The Pulmonary Hypertension Association (PHA) welcomed 2021 with important milestones and new beginnings. The year marked the start of PHA’ s 30th anniversary, the launch of COVID-19 vaccines in the United States and new leadership We celebrate those milestones and other achievements in our first digital annual report.
The year began with a new CEO when Matt Granato joined PHA as its third president and CEO. One of his key priorities was to ensure patient care remains at the center of PHA’ s programs As the year progressed, the staff found new ways to promote his vision, leading to improvements in operations, patient services and culture.
A sense of hope existed as vaccines became available for the general public and COVID-19 testing became more accessible. However, new variants that spiked infection rates continued to affect PHA and the pulmonary hypertension community
Although PHA’ s Silver Spring headquarters remained closed, our team was as productive, if not more so, than before the pandemic We opened our jobs to people outside of the Washington, D C , area, building a culture based on the new reality
PHA is now truly a national organization with staff in many states and the District of Columbia working together to accomplish our mission We have new talent and a diversity of experiences that weren ’t possible before the pandemic That makes PHA stronger and more reflective of the communities we serve
Throughout 2021, PHA’ s main achievement was keeping the community together and informed despite ever-changing pandemic challenges We provided scientifically accurate resources, even as new information emerged.
This 30th anniversary gave PHA an opportunity to review what we have achieved and what lies ahead. While it was challenging to plan long-term in such a fluid and constantly changing situation, PHA history shows that we always get back up and move forward
As PHA entered a new decade, we continued to strive to extend and improve the lives of people affected by PH PHA staff, leadership, volunteers and members are critical to that mission. Our corporate partners and financial supporters have stuck with us, a testament to their commitment to our members and our mission
The PHA 2021 Annual Report highlights our accomplishments in our 30th anniversary year Those achievements reflect how we ’ ve adapted to life

In 2021, the Pulmonary Hypertension Association marked its 30th anniversary by celebrating the vision of its four founders. Our anniversary campaign, “Lighting the Way Forward,” honored PHA’ s history while looking toward the future
We created an interactive timeline that detailed the shared history of PHA and the pulmonary hypertension (PH) community Users scrolled through “30 Milestones in 30 Years” to discover people and events that helped make PHA what it is today. We promoted the timeline by sharing a below

On social media, we asked the PH community to participate in our "Share Your Vision" campaign We used the hashtag #PHA30 to mark our 30th anniversary posts We posted stories on our website of people with long ties to PHA, including people with PH and health care professionals Several patients were diagnosed in the 1980s and 1990s when limited resources and information about PH were available. We shared their accounts in the Pathlight magazine. That issue won a silver Trendy award from Association Trends, in recognition of outstanding communications and marketing by nonprofit organizations

The Pulmonary Hypertension Association raises awareness of pulmonary hypertension all year long. Throughout the year, we educate the public about symptoms, risks, diagnosis and treatment culminating in our annual PH Awareness Month campaign each November During PH Awareness Month and all year long, we raise awareness through our website, social media promotions, digital newsletters and print magazine. Raising PH awareness brings us closer to our mission to extend and improve the lives of those affected by PH
PH Awareness Month
In 2021, the PH Awareness Month theme was “Spark Awareness,” which complimented our “Lighting the Way Forward” anniversary campaign We brought back our award-winning awareness month calendar, which each day revealed an activity, fact or resource about PH to Spark Awareness.
PH Awareness Month 2021 focused on PH caused by left heart disease, the most common form of PH. We published a new free brochure, “PH & Left Heart Disease” and presented a webinar with Vineet Agrawal, M D , Ph.D , as part of our PHA Live series

Thousands of people with pulmonary hypertension and caregivers receive support from peers and learn about their disease through Pulmonary Hypertension Association (PHA) support programs
Patient and caregiver support has been a priority since PHA’ s first support group meetings in 1991, and PHA made sure that support continued throughout the pandemic.
In 2021, PHA continued to prioritize remote support programs rather than in-person meetings In addition to virtual support group meetings, we offered our toll-free patient-to-patient help line, Email PHriends mentoring, telephone support groups, and Facebook groups
PHA added a Spanish-language group in 2021 to its monthly telephone support groups. Spanish-speaking patients can connect, ask questions and receive support Because the meetings take place by video call, patients can join from anywhere in the world
As part of our efforts to connect people who have PH and associated conditions, PHA worked with the Sjogren’ s community in 2021 to create a support group so people with PH and Sjogren’ s syndrome could share experiences and tips for living with both diseases. We presented interactive webinars about PH and Sjogren's and connective tissue disease.
In recognition of the stress many volunteers face in their personal lives, particularly during the pandemic, PHA enhanced support for volunteer leaders. We continued live Facebook check-ins with volunteers and scheduled quarterly virtual meetups so volunteers could socialize and network.
Also in 2021, PHA created a new training manual for virtual volunteers The manual helps volunteers understand their roles, how they can best support the PH community and how to manage meetings and difficult situations.

The Pulmonary Hypertension Association strengthened and expanded its grassroots advocate network in 2021 through monthly action alerts and virtual advocacy activities PHA members sent thousands of messages to Congress about proposed legislation to improve life for people with pulmonary hypertension
The messages called for insurance reforms, such as decreasing step-therapy requirements and ending copay accumulators. The PH community also urged Congress to increase National Institutes of Health funding and end barriers to specialized health care during the COIVD-19 pandemic. We also created an Advocacy Committee to strengthen PHA’ s grassroots advocate network The 10 members connect with new and existing advocates They also provide coaching for legislative visits and disseminate advocacy news The members include a diverse mix of five patients and five health care professionals who represent specific geographic regions.
Pulmonary Hypertension Association Annual Report 2021 Page 29
Several Advocacy Committee members began their terms by participating in virtual legislative visits during the PH Professional Network Advocacy Day Committee members provided support to new advocates Many Advocacy Day attendees participated to advocate for their PH patients
Our 2021 victories included the enactment of a Connecticut law that prohibitscopay accumulators The law improves health care access for Connecticut residents with PH and other complex, costly health conditions. The victory is the result of many years of lobbying by PHA, including repeated testimony, calls and letters from 2020-21 Board of Trustees Chair Colleen Brunetti and other rare disease advocates

The Pulmonary Hypertension Association’ s treatment access staff responded to nearly 100 inquiries in 2021 They provided personalized support for questions about financial assistance for copay costs, challenges in accessing oxygen, and the transition from employer-based insurance to Medicare.
PHA also reached out directly to patients about the availability of new financial assistance.
At year end, our 2021 open enrollment campaign provided information and resources to help people select and sign up for Medicare, insurance marketplaces and employer-based insurance
Throughout the year, we provided information and updates on our online insurance and treatment access guide The updates included news about charitable assistance funds opening and closing.
Several submissions to PHA's Specialty Pharmacy Feedback Form helped patients resolve billing and refill issues they previously struggled to resolve

Despite the challenges of the pandemic, the Pulmonary Hypertension Association increased access to quality pulmonary hypertension care through the PHA-accredited PH Care Centers The PH Care Center network aims to increase access to specialized PH care; enhance health care provider and patient education; secure earlier diagnosis; and improve patient outcomes
In response to COVID-19 and limited in-person site visits at PH care centers, PHA adapted the accreditation review process a remote format. PHA retained all of its accredited care centers in 2021 and added one new center. The interest in accreditation remains strong, with 24 site visits and four new applications to join the PH Care Center network

A Quest to Reverse Lung Damage
Vineet Agrawal, M.D., Ph.D., didn’t plan a career in medicine. Growing up, he wanted to be an engineer and studied biomedical engineering at Duke University At Duke, he worked in a laboratory focused on medical devices There, he was inspired to pursue a medical degree and continue his research.
Dr Agrawal’ s path toward pulmonary hypertension (PH) was gradual It also was personal “I have a family member who suffers from pulmonary arterial hypertension (PAH) I get to see firsthand the effect that PAH has on the life of patients and families.”
Dr Agrawal seeks to understand how to reverse the destruction of lung vessels, one of the fundamental causes of PAH He and his colleagues have identified a new protein that could restore normal metabolism in PAH lung vessels. He hopes his findings th lt ti i l l d l d t

The Pulmonary Hypertension Association (PHA) had a record fundraising year in 2021 despite pandemic-related obstacles.
In November, PHA conducted its best Giving Tuesday ever A $30,000 match in honor of PHA’ s 30th anniversary helped drive unprecedented donations for the online fundraiser The match came from generous donors within the PH community who lost family members to PH and wanted to help prevent similar losses.
Pulmonary Hypertension Association Annual Report 2021 Page 49
The total raised for Giving Tuesday 2021 surpassed $121,000, more than 20% higher than the previous year. Those donations, along with endowments, corporate support, company matches and foundation gifts, funded PHA programs at a critical time
PHA’ s monthly giving club, the Sustainers Circle, also saw a large boost in 2021 Doug Taylor, a member of PHA’ s Board of Trustees with PH, led the charge to increase monthly contributions through our Summer Step-Up Campaign.
Other donors also shared their inspirational stories to encourage people to give what they could, resulting in some of the highest performing fundraising campaigns in PHA’ s history

Engaging the community
During our 30th anniversary year, PHA ran fundraising campaigns that featured three people with PH and a caregiver. The campaigns resulted in some of the highest performing fundraising efforts in PHA’ s history, especially the spring campaign featuring Monica Penaranda, which brought in more than $28,000. That campaign alone raised more than twice the spring campaign of 2020.
Fundraising events
PHA fundraising events raised more than $200,000 in 2021 The majority of the fundraisers were PHA O2breathe walks conducted virtually Participants raised money through an online platform before the celebratory interactive events Each virtual event featured participant photos, individual and team fundraising recognition and prize drawings. They also highlighted information about pulmonary hypertension, the 30th anniversary celebration and PHA programs that depend on fundraising
The grassroots event that raised the most money ($60,000) was a golf tournament organized by PHA board member Matt Wall and his wife Jayna, whose son Jackson has idiopathic pulmonary arterial hypertension.
The virtual event that raised the most money was the Southern California O2breathe Walk. The event raised about $22,000.
