? Parents of 18-year-olds recognize that while their child is technically an adult, it may be much too early to expect thoughtful decisions regarding their future and their finances.

► Single without children: If you are single without children, keep in mind that without the existence of a properly drafted and executed will, Florida law provides that your assets be inherited by your parents. If your parents do not survive you, your assets will be inherited by your surviving siblings. Having a will is essential if you wish to leave your assets to anyone other than your immediate family. ► Illness or disability: Whether you have children or not, there are important decisions to make regarding who is authorized by you to assist with medical and financial matters should you become ill or disabled. Advance legal directives, in the form of health care powers of attorney, durable powers of attorney, living wills and HIPAA releases, insure your bases are covered in various scenarios. Without such documents, urgent medical care, managing a business or your finances, and communicating with governmental agencies such as the IRS or Social Security Administration, may need to be determined by the court. Life can change in an instant. What are you waiting for? LEGALLY SPEAKING

Divorcing After 12/31/18 BY EVAN BARON eading up to the end of 2018, there was a race not to the Courthouse but out of it. Spouses who had already filed for divorce, whether recently or those who had long pending cases, and who would be responsible for paying alimony were actively racing to finalize their cases prior to the end of the year. The reason: the changes to the tax code implemented by the 2017 tax law. For decades, a long-standing staple of alimony negotiations was the subsidizing effect of payments that were tax deductible to the paying spouse and taxable as income to the spouse receiving alimony. This dynamic helped facilitate negotiations that are often difficult for the parties, both financially and emotionally. Previously, the deductible nature of alimony incentivized a paying spouse to agree to a larger number because doing so would result in a tax benefit down the road for the length of the alimony award. This was especially useful when trying to reconcile each spouse’s perception and preference as to what number was fair and reasonable based on the couples’ particular circumstances. Individuals seeking to dissolve their marriage in a post 2018 landscape will be some of the first to experience negotiations that exclude this useful incentive. However, it is important to keep in mind that while the paying spouse will no longer have the option to deduct their alimony payment, the spouse receiving the alimony award will no longer have to claim these funds as income. Previously, a rationale for asking for a higher alimony award was due to alimony being taxable to the receiving spouse. This meant that the number agreed to on paper, depending on the individual’s tax bracket, could constitute a very different net amount once taxes were considered. Under the new tax code, the number agreed to will be the amount that will be paid to the receiving spouse. There is no longer an ambiguity as to what these funds will look like at the end of the year. It is possible that this dynamic too may rise to be a useful aspect of the new tax laws when spouses are negotiating the topic of alimony. The 2017 Tax Law brought with it several changes that will reverberate in countless ways across all economic classes of individuals. The same is true when applied in the legal arena of family law. The deductible/taxable nature of alimony was not the only change to come about. For example, the new tax law has eliminated the previously existing exemption for dependents, while also increasing the child tax credit. It is important for individuals considering divorce to educate themselves about the new changes and, prior to seeking legal counsel for their dissolution of marriage, speak to an Accountant, financial advisor or requisite professional to gain awareness of any resulting implications in order to determine what financial strategy is best suited for his or her circumstances. Doing so will alleviate the stress of the unknown and will assist individuals seeking divorce in providing their Family Law attorney with information necessary to help accomplish the financial goals a spouse hopes to attain in their family case.

Fretting About Colonoscopy? New Prep Is Easier to Swallow. When your doctor says it’s time to schedule a colonoscopy, do you start thinking up excuses to put it off? You may be concerned about unpleasantness and inconvenience as you prepare for the exam. But it’s time to stop worrying – the prep process is much easier these days.

• Switch to clear liquids. The day before the procedure, consume only clear liquids. “It’s important to keep patients hydrated and flush the prep through the intestinal tract,” advises Dr. Maric. “The clear liquids aid in that process.”

Yes, one of the keys to a successful colonoscopy is the “clean-out” beforehand. But improvements – in the liquid you drink, the amount you must drink and the timing – make the whole process easier to swallow.

• Start the prep. Start the prep. Many physicians today prescribe a split-dose regimen, where half the prep is taken the night before and the other half in the morning. But, if the exam is in the afternoon, the prep is taken the morning of the colonoscopy.

Regular colonoscopies can save your life Colorectal cancer is the second leading cause of cancerrelated deaths among men and the third leading cause among women in the United States. Most patients who develop colon cancer have no obvious risk factors. “The colon, itself, is a risk factor,” says Leyla Maric, MD, a gastroenterologist at Cleveland Clinic Florida’s Weston maincampus. Bowel residue polyps are a precursor for Leyla Maric, MD cancer, and they can be overlooked if the colon lining is not clear of solid and liquid. “Many people fear drinking the laxative prep to cleanse the bowel more than the colonoscopy itself,” says Dr. Maric. But it’s a key part of the procedure. Three steps for a good prep: • Cut out fiber. Two days before the exam, start a low-fiber diet.

A better-tasting solution “The liquid bowel-cleansing agents now taste better and less of the liquid is consumed. This means that the prep is, quite literally, easier to swallow,” explains Dr. Maric. Those two factors, combined with the split-dose approach, improves the experience for patients. Regular colonoscopies can detect colon cancer early on. It’s important to follow your physician’s screening recommendations. To schedule a colonoscopy today, call 800.639.DOCTOR.
