Project Little Dream (PLD) is a registered charity in Hong Kong that carries out projects and research in rural villages of Cambodia.

Today, 530 Cambodian children are studying in the three schools built by Project Little Dream in separate villages. More than 60 volunteers from Hong Kong participate in PLD projects each year. In 2011, Project Little Dream has expanded into Education, Architecture, and Healthcare teams. Composed of university students interested in public health, the Healthcare Team is a focus group in PLD’s Research and Development Department that carries out related research and projects with the aim of improving living standards of the underprivileged.

Why Healthcare?
Health is the basis of a mortal life, hence the foundation of a population. To empower a community, we must give them the means to appreciate this significance. Healthcare stems from social, cultural and historical background of a place, and is often dependent on the economic growth of society and financial status of individuals. In post-conflict Cambodia, ‘healthcare’ is conveniently neglected due to not simply its lack of contribution towards the economy, but more so its tendency to become a burden before displaced by its reward. Healthcare in developing countries is not glamorous, but is definitely essential. Project Little Dream values empowerment through education. As health is the most vital asset of life, investing in healthcare provides a more solid foundation for the development of other potentials.

2013 Planning: Abstract After two years of research and needs assessments, the Healthcare team hopes to expand our scope of intervention and put forward a primary healthcare scheme addressing issues in rural villages of Cambodia. Our planning involves the following:
SHIELD
A year-long village health campaign with the vision “Deflect and protect” will be launched in Thon Mun Village. The campaign aims to target a wide spectrum of villagers.
Health education
In collaboration with PLD Education team, the subject healthcare will be incorporated into the curriculum of PLD village schools.
Research
Continuous needs assessment and evaluation will be conducted to evaluate project effectiveness.
Volunteer experience
A package with a variety of volunteer involvement is planned to assist young people in discovering their place and vision in helping the underprivileged. 4

Thon Mun Sanitation Facilities In 2012, Project Little Dream continued the architecture project in Thon Mun by constructing a sanitation facility next to the school, with toilets, water harvesting facilities and hand washing areas.

Access to Healthcare in Thon Mun Village. Government hospitals in Cambodia are divided into four main categories in descending order of scale and resources available: National hospitals, provincial hospitals, district hospitals, and commune hospitals. A provincial and a commune hospital are within reasonable distance from the residents of Thon Mun Village.

Facilities Complete as a general hospital, with operation theatres, ICU, radiology department etc. Specialized departments are not present.
Facilities Resembles a clinic with very basic medical equipment and medication. Doctors arrive only when called.
Target group Patients with severe symptoms. Do not welcome minor presenting complaints.

Cambodia 2011: Research Findings The predecessor of the Healthcare team is a research team established in 2011. Research was conducted in the form of a questionnaire, aiming to identify the most pressing local health problems. Focusing on three villages in Takeo Province, our research showed the following: • Water contamination and the lack of sanitation were predominant problems in Takeo, when compared with the rest of Cambodia and South East Asia. • Among all three villages, Thon Mun presented with the most pressing health-related issues.

Cambodia 2012. This trip was bisected into school hygiene promotion and research.
Research Healthcare institution visitations Objective: To understand different sectors of healthcare and the structure of the healthcare system in Cambodia. We visited 12 healthcare institutions from the public, private and NGO sectors in both Phnom Penh and Takeo Province, and were given tours around the facilities. Village needs assessment Objective: To identify specific potential areas of improvement In Thon Mun Village. A survey was conducted in the form of home visits, focusing on basic living conditions, water, diet and nutrition, hygiene, common illnesses and diseases, and lifestyle. Health Check A basic health checkup measuring the village school students’ height, weight and mid-upper arm circumferences was conducted 8
Hygiene Promotion In the light of toilet facilities being constructed by the architecture project, four interactive lessons on basic hand and dental hygiene were organized in Thon Mun Village School.  
Cambodia 2013 Planning. (I) SHIELD Campaign For Cambodia 2013, we are planning to launch a healthcare campaign in Thon Mun Village that will extend to the end of 2014. The campaign is named SHIELD, with the vision of “Deflect and Protect”, and the focus on preventative healthcare. SHIELD is mainly divided into three components:

Curriculum: A list of topics compiled according to needs assessment results from Cambodia 2012 • Basic sanitation (e.g. Hand hygiene) • Dental care • Common Illnesses (e.g. Fever, diarrhea) • Food and diet (e.g. Snacks, nutrition) • Lifestyle (e.g. Smoking, slcohol) • Wound management
*These make up the curriculum for January to June 2014. Contents for the rest of the year will be set after further evaluation and assessment.

A new topic will be introduced at the beginning of each month, and then further reinforced weekly. 9

C. Village-wide Healthcare Promotion Target:

B. Village Health Awareness Day
Frequency: Once every half a year Organizer:
PLD Healthcare Team and volunteers, in collaboration with local NGOs and PLD allies.
Details: Different interactive stations will be set up, each aiming to focus on a healthcare topic. Additional emphasis will be put on knowledge regarding the caretaker role of parents and older siblings.
Distrubuter: Local health promoters
Medical screening stations will also be run by other NGOs. Villagers with identifiable health problems will be adviced to seek furher help.

The evaluation will be done in the form of a survey designed according to the topics in our campaign. The lifestyle habbits of the villagers will hopfully be reflected.
Key performance indicators (KPI) will be used. By processing and analyzing the collected data, we hope to evaluate: - Immediate behavioral change (e.g. No. of times they brush their teeth) - Long-term health improvements (e.g. Number of diarrhea cases per unit time)

Cambodia 2013 Planning. Why Healthcare Campaign? A healthy lifestyle must be encouraged, not just taught, and ideally as a community effort. The purpose of SHIELD is to bring attention to important and potentially lifechanging healthcare concepts, as well as assist them in applying these in real life.
Villagers: Lack Awareness
Project Little Dream: Our assets
Low awareness often leads to poor decision making. For example, oblivion towards the causal relationship between high-sugar diet and high prevalence in diabetes mellitus, results in unhealthy cooking habits. And poor adherence to medical treatment stems from ignorance towards severe consequences.
A promotional campaign allows PLD to confidently make good use of our knowledge in assisting the villagers without exceeding our limits, confined by level of education and qualification.
Villagers: Lifestyle and habits Smoking, binge drinking, consumption of junk food – they are all habits passed on from the previous generation. Some of these are even considered social necessities.
Thon Mun Village School is one of Project Little Dream’s accomplishments. This provides a venue for workshops, as well as a positive appealing factor for mass gatherings. Our presence in the village for the past two years has also built up trust and communication between PLD and the locals, which will be extremely useful in convincing them to consider our suggestions.
Commercial businesses: Market Globalization has allowed for different business opportunities. With the raised public health awareness in developed countries, a lot of businesses profiting from tobacco and other unhealthy products set eyes on developing countries.

Health Education Integration
Needs Assessment
The Healthcare Team will be cooperating with PLD Education Team with designing a new curriculum.
We hope that each intervention plan makes suit the needs of individual village. Needs assessment is therefore performed before deciding on future schemes. Its main objective is to identify the predominant healthcare problems in each village through talking to villagers and conducting a structured survey.
The PLD Education Team was established in 2011 to improve quality of teachers, and to refine the curriculum of PLD schools. The incorporation of healthcare into the school education will ensure that basic fundamental concepts are passed on in the long run. Healthcare lessons will be taught in Thnouh Village School every Sunday. This targets students who are in Grade 3, as well as volunteers recruited from nearby secondary schools.

The needs assessment in Thnouh Village will follow a similar protocol as that done in Thon Mun Village. This is also a great opportunity for volunteers to gain first-hand understanding to local lifestyle and struggles.
Volunteer Involvement. Much emphasis is put into sharing our experiences and vision with volunteers. It is an opportunity for them to explore the field of charity work, as well as a platform to inject new inspiration and ideas in assisting the underprivileged. We realize that volunteers gain more when they have time to assimilate information and prepare for the trip beforehand. Therefore, we will expect more commitment from future volunteers, and they will be given the opportunities to experience the following:

We plan to bring 8-10 volunteers to Cambodia in December 2013.

Hygiene Textbook.
Dr. YK Ching Memorial Scholarship for Service Learning
In the summer of 2012, a group of design students from The Hong Kong Polytechnic University was granted the Dr. YK Ching Memorial Scholarship for Service. This provided them with funds to design and publish some teaching materials for the village schools. Using Prey Run and Thon Mun Village as reference, research and local observation were conducted to identify local needs. The group then decided on the theme – hygiene. Local textbooks were analyzed for mistakes and potential problems. Back in Hong Kong, the chapters and contents were then created, pieced together and translated into Khmer. The Healthcare team will include the publishing of this textbook into our agenda for 2013.

Project Timeline. 2013 Survey Trip
A 9-day trip to Cambodia will be arranged in July for two staff from Project Little Dream’s Healthcare team, and one trainee. The objectives are as follows: • Contact local healthcare institutions and confirm visitation arrangements for the main trip in December 2013. • Arrange and confirm details of intensive training and education with parties involved. • Promote the healthcare workshop in Thon Mun Village. • Introduce our trainee to Cambodia, and PLD’s friendly allies. • Allow our trainee to gain a first-hand experience on working as a PLD staff in Cambodia. • Conduct local surveillance on Thnouh Village, Takeo Province, a potential target group for future PLD healthcare intervention plans.

Healthcare Promoter salary
Notes 1. Contains details of the curriculum designed by PLD, translated and printed for the local healthcare workers, village school teachers, and village authorities. The teaching guide will be translated into Khmer, as healthcare workers and village authorities do not understand English. 10 copies of the teaching guide will be printed and distributed. 2. Materials like models and photos are going to be used for demonstration purposes at the workshops. 3. For promoting the workshop, and facilitating the teaching. 4. To assist in running the workshop. 5. Every household who attends our workshop will receive a sanitation pack, which includes the following: - Toothbrush, toothpaste, soap, bandage, healthcare, leaflets. 6. A local Cambodian in Takeo Province who can also communicate with PLD in English. USD$20/month. 7. 2 local translators who will be assisting us closely in training and educating the villagers. 8.
