IRRITABLE BOWEL SYNDROME
Irritable bowel syndrome (IBS) is a common disorder of the intestine, causing abdominal pain with constipation, diarrhea, or alternating periods of both. IBS is also known as spastic colon or spastic bowel (terms that have fallen out of favour now) and functional bowel disorder. It's sometimes mistaken for colitis, an inflammatory bowel disease that is actually a more serious condition involving damage to the colon. IBS doesn't do any damage to the colon. IBS affects about 20% of the population and is second only to the common cold as the most frequent cause of days lost to work and school. IBS symptoms can disappear for periods of time and then return. Unlike inflammatory bowel disease (ulcerative colitis and Crohn's disease), IBS brings no extra risk of cancer of the colon. However, if symptoms of IBS begin in people over age 50, it's best to rule out polyps and colon cancer by having a direct inspection of the lower bowel done by a doctor. Causes The cause of IBS isn't known, but it's currently thought to be due to the large and small intestines over - or under-sensing factors that may lead to abnormal bowel function. For people with IBS, some situations may trigger pain and discomfort: - emotional stress - eating - excessive gas - female hormones (for women with IBS, symptoms often worsen during their periods) - certain medications and food (e.g., alcohol, 6 People First Join us on

chocolate, coffee, dairy products, fructose, carbohydrates) - after a bout of gastroenteritis - an overgrowth of bacteria in the small intestine Symptoms and Complications Symptoms of IBS include: - abdominal pain and cramps, which may come in "on-again, off-again" bouts or as a continuous dull ache - constipation or diarrhea, or alternating periods of both - urgent need to have a bowel movement - excessive gas - bloating - changes in bowel habits - mucus in the stool Blood in the stool is never a symptom of IBS. People who have blood in their stool, constant pain, or who have a fever should see a doctor. For other possible causes of these symptoms, refer to our disease articles on Crohn's disease, ulcerative colitis, and hemorrhoids. Making the iagnosis A doctor will get a medical history and perform a physical exam, looking for "alarm" symptoms like fever, bleeding, or drastic weight loss that could be due to something more serious. 2-rays or lower gastrointestinal endoscopy (using a tube with a tiny camera on the end) may be used to look inside the colon, especially for older individuals. Sometimes, abdominal ultrasounds or 2-rays of the intestines are done. Continued On Page

Irritable Bowel Syndrome...Continued From Page 6
Treatment and Prevention People with IBS can usually manage symptoms by making adjustments to their lifestyle. Eating a healthy diet can lessen the symptoms. Some people find that following the diet recommended by Canada's Food Guide to be helpful. Dietary fibre (such as whole-grain breads and cereals, beans, fruits, and vegetables) prevents stools from drying out too much and helps to keep things moving regularly in the colon. Switching to a high-fibre diet might cause bloating and gas at first, but this usually goes away in a few weeks and can be reduced by making a gradual change to the amount of fibre consumed. It's also important to drink plenty of fluids, particularly water, to prevent or reduce constipation. Other people find that avoiding certain food triggers can lessen their symptoms. Eating smaller, more frequent meals can also help reduce cramping and diarrhea. Since stress can bring on symptoms of IBS, stress management is an important way to deal with this condition. Exercise and some kind of relaxation training (such as meditation) are often recommended. Your doctor might also suggest talking with a counsellor to learn how to cope better. If you
experience other symptoms such as anxiety, talk to your doctor on how to manage your condition. Medication for IBS is aimed at treating symptoms. Medications are available to slow down the movement of food through the digestive system and to control diarrhea. Laxatives are sometimes helpful for problem constipation, but people should not depend on them for regular bowel movements. Antidiarrhea medications (e.g., loperamide*) may be helpful for people who have mostly diarrhea as a symptom. Many other medications are available and your doctor can discuss them with you. Probiotics are bacteria that normally live in your intestines and are found in certain foods, so they are considered "good" bacteria. Some studies suggest that IBS may be due to an imbalance or disruption of the normal "good" bacteria that's present in the intestines. Probiotics may help the symptoms experienced by people with IBS by restoring this balance. However, further research into the use of probiotics in IBS needs to be done. Medication non-compliance accounts for approximately 25% of all hospital admissions among seniors Many people today are on more than one medication, and when you combine this with a busy and active life, it can lead to the confusion of properly taking your medications. Medication non-compliance is a serious concern and can be harmful to your health and wellness. Peoples Drug Mart offers a medication compliance program called “Med Manager.” The Med Manager is a medication compliance card that conveniently organizes your medications for a full week, and helps you easily identify what medications are to be taken at what time of the day. Be safe, take the guesswork out of your daily medications.

calcium and your body also uses it to make bone. The minerals zinc, copper and manganese also play a role in bone health. A good multi-vitamin has adequate amounts of all of these minerals, except magnesium. It is also important you get enough vitamin D. Your body needs vitamin D to absorb and use calcium. Some people ask me if they still need to take vitamin D, since the body can make its own supply from sunlight. I recommend taking an extra 1000- 2000IU of vitamin D a day just to be sure. You may also have read that vitamin K can help with osteoporosis. This is partially true. A special form of vitamin K (menaquinone) might be helpful for increasing bone mineral density. This is different from the form (phytonadione) found in some multivitamins, which might not be as effective. For now, just eat more fruits and green leafy vegetables; which, coincidentally, have been associated with a lower risk of developing osteoporosis. Some women may experience these troublesome symptoms of menopause: hot flashes, insomnia, mood swings and depression. Can herbs and supplements help? Yes and no. Some of the most promising products are soy, red clover and black cohosh. Soy and other legumes (peas, beans and red clover) contain substances called phytoestrogens. These natural plant-based estrogens act like estrogen in the body, only considerably weaker. In one study, women who took 50mg soy isoflavones/day reported a significant reduction in the number and severity of hot flushes after only six weeks. In another 6 month study, 120mg of soy isoflavones was as effective as 0.625mg of conjugated equine estrogens in reducing menopausal symptoms. Increasing the amount of soy in one’s diet has also been shown to help reduce the risk of heart disease. In one study, women who took soy protein containing 34 mg of phytoestrogens not only reported significant improvements in hot flashes, but they also showed declines in total cholesterol (6%), LDL (7%) and blood pressure (5mm Hg). The average North American diet provides about 3mg of phytoestrogens daily. Yet the average Asian diet provides about 40 to 80mg of phytoestrogens daily, most likely due to the larger consumption of soy products, such as tofu, tempeh, and soy milk. This is thought to be one of the reasons why Asian women, who eat a traditional soy rich diet, experience fewer menopausal side effects. Try to incorporate more tofu, soymilk, “meat-like” soy products like tofu wieners or deli slices, tofu-almond desserts and dry roasted soy nuts into your diet. I recommend unprocessed soy products over supplements, because soy foods also provide vitamins, minerals and fibre. Soy products are usually low in saturated fat and

Focus On Vitamin B12 Helps Keep Nerve & Blood Cells Healthy In addition to producing red blood cells and keeping nerves healthy, B12 helps make DNA, the genetic material in all cells. B12 also helps the body metabolize fats, proteins, and carbohydrates. In some people, especially those over age 50, the body does not produce sufficient “intrinsic factor,” a component of stomach acid required for absorbing vitamin B12. Likewise, vitamin B12 is poorly absorbed by people who suffer digestive disorders, such as Crohn’s or celiac disease. Certain acid reducing medications can interfere with the body's absorption or use of vitamin B12 and a supplement should be considered to maintain health and wellness. Talk to a Peoples Pharmacist about the health benefits of vitamin B12.

cholesterol. Regular dietary soy consumption is safe, but those taking high dose supplements should talk to their doctor or Peoples Pharmacist to see if these products are right for them. Another herbal product recommended to help with menopausal symptoms is black cohosh. This herb has been shown to reduce the symptoms of menopause such as: hot flashes, sweating, and headaches. It is not clear how black cohosh works. However, it is believed that it does not act like estrogen in the body. This product may take up to 4 to 6 weeks to fully work. Side effects are rare, with only the occasional stomach upset reported. Due to a lack of long-term safety studies, therapy is recommended to be no longer than six months at a time. Yet, black cohosh has had over 60 years of safe clinical use in Germany. The most common dosage for black cohosh is one or two tablets, twice a day, of a standardized extract, made to contain 1 mg of 27-deoxyacteine per tablet. There are many combination menopause formulas on the shelves, look for ones that contain standardized black cohosh as their main ingredient. Perhaps the most prominent health risk for menopausal women is heart disease. In fact cardiovascular disease (heart attacks and strokes) kills more women yearly than any other disease. So what can be done to help counteract the effects of both osteoporosis and heart disease? Regular weight-bearing exercise is the answer. A weight bearing exercise is one where the muscles must work against gravity. These exercises could include walking, stair climbing and dancing. While bicycling and swimming are excellent exercises to strengthen the cardiovascular system, they are not weight bearing and may not be helpful for preventing osteoporosis. Resistance exercises, including weight lifting or using elastic exercise bands, can also be helpful in preventing bone loss. Remember, it is best to talk to your Physician before starting any new exercise program. Menopause can be a very troubling and confusing time of life, especially considering all the treatment options that are available. There are both natural and pharmaceutical treatments available and sometimes no treatment is required. Talk to your Doctor or Peoples Pharmacist about which option is best for you. Your Peoples Pharmacist may also hold in-store clinics that can provide information about menopause, heart disease and osteoporosis. As always they best advice is to get regular exercise, eat well and take a multivitamin. Written By Ian Lloyd, Pharmacist & Chartered Herbalist

HEALTHY BONES
An adult skeleton is comprised of 206 bones. While the main job of the skeleton is to serve as an internal framework for our body, bones are far from static. Bone is living tissue and bone cells are constantly in flux in response to our physiology, diet, lifestyle and environment. In addition to their structural job, bones provide anchorage for tendons, ligaments and muscle, protect vital organs, house bone marrow for blood cell production and store important minerals like calcium. We reach our peak bone mass somewhere around age 30 and then we begin to make more withdrawals from the bone bank than deposits. However, a good strong foundation is less at risk than a weak one. It is true that gender and genetics may increase an individual’s risk of bone loss but with awareness and focus, much can be done to support healthy bones at every age. Our best overall health insurance begins with nutrition; bone health is no different. Choose to build menu plans around a wide variety of nutrient dense foods. From a nutritional standpoint, calcium is a bone-building superstar but it does not work alone. Calcium works best with the support of potassium, magnesium and vitamin D. Other nutritional supports for bones include phosphorus, vitamin K, vitamin C and many trace minerals such as zinc and boron. Dietary choices that support strong bones include dairy, canned fish with bones, dark leafy green vegetables, coldwater fish, nuts, seeds, fresh fruits and lean proteins. Maintaining healthy intestinal flora with the addition of probiotics and the ingestion of fermented foods increases absorption and assimilation of all nutrients needed for building strong bones. A high quality, broad-spectrum vitamin and mineral complex formulated towards bone health is advised for those who feel their diet needs support or who are in a higher risk category for bone loss. While the every day diet is the cornerstone of good health, some of the other good things in life should be enjoyed in moderation or avoided altogether. If you smoke P now is always a good time to quit. There are just too many risk factors associated with cigarettes and smokers also tend to have decreased bone density in comparison to non-smok-

ers. Pass on the saltshaker; salt leaches calcium from the bones as do too many caffeinated beverages and carbonated soft drinks. We know that a glass or two of wine now and then can be a health benefit, but heavy drinkers be aware that bone loss is a companion to your hangover. Too much alcohol can interfere with calcium absorption on a number of different levels. Moderation may take the fun out of happy hour but your bones will thank you for it later. Daily exercise is mandatory; use it or lose it is visually apparent when it comes to muscle tissue but it also applies to our bones. The human body is not designed to be sitting at a desk all day and channel surfing all night. If you think back to the lifestyle of the hunter-gatherers, they were actually burning calories in the search of consuming calories. The only calories some of us burn are when we push the grocery cart through the checkout line. Specifically, all weight bearing exercise stimulates the bone building process; join a gym, explore resistance training or get out for a daily run or jog. Consult a personal trainer to develop a personal full body routine that works for you. You don’t need fancy equipment to do squats, push-ups and jumping jacks; just do whatever it takes to inspire you towards regular weight bearing exercise. Start off slow with any new activity and gradually incorporate exercise into your bone healthy lifestyle. I might even go so far to suggest that one day you will actually begin to look forward to your workouts. Sometimes, despite our best efforts bone loss can happen for no apparent reason. Aging in itself is a time when we see more changes in our tissues and bone structure. These changes are not always welcome ones. Health challenges, some medicines, or hormonal fluctuations may contribute to some of the decline. For women over the age of 50, a drop in estrogen levels can affect bone density. For men, usually around age 70, lower testosterone levels can contribute to bone density loss. Disease processes, diet, lifestyle and environment can affect hormonal health; it is important to have regular check ups and dialogues with a trusted health care provider to make sure your bones will support you well into your golden years.
