Using Square’s hardware, software, and card processing, Modern Payments Processing handles all types of payments — from in-person at the point of sale to online and mobile. This eliminates the need for multiple solutions and helps streamline operations for an easier, more efficient experience. Plus, Square handles all payment fraud, risk, and disputes. They are also Payment Card Industry (PCI) certified, which means payments always comply with current regulations.

TRANSPARENT PROCESSING COSTS
With this collaboration, Square offers Cardinal Health independent pharmacies a discounted and fixed processing rate. This includes card present and not present transactions for online or at-home delivery purchases.
“I was very concerned about the rising cost of card processing — fees on top of fees,” says Imad Karaze, owner and pharmacist in charge for Beaumont Pharmacy. He also saw line items on his card processing invoices that made no sense. “I was looking for more predictable costs.” The Modern Payments Processing solution gives him just that, he says. “Now I know what each transaction will cost — every time, all the time.”
Vijay, a pharmacist at Marble Hill Pharmacy and one of the very first to pilot the program, agrees. “The solution is very easy to use, and because Square provides flat rates, I don’t have to worry about additional processing fees as with other processors.”
INCREASED OPERATIONAL EFFICIENCY
Implementing the Modern Payments Processing solution enables pharmacies to take a full range of payments — text, online, point of sale, and mobile tablets. This eliminates the need to work with multiple processors, which may have inconsistent rates and reporting — making it difficult to get a handle on the actual bottom line.
Square’s open APIs also allow for direct integration with any pharmacy management system for an end-to-end payment solution. A range of built-in business tools and analytics also help streamline operational workflows for greater efficiency. For instance, an intuitive dashboard puts all payment data in one place — which can be accessed via computer or mobile device for anytime, anywhere views of transaction history. This allows pharmacies to track sales and payment transactions whether customers are paying online, at the drive-thru, at the counter, or at home delivery. “The integration has made things even easier on my stores,” says Ankit Desai, who has several pharmacies using the Modern Payments Processing solution powered by Square. “It has enhanced the parts of Square I already enjoyed, including straightforward processing and flat fees, and eliminated a lot of the manual work I was doing when the terminal was not able to sync payment information to my software system."
Plus, since Square is PCI certified — pharmacies no longer need to individually validate their PCI compliance. Vishal Patel of Bourbonnais Pharmacy says this, combined with the transparency of the fee structure, provides value to his store. “Not only do I have greater visibility into my operational costs, I can ensure my pharmacy remains compliant without any additional work on my part.”

BUYING YOUR OWN PHARMACY
What makes a pharmacist want to buy their own independent pharmacy? In one word, freedom. You want to practice pharmacy on your own terms. And you want to do more for the health of your patients. But where you are now might not give you enough time or opportunity to do that. You’re not alone. Many pharmacists feel the same way. When it comes time to buy a pharmacy, there are a few key things to keep in mind. The first step is to determine your goals and what you hope to accomplish in the purchase. Are you looking to start or expand? Are you looking for a particular location? Are you familiar with the pharmacy landscape and the current state of the industry? The following is a list of some of the most important considerations when looking to buy a pharmacy.
PUT TOGETHER A PRELIMINARY CHECKLIST
Buying an independent pharmacy should be a detailed, thoughtful process. Before buying a pharmacy, put together a checklist of things you’ll need before you start looking. The first thing on your checklist should be a systematic way to know which pharmacies are for sale. It’s not like buying a house. Pharmacy owners aren’t putting for-sale signs in their windows or on the street in front of their stores, it’s a secretive process. Owners don’t want their patients to know because they might take their business elsewhere. Owners don’t want employees to know because they might leave. Both reasons could lessen the potential worth of a pharmacy owner’s business.
If you’re a pharmacist interested in buying a pharmacy, you can:
• Work with a third-party vendor that maintains a propriety database of pharmacies.
• Check the pharmacy sales listings on national and state pharmacy association websites.
• Hire a broker.
• Network with your peers and let them know you’re in the market. Word-of-mouth that you are looking can be a good strategy.
Next on your checklist should be assembling a team of trusted advisors. Don’t go through this process alone. Your team should include some, if not all, of the following people:
• An industry subject-matter-expert (SME) who’s experienced in buying and selling independent pharmacies.
• An accountant experienced in pharmacy financials.
• A lawyer who can review and handle all your contracts and contract language.
• A banker who can work with you to identify the best financing options.
• A pharmacy owner who can give you support and advice from their own experience.
Next on your checklist should be financing. A lender likely won’t pre-approve you for a loan because the purchase price can vary significantly based on the size and value of the pharmacy that you end up buying. But a lender will want to see how much money you have to put toward a down payment (skin in the game). They’ll likely want to see a minimum of 10% of the total purchase price.
Your financing options and considerations may include one or a combination of the following:
• A conventional loan.
• A small business administration (SBA) loan.
• A loan from a pharmaceutical supplier or wholesaler.
• Owner hold back note.
• A personal loan from family or friends.
With those three things in place — a list of available pharmacies, a team of advisors, and a range of viable financing options — you’re ready to go shopping.
IDENTIFY THE PHARMACY THAT’S RIGHT FOR YOU
Choosing the right pharmacy to buy is a combination of art and science. The science is the business side of your decision. You need to dig deep into the pharmacy’s financial statements. Before you make an offer, you need to know:
• How profitable the business is.
• Whether revenue is trending up or down.
• The potential for growing the business.
• Whether the owner incurs expenses that you, as the new owner, wouldn’t incur.
• Opportunities for cost savings.
• The volume and mix of prescriptions.
• The payer mix of the pharmacy’s patients.
• Which long-term care contracts (if any) are in place, are expiring, or need to be renewed. Go through line by line so you know exactly what you’re buying. You should research at least three years’ worth of financial documents.
The art of buying a pharmacy is the personal side of your decision. This is about you and your patients. Are you going to move to the same town as the pharmacy? Can you commute to the pharmacy from where you live now? Is this where you want to raise your family? Is there community support for the pharmacy? Are there other businesses in town that help support the pharmacy? Is this where you want to spend most of your time?
The most important questions to ask — and the ones that likely sparked your desire to buy your own pharmacy business are: Is this the pharmacy that gives me the opportunity to make the changes that I want to make? Will it let me practice pharmacy at the top of my license, so I can help my patients lead healthier lives?
You’ll know it in your gut. And if the answers are yes, then you’ll know it’s the right pharmacy for you.
STEPS
TO EXECUTE THE PURCHASE OF YOUR PHARMACY
You’re a pharmacist, and you’ve found the independent pharmacy business that’s right for you. Here are the five steps you need to take to close the deal:

Valuation
How much is the pharmacy business worth? Several factors can affect the value of a pharmacy. They include prescription volume, market share, cash flow, debt, annual profit and loss, property value, inventory, and store aesthetics. It’s crucial for you and your team to identify all the things that can affect the pharmacy’s value now and in the future.
2 Negotiation
It’s common for you and a seller to be far apart on price. You should explain to the seller how you came up with your number. Then ask the seller how they got their number. You or the seller may have missed something that affects the valuation and price. Then you can talk about it and hopefully come up with the right number for both parties.

Contracting
Once you agree on a price, you sign a letter of intent to buy the pharmacy with a closing date. This can get tricky, as some health plans and pharmacy benefit managers (PBMs) require a pharmacy to re-enroll with them if it changes hands. Re-credentialing can hurt your cash flow if you buy the pharmacy, but the plans and PBMs won’t pay you until you’re re-credentialed. Your sales agreement may include a power of attorney (POA) allowing you to operate under the previous owner’s license until you complete those re-enrollments. Compare the benefits/risks of a stock purchase versus an asset purchase.

Next comes the financing. It’s important not to pigeonhole yourself before the deal. Consider all your options. Work with your team and pick the one that works the best for you personally, professionally, and financially.

4 Closing
The last step is the close and ownership transfer. You will need to transfer the previous owner’s reimbursement contracts, supplier agreements and licenses to yourself. This step can vary greatly depend in on deal structure (asset purchase versus stock purchase). You will also need to notify your state’s pharmacy board and the U.S. Drug Enforcement Administration (DEA). It also includes a lot of smaller things that many new pharmacy owners forget. Think: utility bills, lease agreements, and whether you’re a tenant or a landlord.
Buying your own pharmacy doesn’t have to be complicated. By following the path described, you can buy your pharmacy, run it your way, and work toward improving patient outcomes in your community.

Adherence packaging is a system that allows pharmacies to dispense medication in organized pouches or blisters that are customized to a patient’s specific medication regimen. The purpose of dispensing in adherence packaging vs. traditional bottles is to support patient medication adherence, making it easier for a patient to independently take the correct dose of their medication at the correct time of day.

Adherence packaging offers benefits for patients, providers, and pharmacies.
Pharmacists are supporting patients beyond the pharmacy counter and into their homes by securely multi-dose packaging medications that patients would otherwise be left to organize on their own. This support creates a simplified medication management system for patients that offers greater adherence, fewer errors, and better outcomes.
By reducing the time and resources needed to manage complex medication regimens, pharmacies with an adherence packaging focus create an environment where providers and the pharmacy team collaborate to reach patient health goals and quality metrics better. Workflow and Procedures: Are you willing to look at pharmacy differently? Adherence packaging requires workflows and procedures, such as medication synchronization, that (once implemented) improve the pharmacy workday but takes time to organize and execute. This workflow is very different from a traditional ‘bill-and-fill’ workflow and requires pharmacy teams to think ‘outside the box’ about how patients are managed.
Marketing: You know all the benefits of adherence packaging, but patients and providers require education on why dispensing medication in multi-dose packaging can be a better option. As my dad told me when we were preparing to install our first adherence robot — “That machine is not going to sell itself.” Do you have relationships with providers you can educate on an adherence program? Do you have long-time patients you trust to ‘trial’ the packaging and provide you with feedback? Be sure to have a marketing plan in place to start and grow your adherence program.
Q. What types of patients are good candidates for adherence packaging?
The most successful adherence packaging patients are those whose medication regimen is simplified for them to take their medications independently in the home setting they choose. As stated previously, incorporating adherence packaging technology into your pharmacy requires planning and preparation. Here are some tips to get started:
Choose the Right Technology: Research and select technology that meets the needs of your patient population.
Prepare Staff: The fact of the matter is your team may not be happy that you want to implement robots. Take time to listen to the concerns of your team, include them in the planning and preparation of the pharmacy and new procedures, and keep a positive work environment so they don’t feel as though their job is threatened.
Create a standard operating procedure. If you only formally create one document that outlines your adherence technology procedures, make it a standard operating procedure (SOP) to ensure consistency and efficiency in your workflow. Think about all the steps you take in your pharmacy today to fill a patient’s medication and what will be needed to switch from bottles to packaging. Some things you should be considering in your SOP’s are synchronization of medication, refills, billing, billing issues, and patient organization.
Market, market, market! Even before your technology is installed, start sharing that you will provide adherence packaging to patients you’ve identified as good candidates. Reach out to providers and ask if you can provide them with information on your program. Create a social media or print campaign that targets the population you want to reach.
Q. What training will my pharmacy staff need in adherence packaging?
Specific training requirements will depend on the type of technology you choose, but generally, the operations and safety of any adherence technology should be provided by the company you purchase your technology from. Your goal as a pharmacy owner is to make sure you have “buy in” from your team on your new technology and are providing your staff with what they need to educate patients on the technology, have discussions with providers who call and ask questions, and feel confident that adherence technology is the correct path for your pharmacy.
Q. How do I measure the success of my adherence packaging program?
Measuring the success of your adherence program is essential to evaluate its effectiveness and identify areas for improvement. Some of my go-to metrics to measure success include:
Quality Metrics: One of the primary goals of adherence packaging is to improve quality metrics. By closely following medication adherence metrics and CMS-measured metrics such as proportionof-days-covered, you can determine success and use improved metric numbers to market your adherence program.
Medication Errors: Adherence technology can help reduce medication errors by improving the accuracy of medication dispensing.
Patient Satisfaction: The overall end user of your packaging system is the patient. You can survey patients to gather feedback on their experience with your program, including ease of use and overall satisfaction.
BIOGRAPHY
Workflow Efficiency: Adherence technology can help improve workflow by streamlining the dispensing process. You can measure workflow efficiency by tracking dispensing times, the number of prescriptions and/or patients, and the time it takes to operate the technology.
Return On Investment: Adherence packaging can be a valuable revenue stream for your pharmacy. Not only can you bring in new patients with marketing and unique partnerships, but you’d be surprised with how many of your patients don’t fill their entire prescription regimen at your pharmacy! The packaging not only gives you an opportunity to transfer in scripts that you may not be filling but also for billable clinical work such as Comprehensive Medication Reviews.
It’s also important to remember that needing adjustments or getting a bad review doesn’t mean the program is unsuccessful. Not every patient will be best supported with adherence packaging, and not every provider will like the technology you chose. That’s OK! You will learn more from the few negative interactions than if all were positive.
Q. How do I market my adherence packaging program?
Marketing your technology and adherence program is essential to ensure your community is aware of the service and understands its benefits. Some of my favorite strategies include:
Create Your Pharmacy’s Pitch: Every team member in your pharmacy should know a 30-second blurb on the technology and its benefits. This message could be the same or different when speaking to patients vs. caregivers vs. providers but it should be an educational moment for the end-user when certain criteria are identified (such as a patient on six or more medications). Don’t forget that the team also needs to be able to follow up in more detail if required!
Display Signage: Posters, flyers, and floor stickers in high-traffic areas of your pharmacy, such as the waiting area and checkout counter, are a must. I also recommend stapling brochures to every prescription bag that leaves your pharmacy.
Collaborate With Healthcare Providers: Educating healthcare providers on your adherence technology can help providers identify potential patients that could benefit from your program. Identify specialty practices, home care agencies, and social worker groups in your community who you can reach out to.

AFA PHARMACY’S JOURNEY TO AUTOMATED DISPENSING AND FULFILLMENT
INTRODUCTION AND BACKGROUND
Jenny DeRose, Pharm.D Pharmacist in Charge ~ AFA PharmacyThirty Madison exists to deliver on healthcare’s biggest opportunities: access and affordability; patient outcomes; and patient experience. As such, the company’s specialized brands focused on personalized care for patients with ongoing conditions. When Thirty Madison launched its first online order fulfillment center, AFA Pharmacy in Missouri City, Texas — their long-term vision was to leverage automation technology to meet projected volumes and create a better pharmacy experience for their patients. That’s when they began looking for providers who could help them start working toward implementing automation, even though it would be some time before they needed it.
According to Omar Khokhar, Pharm.D, pharmacy manager for AFA, Thirty Madison chose Tension, a leader in packaging equipment and automation systems for the e-commerce and pharmacy industries, for several reasons. “Tension already knew their APIs could integrate with our upstream platforms because they’d worked with our pharmacy management software before,” he says. Tension was also known in the industry for having a cost-effective prescription fulfillment system that could process high volumes of unit of use drugs efficiently.
Omar Khokhar, Pharm.D Pharmacy Manager ~ AFA PharmacyTo build out Thirty Madison’s innovative mail-order pharmacy, Tension recommended its LDU-Flex Pharmacy System utilizing Tension’s LDU and backend WorkFlow Software (WFS) system. The LDU (Linear Dispensing Unit) is a unit of use picking, labeling, verifying, and dispensing machine that can process over 1,000 items per hour. The WFS software drives the LDU Flex Dispensing System. The AFA Pharmacy front-end software receives and processes orders, checks inventory and then sends orders to the Tension WFS. From there, the WFS orchestrates everything, managing all order routing to the LDU and to the dispensing, verification, packaging, and manifesting workstations in the workflow.
CASE STUDY
THE WORKFLOW SOFTWARE SYSTEM
Today, AFA operates out of a 40,000-square-foot facility that includes a warehouse and fulfillment center. Jenny DeRose, Pharm.D, AFA’s pharmacist in charge, says the pharmacy — which specializes in hair loss treatments and prescriptions covering conditions such as birth control and hair loss as well as migraine treatments and migraine prescriptions — currently processes 4,000 to 5,000 orders daily. “It wasn’t always like this,” she explains.
“At first, we weren’t processing anywhere near that number of orders, we knew we weren’t ready for automation right away.” At the same time, however, the team at AFA was eager to get started with Tension’s WFS — even before they installed the LDU, which would ultimately automate processes.
“It wasn’t really their standard operating procedure,” DeRose says, “But Tension agreed to let us use the software to fulfill orders manually until we installed the LDU.” To do that, the company developed an entire manual pharmacy using the Tension WFS that mirrored what the automated workflow would eventually look like. They started by analyzing the specs for AFA’s fulfillment center, making recommendations on how best to configure the workstations and processes. “They even coordinated with our contractors to make sure all the specifications we needed to accommodate the workflow were clear,” says Khokhar. DeRose adds that the company also helped make sure the system was futureproof. “In addition to automating, we knew we wanted to install Parata machines and potentially add cold chain to our processes at some point,” she explains. “So, Tension was sure to carve out space for us to implement these when we are ready.”
LDU FLEX WITH WFS AUTOMATED DISPENSING
Gaining experience with the WFS user interface before installing the LDU was invaluable for AFA, says Khokhar. “It gave us so much insight into best practices in terms of packaging, verification, and shipping that implementing the automated process was seamless.” So once order volume began to increase steadily — they felt ready to install the LDU Flex System.
Not much changed from a software or workflow standpoint, explains Khokhar. “The only different bit of software introduced was related to the LDU — and even then, Tension held our hand through the whole process.” In fact, Tension’s lead developer stayed on-site for about four weeks once the LDU was operational to ensure everything went as smoothly as possible. “He told us from the beginning, it’s a new system that is learning you as much as you are learning it — and there will be hiccups.”
This level of support is something DeRose didn’t quite expect. “Everyone at Tension is incredibly responsive,” she says. For example, at one point, the pharmacy was experiencing some small errors with its conveyor transition arms. When the provider for the conveyor couldn’t figure out the errors, one of Tension’s coders stepped in to troubleshoot. “Even though I know they have other clients — I always feel they are completely dedicated to us.”

Tension reps now visit AFA quarterly for regular maintenance on the LDU and conveyor system. Khokhar adds, they are always available for one-off visits to resolve a specific issue.
HOW THE AUTOMATED PROCESS WORKS
Thirty Madison’s upstream platform is connected to the WFS at AFA so it can push orders to the pharmacy for fulfillment. With this, once orders have been processed by Thirty Madison they go directly to the WFS to be routed to the LDU and other workstations. Depending on what’s in the order, the LDU will dispense the medications. AFA has operators behind the LDU who stock the channels to be sure there is always enough product and at the front side of the LDU to handle any dispensing issues.
Once products are dispensed out of the LDU into totes, they go to a manual dispensing workstation. This allows for slower moving medications to be added to the order if they weren’t stocked in the LDU. It’s also where anything that needs to be hand counted is dispensed. After the dispensing and labeling stations, the order moves on to pharmacist verification — where a pharmacist physically checks the order to ensure the right products have been dispensed. The WFS also allows for virtual verification. From here, the order goes on to packaging, where operators choose the most appropriate packaging, whether bubble mailers or boxes. They then pack the order along with all required documentation such as patient leaflets and medication guidelines — and send it off to the shipping workstation.
“Tension’s software is just so good, it’s almost foolproof,” says DeRose. “When you scan something — the software tells you exactly what to do next.” Khokhar agrees, saying the software is so intuitive it’s almost impossible to make a misstep.
ENHANCED VISIBILITY INTO INVENTORY
Tension’s WFS includes a customizable dashboard that companies can use to for ata-glance visibility into their operations. For AFA, the dashboard focuses on tracking inventory and monitoring what orders have been fulfilled. Initially, the data only included the name of the patient and the products received. As orders increased, Tension added a tracking number to the dashboard and eventually added the patient’s address and the lot expiration date. AFA uses this information to see what they’ve dispensed throughout the day — to manage inventory and ensure they hit output targets. “Tension has been very accommodating in customizing our interface for us,” says DeRose. “They’re always happy to add things as we define our process a little bit more,” Khokhar adds.
When Thirty Madison launched AFA Pharmacy, they had a vision for where they wanted to go — and that’s where they started with Tension. “When we began working with the software and saw the business growing in a way we didn’t necessarily anticipate, we realized we had to pivot,” Khokhar explains. No matter what it was, even if it was outside the original scope of the project, Tension always said, ‘Anything is possible.’ “Any ask we have — Tension always makes it happen,” he says. “That’s how we know this system will always be able to evolve with our business.”

Aging at Home: The Future of Long-Term Care
Initially fueled by the pandemic, the world has seen a large increase in the number of patients “aging in place” in the comfort of their own homes.(1) A survey conducted by AARP demonstrated that most Americans are able to stay healthy and connected to their communities, avoiding having to move into a long-term care (LTC) environment.(2) The switch from LTC facilities to at-home services has opened the door to a multitude of challenges for pharmacies in defining LTC.
Underappreciation of LTC Pharmacists: From a Care Team and Payer Perspective
The pandemic and subsequent increase in patients aging at home has highlighted the role of a pharmacist as a key player on the LTC team, yet they are still undervalued by many. Pharmacists in LTC settings have a myriad of responsibilities ranging from monitoring of conditions to medication management. Understanding the role of a pharmacist will help increase awareness of their worth and, in turn, may increase their value deemed from payers. Many pharmacies providing LTC services are still not classified or reimbursed for their level of service, as payment models are based on patient location, not service provided. Many pharmacies offer delivery of prescriptions by mail or courier and are labeled as a mail-order pharmacy
despite the fact that they often facilitate delivery services face-toface and provide additional services. In 2018, medicare advised payers to review the magnitude of the pharmacy’s services before classifying them as a retail or mail-order pharmacy.(4) Over the years, there has been a push to define LTC in a way that acknowledges current pharmacists’ level of involvement, especially now in an at-home setting.
Defining LTC — Long-Term Care Definition Act of 2021
The Long-Term Care Definition Act of 2021 sought to clarify the regulatory framework of LTC pharmacies.(3) This act defined LTC pharmacy as “a pharmacy licensed under applicable state law that is able to provide enhanced pharmacy and clinical services and reside in a facility.”(4) Enhanced pharmacy and clinical services that were defined in this act included medications requiring specialized packaging, medication reconciliation services and other necessary clinical management, drug utilization review, emergency supplies of medication, and 24/ seven availability of medication delivery and pharmacist clinical services. LTC reimbursement needs to be defined by the service they provide, not their location of facility. In paragraph four of the LTC Definition Act, facility is defined as those previously
defined in section 1819(a), 1919(a), and 1905(d), as well as “any other setting in which individuals who require enhanced medication services reside as determined by the Secretary.”(4) There is still a need to highlight and incorporate at-home services in this definition.
Technological Advances Needed to Support Patients at Home
As many are opting to age at home, technological advancements are needed to support the change in trends. Pharmacies, including LTC and retail-LTC combination pharmacies, have the opportunity to break into a new market by providing LTC services to those at home. In order to serve these patients, pharmacies will now have to include newer delivery care models, adherence tactics, and monitoring strategies to ensure the at-home lifestyle is conducive to proper management of conditions and medications.(7)
Many home apps have been created to assist the elderly with communication with caregivers and providers as well as management of medications. Apps including Jointly and Remind Me Care are available to assist with communication about an individual’s care, providing families and caregivers with the ability to create group messages, share calendars, and provide to-do lists and medication updates.(5) There are also various apps available, including Medisafe Pill Reminder and Dosecast, that remind individuals to take their medications at the right time every day.(6) If daily reminders fail to improve adherence, there are also pill dispensers available that essentially mimic an eMAR system, dispensing the medication and documenting doses taken daily. Additionally, as these patients may be alone at times or the majority of the time, there are also apps and panic buttons available to ensure that they receive the help that they need in an emergency.(3)
Telehealth and remote monitoring have become increasingly common as a healthcare option. In the LTC setting, there are numerous conditions and medications that can be monitored remotely. Using blood
pressure monitors, blood glucose meters, pulse oximeters, and weight scales, healthcare professionals can now manage disease states including hypertension, diabetes, weight loss/ gain, heart conditions, sleep apnea, COPD, and asthma.(7) Living in a technologically
imperative to providing at-home services. As there are many specialty medications given in this setting, pharmacies need to ensure that medications are packaged and stored at proper temperatures in order to be delivered. These settings should consider integrating software that calculates the most efficient driver routes and updates patients with the status of their medication.
Future of LTC
The decrease in the number of patients in LTC facilities and increase in those aging at home will likely be the new normal. To prepare for the heavier workload expected, LTC pharmacies will need to implement newer measures to support the monitoring and management of these patients. Taking advantage of the new advancements in technology through new apps as well as delivery software, remote monitoring, and adherence strategies will increase the efficacy of patient monitoring. By staying ahead of trends, LTC pharmacies will be able to operate and serve patients comparably to those that they serve in person at facilities.
