TALK TOUGH: TODAY’S Imaging Asset Management Understanding the Rules of the Game INSIGHTS from RSTI PRESIDENT, DALE COVER One of the key metrics used to measure performance is up-time ratings. How uptime is calculated can have a significant effect on your percentages. For instance, is the calculation based on 24/7/365 operations or 8 to 5/5 days a week? Does PM time count as up or down time? Does it not count at all if it is done during off hours? If you have to wait for parts, does that count against you? Is OEM travel time included in down-time or does only the time the manufacturer spends actually working on the system count?

s we kick off another football season, my home team, the Cleveland Browns, are yet again an unknown commodity. Our record over the past few years would lead one to believe that if we win more than we lose we should be happy. For hospitals the forecast for the future is slightly less vague. They know they will get their budgets squeezed more and more as time passes. With the payer mix continuing to move toward 50/50, hospitals are seeing the same number of patients or more and having to do it with less income. The old payer mix was roughly 30/70 government to private. The problem for hospital budgeting as this ratio changes is the government pays less of a percentage of the cost of treatment than private insurance companies do. Winning is hard to define in any technology management environment. In the medical field there are so many ways to excel or to grade the performance of the service provider. 2 | focalspot.FALL.2014

Cost of service ratios (COSR) is one tool for measuring performance from a fiscal perspective. COSR is where the initial cost of a system is weighed against the annual cost to maintain that system. Straight budget performance is another way to gauge effectiveness. Budget and expensing formulas can get very complex and predicting exactly what systems are going to fail and how much it will cost to repair them is a bit like throwing a Hail Mary, the unexpected failure of one expensive digital detector or a $200k dollar X-Ray tube and you have blown your budget for the whole year. Some facilities regard X-Ray tubes and detectors as consumables which can make budgeting easier. As long as benchmarks can be established for mean time between failure and average cost per repair, calculations can be made to estimate annual maintenance costs. Customer service skills and recognizing the customers’ expectations are critical aspects of the service professionals’ job. Regardless whether the facility is up-time or budget focused, customer service and customer satisfaction will rule the perception of how well the maintenance program is working within


the department. While the C suite is concerned primarily with the budget, constant complaints from department managers will cause them to reevaluate the value of the present technology management program. Technology asset management can be accomplished through OEM service contracts, independent service organizations (ISO), in-house service personnel or insurance. Well managed facilities will use a blend of all of these methods to optimize up-time, minimize costs and best utilize the talents of the in-house service engineers. Each of these service options has its own benefits and drawbacks.

Winning is hard to define in any technology management environment.

OEM service has access to parts and extensive technical support but unless they keep an engineer on site, response time can lag. ISO’s tend to have highly skilled engineers and are less expensive than OEM service but do not have the same support services as the OEM, which may extend repair times. In-house service engineers offer near immediate response times and a more personal relationship with the end user. Most in-house engineers with training can handle first level repairs quite handily and at a fraction of the cost of all of the other service providers. Add some specific training and their skill level can approach that of the OEM engineer. The difficulties for the in-house engineer will be parts and technical support availability.

Imaging is the highest cost department in the hospital to own, operate and maintain – so there should be no question why it gets so much focus. Whether your facility has an active service program for imaging equipment or not, attending a course like our Capital Asset Management will help you to understand the issues associated with starting, maintaining or improving an effective asset management program. Everyone involved, from managers to service engineers, would benefit from a better understanding of imaging asset management.

fter a couple of course rebuilding years, RSTI is excited to announce that we are bringing back the Multivendor Ultrasound Training courses start as early as December, 2014! Our structure will remain similar to our previous ultrasound program only with updated course curriculum and minor registration changes. We are offering a three day introduction to Ultrasound class, “Principles of Servicing Diagnostic Ultrasound Systems”, following up the next week by vendor specific training. The OEMs that will be covered on our initial run, out of the gates, will be:

However, without previous experience in ultrasound imaging, the introductory course will be a prerequisite. Technical Chops Kicking the Thumb Drive By Mike Petelin RSTI Instructor

M

any of us in the medical imaging service industry are using USB storage devices, commonly known as “thumb drives” or “memory sticks.” These devices can be a lifesaver when they contain calibration backups of a system that has recently crashed. There is one thing we must keep in mind; these devices were not designed to be long-term storage devices. Thumb drives have a limited number of erase and write cycles. Most midrange devices, like the ones that can be purchased from big box retailers, can only support a few thousand erase and write cycles, and as the device ages these cycles can become slow.

This limitation can cause irrecoverable loss of data, which is the main argument to make backups to your PC on a regular basis. Many of these limitations plague solid-state drives (SSD) as well. Read cycles do not do nearly the harm that a write cycle does. So having a hybrid setup, a SSD and a traditional magnetic hard drive, can help alleviate issues. In this setup, programs and the operating system would reside on the SSD as they would benefit from the speed increase, while editable files (images, videos, documents, etc.) should reside on the hard drive as they will potentially be rewritten numerous times. Nothing can turn a system recovery into a long calibration process like losing a beloved backup or a failed drive. So, we need to be sure to backup our thumb drives and use them for their intended use, as a convenience.

Shoes to Die For “Dave’s Rave” By Dave Domanski RSTI Instructor

I

maging technology changes constantly and imaging education should definitely look to the future. A complete education should also provide a window to the past. With that in mind, as Fall kicks off and Summer heads for the showers, I’m pleased to announce the recent facelift of the RSTI X-Ray Museum! Now featuring ten vintage and antique imaging systems spanning from 1935 to 1958, several 100+ year old X-Ray tubes a variety of vintage ephemera and odd medical devices. Today I would like to focus on a crowd favorite. Our circa 1945 Adrian X-Ray shoe fitter. Pointing to a darker time in X-Ray history when ionizing radiation was used gleefully for fun and profit, the fluoroscopic shoe fitter (Pedoscope in the UK) was a sales aid whose invention is popularly attributed to Dr. Jacob Lowe. Dr. Lowe applied for a US patent in 1919 and received it in 1927. Though the Adrian brothers of Milwaukee, Wisconsin had been building X-Ray shoe fitters contemporarily and also claimed to be the originator of the device, the Adrian Company of Milwaukee eventually purchased Lowe’s patent for $15,000. That may not seem like much, but that is the equivalent of around $200,000 today! The basic shoe fitter contained a stationary anode, hot cathode X-Ray tube in its base below an opening for the customer’s feet and a fluoroscopic screen above. The device provided viewing ports for the salesman and a pair for customers and may offer a timer and current adjustments. The bogus sales pitch was that X-Ray imaging could provide the best possible fit. This system does not, however, take soft tissue into account at all and actually succeeded only in allowing customers to see their metatarsals and wiggling phalanges. Oh yeah, and dosing the crap out of them and everyone else within six feet of the machine. Right about now you might be thinking “Wait, we’re selling shoes here, right? How much dose could it possibly be?” Quite a lot, actually. Radiation surveys showed that American machines delivered an average

of 13 Roentgens (R) or roughly 0.13 Sieverts (Sv) of equivalent dose to the customer’s feet during a typical 20 second viewing and some machines topping out @116 R (@1 Sv) for a similarly timed exposure! A typical chest X-Ray delivers approximately 2 mR or 0.02 mSv of dose by comparison. The novelty of the device and sheer ignorance of its inherent dangers led to massive popularity for several decades. At the height of use there were as many as 10,000 shoe fitters in the US, 1000 in Canada, 3000 in the UK and 1500 in Switzerland. Though the UK machines operated at lower dose rates the effects could still be quite harmful and may have led to the development of cancers years after the fact. I personally know a gentleman who developed a deformed foot as a result of repeated exposure to US machines as a child in the 1950’s. Eventually we wised up and shoe fitters were nearly universally banned by the early 1970’s, though the last one in use at a department store in Madison, West Virginia wasn’t unplugged until 1981. Cancellation Policy - Tuition is only refundable if cancellation is received in writing by RSTI at least 14 calendar days prior to the course or seminar date. Allow 30 days for receipt of the refund. Registrants who cancel within 14 days of the course or seminar date will have the tuition transferred to another course or seminar on a space available basis. In the event that a course does not attain our minimum enrollment, RSTI may postpone it. Paid tuition will remain on account to be applied to the rescheduled class.

Payment Due Policy - Payment for all courses and/or seminars is due three full weeks in advance of the course start date. Purchase orders are accepted, but also must be paid one week prior to the course start date. We also accept VISA, MasterCard, American Express, and Discover as a method of payment. All past due invoices will be assessed a late fee of 1.5% per month on any unpaid balances.

Schedule - Typically, courses run from 8:30 am-4:30 pm Monday through Friday. You will receive a detailed schedule when we confirm your registration. You should schedule your incoming flight to arrive Sunday afternoon or evening. On the last Friday, class ends at 12:00 noon. You may schedule your return flight any time in the mid- to late-afternoon.

Full Coverage Package - You are responsible for expenses you incur while attending courses. However, RSTI has negotiated a reduced package rate that includes 12 nights hotel accommodations, transportation between the hotel and training center, breakfast, and lunch. We will notify you of this rate when you enroll; if you wish, we can add the full coverage package cost to your tuition.
