ENLARGED PROSTATE I spend my life reading medical stories (yes, that really doesn’t make for much of a life, but hey, it’s the only one I have) so every day I discover dozens of new findings, far too many of which proclaim, to my great chagrin, to be “breakthroughs” and “amazing advances” but most of which draw nothing more from me than a yawn and a “Well, really, is that all there is to that?”, a remark, which, by the way, I’ve learned from my wife who uses it often, albeit in other contexts. However, I must say that when I saw a recent medical news headline, I immediately whooped “Woo Hoo” and channeled George Costanza with a “Bring it on, baby” as I raced downstairs to celebrate with a big glass of my favourite Chateau Rotgut. So what news made me jump and shout, you ask? This headline: “New Procedure May Shrink Enlarged Prostate Without Surgery”, a story about a study presented at the recent Society of Interventional Radiology meeting in New Orleans. Now, I realize that many readers, especially the women, of course, will not be nearly as overwhelmed with news about a new prostate procedure as I was, but every male reading this should, I think, whoop the way I did, because the great majority of us guys will eventually come to rue our prostates, and new non-surgical prostate interventions should be a source of much glee. But first, let’s start with this: why is the prostate

such a bane to aging men? Because for reasons that can only be speculated (my own guess is that it’s just another of God’s many jokes-gone-bad), the prostate has ended up in an odd location (remember that in much of life, location, location, location is all that matters) in which it surrounds the urethra, the tube through which urine passes from the bladder to wherever a man deposits it, which means that when the prostate grows or swells for any reason at all, it squeezes the urethra. And alas, the prostate does grow steadily with age, so that by our sixties, roughly 50 % of men already have some symptoms (see below) attributable to what doctors call benign prostatic hyperplasia or BPH, which is a good time to note that BPH is a “benign” condition and not a malignant swelling from prostate cancer, which is a different problem entirely. The rate of BPH rises relentlessly with age, so that the experts claim that if a man lives long enough, he has virtually a 100 % chance of ending up with an enlarged prostate, although not every enlarged prostate produces symptoms, thank God. The most common symptoms from BPH include: • Frequency: having to pee often because the bladder never empties fully • Urgency: the feeling that you need to go again soon after you’ve already gone • Leaking, dribbling, weak flow, and incontinence (I don’t think those need any extra description) Continued On Page 11

FLU VACCINATIONS Every year we hear the call go out to 'get your flu shot.’ What does this really mean? Most of us know that the flu (influenza) is a yearly virus that can make us feel miserable. How does this world traveling virus turn into a yearly vaccine? How is this vaccine made? How does it protect me from getting the yearly flu? This shall all be explained in this month’s article . Who makes this yearly vaccine? The answer is also WHO; the World Health Organization actually. Each year, laboratories all over the world track which kinds of influenza causing viruses are in their community. They get samples from people, who come into the hospitals with flu-like symptoms. Once a year, usually in February, these labs send their findings to the WHO. A vaccine choice is made based upon which influenza virus strains are the most prevalent and troublesome. Three or four virus strains are chosen to be included in that year’s vaccine and then production starts. What exactly is in a vaccine? The chosen vaccine strains are sent to manufacturing plants all over the world. Our vaccines are made in North America. Viruses are injected into chicken eggs and allowed to replicate (grow). Once the viruses have had sufficient time to replicate, they are harvested from the eggs. Various stages of purification remove the viruses from the egg fluid. into pieces for a few good reasons. The first is to ensure that they are not likely to actually give you the flu. The second reason is that you don't need the whole virus to vaccinate the body. Your immune system fights off bacteria and viruses by first identifying the unique proteins of these foreign invaders. These proteins are collectively called antigens. Vaccines are concentrated mixtures of these antigens, which teach the immune system to fight off the real viruses easily. These split virus, antigen vaccines are then packaged and shipped to pharmacies and health organizations. Unfortunately, every so often, viruses change and so do their specific antigens. This is also why new vaccines have to be made each year. When does the flu season start? As I mentioned above, the yearly production of influenza vaccine starts in February. Community pharmacies and health organizations place their vaccine orders in early July with an expected delivery date of late September. We usually recommend that people wait to get their vaccine until late October or early November. This is because the vaccine offers protection for about six months. Vaccination in October will provide coverage until the flu season is over in March. Generally, the peak of flu season occurs in January and February. For most people, catching the yearly flu is only a minor inconvenience. They get sick for a few days, spend some time in bed and things are fine. However, for people with chronic medical conditions (asthma, diabetes, heart disease), the elderly and those with compromised immune systems, catching the flu can be very dangerous. This is why these individuals, their caregivers and friends should get a yearly flu vaccine. To explain why vaccination is a good idea, let's learn about the principle of herd immunity. Essentially, this means that if a significant amount of those around you are vaccinated or immune to a disease, you are most likely protected. Some businesses offer free vaccines in order to keep their employees healthy. Can

you spare the time to spend three to four days in bed? While you are with your Peoples Pharmacist getting a flu shot, ask about other vaccines they can provide. One that my patients have been asking about is the shingles vaccine, Zostavax. It is thought that the virus that causes chickenpox (the varicella-zoster virus) is also responsible for shingles. After your body successfully fights off chickenpox, some of the virus travels up the nerve fibers and becomes dormant for many years. Later, the virus reactivates and travels down the nerve fibers to the skin surface.This reactivation is responsible for causing a painful rash, which could last for months or even years. • Hesitancy, or difficulty starting to pee and taking longer to do the job (you could cook an entire meal – and eat it, too - during the time some of us geezers spend at a urinal) • Dysuria: pain during urination • Nocturia: the need to pee at night, which is associated with several extra health risks, especially a greater risk of falling, and poorer sleep (no kidding!). Those are the main symptoms of BPH, but those problems can also be caused by prostate cancer, so any new such symptom should always be checked by a doctor. Now back to BPH. As well as those mostly irritating symptoms, BPH can also lead to urinary obstruction, which happens when an enlarged prostate completely shuts off the ability to urinate, and alas, the way to open a blocked bladder is to insert a catheter along the relative highway of a male urethra, which is not only uncomfortable (no kidding!), but a procedure that also carries a risk of traumatic injury and/or infection. Anyway, here’s the bottom line (so to speak): most men will get symptoms from BPH, and that will require some kind of therapy. Now I wish I could say that there were lifestyle adjustments to either delay prostate growth or improve the symptoms, but unfortunately, we just don’t know of specific lifestyle changes that make any difference. (A huge question, of course, has always revolved – for obvious reasons – around whether ejaculation makes BPH better or worse, but the jury is still out on that: some studies have found that more frequent ejaculation may be helpful, just as many others have found the opposite to be true, so I guess you have to decide for yourself which side you want to believe). One “natural” product for BPH that has received much publicity over the years is saw palmetto, and while there doesn’t seem to be much risk in taking it, in most good studies, saw palmetto has proven no better than placebo, although that said, it must be added that lots of men with BPH respond beautifully to placebos, so hey, if you believe in one, go for it,

I say. Among the several oft-prescribed drugs for BPH, it’s important to note that they all have important potential side effects, so if you choose drug treatment, speak to your doctor and especially your pharmacist about what you can expect from those products. You should also be aware that several OTC meds – anti-histamines, decongestants, and allergy meds – can make BPH symptoms worse, so if you’re an aging male, always check with your pharmacist before starting on a new drug, which is actually a good rule-of-thumb for everyone to follow, not just aging geezers. A common option for BPH relief is some form of surgery, and there’s a smorgasbord of options with acronyms like TUNA and TUMT to choose among depending on whether you opt for needles or freezing or lasers or even micro-waving. All these procedures are effective but they also all carry risks especially for that terrible “I” trio of infection, incontinence, and impotence, which is why that new study I mentioned earlier might eventually bring a lot of joy to a lot of guys. In this very small study (18 men), surgeons evaluated a “nonsurgical” option called prostatic artery embolization (PAE) in which they threaded a catheter from a leg artery into the major artery for the prostate, which they then plugged with some beads, and blocking the prostate’s blood supply resulted in significant shrinking of the gland, the researchers claim. This is a preliminary study, of course, so clearly, these results need to be duplicated in a much larger study with a long follow-up period, but if those results do pan out, speaking as a prostate owner who really never wants anyone to insert anything into a location in which they’re not wanted, should I ever need treatment for BPH, for me at any rate this new procedure sounds like it might just be akin to Neo in the Matrix, namely, The One. MY TOP FIVE ESSENTIAL SUPPLEMENTS Preventative health has always been a passion for me. My relationship with healthy nutrition began in the 1970’s when health food stores were the anomaly and the processed food industry was fast gaining momentum. Over the years, I have watched the health food industry evolve and expand to the point where most supermarkets and drug stores now have a well-stocked supplement area. With so much access, the consumer can often feel overwhelmed as to just which supplements are right for them. Working with a qualified health consultant can help hone in on individual needs, but there are some supplements that I feel most people can benefit from. Throughout my years on this planet, my own relationship with supplements has gone through many transitions. There were times when I practically needed a suitcase to haul around my daily regime and other times when I would go long periods without taking any. From the perspective of where I sit now, I tend toward taking some basic supplements as “health insurance” and include other specialty supplements as needed on a therapeutic basis only. These are my top five essential supplements: 1. If there was only one supplement on my list, a high quality daily multi vitamin would be it. Optimal health is dependent on a wide variety of vitamins and minerals working synergistically; sometimes deficiency of even one trace mineral can be enough to affect our well-being. A good quality multi will have a high absorption rate and often be gender, age and activity specific. It only makes sense that an active male teen would have different requirements than a menopausal female. 2. Omega 3’s come next on my list. These essential fatty acids are often out of balance in our mainstream diets and their influence is needed to maintain the integrity of every cell in our body. Omega 3’s, EPA and DHA, are found in fish, shellfish and algae; these are the most available forms of omega 3’s. LNA is precursor to EPA and DHA; flaxseed and walnuts are good sources of LNA. When it comes to antioxidants there are a wide variety to choose from; whenever possible, I like to derive my nutrients from a number of different sources, so finding an antioxidant blend is my preference. Look for a formula that may include a combination of the popular basics - vitamin A, C, E, lutein, lycopene, selenium and beta-carotene. The list of antioxidant superfoods continues to expand but you may also run across resveratrol, coenzyme )10, ubiquinol, alpha lipoic acid, astaxanthin, grapeseed oil, and a few others in the antioxidant aisle. 4. Vitamin D may be plentiful in tropical climates where the sun shines more than it rains, but it is a common deficiency in our northern climate. Adequate levels of vitamin D can play a role in cancer prevention, maintaining bone density and heart health; vitamin D deficiency has also been linked to depression and weight gain. Because vitamin D is a fat-soluble vitamin, there is a fine line between enough and too much. Work with your MD to see what your optimal vitamin D levels should be. 5. Last on my list, but not of less importance are probiotics. Probiotics are healthy live bacteria that keep the ecosystem of our digestive tract in balance. They aid in supporting optimal digestion and assimilation of nutrients; they also help support our immune system and defend us against unbeneficial microorganisms that can make us sick. I have read that over 80% of degenerative disease begins in our digestive tract; probiotics help ensure good health by supporting optimal digestion. Now before you rush to the store and stock up on a brand new supplement regime, I suggest you sit for a minute and check in with your body. Rather than add a whole bunch of stuff all at once, start with one new supplement and check in daily to see if you notice any positive or negative influences. Is there an improvement in body mind or spirit? Sometimes it is very subtle. When you are ready, try adding another one. Keep in mind that positive benefits may not always be immediately measurable but if something feels off, then consult your health care practitioner to see if it is a normal reaction. Wild Mushroom Risotto Cakes A creamy mushroom and rice cake with a cheesy surprise. This dish is a bit time consuming to prepare, but it is definitely worth the effort. 6 cups (1.5 L) chicken or vegetable stock or water 3 Tbsp (45 mL) butter, divided 1 cup (250 mL) chanterelle or oyster mushroom, rough chop 2 shallots, diced 2 cloves garlic, diced 1 ⁄2 tsp (2 mL) chopped fresh rosemary 1 1/2 cups (375 mL) arborio rice 1 Tbsp (15 mL) white wine vinegar 2 tsp (10 mL) lemon juice 1 ⁄2 tsp (2 mL) chopped fresh thyme 1 ⁄2 tsp (2 mL) salt 1 ⁄2 cup (125 mL) grated Swiss cheese 2 Tbsp (30 mL) chopped fresh parsley 1 tsp (5 mL) grainy Dijon mustard

Wild Mushroom Risotto Cakes

Bring stock to a boil in a large pot. Melt 2 Tbsp (30 mL) butter in a large heavy bottomed pot over medium heat, then add mushrooms and cook until they begin to brown lightly. Mix in shallots, garlic and rosemary, sauté lightly until shallots begin to sweat, about 2 minutes. Stir in rice and cook for an additional minute. Add 1 ladle of stock to the rice mixture and stir vigorously. Once stock is absorbed, add another ladle. Repeat this process until all stock has been used and rice is tender, adding more hot tap water if necessary. Fold in vinegar, lemon juice, thyme, salt, Swiss cheese, parsley and mustard. Pour risotto on a large tray or sheet and place in refrigerator until completely cool. Preheat oven to high broil. Use a cookie cutter to shape cooled risotto into a 3 inch (7.5 cm) cake and punch a cube of Cheddar into centre. Repeat with remaining rice. Line a large baking sheet with parchment paper and grease lightly with remaining 1 Tbsp (15 mL) butter. Place cakes on tray and broil on middle rack in oven until cakes turn a golden brown, about 15 minutes. Flip cakes and repeat to brown bottom. Allow to cool for 4 minutes before serving. Tip: When cooking risotto, keep a close eye on your rice and remember to keep stirring throughout the whole process, scraping down the sides and bottom of the pot. If neglected, your risotto will not have the creamy texture it should, and it may burn to the bottom of the pan. 1 serving: 410 Calories; 24 g Total Fat (7 g Mono, 1.5 g Poly, 12 g Sat); 280 mg Cholesterol; 27 g Carbohydrate; 3 g Fibre; 28 g Protein; 760 mg Sodium
