Letter from the Publisher Dear Readers, From our family to yours, we want to thank you for picking up your copy of Retirement Connection. I joined my daughter, Amy, in order to better our presence in the Greater Salem area. I am excited to be working with family and amazed to watch the guide flourish in Marion, Polk and Yamhill Counties. As a former advertiser in the guide, I witnessed families struggle while trying to navigate the maze of retirement issues. I shared their relief when I was able to hand them a copy of a guide and show them the wealth of resources available within our community. My experience as a nurse, family caregiver, and 10 years as an administrator for senior housing will continue to be an asset in growing this guide to be the most comprehensive resource available for those 50 and better. The table of contents covers more than 50 topics of information and is color coded to the page tabs for easy reference. An alphabetical index and advertiser index are in the back of the guide for quick searching. Comparison Grids give specific details about services, amenities and pricing. A Caregiver Checklist and Information Request Form

are both found in the back of the guide to provide ongoing support. Parkinson’s Disease and Exercise Parkinson’s disease, (PD), is a progressive, incurable, neurological disorder that affects movement – among other symptoms. As the disease progresses, people will often develop problems with trembling, muscle rigidity, balance and gait. Exercise has been shown to help combat symptoms of Parkinson’s disease as well as enable the person with PD to maintain a positive mental attitude and keep depression at bay. Most people diagnosed with Parkinson’s disease are over the age of fifty, and have already experienced age related muscle de-conditioning. Evidence suggests that exercise can help improve some of these symptoms while improving muscle strength, flexibility, and reducing muscle cramps. Motions in the joints become more restricted, and as a result, flexibility decreases, tendons become less forgiving and muscles begin to shrink and lose strength. As with any new physical activity, it’s important to check with your doctor. Consider starting out with a modest goal, perhaps with a 15 minute walk if your gait and balance is not yet affected, followed by 5-10 minutes of stretching. If your gait or balance is affected by Parkinson’s there are many exercises that can be performed by sitting or holding onto a chair. You have the power to create your own adventure with exercise. Some

people find that they are more energetic in the morning, while some prefer to wait longer for their body to “loosen up” a bit. Perhaps the best time for you is when your medication is working well and you are feeling the most energetic. There is no “wrong” time of the day to do your exercises, it’s just important that you do them! In order to make your exercise more appealing, fun, and not so much of a chore, try joining a local exercise class, grab a work out buddy, or do your exercises listening to your favorite music. Some people find a pool workout not only fun, but invigorating. The most important thing is to find a program that works best for you and be diligent. Regular exercise has been shown to delay the symptoms of PD, so get up and get moving! Cooking for a Cure It’s a fact. The lifestyle choices we make can dramatically influence our likelihood to prevent or deter the progression of cancer and other chronic diseases. Registered Dietitian Sarah Fronza says the recipe is simple. Eat plants. Manage stress. And get moving.

Though the tenants to living healthfully are simple, people often need help in figuring out just how to do it. Exercise is often the easiest place to begin, since any movement at all is beneficial. Nutrition guidelines are however, sometimes more difficult to unravel.

According to the Oregon Partnership for Cancer Control’s Breast Health Task Force, by the year 2030, 1 in 2 men and 1 in 3 women will be diagnosed with some form of cancer over the course of their lifetime. The American Institute for Cancer Research estimates that overall, about one-third of cancers could be prevented by maintaining a healthy weight, focusing on a low fat plant based diet, and exercising at least 30 minutes a day.

“The field of nutrition progresses rapidly,” says Fronza. “We know a lot more about what the individual can do to reduce their disease risk just by changing their diet.” Fruits, vegetables, whole grains, and legumes have all been shown to have important nutrients and other diseasefighting substances.”

“We all have a choice in this,” says Fronza. “It’s not only those with family history that should be concerned about chronic diseases such as cancer, heart disease, osteoporosis and diabetes,” says Fronza. “Everyone can benefit from a healthy diet and regular exercise.” For cancer survivors, the benefits are even greater. “Cancer survivors are at increased risk for second cancers,” says Fronza. “Taking steps to reduce the odds of recurrence can greatly improve a survivor’s quality of life.”

Telephone Assistance Programs help Oregonians In 1987, Oregon Legislature passed a law assuring adequate and affordable telephone service is available to all Oregonians. Based on that legislation, the Public Utility Commission of Oregon implemented these three programs:

Telecommunication Devices Access Program (TDAP) Loans adaptive telephone equipment at no cost and with no income restrictions to eligible Oregonians who have a severe hearing, severe speech, vision, cognitive or mobility impairment and cannot benefit from the use of a standard telephone. To receive the equipment, qualifying customers must complete and sign Section A of the application available in English and Spanish. The appropriate licensed physician, audiologist, speech-language pathologist or rehabilitation counselor must complete Section B certifying that the person requires adaptive equipment to use the telephone.

Oregon Telephone Assistance Program (OTAP) —Link-Up America Oregonians who receive one of these benefits may receive up to a $13.50 reduction in their monthly bill for local residential telephone service:  Food Stamps  Temporary Assistance to Needy Families (TANF)  Supplemental Security Income  State Medical Programs or Medicaid

The Link-Up America Program helps qualified individuals by paying for one-half (up to $30) of the line connection charges for new residential telephone service. Many residential telephone and cellular companies participate in OTAP. Customers can complete or download applications online. Application are available in Spanish, Russian and Vietnamese.

Oregon Telecommunications Relay Service (OTRS) As required by the Americans with Disabilities Act (ADA) is a telephone service that is available for all Oregonians and provides telecommunications access that is functionally equivalent for persons with and without hearing or speech disabilities to communicate via the telephone. There are several forms of the OTRS available depending on the needs of the user including Captioned Telephone Service, Voice Carry Over, Hearing Carry Over, Speech to Speech Relay Service, Shared Non-English Language Relay Services and Text-to-Voice TTY-based Relay Service. Home Health Definitions: Cardiac Care refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac rehabilitation is a program designed to help a person with heart disease return to health. Cardiac rehabilitation may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.

Diabetic Management refers to services including patient assessment, diet information, management of the disease process, medication management and education on all of the above. Services are provided by a Registered Nurse under the supervision of an M.D. Some agencies may employ certified diabetic nurse instructors or cardiac care specialty RN’s. Home Health Aides provide support services such as personal care which do not require a licensed nurse. Medicare does not cover home health aides unless the person is also receiving skilled nursing care or other therapy. Certified Nursing Assistants (CNAs) provide personal care such as bathing, dressing, transporting and other essential activities.

Hospice is a comprehensive program of care to patients and families facing a life threatening illness. Hospice emphasizes palliative rather than curative treatment. The dying are comforted. Professional medical care is given, and sophisticated symptom relief provided. Very few Home Health Agencies are also licensed for Hospice care. In-Home Care/Personal Care is care related to activities of daily living, including: mobility, eating, toileting, bathing and personal hygiene, dressing and grooming or cognition support. These services are usually provided on a private pay basis and do not require a physician’s order. Intravenous Therapy is any medical treatments/medications that need to be introduced to the body via a patient’s veins. Numerous IV Therapies can be provided inhome under supervision of a MD. Some of these include: inter-infective/antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance. Psychiatric Services refers to the agencies ability to offer services related to psychiatric support with the prevention, diagnosis, and treatment of mental illness. Services may include counseling or administering of medications. Registry Services refer, but do not employ, independent caregivers. Clients control all aspects of the arrangement, including compensation, directly with their caregivers. Registries may include free non-profit listing services or companies that require training, bonding and insurance of their caregivers. Clients are charged a referral fee.

Skilled Nursing is a higher level of care where services are preformed by a licensed Registered Nurse or Licensed Practical Nurse. Skilled Nursing includes, but is not limited to patient assessments, performance of prescribed medical treatments, administration of prescribed medications, education for the patient and the caregiver (if available) related to the disease process, treatment, health measures and medication management. Skilled Nurses provide medical care as prescribed by physician.

24-hour care is “round the clock treatmentâ€? in a home environment. The care may be shared by multiple caregivers or by a single caregiver. Therapy Services (ST, PT, OT, RT) Occupational Therapy, (OT) refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function. The goal is to help patients become as independent, productive, and satisfied as possible. Physical Therapy (PT) refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills. Exercise techniques are only part of the special skills used by therapists to treat patients to help lessen disability, pain and improve overall function and quality of life. Respiratory Therapy (RT) refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways. Speech Therapy (ST) refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing. A Therapist provides individually designed treatment programs to maximize communication and swallowing.

Easter Seals Oregon: Learning A Living Program Oregon employers will benefit from the two five year grants the Department of Education awarded to Easter Seals Oregon. These awards could bring in new training and employment resources to Oregon at a time when economic conditions are stressing the capacity of employment and training funds. The project will serve persons living in the Multnomah, Clackamas, Marion and Polk Counties. These funds will provide job seeking skills, computer training, direct placement services into permanent employment and self employment support to persons over the age of 40 with a documented disability. It also will serve as an important resource to our returning veterans who may no longer be able to work in their preservice occupations. The program is actively enrolling clients who desire the outlined services, and meet the program’s eligibility requirements, in addition to employers focused on diversifying their recruitment resources and staff.

40 years and older A documented disability A willingness to fully participate in the program A commitment to becoming and remaining employed

Housing Definitions: Adult Day Care Programs offer a protective setting during the day, for those who cannot be left alone due to health care and social needs. The structured program within a community setting offers: socialization, planned activities, care services, companionship and meals. Adult Day Care provides daytime respite for the caregiver who typically works, needs time for other responsibilities, to simply to take a break. Some programs are designed for older adults with dementia and/or physical limitations. Programs through senior housing communities that are listed in this guide must be able accept non-residents to be included. Adult Care Home or Family Care Home are private residences that provides a home-like setting, and serves 5 or less residents. The caregiver may perform several functions, such as personal care, housekeeping and activities and group meals. There are three levels of Adult Foster Care Home licenses. The classification system is based on level of care the Adult Foster Home may provide to residents who live in the home as well as the experience and training of the providers and their ability to assist residents with: personal hygiene, mobility, eating, dressing, toileting, and behavior management. Residents may require very light assistance or may be dependent with several care needs; it is best to ask individual locations for their specific policies and license. Assisted Living Communities are State-regulated rental properties where six or more residents may reside. Care assistance is available, including: medication management, bathing, dressing, toileting and eating. Apartments must be fully self-contained private living units with a lockable door, private bathroom, and kitchenette facilities. The fee schedule is regular monthly rent along with additional fees for specific services and amenities. Assisted living communities are best suited for individuals who want to remain as independent as possible and who are able to direct their own care. CCRCs (Continuing Care Retirement Communities) are communities that include a continuum of care from independent living, assisted living, residential care and skilled nursing services all on one campus, allowing individuals to live within the same community as their needs progress through the spectrum of care. They typically offer the full selection of amenities associated with retirement living. An endowment fee in addition to a monthly maintenance fee can be expected. Independent Living Communities provide residents an independent living setting without the burden of home ownership. Typically, residency is established on a monthly rental basis. Residents live in fully equipped private apartments or cottages from studios to large two-bedroom units that may be rental-assisted or market-rate depending on the community. Amenities and hospitality services such as housekeeping, linen service, transportation and social and recreational activities may be included, provided for an additional charge, or may not be available at all. These communities do not provide assistance with activities of daily living or personal care.

Housing Category

Memory Care or Dementia Communities offer or provide care for residents with Alzheimer’s disease or other dementia in a home like environment. These communities, must receive an endorsement and is governed by additional regulations that are specifically intended to support individuals with dementia, including: a secure building that alerts staff if a resident has exited, a secure outdoor area that provides outdoor freedom safely, interior finishes that are non-glare and well lit, and visual contrasts between floors, walls and doorways. Alzheimer’s units must also have programs, which include: gross motor, self care, social, craft, sensory enhancement and outdoor activities. Rehabilitation & Therapy is treatment for an injury, illness, or pain with the goal of restoring function, including nursing and therapy services. Rehab is ordered by a physician and services are provided by nurses and physical, occupational, and speech therapists. Examples include working with a physical therapist to help you walk and with an occupational therapist to help you get dressed. Residential Care Facilities provide housing and supportive services for six or more people who do not require 24 hour nursing care. Pricing can vary greatly depending on the level of care provided and the size and amenities of each apartment. Accommodations are typically in a home like setting and may range from a shared bedroom with common bathroom, to private apartments with kitchenette and private bathroom. Monthly fees vary greatly based on unit amenities and care services. Many residential care facilities specialize in caring for individuals with Alzheimer’s or dementia. Respite Care is short term relief for family caregivers to take a break from the caregiving responsibilities and allows for a healthier and better quality of life for both the caregiver and care receiver.. Respite services include: companionship, personal care, recreational activities and security. Respite care can be provided as short stay or for a longer period that allows the family caregiver to go on vacation or a business trip. Longer duration respite care is generally found at select senior housing communities although some Companion services may provide this service in the home. Skilled Nursing Facility (SNF) or Nursing Homes are Statelicensed facilities that provide a safe, therapeutic environment for individuals who require rehabilitative care or can no longer live independently due to functional or cognitive impairment. Nursing Homes offer 24-hour skilled nursing care and medical services by registered nurses and support professionals. This is the highest level of care that can be provided that is not hospitalization. Additionally, nursing facilities offer residents planned social, recreational and spiritual activities. Nursing facilities are most appropriate for people who need 24-hour medical oversight and a protective/structured setting. Most residents must share their room, but residents are allowed to bring personal items to encourage a more home-like atmosphere. Assisted Living Resources Licensing requirements for assisted living facilities vary by state and can be known by as many as 26 different names including: residential care, board and care, congregate care, and personal care. Choosing A Community A good match between a community anda resident’s needs depends as much on the philosophy and services as it does on the quality of care. The following suggestions can help you get started in your search for a safe, comfortable and appropriate assisted living community:  What are your future needs and how will the community meet those needs?  Is the community close to family and friends? Are there any shopping centers or other businesses nearby (within walking distance)?  Visit each community more than once, sometimes unannounced.  Visit at meal times, sample the food, and observe the quality.  Observe interactions among residents and staff and ask questions.  Ask about social, recreational, and spiritual activities.  Ask about the type and frequency of staff trainings.  Review state licensing reports.

These steps should also be considered:  Contact your state’s long-term care ombudsman to see if any complaints have recently been filed against the assisted living community you are interested in. In many states, the ombudsman checks on conditions at assisted living units and nursing homes.  If the assisted living community is connected to a nursing home, ask for information about it, too. When is the right time to move? Most families struggle with this question. Sometimes the answer can be obvious, but it is not always that simple. Everyone wants to maintain an independent lifestyle, but if assistance is needed, it might be time to talk about alternatives. Watch for these signs:  Accidental falls due to loss of balance  Memory lapses are a sign of health issues, an empty refrigerator means your parents are forgetting daily nutrition, and unopened mail indicates memory loss.  Hygiene issues like going days without bathing, piles of laundry, or wearing the same clothes for days.  Loss of interest in daily activities and socializing with friends and family.

What type of community is most appropriate? When it’s time to find senior housing for your loved one it’s easy to get confused. Some options are:

Independent Living provides basic services such as meals, housekeeping, activities, transportation, and social activities.

Assisted Living is a state-licensed community that gives the basic services independent living provides, but also offers assistance with bathing, medication management, dressing, personal care, and typically have a full time registered nurse, certified med-aides, and caregivers 24 hours a day.

Memory Care is a state-licensed residential care community that provides personal assistance for those with Alzheimer’s and dementia while maintaining a high level of independence and promoting privacy, dignity, and choice. The memory care program provides a daily structure that allows the residents to maintain their abilities and encourage the use of their remaining skills. When determining which option is appropriate, ask yourself these questions:  Do they need help performing basic activities such as eating, bathing, dressing, or walking? If so, it makes sense to move them to an assisted living community.  Are they falling? Falls can result in costly trips to the hospital and potentially put their life at risk if they’re unable to get help. Independent living community on-site manager(s) monitors falls and are trained to assist a resident in getting up. Independent living community’s on-site manager(s) have to call 911 if the resident cannot get up on their own.  Are they experiencing memory loss? This could be an early sign of Alzheimer’s or dementia. A memory care community has 24-hour staff who are always available and provide a safe environment for your loved one.

Senior Living Communities Staff and Residents Support Casting for Recovery Brookdale Senior Living and Casting for Recovery have teamed up together to assist women of all ages, and at all stages of treatment and recovery from breast cancer to learn new skills in coping with the diagnosis and providing help in moving forward to a healthier life. Brookdale Senior Living, industry-leading owners and operators of senior living communities, has raised approximately $13,500 to date for Casting for Recovery, a national non-profit organization which supports women recovering from breast cancer in a program that combines instruction in fly-fishing, counseling, and medical information to build a focus on wellness instead of illness. Ten thousand dollars of this amount is a matching donation made by Consonus/VitaLife, Brookdale’s pharmaceutical partner for the Northwest region. Brookdale’s goal is to raise $100,000 in 2010 for Casting for Recovery. In addition to fundraising events, Brookdale is working with local Casting for Recovery contacts in each region to form personal connections between residents and associates. Local Casting for Recovery volunteers or fly fishing club members assist and connect with each community and its residents who embrace the cause, including casting sessions with associates and residents.

Talking to Your Family About Aging and Incapacity The next time you have a family gathering, one of the best things you can do for yourself and for your family is to engage everyone in an honest and open discussion about aging and disability planning. Many adult children and their parents think about the potential for incapacity, but the majority of them never bring up the matter with each other. Top of the list of worries are money, financial management and health care decisions:

1. Your Financial Situation One of the biggest worries for parents and their children is money. Parents and children each worry about whether the parents will have enough money so they can enjoy their retirement and, if necessary, pay the costs of long term care. Maybe this is a conversation you also want to have with a financial planner to make sure you have covered all the bases. A family discussion about this matter can take the fear and emotion out of this topic for both parents and children.

2. Who Will Handle Your Finances If You Become Ill? Decide who you want as your agent to handle your finances if you are incapacitated and how you want your finances managed, When you have made these decisions, share this information with your loved ones. And, seek out an

Discuss these matters with your loved ones and put them in writing in an Advance Directive for Health Care. How to Choose a Financial Advisor How do you choose the right Financial Advisor? For starters, ask your friends, relatives and co-workers whom they use. Then interview some of the people they recommend. What questions should you ask at such an interview? Consider these:

What are your qualifications? Make sure you are talking to someone who, at a minimum, has all the required licenses for selling securities.

What type of experience do you have? Find out how long someone has been a financial advisor, but don’t rule out a person with only a limited amount of experience—a new financial advisor frequently brings a great deal of enthusiasm to his or her work. A financial advisor’s longevity is less important than whether he or she has had experience working with someone like you, someone in your financial situation, with your goals and your investment preferences.

What is your investment philosophy? Try to learn if someone favors a specific style of investing or a particular class of investments. These styles or classes may be well-suited for some investors but inappropriate for others. If you believe the person you’re talking to has a “one size fits all” mentality, you might want to look elsewhere.

How will you communicate with me? Financial advisors run their business in different ways, so there’s no one “right” way of communicating with clients. However, you need to feel comfortable that someone will always be available to answer your questions, review your accounts, evaluate your situation and make appropriate recommendations. If you are interviewing someone who has a partner or an assistant, find out whom you are likely to be communicating with, should you decide to become a client.

What services do you provide? Find out just how a prospective financial advisor can help you. For example, some people sell investments only, while others offer investments and insurance. Alzheimer’s: Recognizing The Symptoms Every 70 seconds someone in America is diagnosed with Alzheimer’s. As the 7th leading cause of death, the prevalence of Alzheimer’s is higher than ever. With symptoms beginning at the higher risk senior citizens, 55-65, the disease symptoms and problems become worse as the patient ages. Patients reaching these ages for Alzheimer’s complain about similar types and themes of symptoms. The patient may seem more irritable and uncomfortable in his or her surroundings. This may be a rare occasional onset, but rest assured that these are the main early symptoms of Alzheimer’s. Progression of this may lead to increased confusion and override rational decision making. Still at the early stages, patients may complain about problems formulating proper words. This will also involve math problems, decision making processes for abstract situations, as well as loss of stream of consciousness and thought. Patients who are suffering from early stages of Alzheimer’s may also have noticeable differences in their attitudes about things they once enjoyed, as well as changes in personality. This becomes more evident with the progression of the disease. Patients may seem apathetic and depressed about activities or thoughts that once made them happy and content. They may also begin to stumble during words and forget places and people that

they had a vague recollection of. Again, it is important to note that at the early stages, occurrences such as these are rare and happen infrequently. Progression to medium stages will disable the patient further, where more faces, places, and events will be forgotten to a greater degree. Finally towards the end of the early stages of Alzheimer’s, symptoms may become evident in the personality of the patient suffering from the disease. The patient may be noticeably different in his personality, gaining new and weird behaviors. They may speak or act inappropriately, with this lack of judgment. As the early stages of Alzheimer’s, it is important to make a checklist and record differences in activity and behavior. If one notices the patient in a more aloof and gloomy mood, reflective versus participatory in their activities (and over the age of 60) then it may be symptomatic of Alzheimer’s. It is key to accept the changes in behavior and understand that paranoia, hiding, and irrational behavior are part of the suffering for both family as well as patient.
