Baby envy
Okay, I’m just going to say it: Working on this guide always gives me a major case of baby envy! It is a particularly poignant case this time around, because my youngest child — my baby — will be leaving the nest to head off to university this year. It is so true what they say about parenting: “The days are long, but the years are short.”
When I was casting about for a theme for my editor’s note for this issue of PNW Baby, I was inspired by parental burnout prevention strategist Shelley Kemmerer’s advice for prenatal family planning specific to cultivating resources and formulating a postpartum action plan that will balance the mental load of parenthood (p. 46). Corny, perhaps, but the Boy Scouts’ century-old “Be Prepared!” motto sprang readily to mind. Reflecting back on my own early parenting experience, I don’t know that I was terribly proficient at this type of preparation, to be honest.
However we go about becoming parents, the truth is that every family requires a unique set of resources to outfit and inspire their particular journey — and resource discovery and navigation is the chief purpose of this annual PNW Baby guide!

It’s a Messy Topic, But Let’s Talk About Poop
There are a lot of ways to describe it: soft, hard, brown, green, runny, and certainly stinky. It’s a messy topic often discussed behind closed doors, but let’s break down the scoop on poop — what parents need to know, from its color, consistency and frequency to what to do when a child is “backed up.”
According to Mollie Grow, M.D., a pediatrician at Seattle Children’s and the University of Washington, it can be challenging for some families to have frank conversations about the body’s natural process of elimination. In other words, it can be hard to talk about poop.
“As pediatricians, we get really comfortable talking about poop,” says Grow. “We try to normalize the conversation with patients and families.”
Breaking it down
According to Grow, normal, healthy poop can range in color. This is especially true for babies. Since parents are more aware of changes in their baby’s poop day to day, Grow says to think about the colors of fall when evaluating if there is cause for concern.
“Green, brown and yellow can all be normal,” says Grow. “But if the colors are more like [shades of] Mickey Mouse — black, white and red — then you should call your child’s doctor. These colors are abnormal and can indicate something might be wrong.”
If a child’s poop is black, it can be a sign of internal bleeding. If there is red,
or blood, in the stool, a child may have a cut around their bottom or in their intestine. White poop occurs when there is a lack of bile, which may indicate an underlying problem, but according to Grow, this is very rare.
It’s also important to note that diet can play a role in the color of a child’s poop. For example, eating a lot of iron could cause poop to look darker and eating beets could make a child’s poop look red.
When it comes to frequency, Grow says children should poop every one to two days, and the consistency should be soft and formed, much like softserve ice cream. If a child’s poop is hard or dry, resembles pellets, or if a child has to strain to poop, they may be constipated.
According to Grow, constipation is one of the most common complaints she sees during clinic visits, and onequarter of referrals to gastroenterologists are related to constipation.
“One of the most common symptoms is abdominal pain,” says Grow. “If a child has tummy pain, the first question you should ask is, ‘When was the last time you pooped?’”
All backed up
According to Grow, there are a number of risk factors related to constipation. “Low intake of fiber and fluids can cause constipation,” says Grow. “It’s important to make sure kids are drinking water between meals and eating enough whole fruits and vegetables.

Pitted fruits, such as apricots or plums and also pears, tend to be really good sources of fiber. Don’t overdo it on bananas, which can be constipating. Also, avoid too much dairy and offer primarily whole grains. Many kids suffer from constipation due to high intake of fast foods and processed foods, which tend to have very little fiber. Admittedly, it is a challenge for families that are always on the go. Looking for ways to add whole foods and food cooked at home is helpful.”
Grow also recommends regular exercise to keep constipation at bay. “Our digestive system works better when we’re active,” says Grow. “Preschoolers need lots of active playtime. They should get two hours each day. Older children should get at least 60 minutes of daily activity. A great way to stay active is by encouraging outdoor time and physical play. A sedentary lifestyle can increase your risk for constipation.”
Seeking intervention
If a child is constipated, it’s key to seek early intervention.
According to Grow, the biggest concern in not treating the problem, it’s that constipation can lead to a child’s inability to feel when it’s time to go to the bathroom. If a child is constipated and has a large collection of stool in their rectum, they can leak poop, a condition called encopresis. If a child holds their poop, they can also lose tone in their rectum.
Another sign a child could be constipated is if they have a sudden increase in bedwetting. When a child holds in their poop, the collection of feces can push on their bladder. With early intervention, the issue can be managed.
“The combination of what we can do from a feeding standpoint and activity standpoint work hand in hand,” says Grow. “Constipation is a common cause of distress in families. The earlier we can intervene, the better. Often, diet changes can help manage it, but if they don’t, there are good medicines that can help.”
Talking about poop, just like going to the bathroom, doesn’t need to be a straining experience. Talk to children about poop and make the experience enjoyable.
“We want kids to not have a fear of poop, especially when they are learning to toilet-train,” says Grow.
Grow also recommends talking to children about their poop as they get older. Parents will be less in tune with their child’s toilet habits as they grow older, and so being able to talk about poop will help parents identify if something needs to be addressed with their doctor.
So, the next time a child says, “My tummy hurts,” it’s helpful to ask, “When was the last time you pooped?” ■

Burnout Prevention as a Component of Family Planning
3 steps to sharing the load

Let’s envision arriving at home with our newborn. Prior to that arrival, we have prepared a space within our home for our new baby. Within that dedicated space, we have stocked diapers, clothing, supplies for bathing and feeding, and so forth. This is what I like to call “mise en place for parents”: We gather and arrange these items for immediate use and replenish them when our supplies run low.
Could we prepare in a similar fashion to help prevent parental burnout? To a certain degree, we can. As a burnout prevention strategist, I propose an additional component of prenatal family planning: burnout prevention, coupled with intentional support and resource cultivation.
Resource cultivation and integration as a component of burnout prevention incorporates discovery, time management, and the ability to strategize and adapt to “parental unknowns.” A recent survey published by researchers at Ohio State University found that 66 percent of working parents meet the criteria for parental burnout, which can be defined as a prolonged response to chronic and overwhelming parental stress, characterized by extreme physical and mental exhaustion, emotional distancing from one’s child(ren) and feeling ineffective in one’s parental role.
As parents, we frequently encounter cancellations and unanticipated scheduling conflicts that can derail the flow of our regular routines. Here are three tiers of burnout prevention that we can implement to help offset impacts:
1. Plan for reliable child-care support. If you are in a partnered relationship, you
can decide who will provide backup care if the other parent works outside of the household. If both parents work remotely, you can schedule specific work windows and break periods in order to look after your child(ren) during work hours. For purposes of transparency and flexibility, discuss this plan in advance with your direct supervisor or manager. If you are a single or solo parent, this is an opportunity to explore your local options and to gather resources specific to child care. If you have trusted friends or family members who can provide additional child-care support, lean on them. If you are struggling to find support, you can research your state’s resources, connect with a resource hub or explore referral agencies.
2. Assemble reputable, reliable, easily accessible resources. Initiate this discussion with your health-care team during a prenatal visit. Some options include:
• Compiling a list of important contacts for health-care providers: OB/GYNs, midwives, postpartum doulas, pediatricians, licensed mental health specialists (if applicable), emergency contact numbers, crisis hotline numbers and others.
• Developing a flexible, customizable postpartum action plan outlining household goals, redistribution of household tasks, meal preparation plans for busy days and protected time for self-care.
3. Develop a flexible sleep schedule. Although obtaining a full night’s sleep may be a challenge during the early stages of parenthood, there are a few factors that we can control and modify: room environment; food, water, caffeine and alcohol intake; and pre-bedtime activities, particularly screen time.
Shelby Harris, Psy.D., recommends sleeping in a cool, dark and quiet environment (similar to a cave) as melatonin production increases with darkness and cooler temperatures. Blue-light exposure before bedtime encourages wakefulness and rumination

while decreasing melatonin production. She recommends weaning yourself off screens gradually, increasing by five-minute intervals each night, with a goal of reaching 60 minutes of screen-free time before bed. She also recommends developing a routine for winding down that incorporates restorative yoga or gentle, provider-approved stretching.
Another crucial element in the prevention of parental burnout is balancing the mental load (or cognitive load) of parenthood. These mental-load tasks include scheduling health-care appointments, event planning and travel, and household task management. Such responsibilities, commonly referred to as the “invisible labor” of parenting, are frequently performed by mothers. What we often overlook as we prepare for parenthood are the components of family planning that emphasize the parents’ well-being, and that must include an intentional redistribution of household and child-related responsibilities.
When we peel back the layers of mental labor, we can distill it to individual components and apply them to burnout prevention as an extension of prenatal family planning.
1. Being vigilant about family members’ emotional, developmental and physical needs
2. Preparing meals
3. Maintaining physical health
4. Monitoring resources
5. Anticipating moods and emotional needs
6. Strategizing and planning for all of the above
At the core of each of these distinct categories lies meta-parenting, or the thoughts, considerations and philosophies that we embody as individual parents. Meta-parenting is, in essence, our own uniquely developed “guide” for how we raise our children and the values that underscore our parenting styles. As we prepare for a new addition to our household, we develop tools and strategies that we can use when the unexpected occurs. Part of family planning includes emotional and mental preparation for
parenthood as well as establishing a trusted multidisciplinary care team. This can be assembled in advance of any expected or unanticipated events involving family.
According to the U.S. Agency for International Development, two of the benefits of family planning include “improvement of women’s opportunities for education, employment and full participation in society; and contribution to economic growth at the family, community and household levels which subsequently reduces poverty.” If we, as a society, want to provide mothers with more workforce participation and opportunities, we have to continue to promote workplace equity. In order to do so, we must continue to address wage gaps, offer comprehensive paid parental leave and promote healthy, flexible employment environments where working parents can thrive.
In addition, parents must be offered free and low-cost opportunities to participate in parenting courses and support groups, which educate parents about parental stress reduction, early child development, time management and the cultivation of household equity. Such resources help new parents develop a positive parenting approach that is beneficial to both child and parent(s). These topics can be discussed with one’s health-care team and, when applicable, specific referrals can be made to help facilitate resource connections.
These burnout-prevention strategies work together to cohesively form an adaptable framework for any household. These strategies do not replace ongoing clinical care, but rather complement your health-care provider’s recommendations. By cultivating these support factors as early as possible, it becomes feasible to decrease the time spent on resource navigation and improve household harmony through enhanced collaboration. ■

Using Signing Exact English (S.E.E.) and oral communication, we prepare deaf and hard-ofhearing children to be self-confident, articulate and academically competitive learners, and engaged community members.
Family Conversations is Northwest School for Deaf and Hard-of-Hearing Children’s birth-to-three program. Children from birth to age three who are deaf or hard of hearing in King, Pierce and Snohomish counties may enroll in services at no cost to the family. Family Conversations works together with families to maximize each child’s natural ability for language acquisition, using both signed and spoken language.

Help serve more children and families impacted by prenatal substance exposure Hope RISING Clinic was founded by Wonderland Child & Family Services. Wonderland has served infants and toddlers for more than 50 years and identified a growing need for comprehensive prenatal substance exposure (PSE) services in our community. To meet this need, we opened Hope RISING Clinic in 2019.
One in 10 children in the United States are born with PSE. Hope RISING Clinic is the only clinic in Washington state offering comprehensive PSE services for children. We provide trauma-informed, evidence-based therapies and supports for families. Prenatal substance exposure is preventable and treatable.
