CROHN’S & COLITIS

Crohn's disease is a type of inflammatory bowel disease (IBD) that can damage the lining of any part of the digestive system, including the mouth, esophagus, stomach, small intestine, large intestine, rectum, or anus. This inflammatory disease most often affects the small intestine, especially a portion known as the terminal ileum. More than 110,000 people in Canada are affected by Crohn's disease. Crohn's disease can affect people of any age, and it is usually diagnosed when people are between 15 to 25 years old or 45 to 55 years old. The disease can have a devastating impact on people's lives. Ulcerative colitis is a type of inflammatory bowel disease (IBD) that affects the lining of the colon (large intestine). Ulcerative colitis is known as an inflammatory disease because it causes inflammation, sores, bleeding, and scarring of the inner walls of the colon. The inflammation usually starts near the rectum and continues up the colon. The inflammation involves continuous areas of the wall rather than small patches. What are Crohn's disease and ulcerative colitis? Your digestive system helps you digest food and eliminate solid waste from your body. It includes 6 PEOPLES Health Join us on

your mouth, esophagus, stomach, small and large intestines, rectum, and anus. Figure 1 shows the different parts of the digestive system. "Crohn's and colitis," also known as inflammatory bowel disease (IBD), is a medical term that describes two separate diseases affecting the digestive system: • Crohn's disease • Ulcerative colitis Both Crohn's disease and ulcerative colitis damage the lining of the digestive system by causing inflammation, sores, bleeding, and scarring. This can lead to painful, embarrassing symptoms, such as abdominal pain and cramps and diarrhea (which may be bloody). Other symptoms can include loss of appetite, malnutrition, and weight loss. Crohn's disease and ulcerative colitis can also cause symptoms outside of the digestive system, such as fatigue, fever, mouth ulcers, skin rashes, eye inflammation, or joint pain. Crohn's disease and ulcerative colitis have many symptoms in common, but they are separate medical conditions. So what's the difference between Crohn's disease and ulcerative colitis? • Crohn's disease can affect any part of the digestive system, while ulcerative colitis affects only the Continued On Page 8

colon (large intestine). • The inflammation caused by Crohn's disease can penetrate deep into the walls of the bowel tissue. Ulcerative colitis tends to affect a single layer of the bowel wall (inner lining). • Crohn's disease inflammation tends to occur in patches throughout the digestive system, while ulcerative colitis inflammation tends to affect a continuous area of the colon, usually starting from the rectum and moving up through the colon. • Although Crohn's disease can be effectively managed with medications, it cannot be cured. Ulcerative colitis can be controlled with medications or can be cured surgically by removing the colon. • Both Crohn's disease and ulcerative colitis are lifelong conditions that can have a serious effect on your life. These conditions are unpredictable - people with Crohn's disease and/or ulcerative colitis have attacks (also called "flare-ups") followed by periods of "remission" when the symptoms go away. During a flare-up, you may be unable to work or do your usual activities because of debilitating symptoms. You may end up in the hospital or require surgery. All of these things can take a toll on you and your life.

Treating Crohn's disease: What to expect from your treatment Crohn's disease is unpredictable. People with Crohn's disease experience flare-ups of symptoms, also called attacks (or relapses if they happen more than once), followed by long periods of time, often weeks to years, when the symptoms go away, also called remissions. Flare-ups can cause you to frequently miss work, meals (which can have a negative impact on your nutrition), sleep, or time with your family and friends, and even end up in the hospital. Because the effects of Crohn's disease vary from person to person, treatment goals may also vary. Talk to your gastroenterologist about your personal treatment goals and creating a treatment plan. See your doctor regularly so you can track your progress. Treatment for Crohn's disease involves nutritional therapy (to prevent malnutrition that can occur), medications, and possibly surgery. In general, Crohn's disease treatment aims to: • bring your disease under control (relieve symptoms and reduce the frequency of flare-ups) • improve your quality of life and help you live a normal, active life Continued On Page 10

Medication non-compliance accounts for approximately 25% of all hospital admissions among seniors Many people today are on more than one medication, and when you combine this with a busy and active life, it can lead to the confusion of properly taking your medications. Medication non-compliance is a serious concern and can be harmful to your health and wellness. Peoples Drug Mart offers a medication compliance program called “Med Manager.” The Med Manager is a medication compliance card that conveniently organizes your medications for a full week, and helps you easily identify what medications are to be taken at what time of the day. Be safe, take the guesswork out of your daily medications.

• prevent and treat complications, such as malnutrition, osteoporosis (thin, brittle bones), and blood clots in the legs, since people with Crohn's disease may be at increased risk for these complications It's important to ask yourself, "Is my Crohn's disease as well controlled as it could be?" If your Crohn's disease is not well controlled, talk to your gastroenterologist about the treatment options available to you. Medications have a variety of roles in treating Crohn's disease. Depending on the medication, the role may be to: • relieve symptoms during a flare-up • keep the condition in remission • control the disease itself, not just the signs and symptoms Everyone's experience with Crohn's disease is slightly different. But you can take control by working with your doctor to find a treatment that's right for you. This can help relieve your symptoms and reduce the number of flare-ups, thus providing long periods of remission. Can I expect a cure for Ulcerative colitis? Ulcerative colitis can be cured by surgically removing the colon. However, surgery is not for everyone, and is usually reserved for severe cases. Surgery comes with risks, and after surgery you may have a pouch inside your body to collect waste and that needs to be emptied via a valve that leads to the outside of your body, or you may need to wear a bag outside the body to collect solid waste. What else can I expect from treatment? Because the effects of ulcerative colitis vary from person to person, treatment goals may also vary. Talk to your gastroenterologist about your personal treat-

aisles of your local Peoples Drug Mart or Peoples Pharmacy; so let your pharmacists help you choose the best product to treat your specific symptoms. One specialty that pharmacists are rediscovering is medication compounding. This has always been the job of the Pharmacist, but today the complexity and range of compounds available are staggering. If you have allergies to gluten or lactose, your compounding pharmacist can make you a product without these ingredients. Many pet medications can also be prepared, often with your pet's favourite flavours. If you doctor or specialist has a custom drug product they wish to try, odds are your compounding Peoples Pharmacist can make it. If you have any questions about your medications, the person to ask is your Peoples Pharmacist. If you would like a more in depth chat about your medications or health concerns, talk to your pharmacist about a medication review. During a medication review, your Peoples Pharmacist makes the time to answer all of your pressing health related questions. They will also ask questions about any vitamins, over the counter products or other supplements you might be taking. This is done to determine if these supplements might conflict with your current medications or conditions. In some cases, there might be a vitamin or supplement that could improve your health. Many times your Peoples Pharmacist can find solutions to medication issues that you and your doctor might not have even been aware of. If you have questions or concerns, ask your pharmacist about having a medication review. Something you might also not be aware of is that your local pharmacist might be able to refill your prescriptions without you having to visiting your doctor. In February 2008, the provincial government acknowledged that pharmacists play an important role in the managing of patients

Focus On Vitamin D Helps Prevent Major Disease Vitamin D, often referred to as the “sunshine vitamin," is a fat-soluble vitamin. Vitamin D is well known for the important role it plays in helping the body to absorb calcium as well as maintaining strong bones to prevent osteoporosis. New studies show that Vitamin D may also be effective in helping reduce the risk of certain cancers by almost 60 percent. Evidence suggests that the current Vitamin D recommendations — 200 to 600 IU per day, depending on age — are too low. Many researchers now advise increasing daily Vitamin D intake to 1,000 IU. Talk to a Peoples Pharmacist about the benefits of Vitamin D.

drug therapy. To ease some of the pressure on a busy health care system, pharmacists might be able to renew your existing prescriptions. This means that you can spend more time with your doctor discussing health care goals and less time with routine refill prescriptions. Pharmacists can not refill every medication that you are taking. If there is a long term medication that is running out and you are unable to meet with your doctor, ask your Peoples Pharmacist if they can help. They might be able to adapt your prescription and provide an extra refill. If you have any questions about vaccinations or medications needed for traveling, the best person to ask is your Peoples Pharmacist. They may have knowledge about the items you could need for a safe journey. In some cases, your local Pharmacy might hold travel medicine clinics. These clinics specialize in what medications and vaccinations you will need during your travels. Your community pharmacist can determine any health concerns you might encounter during your travels. Then they can work with your doctor to ensure you receive any vaccinations or precautionary medications you might need. Ask your Peoples Drug Mart Pharmacist for more information about the medications you might need on your travels. With a little planning, your well deserved vacation can be truly relaxing. I bet you did not know your local Peoples Pharmacist was capable of so many things. This only a small sampling of what happens in a Pharmacy. I left out the paragraphs about collaborating with your doctor to keep you healthy, maintaining drug inventories, planning in-store health clinics, blister packing medications and providing great customer service. If you have any questions about your health or medications, ask your community Peoples Pharmacist. The Pharmacist is in!

SLEEP WELL

Sleep deprivation is a quiet epidemic that intersects with sociology, psychology and physiology. A by product of modern living, sleep deficiency is a symptom of living in a fast paced society that values being able to squeak out every minute in a 24-hour cycle. We admire productivity and those who have the stamina to seemingly survive on less than 8 hours of sleep. Often, the sleep deprived will embrace caffeine and other stimulants, wearing their daily dose as a badge of honor. We rarely connect the dots when this strain of super human individual has a physical or psychological setback. Energy drinks and sleeping aids are top sellers in today’s market but these crutches are not cures. Sleep deprivation is both a symptom of a body being out of balance with the natural rhythm of life and a contributing risk factor for illness and disease. Studies suggest lack of sleep is more common for adolescents and those on the younger side of middle age but no single age group is immune. Show me a parent today who is not sleep deprived from trying to balance work, family and play. Maternity leave is the most humane thing our society has invented and I can’t emphasize enough to young parents the need to rest when you can during these months. Before you know it, the infant is an active toddler with a newfound sense of self and independence that ramps up around nap time and bedtime. Parental reserves are often drained by the end of the day but it is worth the effort to instil good sleep habits early in life, as the payoff later is enormous. Teenage years can be challenging from the parental perspective but we also have to embrace empathy for the developing teen. These individuals are growing up in a rapidly changing society that expects a lot from them. They have early school starts, homework, social activities, family obligations, work and screen time competing for their waking hours. Add into the mix the changing body chemistry and the physiological need for more sleep (not less) and it is no wonder that a minor set back can set the tone for a teenage apocalyptic outburst. By the time we reach adulthood, sleep deprivation has often become the norm. We are conditioned to dance the dance of Ogive to others before yourselfR and thus much of the

population is showing up to work foggy brained and half asleep. Is it any wonder that there is a coffee shop on every street corner? If any of this sounds familiar, perhaps it is time to switch to a slower gear and make optimal health a priority. The first step begins with awareness. Try some of these tips to work toward a better night’s slumber: 1. Kick caffeine and other stimulants to the curb (especially after lunch). They only give you a false sense of energy and in the long run they can elevate your stress hormones, which make it hard to fall asleep at night. Be aware that while alcohol can initially relax you, it interferes with the natural sleep cycle. Consume alcohol early in the evening and give your body a chance to process it before bedtime. 2. Plan a short 10 P 20 minute nap into your afternoon. Napping has been shown to improve performance on both physical and cognitive levels. If you find it hard to sleep during the day then spend the time with eyes closed in a meditative state. 3. Ease into a relaxation. Turn off all screens 2 hours before bedtime and relax into a hot bath or a good book. Do some gentle yoga. Set the tone with mellow music and turn the lights down low. See the evening as a transition to slumber rather than a continuation of the hectic pace of the day. 4. Moderate daily exercise has been shown to contribute to a sounder sleep. Some people find that exertion in the evening hours can be too stimulating and counter effective so find a time for exercise that works best for your psyche and schedule. As I ease into the lukewarm water of my elder years, I find great irony that the other side of the empty nest has come with its own set of sleep challenges. When the time finally came that I could actually sleep with both eyes closed, the hormonal flux of menopause had its own agenda. How I long to have the deep slumber of a newborn once again!
