Mark manages rehabilitation units to exceptionally high standards of care under often tough circumstances. He oversees rehabilitation and conditioning for servicemen and women in the UK and overseas.

Row2Recovery is the story of four servicemen with five legs and seven arms between them and their attempt to row unsupported across an ocean as part of the Talisker Whiskey Atlantic challenge in 2014 - a true story that demonstrates disability does not define what can and cannot be achieved. Over an exhausting 48 days, 9 hours and 13 minutes at sea the team completed their challenge landing on the Caribbean island of Antigua, finishing in an amazing third place overall! “When you put yourself forward for a challenge like this, there is always a good chance of failure. But the journey has its own rewards and our example proves what can be achieved if you take one step at a time.”

It is generally accepted that there are 0.1% of the population in all developed nations that need an artificial eye. We are a small group of experts in this field with clinics/laboratories in the UK and other European centres. The founder of the European College of Bowen Studies - he has taught thousands of students The Bowen Technique, a non-invasive system of fascial release. He is the author of two books on Bowen and fascia and has contributed hundreds of articles about bodywork, fascia, dissection and the human form. Julian has presented widely at conferences around the world and is on the organising committee of the British Fascia Symposium. Since 2006 he has been leading human dissection courses throughout the UK focusing on fascia and connective tissue structures, rather than standard anatomy. From these studies we are able to see that the traditional way of teaching how and why the body works needs adjustment and a shift in understanding.

WEDNESDAY | 14.45 | SEMINAR THEATRE 7 functional brain imaging to understand the mechanisms of recovery for movement after stroke so that he can predict both optimal treatments of upper limb impairment and long term outcomes after stroke.

Anatomy teaching started 300 years ago and has changed little since then. We still learn our gross anatomy from twodimensional images in books. Is it time for a more functional approach and what would this look like? Traditional anatomy is reductionist and doesn’t address the whole body. Our functional understanding is often limited in its approaches and leaves a gap in many therapeutic practices. A wider understanding of body wide relationships is essential for the clinician.

THE QUEEN SQUARE UPPER LIMB NEUROREHABILITATION PROGRAMME Upper limb rehabilitation after stroke is neglected. Consequently, there is a nihilistic view about what clinically relevant changes are possible. Nick will discuss the outcomes of patients admitted to the Queen Square upper limb rehabilitation program. Sharing results that suggest that chronic stroke patients can change by clinically important differences in measures of both impairment and function following intensive upper limb rehabilitation. Interestingly, continued benefits were often seen for at least six months beyond the programme. Ongoing work will identify characteristics of those patients most likely to respond to intensive therapy; determine the effect of this intervention on quality of life (Stroke Impact Scale) and describe the key elements in the programme - time on task, repetitions, category of task (e.g. active, sensory, functional, education, self-directed learning).

She offers tailored expert advice to all Cytoplan customers and support to practitioners and students. Helen has a BSc (Hons) in Biological Sciences from Lancaster University and a Diploma in Nutritional Therapy from the College of Naturopathic Medicine where she graduated in 2009.

As a BANT and CNHC Registered Nutritional Therapist, Helen has worked in private practice in both South East London and Kent, working with clients suffering from a wide range of conditions including fertility and hormonal imbalances, digestive issues, skin conditions, arthritis and weight management. In September 2012, Helen also became a lecturer at the College of Naturopathic Medicine in both London and Brighton.

David Clancy pursued his interest in sports medicine after having represented Ireland under-age in basketball. Upon completion of a Physiology and Health Science certificate in Carlow I.T. and an under-graduate degree in physiotherapy in Keele University, David completed the MSc Sports Medicine in Trinity College Dublin in 2009. David moved to London in 2013 where he has worked for Isokinetic Medical Group, FIFA Medical Centre of Excellence as a chartered physiotherapist specialising in sports medicine, rehabilitation and movement science.

THE ROLE OF NEURO-MOTOR EXERCISE TRAINING IN THE RETURN-TO-TRAINING STRATEGY Neuro-motor training (NMT) is the crucial, usually under-valued missing link in the rehabilitation pathway. It involves targeting motor skills such as coordination, agility and proprioception. A new direction for NMT to optimise return-to-training strategies involves focusing significantly on reforming correct movement patterning schemas earlier and comprehensively during the rehabilitation process. This new paradigm of training should help reduce the potential for reinjury or secondary injuries upon return-to-play.

The force between the foot and the shoe is accurately measured any time the foot or part of it is making contact with the ground. Other systems are commercially available: they have smaller, localised pressure sensors with large gaps between measurement areas, causing the total force to be merely an estimation. For rehabilitation or biomechanic applications, in which the distribution of localised loads is not of interest, but rather the total load on the foot is of importance, the new Pedoped® system is the optimal solution.

DIAGNOSIS OF MUSCULAR FUNCTION USING JUMPING MECHANOGRAPHY Muscle function is a vital component of healthy living. It can be negatively affected by ageing or diseases. Decreasing muscle function is a main risk factor for disability, frailty, hospitalisation and premature death. Many tests to assess muscle function are based on isometric or isokinetic principles. These tests determine muscle function at no or slow speed and in a guided movement. Maximum force is not accessible with these methods but only during eccentric movements. Mechanography is a clinical tool where you can quantify and follow-up within five minutes with clinical relevant outcomes.

Cryoablation is a technique used to destroy or damage tissue. It has been utilised in the medical field for many years and involves delivering extreme cold to the tip of a fine needle to destroy targeted tissues. It has been used for many diverse conditions including tumours, neuralgia and arrhythmias. The procedure involves infiltration of a local anaesthetic into the foot and inserting a 17-Gauge needle under ultrasound guidance. It is an ambulatory day case procedure.

In this seminar, 3rd year PhD student Alix will introduce a new method she has developed for the visualisation of patterns in upper limb activity over time. This is the first study to use activity monitors to assess prosthesis usage. These methods allow researchers and clinicians to quickly establish the level of reliance on the anatomic hand, and detect patterns in prosthesis use/ non-use. Having gained a grounding in Medical Engineering at the University of Bath,Alix now works with a multi-disciplinary team including Engineers, a Prosthetist and a Psychologist as part of Salford’s Rehabilitation Technologies and Biomedical Engineering Research Group. To date, Wagih El Masri has personally treated around 10,000 patients with spinal injuries, personally managing all the medical, surgical and non-medical effects of paralysis with a team of knowledgeable and experienced healthcare professionals. NEUROLOGICAL RECOVERY FOLLOWING TRAUMATIC SPINAL CORD INJURY The history of neurological recovery following traumatic Spinal Cord Injuries (SCI) show that the severity will be presented within the first 48 hours of injury. Wagih will demonstrate that with adequate management of the biomechanical instability of the spinal column, the physiological instability of the injured spinal cord and the handling of the physiological impairment and malfunction multi-system - leads to the majority of patients with complete motor loss to be spared with reduced severity of sensory damage and those with motor sparing, however little, able to make a significant neurological recovery. Wagih will demonstrate how this recovery occurs irrespective of the degree of both the biomechanical instability at the site of the fracture and the degree of traumatic cord compression - without the use of surgical stabilisation and surgical decompression, or any other intervention.

Dr Tacson Fernandez is a chronic pain management specialist and lead clinician for neuromodulation services and acute pain services. Dr Fernandez also provides anaesthetic services for complex spinal surgery and regional anaesthesia for upper limb surgery. Dr Fernandez set up the Neuromodulation Centre at the Royal National Orthopaedic Hospital, offering complex chronic pain patients the option of using spinal cord stimulators and peripheral nerve stimulators in accordance to NICE guidelines (TA159). Maintaining an active interest in neuromodulation in complex chronic pain patient groups, Dr Fernandez areas of interest also include acute

SPINAL CORD STIMULATION IN COMPLEX CHRONIC PAIN PATIENT GROUPS This talk explores the evolution and management of chronic pain in select complex patient groups who gained significant benefit following the implementation of a spinal cord stimulator at our Neuromodulation centre. It provides insight into the complexity of these patients, the mechanisms of chronic pain and role of neuromodulation (spinal cord stimulators). Wearable technologies to support self-management can increase intensity of therapy. An example is M-MARK, a wearable system to support self-management of arm rehabilitation post-stroke. It provides therapists with detailed objective assessments of impairment such as range and smoothness of movement, muscle synergies and motor control. The neuroscience and motor learning principles are currently experiencing the M-MARK concept, with the research processes and key findings being presented at the seminar. M-MARK comprises a garment with embedded sensors measuring mechanical muscle activity (mechanomyography) and movement (Inertial Measurement Units).

CREATING APPROPRIATE ENVIRONMENTS TO ENABLE ACCESS TO TREATMENT AND HEALTHCARE Providing an appropriate and safe environment to provide treatment for someone with complex needs, brain injury, movement disorders or challenging behaviours can be problematic. Gillian will talk about how to create customised, soft, safe, adaptable care environments with particular reference to high sided beds and safe rooms. These spaces can be adapted to meet a range of needs, to provide a low or high stimulation environment for treatment, rehabilitation or therapeutic work. Gillian can explain how a soft space can be used for sensory integration work.

FES is an effective intervention for dropped foot and has a mean usage of 4.9 years. The National Institute for Health and Clinical Excellence applies a cost-utility ceiling of £20,000 per Quality Adjusted Life Year (QALY) for an intervention to be considered. Previously the QALY gain derived from the effect of FES on walking speed gave a cost per QALY of £15,406. However, analysis of new data, presented at this seminar, gives a mean cost per QALY of £5,705 making it comparable to the cost per QALY of hip and knee replacements. The above analysis was carried out using FES with people who had MS or who had had a stroke. Recently we have been investigating the use of FES for the reduction of Bradykinesia in Parkinson’s Disease in a randomised controlled trial (n=68) funded by NIHR. The trial protocol, treatment methods and preliminary results will be presented in the seminar.

He considers combining these into a patient’s recovery and a return to activities of daily living to be essential in providing the best medical care for patients. “There is a current trend to promote a ‘hands on only’ approach to physical therapy these days” Cliff says. “Many physical therapists I have spoken to cite lack of understanding or the lack of evidence to support the efficacy of using electrotherapy” he continued. “Due to the reduced emphasis placed on electrotherapy at undergraduate level for physiotherapists the former may be unsurprising, human nature being what it is. It will be understandable that newly qualified physiotherapists may be anxious about using electrotherapy; a lack of evidence there is not. There is, however, a lack of awareness and understanding of the relevant literature and that is what we wish to promote on these courses. In this way we aim to ensure that therapists feel confident in using electrotherapy within their treatment paradigms”.
