BLOOD CLOTS Deep Venous Thrombosis 路 Thrombophlebitis 路 Phlebitis

Blood clots are a collection of sticky blood cells that form when a blood vessel is damaged. The body creates blood clots as a normal response to blood vessel damage. The main job of a blood clot is to seal the leak in a damaged blood vessel. This prevents the blood from leaking out and protects the person from bleeding. Clots (or thrombi) that block the arteries and prevent flow of blood and oxygen to an organ can lead to areas of tissue damage (infarcts). When blood clots break (called an embolism) away from the area they're meant to protect, they can endanger other organs. Clots that block blood flow are the main culprits in most heart attacks and strokes. They can also damage other organs: - When a blood clot (thrombus) forms in one or more arteries that supply blood to the heart, it blocks the blood flow to a part of the heart muscle, reducing or completely cutting off the oxygen supply to the cells in that area. As a result, the part of the heart muscle that is deprived of oxygen dies, and a heart attack occurs. - Clots that block the flow of oxygen to the brain are the primary cause of strokes. - Clots that form in the eye may cause sudden blindness. Presence of an obstructing blood clot (thrombus) is referred to as thrombosis. Thrombosis in a vein is almost always associated with phlebitis (inflammation of a vein). Thrombophlebitis is an inflammation of a vein in the area where a blood clot has formed. Thrombophlebitis is classified as either superficial or deep. In other words, thrombosis can affect either superficial (surface) or deep (below the surface) veins, causing thrombophlebitis. Superficial thrombophlebitis occurs when a blood clot affects veins near the skin surface, or superficial veins. Deep venous thrombosis (DVT) occurs when a blood clot affects deeper, larger veins, such as those in the lower legs and thighs. DVT is more worrisome than superficial thrombophlebitis. These clots can break away (called emboli) from a blood vessel and cause a pulmonary embolism if they travel to the lung. (For more information on pulmonary embolism, see the section "Symptoms and Complications.") DVT is more common for people over 40 years of age. Causes Ischemic strokes (strokes caused by blood clots blocking an artery that supplies the blood to the brain) can be due to blood clots

that have formed in the heart as a result of rhythm disorders known as atrial fibrillation and atrial flutter. Atrial fibrillation is a type of an irregular heartbeat (arrhythmia) that triggers a rapid, quivering beat in the upper chambers of the heart (the atria). This irregular pumping may cause some blood to remain in the heart chamber and form clots, which can then travel to the brain. Emboli can also form at the site of artificial heart valves, after a heart attack, or as a result of heart valve disorder or heart failure. Cardiogenic embolism refers to clots that form inside the heart and travel to the brain. A stroke may occur in someone who has suffered a heart attack. A heart that has been damaged by a heart attack doesn't pump blood properly, which can cause formation of a blood clot that travels to the brain. The presence of artificial replacement heart valves can also lead to blood clot formation. By taking blood-thinning medications, people with artificial heart valves help to prevent clots from forming. Some blood clots may form in a narrowed artery as a result of atherosclerosis, commonly known as hardening of the arteries. In this case, the arterial walls slowly thicken, harden, and narrow over time until blood flow is reduced. As a result, these arteries become vulnerable to injury. If they tear, a blood clot may form, completely blocking the already narrowed artery and shutting off oxygen to part of the heart or brain. Blood clotting problems and rare blood disorders also cause blood clots to form, but the cause of blood clots is not always known. For some women, the use of oral contraceptives (birth control pills) may increase the risk blood clot formation. The risk is higher for women over 35 years of age who smoke or have a history of previous blood clots. A variety of things can cause inflammation of a superficial vein. One common cause is due to trauma or injury, for example from solutions or medications given intravenously (into a vein) in hospitals. Piercing the vein to give the solution or medication can cause irritation. Any trauma to a vein (such as an injury from a car accident) will trigger inflammation in the area, leading to pain, discomfort, redness, and swelling. forms in the inflamed or injured area of the vein. Superficial thrombophlebitis is an uncomfortable condition but rarely causes serious problems. Sometimes, thrombophlebitis is caused by a bacterial infection in the vein. The usual culprit is a bacteria called Staphylococcus, commonly found on the skin. In certain cases, thrombophlebitis develops without an obvious reason. It may develop in the leg veins of pregnant women, in people with varicose veins, and in some people with cancer in the abdomen (particularly the pancreas). Women over the age of 35 years who smoke and take oral contraceptives (birth control pills) are at a higher risk of developing blood clots. DVT occurs when blood clots form in the deep veins of the legs or pelvis, and is often caused by: - prolonged sitting or bed rest - surgery or trauma (especially hip surgery, gynecological surgery, heart surgery) - medications such as estrogen, and birth control pills with higher levels of estrogen - injury to the leg or immobilization (such as casting after a broken bone) Certain inherited conditions can make DVT more likely to occur. Blood flow in the veins depends on contraction of surrounding muscles, and with inactivity, such as extended bed rest, the blood starts to collect and blood clots can easily form. Symptoms and Complications Blood clots that result in a heart attack may cause chest pain (angina) that usually starts in the centre of the chest and moves to the jaw, the back, the left arm, or occasionally the right arm. Less commonly, the pain may be felt in the abdomen. Heart attack pain is usually severe but not always. Some people have "silent heart attacks" without any symptoms. Some people have reported feeling a sense of impending doom as a heart attack comes on. There's tightness and often a pounding in the chest. The heart may speed up and beat irregularly. Shortness of breath or difficulty breathing, nausea or vomiting, fainting, or collapse may also occur. Women may experience slightly different heart attack symptoms than men. Women are more likely than men to have nausea, and less likely to have sweating as a symptom of a heart attack. Women may not feel the chest pain as distinctly as men. The most common places for women's chest pain to spread to are the neck, jaw, or back. Blood clots that result in a stroke usually cause symptoms on the opposite side of the body from where they are causing the blockage in the brain. This may result in loss of feeling on one side of the

face, arm or leg, or blindness. If the left side of the brain is affected, speech problems can occur. Affected people may be unable to speak or to understand spoken words. Other symptoms of stroke include confusion, a severe headache, or sudden loss of coordination or balance. Signs of stroke should not be ignored, however brief or seemingly insignificant. Even if symptoms only last a few minutes and then vanish completely, it is important to seek medical attention right away. Inflammation in superficial or surface veins (such as those used to insert intravenous (IV) lines in the arm or to draw blood) produces pain and discomfort but it usually isn't considered serious. Blood clots that form in superficial veins rarely break loose and travel in the blood to cause blockage (thromboembolism) and complications in organs such as the lungs. In the case of DVT, a blood clot in the leg may cause pain, swelling, redness, and increased warmth. The leg may ache when standing. This usually occurs in one leg only. While many people with DVT have no signs or symptoms, the classic symptoms are: - firm swelling - pain or tenderness over a vein - sharp pain when the foot is flexed upwards - redness - warm sensation over the affected area - dull, aching tightness in the calf, especially with walking - dilation (widening) of the surface veins of the leg DVT can lead to serious complications. A blood clot that formed in deeper and larger veins, such as those of the legs, abdomen, and pelvis, can break away and become a travelling blood clot, or embolus. The embolus can travel and lodge in the lung, a condition called pulmonary embolism. Because a clot in a deep vein may not cause symptoms early on, the first sign may occur when the clot has broken loose and travelled to the lung. Symptoms of a pulmonary (lung) embolism are breathlessness, chest pain, and bloody sputum. If you have any of these symptoms, get emergency medical care right away. Making the Diagnosis Tests to check for presence of blood clots may include: - computed tomography (CT or CAT scan; a special technique that uses a computer to combine many 3-ray images into a detailed image of an area body that is 100 times more clear than a regular 3-ray) - magnetic resonance imaging (MRI) - ultrasound studies of leg veins or the arteries of the head & neck - angiograms or venograms - an ultrasound of the heart (echocardiogram) Continued On Page 1

should carry an epinephrine injector pen (Epi-Pen, Allerject). They should know how to use this device, but what if the reaction is severe that they are unable to inject themselves? I recommend that everyone who has a friend or family member with an epinephrine pen should know how to use it. I have a few tips for using adrenaline pens. The first is to hold the device firmly in your hand and do not put your thumb on the end of the device. It is a natural thought to try and apply more pressure upon injection. However, if you put your thumb on the wrong end, you risk injecting your thumb. There is often a concern about whether the reaction is severe enough to use an injector. The current consensus is that if you are unsure, it is best to give the injection anyways. The allergic reaction could still be worsening. It is true that these pens are expensive, but what is the cost of potentially saving a life? Another great idea is to regularly check the expiry dates on your epinephrine injector pen, they generally expire every 16 months. If you do have an expired pen, bring it into your Peoples Pharmacist for a demonstration of how to use it. I always make my patients practice injection technique into a phone book. Please don't practice injecting an orange. These pens are powerful and oranges are small and soft. You might hurt yourself and waste a nutritious orange. It is important that you know how to use an epinephrine inject pen, please visit your Peoples Pharmacist for a demonstration. Hopefully I have not scared you too much. We should mention those reactions that fall in the middle, those that are troublesome but not life threatening. The classic examples of these are seasonal allergies. Most of us experience symptoms of nasal congestion, sneezing or watering eyes. As with all allergies, the exact cause might not be known. However with seasonal allergies, the time of year when you are affected can be predicted. Congestion might affect you in early spring, or perhaps you suffer more in the fall. Whatever time of year it is, take notes on your calendar as to when your allergies flare up. This will tell you a few important things; when your allergies hit, what has worked and what has not worked. It you wish to use natural supplements, many of these supplements work best if they are taken in advance. With this knowledge you can ensure you have the items on hand needed for symptom relief.

Focus On Sunscreen Helps Protect Against Skin Cancer Sunscreens help prevent the sun's ultraviolet (UV) radiation from reaching the skin. Two types of ultraviolet radiation, UVA and UVB, damage the skin and increase your risk of skin cancer. SPF – or Sun Protection Factor – is a measure of a sunscreen's ability to prevent UVB from damaging the skin. Choose a SPF of 30 or higher and a sunscreen that is water resistant. The most common mistake with sunscreen usage is that most people do not apply enough sunscreen onto their skin. Most adults need at least one ounce of sunscreen, about the amount you can hold in your palm, to fully cover all exposed areas of your body. It’s also recommended to reapply sunscreen at least every two hours to remain protected.

- electrocardiograms (ECGs) - Certain specialized blood tests may indicate if someone has had a recent heart attack. Superficial thrombophlebitis is usually diagnosed according to your symptoms. A doctor will take your medical history by asking about your symptoms and conducting a physical examination. In the case of thrombophlebitis, an ultrasound of the suspected veins may be done to confirm the diagnosis. Since the leg pain associated with DVT is very similar to muscle pain, your doctor might look for signs of swelling and enlargement of the calf due to swollen leg veins. A diagnosis of DVT is usually confirmed with a compression ultrasound. Compression ultrasound detects differences in echoes or sounds made by flowing blood, and can easily detect the presence of blood clots in deep veins. Treatment and Prevention Medications are usually used to stop progression of DVT and prevent the blood clot from worsening, breaking away, and moving to the lungs. If you think you may have DVT, seek medical attention right away. Blood-thinning medications such as warfarin* or heparin are usually recommended. These medications may be continued for several months after a blood clot has been diagnosed. Most people do not require admission to a hospital to treat DVT, and those with DVT can usually return to normal activities within 2 to 3 weeks. Your doctor may also recommend that you wear a compression support stocking on your leg to prevent DVT. Painkillers may be used to reduce the pain.

To relieve mild inflammation and discomfort, the affected area should be elevated and warm, moist packs applied for 15 to 20 minutes at a time throughout the day. For people with superficial thrombophlebitis, activities such as walking are recommended. If the inflammation and symptoms last longer than a day or two, or if symptoms become worse, see a doctor as soon as possible. In cases where the thrombophlebitis is due to an infection, treatment with antibiotics often takes care of the problem. In rare cases, when the antibiotics aren't enough to control the infection, surgical removal of the inflamed portion of the vein may be required. To help prevent DVT, avoid long periods of immobility such as those during long car trips or airplane flights. Try to walk around and stretch for a few minutes every hour or so. Elevate your legs above your heart level if possible, and if you have a history of blood clots, wear support stockings or socks. Prevention of blood clots is the best way to deal with the problems of heart disease and stroke. It is important to reduce or quit smoking and to control high blood pressure. High cholesterol levels also present a risk for blood clots and may be checked by your doctor. A healthy diet and regular exercise also help to reduce the risk of heart attack and stroke. *All medications have both common (generic) and brand names. Whenever I’m asked about allergies, I recommend several lifestyle changes before using a supplement. I feel the best basic help for any condition is to drink lots of water. Extra water will make any nasal/sinus secretions thinner and easier to clear. Saline nasal rinses are also found to be helpful in removing pollen and nasal secretions. There is an easy to use sinus rinse product called ‘Sinus Rinse’ by Neilmed. Ask your Peoples Pharmacist for more information. Washing one's hair before bed is also a good idea. This will remove any pollens that are trapped in the hair and prevent them from falling onto the pillow during the night. This way, you won't be sleeping with a lot of pollen close to your face and nose. I recommend changing the pillowcase every few days. Avoidance of certain allergenic foods will often help. The most common culprits are milk and other dairy, eggs, wheat, nuts and artificial dyes. Another idea is to watch the newspapers or weather channel for your local pollen count. This might help you plan your week in order to avoid days when your allergies might be at their worst. This may be especially helpful for people who exercise outdoors. Another tip, my wife mentioned to me, is the miracle of the Vicks inhaler and Tiger Balm. At night, when symptoms might be bad, place a LITTLE bit of Tiger Balm under the nose. The aromatic vapors will help to keep the nasal passages open so you can rest better. Also, keep a glass of water by your bedside, in case you wake up with a dry mouth in the middle of the night. This happens often with people who have allergies. Summer and food allergies can be troublesome and potentially dangerous for those affected. To learn how to treat summer and food allergies, visit your Peoples Pharmacist. They can help you choose the right anti-histamine or supplement to treat your symptoms. Don't forget to write down what products worked for your allergies in the past. If I had a nickel for every time someone has said that they have forgotten what they took last year, that worked really well, I wouldn’t have to write these articles anymore. I would still choose too, but I wouldn’t have to. Also don't forget to check the expiry dates on your adrenaline pen, and all your medications.

WITH YOUR BODY IMAGE

We live in a society where appearance is the platform by which self worth is measured. Media further objectifies the human body by using images of thin, athletic people to sell product; the overarching message is that a lean body is the gateway to health, happiness, love, wealth and popularity. Anyone who does not fit into the mainstream body image belongs in the “other” box. Never mind that few people belong to the “in” group without plenty of hard work or sacrifice and many of the images promoted by media are photo shopped beyond natural human form. This can set up an individual to center primarily on perceived flaws and live their life through internalized deficiency dialogues based on not measuring up. The story that goes along with that thinking is the “I will be happy when” story. True self-esteem is not based on a future outcome, it comes from the honouring and acceptance of our own unique individual attributes and working from that baseline. When it comes to body image and self-esteem we add further insult to the equation with social shaming. Weightism, is a culturally induced disdain for the plus sizes and a by-product of the social emphasis on thin. Though we cannot deny the health risks attached to obesity, not everyone was born to be a size zero. The stigma attached to being overweight contributes to a low self esteem loop that can actually exacerbate the problem it was hoping to resolve. Shame often drives people deeper into the shadows where denial and fear get in the way of forging a healthier relationship with their body and body image. Another sad truth is that our children are being programmed with this message as soon as they are introduced to popular media. Movies and cartoons are not kind to overweight individuals as they are often portrayed as villains, goofs, lazy and easy targets for public displays of humiliation. When did it become okay to discriminate against a group of people based solely on body shape? Is this really the society we want to create? I would much rather teach my child about the diversity of the human form and all of the wonderful ways to support and celebrate their own unique body.

If you find yourself standing in front of the mirror and seeing nothing but flaws, perhaps the following tips will help reset your body image barometer and boost self acceptance and self-esteem: 1. Beware of comparisons There is always going to be a short to a tall, a curly to a straight and a blue to a brown. We often only see the things that we would like to change about ourselves and forget to celebrate and accentuate our own best attributes. Acceptance is the key. Be the best you that you can be 2. Focus on that which you can change and let the other stuff go. You cannot magically wake up tomorrow and be a different person but you could join a gym and tone up your 5’ 3” frame or ask your hairdresser to work their own brand of magic on your hair. Dung out your closet and get rid of the “someday” clothes; buy clothes that not only feel good but look great 3. Treat your body as a temple and commit to your health. Poor health comes in all shapes and sizes. Starving yourself to be a few pounds lighter or binge eating your way to comfort are equally distressing. Choosing optimal health means that you start making food choices that nourish your body, mind and spirit instead of just feeding your appetite and insecurities. 4. Put an end to negative self talk. When the inner critic is speaking, ask yourself if what the critic is saying is actually true. We so often get bogged down with critical self talk that we actually start believing it. When you start to question it, you can actually begin to step away from self depreciation to self appreciation. Your body is your temple. It makes no sense to be at war with it. Be kind to your body and listen to what it is trying to tell you. Marilyn Monroe had it right when she said, “wanting to be someone else is a waste of the person you are.” The true key to peace and happiness lies not in appearance but acceptance. Lets work with what Mother Nature gave us rather than fight a battle that can never be won. Good Health to You


Western Portobello Burgers A portobello mushroom filled with scrambled eggs, smothered in melted cheese, topped with crispy lettuce on a toasted kaiser roll… what more could you want? Western Portobello Burgers

Arrange mushrooms, stem-side up, on microwave-safe plate. Microwave on high (100%) for about 3 minutes until softened (see Tip, page 133). Place stem-side up in greased 9 x 13 inch (23 x 33 cm) pan. Heat cooking oil in large frying pan on medium. Add green pepper and onion. Cook for about 8 minutes, stirring often, until pepper is softened. Whisk next 4 ingredients in medium bowl. Pour over onion mixture. Reduce heat to medium-low. Stir slowly with spatula while egg is starting to set, scraping sides and bottom of pan until almost cooked but still wet. Spoon egg mixture into mushrooms. Arrange cheese slices over egg mixture. Cover with greased foil. Bake in 350ºF (175ºC) oven for about 20 minutes until mushrooms are tender and cheese is melted. Serve mushrooms, topped with lettuce, in rolls. Makes 4 burgers. 1 mushroom burger: 350 Calories; 13 g Total Fat (2.5 g Mono, 1 g Poly, 6 g Sat); 160 mg Cholesterol; 39 g Carbohydrate; 6 g Fibre; 20 g Protein; 750 mg Sodium TIP: Because the gills can sometimes be bitter, make sure to remove them from the portobellos. Scrape out and discard the gills with a small spoon.
