Moments of Kindness T
his year we will mark the 20th anniversary of the worst terrorist attack in American history. Like many, we will never forget the moment we learned of the tragedy and vowed to never forget. We were living in Hong Kong, on a temporary work assignment at the time of the attack. After confirming the safety of our friends, coworkers, and loved ones we spent the remainder of that day wandering through that great city in a bit of a numb fog. What we will also never forget was the omnipresent kindness of strangers on that day. Countless Chinese and Cantonese strangers approached us that day to offer condolences. Our appearance and mannerisms made it fairly obvious we were Americans. Strangers simply wanted us to know they felt the same grief we did. Outside the United States Consulate people had begun placing flowers in a sort-of make-shift memorial. We attempted to purchase flowers to place but were told they would be free for us on that day. A young girl gave us a single yellow flower as if we were somehow representative of those who had been lost. Like everyone, remembering that terrible day will always conjure images of the worst people can be. But, for us, we also remember those moments of kindness and shared humanity. We are hopeful, even in our darkest moments, that the “greater angels” of our souls will win out. Sincerely,
Christy & Terry Jaillet Christy & Terry Jaillet Publishers

Answers for back pain Miami Neuroscience Institute’s Spine Center offers comprehensive care
Y
ou bend to grab a piece of paper off the floor and a bolt of pain shoots from your back down your leg. With back and neck pain affecting 80 percent of Americans at some point in their life, it’s an all too common occurrence. But how do you know if the cause of your pain is something serious? The experts at Miami Neuroscience Institute’s Spine Center, a part of Baptist Health, suggest a comprehensive evaluation to get to the root of your problem, alleviate your pain and prevent long-term issues. Physiatrists — physical medicine and rehabilitation specialists — at the Center’s multiple locations deal with issues affecting the spinal cord, including problems with bones, joints, nerves, muscles and tendons. “We almost always isolate the issue, and with a proper diagnosis comes the proper treatment,” said Ronald Tolchin, D.O., medical director. Causes of back and neck pain include degenerative disc disease, arthritis, skeletal abnormalities, spinal stenosis, herniated discs and more. Even screen time and texting can throw the neck out of alignment. And with the COVID-19 pandemic, doctors are seeing an increase in back problems
due to too much sitting, not enough exercise, weight gain and working at makeshift home offices that lack chairs with lumbar support and armrests. The Center works with everyone from athletes to the elderly, and handles extremely complex cases. For some patients, an initial assessment is followed by imaging studies — an MRI or CT scan, for example — that reveal problems such as a narrowing of the spinal column, a bone spur pressing on a nerve, bulging discs or arthritis. Nerve conduction studies are performed on-site. The good news is that medical advances have made it possible for most patients to avoid surgery. “We have a whole armamentarium of noninvasive and holistic treatments available today,” Dr. Tolchin said. The Spine Center team may recommend stretching and physical therapy, anti-inflammatory and injectable medications, weight loss with an antiinflammatory diet and safe exercise. In addition to injections that are administered in the office, minimally invasive radiofrequency ablation (RFA) is performed at an outpatient location. RFA uses heat to disrupt the nerve tissue’s
14 / Our City Plantation / September 2021 / OurCityMedia.com
transmission of pain signals to the brain. Epidural and facet joint injections are also handled on an outpatient basis. Some 90 percent of back and neck problems are improved with these noninvasive approaches. For the 10 percent who require surgery, the Center’s team works closely with the neurosurgeons at Miami Neuroscience Institute. “As a comprehensive spine center, not only do we look at you holistically and provide many services in one location,” Dr. Tolchin said, “but I can literally walk down the hall to any of my neurosurgical colleagues, pull up images and discuss a patient.” Follow-up with back and neck pain patients is also important, according to Dr. Tolchin. “We don’t just treat you and say, ‘See you in five years.’ For some patients, the condition can be chronic. We’re a place where they can find salvation, and we will help them get through it.”
With locations throughout Miami-Dade and Broward counties, Spine Center appointments can be made by calling 786-596-3876 or visiting BaptistHealth.net/Spine.
GET BACK TO A LIFE WITHOUT PAIN. At Miami Neuroscience Institute’s Spine Center, we want to help you get back to a life free of neck and back pain. ongratulations to Sandpiper Elementary School student, Daniella Meijberg, Grade 5, who earned Honorable Mention in the The Jean & Jules Kluger Visual Arts Contest. The Holocaust Documentation & Education Center (HDEC) continues to educate students on how to combat bullying and modern-day racism through its annual Visual Arts & Writing Contests: The Jean & Jules Kluger Annual Visual Arts and Writing Contest and the “A Simple Act of Kindness” Contest. This year, students created a wide array of artistic creations, including paintings, poems, essays, and multimedia presentations. There was a total of 149 entries and 23 winners. The Jean & Jules Kluger Annual Visual Arts and Writing Contest seeks to engage students from grades K – 12 and empower them to submit either a literary, visual arts or musical creative expression addressing the contest theme. This year the theme was: “How Can We Be Our Brother’s Keeper and Stand Up for One Another?”
For the second year, students faced the added obstacle of COVID-19 and, despite the fact that schools were operating both in-person and virtually, all the students and teachers did everything they could to participate in the contests. “Year after year, I continue to be overwhelmingly impressed and moved by the contest submissions from students as young as elementary school up to the seniors in high school,” said Rositta Kenigsberg, president of the Holocaust Documentation and Education Center. “We are proud to see that they are truly representing part of our mission in promoting kindness and understanding the ramifications of prejudice and hate.”

Gynecology Myths By Scott Jordan MD
Myth 1: Pap
smears test for all gynecologic
cancers
Pap smears are only designed to detect cervical cancer (or precancer cells). They do not screen for ovarian or uterine cancer.
Myth 2: A “total” hysterectomy removes the uterus, cervix, fallopian tubes and ovaries, while a “partial” hysterectomy only removes the uterus and cervix Removal of the fallopian tubes and ovaries, called a salpingo-oophorectomy, is always a separate procedure from a hysterectomy, which is only the removal of the
uterus. Technically, a “total” hysterectomy includes removal of the cervix, while a “partial” or “supra-cervical” hysterectomy, leaves the cervix behind.
Myth 3: Some spotting is normal after menopause Uterine bleeding is never normal after menopause and should always be investigated by a gynecologist. It can be a warning sign of endometrial cancer or a precancerous lesion and may need to be treated.
Myth 4: After my hysterectomy, I do not need to see a gynecologist All women should continue to follow with a gynecologist for annual exams. If ovaries are left behind, there is still a risk of developing ovarian cysts or
22 / Our City Plantation / September 2021 / OurCityMedia.com
even ovarian cancer. Vulvar and vaginal symptoms can also be evaluated. An annual pelvic exam can detect abnormalities early, before a potential problem becomes worse.
Myth 5: If I am post-menopausal, there is no reason to keep my ovaries at the time of my hysterectomy Very large population-based studies have demonstrated a decrease in risk of death from any cause in women under age 65 who kept their ovaries at the time of their hysterectomy if there was no indication to remove them. There is also a decreased risk of Alzheimer’s disease, and improved heart health. Even after menopause, your ovaries still work to keep you healthy!

Lynne Kunins
FLIPANY Founder, President, CEO (Florida Introduces Physical Activity and Nutrition to Youth)
•
HER STORY
Lynne Kunins discovered her true calling when she volunteered to work with South Florida kids of parents that were serving time in prison. She led the kids through after-school team-building games, fun routines for building physical skills, and provided hands on experiences with new and healthy foods. She felt such a strong connection to these kids that she promised to find a way to sustain and expand the program. Lynne founded FLIPANY in 2005 and started writing grant proposals. It was a struggle in the early days, working alone and learning the hard way. Lynne’s first hire was a registered dietitian — a commitment to ensuring program quality — and in the first year of operations she hosted the first Annual FLIPANY Fun Run, became the designated Florida Partner for Cooking Matters food skills education programs, and managed an operating budget of $106,000. Sixteen years later, FLIPANY operates a budget of over 6 million dollars with a team of 26 full time staff and management team. During Covid-19 pandemic, FLIPANY retooled its programming to distribute healthy meals to kids and adults and to provide virtual cooking classes, fitness classes, and nutrition education activities.
24 / Our City Plantation / September 2021 / OurCityMedia.com
By the end of 2020, FLIPANY had partnered with over 250 South Florida meal distribution locations to deliver over 2 million healthy meals.
WHAT ADVICE WOULD YOU GIVE TO YOUNG WOMEN?
Lynne’s number one tip for aspiring women in business would be to work on something you want to wake up to everyday and make a difference; make mistakes, learn, grow, and partner with people who want to see you and your business succeed. Collaborations are very important because you learn from other businesses and that’s exactly what Lynne did since day one; asked questions and sometimes for forgiveness but never stopped building relationships with her community.
CREATING HEALTHY LIVES
Lynne has been through all walks of life but through her tenacity and resilience she has been able to build a place in South Florida that brings access to health and wellness in areas that can’t get access easily. She believes that FLIPANY’s work is not done until everyone has the opportunity to live a healthy life.

Angie Bolanos
Occupational Therapist & Health Coach
•
HER STORY
Angie is the founder and owner of Bolanos Associates. Her firm provides Physical and Occupational Therapy to patients at three locations, plus two satellite locations. While much of her time is spent managing and developing her business, she still takes time to work personally with patients. Angie specializes in treating the shoulder, elbow, wrist and hand. She recently became a Certified Health Coach.
WHAT ADVICE WOULD YOU GIVE TO YOUNG WOMEN?
I encourage girls or young women with a dream of owning a business to not allow society set limitations or expectations to hold them back. As women, we are capable of managing a business and having a family, and managing a household if that is what you desire.
HOW HAS BEING A WOMAN AFFECTED YOUR CAREER?
Being a women business owner has had its challenges, but I do not feel it has negatively affected my career. When I openned Bolanos Associates, people were surprised that I was the owner because I was a woman. Many times they asked if this was a family business, meaning my father or a husband. My biggest challenge as a woman business owner was feeling that I had to prove myself to everyone. Establishing meetings, accounts, managing staff, various locations, and balancing my life with my family and son were challenging. Being a business owner is very demanding and requires a lot of hours of work and dedication. My goal was to establish a business recognized in the community for providing excellent service to our patients. However, I am a mom, and when I started my business, I was a single mom with a 5-year-old son. My challenge was working long hours when my son was young. I had to juggle my business with baseball activities, school work, and extra school activities all parents want to attend.

Plantation Resident Makes Sure No One is Alone By Beverly Perkins
T
he nation’s largest grassroots mental health organization, the National Alliance on Mental Illness (NAMI), is dedicated to building better lives for millions of Americans affected by mental illness. Their statistics show that 1 in 5 Americans live with a mental health condition.
Linda Gutierrez with Plantation Councilmember Nick Sortal
The ambitious organization provides real-time mental health support and guidance every single day to anyone who needs it. They also encourage people to share their personal stories as it can change the hearts and minds of policymakers. Additionally, they continually ask municipalities across the country to increase their awareness of mental health crisis prevention by issuing proclamations. Most recently, Plantation resident Linda Gutierrez, an advocate for mental health awareness, accepted a proclamation from the Plantation City Council as part of the nationwide endeavor. Gutierrez, a member of the NAMI Advocate Group, proudly accepted the proclamation and briefly addressed the council on mental health awareness.
“It is not only more humane for society to treat people for mental illness, but it is also more efficient,” said Gutierrez, noting that early treatment can prevent crimes, long hospital stays, and family debts. Councilmember Nick Sortal read the proclamation and spoke briefly about the importance of mental health awareness. Portions of the proclamation read: “Stigma and fear of discrimination over their condition prevent many from seeking help and “people can recover if given the necessary support.” Sortal also suggested that people scan through NAMIBroward.org to be prepared if a crisis ever occurs. “So many people can be tiptoeing right up to the edge of being in a bad place. It’s critically
important that everyone view those people as an opportunity to help, rather than form some kind of a judgment,” explained Sortal. “So if a crisis comes, people are aware of what resources they can immediately access,” Sortal said. “Their slogan – “You are not alone” – is a message that’s incredibly vital in these times.” NAMI’s grassroots advocates have helped pass federal parity laws and mental health reform, expanded coverage through the Affordable Care Act, and successfully fought back attempts to end patient protections.

Testicular Cancer, the 4-1-1
M
By Michelle L. Walton
any people know about breast cancer, but how many people know about Testicular Cancer (TC)? While the numbers of occurrence of TC might be low in comparison to occurrences of breast cancer, TC accounts for 1% - 2% of all cancers that occur in males. The good news is, if caught early, the survival rate is higher than 90% among males that receive treatment. About 1 in 250 males will develop testicular cancer in their lifetime, and TC most commonly occurs in young males ages 20 – 35. Risk factors include smoking, heavy cannabis use, family history, white race, and cryptorchidism – when the testicles fail to descend normally. In the early stages, there are usually no symptoms beyond a mass within the testes. So the question becomes, how do men know whether or not they have testicular cancer? The biggest issue might be getting men to pay better attention to their health. Some men are hesitant about going to a doctor, asking for help, or even asking for directions. This stereotype may be due to masculine gender socialization: boys are raised not to show emotion or vulnerability, thus equating
seeing a doctor or asking for help or showing themselves to be vulnerable as weakness.1 Visiting a doctor for regular checkups is not a sign of weakness. It is just good sense. Learning how to perform a TSC (Testicular Self-Examination) is also just good sense.

Talar OCD By Dr. Warren Windram D.P.M.
W
hen your simple ankle sprain doesn’t get better and still hurts after one month, what do you do? Talar dome lesions are usually caused by an injury, such as an ankle sprain. A talar dome lesion is an injury to the cartilage and underlying bone of the talus within the ankle joint. It is also called an osteochondral defect (OCD) or osteochondral lesion of the talus (OLT). “Osteo” means bone and “chondral” refers to cartilage. If the cartilage doesn’t heal properly following the injury, it softens and begins to break off. Sometimes a broken piece of the damaged cartilage and bone will “float” in the ankle. Visiting a podiatrist to diagnose and treat these injuries can prevent further degradation and arthritic breakdown of the ankle joint. Non-surgical treatments such as bracing, oral non-steroidal anti inflammatory medications, and physical therapy may help
alleviate stable lesions. Surgical treatment with restorative techniques to the talar dome lesion include biologic augmentation (Platelet Rich Plasma Injections), marrow stem cell stimulation with micro-drilling, allografting and autografting transfer and implantation techniques. While there are a variety of surgical techniques available to accomplish this, the podiatric surgeon will select the best procedure based on the specific injury.

THE DONOWAY CENTER New Clinical Trial Available Now for Patients with Advanced & Metastatic Pancreatic Cancer
Are you, or someone you know, being treated for Advanced or Metastatic Pancreatic Cancer with chemotherapy alone? Contact Dr. Robert Donoway at Atlas Oncology in Hollywood, Florida. Dr. Donoway is leading a new clinical trial using multi-modality therapy: combining Pancreatic Nanoknife Ablation with chemoimmuno-radiotherapy to treat advanced stage cancer patients from around the world. With over 35 years of experience performing pancreatic surgery in South Florida, Dr. Donoway recognized that, because of its late diagnosis, up to 85% of the patients treated with surgery for pancreatic cancer already had metastatic disease. This fact limits the benefit a patient might get from the Whipple surgery. Recent evidence shows that when even advanced disease is treated aggressively in an integrated approach, incorporating Nanoknife surgery with chemotherapy, long-term survivorship can be achieved, especially compared to chemotherapy alone. Dr. Donoway has been a pioneer in the use of Pancreatic NanoKnife, especially for complex advanced cases. During the past decade, he remains the only surgical oncologist performing the procedure in South Florida. Patients from around the globe have traveled to benefit from this ground-breaking treatment, which is
FDA approved. Dr. Donoway also treats other advanced cancers, such as liver, breast, and sarcomas, using Nanoknife surgery. A brief recovery means the patient can start chemotherapy and radiation that much sooner. Dr. Donoway feels that “we are long overdue to revise our approach to treating pancreatic cancer. The treatment has remained largely unchanged for over 80 years and has just broken the 9-10% five-year survivorship barrier. Only thru thinking outside the box will we accomplish the positive. Otherwise, all we’re doing is confirming Einstein’s definition of Insanity.” Dr. Donoway is participating in several clinical studies identifying the best chemotherapy and immunotherapies to be used along with IRE. Dr. Donoway states, “The results so far have been very encouraging and we have begun using it for other select cancers. Among his longest survivors are now patients over eight years without evidence of cancer recurrence. In the past, without IRE they would have been likely to have only lived a year and so this technique opens the whole new avenue for treating those eligible patients in a more targeted fashion.” Dr. Donoway will be presenting the long-term results of his pancreatic treatment paradigm for advanced and metastatic disease later this fall at an International cancer society in Europe For more information about Dr. Donoway and the PancreaticIRE/NanoKnife Program and clinical trials call 305-682-1995 and visit www.AtlasOncology.com.

egacy means so much more than the assets we leave behind in the form of an inheritance. Most of us never give much thought to what our legacy might be, but rather we think about a will or trust that carefully lays out how our property is distributed after we are gone. It’s usually about our “stuff.” Interestingly, studies on adults and their inheritances may surprise and disappoint you. The Journal of Family and Economic Issues reported that adults who receive an inheritance save only half of what they receive. Other studies point to statistics showing virtually all money or property left to loved ones in the form of an inheritance is spent within nine months. So thinking in terms of money and property, a legacy has a very short shelf life. With that in mind, let’s explore other ways to leave a legacy that will live on long after you are gone. Charitable Giving. Most obviously charitable giving provides support to a cause or causes you deem important. But it is also a teaching moment for your loved ones; it reinforces the importance of providing for the greater good. Many of us have made quiet personal promises to support a charity or cause “later,” or “when we can better afford it.” Making room in your final wishes for charitable giving answers those promises. Aside from the tax advantage charitable bequests generate, this selfless act can echo for generations. Many people leave a percentage of their estate to charity instead of a specific dollar amount. By leaving a percentage, your family and charity will receive proportionate shares regardless of how large or small your estate is. Instead of leaving money to a charity in a will, you may also consider naming a charity as a beneficiary of an insurance policy or retirement plan. Ethical will. An ethical will has ancient origins, originally appearing in the Book of Genesis when a dying Jacob spoke with his sons to share wisdom and his hopes for how they should live their lives. Originally, ethical wills were communicated orally, but were later written as letters. Today, ethical wills are known as “legacy letters.” It is a way to communicate to loved ones your hopes and dreams, regrets, and lessons learned. Often apologies are offered and forgiveness is sought. Family lore, favorite jobs, funny stories may all be part of your letter. Think of a legacy letter as a way to pass along wisdom and love to a future generation. Preserving family history. Consider writing down your family stories on your own or through a subscription such as StoryWorth. It’s a way to collect family stories and preserve them in a keepsake book. The recipient of a StoryWorth gift is emailed questions each week for a year, but they can skip a week or select their own prompts. These questions are a starting point to tell your story and document your family history. A true gift for your family. Life can change in an instant. What are you waiting for?
