We would like to introduce the concept of “the New Basic.” The New Basic is: an x-ray system retrofitted with a DR plate; a 3-phase x-ray system using CR cassettes; high frequency x-ray generators; digital fluoroscopy; digital radiography rooms; completely digital mammography departments; PC based ultrasound; and multi-slice CT systems. All of these systems are interfaced with PACS systems for storage; HIS/RIS systems for patient scheduling; and a broad array of printers and review work stations. While RSTI is doing everything we can to add the latest equipment to our labs, there are others who will try to tell you that by not putting any effort into improving the equipment they use in the labs for training, they are doing you a service because they are teaching you “the basics.” This is the same as saying you will be qualified to service all the life-support systems in the O.R. because you’ve been trained on the LIFEPAK 5.

Our commitment to improving your training experience appears to be working. Many of our classes are full well in advance of their scheduled dates. If you have interest in upcoming classes please be sure to let us know as soon as possible. We will be able to register your interest by “penciling” you in for a class but that cannot hold the seat. You will need to “confirm” your attendance before the class fills up. With classes as far away as October filling fast, a confirmed booking is the only way to ensure that you will be able to get in a class. Check out the schedule on pages 8 & 9 in this issue. We have marked the classes that are already full to help you with your planning. If you wish you can also be added to a wait list for a particular class. If someone cancels we will contact you.

The New Basic is what the modern service professional faces every day, so RSTI makes sure the New Basic equipment is in our labs for you to train on. We do recognize that there are still a significant number of the older systems out there, so we keep a few older systems to cover the “old basic” technology. RSTI has added an OEC 9900 to our Mobile C-Arms group of classes. Since there was a shortage of quality units on the used market we decided to purchase a new one directly from GE. That isn’t all that remarkable if you consider our commitment to have the best hands-on training available.

Letter from the President If you have not read the following article that I lifted from the publication “DOTmed”, and you are in the health care business today, you should:

NY lawmakers cut $775 million from health care budget June 08, 2010 by Brendon Nafziger, Writer

The article specifies that New York hospitals just lost $250,000 in subsidies, and that even with the cut the State of New York will run a $10 billion deficit in 2010. There are no details on the rest of the $775 million. Normally I do as most of you out there in the trenches keeping equipment running at optimum level for a quality diagnosis, and that is, I wonder when all the bad news will stop…Then it hit me that there are no ratings in good news. Good news does not sell anything, so we have to emphasize the bad to a point that drowns out all the good. I am here to tell you that RSTI is teaching hospital service professionals, ISO’s and 3rd Parties how to SAVE money with quality hands-on training. This is not some accounting pencil magic that allows you to depreciate at a faster rate. This is hard

Look at what you spend each month on OEM contracts for your imaging equipment. Look at the money spent on your last CT tube or Image Intensifier. You could save your facility a bundle if you can get by the OEM scare tactics. I truly don’t know what is worse, their scare tactics or the $400-$600 per hour they charge from portal to portal. I know lots of hospitals are closing across the country because I get invited to their auctions. No one really knows the effects of our new health care legislation. It is so complex and has been altered so many times, that the lawmakers themselves don’t know what they voted for this time. If I ran a hospital today, I would look at imaging training as one way to save me hundreds of thousands each year, and I would find some companies that can supply me with quality, used equipment and parts. I know of no facility that has not been hit and hit hard by the economic and political distress put upon them. Within 20 miles of me there are two of the best facilities in the world, The Cleveland Clinic and University Hospitals. With the new medical mathematics today, even these giants have realized that it is NOT business as usual. Time for you to do the same.

Like the other great classes offered by RSTI, our one week OEC 9900 course is chock full of the hands-on experience that our students demand and get here in our worldclass facility.

The OEC 9900 course is a mixture of lecture and practical lab experience. Students will be getting plenty of hands-on experience with a brand-spanking-new OEC 9900. They will perform all applicable image system calibrations, including X-ray tube and image intensifier removal and replacement, beam alignment, collimator centering and calibration, camera decentration, camera electronic centering, lens fo c u s, i m a g e i nte n s i f i e r electronic size and focus, iris stop adjustments and more.

Many of the changes made to the 9900 are found in the monitor cart. These will be covered as well including the 9900 specific direct connect procedure, log retrieval and the greatest down-time preventer and field service hero maker of them all, hard drive cloning.

Greetings from the labs at RSTI! Hospitals are literally saving millions of dollars by training their own talented engineers and managing their own equipment. A very popular first step to taking equipment off bloated service contracts is to start with the much abused portables. The mere addition of mobility to any x-ray system increases the likelihood that it will come into harm’s way exponentially. Make that portable system a high end digital fluoro unit and you could be looking at serious headaches, especially if you have to rely on the vendor alone for service. It is for this very reason that I am giddy with anticipation and very much looking forward to teaching our new OEC 9900 Elite C-Arm class.

Cover’s Column: 25 Years of Change When RSTI first opened its doors OEMs ruled the roost when it came to the Imaging Departments of nearly all of the hospitals in the country. Our mission, at that time, was to provide high quality educational opportunities that would make it possible for the hospitals to service their own imaging equipment and save the expense and frustration of having only one source for their service needs. Today, there are many Hospitals and Independent Service Organizations that provide imaging system service and maintenance programs. Even the OEMs are getting into the game. Hospitals have options now. They can maintain their service contracts with the OEM, hire an Independent Service Organization or develop their inhouse service department and service their own equipment themselves.

Twenty five years ago there were no digital radiographic rooms; Mammography ACR Accreditation was voluntary; very few High Frequency X-Ray systems existed; and CT was not only single slice but actually had hard drives with removable disks that required alignment of the read/write heads. Back then we had equipment in our labs that reflected the technology of the day. Today it would be almost impossible to buy a new 3-Phase X-ray system; they are all high frequency of one form or another. There isn’t a modality that hasn’t been touched by digital imaging, even veterinary medicine is moving to digital imaging and it seems there is no limit to how high we can go in slice count for CT systems. As these new technologies rolled out at a relatively fast pace the equipment prices rose even faster. Despite the dramatic increase in the price of current systems, RSTI has managed to

assemble a collection of systems that reflect current technology. These systems come at a very high cost compared to what we paid 25 years ago. Even with the skyrocketing cost of the high tech equipment in our labs RSTI remains a fantastic value. When RSTI began, the cost per hour for OEM service was about $100 and the cost for training was roughly $750 per week. Compare that with the current hourly charge of about $300 for General Radiographic/ Fluoroscopic systems and the cost of our RSTI Phase training is for $2200 per week. Simply put, over 25 years the cost ratios between service and RSTI training have stayed roughly the same. Given all the changes in technology you’ll be pleased to note that some things remain the same. RSTI still provides the best handson experiences you can get anywhere.

MIKE’S TECHNICAL TIP: Uninterruptable Power Supplies, UPS With many of the systems we work on going the way of the computer we are seeing the increased use of UPS’s or Uninterruptable Power Supplies. The primary role of a UPS is to supply adequate power for a period of time for the PC to shut down properly. With the use of UPS’s we encounter some new challenges; especially with UNIX based computers. As you may already know computers do not like to be shut down “hard”, or powered off without going through the proper shutdown procedure, especially UNIX based computers. When these computers are shut down incorrectly there is the possibility of having files corrupted on the PC, usually requiring a service call. To avoid these potential issues we need to take notice of the UPS system that is included with the medical equipment. The UPS is often overlooked when doing preventative maintenance. Usually the batteries are the primary problem, as batteries age and hold less of a charge over time. Another possible issue is loading of the UPS; if too much power is being consumed from the batteries while there is a power outage the batteries may not operate at their rated output voltage for an adequate time for the PC to shut down properly. Most UPS’s have a test button to check its functions. Often this will put the PC into a shutdown procedure, which will tell you if the corresponding software on the PC is working properly. This button will also do a basic battery check letting you know the battery’s status. If the PC does not go into a shutdown procedure we will need to investigate whether there is a cable disconnected between the PC and the UPS, or if the UPS software package on the PC is not installed and/ or not configured properly. If the battery’s health is reduced, we will need to replace the UPS, or for the budget minded, the batteries can usually be purchased and replaced in the UPS on their own.

Phase IV: The Final Chapter For the past three newsletters, we’ve been talking to you about the importance of the RSTI X-Ray Certificate Series. So far you have learned about physics and maintenance of radiographic & fluoroscopic systems. No question this will get you off to a great start, but don’t quit now that you are almost done. Phase IV is where it all comes together. In this class, most of the time is spent in the labs. This is a very hands -on environment. It gives you time to practice and put to use all the theory discussed in the previous three phases and then take it to the next level. Phase IV is going to build on the knowledge base you have acquired to this point. You already know how to replace an x-ray tube. Now you will have the knowledge to evaluate the x-ray tube to determine if it is actually the correct tube for your equipment. You will be able to save money by determining if it is the x-ray tube the sales department makes the most money on or if it is the tube that is best for the way the system is used. Phase IV will cover all the parameters you need to look at to decide. How about the Image Intensifier? It is a very expensive item to replace. In this Phase as with the x-ray tube not only will you get to remove and replace the II on one of our systems, we will tell you how to choose the best II for your system. After installation you will perform all the necessary calibrations and evaluations to ensure it is performing at its best! And don’t forget about the plaque you will receive after completing the X-Ray Certificate Series! So do not cut your education short. Finish what you started. Get to RSTI and finish up the X-Ray Certificate Series.

The Residence Inn recently completed an entire renovation, including updating the lobby, common area and all rooms. In addition to the aesthetic facelift, several other improvements have been made or are planned for the near future. Flat screen TV’s will be installed in all the guest rooms this Fall. Take the cash!

Before you throw it away or call someone else, think of your training center. We are always looking for used equipment. RSTI will purchase your pre-owned equipment with CASH or Training Credits. In today’s economic climate this may be the way to continue to provide training for your department throughout this downturn. A few things to keep in mind when calling are: • Age of equipment • Condition • Time frame for removal At RSTI we like to think of it as keeping it in the family!

Everybody Wins The used equipment business has been picking up again. We have seen clients begin to buy again over the last few months. The overseas business in particular has been growing fast.

We are looking for new avenues to acquire used equipment.

We are looking for new avenues to acquire used equipment and you and your hospital can benefit from this. While we still enjoy buying from OEMs, and other brokers, we are looking to the facilities themselves as a great way to do this. RSTI is very fortunate, as we are able to not only buy these items and help facilities with their cash flow, but we are also able to trade training credits. The more of this we are able to do, the more our customers are able to see real benefits in other areas. The amount they are spending on training is decreased even though the amount of training our clients have been able to acquire has not. Obviously, this has been great for RSTI also, as we are able to enjoy new students that otherwise may not have been able to attend a class because of budget reasons.

Another way this is saving facilities money is they are paying less for service contracts because they now have employees on staff that can service more modalities. We see it as a win-win situation for everyone. Some items we are currently looking for; Mammography units, both analog and digital CT units Complete radiographic rooms Complete R/F rooms Ultrasound units C-arms Some of you might think the units you have sitting around unused are not worth anything. We challenge you to give us a try. We find some value in units that most people think will only be scrap. While we also want units that are only newer, the reality is that as long as the units are in good working order we can usually find some value in them. Please let us know what you have and we will let you know how we can work together to make it worth your while. Most of you have a few units that are not being used tucked away in a closet somewhere. Let’s make a deal on those items first so you can see how this works. Many of you could be pleasantly surprised at what that unit might be worth. The 2010 AAMI convention in Tampa, FL was great! It is so nice to see hospitals spending money for their employees to travel and get some training. I saw a lot of familiar faces, but I also met quite a few people for the very first time.

This year we celebrated 25 years in business and to show our appreciation to our students past and present, vendors and supporters of RSTI we threw an anniversary party. I would like to thank everyone who worked so hard to get the event together and I want to say thank you to everyone that attended. We had a great band and excellent food. I was very disappointed by the lack of dancing on the men’s part so gentlemen reading my article let me give you a tip! Women like men who can dance or at least attempt to dance, so get over your two left feet and shake your money makeryou have nothing to lose! Here are some photographs from the show and the party. SOCIETIES IN THE NEWS: Health Connect Partners What a great show! Terry Ryan and Darrel had the pleasure of attending our first “reverse tradeshow” this spring in San Diego. The show was very interesting as the “decision makers from the healthcare facilities” were behind the table in the booths and vendors got to walk the show. This proved to be a great idea, from our point of view since we were able to speak with everyone at the show. One did not have to wait for new clients to come to us while we sit in a booth for three days and hope they stop. But rather a real opportunity to speak with new and interesting leads. As always there was some overlap but not nearly as much as at a normal trade show. There were 200 organizations represented. We also were able to enjoy some informative seminars to help us understand the workings and thought processes inside of a hospital. This undoubtedly will help us when we have the opportunity to sit with purchasers and decision makers in the future. All of this was a great learning experience. We now have a better idea of how these shows work and will be able to really get the most out them. Hope to have a report for you about the next one that is scheduled for San Antonio May 2-4, 2011.

Preview for FIME As many of you know the annual FIME show is coming up. The show is held every August in Miami Florida. This will be RSTI’s fifth consecutive year exhibiting at the show. While many may think this is just an excuse to go to sunny Miami, this has consistently been a great show for RSTI. Not only in the used equipment side of the business but also in training. As the economy is starting to get back to normal, we are hoping that everyone has seen the benefit our training has to offer. The show attracts many people from all over South America. While having attended for a few years we do see many clients that we only see at this show. This may be because of our respective geographic location but it is always great to see everyone. We also have the opportunity to meet new contacts every year. Every year the show has proven to be more successful than the last. While there have been some language barriers we have had to contend with, we truly enjoy meeting prospective clients from all over the southern hemisphere. Once you are able to communicate you begin to realize just how much what we do in used equipment truly helps. Fortunately we work in a business where we not only get to make some money but we really get to better the lives of people all over the world. I reconnected with clients that I met at this show four or five years ago, who have shared with me stories of how our relationship has changed their ability to help people in their countries. I feel very fortunate to have had these opportunities and look forward to them every year. Hope to see more of you in Miami this year!!
