EMERGENCY PREPAREDNESS This is one of my favourite subjects to discuss, emergency preparedness. It is also the least favourite subject to discuss with my daughter. “Daaaddd,” she says, “nothing bad is ever going to happen to us here.” I reassure her that she is probably right, but one of my duties is to make plans in case things do go wrong. Certainly there were fathers having similar conversations with their daughters in Fort McMurray, Montreal (1998 ice storms) or Newfoundland (hurricane Igor 2010). Unexpected emergencies can, and do, happen. It hit closer to home when the house next door had a small fire. The fire department could have asked us to leave our house for safety. There is so much to think about for emergency preparedness. I can't cover it all, but I can mention things you can do to ensure your medical supplies are taken care of. The first step with any emergency preparedness is planning. While this might seem obvious it is important to know what you are planning for. People on the west coast should prepare for earthquakes, people in wooded areas should prepare for forest fires and people in the north should be ready for cold weather. A great place to start is the “Get Prepared” Government of Canada Website. This site lists various risks in your local area and ways to prepare. This might be the best place to begin. My focus is on your medications and personal health care needs. The first thing I recommend people do is to make a list of personal care items they use on a regular basis. It can be a piece of paper on the calendar or an app of your smart 4 PEOPLES Health

phone; whatever works for you. Keep track of what you use. Our family list for the past month included items like toilet paper, contact lens solution, cat food and canker sore medication. These are all items that members of our family use on a regular basis. We consider the cat an important part of our family. Your list might be different and mention prescription medication, heartburn relief, pain relievers, etc. Whatever is on your list, make sure you have at least two weeks supply (or more) in the house at all times. This makes sense and will also help you plan your time and money better. Everyone uses toilet paper, so why should you ever run out? Something that is important for all emergency preparedness is keeping track of our documents. Do you have a copy of your will, marriage certificate, house deed and insurance papers? These items are important and would be difficult to replace. All it takes is to make a photocopy of your important papers and mail these copies to a trusted family member for safekeeping. What about a list of your medications? This might be a good time to sit down with your People Pharmacist for a medication review. At the end of the review, your Peoples Pharmacist provides you with a list of your medications and supplements and their uses. This would be a great record of what your are taking in case of an emergency. It might be a good idea to take a picture of this medication review to keep on your smartphone. GOUT Gout is a type of arthritis that is characterized by sudden, severe attacks of joint pain with redness, warmth, and swelling in the affected area. These attacks can happen over and over unless gout is treated. Over time, they can harm your joints, tendons, and other tissues. Gout is most common in men and it is most common in countries with high standards of living, mainly because diet plays a large role in this condition. Gout is caused by too much uric acid in the blood. Most of the time, having too much uric acid isn't harmful. Many people with high levels in their blood never get gout. But when uric acid levels in your blood are too high, the uric acid may form hard crystals in your joints. Certain high-protein foods can make the body produce too much uric acid, triggering gout. Beverages such as tea, coffee, cocoa, and especially alcohol in any form lead to extra water loss from the body, which can cause an attack. Some medicines, such as water pills (diuretics), can also bring on gout. What are the symptoms of gout? The most common indicator of gout is an evening attack of tenderness, swelling, redness, and sharp pain in your big toe. Typically, a person will go to bed feeling fine, then wake up during the night with intense pain in the big toe (three-quarters of gout cases involve this joint). You can also get gout attacks in your foot, ankle, or knees, or other joints. The affected area also becomes extremely sensitive to touch. The swelling often spreads over 6 PEOPLES Health Join us on

the whole foot, making it impossible to put on a shoe. The attacks can last several days or weeks before the pain goes away. Another attack may not happen for months or even years. See your doctor even if your pain from gout is gone. The buildup of uric acid that led to your gout attack can still harm your joints. To stop a gout attack, your doctor can give you a shot of corticosteroids or prescribe a large daily dose of one or more medicines. The doses will get smaller as your symptoms go away. Relief from a gout attack often begins within 24 hours if you start treatment right away. To ease the pain during a gout attack, rest the joint that hurts. Taking ibuprofen or another antiinflammatory medicine can also help you feel better. But don't take ASA. It can make gout worse by raising the uric acid level in the blood. To prevent future attacks, your doctor can prescribe a medicine to reduce uric acid buildup in your blood. It's crucial to control weight and blood pressure and to be well-hydrated. A healthy diet is very important and can help minimize attacks. This includes: • Avoiding alcohol overuse (>2 servings per day for a male or > 1 serving per day for a female.) Avoid alcohol consumption during periods of frequent attacks or poorly controlled gout. • Limiting consumption of beef, lamb, pork, seafood (sardines, shellfish). • Limiting consumption of naturally sweet fruit Continued On Page 8

juices, sweetened beverages, and table salt • Avoiding organ meats high in purine content (liver, kidney). • Avoiding high fructose corn syrup-sweetened sodas, beverages, or foods. Medicine treatment for gout usually involves some combination of short- and long-term medicines. You use medicine to treat an attack of gout and to reduce the uric acid in the blood. Reducing uric acid helps reduce how often you have attacks. Short-Term Medicines Short-term medicine relieves pain and reduces inflammation during an acute attack or prevents a recurrence of an acute attack. These medicines may include: • Non-steroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen, indomethacin, or naproxen. Do not take aspirin (ASA), which should never be used to relieve pain during a gout attack. Aspirin (ASA) may change uric acid levels in the blood and may make the attack worse • Corticosteroids, which may be given in pills or as a shot for cases of gout that don't respond to NSAIDs or colchicines. If treatment is started right away, relief from symptoms often occurs within 24 hours. During a gout attack, your doctor will prescribe a maximum daily dose of one or more medicines used for short-term treatment to stop the attack. Doses are then reduced

as the symptoms go away. Long-Term Medicines Long-term treatment uses medicines to lower uric acid levels in the blood. This can reduce how often you have gout attacks and how severe they are. There are several ways to decrease the pain of an acute attack: • Elevate painful joints. • Rest the affected joint until the attack eases and for 24 hours after the attack. • Use ice to reduce swelling. • Relieve inflammation by taking non-steroidal antiinflammatory drugs (NSAIDs). But don't take aspirin (ASA), which may abruptly change uric acid levels and may make symptoms worse. In the past, gout was thought to be caused by drinking too much alcohol and eating excess rich foods. Although eating certain foods and drinking alcohol may trigger a rise in the level of uric acid in the body, these habits may not by themselves cause gout. Gout is most often caused by an overproduction of uric acid (due to metabolism problems) or decreased elimination of uric acid by the kidneys. With early diagnosis and treatment, it's possible to control gout, prevent joint damage, and live a normal life. *The contents of this article provides basic information, but is not a substitute for professional medical advice. Most sources recommend that you keep at least 72 hours of medications and personal health care needs. I am an advocate for keeping at least one week of health care needs at all times. This will not only make it easier to plan your week, so you are not rushing around doing last minute errands. It will also prevent you from running out of medication during a long weekend. The way I recommend people set aside their medication for this week is with a seven day plastic pill container or dosette. Your local Peoples Drug Mart Pharmacy should have plenty of these items in stock. Once a year, make a reminder on the calender or in your smart phone, take some time to rotate new medications in and use the medication from last year. Place this full dosette, and a copy of your medication list, in a zip-lock bag and store in a safe place. I generally don't recommend adding vitamins, it might make the dosette too bulky and heavy. What should you do if you have medications that need to be kept cool, like insulin or biological injections? Not to worry, most refrigerators will stay cool for a day or two without power. Just don't open it up too often. Many refrigerated medications might be stable at room temperature for longer than you think. Insulin is generally stable at room temperature for 30 days. Check with your Peoples Pharmacist for more information about the stability of your refrigerated medications. If you are concerned about keeping your medications cool for longer, I have a plan. Most people have a small, portable hard sided cooler in their house. Make sure to keep a few bottles of frozen water in your freezer. Wrap the refrigerated medication in a towel and place in the cooler with a few bottles of frozen water. Ensure that the medication does not come into direct contact with the frozen bottles. Depending on the size of the cooler and amount of frozen water, this could keep things cool for up to a week. You could place a comforter or blanket on top of the cooler for added insulation. Don't forget about first aid supplies. I think the first, and most important item, to have is a first aid kit. Not just one, but two. The primary first aid kit should be a larger, well stocked one. It will have

Focus On Melatonin Sleep Support Melatonin is a hormone secreted by your body's pineal gland. It helps regulate other hormones and maintains the body’s circadian rhythm, an internal 24-hour timekeeping system that affects how and when we fall asleep and wake up. During the day the pineal is inactive. When the sun goes down and darkness occurs, the pineal is "turned on" by the supra-chiasmatic nucleus and begins to actively produce melatonin. The amount of melatonin released at night varies among individuals and appears to diminish with age. When your sleep patterns are disturbed, by travel or shift work for example, supplemental melatonin can help reset your body’s ‘clock’ and improve length of sleep and sleep quality. more items such as larger gauze pads, pressure bandages, latex gloves, arm slings, scissors, a first aid book and lots of bandages. Never open this one unless it is a true emergency. Most of these items never expire and can be valuable in a true emergency. The second first aid is called the “BooBoo” kit. This kit will have the more commonly used items like bandages, antibiotic cream and a tensor bandage. Having a small and easily accessible “Boo-Boo” kit will ensure that no one takes important items from your larger first aid kit. Lastly, you should have items ready to go at a moments notice. Each member of our family has a 72 hour bag at the front door, including the cat. There are many opinions about what should be in this bag. I feel it should be light enough to carry and contain enough supplies to keep you self sufficient in a hotel room for the weekend. Look at the “Get Prepared” Government of Canada Website for a suggested packing list. This bag does not need to contain the latest in high tech gadgets or sturdy supplies. Just a change of clothes, some minimal toiletries, snack bars, water, your medications and something interesting to pass the time. Your should also know exactly where the most important items in your house are kept. In a true emergency you will have little time to think or gather items. Our important documents are in a fire proof safe in the basement. Wedding and family photo albums are kept on the bookshelf in my office. If needed, we could gather our important items and be on the front lawn in under one minute. Perhaps none of these emergency preparations will be needed. Hopefully that is the case. Either way, if there is an emergency my family will be glad we got things in order. Emergency preparedness does not have to be an isolated event. Having extra household supplies on hand will reduce amount of last minute shopping trips. It will also allow you to wait until your favourite items go on sale. INFLUENZA SURVIVAL STRATEGIES Happy November! This is the month where we start to anticipate and plan for a busy holiday schedule; it is also the month that heralds the beginning of our Canadian influenza season. Yes, from now until March, high fevers, coughing, nausea and aching body parts will derail many well-intended plans. It is tempting to think that one can just hide away in a metaphorical cave and avoid exposure to this nasty bug. The hermit’s life is certainly a strategy but it is totally unrealistic as we balance the everyday obligations of a well-lived existence with a busy season of planned gatherings and celebrations. While there are no guarantees the flu bug will pass you by, there are some strategies that can help minimize the duration of it’s visit and the severity of the wake it leaves behind. Influenza is a viral infection that spreads easily through air born droplets when an infected person talks, coughs or sneezes. As we tend to gather in groups, it can easily sweep through schools, health institutions and the business sector. While most people can ride it out without medical intervention, young children, seniors and people with other medical conditions are at risk of developing flu related complications that can result in hospitalization and sometimes death. Prevention is always the best cure and when it comes to surviving the flu season, preventative measures are the front line of defence. Be proactive when it comes to your health this winter and keep the following tips in mind: 1 According to the World Health Organization, an annual flu vaccine is your first line of defence. While it may not always prevent the illness, it may help reduce the risk of complications and shorten the duration of illness. Vaccination is highly recommended for those who are most at risk and for those who work with people who are most at risk. 2 When out and about, wash your hands frequently and avoid touching your mouth, nose or eyes with your hands. Carry a hand sanitizer on your person and use it often. Keep your workplace clean by disinfecting objects and surfaces like doorknobs, light switches, phones, remotes and computer stations. 3 If you are sick, stay home. If you can’t stay home, cover your mouth and nose with a tissue when you sneeze or

It is becoming more common to see people wearing a face mask to avoid transmission as well as catching the bug. 4 Live a preventative lifestyle that includes eating a healthy whole foods diet rich in immune boosting foods such as garlic, green tea, ginger, onions, shiitake mushrooms and probiotic or fermented foods. Reduce or avoid junk food and other habits that work against optimal health. 5 Stock up on your favourite immune boosting supplements. There are many great immune supports on the market and you can chat with a natural health advocate to find one that works for you. Many popular ones include mushroom blends, oil of oregano, echinacea, olive leaf and other healing herbs. It is a good idea to boost vitamin C, vitamin E and B-complex vitamins during flu season as well. 6 Hydrate your body. Water is needed to keep oxygen and nutrients circulating in the bloodstream and to keep the lymph system working to transport white blood cells and remove waste from the bloodstream. Hydration is also important to keep all the eliminatory organs functioning optimally. 7 Daily moderate exercise supports your immune system by increasing oxygen levels and circulation of immune cells in your bloodstream. It is also a key support to help reduce the effects of chronic stress on your body. Remember that the body is actually quite capable of healing and regenerating if we give it the tools needed to do so. If by chance you do fall prey to a cold or flu this season, don’t underestimate the healing properties of plenty of rest, good old-fashioned chicken soup and sipping ginger lemon tea. Herbal teas and broths are not only soothing and nourishing, they can help to keep your fluid intake up. Adequate sleep and quiet time are both a preventative measure as well as a recovery aid. Meditate, visualize and take a nap whenever you can fit it in. In the meantime, wash your hands often, keep stress to a minimum and fill your lungs daily with fresh air by prioritizing a brisk walk. Fruity Lamb Tagine

Combine first 3 ingredients in large resealable freezer bag. Add lamb. Seal bag and toss until lamb is coated. Transfer lamb to a 3½ to 4 quart (3.5 to 4 L) slow cooker. Combine next 10 ingredients in a large bowl. Add to lamb and stir. Cook, covered, on Low for 7 to 8 hours, or on High for 3½ to 4 hours. Scatter almonds over top.
