of Eastern medicine and has been commercially synthesized and available for use as a hepatoprotective agent in Japan since the 1930s. Since the 1970s ursodeoxycholic acid has been used in Western human medicine for dissolution of gallstones. More recently, it has been used in the management of chronic hepatic diseases in humans such as primary biliary cirrhosis, biliary disease secondary to cystic fibrosis, intrahepatic cholestasis of pregnancy, nonalcoholic steatohepatitis, idiopathic chronic hepatitis, autoimmune hepatitis, primary sclerosing cholangitis and alcoholic hepatitis. However, its therapeutic efficacy in some of these disorders has not been firmly established.
Ursodeoxycholic acid was approved for use in the United States in December 1987, and was designated an orphan drug. It is the only FDA approved drug to treat primary biliary cirrhosis.
Ursodeoxycholic acid goes by the trade names Actigall, Ursosan, Ursofalk, Urso, and Urso Forte. In Italy and Switzerland, it is marketed under the name Deursil. Ursodeoxycholic acid currently on the market mainly appears in the form of tablets, capsules, liquids and powders, and is also sold in the United States, China, Japan and some European countries.

It is used for the treatment of primary biliary cirrhosis (PBC), dissolution of radiolucent gallstones in patients with a functioning gallbladder, and treatment of hepatobiliary disorders associated with cystic fibrosis in pediatric patients. It can be used as a general medicine.
As an epimer of chenodeoxycholic acid, the administration of ursodeoxycholic acid powder changes the composition of bile and may dissolve gallstones. It is used as a choleretic and choleretic.

In addition, UDCA significantly reduces bile cholesterol saturation by inhibiting the absorption of cholesterol in the intestine and its secretion into bile, which can be demonstrated by the cholesterol partial reduction of bile lipids.
Choleretic effect Ursodeoxycholic acid reduces elevated liver enzyme levels by promoting bile flow through the liver and protecting liver cells. The main mechanism is anti-gallstone. Although the exact process of ursodiol's anti-gallstone effect is not fully understood, it is believed that the drug concentrates in bile and reduces bile cholesterol by inhibiting the synthesis and secretion of cholesterol by the liver and inhibiting its intestinal absorption. The reduced cholesterol saturation allows the cholesterol in gallstones to gradually dissolve, causing them to eventually dissolve.
Immunomodulatory effect Ursodeoxycholic acid powder has also been experimentally proven to inhibit immune response, such as immune cell phagocytosis. Long-term exposure and/or increased amounts of ursodeoxycholic acid throughout the body (whole body, not just the digestive system) may be toxic.

Ursodeoxycholic acid has been shown to have anti-inflammatory and protective effects on human gastrointestinal epithelial cells. It has been linked to the regulation of the immune regulatory response through the regulation of cytokines, antimicrobial peptide defensins, and has taken an active role in increasing the return of wounds in the colon. In addition, the role of UDCA powder has been shown to work outside the epithelial cells. In addition, ursodeoxycholic acid dissolves or prevents cholesterol gallstones by preventing the production of cholesterol in the liver and reducing bile cholesterol. It also has the effects of improving the bile flow of newborns with impaired blood flow, anti-inflammatory and immune regulation.
UDCA has also been in effective in non-alcoholic fatty liver disease, in liver bile duct-paucity syndromes. However, it is contraindicated in obstruction of biliary tracts such as biliary atresia. It's not effective in liver allograft rejection, and in Graft-versus-host disease involving the liver.
