GPs team up to make a difference

A WANGARATTA GP has teamed up with a metropolitan counterpart in a ‘matchmaking’ national pilot program aimed at improving access and quality of care for patients. Dr Kate Davey from Ovens Medical Group is working with Melbourne-based Dr Anita Munoz, chair of the Royal Australian College of General Practitioners (RACGP) Victoria, as part of the Practice to Practice program. The RACGP initiative, which kicked off on March 1 and runs until December 31, aims to connect practices and GPs in diverse geographical areas, with urban practices and GPs being matched with those in rural and remote Australia to share skills and support, and maximise quality services for patients. RACGP rural chair Dr Michael Clements, who practises in Townsville, said the pilot program was off to a flying start, with more than 40 practices submitting expressions of interest before the launch. “As Australia’s largest professional general practice organisation representing urban, rural and remote GPs, the RACGP is perfectly placed to facilitate these connections,” he said. A GP in Wangaratta for 38 years, Dr Davey was approaching retirement and facing a problem all too common for rural GPs: nobody wanted to take her rural job, and she didn’t want to leave her patients without someone to care for them. So, after hearing about Practice to Practice, she teamed up with Dr Munoz to develop a model aimed at building local relationships and supporting interested metropolitan GPs to work and live in Wangaratta.

Backyard caterpillars LOVE them or hate them, caterpillars are an important part of the Australian environment. They can bring joy in the form of the promise of a beautiful butterfly, despair as they devour tender young broccoli plants, or itching and pain as a spitfire caterpillar brushes against your bare skin. All caterpillars turn into either moths or butterflies. Australia is home to roughly 400 known species of butterflies, about 10,000 known species of moths and probably as many moth species again yet to be described. But some caterpillars aren’t really caterpillars at all. Certain insect larvae are often mistaken for caterpillars. These include saw fly larvae, witchetty grubs, leaf beetle grubs, ladybird larvae, cherry slugs and Bardee grubs. The way to tell the difference is to check out the legs. Caterpillars have 16 legs: six true legs plus an extra 10 small legs called prolegs. Insect larvae on the other hand only have six legs. Be on particular lookout for spitfires in your backyard. These caterpillars don’t actually spit but rather, have pockets of stinging spines that they stick out when they feel threatened.

If you have chooks, they love caterpillars , or you can cover your patch of seedlings with soft mesh over a frame to keep the Cabbage Whites from laying their eggs and the hatched caterpillars eating your veggies. Caterpillars in the family Papilionidae and Lasiocampinae have a different defence: they can suddenly produce a pair of soft ‘horns’ from behind their head which make a pungent, aromatic smell but are entirely harmless. Did you know that caterpillars are great escape artists? Although they don’t have the luxury of running away from a predator, they can bungee jump instead. When threatened, many caterpillars drop off the leaf but remain attached by a fine piece of silk. When the coast is clear, they can climb back up to safety. Get to know the local species of butterfly in your area and establish plants in your garden which will provide food for their caterpillars.

John Gehrig Wines King Valley Gamay 2017 WHEN the heat is on, it can sometimes be tricky for red lovers to find a wine to share with friends that will work alongside their barbecue lunch. Refreshing whites will always have their place on a hot day and some fruitiness and mineral acidity is great with chicken or seafood, but frankly it’s not much chop with a steak. Even beefy snags or burgers have a depth of flavour and earthiness that is calling out for something which will complement their fatty texture, without being washed away. A great option might be John Gehrig Wines unoaked Gamay. Made by fifth generation winemaker Ross Gehrig on site in Oxley, this wine is a lighter, more aromatic alternative to heavier style reds, without dialling down on flavour. This Day in History, 1891 BEFORE the days of motion pictures, the next best thing was a Cyclorama, a huge mural of some particularly gripping episode in history, which surrounded the viewer. Thus it was that the 1891 Cyclorama of the Battle of Waterloo in Melbourne drew huge crowds. The promotors of the Cyclorama discovered, in the Beechworth Benevolent Home, a soldier who had been involved in the battle. His name was Jeremiah Brown and he was just short of this ninety-ninth birthday. They found an old uniform for Jeremiah and gave him a place of honour in the display, where he told wide eyed boys of all ages of his adventures as a 14 year old at the Battle of Waterloo.

* Move away from any background noise; * Speak slowly and clearly, clarify what was heard if you are having difficulty understanding; * For video appointments, remove any face coverings (if possible) and sit facing a window or have adequate lighting as poor sound quality can be compensated by lip-reading; * Have a notepad and pen ready with questions or to scribble down any notes; * Use a speakerphone for a clearer signal or for another person to participate. If the volume is not adequate, try placing the phone in a bowl to amplify the volume; * Use detailed visual descriptions of your problem wherever possible. Dr Davey said the innovative ideas behind Practice to Practice were appealing as a means to overcome the difficulty involved in attracting GPs to the local area. “We are hopeful that enabling metropolitan doctors to come and experience working in the rural environment may encourage them to come and join our workforce on a more permanent basis,” she said. “I believe that this new program may appeal to doctors who had not previously thought of working in the rural environment. “It was with Anita that I first started discussing the challenges that we were facing with succession planning in the rural environment, which became apparent when the standard job advertisements were eliciting little if any interest. “Our model is still evolving; we are working together with the Rural City of Wangaratta and a variety of other stakeholders,

including RACGP, Rural Workforce Agency Victoria, Melbourne University and Northeast Health Wangaratta and other general practices, to develop a support network for the prospective medical workforce. “It is hoped that this will include information about accommodation, schools, sporting and leisure activities, job opportunities for medical partners and other ideas that may be attractive to applicants.” Dr Davey said she believed the “whole of community approach” was likely to be successful. As well as hopefully attracting metropolitan GPs to work in this area, the program’s ability to foster partnerships between rural and metropolitan practices is expected to lead to a greater understanding of the challenges in both environments. “I firmly believe that sharing our ideas will lead to benefits for both the GPs and their patients,” she said.

“I think the Practice to Practice program is an exciting new idea, and I would encourage other general practices to register so that we can offer a broad suite of opportunities to our metropolitan colleagues.” Dr Davey said while retirement was on her radar, it was not in the immediate future. “I am trying to plan a smooth transition to the new generation of GPs who will care for our community into the future,” she said. Practices and GPs can register their interest to be involved in Practice to Practice on the RACGP website. The RACGP will facilitate the pilot program, providing guidelines and resources for building the relationship between partner practices, recommending training courses for upskilling, and promoting the program among its membership of more than 41,000 GPs in urban, rural and remote Australia.

GONE FISHING

◆ GOOD SIGNS: A small silver perch caught in Merriwa Park Lake on Sunday morning using a bunch of worms and a paternoster rig. Silver perch have a minimum size limit of 30cm in these dams and are totally protected in rivers.

Ponds, dams offer some good fishing alternatives WELL the summer that we never had has ended, paving the way to some great autumn fishing. I have been having a great time fishing in some of the many small ponds around the region that are stocked with fish. Most of my pond fishing has been at Merriwa Park Lake, Eldorado Dredge Hole and Stanley Dam. I have been catching a few silver perch in Merriwa Park Lake, as well as a few carp. I have caught a lot of redfin in the Eldorado Dredge Hole although they have been quite small. In Stanley Dam I have caught a number of both rainbow trout and golden perch (yellowbelly). My fish have all been caught using worms with a small hook and sinker. Some have been caught using a paternoster rig, and some have been caught fishing underneath a bubble float. If you go to the Victorian Fisheries Authority website( vfa.vic. gov.au) you can find a full list of these small ponds that have been stocked with fish. Others in the area include Tronoh Dredge at Harrietville, Lake Sambell at Beechworth, Allan’s Flat Waterhole at Yack-

I have heard that it has slowed down a little bit, however, a few redfin are still being caught and it is worth fishing. Lake Nillahcootie has been producing some nice Murray cod and yellowbelly, along with the odd silver perch which are a reasonably new addition to the fish population at Lake Nillahcootie. The Ovens River in Wangaratta has been producing a few Macquarie perch which are protected and must be released, as well as a few small yellowbelly and Murray cod, although the cod fishing has slowed down quite a bit over the last two weeks which is quite common for this time of the year.
