I
n our continuing commitment to enhance the Healthcare Technology Management profession and its professionals, RSTI has come to an agreement with DITEC, Inc. to purchase their assets. Additionally we are happy to have retained the services of Manny Roman and Joe Weidner for special assignments, such as attending shows and developing Nuclear Medicine and other programs. During my 24 years at RSTI, I continue to witness change. There are times when it’s tough coming up with the next new story, worthy of printing in this newsletter; however, this is not one of those times. I am delighted at the prospect of combining the two training centers. Since 1992, DITEC has provided effective, basic imaging training with qualified instructors and topnotch curriculum. AS A RESULT OF THE ACQUISITION, HERE ARE SOME THINGS YOU MUST KNOW: 1.
RSTI will adopt and maintain the student records of the service professionals trained at DITEC.

Building six new service bays that will house 12 new imaging systems
The new construction will increase the equipment in our labs dedicated to imaging training to 33 handson X-ray rooms. This does not include the various portables, C-Arms, Bone Dense, Ultrasound and Mammography systems. As I mentioned earlier, Manny Roman, DITEC’s President, will be consulting for RSTI, primarily in the areas of trade shows and marketing. In a similar arrangement, Joe Weidner will be assisting RSTI in the creation of a revised Nuclear Medicine training program. These consulting roles were developed so the industry can continue to grow as a result of their individual areas of expertise. I am certain that all of our students will benefit from their involvement. RSTI and DITEC have long worked to bring recognition to imaging service professionals through education, ASRT and active support of state level biomedical associations. We will continue that support and intend to expand our relationships with trade shows, biomed groups, AAMI, Certification Boards, and where applicable, regulatory issues.
Again, let me assure you that if you have received previous training from DITEC, particularly their Level series, it will be accepted as meeting the prerequisites of our more advanced courses. RSTI will review the training records from DITEC, and when this review is complete, we will be able to approve the rest of their courses as well. PLEASE BEAR WITH US AS WE GET THROUGH THESE CHANGES. The staff at RSTI and I look forward to helping any and all aspiring service professionals achieve their goals, expand their horizons and build meaningful careers. Course Overview Training on Siemens Mobilett By Matt Mondello RSTI Instructor As mentioned in our previous newsletter, we are in the process of developing the course curriculum for the Siemens Mobilett to fulfill a demand by service professionals across the country. The training will cover multiple models but as with most portable systems and their current and previous generations, many of the components and operations are forward and backward comparable making it easier to cover more systems than what is physically on hand.

Some of the manuals are easy to come by via download at the Siemens website but can be quite costly if they need to be physically replaced. There are alternatives on the World Wide Web but they are not always accepted by inspectors as “official� service documentation and are not always 100% complete. Be careful what you download to avoid unwanted files and viruses!
As with most product specific courses, we will start with system and component identification, work our way through the manuals, cover removal and replacement of parts, and wrap up with calibrations and preventive maintenance procedures. The system as a whole requires minimal effort to move and even less to get into. The covers are easily removed revealing the circuit boards and batteries which are all easily accessible for testing and troubleshooting.
Helpful Hint: A good way to stay in the good graces of your facility is to make sure the bottom surface of the Turn Plate is clear of any foreign objects. This will prevent any unnecessary marking/gouging of the floor surface during the turn, keeping your operating/work areas looking good!

Dave’s Rave > Cloning Hard Drives By Dave Domanski RSTI Instructor Of all the tools that have become available to field service engineers in the past twenty years, I can’t think of any that are a greater hero maker than the ability to accurately duplicate the hard drives in our diagnostic imaging systems. Sure, we’ve been able to copy files from one drive to another for decades, but what we’re talking about here is a bit for bit replica, what some refer to as a “forensic copy”. There has been cloning software around for years and much of it is just fine for making a backup of your home PC’s hard drive, but the landscape changes drastically when we move to the drives found in imaging systems. Many systems don’t run Windows, instead relying on Linux or proprietary real time operating systems or sometimes both. Even something as “simple” as a portable C-arm like the OEC 9900 GSP has two separate onboard computers running two different operating systems and sharing a single hard drive! Many software based cloners would have a hard time with this and may not be able to replicate the partitioning or could miss hidden files. I have heard from many students that they have good luck with Acronis products though I have not tried them myself. I prefer to use a hardware style cloner. Yes, they can cost several thousand dollars, but they have many benefits, speed being one of them. Modern hard drive duplicators can operate at speeds in excess of 7GB/ minute. They can also be purchased with the ability to copy drive types still found in many imaging systems such as SCSI, ATA, PCMCIA SRAM cards and PCI-Express cards. They can even allow you to store a disk image on a server at your facility (assuming you are in-house) so that you don’t have to have boxes of backup drives taking up space. You simply make them as you need them. My favorite method is to clone the original hard drive and then store it, operating the system using the clone. Why? Let’s say you have to call in the manufacturer for a non-hard drive related issue. Some vendor field service engineers will refuse to work on a system if it does not have the manufacturer’s hard drive in place. 6 | FOCALSPOT.FALL.2013
Pop the original drive in before they arrive and not only is the crisis averted, but any software upgrades that may need to be done during the service will happen on the usually stored original. Simply reclone and you are back in business. Some of you may be thinking, “Dave, all of my equipment has been upgraded to solid-state drives, hardware failures are a thing of the past!” Well. Solidstate drives are more robust to be sure, but even they have read/write limits that can lead to failure or corrupted software.
You may also be thinking, “There is no WAY my boss/ department head is going to budget several thousand dollars for the purchase of this equipment!” This isn’t about taking a chunk out of your budget, this is about cost avoidance. Depending on the manufacturer, they may charge you upwards of $5000 for a SINGLE hard drive that you could buy on line for $45! That’s if they are willing to ONLY replace the bad drive. Some will only sell you the entire computer for $20,000 - $30,000! Does that cloner still seem pricey? Try this on for size. Your CT scanner is down due to a
bad hard drive, maybe it’s even under a service contract, and the vendor FSE for your region can’t get there for three days due to another call three states away. The imaging department now loses hundreds of thousands of dollars due to that down time and loss of patient throughput. With a clone on sight the system could have been back up in 30 minutes or less! Even a top of the line $6000 duplicator now looks like quite the bargain to any administrator worth their salt.
So, in review: Hard drive cloner, $1200 to $6000... Backup drives, $20 to $300... Saving your facility perhaps millions of dollars?

FIRST CALL SUPPORT Insights from the Field An Unfors RaySafe First Call TechNote Excerpt WHAT IS FIRST CALL SUPPORT? First Call Support is the first line of defense when imaging equipment fails. The in-house imaging biomed is the first person hospital staff contact when there is a problem. By responding quickly, the biomed can begin the diagnostic work needed to determine if a field service engineer must make an on-site visit to resolve the issue or if it can be repaired with in-house expertise. WHY IMPLEMENT A FIRST CALL SOLUTION? Medical facilities implement a First Call solution for several reasons including minimizing imaging system downtime and reducing service contract costs via inhouse technical support.
WHAT ARE THE KEY BENEFITS OF FIRST CALL? People surveyed noted that the top benefits of implementing a First Call support solution are to reduce imaging system downtime and not disrupt throughput. Often an issue can be resolved without needing a technician or the OEM to conduct an onsite visit – which saves time and money! Biomed imaging professionals also reap the benefits of First Call support. Troubleshooting and resolving issues on their own without having to bring in an outside technician increases their value within the medical facility. According to those interviewed, anywhere from 50-80% of the time problems were resolved without having an outside resource come on site. Since imaging departments are profit centers, biomed engineers who can keep that equipment up and running are having a positive effect on the profits of the facility.
HOW DOES FIRST CALL SUPPORT SAVE YOU MONEY?
WHAT ROLE DOES TRAINING PLAY IN FIRST CALL?
First Call support reduces the cost of your service contract. But more importantly, it helps to reduce downtime. Since imaging departments are profit centers, minimizing downtime directly corresponds to the profitability of the medical facility. Depending on the facility, CT scanning equipment can lose approximately $2,500 for every 15 minutes of downtime ($10,000/hr) and MRI equipment can lose up to $3,500 for every 15 minutes of downtime. ($14,000/hr)
All of the First Call responders Unfors RaySafe interviewed sited training as one of the key factors in delivering First Call support successfully. Biomedical engineers will require a certain level of training to ensure they are up to date with the imaging systems and some of the diagnostic readings that will be needed. Going through this training will enable the imaging engineer to gain confidence with the equipment they are supporting and better identify and resolve problems.

Open for Business The Exchange is Here! By Jim Monro RSTI Chairman RSTI has spent the better part of 18 months working on the launch of our new used equipment website for diagnostic imaging equipment. Yes, it will be sleek, fast and full of the information you need to make an intelligent decision about buying from us or selling to us. Since you may not understand the reasons behind our decision to expand this business, I thought I would take a minute and give you a verbal tour (not virtual) of our reasoning from start to finish. 1.
For the last 13 years of our 28 year history the imaging world has been turned upside down with advances in technology. Camera tubes became CCD cameras and CCDs were replaced by CR imagers, and finally true DR arrived starting in Mammography.

RSTI had to make the choice of teaching students from an historical/analog perspective, which would save time, money and talent requirements or boldly go where no other training center had gone before. Going into the mysterious digital world of diagnostic imaging was indeed a daunting task with high risk written all over it.

Within our tiny organization we had to decide if we wanted to be in the major leagues or stay triple A forever and follow the path of other companies that only look backwards with 20/20 vision. Think of those companies that never believed we would have a gasoline engine, a plane that would fly, or a laptop, a cell phone or new an actual computer on your wrist or to wear as glasses.
4. Technology, in any industry, that will provide faster, better actionable results is the wave of the 21st century. You can get on or get buried in someone else’s wake. RSTI chose to launch a two-pronged approach about 10 years ago, geared to maintain the existing analog franchise and usher in digital change. Out went the old equation for success and in came “debt service,” a term 12 | FOCALSPOT.FALL.2013
not generally used in imaging training. Here is how that progressed for us: 1.
Hologic, Inc. hired us to teach their then brand new digital breast mammography system, aka Selenia. We studied with their engineers and their service organization over an extended period of time, never really knowing that they planted a seed that would germinate over time allowing us to grow for the subsequent 10 years.

From there came Phase 5 and PACS, and it’s been downhill ever since.

We went from 10-15 year old equipment to 5 year old equipment in a blink of the eye.
In order to keep up with technology change, we had to sell what we owned and buy what we needed. This simple cycle of keeping training on the leading edge spawned the need for a new business of buying and selling used imaging equipment. Today this fledgling business has grown into its own business segment at RSTI, and it is called:
RSTI EXCHANGE, INC. This new business will be launched online in the upcoming weeks with its own website, replete with a
whole new look at how to buy and sell imaging equipment. The goal will always be to feed our training labs with new, vital technology, but now we are proud to lend our expertise to buying and selling globally, installing and de-installing. Look for the launch in the next several weeks, and then ask yourself, “why would I buy or sell my equipment anywhere else than my training center?” We are a 28 year old company that is still standing and providing top notch credentials for service repair and maintenance across the globe. Our educational mantra will remain the same: The right equipment, the right curriculum and the right instructor will save hospitals and ISOs time and money into the hundreds of thousands of dollars. Our used equipment mantra will be revised to look like this: The finest equipment our customer can afford guaranteed to be in excellent working condition, because it has been purchased, repaired and maintained by the men and women of RSTI.

RSTI Registration Process
A
s many of you may have already heard, RSTI has acquired Ditec’s assets to include all student records. My primary goal is to keep Ditec’s student records intact and maintain them here at RSTI to preserve the information and have it available as needed. If you have taken a class at Ditec, rest assured that we will have a record of your training here at RSTI. Further, Ditec’s classes will be accepted to meet prerequisites for more advanced training here at RSTI. For example, the OEC classes require completion of RSTI’s Phase III “Advanced Fluoroscopic Systems Maintenance” or equivalent training or experience. In this case, Ditec’s Level III “Advanced Concepts of Fluoroscopic Imaging Maintenance” will be considered an equivalent to our Phase III. This will most likely hold true for all “phase” classes. Likewise, you can continue with your phase training here at RSTI without having to re-take or test out of any phase classes you may have taken at Ditec. At the completion of Phase IV “Advanced Diagnostic Imaging Systems Analysis,” you will receive recognition as a “Certified Diagnostic Imaging Instrumentation Specialist” from RSTI. Registering for a class here at RSTI is easy. You can phone, fax or email your registration. It’s just a matter of completing your information (name, institution, address, phone and email) and selecting what class or classes you want. Generally, it’s a good idea to sign up for a class even though it may not be approved yet. This will prompt us to enter a tentative registration, or “pencil you in,” and then be able to contact you if there are any changes or if a class starts filling up. When approval is obtained, you can confirm your registration by calling, faxing, or emailing a guaranteed method of payment for the class. RSTI accepts purchase order numbers (PO), checks which can be mailed in advance or brought to class, and credit cards. We advise holding off on making any travel arrangements until after the confirmation process is complete. In general, detailed confirmation letters are emailed directly to each student about 2 weeks before class begins but students are welcome to call or email at any time to verify the status of their registration. We strive to make registering for a class at RSTI as easy as possible and are ready to welcome Ditec students into the RSTI family. RSTI Recycles One of the main objectives of RSTI Exchange is to give healthcare providers another option with the equipment they are retiring. As we all know, many of these units are bought and sold in the “used market”. Many of these units should not go back out into the world as they are out of date and many times in disrepair. Units such as these are what RSTI Exchange earmarks for our recycling program. We bring these units into one of our warehouses to be stripped down and disposed of. We don’t simply throw them away. We strip them down to each material and take them to the proper facility to be reprocessed and reused. If you think about all the materials used in one mammography unit, or one radiographic room, there are so many different metals and plastics involved. We separate these materials and send them to select waste management companies to be broken down and repurposed. Please let us know if you have any equipment you are looking to retire. While there may be a charge for us to remove these items, RSTI Exchange would be more than happy to take care of them for you. We have a professional de-install team that can be dispatched to remove the units and take them off your hands.

Cancellation Policy - Tuition is only refundable if cancellation is received in writing by RSTI at least 14 calendar days prior to the course or seminar date. Allow 30 days for receipt of the refund. Registrants who cancel within 14 days of the course or seminar date will have the tuition transferred to another course or seminar on a space available basis. In the event that a course does not attain our minimum enrollment, RSTI may postpone it. Paid tuition will remain on account to be applied to the rescheduled class.
Payment Due Policy - Payment for all courses and/or seminars is due three full weeks in advance of the course start date. Purchase orders are accepted, but also must be paid one week prior to the course start date. We also accept VISA, MasterCard, American Express, and Discover as a method of payment. All past due invoices will be assessed a late fee of 1.5% per month on any unpaid balances.
Schedule - Typically, courses run from 8:30 am-4:30 pm Monday through Friday. You will receive a detailed schedule when we confirm your registration. You should schedule your incoming flight to arrive Sunday afternoon or evening. On the last Friday, class ends at 12:00 noon. You may schedule your return flight any time in the mid- to late-afternoon.
Full Coverage Package - You are responsible for expenses you incur while attending courses. However, RSTI has negotiated a reduced package rate that includes 12 nights hotel accommodations, transportation between the hotel and training center, breakfast, and lunch. We will notify you of this rate when you enroll; if you wish, we can add the full coverage package cost to your tuition.
