MEDICATION REVIEW This is an area where your community Peoples Pharmacist has the most experience and expertise. Pharmacists are specialists about medication use and its safety. Take this opportunity to talk to your Peoples Pharmacist, one-on-one, about your medications. You would be surprised how much you can learn about your health after a chat with your Peoples Pharmacist. The Medication Review Service is a provincially funded service which gives Pharmacists the opportunity to better educate their patients. Peoples Pharmacists have always reviewed your medications, but this provides a more formal approach. If you a BC resident, with a valid BC Services card and take at least five prescription medications, you might be eligible for a funded medication review. If you don't meet these criteria, don't worry. Your Peoples Pharmacist still reviews all of your medications. What will happen during a medication review? Well, the name defines the service perfectly. Your Peoples Pharmacist will review all your prescription medications, including vitamins and supplements. are used for, the best time to take them, any potential side effects and possible long term complications. Depending upon your pharmacist's level of expertise, they may also discuss possible drug induced nutrient depletion. This side effect occurs when a prescription medication causes your body to lose valuable vitamins or minerals. For information about drug-induced nutrient depletion, read the January 2015 issue of Peoples Health magazine. You can find all previous issues at www.peoplesdrugmart.com. What should you bring to your medication review? Naturally you should bring a list of all the over the counter items and supplements that you take. This should include vitamins and herbal preparations. Your Peoples Pharmacist will have a complete list of the prescription medications that you take. It is very helpful for your Pharmacist to know what supplements you take on a regular basis. Sometimes vitamins or supplements can interact with prescription medications or conflict with medical conditions. Your Peoples Pharmacist can also advise you whether these supplements are of value to you. It will to helpful to bring a list of questions that you might have about your medContinued On Page 11

CONSTIPATION

Constipation is when you have bowel movements (stools) that are hard to pass or are less often than what is usual for you. Being constipated means your bowel movements are difficult or happen less often than normal. Most people have it at some point in their life, and it's usually not serious. Still, you'll feel much better when your system is back to normal. Some people are overly concerned with the frequency of their bowel movements, because they have been taught that a healthy person has a bowel movement every day. This is not true. Most people pass stools anywhere from 3 times a day to 3 times a week. If your stools are soft and pass easily, you are not constipated. What causes constipation? There are many causes of constipation, including: • not drinking enough fluids • a low fibre diet • low physical activity • ignoring the urge when you have to go • stress • pregnancy • overuse of laxatives • a change in regular routine or travelling • some medications such as calcium, antacids with aluminum or calcium, antidepressants, antihistamines, narcotics, antispasmodics, diuretics, tranquilizers, some heart medications; some supplements such as iron. • health conditions such as irritable bowel 6 PEOPLES Health Join us on

syndrome, diabetes, colorectal cancer, eating disorders, underactive thyroid, Parkinson’s disease, multiple sclerosis, celiac disease, and depression. Ways to Ease and Prevent Constipation • Get some exercise every day. Try to do moderate to vigorous activity at least 2½ hours a week. It's fine to be active in blocks of 10 minutes or more throughout your day and week. • Aim to drink at least two litres of fluid per day. You will pass much of the fluid as urine but some is passed out in the gut and softens the stools. Note that alcoholic drinks can cause dehydration and may cause constipation. • Take a fibre supplement, such as Biomedic pysllium fibre, every day if needed. • Schedule time each day for a bowel movement. Having a daily routine may help. • Do not strain when you are having a bowel movement, its best to take your time. Eat Foods That Contain Plenty Of Fibre Fibre (roughage) is the part of plant food that is not digested. It stays in your gut and is passed in the stools. Fibre adds bulk and some softness to the stools. High-fibre foods include the following: • Fruit and vegetables. Try to eat at least five portions of a variety of fruit and vegetables daily. One portion is: one large fruit such as an apple, orange, banana, or a large slice of melon; OR two smaller Continued On Page 8

fruits such as plums, etc; OR one cup of small fruits such as strawberries, raspberries, grapes, etc. • Wholemeal or whole-wheat bread and flour. • Wholegrain breakfast cereals. A simple change of eating a fibre rich breakfast cereal can make a big difference to the amount of fibre you eat each day. • Brown rice, and wholemeal pasta. Although the effects of a high-fibre diet may be seen in a few days, it may take as long as three tofour weeks. You may find that if you eat more fibre (or take fibre supplements), you may have some bloating and gas at first. This is often temporary. As your gut becomes used to extra fibre. If you are not used to a high-fibre diet, it is best to gradually increase the amount of daily fibre . What if whole-grain fiber and fruits don't help constipation? Then try foods that contain psyllium seed husk, bran, and methylcellulose. These natural products increase stool weight and have a laxative effect. cause constipation. Fiber must have water in order to sweep the colon and move the stool out of your body. Laxatives and Constipation Bulk Formimg Bulk formimg laxatives are known as fibre supplements. These increase the bulk of your stools in a similar way to fibre. They can have some effect within 12-24 hours but their full effect may take several days to develop. Unprocessed bran is a inexpensive fibre supplement. If you take bran, it is best to build up the amount gradually. Start with two teaspoons a day, and double the amount every five days until you reach about about 1-3 tablespoons per day. Other fibre supplements include ispaghula (psyllium), methylcellulose, sterculia, wheat dextrin, inulin fibre and whole linseeds (soaked in water). Medication Manager We can help you know exactly when to take your daily medications Many people today are on more than one medication, and when you combine this with a busy and active life, it can lead to the confusion of properly taking your medications. Medication non-compliance is a serious concern and can be harmful to your health. Peoples offers a medication compliance program called “Med Manager.” This medication compliance card conveniently organizes your medications for a full week, and helps you easily identify what medications are to be taken at what time of the day.

the large bowel to squeeze harder than usual. This helps push the stools along and out. Their effect is usually within 8-12 hours. A bedtime dose is recommended so you are likely to feel the urge to go to the toilet the next morning. Laxatives are substances that can help relieve occasional constipation. However the long-term use of laxatives can make your body depend on them. It is best to try changes to your diet and lifestyle first. If your constipation does not get better, talk with your health care provider. When should I see a health care provider? You should see a health care provider if: • you have been constipated for several days • there is a sudden change in your bowel movements without a change in your daily routine; • and diet changes have not helped • you have intense pain in your belly or rectum • you have blood in your stool 10 PEOPLES Health

• you have unexplained weight loss • you have constipation that alternates with diarrhea. Increasing the fiber in your diet, daily exercising, drinking more water can often help constipation. But chronic constipation can signal a real problem. It can indicate a poorly functioning thyroid gland or diabetes. It can be the result of Parkinson's disease or stroke, or a side effect of medications. In rare cases, it can signal illnesses such as colorectal cancer or autoimmune disease. See a doctor if symptoms last more than two weeks or you have blood in your stools, severe pain with bowel movements, or unexplained weight loss. *The contents of this article provides basic information, but is not a substitute for professional medical advice. ications or medical conditions. While it might not be possible to answer all of your questions at one sitting, this provides a great starting point. Medication reviews are not only about information, they are also about building a relationship with your Peoples Pharmacist. Another important part of the medication review process is to improve something Pharmacists like the call adherence. Personally, I never liked the term adherence. It sounds like something associated with masking tape or glue. Adherence essentially means taking the right medication at the right time. We can all agree that taking your medication at the right time is important. How does a Pharmacist improve adherence? This is done through education. If you understand your medical condition and the medication to treat it, you are more likely to remember to take your medication. Sometimes proper adherence can lead to a reduction in a medication dose. How is this possible? I have seen it happen. If you are taking a medication to help reduce blood pressure, your Doctor or Pharmacist will be monitoring your blood pressure readings. If a specific dose of medication didn't cause a significant enough drop in blood pressure, then your Doctor might increase the dose. Your Doctor assumes that this dose of medication, taken at consistently regular intervals, was not effective and the dose might need to be increased. But what if you only remembered to take your medication half the times that it was prescribed? Ask your Peoples Drug Mart Pharmacist if there are any ways that they can help you improve your medication adherence Part of the medication review service is providing a Best Possible Medication History. During your review, your Peoples Pharmacist will prepare a written record of all your prescription medications, vitamins and supplements. On this record will the reasons for use and when it is the best time to take them. This record can make other health care professional appointments easier and more efficient. Focus On Psyllium Fibre Helps Lower Cholesterol Studies have shown that eating psyllium fibre can lower mildly to moderately elevated levels of cholesterol in adults when taken with a low-fat diet. Psyllium fibre is thought to stimulate the conversion of cholesterol into bile acid and then increase bile acid excretion. It may also decrease the intestinal absorption of cholesterol. By lowering your cholesterol, you can dramatically reduce your risk of heart disease and stroke. Fibre can be divided into two major types: soluble and insoluble. Psyllium seed husk is a soluble fibre and is the most common dietary fibre supplement. Talk to a Peoples Pharmacist about the health benefits of natural source psyllium fibre.

specialists may want to know what medications you are taking. If you have to go to the hospital for any reason, a detailed medication list will make your admission far more efficient and safer. If you travel with your medication, a written document could make your life easier. There are many good reasons to have a medication list which your Peoples Pharmacist can prepare for you. Another reason to have a printed medication list available involves emergency preparedness. What if the power goes out for an extended period of time? Your computerized records might not be available for some time. When the lights go out, paper always works. There will be more emergency preparedness ideas in the upcoming November 2016 issue of the Peoples Health magazine. If you find that carrying around this piece of paper is too cumbersome, why not take a picture of it with your smart phone? It always pays to be prepared. Do you find that you are regularly visiting your Doctor for refills? It might be a good idea to visit with your Peoples Pharmacist before your next doctor or specialist visit. Your Peoples Pharmacist can check your medication records to see which of your medications need refills. This will save both you and your doctor time with fewer office visits. Your community Pharmacist might also be able to write down a few suggestions, or questions, about your drug therapy for you to ask your doctor during your next appointment. There is so much more to your Peoples Pharmacist than medications and supplements. They can be a wealth of information about your health and ways to make your life easier. A medication review is but one of these tools to help you take charge of your health. The more you know about your medications and medical conditions, the healthier you can be.

DISTRACTED DRIVING Henry Ford changed the world by inventing an affordable car for the masses. You would have to be extremely present when driving a Model T as power mechanisms and the automated features of today’s vehicles did not exist. Imagine the anticipation of a sharp curve ahead. You decelerate by vigorously pumping brakes while your upper body works the turn of a manual steering wheel. Your distraction factor back in the day likely involved an animal, passenger, pedestrian, road condition or other driver. Now fast-forward to present time where we drive automated cars at much higher speeds within ever increasingly complex road networks. All this compounded by an exponential increase in distraction factors. Is it any wonder that according to ICBC, distracted driving is second only to impaired driving when it comes to crash fatalities in BC? At the mention of distracted driving, cell phones top the list and rightly so. They are one of the most visible contributors to the problem but not the only one. Distracted driving is defined as anything that impairs your ability to focus on the road ahead. In today’s world of shortened attention spans, that definition pretty much includes everything including, but not limited to, the following: 1. Sound: Audible distractions consist of a noisy interior or exterior driving environment. This could involve navigating traffic with a raucous passenger on board or being shaken from your thought trance by approaching sirens. Maybe you couldn’t hear the sirens because your music was loud enough to distract the driver in the next lane. Passengers should be reminded to respect your need to focus. If someone demands your attention, pull off the road and attend to their needs rather than try and keep going in spite of a ruckus. (And no, turning up the music to drown them out is not a good strategy!) 2. Sight: Visual distractions can be anything from a street sign to beautiful scenery. Thankfully, GPS systems have made arriving at a new destination more of an auditory experience rather than visual. If you are travelling new territory, find a place to pull off the road to orient rather than chance driving off into the sunset and taking other vehicles with you. If having a DVD player on board for the kids is the only way you

R.H.N., B.S.W.

can get them to sit quietly, make sure it does not interfere with your ability to concentrate. 3. Stress: Psychological distractions are when the stress of the job or the fight you had with a loved one follows you into the car and commandeers your thoughts. Or perhaps another distracted driver just cut you off and your emotions spill over into a pool of imaginary road rage and retaliation. When your mind is preoccupied, your ability to react is greatly hampered. 4. Physical: These distractions include eating, drinking, grooming, adjusting controls and pretty much everything that takes your hand(s) off the wheel. Yes, this would include texting and talking on your cell phone as well. While hands free devices negate the physical distraction factor, they are still audible and psychological by nature. If you get a text or a call wait until you are in a safe place to pull over and deal with it. 5. Monotony and Fatigue: Bells and whistles such as cruise control and advanced driver assistance systems do afford some performance and safety benefits but they may have some pitfalls as well. A study in France has found that those who used cruise control were more prone to drowsiness than those who do not. It boils down to the brain actually become less vigilant when exposed to monotonous stimuli. In BC you can be fined $167.00 and receive 3 demerits on your licence if you are caught using your phone while driving. Still, distracted driving is on the rise and it poses a significant safety risk to all those who use the road. Not only are cyclists and pedestrians more vulnerable at the hands of a distracted driver, they are sometimes part of the problem as well. I have seen pedestrians so engrossed in their hand held device that they have literally walked into oncoming traffic. I have also watched cyclists wind their way in and out of traffic with a phone held up to their ear. In short, multi tasking may be well and great when you are trying to impress your boss, but it has no place on the road. We have come a long way since the advent of the Model T but we still have miles to go. Good Health to You!

Peoples Drug Mart BC Walks For ALS Amyotrophic Lateral Sclerosis (ALS) is a rapidly progressive, neuromuscular disease. It attacks the motor neurons that transmit electrical impulses from the brain to the voluntary muscles in the body. When they fail to receive messages, the muscles lose strength, atrophy and die. Since the disease frequently takes its toll before being positively diagnosed, many patients are debilitated before learning they have ALS. The disease wreaks a devastating effect on patients as well as their families. Peoples Drug Mart is pleased to support the The ALS Society of BC by being title sponsor for the BC & Yukon Walks For ALS. Peoples Drug Mart and The ALS Society of BC have partnered together for the BC Walks for ALS since 2005. Since then, the Walks for ALS in BC & Yukon has raised over $4,371,921. The ALS Society of BC raises funds for both research and to provide direct services and support to those currently living with ALS. Until a cure is found, the Society provides patient services to improve the quality of life of those diagnosed with this disease by offering an extensive equipment loan program, support groups, and educational materials.
