Plato said, “Excellence is rare.” While today, excellence has been reduced to a derivative, utilized mostly to signify something of quality, if we view this word within its Ancient Greek context, we find that what Plato said remains true.

In its purest form, excellence is a practice, and an ideal wrapped in reverence for an achievement utilizing clear vision and raw determination in order to reach higher than anyone previously.

When Plato spoke of excellence, he was telling a great story.

Weathering seasons of economic uncertainty, the AIDS epidemic of the 1980s, and an on-going battle against the daunting stigma still surrounding the word “hospice,” Providence TrinityCare Hospice and its Foundation has persevered, delivering support for the comfort and care of those who need it, regardless of race, religion, or financial status, for decades. Clearly, TrinityCare has one of those great stories, which is why the Foundation has chosen “excellence” as our theme for our 2011 Annual Report.

Certainly, 2011 was not without its challenges, but we came through it stronger, and imbued with a renewed sense of determination to push on to bigger and brighter dreams of care, education, and even a new location. But this story would not be told, our dreams would not be feasible, and 2011’s accomplishments would not have been possible were it not for you, our loyal friends, our community, our volunteers, and our donors.

In 2011 Providence Trinity Care Hospice (PTCH) and Providence Trinity Care Hospice Foundation (PTCHF) took some important and exciting steps, ones that I truly believe will help ensure that we are One Ministry, Committed to Excellence, and I’d like to take a moment to share more about them.

Up until very recently, Providence TrinityCare Hospice and its Foundation utilized two boards, which have worked separately to ensure the goals of our ministry were met. However, with so many exciting developments on the horizon, and as we developed our Strategic Plan, the Board of Directors of PTCH and the Board of Trustees of PTCHF worked closely together and began meeting together more frequently.

After sharing a great deal of time, energy and information, careful deliberation that included planning sessions and interviews of directors, trustees and leadership of the organizations, and after a tremendous effort put forth by the Strategic Planning Task Force, both boards unanimously voted to change our governance structure and merge into one Board. PTCH and PTCHF share the governance of our two non-profit corporations. While this is a high-level policy board, the directors also lead subcommittees focusing on the different specific areas to advise senior management on the direction and the strategy of our organizations.

The new Providence TrinityCare Board grew out of years of strong volunteer leadership, most-recently from Jim Zapp, Chair of PTCH Board of Directors, and Scott Sharpe, Chair of PTCHF Board of Trustees. These leaders, our Strategic Planning Task Force chaired by Ned Mansour, and the members of each board, selflessly examined what would be best for our patients, and our families, and what governance structure would make our beloved organizations most effective in fulfilling our mission.

It is with heartfelt appreciation therefore, that I acknowledge the work of our volunteer leaders – especially the chairs – in guiding us into a new structure, and a new era of governance.

We look forward to the future of Providence TrinityCare and are confident that our new Board of Directors will guide us in the right direction.

EXCELLENCE IN CARE The Center of Excellence In 2011 Providence Health & Services and Providence TrinityCare Hospice were excited to announce official plans for the Palliative Care Education Institute, also known as The Center of Excellence. As you know, we here at Providence TrinityCare Hospice work hard to make sure that everyone who comes to us for excellent care receives it regardless of race, religion, age, or financial status. Thanks to you, our amazing supporters, that remained financially possible in 2011. And thanks to our visionary Chief Medical Officer, Dr. Glen Komatsu, it was medically possible.

Dr. Komatsu has maintained PTCH’s cutting-edge position in Hospice and Palliative Care, exceeding expectations, and in cases defining “excellence” in pain and symptom management, especially within TrinityKids Care, the only hospice service dedicated to children in Los Angeles and Orange counties.

Starting in 2012, Dr. Komatsu and PTCH will begin to initiate programs with a strong foundation of curriculum which will be available to our colleagues throughout the medical industry. This is our goal and the goal of the Center. The need for education is great. Palliative Care is a growing medical specialty and we are fortunate to be experts in this field. Under Dr. Komatsu’s educational leadership, we will be able to train clinicians across the world and help those who spend their lives dedicated to this mission.

EXTending CARE to Children Concurrent Care No parent wants to think about their child being forced to face a life-threatening illness. What may be even harder for some to believe is that parents have been placed in the impossible position of choosing between hospice and palliative care, and curative measures. Because parents should never be forced to choose between alleviating the suffering of their child and the possibility of a cure, TrinityKids Care is thrilled to be part of a new federal program. TKC is now able to provide hospice services for children on Medi-Cal at the same time the family continues to seek curative treatments.

“Partners for Children” In 2011 TrinityKids Care was honored to continue and expand its participation in “Partners for Children,” a California pilot program for children on Medi-Cal who suffer from specific chronic, life-threatening illness. Established in 13 counties throughout the state, and commonly referred to as “The Waiver,” this program aims at providing families with a care coordinator strongly emphasizing the quality of life. For qualified participants and families on the Waiver, care coordinators integrate pain and symptom management with programs such as counseling, art and music therapy, and respite care, which address the less-tangible ways our patients and families are suffering through this difficult time.

The Scriba Family Endowment Fund Providence TrinityCare Hospice Foundation is honored to announce the unveiling of The Scriba Family Endowment Fund – our very first donor directed endowed fund, since the inception of the Foundation in 1987.

Our relationship with the Scriba Family has been a consistent source of support and friendship for over 20 years. This past year, Mr. Scriba and his family displayed a dedicated desire to make a contribution that would set an example for a solid pattern of community commitment to Providence TrinityCare Hospice Foundation. The Scriba family chose to establish an endowment fund.

An endowed fund is a sum of money that acts as a financial anchor for a non-profit. While the principle amount of the original sum stays intact, the interest that the fund accrues may be put to work for the foundation.

The primary donor, if they wish, can choose the way in which this interest is used to support our many programs. In addition, other donors can add to the endowment, thereby increasing the amount of income earned.

Having endowed funds positively impacts other funding sources. To other entities in the non-profit sector, no matter how long a non-profit has been in existence, an endowment is the ultimate signifier of a foundation’s sustainability. Therefore, having endowed funds will allow us to successfully apply for grants larger than we had in the past. Grants at this new level could help support our new endeavors such as The Institute of Education for Hospice and Palliative Care, as well as richly aid our other existing programs.

We are grateful for this gift and the possibilities it will bring to Providence TrinityCare Hospice Foundation, and ultimately, our patients and their families. Thank you!

EXCELLENCE IN OUR COMMUNITY...

Caring Hearts, Healing Hands Thanks to the Tarsadia Foundation, who provided the startup support for much-need supplies, The Gathering Place was able to launch Caring Hands, Healing Hearts in late 2011. This therapeutic program allows children who have lost their sibling to express their grief, their memories, and their thoughts about their family through art exercises. To pilot this program we collaborated with “Walk with Sally,” who trained and provided some excellent volunteers in the early phase of the program.

Our volunteers, who often work with more than one sibling at a time, guide children through a curriculum filled with color, shapes, textures and pictures in the family home. In these sessions, children are reminded that their sibling always will be a source of inspiration and love in the family.

COMMUNITY ORGANIZATIONS What would we do without our friends in the community? We are grateful to so many organizations that play such a large role in the PTCHF story. Palos Verdes Juniors, National Charity League, Las Candalistas, Vistas for Children, Rolling Hills Country Day School and Sandpipers have donated a steady stream of supplies and funds each and every year. From Walk on the Wild Side, to For Our Children, to adopting families, our community groups meet everything from the basic necessities of life for our patients and their families, to sustaining our growing programs and services.

Toyota 100 Cars For Good In early 2011 the Foundation was invited to participate in the “Toyota 100 Cars for Good” contest. Toyota was looking for nonprofits across the country that were in need of a vehicle to help carry out their respective missions. At the moment of the submission, had you asked anyone on the Foundation team about what they felt the odds were that TrinityKids Care would win a van, they would have said that it was a long-shot. But sure enough, the TrinityKids Care team found themselves shooting a mini- movie with a 100 Cars for Good Flip phone, and before we knew it, thousands of people had rallied around TKC. Our amazing community, plus many individuals who had simply followed Toyota’s campaign voted on Facebook, making us the winner of a 2011 Toyota Sienna van with a specialized seat to accomodate a child with mobility issues.

AND OUR FRIENDS Honda Presents the Annual For Our Children Food and Wine Festival 2011 was the first year that TrinityKids Care was chosen, along with Sandpipers, as the co-benefactor in the For Our Children Food and Wine Festival presented by Honda (FOC). The event raised over $70,000 each for both TKC and Sandpipers philanthropic programs. What began as a humble wine and cheese fundraiser, has blossomed into one of the community’s most-anticipated events and continues to make a difference in the lives of children throughout the South Bay community.

Songs for a Sunday For Jeralyn Glass Refeld, an internationally celebrated vocalist, striving for excellence is an undertaking easily understood.

Perhaps that is why Jeralyn has been gracious enough to put together Songs for a Sunday three years in a row for PTCHF. On a beautiful Sunday in January, accompanied by pianist Christoph Weinhart, 200 family and friends of Providence TrinityCare Hospice Foundation once again witnessed the fruits of Jeralyn’s excellent career.

Sunday By The Sea Food and WIne Tasting Gala For many years, our annual food and wine gala, Sunday by the Sea, has invited guests to enjoy premier restaurants and beverage purveyors, and a fabulous auction.

Many of you have come to enjoy (and count on) Sunday by the Sea, which netted $200,000 in support of the programs and services of Providence TrinityCare Hospice in 2011. But there is someone connected to this event who you may not be aware of – Gloria Mata. As property manager for the Elliott family, hosts of Sunday by the Sea for the past eleven years, Gloria is the cornerstone of the September event. Gloria runs the affairs of the home, and according to Carolyn Elliott, there would not be a Sunday by the Sea at the Elliott’s without Gloria. “We depend upon her for so many things. As manager of our home, Gloria is there to work with TrinityCare in all the logistics of the event. She has a heart of gold and she’s a hard worker, coordinating the coming and going of all the volunteers and vendors. She treats everyone with the utmost respect and is a joy to be around,” says Mrs. Elliott. We couldn’t agree more. Thank you Gloria!

Trees Light Up a Life Throughout December, trees at the Westin Long Beach Hotel, Red Car Brewery in Old Torrance and Unionbanc Investment Services in Sherman Oaks were filled with special stars donated in memory of lost loved ones. Gifts from these three special trees totaled over $30,000, and were used to provide the comfort of hospice and warmth of the holiday season to the lives of our neediest patients and their families all year long.

In 2011, TrinityCare Hospice served more than 2,000 patients and their families, including 156 children on hospice. We cover more than 4,000 square miles across Los Angeles and Orange counties to provide comfort and care where it’s needed – in the homes and communities in which our patients and their families live.

Jacqueline M. Glass Awarded HONORS

The Providence TrinityCare Hospice Foundation team would like to offer a special thanks to all PTCH employees.

Through thick and thin, our employees have consistently taken our core values of Respect, Compassion, Justice, Excellence and Stewardship to heart, bringing them into the homes of the patients and families we serve.

It is our employees who carry-out our mission of excellence and truly write our story, making the job we do here at the Foundation successful and rewarding.

Our employees were responsible for over $30,000 in funds raised during the most recent campaign.

2011 was a year filled with well-earned recognition for Jacqueline M. Glass, whose 25-year-long relationship with our foundation has had a profound effect on our foundation, and the hospice and palliative care fields.

Evelyn Worthy-Collins, a PTCH nurse has been giving back to this foundation since she joined us. Having worked with patients of all kinds she says that in hospice, she believes that she has found her niche.

“Everything I’ve ever done in nursing has been about asking, ‘Have I done all I can to make my patient comfortable?’

In hospice work, I was finally was able to really do that…In our mission statement it talks about excellence – this is what we are supposed to do.”

In early 2011, Providence TrinityCare Hospice Foundation celebrated with over 130 longtime friends and donors at the Embassy Dinner. Held at the Westin Long Beach Hotel, a special honor was bestowed upon our Community Ambassador, Jacqueline M. Glass and her loving family for their dedication to and heartfelt support of the many programs and services of Providence TrinityCare Hospice Foundation. This award, now named the Glass Family Award for Philanthropy, Leadership and Service, will continue to be presented annually.

In October, the California Hospice and Palliative Care Association honored Jacky with their prestigious Gift of the Heart award. Later in the year, it was announced that Jacky would be presented the Mother Joseph of the Sacred Heart Award by the Sisters of Providence, given to recognize a volunteer who exemplifies the values and courage of Mother Joseph, a woman known to be of extraordinary compassion and vision who developed charitable services for those suffering the difficulty of life on the frontier. (award shown below) Like Mother Joseph, as a volunteer, Jacky is a pioneer. When she began her work in hospice as a bereavement volunteer and patient care volunteer for women and children living with AIDS, hospice was in its fledgling state. Now after 25 years of service, we commend Jacky and her family, and look forward to the future.
