EDITOR SPEAK

A TRIBUTE TO WOMEN IN HEALTHCARE

W

Md. Afzal Kamal Editor-in-Chief Medgate Today

omen have always been synonymous with healthcare. From our childhood days when we fell sick, it was due to the intervention of women such as our mother, sister or aunt that one was restored back to health. They were the ones who administered medicine and provided suitable nutrition and essential nursing. Although universal and huge, for health and well being of humankind, it used to be a role played away from fanfare and limelight.

The past two centuries though, have seen women emerging from the restrictions of homes to make path-breaking contributions to every aspect of healthcare. “The Lady with the Lamp” – Florence Nightingale laid the foundations of modern nursing by mid-9th century. She wrote her classic book on nursing – the principles she outlined are unchanged till date. She founded the first formal school for nurses and was a principal advisor in the creation of the Royal Army Medical Hospital. She invented the “Rose Diagram” – statistical evaluation of mortality – fundamental to the developments in AI, Blockchain and Cloud for healthcare data-bases, now under development. And thus she became the first female member of the Royal Statistical Society in 1858. Thereafter, in every field of medicine, contributions by the distaff gender followed thick and fast. Despite persecution Margaret Sanger gave the world safe and effective birth control, along with its first birth control clinic and first oral contraceptive. Modern diagnostics was only possible because of two time Nobel Prize winner Marie Curie’s studies of radiation (particularly X-Rays). Along with her daughter, the duo built the world’s first mobile X-Ray machine and radiology unit. She was also the founder of the Curie Institute in Paris, which remains at the forefront of cancer research today. Other outstanding contributions by women have included: identification of HIV as the cause of AIDS (Francoise Barré-Sinoussi); determining the structure of DNA (Rosalind Franklin); understanding neurological disease – dementia, Alzheimer’s, Parkinson’s, schizophrenia and ADHD (Patricia Goldman-Rakic); and developing the Apgar test for infants (Virginia Apgar). The impact made by all these women on modern day healthcare is massive. They pave the way for others to follow – regardless of gender.

HCG CANCER HOSPITAL COLLABORATES WITH INTUITIVE TO DEEPEN ACCESSIBILITY OF ROBOTIC-ASSISTED SURGERY ACROSS INDIA; BEGINS TO INSTALL 4TH GEN DA VINCI SYSTEMS

H

ealthCare Global Enterprises Limited (HCG), the largest cancer care network in India, collaborates with Intuitive to augment the accessibility of new-age surgical technology and robotic-assisted surgery (RAS), across India, including nonmetro cities. As a part of that, the hospital begins to install the 4th Gen Da Vinci surgical robot by US-based Intuitive in Mumbai, Kolkata, and a few other cities to make this technology accessible to a wider population in India. The leading cancer hospital chain has expanded its network, further yielding fruitful results with better clinical outcomes from their existing robotic-assisted surgery units in Bengaluru and Ahmedabad. Since the inception of HCG’s cancer centre network in 1989, the company has been taking remarkable steps towards breakthroughs in cancer treatment and diagnosis, which includes immunotherapy as well as advanced screening techniques, all to envision a world without the fear of cancer. Now with the collaboration with Intuitive, HCG is striving towards attaining the best possible clinical outcome for patients across the length and breadth of the country. Commenting on the collaboration, Dr. B S Ajaikumar, Executive Chairman, Healthcare Global Enterprises Limited said, “It has always been our priority to make advanced treatment modalities accessible to patients not only in the metro cities but also in other parts of the country. Over the years, it can be observed that there are vast disparities in urban and rural healthcare ecosystems due to a lack of resources in the latter, with only 20% of the rural population in India having access to high-quality care. However, we have been doing

our best to provide the same quality of cancer treatment, irrespective of region. Through this collaboration with Intuitive, we aim to further the discourse of quality and precise oncological surgery options of robotic-assisted surgery to the various tier 2 and 3 cities of the country, as well.” Speaking on the collaboration, Raj Gore, CEO, Healthcare Global Enterprises Limited said, “HCG has been the pioneer in delivering cutting-edge technology, to improve cancer diagnosis and treatment. We believe that access to life-saving treatments should not be determined by one's location and by partnering with Intuitive, HCG is helping to narrow the inequality gaps in technology, talent, & treatment outcomes of cancer care in India. The installation of the roboticassisted surgery- 4th Gen Da Vinci systems in cities across India means that patients in both urban and rural areas will have access to Precision OncoSurgery. This collaboration marks a new chapter in HCG's mission of not just extending the life of cancer patients but also improving their quality of life.” The joint effort of HCG and Intuitive Surgical will not only make this advanced surgical technology available to a larger community of beneficiaries but also drive familiarization and awareness about RAS to the medical fraternity across India, including Tier 2 and Tier 3 cities.

SKILL TREE AND USAID-SUPPORTED SAMRIDH JOIN HANDS TO PROMOTE ENTREPRENEURSHIP IN HEALTHCARE SECTOR

T

he SAMRIDH Healthcare Blended Finance Facility, supported by the United States Agency for International Development (USAID) and implemented by IPE Global, has committed INR 5.98 Crore (USD 8,54,541) to Skill Tree Pvt. Ltd., a leading skill training organisation in India, to provide healthcare related skill development and livelihood support to 2,553 youth from remote locations across India, especially Maharashtra. The objective of this project is to motivate and train youth from marginalized sections of society, primarily women, to pursue employment and entrepreneurship in the healthcare sector as their primary career option. The COVID-19 pandemic revealed major gaps in India’s healthcare system including limited medical infrastructure, insufficient number of trained healthcare providers and unequal access to health services across geographies. This brought forth an urgent need to strengthen the health system, especially rural and remote areas. Under the aegis of Healthcare Sector Skill Council (HSSC) and National Skill Development Cooperation (NSDC), the project will impart trainings to develop phlebotomy technicians, dietetic aides and tele-health service coordinators. The certified candidates will set up a total of 14 telehealth centers, take up door-to-door diagnostic tests and opt for wage-linked opportunities in the industry. For the candidates qualified to set up the telehealth centers, Skill Tree will facilitate subsidized loans through various government-led schemes and other financing sources.

A MAN AGED 63, RECEIVES SECOND LIFE THROUGH A SUCCESSFUL HEART TRANSPLANT AT KAUVERY HOSPITAL • In a first-of- its- kind in India, where Impella mechanical circulatory support was given as bridge to heart transplant in an adult. • The individual had a medical history of diabetes, hypertension, massive anterior wall myocardial infarction, ventricular tachycardia (abnormal heart rhythm), and cardiogenic shock.

K

auvery Hospital Chennai, a unit of Kauvery Group of Hospitals, a leading multispecialty healthcare chain in Tamil Nadu, successfully performed a heart transplant for a 63-year-old with cardiogenic shock and a complex medical history. In a first-of-its-kind approach in India, the man was given Impella percutaneous left ventricular assist device (pVAD) , to support his heart until he received a heart transplant. The 63-year-old man had a sudden onset of breathlessness and was admitted to a local hospital two weeks after an episode of severe chest pain, where he was managed for low blood pressure. During the same time, he had episodes of Ventricular Tachycardia ( an abnormally fast heart rate that eventually leads to cardiac arrest), and he was revived through electrical shocks. He was then given multiple medications to improve the pumping function of the heart, despite which he was rapidly deteriorating and was on the verge of losing his life. He was shifted to Kauvery Hospital as an emergency for further treatment. “We immediately moved him to the hybrid cath lab with consent of the family for an emergency angiogram and further treatment. His left anterior descending artery was totally blocked, leading to total irreversible damage, thinning of his left ventricle and extremely poor heart

10 March 2023 w w w. m e dg a te to d a y.co m

function. At this stage, his expected survival rate was less than 10%. As we have a 'Heart Recovery' program, we were able to implant an Impella device immediately to save him and prevent damage to other organs of the body including brain, liver and kidneys. Without the 'Heart Recovery' program, it may take upto 24 to 48 hours for the company to arrange Impella. The Impella pVAD support system removes blood from left ventricle and pumps to rest of the body, thereby allowing the left ventricle to rest and recover, while other organ functions are preserved. This temporary mechanical cardiovascular support system can sustain a patient's heart for up to seven days or longer. He was intubated in the critical care unit for pulmonary edema and low BP, and the Impella worked well,” says Dr R. Anantharaman, Senior Interventional Cardiologist and Lead for Circulatory Support at Kauvery Hospital. Due to reduced pumping capacity of heart, kidneys were initially affected. Once the Impella was given, the urinary output improved and his heart failure symptoms, respiratory functions improved, allowing us to remove him from ventilatory support. However, the heart function did not improve due to recent massive heart attack he had 2 weeks prior to admission, and he was dependent on the Impella support for survival. He was assessed by the Heart and Lung Transplant team at Kauvery Hospital and advised heart transplant or a permanent Left Ventricular Assist Device (LVAD) as a bridge to transplant if there was no donor heart available in time. He was put on the transplant waiting list, and was preparing for

LHVAD simultaneously. He remained stable on Impella and was scheduled for elective LVAD by day 8. However, he received donor heart the day prior to LVAD implantation. The transplant surgery was performed under the Heart and Lung transplant team led by Dr Kumud Dhital, along with Cardiologists, Cardiothoracic Anesthetists and Transplant Critical care team support. Dr. Aravindan Selvaraj- Co-Founder and Executive Director of Kauvery Hospital Chennai, said, “This particular scenario was very challenging as the individual had a complicated medical history and his heart was weakened by the time he reached our hospital. We have used Impella and successfully sustained the heart before the transplant. This was possible due the world class infrastructure and the expertise of the dedicated Heart Recovery program / Heart & Lung Transplant program team available at Kauvery Hospital. Also, Kauvery Hospital is one of the very few centers to facilitate the in-house Impella Implantation. The Hybrid Cath Lab which is well equipped helps in performing highly complex procedures and surgeries in the same room without having to shift the patient and it is also the safest place to perform complex procedures. The best expertise combined with world class infrastructure makes it possible to save lives. I congratulate Dr. Kumud Dhital - Head of Heart and Lung Transplant Unit, Dr Ananthraman, Senior Interventional Cardiologist and Dr. Srinivas Rajagopala- Director Transplant Pulmonology and Lung Failure Unit and the entire team for giving a second life to the 63-year-old gentleman..”
