LEUPROLIDE ACETATE INJECTION DEPOT (leuprolide acetate for depot suspension) 22.5 mg for 3-month administration (leuprolide acetate) is indicated for palliative treatment of advanced prostate cancer.

IMPORTANT SAFETY INFORMATION1

Contraindications: Hypersensitivity: LEUPROLIDE ACETATE INJECTION DEPOT is contraindicated in individuals with known hypersensitivity to GnRH agonists or any of the excipients in LEUPROLIDE ACETATE INJECTION DEPOT. Anaphylactic reactions to GnRH agonists have been reported.

Warnings And Precautions:

x Tumor Flare: LEUPROLIDE ACETATE INJECTION DEPOT causes increases in serum levels of testosterone during the first weeks of treatment. Isolated cases of ureteral obstruction and spinal cord compression have been observed, which may contribute to paralysis with or without fatal complications. Transient worsening of symptoms may develop. Patients may experience a temporary increase in bone pain. Patients with metastatic vertebral lesions and/or with urinary tract obstruction should be closely observed.

x Hyperglycemia and Diabetes: Hyperglycemia and an increased risk of developing diabetes have been reported in men receiving GnRH agonists. Monitor blood glucose and/or glycosylated hemoglobin (HbA1c) periodically in patients receiving a GnRH.

x Cardiovascular Diseases: Increased risk of developing myocardial infarction, sudden cardiac death and stroke has been reported, and should be evaluated carefully along with cardiovascular risk factors. Patients receiving a GnRH agonist should be monitored for symptoms and signs suggestive of development of cardiovascular disease and be managed accordingly.

x Effect on QT/QTc Interval: Androgen deprivation therapy may prolong the QT/QTc interval. Providers should consider whether the benefits of androgen deprivation therapy outweigh the potential risks in patients with congenital long QT syndrome, congestive heart failure, frequent electrolyte abnormalities, and in patients taking drugs known to prolong the QT interval. Electrolyte abnormalities should be corrected. Consider periodic monitoring of electrocardiograms and electrolytes.

x Embryo-Fetal Toxicity: LEUPROLIDE ACETATE INJECTION DEPOT may cause fetal harm when administered to a pregnant woman. Advise pregnant patients and females of reproductive potential of the potential risk to the fetus. Most common adverse events (incidences >10%) are hot flushes, upper respiratory infection, fatigue, diarrhea, pollakiuria, arthralgia, and injection site pain.

Use in specific populations: Lactation: A decision should be made to discontinue breastfeeding or discontinue the LEUPROLIDE ACETATE INJECTION DEPOT.

Females and Males of Reproductive Potential: LEUPROLIDE ACETATE INJECTION DEPOT may impair fertility in males of reproductive potential.

Pediatric Use: The safety and effectiveness of LEUPROLIDE ACETATE INJECTION DEPOT in pediatric patients have not been established.

Geriatric Use: Hot flushes occurred with equal frequency in those ≤ 65 years of age.

The Company

HAC Pharmacies

HAC Pharmacies serves multiple communities in Oklahoma through 24 locations in HAC grocery stores. These include Homeland, HAC’s largest banner and the state’s largest locally owned grocery store. HAC has been 100% employee-owned since 2011.

The Challenge

While continuing to provide high-quality care to patients statewide, HAC needed to reduce its cost of goods sold (COGS) as much as possible. They needed strategy and tools to save money without costing them too much time.

Like many retail pharmacies, HAC had to contend with a complex pharmacy purchasing ecosystem not designed for transparency. Medication prices change daily, and thousands of products are listed across multiple vendors and purchasing interfaces. Purchasing sta at HAC were stuck flipping back and forth between wholesalers. They often bought in bulk when they found a low price from a secondary, only to miss out when their primary vendor began o ering a better price.

HAC was also losing time and money. For example, Joshua Miller, HAC’s director of pharmacy, was dedicating up to an hour twice a week to compile a list of commonly purchased items from their secondary wholesaler. But it was ultimately just the best guess based on what he could see at that time; Josh knew he was missing better purchasing opportunities buried in the data.

While Josh was spending too much time on purchasing, others at HAC pharmacies weren’t spending any time on it. COGS wasn’t a concern. A wave of 100% gross on COVID-19 vaccines shifted the focus away from COGS.

Yet the pharmacy retail market has changed drastically over the past few years. The additional cost of vaccines, this year’s double DIR “hangover,” lower reimbursements, and shipping delays are a few factors that make it crucial to focus on saving money wherever possible.

The Solution

When Josh assumed a leadership role at HAC in 2022, he was determined to find ways to reduce COGS, compensate for DIR fees and find reliable vendors. One primary wholesaler suggested their purchasing tool, but Josh and his team wanted a “neutral” solution.

They needed to enhance purchasing from their entire catalog and empower them to make purchasing decisions based on their business goals and strategy.

Josh selected SureCost for greater purchasing transparency and its flexible technology. HAC worked with the SureCost team to determine the best onboarding process. They decided on three live webinars for groups of six to eight employees at each of the 24 stores. The onboarding went smoothly, and HAC has been using SureCost for over a year now. They started with SureCost in August 2023, all stores live and ordering in late October 2023.

How SureCost Helped

SureCost puts everyone making a purchase at HAC on the same page. Sta across all 24 locations use this solution to find the best purchasing option—which isn’t always the lowest-priced item. As Josh learned, a product that costs a little more upfront could allow them to meet their minimum volume. They can also spot “one-o ” purchasing options where they can save a lot of money. In some instances, it may be a commonly sold item that one vendor hasn’t focused on.

HAC pharmacy sta frequently consult SureCost’s “potential savings” tab to maximize the savings. With large purchase orders of 150 items at a time, they check the tab and discover additional savings from comparable products. That’s while purchasing from all six of HAC’s regular vendors and still complying with their primary vendor and other purchasing agreements. Leadership also has the option to configure SureCost to drive purchases to selected items based on potential reimbursement and other considerations.

SureCost’s reporting also enhanced how HAC purchases. The “order substitution” report allows them to readjust when ordering items close to depletion. “Purchase history” provides business intelligence around trends or potential losses. “Vendor purchasing breakdown” lets HAC analyze their spending. Josh notes that his team has only begun to explore the wide range of reports in SureCost. He also advises that "if you’re a data nerd, there’s every possible report you could think of.”

Benefits

Josh calculates that HAC has saved approximately $200K each month after using SureCost for just four months. He’s also saving up to four hours a week otherwise lost to the tedious process of manually comparing catalogs. Speaking about his colleagues at HAC, he explains that “having [all vendors] in one place to look has made it easier for them to find the best product for the best price for the patient in a timely manner without wasting hours doing it.”

HAC is also more prepared to mitigate shortages and issues with suppliers. “We can go to one place and see if the primary vendor or a secondary has it available, with updated on-hand amounts, and know we’ll get it the next day so it’s there for patients.”

Even locations that weren’t analyzing their costs now spend a modest amount of time on smarter purchasing. He admits that “it’s not as easy as just clicking the ‘Order’ button. It requires some thought and a little time,” but adds that “the savings have been worth it."

“At the end of the day, we all need to realize that everyone is part of a business looking out for its patients and best options. We only survive as well as what we can bring to our bottom line. As leaders, we need to protect our business’s interests. That means not always taking the comfortable or familiar choice, and instead going outside to find a neutral party.”

Secondary wholesalers play a vital role in the pharmacy supply chain; and in addition to primary wholesalers, pharmacies have a plethora of options for finding better deals, finding access to hard-to-find products, and even better payment terms. Almost every primary contract has a carve-out that allows pharmacies to purchase from secondary suppliers, so let’s discuss it!

Q. With negative reimbursements at an all-time high, what’s the best way to shop for brands and generics?

Pharmacies need to find the profitable nuggets and dig deeper to squeeze out more profits. Relying on a single source vendor is often not the best decision. There are over 50 licensed drug wholesalers across the country that have really great prices on the same items the primary suppliers carry.

Q. What products do the secondary wholesalers carry?

While a primary consists of brands, specialities, generics, OTCs, HBAs, and controls, secondaries have one or more of the above. They may carry some brands and many generics, while some will carry only generics and short dates, something the primaries don’t. In addition, while primaries will lean on supplying specific customers

with items during a shortage, there are always some secondaries that may have the item when your primary supplier is out.

Q. What prices do the secondary wholesalers have compared to a primary contract?

On many items, secondary wholesalers can have it up to 50% cheaper than a primary. This is even on regular items and not a short date. Secondary suppliers negotiate with a different set of manufacturers that aren’t carried by the primaries and have good deals. In addition, while the primaries are forced to make a margin due to brand constraints, secondaries can sell items for cheaper on a lesser margin.

Q. How do I manage my GCR when buying on the secondary?

Something hard to calculate live is your GCR which leads many pharmacies to overspend toward the end of the month to get a better discount on their brands and higher rebate, and while this looks good on screen, in reality, it’s a juxtaposition, as there are items that can save you thousands in the short run that will have an ever drastic marginal gain in the long run. Pharmacies might want to take a smaller discount on their brands but in turn be able to shop generics, which are often times where the biggest savings are seen and where the primaries are forced to make up their margins on. What’s important to do is set a mental number in your head, say 20%, and if any item is 20% cheaper on the secondary market, buy it there.

Q. How do I know a secondary supplier is a good supplier?

Secondary suppliers should be NABP accredited, meaning they buy every item directly from a manufacturer or close to it. They should have a good list of products, good shipping minimums, and be in good standing with the state board and DEA. Pharmacies should be weary of suppliers selling brand items at discounts larger than a primary.

Q. Do any secondary wholesalers have controls?

Yes, many secondary suppliers have controls and will each have their own parameters for purchase. Some will have CIII and above, while some will carry a select few.

Q. How will DSCSA be managed with a secondary supplier?

Every legitimate supplier will either be completely set up to handle DSCSA requirements or be close to it. Wholesalers should be able to send the EPCIS files directly to a provider of your choice and have the process enabled. While the requirements were delayed for smaller dispensers, they are still required for larger entities, and the deadline is coming up.

Q. How do shipping minimums and delivery times work with secondary wholesalers?

While some exceptions exist, most secondary wholesalers pack items into a box and ship it through either UPS or Fedex. Depending on where they are located in relation

to each pharmacy, they may have different minimums and shipping prices. The larger secondary wholesalers should charge between $100 and $150 for overnight delivery, while smaller wholesalers can charge upward of $500 for overnight delivery. Some even ship for Saturday delivery upon request.

Q. I get steady calls from wholesalers every day, is there a way to streamline purchases in an orderly fashion and find the best prices between all of them?

DSCSA Update: What You Need to Know About the Small Dispenser Exemption and Other Waivers or Exemptions

FDA determined that pharmacies and other “small dispensers” are exempt from certain DSCSA interoperable system requirements until November 27, 2026. NCPA is thrilled with this win, given our continued and consistent advocacy efforts on this issue spanning more than 10 years. Additionally, dispensers that do not qualify as small dispensers may be able to obtain a separate waiver or exemption from certain requirements of the DSCSA.

Who is a “small dispenser”?

The FDA considers a dispenser to be a “small dispenser” if the corporate entity that owns the dispenser has a total of 25 or fewer full-time employees1 licensed as pharmacists or qualified as pharmacy technicians as of November 27, 2024.

What are the small dispensers exempt from?

Until November 27, 2026, these small dispensers are exempt from:

• Requirements to verify the product identifier of the statutorily designated proportion of suspect or illegitimate product in the dispenser’s possession or control.

• Small dispensers are still obligated to meet all other verification requirements.

• The requirement that the transaction information and the transaction statements be exchanged in a secure, interoperable, electronic manner.

• Small dispensers and their trading partners may continue to rely on current methods for providing, capturing, and maintaining transaction information and transaction statements for transactions of product with each other

during the exemption period. Small dispensers should verify that their authorized trading partners (ATPs) have this capacity, or that they have a waiver or exemption.

• The requirement that the transaction information required to be exchanged include the product identifier at the package level for each package included in the transaction.

• For transactions to which small dispensers are a party, small dispensers and their trading partners may continue to exchange transaction information with each other that does not include the product identifier at the package level for each package included in the transaction.

• The requirement for systems and processes for verification of product at the package level, including the standardized numerical identifier.

• Small dispensers and their trading partners may continue to rely on current methods for verification activities with each other during the exemption period.

• The requirement for systems and processes necessary to promptly respond with the transaction information and transaction statement for a product upon a request by the secretary, or other appropriate federal or state official, in the event of a recall or for the purposes of investigating a suspect product or an illegitimate product. Small dispensers may continue to rely on current methods to respond to such requests for such information. All dispensers and their ATPs are required to respond to a request for information from an ATP, FDA, or any other appropriate federal or state official within two business days in the event of a recall or to investigate a suspect or illegitimate product.

• The requirement for systems and processes necessary to promptly facilitate gathering the information necessary to produce the transaction information for each transaction going back to the manufacturer, as applicable (i) in the event of a request by the secretary, or other appropriate federal or state official, on account of a recall or for the purposes of investigating a suspect product or an illegitimate product; or (ii) in the event of a request by an ATP, in a secure manner that ensures the protection of confidential commercial information and trade secrets, for purposes of investigating a suspect product or assisting the secretary, or other appropriate federal or state official, with a request described in clause (i). Small dispensers may use current methods to respond to such requests with their relevant transaction information if they directly transacted the product(s) subject to the request. If a small dispenser is claiming such an exemption, it does not need to formally file such an exemption with FDA. However, FDA recommends that small dispensers should communicate their exercising their small dispenser exception to their trading

partners as needed to further facilitate timely and efficient distribution of product.

FDA has stated that these exemptions are not intended to provide and should not be viewed as providing a justification for delaying efforts by small dispensers to adhere to requirements under the DSCSA.

How to Apply For a Waiver or Exemption if You

Don’t Meet the Small Dispenser Exemptions

The DSCSA one-year stabilization period ends on November 27, 2024, and the agency is not extending the stabilization period beyond this date.

Trading partners including pharmacies who do not qualify for the small dispenser exemptions and are unable to meet the enhanced drug distribution security requirements of section 582 of the FD&C Act by November 27, 2024, may request a waiver or exemption from those requirements through FDA’s CDER NextGen portal.2 Pharmacies may request a waiver if the requirements would result in an undue economic hardship or for emergency medical reasons, including a public health emergency declaration pursuant to section 319 of the Public

Health Service Act. A pharmacy may request an exemption for other products or transactions from certain requirements in section 582 of the FD&C Act to maintain public health or is otherwise appropriate. Although requests can be submitted at any time, the FDA recommends trading partners submit a waiver or an exemption request by August 1, 2024. The agency cannot guarantee it will grant or deny the waiver or exemption by November 27, 2024, but will make every effort to do so.

What if a dispenser that does not qualify for the small dispenser exemption has systems and processes in place to comply with DSCSA, but only receives partial data from suppliers? Is the dispenser compliant?

In this case, the dispenser is not compliant. While some pharmacies may have electronic and interoperable systems in place and may have connections established with their trading partners, if these pharmacies are not receiving complete data from their upstream trading partners, they are not in compliance with the DSCSA and should consider filing a waiver or exemption.

What additional resources are available to assist with DSCSA compliance?

The dispenser community has collaborated to make DSCSA educational resources available at www.dscsa.pharmacy. NCPA has resources for its members to ensure they comply with the aspects of DSCSA that are enforceable today and for use in preparing for the end of the small dispenser exemption in November 2026, including its DSCSA checklist. Additionally, the National Association of Boards of Pharmacy has developed DSCSA educational and compliance tools that are available at no cost.
