Group StRep-B

What you need to know G

Group B Streptococcus (GBS) is a normal bacterium which is carried by 20-30% of adults, most commonly in the gut, and for up to 25% of women, in the vagina, usually without symptoms or side-effects.

GBS can occasionally cause infection, most commonly in newborn babies, sometimes in adults and, very rarely, during pregnancy and before labour. GBS is not a sexually transmitted disease. Treatment of a woman and her partner carrying GBS does not prevent recolonisation.

Group B Strep Infection in Newborn babies

Late-onset GBS infection – usually presenting as GBS meningitis – occurs between age 6 days and 1 month and, more rarely, up to age 3 months. After 3 months’ old, GBS infection in babies is extremely rare. GBS is recognised to cause preterm delivery, maternal infections, stillbirths and late miscarriages; and preterm babies are known to be at particular risk of GBS infection as their immune systems are not as well developed.

There are two types of GBS infection in newborns: early and late-onset.

Overall, approximately one in every 1,000 babies born in the UK develops group B Strep infection.

Early-onset GBS infection is more common (2/3 of cases in babies) and occurs when the baby is up to 6 days old; a key symptom is the rapid development of breathing problems, associated with blood poisoning.

On average in the UK, at least two babies a day develop a group B Strep infection one baby a week dies from their GBS infection, and one baby a week survives with long-term disabilities – physical, mental or both.

Currently, UK guidelines recommend a ‘risk factor’ approach to determine which women should be offered antibiotics in labour to prevent GBS infection in newborn babies. Testing is the only way to know if you are carrying GBS. If you are, steps can be taken that minimise the risk of your newborn baby developing GBS infection. However, routine screening of all pregnant women for GBS is not currently recommended by the UK National Screening Committee or the Royal College of Obstetricians & Gynaecologists.


PREGNANCY

Group B Streptococcus (GBS) is a normal bacterium carried by up to a third of adults, most commonly in the gut, and for up to 25% of women in the vagina, usually with no symptoms or side-effects. It is not a sexually transmitted disease. In the UK, pregnant women are not offered testing routinely for group B Strep carriage, unlike in many other developed countries. Sensitive tests for GBS carriage are not widely available within the NHS, though they are available privately (click here) from less than £40. If you want to find out whether you carry group B Strep, the sensitive ‘ECM’ (Enriched Culture Medium) test is available from those listed at www.gbss. org.uk/test who follow Public Health England’s guidelines for this test. Group B Strep carriage may be detected when samples are taken for other reasons – but often it’s not, because the widely used NHS test was not developed to find group B Strep and, unsurprisingly, is not very good at doing so. The standard ‘non-specific’ NHS tests give a high proportion of falsely-negative results, though a positive result is reliable. It’s good to know if you carry group B Strep during your pregnancy so preventative action can be taken to minimise the chance of your newborn baby developing group B Strep infection.

Risk factors for group B Strep infection Pregnant women carrying group B Strep is perfectly normal. GBS can be present at any time – in a woman’s first pregnancy, or in one or more subsequent pregnancies. It can be a threat during pregnancy, around delivery and afterwards. There are certain situations which increase the chance a newborn baby may develop GBS infection.

These include: Mothers who have previously had a baby infected with GBS – risk is increased 10 times. Mothers where GBS has been found in the urine at any time in this pregnancy – risk is increased 4 times. Mothers who have been shown to carry GBS in this pregnancy – risk is increased 3 times. The following clinical risk factors also increase the chance of a baby developing a GBS infection Mothers who have a raised temperature* during labour of 37.8°C or higher – risk is increased 4 times Labour starts or waters break before 37 weeks of pregnancy (i.e. preterm) – risk is increased 3 times Waters break more than 18 hours before delivery – risk is increased 3 times *A slightly raised temperature may be of less significance in a woman who has an epidural than in a woman who hasn’t.

Q Can I breastfeed if I have GBS? Our medical advisory panel strongly recommends you should be encouraged to breastfeed your baby. Although there have been isolated cases describing GBS infection possibly related to breast milk contamination, the advantages of breast feeding will, in our medical advisory panel’s opinion, greatly outweigh the remote risk of transmitting GBS via breast feeding. High hygiene standards need to be maintained for all breastfeeding mothers, with the hands and nipple areas being kept clean.

If group B Strep is detected at any time during your current pregnancy, you should be offered intravenous antibiotics from the start of your labour and regularly until your baby is born. If GBS has been detected during your current pregnancy from a vaginal or rectal swab If group B Strep has been detected during your current pregnancy from a urine sample If group B Strep was detected during a previous pregnancy If a previous baby developed group B Strep infection. Not every pregnant Mum who has GBS detected during her pregnancy will want intravenous antibiotics in labour. Many will, but others may decide not to have them unless there are other additional risk factors. Do discuss this with your midwife or doctor, or contact us. If you decide against antibiotics in labour, it would be prudent for your newborn baby to be observed by trained staff for at least 12 hours.

July is Group B Strep Awareness Month Group B Strep is the UK’s most common cause of life-threatening infection in newborn babies. Strepelle is a home-to-laboratory Group B Streptococcus (GBS) test which can be bought at www.strepelle.com or at independent pharmacies from £39.99, it simply allows pregnant women the opportunity to test for this devastating infection from 35 weeks, with the highest accuracy ECM testing. If positive they are sent a letter to give them access to the potentially life-saving antibiotics during labour.

Are you Ready for a Baby? Career, life goals, holiday plans… is there ever a right time to have a baby? Kerry Potter examines the biggest decision you’ll ever make. When a woman takes a pregnancy test in a film and sees a thin blue line, one of two things happen: she whoops with joy or her world falls apart. She either wants a baby or she doesn’t – it’s black and white. Real life, however, is more complicated. I remember sitting in my bathroom looking at my very own thin blue line with mixed emotions. I was mainly thrilled – I was 32 and married, we owned a house, this was our plan – a plan discussed and dissected with a large number of my friends at that time (to the point where four of my five university friends got pregnant within six months of each other). But even so, I was also slightly scared. I couldn’t even keep my plants adequately fed and watered, how would I cope with a real live baby? And there was a third thing too – had I done everything I wanted to do with my life yet? Should I have got another promotion under my belt? Should I have tried harder to fulfill my ambition of working in New York? Should I have gone to Glastonbury one last time? Had I completed enough of Project Life to begin Project Baby? Difficult stuff to ponder on a normal day – let alone one when you’re

so pumped with hormones, the breakfast news makes you cry and you’ve got such bad morning sickness you vomit when you brush your teeth. Like most women my age, since turning 30, I have had many, many conversations with female friends about when to start trying for a baby; all of us increasingly spooked by the ‘fertility time-bomb’ headlines, accusing professional young women like us of putting our careers before motherhood. A GP friend was busy saving up to cover future maternity leave. A party-girl friend who had just got married described how her clubbing crew begged her to do another summer in Ibiza before she had babies. Another was experiencing the choice between IVF or continuing to try and conceive naturally after two unsuccessful years of trying.

A survey of 3,000 women by Centrum last year showed that some women take up to five years to plan motherhood, with a third aiming for certain career goals, a quarter saving £5,000 and 58% aiming to own a property. Meanwhile, the bucket list – once restricted to things to do before you die – now pops up on parenting websites, in the guise of ‘things to do before you get pregnant’, on the premise that there won’t be a lot of bungee jumping going on once you’ve given birth.

There is a sense that we need to get our (metaphorical and literal) houses in order – and live a little – before we take the plunge. The career sabbatical for the gap year you didn’t have at 18, finally running a marathon, writing that novel, going on holiday on your own, travelling to three continents – all things you want to tick off before you settle down to a life of designer pushchairs and Ofsted ratings.

perfect timing So when is best to have a baby? And can we really plan and control this aspect of our lives? Statistics show that we’re leaving it later, sometimes to our cost. The average age of women giving birth now is a year older than it was a decade ago; 29.5 in 2010 compared with 28.5 in 2000, according to the Office for National Statistics (ONS). The Department of Health research shows the abortion rate for women aged 30-34 increased by 10% between 2009 and 2011 too, indicating more women choosing to delay motherhood until their late 30s.

There’s no getting away from the fact that the biological reality is stark. “The aim should be to have a baby before 35, certainly by 37,” says Dr Magdy Asaad of London Fertility Centre. “It becomes much more difficult after 35. The number and quality of your eggs are reduced. It’s difficult for women because life expectancy is increasing – it’s now 80-85 years – so a 40-year-old still feels young. But it’s a different story with your ovaries. If circumstances allow, you should be considering babies in your late 20s or early 30s.” Ah, yes, “if circumstances allow” – and that’s the crux. When do any of us feel the timing is 100% right? There is no completely perfect time to become a mum. And, of course, it’s not always a simple weighing up of whether your job, relationship and experiences are where you’d like them, there’s also the very real possibility that pregnancy won’t come when you want it. On average, 84% of women will fall pregnant within the first year of trying and one in seven couples have fertility struggles, adding yet another dimension to the “when is the right time” question.

a certain amount of experience and credibility, so you know how to juggle and delegate, and your firm trusts you to work flexibly.” Then there is your relationship to consider. It sounds obvious, but you need to make sure your partner definitely wants a baby too – one US study showed 100% of couples who’d had a baby when the husband wasn’t initially keen had divorced within six years. “If you can’t talk about it and decide when a good time is, how on earth are you going to raise a child?” says psychology consultant Dr Sandra Wheatley. “It’s about coming to a joint decision that’ll make you both happy.” It helps if you’re married too – ONS statistics showed that 80% of married couples in 1991 were still together a decade on, compared to 60% of cohabiting couples.

“Women can feel overwhelmed by everyone else’s opinion. Listen to your own voice and feel confident in it” The cost of having a baby is also on the rise – so can you afford to pay for childcare? If you’re working in London and need a full-time nanny, it could cost you £24,000 per year (which means you’d need to earn almost £30,000 gross to break even). According to Daycare Trust figures, a part-time nursery place (25 hours a week) in London costs on average £6,340 per year, but the most expensive ones go up to £15,000. It’s a bit cheaper in Manchester at £4,592 for 25 hours

care but in Edinburgh it rises again to £5,178. Wherever you live, and whatever you earn, it will be a significant financial factor in your life. A house with your name on the title deeds and a spare room is also a prime concern for potential mums. But the average age of the UK first-time buyer was 29 years old in 2012, rising to 32 in London. Given the average age of a woman giving birth is 29.5, the figures don’t really add up. This may be a case of just having to change your mindset – in the current financial climate, renting a family home is perfectly normal – a new study from Shelter says by 2025, 27% of households will be renting UK-wide. But do the maths first – experts say you shouldn’t be spending more than 35% of the family income on rent to be on an even keel financially. You might not think you have to worry about your friendships, but it can be lonely when you’re the first among your peers to have a baby. A CLIC Sargent survey showed that 40% of expectant mothers worried about being socially isolated after giving birth. So if at least two or three friends are already mums, you’ll have advice and support on tap. You’ll also make new friends – mothers in your NCT group or antenatal class – to help you through those first few years. All in order? Congratulations! You are ready to have a child. But not everything in life fits neatly on a spreadsheet. We are a generation used to being in full control of our destiny, but pregnancy and motherhood simply aren’t like that – they’re unpredictable, wonderful and frustrating in equal measure.

“I don’t think there is a ‘right’ time,” says Nifa McLaughlin, editor of parenting website gurgle.com. “There will always be something else going on: an amazing career opportunity, your house isn’t big enough, you have that holiday planned. I don’t think you should be ruled too much by life itself, you should just go on your instincts.”

The best starting place is to work out which barriers are surmountable, practically and emotionally, in your current situation. First up for many women comes career. “Maternity packages differ but usually you need to be in your job for 18-24 months to reap the full benefits,” says Kirstie Burn from recruitment firm PSD Group. Burn believes the best time to have a baby is seven to eight years into your career. “You should be at a certain level of seniority so that when you come back you won’t have to start all over again. You’ll have

If you’re keen to get your pre-baby body back, grab your baby, the buggy, and try this one out... What it does: Defies gravity and makes buns of steel! Strengthens the bum and backs/fronts of thighs. The UK’s first womb transplant is set to take place next year as part of a clinical trial in which 10 women will get the chance to carry their own babies.

Following the birth of a baby boy last year after a successful procedure in Sweden, the Health Research Authority has granted ethical approval for 10 transplants. The first British baby born from a transplanted womb could arrive as soon as late 2017 or 2018.

“Donor retrieval is a bigger operation than transplanting the uterus into the recipient,” the surgeon said. “We don’t want to subject a live donor to that operation.”

More than 100 women have been identified as potential recipients of transplants for a team of surgeons to be led by Dr Richard Smith. About one in 5,000 women – or 50,000 of childbearing age in the UK – were born without a womb, while some cancer sufferers have had theirs removed.

Smith said using deceased donors reflected the complexities of the operation.

Organ donor coordinators have suggested that about five wombs annually could be made available. After 12 months on immunosuppressant drugs and close monitoring, each woman will be implanted with one of her embryos, with the hope of achieving a successful pregnancy. A baby would be delivered by caesarean section to prevent the donor womb suffering the trauma of labour.

Smith said the technique would offer hope to women who could only have children through adoption or surrogacy. The consultant gynaecologist at the Queen Charlotte’s and Chelsea hospital, who has been working on the project for almost 20 years, said he was “really, really pleased” to obtain ethical approval for the transplants.

Six months after giving birth, each woman can try for another child, or the womb will be removed. That would minimise the risk of keeping women on immunosuppressant drugs for the rest of their lives. However, the trial needs to raise £500,000 before any operations can take place. “I’ve always been an enormous optimist,” Smith said. “The project has run with no money from the start. Somehow or other, somebody has always turned up and given us enough money to keep it going.” Just over £40,000 has been donated to the Womb Transplant UK project and contributions can be made here.

“For many couples, childlessness is a disaster. Infertility is a difficult thing to treat for these women,” he said. “Surrogacy is an option but it does not answer the deep desire that women have to carry their own baby. For a woman to carry her own baby – that has to be a wonderful thing.” To take part in the trial a woman must be aged between 25 and 38, have functioning ovaries and their own eggs, a long-term partner and be a healthy weight. Only a third of the 300 women who approached the Womb Transplant UK team met the criteria. Before the trial starts, embryos will be created and frozen using each woman’s eggs and sperm from her partner. They will then undergo a six-hour operation to receive a womb from a donor who is classed as

A 36-year-old Swedish woman gave birth to a baby boy in September 2014 after receiving a donor womb from a 61-year-old family friend who had given birth to two sons. She and her partner - both competitive athletes - named the baby Vincent, which means “to win” in Latin. “As soon as I felt this perfect baby boy on my chest, I had tears of happiness and enormous relief,” the mother said. She had been told at the age of 15 that she did not have a womb.

Top 5 Tips To get the most from your pediatrician visits As a pediatrician for many many years, there are a few things I’ve seen that can make for a more effective visit – I want to share these with you!

1. Be prepared! I love when patients come with a list of questions or concerns. Write them down because often you will forget. Keep a running list on your phone or in a notebook between appointments. If you are asked to fill out any questionnaires, try to do so before coming to the visit. All of this will make the visit more efficient.

2. Share! In addition to getting information, you should be giving information. Since your last visit, has your child been to a specialist or had an injury/ hospitalization? Is your child on any new medications or have there been changes in dosing? If you want to discuss a concern without your child present, please tell the staff before so we can meet with you a few minutes before the visit in another room. Also, don’t wait – start with your most important concern. This way, I will be able to look for information regarding your concern during the physical exam.

3. Be present! Check-ups are important visits and you need to avoid distractions. If possible, try to bring only the child scheduled for the appointment. If that is not possible, I often ask parents if I can send siblings out to color or talk with nursing staff. When you are actively engaged, you will get more out of the visit.

4. Prepare! Before the visit, discuss what will happen with your child. In our office, we want all patients to be undressed to their underwear and in gowns – it allows for a thorough exam. Let them know this may be the case. Do not tell them that shots won’t hurt , and please do not say, “If you do not behave, the doctor will give you a shot. ” That can undermine the relationship between the doctor and your child.

5. Shhh! As soon as possible, let your child communicate. As soon as a child can communicate, I like to talk to them, so do not jump in and answer questions for them. Not only am I learning about them; I am also listening for any language difficulties, temperament issues like shyness or anxiety, and also any concerns they may have themselves. I learn a lot from speaking with the child directly.

After the check-up is over, check in with yourself about how you feel. Never leave a visit confused. Ask for clarification. Write down specific advice. And always know you can call back if you need to double check. Together we can make your visit a success!

Bugaboo Donkey

Single to duo as easy as ABC There is no doubting how easy it is to transform the Donkey from single to double, you just unclick a couple of catches and pull the frame to create a wider frame. You can have the seats in a variety of positions - Twin mode, two carry cots, seat and carrycot, parent and world facing. One of the key features for me is the size of the underneath basket, its huge and when in single mode you have an additonal basket which is really handy, but my little one kept leaning over and grabbing things, all fun and games. Tires do require a pump, so you are risking getting a flat whist walking the dog. The wheels and suspension do both deserve a mention, If your looking for a pram that is suitable on all terrain and is ideal for twins or a pram that will grow with your family this might just be what you are looking for. In single mode it was great for getting on public transport but in double it was a little more tricky. I had to leave the house and set up outside for duo mode, fine in the Summer months but maybe a nightmare in the Winter. The seats are more narrow than most standard prams but still plenty of room. Edward 9 Months and Polly-Ann 2 both were very comfy.

Toddler Tantrums Toddler tantrums are terrible​. ​For many parents, they are inevitable. Learning how to deal with tantrums is an important life skill, and starts right from that moment in the kitchen or in a store where your toddler starts a meltdown. Managing tantrums effectively will help your child learn to cope with frustration and​n ​ ot getting ​their​way, ​both​now​and​as ​they​grow up. The most common tantrum trigger is when a child hears “no”​— ​it represents rage at not getting ​their​way; at not being able to control ​ their​world. T​ antrums​​are more likely ​to occur w ​ hen a child is tired, hungry or frustrated. For parents, a tantrum can be embarrassing and exhausting and there is an urge to give in​​—​​just to make it stop!​Don’t do it! ​It is crucial that you not ever give in to tantrums, because doing so will VERY effectively reinforce the message that bad behavior is an effective way to get what you want. Interestingly, s​ ometimes ​giving in to a tantrum can be​even​worse than always giving in. I​f your child never knows when you might give in, the tantrums will become more frequent and more severe until you finally relent.

The three important steps to take during a tantrum are:

Ignore it: Walk away and don’t talk, yell, or give your child attention during the tantrum. If your child tantrums in public and you don’t have the nerve to wait it out, a good alternative is to pick her up and take her to the car or another room to allow her to finish working through her anger.

For the majority of kids, consistently applying these three steps will effectively minimize and eventually eliminate tantrums. However, a small number of children do not respond positively to this method. Ignoring the tantrum makes it worse and they are not able to calm themselves down​— ​ ​​they might continue for hours when they are not helped to calm down. While this is still not a reason to give in to the tantrum, your child may need you to hold him firmly, talk quietly and rock him until he calms down. If your child has tantrums ​at an intensity level that is very difficult to control, you should consider seeking professional help from a psychologist or other parenting expert.

Hang in there – every parent has been through it and now you’re ready to face it!

Reward good behavior with positive attention: When your child ends the tantrum, give him a big hug and snuggle. Tell him you love him and that you are glad he has calmed down.

Stick to your guns: Even if your child calms down, do not change your mind. Once a child works through the tantrum, it is common for her to say “I calmed down, so can I have a cookie now?” Keep in mind that your reason for saying ‘no’ in the first place has not changed just because your child has calmed down. Don’t be surprised if your child begins to tantrum when she hears your second ‘no’. You can handle it… you’re a pro!

Habits You Should Steal From Your Toddler

Okay, we admit it: Your toddler may not seem like the best source for life lessons, considering that it was you who taught her everything she knows thus far. However, you can learn a lot from some of the things she does naturally. Check out these practices you should adopt from your little pupil turned teacher.

Throw a tantrum Obviously, you’re not going to throw yourself on the floor and start kicking and screaming the next time your boss gets on your nerves (even though you may want to!), but don’t bottle up your emotions. “Toddlers are great at ‘letting it all hang out’ emotionally, and parents should do the same (minus the actual tantrum),” says Tina Gilbertson, LPC, a Denver-based psychotherapist and author of Constructive Wallowing: How to Beat Bad Feelings by Letting Yourself Have Them. “Too often we refuse to let ourselves get mad or cry about something because we think it’s inappropriate,” she says. But there’s nothing

inappropriate, or even harmful, about having a feeling as long as you express it in a way that’s safe for you and others, she adds. So next time you’re feeling some type of way, let it out. “Just be honest—at least with yourself—about how you feel,” Gilbertson says. Talking to a trusted family member or friend, writing in a journal, or having a conversation with the person who’s angering you or making you feel sad are a few ways to express your feelings.

Make physical activity fun When you try to exercise and your toddler joins in, it’s as if she’s having the time of her life. She’s super energetic, full of laughter, and once the workout is over, she’s ready for more. You, on the other hand, are probably the exact opposite. You have to force yourself to work out, and even once you do finally get going, you may slog along reluctantly. Kids naturally gravitate to fun physical activities, yet adults often pick exercise because of an internal “should” (“I should run.” “I should lift weights.” “I should do yoga.”), says Helene Byrne, founder of BeFit-Mom and author of Exercise After Pregnancy: How to Look and Feel Your Best. That makes working out something you’ll dread, which mean you’ll be less likely to stick to it. Instead of doing something just because you feel you should, do something you enjoy. “Think back to when you were a kid, and identify your favorite play activities,” Byrne says. “If you loved soccer, then choosing an athletic group activity like a boot camp class might work for you,” she says. Loved bike riding? Try a spin class. And if you were into dancing, give Zumba a whirl.

She asks for it repeatedly (sometimes throwing in the cutest “please” ever), tries to butter you up by giving you a hug or kiss first, and if all fails, she breaks out in tears. Don’t bring on the waterworks, but when you want something, speak up.

No hitting (yourself) “When toddlers fall down, they don’t waste time and energy beating themselves up for being clumsy,” says Gilbertson. “They cry because something bad happened to them and it hurt,” she says. Once they’re done crying, they move on. This is a strategy you need to master. Instead of criticizing yourself harshly and reliving mistakes in your head over and over, take a power nap, don’t dwell on your failures. Instead,

When was the last time you smiled simply because the sun was shining? As adults, we tend to overlook the small stuff. “However, a child can let out a joyful laugh from something as simple as spinning in circles,” says Dion Metzger, M.D., a board certified psychiatrist and co-author of The Modern Trophy Wife: How To Achieve Your Goals While Thriving At Home. Look for your own little nuggets of happiness. “Recall the last time you felt ‘the happy,’” Dr. Metzger says. “Was it taking a walk on a beautiful day, watching a funny movie, or just watching your toddler dance in front of the television?” Remember those moments so you can seek them again when you need a cup of joy, she says. And make sure you’re attentive to (and appreciate) other tiny sources of delight, like a double rainbow in the sky, a kiss on the forehead from your partner, or hearing a favorite song.

Take a power nap You know how your kid conks out right after lunch every day? Start doing the same. According to the National Sleep Foundation, a short nap (think 20-30 minutes) can help improve mood, alertness, and performance. So if you’re able to, sleep when the baby...er, toddler sleeps.

Your toddler raises her shirt in the grocery store and has no qualms about stripping down to her birthday suit while you have company visiting. No, we’re not suggesting you flash your neighbor or bare all in the middle of the post office, but you should follow your child’s lead and love the skin you’re in. When you feel good about your body, you’ll take better care of it. In addition, the way you talk about and treat your own body will influence your child’s body image. So adore your body, even if you aren’t back to your pre-pregnancy weight. Some ways to boost your body confidence: Eat healthy, balanced meals, work up a sweat on most days of the week (to release feel-good endorphins), and make sure you get enough shuteye.

Just say “no” Is “no” your toddler’s favorite word yet? If not, it’s coming. If you ask him if he’s sleepy, tell him “Good Morning”, or offer him an apple, his response will be the same: “No!” Though his negativity can be frustrating, try following his example. Of course, you don’t have to be as blunt (read: rude) as your toddler, but say no to things you don’t want to do or don’t have the time to do. It will reduce your stress and free up time and energy for things you really enjoy.

Travel Tots Are we there yet? Are we there yet? You may hear that question many, many times while traveling to over the holidays, so we have some fun activities for holiday travel to help!

While travelling with your little one may seem daunting and at times feel endless, with a little preparation you can make the time in a car (or plane) go by more smoothly. These ideas can be used again and again. Also think about keeping a travel kit in your car for unexpected traffic jams, or longer than planned doctor appointments.

Post-it notes and crayons – the glorious sticky paper will be sure to occupy them. Be warned, you may end up with a note stuck on your forehead! (Note from Stephanie of Please and Carrots: my newly potty-trained girls were terrified of the automatic flushing of the toilets in airports – post-it notes were a life saver to block the motion sensor!) Traveling in the dark? Think about some glow-in-the-dark objects – necklaces, bracelets and wands can entertain long after the sun has set. Did you know they make Glow in the Dark Silly Putty? You may be tempted to play with that too!

One of my favorite activities is to make a doctor kit for your child to take care of her sick doll or stuffed animal. Fill a bag or container with Band-Aids, gauze, Q-tips, washcloth, empty plastic bottle, small flashlight, and so on. Show them how to care for their “baby”. Your child will take care of many common illnesses as well as a few you have never heard of!

Cookie sheets are magnetic. You can add a small container with magnetic letters or shapes to make up pictures throughout the trip.

Teach your child a few tried and true games. For children 4+, the Memory game can be made by putting pairs of stickers to two sets of index cards. Start with a few (perhaps 3) pairs until he catches on, then add more cards to make it more interesting, not to mention time consuming. You can also start with younger children – they may not “get” the game, but can enjoy the cards with stickers! Was this a hit? Once they get into Memory, it is time to teach some simple card games like Go Fish or Crazy Eights.

Add another layer of fun to these activities by individually wrapping them and handing them out as presents during the trip. Do not give them out all at once, but during intervals (say, every hour) to make certain each activity gets its proper play time. If there are days of traveling, you might want to switch the activities out and bring in new ones to keep things fresh. An ounce of prevention is worth a pound of cure has never been more true than preparing to keep little ones content while traveling.

A small container with stickers, paper, and crayons can entertain your child for a nice long time. Or you can try a dry erase board and wipe off markers for variety. If you are somewhere where you worry about crayons or markers getting onto other things, you can consider an etch-a-sketch to draw or markers that only work on special paper. Audio books can keep your little one content for hours. Some are available for free at the local libraries, or you can download them. Bring the actual books as well so your child can follow along. A collection of pictures and a small photo album will keep your child busy for a surprisingly long time. They can sort and move the pictures around while discussing the memories that were made.

And remember to also tell them stories about all the people you are going to meet, so they are excited and feel connected when you finally reach! Happy holidays, and wishing you smooth travels! (Note: do not leave children unattended with any of these activities.)

Bird Nest One Way to Deal with Divorce A mothers story

The children simply live at “home” and the separated or divorced parents take turns living with them there, but never at the same time. The core element of this arrangement is that each parent maintains a separate residence where they live when it is not their turn at the “bird’s nest”. When my ex-husband and I were considering divorce, there were many things that paralyzed me with fear. My absolutely most terrifying concern was for my children’s adjustment. Early on I had lunch with a friend who is a family law attorney. She offered some really good advice. She said, “Everybody is different. Your family is not like any other family, your divorce doesn’t have to be like anybody else’s either nor does your custody arrangement. Get something you can both agree to on paper with the mediator, and then do what works and makes you guys happy. If it doesn’t work long term then you revert to what you agreed to on paper. The paper work can be a ‘just in case you stop agreeing about the children’ plan.” That lunch opened up a world of possibilities for me. Although my Ex and I didn’t agree on much, we did agree that the children should have as easy a time of it as possible. It wasn’t their fault after all. I told him of my lunch with this friend and what she said. We both agreed to consider alternative custody arrangements. While googling ‘alternative custody arrangements’ I stumbled on Birdnesting Custody. I was interested and found a few articles, but most were negative. After scouring around the internet a few days, I did find a few actual stories from people that had done it successfully.

Birdnesting custody is an arrangement where the family keeps the marital home, both parents paying into a household account to cover basic household expenses (in lieu of child support) and you rotate time in the ‘nest’. The children never leave, the house has custody of the dog even, and the house becomes the children’s home and source of stability.

Why We Chose to “Nest” Prior to learning about the possibility of Birdnesting my heart would clench inside my chest every time I considered moving the kids or that they would go back and forth from me to their dad’s. We had only recently moved across town into a house that was in the ‘right’ school district. Our children were getting older and it was urgent to be in the right district if we hoped to avoid private schools. We had moved in the nick of time for our oldest to start middle school. We still had boxes that weren’t unpacked. I hated to think of moving them again. Additionally, at the time, the housing market had just crashed to rock bottom. We had no equity in the short time in this house and would be lucky to get what we paid just months before. Also, we have one child with ADHD. Bringing the appropriate things back and forth from school to home is a daily challenge for her. I had no idea how she would function with another home and bag. My ex-husband agreed to try this arrangement based on these factors. Our mediator was initially intrigued but became a great cheerleader over the course of our sessions. She had never heard of it by name or the concept of Birdnesting as anything more than a transitional arrangement, but she encouraged us to try. She wisely counseled us to have specific rules and to never forget that the house is the safe place for the children. In my travels on Google I had found some ideas that other people used to keep boundaries clear in the nest. My ex-husband and I have 50-50 custody. Financially it works like this: we created a house account that for nest expenses. No cash withdrawals are ever made from that account. Mortgage, utilities, cable, groceries, kid expenses, braces, our life insurance policies, etc., are paid from this account. On paydays, we each pay the nest first. It was determined the percentage that each of us made of our household income (60/40). We added up the money required to run the nest and he puts in 60% and I put in 40%. Anything that comes up that we haven’t accounted for falls under the 60/40 rule. Running the nest is less expensive than running our household while married because we each pay for our own cars, insurance, credit cards, and basic life expenses etc. out of our own money.

Our Nest Rules: 1.  We respect the privacy of the other’s spaces in the nest. 2.  Leave the nest like you found it. The parent that rotates on shouldn’t have to come in and clean up the other’s mess. 3.  No overnight guests of the opposite sex in the nest. They may visit but may not stay past bedtime. 4.  The person on nest duty is responsible for groceries in the house, kid laundry, etc.

Some couples that Birdnest share an apartment where the off parent goes and they rotate between the apartment and house, but neither of us thought we could manage that. I have family in town that graciously offered me a guest room when I’m off of nest duty. I try to be as unobtrusive as possible. My ex-husband has a studio apartment for his off times.

Initially we were trying to figure out how we could possibly have money enough after paying for the nest to afford nice apartments for the children and then we realized that the beauty of this is that the kids never go there, so both of us do it as cheaply as possible. They’ve been to his apartment for short visits but they’ve never spent the night.

How Others React I get mixed reactions when I tell people about our custody arrangement. Some cheer me on and commend me for our unique arrangement. I love those people. Equally, some people are very negative and say that it will never work. Well, it has worked for almost 3 years. Some say that it will never work when we have other partners. So far that hasn’t been a problem either. When we date we just tell them up front about our arrangement. Some are confused until they realize that we are never at the nest at the same time. We cross paths several times a week, but we never stop walking as we pass. I did read about several families that Birdnested for many, many years and in that time both spouses remarried. They simply kept the nest going like before and at bedtime their new spouse went back to their shared home. Their kids grew up and then they sold the family home.

The Downside to “Nesting” There are downsides, I won’t lie. It is expensive. But until the housing market improves we are unlikely to be able to sell the house without losing money. Also, I get tired of living out of a bag. When I go back on nest duty I take my dirty clothes out of my bag, add them to the laundry room floor and pack again when I leave. It is an inconvenience. But then I remember that I’m not a child and I made my own decisions about my marriage and divorce. None of which my kids got a vote on. I remind myself that every day I pack the bag is a day my children don’t have to do it and feel proud that at this time they are spared that experience. The Advantages •  Said to be child-centric: it focuses on the needs of the children. •  Children are able to continue to live in their home for a greater sense of stability. •  Minimizes the difficulty for children of living in two places, like forgetting homework, toys, clothing, or other belongings at the other parent’s home. •  Parents might be better able to handle the experience of moving back and forth between their residences than the children might be. The Disadvantages •  It is expensive. Unless the parents are able to share the residence in which the children do not live, they are looking at having three residences, one for the children and one for each parent when they are not living with the children. •  It requires a large degree of civility between the parents and the ability to get along and make decisions together. In general, the absence of conflict is the best indicator of resiliency for children, both in a marriage and post-divorce. •  It does not take into consideration what would happen when one or both parents enter a new relationship. Even before this aspect of “moving on” occurs post-divorce, both parents often need to individuate from each other and establish separate lives in order to disentangle from the marriage relationship they had with each other. Bird nesting does not adequately foster this aspect of divorcing. •  House rules: in order for parents’ movements to occur in a nondisruptive manner, both parents would need to have similar rules and house cleaning sensibilities. As a parent it’s often difficult to know when a child’s development falls within the ‘normal’ range and when there is cause for concern. Just like learning to walk, children develop the ability to communicate at different speeds, and some will encounter difficulties along the way.

What is the difference between speech and language? Speech is how we produce or articulate the sounds that form words. Language refers to the way we receive, understand and express information through all forms of communication - verbal and non-verbal. Although speech problems and language problems are different, they often overlap. A child with a language problem for example, may be good at pronouncing words but be unable to join more than two words together. A child whose speech may be difficult to understand may still use words and phrases to express ideas. And another child may speak clearly but have difficulty understanding instructions.

As a very rough guide, and remembering that each child is different, your toddler should have an expressive vocabulary of around 50 words by the time they are 2 years old, and you should be able to understand about half of what they say. If your little one only uses certain sounds or words repeatedly, and is unable to use spoken language to communicate more than their immediate needs by this age, he or she might have speech or language problems, or a combination of both.

To help a child with a lisp, a therapist will demonstrate the correct production of the ‘s’ and ‘z’ sounds, highlighting to the child how the tongue is hiding behind the teeth. The child is given lots of opportunities to listen to the production of the correct sounds, and then helped to produce ‘s’ and ‘z’ correctly from a sound to use in conversation. For children who stammer, the therapist will try to establish any factors that may be contributing to the problem, and work with the parent on strategies to help the child to talk more easily, including overcoming fear and anxiety.

I wanted to share some tricks and hacks I’ve found that might help you with your picky eater. Anything help is good, right?

1.Dinner Winner Plate Make eating a game!

7. Let go of your end of the emotional rope.

No one likes losing (especially if you are competing against your brother or sister) and at the end, if you make it all the way through, there is a surprise!

You can’t force your child to do anything, especially eat, so just stop trying. Simply offer her nutritious, varied foods—and eat them yourself. She can have hers, or not, but you’re showing her how. Do you still remember having to eat your veggies or clean your plate before you could leave the table? Adam Strauss, M.D., a pediatrician in Westwood and Mansfield, MA, offers a word of caution. “When parents demand that their kids eat certain foods, they’re attaching negative connotations to it. Pretty soon, the struggle is worse.” Put the food on her plate, but if it stays there, don’t push her and don’t stress over it.

2.Food Swaps These picky eater swaps make total sense! This chart gives you a list of foods and you choose which things your child already eats. Then you can see if your child likes that, the chances are great that they will like this.

3. Copy Kids This technique is kind of crazy, and I’ll be honest, I had the DVD and lost it before I ever tried it with my kiddos. They say kids can learn best from other children, so this features videos of children eating fruits and vegetables – one for each of the twelve fruits and veggies. You put a bowl of that particular food on the table and pop in the DVD and see what happens. They say your kiddo will start eating!

4.Dip The easiest way I can get my kids to eat something they are unsure of is to dip it. Ketchup, ranch dressing, mild salsa, peanut butter, cheese sauce, yogurt…. There are so many things you can dip fruits, veggies and meats in.

5. Don’t get hung up on the time of day your child eats, or how much she eats at a sitting.

Give straightforward praise, even if he takes only one bite of something new.

For example: “It’s great that you tried the chili!” Basing the praise on how you feel (“Mummy’s so happy!”) sends a questionable message: He controls your emotions with his fork. “I used to feel really attached to my kids’ eating the dishes I’d taken the time to make. My emphasis on my split-pea soup especially made everyone miserable. Finally, one day I ignored the soup but put out some fun sides, and the kids ended up tasting the soup.”

It’s okay if your kid doesn’t eat three square meals every day as long as over the course of a week or two she eats things from each food group.

6. Stick with it. You’ve probably heard it before: A child has to try something ten to 15 times before he likes it. “While that sounds daunting to many parents who have a hard time getting their kids to try something once, it does get easier,” says Dr. Williams. His research shows that once you get the ball rolling on tasting new foods, it takes on average only six attempts for kids to accept them.

Flavouring food with herbs and spices such as turmeric during, and beyond, weaning is far preferable to using salt and can help babies develop an adventurous palate right from the start.”

Salmon, Potato and Beetroot

Julia says: “Beetroot is an increasingly popular and highly nutritious root vegetable. It provides iron; a mineral which many young children do not get enough of in their diets, and is very important for cognitive (brain) development. Babies are born with stores of iron but from around six months these start to deplete, so all dietary sources of iron are extremely important during weaning. Beetroot also contains various other important micronutrients such as folate (naturally-occurring folic acid) and magnesium. Don’t be alarmed if you find your baby’s wee is a bit pink after eating beetroot - this is called beeturia and is not harmful!

Preparation time: 10 minutes Cooking time: 20 minutes

Ingredients: 1 portion of salmon 1 potato 1 medium-sized beetroot 1 teaspoon of oil

Method: 1. Peel the potato and dice into small pieces (1-2cm). 2. Place the pieces into the cooking basket and start a 20 minute cooking cycle. After 5 minutes add the second basket with the salmon in. 3. Peel and dice the beetroot. Add it to the top basket for

the last minute of the cooking cycle. 4. When the cooking cycle has finished, put all the ingrediants into the blender bowl whilst hot, add the oil and blend everything for a beautiful pink puree.

Little Carrot Cakes

Julia says: “Carrots are a great way to give cakes and biscuits a nutritional boost and some natural sweetness. Adding yogurt during baking also provides extra calcium, important for strengthening babies’ growing bones. For a healthier version why not experiment and substitute some of the sugar for raisins or dried apricots instead - dried fruit provides natural sweetness and is also a good source of iron for young children.”

Preparation time: 10 minutes Cooking time: 60 minutes

Ingredients: 2-3 carrots (roughly 150g peeled) 1 orange 200g of plain flour 1 tsp baking powder 100g ground almonds 200g sugar 3 eggs 100g yoghurt 60g butter at room temperature 1 tiny pinch of salt

Method: 1. Preheat the oven to 160 degrees and grease and flour coat one loaf time or smaller shaped ones. 2. Peel the carrots and slice. Put them in a basket and start a 20 minute cooking cycle 3. Squeeze the orange and when cooked, blend the carrots and orange juice into a puree. 4. Transfer to a bowl and keep at room temperature.

5. Add the flour, baking powder, almonds, sugar, eggs, yoghurt, soft butter and pinch of salt to the bowl of the blender. 6. Give it a few pulses, then add the puree. Blend all together. 7. Put the mixture into the greased tin and cook for 30-40 minutes depending on the size of the tin.

HappyEating A survey, launched by leading baby food brand Organix, reveals that one of the biggest barriers to introducing new foods during weaning is baby’s facial expression in response to new tastes. It can be challenging to introduce new foods during weaning and the faces babies pull can be interpreted by parents as disgust or dislike at what they are trying, which can make it all the harder. The Organix weaning survey reveals: •    More than half (55%) of mums say a baby’s facial expression puts them off trying to     encourage their baby to like a new taste and flavour. •    49% say it’s a struggle to keep babies interested in food. •    42% say they find it hard to get babies to try new foods during weaning. From sweet carrot to sour lemon and bitter broccoli, weaning is the beginning of a taste adventure, the first opportunity for your baby to explore and experience taste sensations. Evidence shows that it can take between 10 and 15 times to introduce a new food or flavour and that learning about food is all about familiarity and repetition. So by understanding more about their babies’ facial expressions, parents are likely to feel more confident about introducing new foods. Baby faces and understanding what they mean … But what is the meaning of that face your baby pulls as they try a new food for the first time? What’s your baby really trying to tell you?

Mum Tip “Keep trying - if your little one rejects a new food, offer it again in a few days’ time. If they still say no, try again in a few weeks’ time”

“For example, what mums and dads might interpret as a look of shock or disgust, doesn’t necessarily translate as not liking the taste, what’s more likely is that baby is puzzled by the intensity of a new flavour. A surprised expression could be curiosity about the depth of flavour, the chances are they think the flavour is good but it’s very different to what they’ve eaten before – it’s all part of them experiencing and learning new tastes on their weaning journey.” It can be challenging to keep offering new foods and coming up with new and inspiring ideas to tempt little ones, but it’s important as those first tastes during weaning help define a child’s future relationship with food.

This is a really handy product as it folds up and fits into a rucksack for carrying around with you. The bag for use on its own is fantastic and mess free. The seat fits well as a child loo seat on all the toilets I have tried it on (and it’s been a few!). The reason I haven’t given 5 stars is sometimes it’s hard to refold and more significantly because the seat itself is really small.

Looking at the teat on the bottle it is definitely more designed to mimic a breast and the feeding hole is not placed right at the end of the teat so it squirts straight to the back of the throat. Initially I was considering two other highchairs, but once I saw this one I knew it was the one. It’s a regular chair that adults can use, for a little more it becomes a highchair and it’s easy to clean, looks good and is well-built. My apartment is so small it’s easier to take the highchair elements off and use it as an extra chair rather than fold it and try to store it somewhere. I am Extremely happy with this chair.
