Jason is currently the Global Quality Director for Fagron; an international company focused on personalized medicine. He has spent the last several years supporting the pharmaceutical, medical device, and compounding industries providing technical and regulatory guidance. Jason has nearly 20 years of experience serving in many roles with a particular focus on laboratory operations, quality assurance, and regulatory support. He is a microbiologist by education and expertise, and before joining the QA/QC world, he spent time researching and developing anaerobic microbial catalysts for fuel ethanol production.

One of those requirements centers around bacterial endotoxin testing (BET). The BET detects unsafe levels of microbial cell wall debris from live or dead Gram-negative bacteria. This bacteria has the potential to cause fever, and in the worse cases septic shock. Utilizing the Limulus ameboyte Lysate (LAL) assay, the BET is a mandatory quality control (QC) release test for injectable pharmaceutical products. Microbial identification is another mandatory component and requires a hard-to-find level of expertise, specialized and expensive equipment, and robust libraries that cover the specimen spectrum. Confident microbial identification is critical for an environmental monitoring program to ultimately confirm the security of a compounding area. Fagron Sterile Services (FSS), a leading 503B Outsourcing Facility company, has been able to meet all its needs for both rapid BET and accurate microbial identification through its partnership with Charles River. It’s parent company, Fagron, operates in 35 countries, and they leverage Charles River’s expertise and equipment to ensure their growing customer base is in safe hands. Jason McGuire, Global Quality Director at Fagron Sterile Services, has partnered with Charles River since 2003 when he first entered into the contract testing laboratory space.

THE CHALLENGE Fagron Sterile Services US (FSS), a division of Fagron, is centrally located in Wichita, KS, and operates two FDA and DEA registered and inspected 503B Outsourcing Facilities. FSS has been using Charles River’s Endosafe® rapid endotoxin testing instruments and Endosafe® EndoScan-V™, their endotoxin measuring and analysis software for its in-house program for years to achieve more control and better data integrity compliance. As a 503B outsourcer, quality is critical. FSS complies with all FDA and DEA regulations, cGMP and GDP. For FSS, utilizing a Biotek plate reader in combination with the Endosafe® Kinetic Turbidimetric LAL (KTA) and Endosafe® EndoScan-V™ software has proven to

Oregon Health & Science University (OHSU) teamed up with GSL Solutions as a beta partner, utilizing its family of RFID products to physically track medications while achieving efficiency in operations, accountability, reduction of labor hours, increased compliance, and higher user and patient satisfaction. GSL Solutions IntelliCabs for filled prescriptions (Will Call) and IntelliVault for controlled substances (C2’s) stock storage are integrated in a single software for a fool-proof system. The IntelliCab Will Call storage cabinets replaced the manual “hanging bag” system, and the IntelliVault Controlled Drug storage cabinet replaced the manual biometric “steel safe” and manual perpetual log book.

“Before, the process was pharmacist driven because they were the only ones with access,” says Matthew Spinar, PharmD, pharmacy supervisor of the OHSU Physicians Pavilion Outpatient Pharmacy. If technicians saw an order for a narcotic, they put it to the side because they had to wait for an available pharmacist to actually retrieve the product from their steel safe that was enabled with fingerprint technology. This workflow process caused bottlenecks and periods of long wait times for patients. OHSU also wanted to improve its inventory management, replacing the error-prone manual perpetual log with an automated and electronic log – and also providing an electronic record of all transactions and user activities of anything that is touched, discretely and in real time.

THE DECISION Due to the success of the IntelliCab Will Call cabinet program at OHSU, the decision was made to install the IntelliVault Controlled Drug storage cabinet.

Noise management is important for the healthcare environment. Sound pollution is a common complaint among both patients and healthcare workers. There are various research studies that highlight the negative impact of noise on hospital patients. It is equally important to reduce the noise levels in pharmacies, medication rooms, laboratories, and patient rooms. GX professional medical-grade refrigerators are designed for noisesensitive areas. They utilize OptiCool™ refrigeration technology, which incorporates both Variable Capacity Compressor (VCC) technology and natural hydrocarbon (HC) refrigerants resulting in much quieter operation. When evaluating a conventional undercounter medical-grade refrigerator against the Helmer Scientific GX professional-grade refrigerator, the GX professional medical-grade refrigerator was 17 decibels lower than the conventional model. It is important to note that every increase or decrease on the decibel scale is equivalent to a 10-fold increase or decrease in sound intensity.

While the conventional medical-grade refrigerator operated at 59dB, the Helmer GX refrigerators operated at only 42dB. This is a similar sound level to a quiet office. CASE STUDY Fagron Sterile Services Relies on Charles River for Critical QC Testing

Jason is currently the Global Quality Director for Fagron; an international company focused on personalized medicine. He has spent the last several years supporting the pharmaceutical, medical device, and compounding industries providing technical and regulatory guidance. Jason has nearly 20 years of experience serving in many roles with a particular focus on laboratory operations, quality assurance, and regulatory support. He is a microbiologist by education and expertise, and before joining the QA/QC world, he spent time researching and developing anaerobic microbial catalysts for fuel ethanol production.

PRIMARY INTENDED OUTCOME The pharmaceutical industry is subject to numerous requirements put in place to ensure the safety of patients. One of those requirements centers around bacterial endotoxin testing (BET). The BET detects unsafe levels of microbial cell wall debris from live or dead Gram-negative bacteria. This bacteria has the potential to cause fever, and in the worse cases septic shock. Utilizing the Limulus ameboyte Lysate (LAL) assay, the BET is a mandatory quality control (QC) release test for injectable pharmaceutical products. Microbial identification is another mandatory component and requires a hard-to-find level of expertise, specialized and expensive equipment, and robust libraries that cover the specimen spectrum. Confident microbial identification is critical for an environmental monitoring program to ultimately confirm the security of a compounding area. Fagron Sterile Services (FSS), a leading 503B Outsourcing Facility company, has been able to meet all its needs for both rapid BET and accurate microbial identification through its partnership with Charles River. THE CHALLENGE Fagron Sterile Services US (FSS), a division of Fagron, is centrally located in Wichita, KS, and operates two FDA and DEA registered and inspected 503B Outsourcing Facilities. FSS has been using Charles River’s Endosafe® rapid endotoxin testing instruments and Endosafe® EndoScan-V™, their endotoxin measuring and analysis software for its in-house program for years to achieve more control and better data integrity compliance. As a 503B outsourcer, quality is critical. FSS complies with all FDA and DEA regulations, cGMP and GDP. For FSS, utilizing a Biotek plate reader in combination with the Endosafe® Kinetic Turbidimetric LAL (KTA) and Endosafe® EndoScan-V™ software has proven to

be more cost-effective than outsourcing to a contract lab. Charles River provides FDA-licensed LAL reagents, instruments, and tracking and trending software to make endotoxin testing as simple and reliable as possible. Other companies in the Fagron family have also had great success with the Endosafe® nexgen-PTS™ cartridge technology instrument. It is user-friendly, reliable, and quickly delivers consistent results. As LAL is the most sensitive and specific method for the detection of endotoxins, Charles River has developed and optimized a range of quantitative and qualitative LAL formulations for increased sensitivity and superior interference resistance. Using a combination of these products, FSS has built up a strong and efficient endotoxin testing operation as well as a successful relationship with Charles River. With a new need to explore the services of outsourcing for microbial identification, McGuire knew exactly where to turn. Testing many varieties and materials in a timely and efficient manner while taking in the consideration the volume and dedicated time of personnel are challenges many 503B facilities experience. To that end, “it pays to outsource” microbial identification services, McGuire says. Fagron Sterile Services chose Charles River’s Accugenix® because “the costs of conducting these tasks in-house, particularly maintaining everything needed for a comprehensive identification program are prohibitive.” The equipment alone can cost hundreds of thousands of dollars.

THE DECISION Charles River acquired Accugenix® in 2012. This acquisition included accurate microbial identifications and strain typing solutions to fill out its quality control program and expand beyond endotoxin testing. FSS already had “great faith” in the services of Charles River through its experience with Endosafe®. So, “knowing that they acquired this organization gave us extra confidence that Accugenix® was the place to go for environmental identifications and general identifications as well.” Sample management is challenging and intense, he adds. “When you’re taking hundreds of samples a day or even a week, it’s very challenging to maintain control over those plates and determine whether you need a subculture or not. It’s better for us to send it out. Having a reliable partner to handle all of this was key for us. And, instead of sifting through pages of reports, he and his team can easily crunch the data and use them to improve operations.

Q. Healthcare professionals are saving lives by developing successful opioid stewardship programs. Do you know of some examples, and what creates the success? We’ve navigated from “those are addictive”, through “no pain >2”, and “opioid guidelines released every day that ends in Y.” Extremes never work, and running in a circle gets one nowhere, whereas jogging briskly in a straight line produces progress. As the folks at PAINWeek say, “Education is the best analgesic.” To the same tune, everyone should review various publications, attend conferences (PAINWeek, APhA, etc.), and complete online continuing education. For instance, check out the FreeCE 20 hour certificate program that I developed titled the ‘Pain Management Specialty Pharmacist (PMSP)’. Opioid stewardship program prominent examples include Kaiser Permanente and our Veterans Affairs (VA) Medical Centers. Kaiser Permanente connected pain management experts to their 500 pharmacies and 12 million members to facilitate best practices. The VA and Department of Defense collaborated to disseminate pain management guidelines via their “Academic Detailers” to clinicians in 1,000 hospitals and clinics visited by 9 million of our amazing veterans (mirroring efforts of pharmacy industry “Drug Reps”). In fact, the only pain scale that I utilize in practice today is known as the “Defense & Veterans Pain Rating Scale (DVPRS).” These two opioid stewardship programs, and countless others, aim for strategic utilization instead of simple opioid limitations.

Q. Given that our current opioid crisis primarily involves deaths from illicit opioids, should healthcare professionals continue to limit opioid prescriptions? One study shows chronic utilization of opioid medications increases drastically for initial prescription durations >3 days. Other studies explain that individual opioid medication prescriptions including a daily morphine milligram equivalent, or MME, of greater than 100 MMEs/day, increases the risk of overdose by more than ten-fold (although MMEs are greatly debated). Conversely, there are patients tolerant to higher risk opioid doses within a diverse treatment plan and risk reduction strategy. My dad should never again receive 5 dozen opioid pills for the anticipated pain of a simple outpatient procedure ever again, but he may reasonably need a few days’ worths. 46

Q. It sounds like there is great debate on MMEs, what’s your take? There are at least a half-dozen issues with MMEs, but what’s better? Like them, love them, or dislike them, MMEs are universally utilized today. For any healthcare professional to absolutely avoid the concept of MMEs, is a shortfall better left for conversational urination contests than patient care. Ironically, street chemists were quick on the uptake of MMEs as the observed street price of oxycodone jumped from $1/mg to $1.50/mg directly after the release of the 2016 CDC Opioid Guidelines. Illustrating the easiest way for a healthcare professional to remember that the updated MME factor for oxycodone is now “1.5”, whereas morphine and hydrocodone are both “1.”

Q. Earlier you mentioned the absolute need for risk reduction strategies in pain management, what do you recommend for a patient utilizing an opioid and benzo for the past 20 years? If a healthcare professional has a pulse and a license, that person knows combining opioids and benzos (benzodiazepines) comes with inherent sedation and respiratory depression risks. Multiple studies have shown a 2x, 3x, or even 5x of respective relative MME levels (or risks) when benzos are combined with opioid medications. Action is needed, regardless of how long the combination has been utilized. At the very least, risk reduction strategies must be utilized such as educating on naloxone, medication storage, psychological screenings, and opioid risk screenings, amongst others. In the vast majority of cases, gradual tapering of one of the sedative medications will need to begin imminently, keeping in mind that rapid tapers of opioid medications can cause very negative effects, however, rapid tapers of benzos can actually result in death from seizures.

Q. Speaking of naloxone education, what are your thoughts on this ethical dilemma. How many opioid overdose reversals with naloxone are enough for one person? Regardless of your profession, or even faith, this is one of the single greatest somehow debatable scenarios ever. The moral debate between “enough is enough” and “do no harm” resonates everywhere. Additionally, the vast minority of people still struggle with considering substance use disorder (addiction), as a medical condition, beyond people making repeated bad choices. If a person has a 15th opioid


around for best prices, and sometimes people utilize higher-risk doses of controlled substances within a diverse pain management treatment plan that also includes a verbose risk reduction strategy. Some in our society are aiming to divert medications, yet not everyone. Regardless, people respect what you inspect, not what you expect.

Q. After those profound thoughts on naloxone, anything to say about Medication Assisted Treatment (MAT) the panacea of addiction treatment?

Q. Were prescription opioid medications directly prescribed and dispensed by healthcare professionals the root of the opioid crisis facing our nation today?

I’m not convinced that nutritionists are well settled with diabetes management guidelines recommending metformin first line, jumping past diet and exercise, in our global society mantra of “a pill for every ill.” MAT options (methadone, naltrexone, and buprenorphine), by title alone, but also by FDA approval convey a concentration on the psychological and group therapy so pivotal for success, which is assisted with medication. Medication Assisted Treatment (MAT), even if used as sole treatment (technically off-label), is improving and saving lives every single day across our globe. However, as with any medication, there are concerns with these medications such as have a relative possible level of misuse, abuse, and diversion (as with any controlled substance) and a lack of need for dosing more than once daily (given the long half-life). In order to best help everyone, we simply must proceed with the utmost respect for any medication’s respective pharmacology.

There’s enough responsibility to go around for everyone including particular (but not all) poppy growers, manufacturers, distributors and wholesalers, prescribers, dispensers, insurance companies, government, street dealers, cartels, and even patients. Headlines everywhere contain unfounded “jabs” to our professional/patient relationships (except the vast minority involving criminal activity). Who hasn’t seen the headline “75% of Heroin Utilizers Began with Prescription Opioids” dozens of times? However, details matter, as yes, prescription opioids, but from what source? Another study shows that two-thirds actually attained said prescription opioids from family, friends, or street dealers for free or cash. Yet, we never read those headlines. Regardless, we still have a responsibility to utilize diverse treatment plans and risk reduction strategies. Not to mention the huge need for healthcare professionals to have more discussions on illicit substances with our patients, given that numerous nonopioid substances of abuse are raging our globe.

Q. Drug diversion happens every day across the globe and many times involves the healthcare system. What are the biggest signs of drug diversion that healthcare professionals should be looking for at all times? We as healthcare professionals are on the front lines of the infamous “war on drugs” and have an innate or acquired sixth sense for realizing potential diversion concerns. We would be best served by discussing what to do with the car once the dog catches it, but for the sake of conversation, let’s mention a few red flag concerns. Concerns include cash payments, patients traveling far distances, repetitive prescribing/ dispensing medication patterns, doctor/pharmacy shopping, and early refills, amongst many others. When suspicions arise or are even confirmed, proceeding with professionalism is of utmost importance. People travel, people do not always have insurance, people shop

Q. Care to highlight some of the most popular substances of abuse across the globe? Everything and anything. I like to paraphrase Paracelsus, “It’s all about the dose!”. Did you know that even nutmeg, that innocent spice in everyone’s pantry, is actually a hallucinogen? Well, if one were to “crank the dose,” they’d see, literally (hallucinogens cause visual disturbances)! A few groups of substances of abuse include hallucinogens, stimulants, marijuana and cannabinoids, sedatives, alcohol, inhalants (or sometimes even inhaled alcohol, it’s a thing, a deadly thing!). CASE STUDY IntelliVault & IntelliCab Transform Prescription Management at OHSU

PRIMARY INTENDED OUTCOME Safely, accurately, and efficiently dispensing medications to patients is the primary function of pharmacies. Oregon Health & Science University (OHSU) teamed up with GSL Solutions as a beta partner, utilizing its family of RFID products to physically track medications while achieving efficiency in operations, accountability, reduction of labor hours, increased compliance, and higher user and patient satisfaction. GSL Solutions IntelliCabs for filled prescriptions (Will Call) and IntelliVault for controlled substances (C2’s) stock storage are integrated in a single software for a fool-proof system. The IntelliCab Will Call storage cabinets replaced the manual “hanging bag” system, and the IntelliVault Controlled Drug storage cabinet replaced the manual biometric “steel safe” and manual perpetual log book.

THE CHALLENGE With its manual “hanging bag” will call system, OHSU was prone to long patient wait time, lost or missing prescriptions, and labor-intensive return to stock activities. Controlled drug storage and management was heavily controlled by pharmacist time and process and procedures (P&P) to maintain inventory control and reporting requirements. OHSU pharmacy rules required pharmacy technicians to have a pharmacist be the only contact with controlled substances. “Before, the process was pharmacist driven because they were the only ones with access,” says Matthew Spinar, PharmD, pharmacy supervisor of the OHSU Physicians Pavilion Outpatient Pharmacy. If technicians saw an order for a narcotic, they put it to the side because they had to wait for an available pharmacist to actually retrieve the product from their steel safe that was enabled with fingerprint technology. This workflow process caused bottlenecks and periods of long wait times for patients. OHSU also wanted to improve its inventory management, replacing the error-prone manual perpetual log with an automated and electronic log – and also providing an electronic record of all transactions and user activities of anything that is touched, discretely and in real time.

THE DECISION Due to the success of the IntelliCab Will Call cabinet program at OHSU, the decision was made to install the IntelliVault Controlled Drug storage cabinet.

Similar to the IntelliCab’s RFID-based random storage and “pick to light” technology, the IntelliVault system provides discrete stock storage and prescription filling of controlled medications (C2-C5), therefore helping OHSU meet several objectives. First, because users must use their badge to open the IntelliVault and can only access the specific medication that is needed, loss is virtually impossible. Simply using the system creates an automatic log without repetitive scanning. An automated C2 log, “AutoLog”, with 100 percent user accountability and medication product tracking vastly increased efficiency and any potential for diversion. This real-time tracking capability provided the level of security that empowered OHSU management to let technicians fill C2 prescriptions, freeing pharmacists to work to their licensure and focus on skilled clinical work. Michael Deam, RPh, Assistant Director of Outpatient Pharmacy, likes that the IntelliVault system allows for tracking of every single drug movement. He also appreciates that they no longer need to spend as much time in the perpetual inventory log manually recording each prescription dispensed. He estimates savings of up to an hour a day just on the log. Installation of IntelliVault took some work but it went off without a hitch, says Spinar, thanks to thorough planning. Spinar oversaw extensive dry testing to make sure the server they set up was going to work. Closer to the transition, they integrated the system with OHSU firewalls and brought the physical piece of hardware on site. During an overnight installation, Spinar and his team, as well as a GSL Solutions crew put the new IntelliVault safe in place. “We prepared for a long time to make sure it would go as planned.” The staff had an adjustment period, he says, mostly due to the changed workflow. IntelliVault provides “full autonomy for technicians. It put pressure on the technicians at first because there’s really no excuse for them not to integrate any prescription [narcotic] into their normal workflow,” Spinar says. “We had to get them to build that into their mindset.” It only takes one time to see just how easy it is to use IntelliVault, he adds. Users simply scan the prescription label and the appropriate drawer and cell lights up. “It’s so simple. It tells you what to do next. Staff-wise, the learning curve is very, very small.”

THE RESULTS Spinar’s pharmacy is right on the hospital’s main campus and dispenses a lot of medications for postsurgical patients. While they don’t handle a large quantity of narcotics, Spinar says the volume of prescriptions is high. This means they might fill up to 100 prescriptions for 10 oxycodone pills a day, which represented a lot of pharmacist time. Shifting this workload onto technicians was meaningful, along with pharmacist time saved from eliminating the manual perpetual log and providing better user accountability. By utilizing technicians to fill C2’s, “we’ve definitely seen a lot less people waiting in the waiting room,” he says. Prescriptions are now ready for pick up by the time the patient comes down from the floor. “We didn’t imagine before the installation how much of a difference this would really make, but it’s huge.” Another benefit centers around reconciling inventory. Before partnering with GSL Solutions, Spinar says they spent more time auditing errors that were self-inflicted than actual discrepancies in that manual log. “All it takes is one prescription you forgot to log,” says Deam. “The counts are off and you’re spending [pharmacist] time trying to figure out the missing prescription so your counts balance at the end of the day. You just don’t have that with IntelliVault,” says Deam, illustrating the high intelligence of the system that enables everything to be accounted for and resolved in real time.

Return to stock processes have been simplified as well, Spinar says. Only the user conducting the return to stock function can pull medications from the IntelliCabs for will call. When a prescription is pulled from will call, the user has one-time access to the IntelliVault C2 safe. The return goes into the IntelliVault, and the door closes back up for a closed loop. The system then automatically increments the correct NDC inventory. IntelliVault’s receiving process adds another layer of safety check, Spinar says. The staff now scans and links the stock to RFID-enabled SmartBaskets, and the system automatically knows the drug name, strength, and more. They also can enter in the lot and expiration date. The linked basket is returned to the IntelliVault anywhere it fits, and this worklow alleviates time spent manually logging the right NDC, doing the math, and rotating stock. “We’ve eliminated sifting through our physical inventory to find any outdated or recalled lots,” Spinar says, since they now can easily track lot and expiration dates. We do a simple electronic database search when there is a recall and mark the product for destruction or return to vendor, he says. They can even track if someone historically received a recalled medication. IntelliVault provides immediate reports of what the pharmacy has on hand. “It’s nice to have a system that’s always tracking real time what’s available in your safe to be dispensed,” says Spinar. Prior to implementing IntelliVault, OHSU saw great success with the IntelliCab Will Call solution. The system provided for more efficient and accurate prescription filling and retrieval of prescriptions ready for patient pick-up, says Spinar. IntelliCabs eliminated misfiling, picking errors, and wasted space while reducing wait times and improving patient satisfaction. Spinar also appreciates the clean, professional appearance of the system. Staff no longer rummage through dozens of bags to find the right prescription. In the past, they used clear hanging bags, which were very error prone and inefficient. People in the lobby could potentially see identifying patient information through the clear bags. Now, with IntelliCabs, randomly filed prescriptions ensure patient privacy. If a staff forgets to link a prescription to a RFID SmartBasket, the system automatically alerts to help correct the issue. The system is self healing and self correcting.

THE CONCLUSION It’s hard to put a specific number on the time and cost savings OHSU has experienced with GSL Solutions, Spinar says. Preventing miss-sells and bundling errors with IntelliCabs “is worth its weight in gold for patient safety.” Improvements in turnaround at the pharmacy counter are hard to measure “but I know it’s a lot faster.” Implementing IntelliVault allows OHSU to use its staff members more efficiently. “Pharmacists don’t have to mess around with the C2 safe all day,” he says. “It’s a great product,” says Deam. “We’ve definitely streamlined our workflow and become more efficient [on how we use our staff ]. I would certainly recommend.” OHSU has plans to implement IntelliCab at its second busiest location on campus, which fills 400-500 prescriptions a day. Spinar and Deam both say the systems have more than met their needs and goals. “Staff members at the pharmacy tell me they don’t know how they’d operate without that technology,” says Deam.
