RESIDENT Resident registration begins Monday, July 25 at 8:30am online at palatineparks.org and in person at Community Center, Birchwood Recreation Center, and Falcon Park Recreation Center.
NON-RESIDENT Non-resident registration begins Tuesday, August 2 at 8:30am online at palatineparks.org and in person at Community Center, Birchwood Recreation Center, and Falcon Park Recreation Center.
HOW TO REGISTER
Use the registration form provided in this catalog (page 100); pick one up at Community Center, Birchwood Recreation Center, or Falcon Park Recreation Center; or download a form from palatineparks.org. 1. List the program number of the chosen program or event in the “PROGRAM #” box. 2. Write the name of the program, participant’s full name, birthdate, gender, and fee. A participant must be age appropriate when the class starts. Write in the fee(s) of the activity. Please include appropriate signatures and emergency information. Incomplete forms will be returned, which will delay the processing of your registration. 3. In-person registration may be processed by cash, check, or credit card. If dropping off a registration form, provide all necessary information and include a check for payment. Forms cannot be processed unless all information is provided. 4. As soon as your registration has been processed, your check will be deposited or your credit card will be charged. Your receipt will indicate if you have been placed on the wait list. Please verify this information. Note: Each household must use their own registration form. Placement with a friend or relative cannot be guaranteed.
CONFIRMATION/RECEIPT
Walk-in registrants receive a receipt immediately. Online registrants may print a confirmation at the time of registration. Class Full: If the class is full, you will be placed on a wait list. Wait List: If you are placed on a wait list for a class, and an opening becomes available, you will be contacted by customer service staff. Within 24 hours of being contacted, payment must be made online or at Community Center, Birchwood Recreation Center, or Falcon Park Recreation Center.
Transfer or Refund: If you wish to be transferred to an alternate class or receive a refund or household credit, fill out the appropriate form and drop it off at Birchwood Recreation Center, Community Center, or Falcon Park Recreation Center. See next page for the transfer/withdrawal and refund policy.
WAIVER REQUIRED
For insurance purposes, Park District programs and activities require a signed waiver. All adult (18+) participants must sign the waiver. Children under 18 must have waivers signed by a parent or legal guardian. Patrons WILL NOT be able to participate in classes or programs if the waiver is not signed.
REGISTER EARLY!
Classes will be canceled starting Monday, August 22 if they do not reach the minimum number of participants.
SPECIAL REGISTRATIONS
Special registration forms and separate payment are needed for affiliate sports, C.A.R.E., the Preschool Program, Palatine Tiger Sharks Swim Team, and Turkey Trot.
SPECIAL NEEDS AND INCLUSION SERVICES
Palatine Park District does not discriminate against eligible residents for participation in programs on the basis of a disabling condition. Residents with special needs should notify staff when registering so that smooth inclusion may occur. In addition to the programs listed in the program catalog, the Park District provides specialized services for people with disabilities through NWSRA (see page 17 for more information). Note: When registering, the parent of the individual should note on the registration form if an NWSRA aide is needed in order for the individual to successfully enjoy the program.
ALLERGIES/ILLNESS
If your child has an allergy or illness, please note it on your registration form. Park District Staff will accomodate to the best of our ability. If you have questions or concerns, contact the Park District.
CATALOG CHANGES/ERRORS DISCLAIMER
Occasionally, there may be an error that appears in print or online regarding days, times, fees, or other information. When such errors occur, Palatine Park District will do everything possible to correct the situation promptly. For the most current information visit palatineparks. org. Thank you for your patience and understanding when these situations arise.
CANCELLATION POLICY
If a program does not reach its minimum number of participants seven days prior to the starting date of class, it may be canceled. If a class is canceled, participants will be called and have the option of transferring to another class or receiving a refund.

Four ways to register
Full payment must be enclosed with registration. Do not send cash through the mail or place cash in the drop boxes. No registration taken by phone or fax. Do not write credit card numbers on forms. All purchases must be paid in full at the time of registration.

MAIL • CHECK ONLY
Mail forms and payment to Palatine Park District, Registration, 250 E. Wood Street, Palatine, IL 60067. Make checks payable to Palatine Park District.
DROP-OFF • CHECK ONLY
Place registration forms and payment in an envelope and drop off at Community Center, Birchwood Recreation Center, or Falcon Park Recreation Center. Outdoor drop boxes are accessible 24 hours a day.
Creating A HOUSEHOLD
A household must be created in the registration system before any registration for programs, events, passes, or memberships can be completed for any members.
HOW TO CREATE A HOUSEHOLD ONLINE
1. Visit palatineparks.org. 2. On a computer, hover over the Register Online tab and select Create a Household from the dropdown menu. On a mobile device, select the Menu tab, tap the Register Online dropdown, and select Create a Household. 3. By default your household will be created with a Non-Resident status. Upon review, if your household should have Resident Status, the account will be corrected and a credit will be applied.
HOW TO CREATE A HOUSEHOLD IN PERSON
Visit one of the registration offices during regular hours to complete a household information form in person.

TRANSFER/WITHDRAWAL AND REFUND POLICY
If you have any questions about Palatine Park District’s Transfer/ Withdrawl and Refund Policy, please contact Christine Hubka, Customer Service Manager, at 847.496.6223 or chubka@palatineparks.org. To submit a refund request, an Application for Transfer/Withdrawal Form must be submitted. The form can be downloaded from palatineparks.org or can be picked up at Community Center, Birchwood Recreation Center, or Falcon Park Recreation Center. Telephone requests will not be accepted. • All approved refunds will be applied as a household credit unless otherwise requested on the Application for Withdrawal/Transfer form. • If a refund other than household credit is requested, a $5 processing fee will be applied. • If requested, approved refunds will be made via check for cash or check payments, or will be refunded back to the original card number used for credit card payments. Please note, refunds via check can take 10–21 days to process. • Approved refunds of less than $10 will only be processed as household credits. • Refunds/Transfers are not given for special events, trips, or one-day programs unless the event is canceled by Palatine Park District. • Refunds/Transfers are not given for passes or memberships. • Some programs have separate refund policies which will be noted next to the program information.
TIMING OF REQUESTS
• A full refund (less a $5 processing fee) will be processed for approved requests received seven days prior to the start of the program. • Requests received less than 7 days will be evaluated and processed accordingly. • Program and other non-refundable materials may be deducted from the refund total.
MEDICAL REFUND REQUESTS
A note from a physician is required for medical exceptions. Approved medical exception refunds will be prorated from the time the request is received.
Medical exceptions cannot be processed after the class has ended.

Affiliates of Palatine Park District process their own refunds. See pages 56-57 for contact information.
HEALTH AND RECREATION CENTER (HRC)

WINTER REGISTRATION
The 2023 Winter Catalog will be available online for viewing Monday, November 21. Registration for residents will begin Monday, December 5 at 8:30am. Non-resident registration begins Tuesday, December 13 at 8:30am.
