PRINCIPAL INVESTIGATOR FOR THE FIRST TRUST IN THE UK TO USE NONINVASIVE ULTRASOUND FOR BRAIN SURGERY Professor Wlayslaw Gedroyc graduated from St Mary’s Hospital Medical School and is an accredited radiologist. He is a long-term researcher in the field of interventional MR and was the principal investigator for the UK’s first ever deep brain surgery whilst using MRI focused ultrasound. Currently, Prof. Gedroyc has multiple projects underway, investigating the utilisation of focused ultrasound with non-interventional means of patient treatment. THURSDAY | 11.45 | THEATRE 4

To date, Wagih El Masri has personally treated around 10,000 patients with spinal injuries, personally managing all the medical, surgical and non-medical effects of paralysis with a team of knowledgeable and experienced healthcare professionals.
The natural history of neurological recovery following Traumatic Spinal Cord Injuries (SCI) of various severity on presentation within the first 48 hours of injury will be presented. I will demonstrate that with adequate management of: the biomechanical instability of the spinal column, the physiological instability of the injured spinal cord and the multisystem physiological impairment and malfunction, the majority of patients with complete motor loss but with sensory sparing and those with motor sparing however little will make a significant neurological recovery. I will demonstrate that this recovery occurs irrespective of the degree of biomechanical instability at the site of the fracture, degree of traumatic cord compression, without surgical stabilisation, surgical decompression or any other intervention.
Throughout his career, Wagih has contributed to over 135 publications - covering a wide range of the aspects of spinal cord injuries and in particular, neurological recovery following traumatic SCI. Among various national and international offices, he was Chairman of the British Association of Spinal Cord Injury Specialists and President of the International Spinal Cord Society. Founder Member of the following charities: SPIRIT for education and training in spinal injuries (Chairman of Trustees), Transhouse for interim accommodation between the hospital and the community (President) and the Oswestry Spinal Injury Trust (Chairman of Trustees). All whilst being a member of the Trust Board of the Institute of Orthopaedics in the RJAH Oswestry.

WORLD’S LEADING SKULL BASE SURGEON AND THE PIONEER OF ENDOSCOPIC SKULL BASE SURGERY IN THE UK
Dr Hugo Spiers is a world-renowned expert on the neural basis of spatial navigation. He holds a PhD in Neuroscience from University College London, where he is a reader in Neuroscience. Hugo is the group leader of the Spatial Cognition Laboratory at UCL where his team use virtual reality, magnetic resonance imaging and single cell recording to develop an in-depth understanding of how brain circuits support spatial navigation.
DEVELOPING A MOBILE APP TO HELP DIAGNOSE SPATIAL NAVIGATION PROBLEMS IN EARLY STAGE ALZHEIMER’S DEMENTIA The seminar will introduce the audience to the app Sea Hero Quest, a mobile game that tests spatial navigation skills. From May 2016 – August 2016 more than 2.2 million people have played the game across the world. This provides an unprecedented baseline to compare patients with Alzheimer’s dementia against. We not only hope the test will be useful for diagnosis, but also beneficial for assessing decline or improvements in performance in potential drug trials.
DR MICHAEL DILLEY
Dr Mike Dilley has years of expertise and experience working within neurorehabilitation, especially caring for traumatic and acquired brain injuries. In this seminar, Dr Dilley will discuss the full extent of the behavioral problems that occur after a Traumatic Brain Injury, covering the various dysfunctions and abnormalities that can instigate for a sufferer. He will explore and explain the importance of managing the pharmacological treatment given to TBI sufferers to coincide with helping with the behavioral problems carers face.

Nick has been working as a consultant neurosurgeon at King’s College Hospital for 19 years, specialising in expert skull base and pituitary surgery. He works with neurosurgical colleagues in the renowned collaborative group called the London Neurosurgery Partnership. Nick serves as President for The British Skull Base Society, is on the committee of The European Skull Base Society and regularly travels abroad to lecture and teach in places as far-reaching as Sri Lanka, where he spent 20 years doing voluntary work.

Dr Mike Dilley, Consultant Neuropsychiatrist in Neurorehabilitation, has specialist expertise in both traumatic and acquired brain injuries and severe and complex functional neurological disorders that require multidisciplinary inpatient rehabilitation. Dr Dilley provides input across the entire integrated care pathway at St George’s Hospital for patients with neuropsychiatric who are under neurological intensive care, on the acute brain injury unit. He cares for inpatients at the Wolfson Neurorehabilitation Centre at Queen Mary’s Hospital and the outpatients at The Wolfson Neurorehabilitation Centre, Wolfson Vocational Rehabilitation Programme and Wandsworth Community Neuro Rehabilitation Team.
FURTHER ADVANCES TO ENDOSCOPIC ENDONASAL APPROACHES TO THE SKULL BASE Safe access to the skull base is achievable with endoscopic endonasal approaches, allowing direct access to certain pathologies in a way that is not possible with more traditional open techniques. These are complex operations requiring unique neurosurgical skills and an understanding of the anatomy from a novel direction. Surgical techniques, anatomy, image guidance, neuromonitoring and educational tools will all be discussed.

Amy Bean graduated in 2002 with a BSc honours in Physiotherapy and completed her Master’s degree in stroke rehabilitation at York St John University in 2012. Amy has worked in a variety of clinical settings both in the UK and in Australia. Having left the NHS in 2010, she became clinical specialist for Saebo UK after observing first hand the clinical benefits of their products. Amy has since further expanded upon her clinical experience by working with and developing exciting and innovative technology.

Dr Stagg is an associate professor and head of the Physiological Neuroimaging Group at the University of Oxford. She has held a Sir Henry Dale Fellowship, funded by the Wellcome Trust and the Royal Society since 2014. Charlotte trained in Physiology and Medicine at Bristol University in the UK. She did her DPhil research at the Oxford Centre for Functional MRI of the Brain (FMRIB), using advanced neuroimaging to study how the brain learns new motor skills. She was then awarded a Junior Research Fellowship at St Edmund Hall in Oxford, continuing to be based at FMRIB for her post-doctoral work, with research periods at University College London and the University of Miami in Florida. Her current interdisciplinary group uses multi-modal neuroimaging and non-invasive brain stimulation approaches to understand the physiological processes underlying motor plasticity, both in the context of learning new motor skills and regaining function after a stroke.

DEMENTIA CLINICAL LEAD AND CONSULTANT NEUROLOGIST AT ST GEORGE’S HOSPITAL Jeremy Isaacs is a consultant neurologist at St George’s and Kingston Hospitals and is dementia clinical lead at St George’s Hospital. He studied medicine at Cambridge and UCL and has a PhD from UCL on the immunology of Creutzfeldt-Jakob disease (CJD). Jeremy has a specialist interest in cognitive neurology and dementia. He runs a multidisciplinary cognitive neurology service at St George’s Hospital offering
TRANSCRANIAL STIMULATION: DEVELOPING A NEW TOOL FOR REHABILITATION?
DIAGNOSIS OF YOUNG ONSET DEMENTIA
Non-invasive brain stimulation (NIBS) techniques such as transcranial direct current stimulation (tDCS) are showing increasing utility both as tools for neuroscientific research and as potential adjunct therapies for rehabilitation after brain injury. Here, Charlotte will review the evidence supporting their use, particularly in the context of motor recovery after a stroke. She will go on to discuss recent work combining NIBS with multimodal neuroimaging to study the mechanisms underpinning the known behavioural effects. She will finish by summarising on-going work to maximise the potential of these techniques and highlight their potential limitations.
An increasing number of younger people are presenting in primary care and to memory and neurology clinics with concerns about their memory and other cognitive abilities. Only a small proportion have an underlying dementia; however, this is an important diagnosis with life changing consequences. Research suggests that people with youngonset dementia experience significant delays in receiving a diagnosis. My talk will focus on the clinical assessment of younger people with cognitive symptoms, including features that help the clinician distinguish benign from pathological symptoms. I will discuss the major diagnoses in younger people in the cognitive clinic including neurodegenerative dementias such as Alzheimer’s disease and frontotemporal dementia and non-organic syndromes such as functional cognitive disorder.

He has a particular interest in profound disorders of consciousness and is working with several teams of researchers to look at ways that these could potentially be treated. James read Medicine at University College London, qualifying in 1999, initially practicing General Internal Medicine and Neurology, prior to re-training in Neurosurgery.
This seminar will detail the different classifications of impairment of consciousness after severe acquired brain injury (traumatic or otherwise), alongside highlighting techniques that are being developed that will enable us to more accurately prognosticate, communicate with and ultimately treat patients affected by these problems.

Professor Nick Alderman is at Priory Healthcare. Priory Brain Injury Services, at Priory Healthcare. He is acknowledged internationally as one of the foremost experts in neurobehavioural rehabilitation, having over three decades experience in this field and a track record in treatment innovation, service development, leadership, research, teaching and academia.
Previously he held senior posts at the Brain Injury Rehabilitation Trust and National Brain Injury Centre (Kemsley Unit). He holds academic appointments including Visiting Professor at the University of the West of England and Honorary Professor at Swansea University. Nick has published numerous journal papers and book chapters, created neuropsychological tests, observational recording measures and rating scales used in neurorehabilitation services across the world. He is Chair of the East Midlands Acquired Brain Injury Forum, and a Director of the Independent Neurorehabilitation Providers Alliance.

Maggie Sargent is a registered general nurse with over 30 years experience of caring for individuals with acquired brain injury and other catastrophic injuries. As director of Maggie Sargent & Associates, a leading provider of expert witness reports, she leads a team of experienced experts. Her wide and varied practical experience enables her to speak with considerable authority and she is an acknowledged leading expert in her field, frequently invited to speak at national and international events. Maggie is also director of CCMS, an award winning specialist provider of case management for rehabilitating severely injured adults and children throughout the United Kingdom, Europe and worldwide. A published author, she has written papers on the costs of care including a paper at The European Brain Injury Symposium at the Vatican about caring for clients with PVS. Additionally, she is the specialist contributing author on expert care and case management in the APIL Guide to Catastrophic Injury.
REHABILITATION OF SEVERE NEUROBEHAVIOURAL DISABILITY AFTER ACQUIRED BRAIN INJURY NBD is associated with poor psychosocial outcome and increased risk of violence and offending. Because the effects of TBI cannot be directly observed it has been labelled as the ‘silent epidemic’. Previously, neurorehabilitation services were not organised to manage NBD, and consequently survivors migrated to inappropriate services. These unmet needs resulted in evolution of neurobehavioural rehabilitation (NbR) which has revolutionised TBI outcomes for over three decades. In this presentation, internationally acclaimed expert Professor Nick Alderman will give an overview of NbR, including clinical, social and financial benefits, to demonstrate how its evidence based outcomes can positively influence recovery from acquired brain injury.

MEDICAL DIRECTOR AT THE ROYAL BUCKINGHAMSHIRE HOSPITAL Dr Ali Jamous qualified in 1973 from the Faculty of Medicine at Aleppo University in Syria. After training as a General Surgeon, he arrived in the UK in June 1977 to begin his neurosurgical training at the Department of Neurosurgery at the Radcliffe Infirmary in Oxford.

STROKE – NEW TRENDS The results of five randomised controlled trials in 2015 showed the efficacy of endovascular treatment over conventional stroke treatment (intravenous thrombolysis) in the management of stroke patients with large vessel occlusion. The current evidence base in the treatment of acute stroke is reviewed with a look into the challenges lying ahead in the implementation of the current evidence.
Dr Kandasamy is a consultant diagnostic and interventional neuroradiology consultant at King’s College Hospital and has been since 2009. King’s College Hospital is a leading tertiary care neurosciences unit and academic health science centre. Featuring an Hyperacute stroke unit which is one of the four major trauma centres in London. Dr Kandasamy has extensive experience in stroke imaging, neuro-vascular intervention and educational lectures.

In this seminar, Dr Jamous will discuss the importance of effective management of severe spasticity in patients with spinal cord injury/disease. With 70% of patients experiencing moderate to severe spasms affecting ADL, wheeling on rough grounds and unsafe to drive, effective management is essential to restore functional independence.

THE CHRONIC PAIN MANAGEMENT SPECIALIST Dr Tacson Fernandez is a chronic pain management specialist and lead clinician for neuromodulation services and acute pain services. Dr Fernandez also provides anaesthetic services for complex spinal surgery and regional anaesthesia for upper limb surgery. Dr Fernandez set up the Neuromodulation Centre at the Royal National Orthopaedic Hospital, offering complex chronic pain patients the option of using spinal cord stimulators and peripheral nerve stimulators in accordance to NICE guidelines (TA159). Maintaining an active interest in neuromodulation in complex chronic pain patient groups, Dr Fernande’z areas of interest

SPINAL CORD STIMULATION IN COMPLEX CHRONIC PAIN PATIENT GROUPS This talk explores the evolution and management of chronic pain in select complex patient groups who gained significant benefit following the implementation of a spinal cord stimulator at our Neuromodulation centre. It provides insight into the complexity of these patients, the mechanisms of chronic pain and role of neuromodulation (spinal cord stimulators). It also explores the role of evidence-based medicine in these patients and its impact on treatment modalities in the future.

GLOBALLY RECOGNISED NEUROPSYCHOLOGIST AND CHAMPION OF MS IN THE UK Dr Anita Rose is currently the consultant neuropsychologist at the Raphael Medical Centre in Tonbridge and also works as an independent consultant neuropsychologist across the globe. She has had a passionate interest in neuropsychology, obtaining extensive experience in this field since 1998. Dr Anita Rose is known as a ‘Champion for Multiple Sclerosis’ and provides significant input into the field of multiple sclerosis via her clinical work, research, publications, booklets and consultancy to MS Groups in the UK as Vice Chair for the European Neuropsychology Special Interest Group in MS. Anita is also the Vice Chair and Clinical Advisor to the MS Society in South Africa. Dr Rose is well known in the field of brain injury. Notably conducting significant research into brain injury and chronic neurological conditions, publishing valuable research and creating many self-help booklets in addition to providing expert consultancy within this area. She has recently coauthored a book looking at late recovery from the vegetative state and is currently working on a number of research projects.
Mr. Bankole Akomolafe is an experienced consultant vascular surgeon at Hull and East Yorkshire Hospitals NHS Trust and an honorary lecturer at Hull York Medical School. Activity Based Restorative Therapy Activity Based Restorative Therapy is a therapeutic intervention used to treat neurological illness and injury based around five key principles: Mass Practice, Locomotor Training, Weight-Bearing, Task-Specific Training and Functional Electrical Stimulation (FES). FES for Dropped Foot: Economic Justification and New Applications in Parkinson’s WEDNESDAY | 15.30 | THEATRE 3
FES is an effective intervention for dropped foot and has a mean usage of 4.9 years. The National Institute for Health and Clinical Excellence applies a cost-utility ceiling of £20,000 per Quality Adjusted Life Year (QALY) for an intervention to be considered. Previously the QALY gain derived from the effect of FES on walking speed gave a cost per QALY of £15,406. However, analysis of new data, presented at this seminar, gives a mean cost per QALY of £5,705 making it comparable to the cost per QALY of hip and knee replacements. The above analysis was carried out using FES with people who had MS or who had had a stroke. Recently we have been investigating the use of FES for the reduction of Bradykinesia in Parkinson’s Disease in a randomised controlled trial (n=68) funded by NIHR. The trial protocol, treatment methods and preliminary results will be presented in the seminar.

Franck is one of the major vestibular rehabilitation specialists in France. Alongside his clinical practice, he has been looking for a range of improved solutions and tools, such as optokinetic stimulators and new solutions for motion sickness. Many people experience brain fog, lack of concentration and poor memory which are all signs of sub-optimal cognitive function. There are many factors that can affect cognitive health including; inflammation, oxidative stress, poor gut function, nutritional deficiencies and excess toxicity (this list is not exhaustive). This talk will outline the importance of supporting the underlying factors including reducing neuroinflammation and supporting synaptic plasticity and Brain Derived Neurotrophic Factor (BDNF) production.
Of the variety of clinical examinations and assessment procedures employed in current practice, evaluation of the range of motion (ROM) is considered the most informative and useful. Active ROM (AROM) and Passive ROM (PROM) measurements are typically taken by therapists using goniometers. It is generally understood that improvements in these ranges are indicative of general and functional recoveries of the hand. A device (the ExoFlex) has been created with assessment features designed to map ROM in each joint of an affected hand.

The Importance of Education and Training in Neurodiagnostics The neurodiagnostic field is comprised of EEG, Evoked Potentials (EP), Nerve Conduction Studies (NCS) and Intraoperative Monitoring (IOM) - and the field is rapidly evolving. IOM, for example, was first used in the 1970’s when severe spinal deformities could be treated with surgical advancements and instrumentation. At that time spinal cord evoked potentials were developed to reduce spinal cord injury. Neuropathic pain is a major problem following CNS injury and is directly related to mood changes, interference in daily life activities and change in the way the individual can cope with their own pain. Little is known of how novel therapeutic strategies could control high-impact types of pain, improve the body’s natural painkilling system, or help people to cope with their pain. The DolSCI Network will bring together experts to improve the objective diagnosis of SCI neuropathic pain using QST (Miguel Rodriguez, Advanced Medical Equipment) and multichannel EEG analysis (Armin Schnürer, Gtec). Brain-Computer Interfaces (BCI) for Stroke Rehabilitation and Consciousness Assessment Lately, BCI systems have become increasingly used in the context of stroke rehabilitation and assessment of consciousness. Many BCI systems are based on motor imagery activity recorded from the sensorimotor cortex, which is translated into continuous control signals for rehabilitation devices. Some devices use Virtual Reality/FES to allow users to observe an avatar’s limb movement. Armin Schnürer has been working in research in the field of EEG based BCI systems and biomedical engineering for more than seven years. Including P300, motor imagery and SSVEP based BCI systems.
DR. ANNE WRIGHT High-level spinal cord injury (SCI) can lead to severe impairment of hand functions making an individual dependent on other people for activities of daily living (ADL). The Soft Extra Muscle (SEM) Glove is a device that could provide both orthotic benefits and rehabilitative effects while remaining compact and light.The objective of this study is to determine the effectiveness of the SEM Glove as an orthotic and rehabilitative device. The participants will use the SEM Glove at home to perform a set task and ADL for at least four hours a day for 12 weeks. They will be assessed before getting the Glove and at six weeks intervals for 18 weeks, using functional and neurological measures. The study is currently on-going. This talk will present the design, challenges and the results so far.

Dr. Jean-Pierre Lin is a consultant paediatric neurologist at Guy’s and St Thomas’ NHS Foundation Trust, King’s Health Partners, proceeding his previous role of being senior registrar in children’s neurology at Great Ormond Street Hospital. Jean-Pierre is President of the British Paediatric Neurology Association, the professional organisation for doctors who specialise in the care of children with neurological disorders. Movement disorders profoundly affect the lives of children, are often painful, interrupt sleep, sitting and daily care while reducing activity and participation in education, play and social interaction. Medication and physical measures are often ineffective and adverse effects common.

EVALUATION OF AN FES SERVICE USING WALKAIDE FOR PEOPLE WITH MULTIPLE SCLEROSIS In people with MS and footdrop FES has been shown to provide both orthotic and therapeutic effects and to sustain safer walking when there is increasing disability. WalkAide is an FES system that uses a tilt sensor to trigger stimulation. Individuals referred via an NHS MS service were assessed and those suitable were offered a 4-8 week home trial to evaluate daily use, effectiveness and acceptability. Following home trial people obtaining their own WalkAide were offered an ongoing follow up review service. An evaluation of the service and its outcomes will be presented with results from a user survey.
Angela Davies Smith is a clinical specialist research physiotherapist and has a research interest in MS. She works in a MS Research Unit based within a large clinical MS service in Bristol UK. Her primary area of research is symptom management with a focus on developing assistive technologies. Previous work has included collaborative studies to develop intelligent functional electrical stimulation and novel methods to measure postural sway. An ongoing research theme has been the measurement and control of intention tremor in MS in collaboration with the Department of Mechanical Engineering, University of Bristol. Other research areas involve fatigue management and fMRI studies of MS related fatigue. She is a member of iMSpire, a European research collaboration. Clinical services include FES and fatigue management (FACETS).
ADVANCES IN DEEP BRAIN STIMULATION NEUROMODULATION FOR MOVEMENT DISORDERS IN CHILDREN Jean-Pierre will explain how deep brain stimulation (DBS) can profoundly reduce the impact of an increasingly recognised number of genetic movement disorders including dystonia (twisting and writhing postures and movements), chorea (dance-like rapid movements), myoclonus (lightning jerks) and tremors. A key area of clinical research is the effect of DBS neuromodulation on dystonia and chorea (also known as dyskinesia) in children with cerebral palsy (CP). The benefits of DBS seem related to the proportion-of-lifelived with CP, indicating that early or very early DBS intervention may be most effective at a time when the developing brain’s ‘critical’ and ‘sensitive windows’ are still open to influence and particularly before the onset of permanent musculoskeletal deformity.

VIBRAMOOV - EARLY REHABILITATION THROUGH FUNCTIONAL PROPRIOCEPTIVE STIMULATION This session will demonstrate the concept and potential of functional Proprioceptive Stimulation in early stage neurological rehabilitation. Many neurological impairments (such as stroke, incomplete spinal cord injury, traumatic brain injury, cerebral palsy, MS and Parkinson’s disease) can dramatically affect our ability to move at will and they disturb our sense of “connection” to, and awareness of, our limbs. Awareness of limb position (proprioception) is impaired. Vibramoov aims to preserve and re-establish the coordination between active movement and the related neurosensory information.

REHABILITATION WITHOUT LIMITS: FUNCTIONAL ELECTRICAL STIMULATION ASSISTED WALKING WITH COMPLETE PARAPLEGIA This workshop will explore how the Xcite (Restorative Therapies, Baltimore, MD) can provide the framework and patterns for functional electrical dtimulation (FES) derived gait in complete paraplegia. The aim is to change perceptions of what can be achieved by those with complete paraplegia and demonstrate the potential of multichannel FES therapy. We will discuss the patient requirements for success, the pattern of muscle activation built into the Xcite before demonstrating the gait pattern in an individual with a complete spinal cord injury at the level of T10.

Neuromonitoring is the act of monitoring and identifying changes in the brain, spinal cord and in peripheral nerve functions, prior to irreversible damage being inflicted. A growing method that has helped many surgeons during dissection is intraoperative monitoring - the technique of analysing and reducing the potential risk of neurological injury after operations on and around the nervous system. As a whole, the worldwide inoperative neuromonitoring market is currently valued at an estimated £1.7 billion and in 2016, research forecasted growth of at least 5% over the course of the next ten years. The current market share is dominated by North America, which is a result of their consistent regulations and guidelines being put in place to support the implementation of neuromonitoring during surgery. Europe maintains an admirable stake within the market share, equating to 28% compared to North America’s 52%, with Asia Pacific having an quickly increasing share of 16%. In the US, inoperative neuromonitoring is continuously growing in terms of its market and its application across surgeries. The ability to be able to monitor surgeries such as spine surgeries, brain mappings and

training and continued protocols, with these developments quickly gathering support. These will lead to an increase in UK based surgeries implementing neuromonitoring and provide reputable support for those who already are enjoying the embetterment of patient treatment that IOM brings.
