Managing Syndrome Metabolic Menopause The metabolic syndrome is a lot like happiness among Gen Xers, the power play unit of the Toronto Maple Leafs, and the TALENTS OF Justin Bieber: there is a terrific amount of debate among the cognoscenti as to whether or not that specific entity even exists. But while the singing talent level – or not – of young Bieber doesn’t matter a whit to even the one billion or so young kids who are currently suffering from Bieber fever or to you and me (you might say that we leave it to Bieber believers to debate that one), and the existence or not of the power play unit of the Maple Leafs doesn’t matter a jot to anyone living outside the 416 and 905 calling areas, unfortunately, the issue of whether or not there is such a thing as a “metabolic syndrome” - and more important, what to do about it if it does exist - matters a great deal to a huge and rapidly increasing number of people. And I do mean a huge number because, a recent American study estimated that there are currently 50 million American adults - more than 1 in 3 of them, in other words - living with the metabolic syndrome and that number, this study also claims, is increasing rapidly, and most alarmingly, perhaps, increasing most rapidly in younger women. So enough mystery, already (as my mother always says when I take too long to get to the point): what exactly is this metabolic syndrome, Art, and why does it matter so much? Well, if you believe in a metabolic syndrome, and
the vast majority of medical experts do believe it exists as an entity unto itself, it’s a cluster of crucial metabolic changes that significantly raises a person’s risk of dying from a heart attack, a stroke, diabetes, several types of cancer and probably even Alzheimer’s disease, too. This cluster of changes includes the following (to be specific, you need 3 or more of these to qualify for the metabolic syndrome club, a club you really don’t want to join): • Abdominal obesity, which is defined as excessive fat tissue in and around the abdomen • Blood fat (or lipid) disorders, which should include these big three: - high triglycerides - low HDL cholesterol (that’s the “good” cholesterol) and high LDL cholesterol (that’s the bad cholesterol) - High blood pressure • Insulin resistance (people who weigh too much especially around the middle and who do very little exercise become “resistant” to their own insulin, and eventually because of that resistance they develop higher glucose levels and diabetes). Whew! By the way, if you’re still pondering that list and wondering what exactly we mean by “excessive abdominal fat tissue”, most experts say that if you measure your waist and it’s over 102 cm (40 inches) for men and 88 cm (35 inches) for females, then you have excessive fat around your middle (some experts Continued On Page 11

Macular Degeneration I like to believe I enjoy seeing things. Trees, flowers, the sky, my paycheque, and my daughter are all things I enjoy gazing upon (I won’t bore you with which one I like the best). What if one day you couldn’t see,or even worse, what if the vision loss was gradual, so you knew it was coming? This is the reality of macular degeneration, the progressive damage of the part of the eye which is responsible for sharp, central vision. This month, I will discuss the early warning signs, prevention and treatments for this sightrobbing condition. The bodily function known as sight is fascinating, complicated and fragile. Light passes into our eye and is focused onto the thin tissue on the back of the eye, called the retina. The central portion of the retina is called the macula and it is this area that is responsible for sharp, central vision. An early symptom of macular degeneration is the gradual blurring in the central area of vision. Another symptom may be when straight lines begin to appear wavy. Macular degeneration may only occur in one eye; in this case no vision changes may be noticed. This is why regular visits to your optometrist are important. There are two forms of macular degeneration (MD), wet and dry. The dry form is the most common; about 85% of cases are of this type. Dry MD occurs when the cells of the macula break down and cause yellow deposits to form under the retina. As the deposits increase in number, there is a greater risk of developing the other form of MD, referred to as wet MD. This occurs when small, fragile blood vessels form under the retina. These fragile blood vessels tend to leak blood and fluid and can rapidly damage the retina and macula. While both types of MD can cause significant vision loss or
blindness, the risk associated with wet MD is greater. In general, both forms of this disease are referred to as age-related macular degeneration (AMD). The degeneration can be caused by other factors, such as a genetic predisposition, but these types are rare. There are many things a person can do to protect their eyes from AMD and other eye conditions. The first is to protect your eyes from sunlight; wear your sunglasses. While sunlight has not been proven to worsen AMD, many people still believe that it can cause damage and it is an easily avoidable risk factor. I think everyone should wear sunglasses, especially young children and infants. Their sensitive eyes need protection from the sun’s harmful rays. Smoking can also increase your risk of developing this and other eye diseases. High cholesterol also may play a part in the development of macular degeneration. Are there any medical treatments for age-related macular degeneration? There is currently no treatment for dry AMD. The use of special drugs and lasers is somewhat helpful in stopping the blood vessels from leaking in the wet form of MD, but these treatments will not restore lost vision. Perhaps the best options for avoiding macular degeneration, are regular eye and medical checkups, certain vitamins and diet. A proper diet can help prevent many diseases, including age-related macular degeneration. Consume more fruits and vegetables that are dark green, yellow or orange in colour. Generally, these types of produce contain two ingredients important for eye health, lutein and zeaxanthin. Lutein and zeaxanthin are the primary pigments found in the macula and are thought to protect the retina from ultraviolet sunlight. pounds called carotenoids, which give most fruits and vegetables their bright colours. Lutein and zeaxanthin can be found in spinach, collard greens, kale, broccoli, papaya, oranges, kiwi, mango and bell peppers. Studies have shown that people, with a higher dietary intake of lutein and zeaxanthin, have a lower risk of developing macular degeneration. Preliminary research suggests that taking 10 mg of lutein a day, for 12 months, can improve some symptoms of macular degeneration, but it doesn’t seem to affect the progression of this eye disease. Many eye care professionals recommend that their patients take lutein; there are few treatment options available and lutein appears somewhat helpful for delaying the progression of this disease. There is a supplement that has been shown to help prevent AMD. A high potency multivitamin was shown to help AMD in the National Eye Institute’s Age-Related Eye Disease Study (AREDS). In this large study, people with and without
AMD took a multivitamin containing zinc, betacarotene, vitamin C, and vitamin E. What the researchers found was that there was a risk reduction of 27% for vision loss in people who had advanced AMD, and a risk reduction of 25% for the progression of AMD to a more advanced stage of this disease. In people with no or early AMD there appeared to be no benefit. The AREDS vitamin formula is 500 milligrams of vitamin C, 400 International Units of vitamin E, 15 milligrams of beta-carotene, 80 milligrams of zinc, and two milligrams of copper. Some vitamin manufacturers are making this formulation; look for the word “AREDS” in the name of the product. There are few contra-indication for taking this product; ask your Peoples Pharmacist if this vitamin is right for you. There is some concern that people who smoke and take beta-carotene increase their risk of developing lung and prostate cancer. One way to reduce this risk is to quit smoking. Melatonin, A Body Hormone That Assists Natural Sleep Melatonin is a hormone secreted by the pineal gland in the brain. It helps regulate other hormones and maintains the body's circadian rhythm, an internal 24-hour time-keeping system that affects how and when we fall asleep and wake up. Levels of melatonin are low during the day and rise during the normal sleeping period at night. The amount of melatonin released at night varies among individuals and appears to diminish with age. When your sleep patterns are disturbed, by travel or shift work for example, supplemental melatonin can help reset your body’s ‘clock’ and improve length of sleep and sleep quality. Melatonin is allnatural, non addictive and very safe. Seniors, shift workers and those who travel to other time zones, are likely to benefit from supplemental use of melatonin. Melatonin facilitates sleep by acting as a natural sedative. It ‘turns down’ the brain’s activating system that controls wakefulness. Insomnia is a condition in which wakefulness overrides the brain’s effort to cause sleepiness. Normally, melatonin levels rise in the bloodstream as light diminishes progressively to full darkness, but changes in sleep
patterns, and aging, can affect levels of melatonin and its effectiveness. Supplementation may be required. Frequent insomnia can cause problems during the day, such as sleepiness, fatigue, difficulty in concentrating, and irritability, as well as other subtle health problems. Those likely to benefit most from use of melatonin are older people who appear to have an agerelated decline in adequate melatonin secretion, as evidenced by frequent or chronic insomnia. Shift workers also find melatonin helpful when they are required to adjust to new sleep patterns due to changing shifts. For jet-lag, taking melatonin can help travelers reset their sleep pattern more quickly in a new time zone.
health news A Fish Diet May Lower Your Risk of Stroke Women who eat more than three servings of fish per week are less likely to experience a stroke, suggests a study published in the American Journal of Clinical Nutrition. Specifically, fish-lovers in Sweden were 16 percent less likely to experience a stroke over a 10-year-period, relative to women who ate fish less than once a week. When choosing fish to eat, it's best to look for fish that are rich in omega-3 fatty acids, found most abundantly in fatty fish like salmon, mackerel and albacore tuna. These fatty acids likely underlie the benefits of fish on stroke risk. Fatty acids may reduce the risk of stroke by reducing blood pressure and blood (fat) concentrations. This is not the first study to suggest that people who eat more fish have a lower risk of stroke, and experts already
recommend a fishy diet to reduce the risk of cardiovascular problems. A 2010 study showed that middle-aged and older men who eat fish every day are less likely than infrequent fish eaters to develop a suite of risk factors for heart disease, diabetes and stroke. The study speculates that certain nutrients in fish, such as fatty acids and vitamin D, might explain its apparent benefits. The Swedish study cannot prove cause and effect for high fish consumption and lowered stroke risk, however. For instance, fish consumption could be a sign of a generally healthier lifestyle or some other mechanism at work.

Ginkgo biloba was thought to be helpful to protect oneself against AMD. It appears that Ginkgo may help repair pre-existing damage to the visual field in patients with normal tension glaucoma. This plant also seems to be helpful in restoring colour vision in patients with early diabetic retinopathy. Yet, there appears to be little evidence that it can be helpful in treating AMD. The same fact holds true for bilberry. While bilberry seems to improve retinal lesions from diabetic or hypertensive retinopathy, there is no evidence that it can help AMD. It has also been reported that DHA (Docosahexaenoic acid), a fatty acid found in fish oil, may help to prevent AMD. This was determined by a prospective study of dietary fat intake. The researchers found that the amount of all dietary fat intake was directly related to an increased risk of developing AMD. Upon looking deeper, they also found that those who ate fish, more that 4 times a week, had a 35% lower risk of developing AMD. DHA
is a fatty acid that is unique to fish, so it was thought that it was the fatty acid that was associated with the reduced risk. As I have mentioned before, results found in studies about dietary habits may not apply if you only take supplements. You may not get the benefits of DHA if you take a few capsules with a couple of burgers from your local fast food restaurant. Instead, the full benefit may only happen if you have the fish in place of red meat for dinner. Be good to your eyes and use them often. I think that we should all try something that many high school students have done, experience the world blindfolded for an afternoon. While it may not be an option for many of us, it could give us a greater appreciation for a bodily sense that we often take for granted. If you ask me, however, a much easier way to tell if you’re carrying too much fat around the middle is simply to pinch your midsection the next time you have your clothes off, and if what you grab between your fingers closely resembles Mick Jagger’s lips, trust me, you have excessive abdominal fat (unfortunately, in most people, excessive fat in the abdomen is not only on the outside, where it’s easy to pinch and spot but also accumulates – sometimes even in “thin” people - around the organs stuffed into the abdomen, and that kind of excessive fat is not detectable with just a pinch). Anyway, the bottom line is that that’s quite a list, so it should come as no wonder that some experts argue that, “hey, that’s not a real syndrome because all you’re doing when you cluster those things together is coming up with a shopping list of the major factors that increase the risk of developing heart disease, strokes, etc,” so it’s also very likely, many of these experts argue, that given how many risk factors you need to make up the so-called metabolic syndrome, you’re actually dealing with several different conditions rather than only one syndrome, and the idea, these non-believers conclude, is not to spend any energy worrying about the existence of a distinct metabolic syndrome, but rather to attack each of those risk factors. Whatever, eh. By which I mean, that this heated debate really doesn’t matter to anyone (besides, maybe) doctors with too much time on their hands, because the bottom line for people whose belts have gotten too tight and who have high blood pressure and who suffer from abnormal cholesterol levels is that if they don’t deal with all those factors pretty quickly, they are seriously raising their risk of developing – and dying from - all those aforementioned crummy health problems. So what to do? Well, given that you should start by doing everything in your power to prevent the complications from those risk factors, as always, preven-

tion starts with lifestyle adjustment, meaning the same-old, same-old of doing some (preferably lots) of exercise, and not just aerobic exercise but also some type of weights because a recent study showed that both types of exercise are essential for proper control of blood sugar levels. • trying to maintain a healthy weight eating a healthy diet (a recent study found that people who ate a lot of dark green and orange and yellow veggies had a significantly lower risk of dying prematurely compared to people who didn’t eat a lot of those types of veggies) • not smoking • drinking no more than moderate levels of alcohol • getting an adequate amount of sleep and although the experts advise an average of 7-8 hours a night, I would say that since none of us is really average, the best way to determine if you’re getting the sleep you need is by how tired you feel the next day, and • trying to control your stress levels (yes, yes, I know, I know, much easier said than done). If lifestyle adjustment is not enough, and sadly it won’t be for the overwhelming majority of people with those risk factors, then you have to turn to medical treatment, which nearly always means a combination of drugs for each of those problems, and unfortunately, this can be a tangled mess since, for example, there are over 30 drugs for blood pressure treatment alone, so the best thing to do if you have to go the medication route is to read, read, read, so that you know as much as you can about what you’re taking and why you’re taking it, and also to make good friends – really good friends - with your pharmacist, because all those medications are powerful biological agents and you want to know as much as you can about the potential drug interactions. Maximizing Your Energy The alarm goes off and you sleepwalk your way over to the coffee maker so you can rev up enough to get yourself organized and out the door. Who has time for breakfast these days? You’ll grab another coffee and a donut on the way to work and count yourself lucky to score a quick sandwich at lunchtime. Before you know it, the clock says three and you have little left to give. If it were possible, you would curl up under your desk and take a nap but you still have at least 2 more hours of work before you pick up the kids at the sitter, make dinner and then muster up the energy to host your weekly book club meeting. Okay, well maybe it is a poker game with the boys but you are now rummaging through your desk for a chocolate bar to go with that double Americano you just ordered. You sit back and wait for the buzz to kick in and all is right with the world for another hour at least. This is not sustainable energy. While the above scenario is a slight exaggeration, it is not too far from the truth for many people. You only have to look at the coffee culture that has sprung up over the past few decades to realize that people have formed quite a dependency on quick energy fixes. We max out our schedules and don’t take the time to nourish ourselves properly so we can actually keep up. The trouble with this practice is that quick fixes like sugar and caffeine are always followed by a blood sugar crash. When this cycle becomes a way of life, it stresses the very systems that are in place to keep us healthy and alert. Here are some tips for maximizing your energy the healthy way: 1. Always start the day with a nutritious breakfast. Slow cooked oatmeal or whole grains are complex carbohydrates that are rich in fibre and release energy slowly into the bloodstream. Add some fruit, nuts, and eggs or yogurt for a balanced meal. If you really aren’t a breakfast person throw some protein powder, fruit, flaxseed and yogurt or juice into the blender and you have a breakfast you can run with. 2. Eat smaller meals more often and don’t skip meals. Carry healthy snacks with you for a mid morning and mid after-

noon snack. Make sure to include protein and healthy fats with your snacks and meals. [A few almonds, a piece of low fat cheese and some grapes can be an ideal snack.] While complex carbs are the preferred main energy source, protein and fat also provide sustainable energy that burns at a slower rate. 3. Don’t depend on caffeine! Be very careful when reaching for a product that is promoted as an energy drink. If you read the labels you will see that caffeine and sugar are often the main ingredients. The exceptions to this rule are the many wonderful greens drink powders on the market. These drinks are full of super foods and micronutrients that promote health, alkalize and energise the body. 4. Drink water. Even minor dehydration is a major energy sap. Know that caffeine is a diuretic and can contribute to dehydration. 5. Energise with exercise. Nothing better than a bloodstream full of fresh oxygen! Get outdoors as often as you can, walk the dog, dance, swim or ride your bike. 6. While optimal sleep hours vary from person to person getting a good night’s sleep is an obvious recommendation here. On average we need about 8 hours, some people need more, others can function well on slightly less. Consistency is helpful in establishing a sleep schedule. 7. Sit for a few minutes and breathe consciously. Take in a full deep breath; expand the belly, the ribcage and chest. Exhale and repeat a number of times and feel your energy stores increase. We all have lifestyle habits that drain our energy. Remember that whenever possible, stay away from junk food and high sugar foods. Use alcohol, coffee and stimulants in moderation. Choose whole foods that are fresh and full of vitality over canned or processed foods; eat more living foods over dead foods. And don’t forget to make some time each and every day for those people in your life that you love and activities that bring you joy! Good Health to You!
