LEADING SCIENCE AND PRODUCT INNOVATION Hospitals from Africa to Australia have benefited from British healthcare expertise, using medical devices, treatments and processes created by innovators in the UK, that have revolutionised the way healthcare is managed and administered the world over. The industry's focus is on innovation and creation of cutting edge, workable and affordable medical devices and technologies. The medical technology sector in the UK consists of 2,771 companies generating £10.6 billion of annual turnover in a complex market, covering products from consumables such as disposable surgical gloves to systems that enable surgeons to see inside the body as they carry out minimally invasive surgery. The UK is home to 340 companies working in the medical field with a turnover that is greater than £5 million. 43% of the companies in the sector are less than ten years old and 67 new companies have been formed in the last two years. The sector is constantly innovating to meet and exceed patient needs and treatments. Four segments; wound care management, in-vitro diagnostics, orthopaedic devices and single use technology are the largest segments by turnover, each with just over £1 billion in sales. Together they make up 40% of the total UK turnover in the healthcare sector. The professional services, invitro diagnostics, single use diagnostics, wound care and orthopaedic devices segments contain 41% of all employees in the sector. The sector is supported by a strong network of specialist suppliers (segmented as professional services), which is the segment with the largest number of companies and employees.
6
The National Health Service (NHS) has been key to driving innovation. An annual budget of over £100 billion has helped suppliers focus on developing products which will help the NHS meet its objectives of patient care, at the best possible price. The size of NHS procurement means that companies can be ambitious in their innovation in the knowledge that a significant volume of sales can be achieved. The NHS has innovation teams which scout for the latest technology and products to raise health standards. This focus and support for innovations has benefited the wider healthcare sector in the UK and beyond. The UK has leading expertise in a number of fields and is open to sharing knowledge and capability with Indian partners. Medical disciplines recognised for their excellence include: cardiology cardio-thoracic surgery gastroenterology orthopaedics & joint replacement surgery neurology critical care medicine nephrology oncology hand & micro surgery reproductive medicine

UK companies produce the full range of high quality, high durability equipments needed in a modern healthcare facility, from essential laboratory equipment and beds, to super speciality operation theatres and associated equipment needed for procedures and diagnostic work. Recent innovations in Telemedicine are being used to help leverage telecommunication technology, to deliver healthcare and education to patients in regions that are geographically less accessible. The UK’s strong science base has been key to the healthcare sector’s success. Academics work closely with hospitals and companies to translate academic solutions into practical products and services. The size of the NHS enables academics and companies to quickly test and assess the clinical applicability of new products and services, due to the NHS having pools of patients ready to participate in research programmes. In addition to universities supporting product research, the UK has over the years developed a sector of specialists who work with healthcare providers to tackle problems and raise standards. These consultants advise on all areas of healthcare, from the setting up of processes for primary care, through to advising on management of hospital facilities, to enable the efficient allocation of resources. Many of the consultants have global networks to assist healthcare providers around the world. Central to recent improvements in UK healthcare provision has been huge investments made with the private sector on Private Finance Initiatives (PFIs). The use of PFI schemes puts healthcare
8
providers and private sector funders together, so that the private sector takes some of the risk of large projects and at the same time commits to running or delivering specific health services for a specific time period (generally about 20 years). The consultants who design the PFI schemes also work with building and facilities management contractors to add value to projects by acting as project managers and as such can help manage the design, build, equipping and running of hospitals. Using the PFI model, Central Manchester hospital was redeveloped at a cost of about £400 million. The now unified hospital complex combines traditional hospital wards, research units as well as academic and industry partners, to encourage better and faster development and delivery of health solutions. PFI has been a recent addition to UK healthcare but it has grown rapidly.

Today the highly regulated sector is focused on innovative products and materials that deliver better results for patients for longer. Today the Dental Appliances Manufacturer Audit Scheme (DAMAS) plays an important role in supporting the manufacture of dentistry related devices. The DAMAS scheme improves efficiency, reduces waste, cuts remake levels and saves times, this in turn enables cost savings to be passed onto customers. UK research in dentistry has a particular focus on the development and the use of innovative materials. Research at the University of Birmingham is concentrating on the development of new filling material with improved depth cure, through monitoring and analysis of refractive index change through light activated fillings.

Scientists at the University of Leeds are investigating novel aesthetic dental restorative materials, with work into mica-based dental glass ceramics, which are particularly suitable for chairside processing. Dentistry schools at the University of Leeds and the University of Sheffield are active in the next generation of glass ceramics with high strength and fracture toughness that can bond to enamel, dentine and bone. Technology developments impacting upon the dental market include new bioceramics, improved alloys, fillers and cements, composites, regenerative materials, bone substitutes and drug eluting meshes. The University of Bristol is looking at the development of automated rapid fabrication of net shape ceramics, work, which could potentially revolutionise current manufacturing technologies in ceramics.
THE UK'S DENTAL INDUSTRY SIZE HAS A HIGHLY TRAINED, HIGHLY SKILLED WORKFORCE. UK DENTAL LABORATORIES ARE WELL REGARDED, PARTICULARLY BY THE PRIVATE SECTOR, WITH BEST PERFORMING LABS WORKING ON IMPLANTS.

Following eight years of R&D, SmartSeal was launched commercially in 2007 by DRFP LTD, UK, as a radically different, biocompatible root filling material that helps to improve endodontic results. Its two-part active system comprising SmartPoint and SmartPaste is designed to be easy and quick to use, requiring no change of technique, no additional equipment and providing excellent long term results. The innovative difference with SmartSeal, is that it's the only obturation system available that utilizes hydrophilic polymer technology. When water is absorbed from the tooth tissue, the SmartPoint obturation point swells in the lateral plane only to create an excellent seal and prevent re-infection of the root canal. The SmartPaste root canal sealer also uses hydrophilic polymers to swell into the lateral canals and irregularities in the root to create a tight mechanic seal.
SmartPoints are available in both constant taper and variable taper sizes to fit most file systems, and the SmartSeal range also includes SmartPaste Bio (bioceramic sealer), SmartGauge (to trim constant taper SmartPoints to the correct apical tip size) and SmartTrim (perfect for trimming the excess at the base of SmartPoint once in the canal).
Scottish company DENFOTEX has been very successful in both the UK and Germany with its chair-side Photo Activated Disinfection (PAD™) - a minimally invasive treatment that takes just 1 to 2 minutes and eliminates bacteria in all types of oral infection viz. in periodontics, perimplantitis, endodontics and caries. Also, Denfotex is now developing PAD for other medical applications including treatment of fungal foot/nail infections.
GP-based products have served their purpose for over a century but their limitations are well documented. SmartSeal's unique properties overcome existing inadequacies to improve outcomes for practitioners and patients alike. Since its launch, the system has grown in popularity with converted practitioners commenting about its simplicity of use, its valuable lateral expansion properties that negate the need for accessory points, and the good match of the taper points.

PUBLIC PRIVATE PARTNERSHIPS (PPP) Any collaboration between public bodies, such as local authorities or central government, and private companies, tends to be referred to a public-private partnership (PPP). The UK Government has used PPP to expand and secure improvements in public health services. The efficiency of private sector companies is combined with health excellence of public sector providers, to run health services in line with agreed performance indicators for set periods of time. In trying to bring the public and private sector together, management skills and financial acumen of the business community help to deliver value for money. The United Kingdom has developed considerable expertise in the use of Public Private Partnerships (PPPs) in the health sector with over 280 schemes worth some ÂŁ13.6 billion, making it the largest health PPP programme in the world. Such projects have ranged from the provision of clean, modern, purpose-built premises for general practitioners through to complex tertiary hospitals such as the 1,248 bed Barts and London Royal Hospital which is a centre of cancer and cardiac excellence and the largest PPP hospital project of its kind in the world.
48
On the public sector side, organisations like Partnerships UK (PUK) and the Department of Health Private Finance Unit, have been heavily involved in helping the UK Government deliver its wide range of PPP health schemes. The UK public sector has developed a wealth of best practice in PPP project preparation, contract development, value for money assessment and contract management. On the back of this programme, the UK private sector has also developed extensive financial, legal, technical, project management, construction and facilities management expertise to support both the public sector procuring authorities and the private sector bidders as well as to finance, construct, maintain and operate these facilities.
EVERSHEDS PPP work is led by their project and infrastructure finance group. Worldwide, this group comprises over 150 specialist projects lawyers and 30 partners. About a third of this resource is based in the UK.
A key example of this is their advice to a contractor who is part of the preferred bidder consortium on the NSK Karolinska health care project, one of the largest hospital projects in Europe and Sweden's first PPP.
Eversheds' infrastructure work has been one of the key platforms for the growth of the firm. They are now one of the largest law firms in the world, with about 400 partners, 4,000 people and 40 offices in 25 countries. Through this network, they are well placed to support their clients across the world, and have been particularly active in Europe, India, the Middle East, Latin America, Africa and China.
Eversheds also regularly advise on the legal issues relating to medical equipment and technology use, both within PPP facilities and in stand alone arrangements. For example, they have advised the London Clinic on purchase and maintenance contracts with suppliers of radiotherapy equipment. For more details visit www.eversheds.com
Their project and infrastructure finance group specialises in PPP projects both in the UK and in international jurisdictions. Within the UK PPP market alone, they have advised on 250 of the 800 closed projects. Many of these have been in the healthcare sector. The PPP market in the UK is well developed. Many other countries with PPP programmes look to the UK model as an established starting point for their own schemes. Eversheds have successfully worked in a number of jurisdictions, including India, to help share the lessons learnt from the UK PPP programme.

DESIGN AND BUILDING OF HEALTHCARE FACILITIES The UK has some of the most renowned architects in the world, but it is not widely known that many of the UK's architects have been closely involved in the design and development of healthcare facilities for the 21st century. Multibillion pound NHS building programs plus heavy investment from the private sector have catalyzed some ground breaking developments.
Recognising the need for a wide range of consultancy and design services to assist and support healthcare clients in India, Technopak and DEVEREUX ARCHITECTS have formed a strategic alliance. This combines Technopak's leading edge consultancy services in the healthcare domain with Devereux Architects' extensive international healthcare design experience and expertise.
Modern healthcare facilities need better functionality to improve hospital efficiency, reduce energy costs and improve hygiene. Scientific analysis of patient processes and flows through buildings, can dramatically improve how healthcare is delivered to patients. With expensive real estate and limited building budgets, architects need to realise maximum value from the land space available. UK firms have produced award winning designs for facilities ranging from small city centre units to large scale centres for growing towns.
Devereux Architects are one of the UK's leading healthcare architects who have experience in Europe, the Middle East, and the Caribbean. The practice's experience covers projects for government health services, such as the UK National Health Service (NHS) and for independent health providers. This experience also covers all types of healthcare facility including acute and tertiary/specialist hospitals; community hospitals/local care centres providing intermediate care; primary care centres and polyclinics; treatment centres; mental health facilities and nursing and care homes.
Energy costs are a major budgetary factor and smart design of lighting, heating, ventilation, and waste systems can deliver major savings. UK architects have developed cutting edge solutions to improving performance in these areas. Hygiene remains the most important factor when considering how to handle patients with different infectious conditions. Good design of facilities and materials used within facilities, can help prevent the spread of germs, and reduce cleaning and maintenance costs.
