THE CHALLENGE Tarrytown Expocare is an LTC pharmacy specializing in intellectual and developmental disability communities nationwide. Their goal is to create efficient processes and provide superior service to home- and community-based providers, intermediate care facilities, and individuals living at home. Founded in 2007, Tarrytown is currently licensed in Texas, New Mexico, Arizona, Oklahoma, Kentucky, Ohio, and Tennessee. Staffing had also grown, but “we were running out of space in the pharmacy,” said Zach Corbell, Pharmacist-in-Charge at Tarrytown. “We were looking for opportunities to increase our production hours without bringing in additional staff.”

Many of these drug therapies, particularly biologics, are highly temperature sensitive and could lose efficacy if not kept within a specific temperature range.

Continually seeking innovative best practices, USSC turned to Temptime when investigating temperature monitoring solutions to help ensure the medications they deliver maintain their integrity throughout shipping.
According to Marlette Oelofsen, CSP, Pharmacist in Charge at USSC, the company ships medications from their Florida and Colorado locations to patients in all 50 states. When shipping from states with such different climates, it can be challenging to develop standardized shipping protocols that ensure medications maintain appropriate temperatures throughout transit. There’s literally hundreds of different courier routes to consider, she says, which can be further complicated by unexpected shipping and delivery delays. “Risk of compromising the integrity of the medications exists throughout the entire shipping process,” Ms. Oelofsen continues. “At pick up, for instance, packages might sit outside on the loading dock for a period of time before the courier arrives. They then typically go through UPS’s hub to the destination city. Temperatures within the courier’s delivery trucks and airplanes as well as at the shipping hub are uncertain and packages may be subjected to outdoor conditions at several points along the way—there is risk of exposure to extreme heat or cold at virtually every stage.”

The DOSIS products each focus on three main objectives: one, reducing the opportunities for pharmacy errors by minimizing the human intervention required; two, reducing disorder in the pharmacy by providing automated, predictable, and repeatable processes; and three, lowering operating costs by reducing labor and eliminating non-valueadded activities for the pharmacy staff. and patient-labeling of single-medication blister card prescriptions, each DOSIS L60 robot has a monthly production capacity exceeding 15,000 blister cards. Using one-of-a-kind dispensing technology to eliminate the need for canister exchanges or on-site canister hardware calibration, DOSIS robots allow generic manufacturers to be changed without changing canisters.
n DOSIS C60: Adapted from the L60 design, the DOSIS C60 offers
identical functionality but is physically adapted for 3"x 9" cards commonly used for narcotics or to fit in med carts configured for strip-packaging. Each DOSIS C60 card holds up to 16 doses.
n DOSIS U60: Based on the L60 platform, DOSIS U60 adds the functionality of full unit-dose labeling within the robot. Each blister of each card can be printed with your requirements of NDC bar code, drug name and strength, lot number, expiration date, etc.
n DOSIS mPAQ Printing: There are two primary aspects to DOSIS

PRIMARY INTENDED OUTCOME There are approximately 22,000 independent pharmacies in the United States, according to the National Community Pharmacists Association. The modern pharmacy landscape is constantly changing and growing to serve communities large and small, while facing hurdles including reimbursement issues, complex medication regimens, and narrowed patient access. Given these challenges, pharmacy managers must choose wisely when investing in equipment and technology intended to maximize space, accuracy, and productivity. The TCGRx Beacon Pharmacy Inventory Management System supplies a range of scalable technologies and strategies to meet pharmacy needs and improve operations no matter the size or scope of a particular facility.
THE CHALLENGE In 2016, John Bodtker was working at an independent pharmacy in the San Francisco Bay Area and seriously considering pharmacy ownership. His direction shifted a bit when a job posting appeared for a startup pharmacy manager in his hometown of rural Placerville, California. The Shingle Springs Health & Wellness Center is an Indian Health Service/ Tribal/Urban Indian Health pharmacy owned and operated by the local Miwok Tribe. Serving a unique community can change some of the usual assumptions of pharmacy management, including the common constraints around relying upon a specific formulary.

CASE STUDY “Our philosophy is not the same as a typical retail pharmacy,” Bodtker noted. For example, he said, “We don’t have a strict focus on lean inventory. We try, instead, to have everything in stock to provide the best customer service possible.” Maintaining an expansive inventory, however, can create spatial, organizational, and access challenges that could potentially contribute to inefficiency and dispensing errors. The right solution would provide storage, enable speed, and enhance safety, all at the same time.
THE DECISION Bodtker was a decision maker involved in choosing equipment for Shingle Springs before the doors even opened in June 2017. He researched shelving options and found that the TCGRx Beacon System’s variety of high-density storage cabinets were a very attractive feature for a new startup pharmacy with large storage needs. “They provided us with a lot of space to start with,” Bodtker said, “and we have room to grow.” Beacon also provides the ability to pull medications directly from storage, without having to rely on or bypass a robotic dispensing system. “This was a key point for us,” Bodtker said.
THE RESULTS Shingle Springs has been accepting commercial, state, and Medicaid plans since December 2017. They currently average 100 prescriptions a day, and they are only picking up speed. “We have a lot of patients with complicated drug regimens and those who live in remote areas,” Bodtker said. “When they come in for their visits, efficiency is nice. We try to button everything up and can usually get things answered pretty quickly.” Working from a number of formularies means the need to shelve a vast inventory. The Beacon storage system consolidates the pharmacy’s footprint through minimizing on-hand inventory, increasing inventory turns, and reducing expired medications.

The will-call system utilizes the same “directed by light” and bar-code identification technology that other TCGRx solutions rely on, minimizing staff members’ learning curve. DeliverRx securely contains all will-call prescriptions in a single, easy-to-find location, further reducing customer wait times and making pick-ups clearer and less prone to error. Bodtker hopes to purchase a compatible refrigeration unit as well.

The pharmacy is also incorporating the TCGRx SmartCardRx™ blister-packaging system within the next year. “We currently have 30 or so patients on manual-fill blister packs,” Bodtker said, “ and we want to establish a solid client base.” Finally, Shingle Springs will be expanding to include a long-term care facility on the reservation, which will require an automated medication dispensing system. “TCGRx’s PharmLoc™ cabinet would be the perfect segue for us,” Bodtker said. When a single company provides a comprehensive, configurable system, growing a pharmacy is easy and quality control stays in sync. “I have worked in pharmacies doing less that felt much busier,” Bodtker said. “We save footsteps and we get prescriptions to people quicker and safer.” For Shingle Springs, Bodtker said, relying on TCGRx and Beacon means increased efficiency along with greater dependability. “Thanks to Beacon, a larger inventory actually means we are less harried and more accurate. Our error binder is thin because there just isn't much going into it.”

Dr. Dearman serves as the Director of Pharmacy and Health Risk Management at Vantage Health Plan, Inc. in Monroe, LA. and has served as a ULM College of Pharmacy preceptor in medication therapy management since 2016. Dr. Dearman graduated with a B.S. in Pharmacy from the University of Louisiana Monroe College of Pharmacy in 1988 and with a Doctor of Pharmacy from the University of Kansas School of Pharmacy in 2009. He holds certifications in diabetes, asthma, tobacco cessation, respiratory care, immunization administration, medication therapy management, and ambulatory care. He is passionate about training future pharmacists, and strives to provide a great experience where students can gain knowledge and skills of a pharmacy world that exists beyond the traditional retail and hospital pharmacy role. “Dr. Dearman is one of the greatest, most approachable preceptors – and human beings – ever!” commented one of his students. “I can talk to him about anything. For Natalie, it was a no-brainer: “I had reached the tipping point,” she said. “I didn’t have the time to practice pharmacy like I knew I wanted to. That low satisfaction meant I was ready for a change.” Maybe you already own an independent pharmacy (or several) and are looking for a different kind of change. Take these steps today 1. First things first: set your priorities. Why do you want to become a pharmacy owner? To serve patients your way? Contribute more to your community? Lead a team that can make a difference? Whatever your vision is, defining it now will help you make better business decisions later. That includes one of the most important of all: choosing the franchise operation that’s the best fit for your business and management philosophy. For Natalie, that meant meeting with owners of major franchise brands. “I did a road trip to talk to some current franchisees, and my visit with the Medicap Pharmacy® owner sealed the deal. 2. Develop your business plan. The idea can seem overwhelming at first—but you can succeed with the right support. Knowing that balance is key to making sure you don’t tie up too much cash in inventory or waste drugs because they expire before being used. • Accounts receivable (A/R). Cash flow is the lifeblood of your business. How will you keep it healthy, even as reimbursements shrink and fees from PBMs (pharmacy benefits managers) add up?
3. Build your team. Whether you’re acquiring an existing location or opening a new one, the right franchisor can give you all the business, brand and patient care support you’ll need. In addition to preparing a business plan, that also includes: • Performing a market analysis. • Assessing revenue potential based on patient population, provider mix and area competition. • Helping you obtain financing, including getting pre-qualified with pharmacy lenders. • Working with a real estate firm to scout retail locations and identify the ideal one. • Implementing business operations— including store layout, staffing, inventory management, workflow management and more.

PRIMARY INTENDED OUTCOME According to the Centers for Disease Control and Prevention, there were nine million Americans receiving services in or through long-term care (LTC) facilities as of the year 2014. Research indicates that the average LTC medication pass involves 73 different pharmaceuticals and can take a nurse nearly two hours to complete. Blister packaging makes medication administration easier for nursing staff by eliminating the need to open multiple prescription bottles for each dose. It also prevents dispensing errors: Packaging labels can clearly indicate days and times that medications should be administered, and staff can confirm at a glance whether or not a dose was given. Despite the benefits of blister packaging, manually loading and labeling is extremely time-consuming and inefficient. Manchac Technologies' DOSIS robots offer a fully automated, cost-effective solution that brings reliable, hands-free blister packaging right into your pharmacy.
THE CHALLENGE Tarrytown Expocare is an LTC pharmacy specializing in intellectual and developmental disability communities nationwide. Their goal is to create efficient processes and provide superior service to home- and community-based providers, intermediate care facilities, and individuals living at home. Founded in 2007, Tarrytown is currently licensed in Texas, New Mexico, Arizona, Oklahoma, Kentucky, Ohio, and Tennessee. Between 2012 and 2015, the pharmacy experienced significant growth, and their volume more than tripled from 300 to approximately 1,000 patients. Staffing had also grown, but “we were running out of space in the pharmacy,” said Zach Corbell, Pharmacist-in-Charge at Tarrytown. “We were looking for opportunities to increase our production hours without bringing in additional staff.”

CASE STUDY Reallocation Conserves Resources Tarrytown started out with a single DOSIS L60 machine in 2015. In 2016, they acquired one of their largest competing pharmacies in Texas, doubling the company’s size literally overnight. This magnitude of growth often means hiring additional technicians without necessarily understanding the new workload demands. Luckily, the acquisition also included two additional DOSIS robots.
“
We knew the amount of volume that would be able to go through those machines,” Corbell said, “so we did not have to over-hire staff.”
Reallocation of existing technicians was able to handle the volume outside of what the DOSIS machines were managing.
THE DECISION As a cycle-fill pharmacy, Tarrytown’s process is designed to ensure that patients avoid any lapses in therapy due to expired refills. Similar to what is known as “pre-packing,” all of Tarrytown’s prescriptions are filled in advance for the first of the following month. They have 12 focused business days of production time for each cycle, and 100% of the solid oral-dose medications they provide are delivered in blister packs. As business grew, Corbell knew they would need to find the right solution. “We knew what we wanted to get out of DOSIS,” Corbell said. The hope was that adopting automation for a large portion of their blister packaging needs would give the techs “more breathing room in their workflow.” As with any new technology, there were anticipated hurdles in integrating the machines, such as potential changes to Tarrytown’s labeling. “Any change in medication labels can cause anxiety and stress to the providers,” Corbell said. Fortunately, DOSIS provided the answer to that concern, as well.
THE RESULTS Tarrytown Expocare now operates three DOSIS L60 units that fill, seal, and label between 18,000 and 20,000 cards per month. “When the machines are queued up with thousands of orders, we will typically get as much as seven or eight hours of additional production time from the machines after all of our employees leave for the day.” DOSIS offers an enhancement in their mPAQ Printing system, which integrates with pharmacy management software to format and route all labels printed throughout the pharmacy. The mPAQ system allows the information that is printed on the blister packs to be fully customized. “Manchac support took our existing label format that we had been using for years and recreated it on the DOSIS label template,” Corbell said. “Our providers were OK with it, because all of the information was in the same spot on the new labels.” DOSIS provides other formatting enhancements, including color-coded med-pass and time-pass tools that add even more efficiencies. “On a new location check, the nurse had four different colored sticky pads and was writing the hour of administration on each pack. I knew we could do better than that with DOSIS,” Corbell said. In addition, LTC providers all have individuals who leave their home or facility to attend workshops or other off-site activities. “We had DOSIS add a time-pass flag to the mid-day dose cards that are split out so they can be easily identified and sent to the workshop locations for the month.”

Our unique name reflects our continuous search for better solutions for the pharmacy industry: Manchac is a Choctaw word meaning "the better way". Native Americans near Baton Rouge had two options to get to the open waters of the Gulf of Mexico to trade with neighboring tribes along the coast. These Choctaw either wound 225 miles down the Mississippi River or went 125 miles through a bayou, a swamp, and Lake Pontchartrain into the Gulf. This Manchac route was not as obvious as the Mississippi, but it was more efficient, cutting the distance almost in half. This beautifully reflects the mission of Manchac Technologies: provide the better solution, even if it is not the easily-discovered one.

Labels can be formatted to include other data that can be useful even before the prescriptions leave the pharmacy. “Data on our labels drives many steps of our workflow,” Corbell said. “Since we are a cycle-fill pharmacy, a large percentage of our monthly dispensing leaves here in a short amount of time – two to three days each month. Using mPAQ, we have streamlined our sorting procedures to make getting blister packs to appropriate locations very easy using the sorting data we have printed on each label.” In addition to label customization, mPAQ’s zone-based printing option routes labels to numerous zones. This advantage reduces missing labels and label-sorting mistakes, and cuts down on the need for technicians to repeatedly walk from workstation to printer. “Before DOSIS, we were printing 100% of our labels on a single printer. One tech would spend an entire day standing there, folding the labels and replenishing the roll. We have six or seven printers now.” Through DOSIS, the mPAQ software is also able to produce labels for those blister packs that must still be filled by hand. “Fully 100% of the prescriptions that go out our door are printed by DOSIS and mPAQ,” Corbell noted.
THE CONCLUSION Before DOSIS, Tarrytown’s blister packs were all filled by hand during regular business hours, necessitating a real production crunch two times a month. “The biggest impact that DOSIS has had in the pharmacy is the ability to continue production after hours with no technician present,” said Corbell. A single technician is dedicated to staffing the three L60 machines that produce more than a third of the pharmacy’s output. A team of seven technicians manually pulls and packages the remaining meds, which require pill-splitting or other special handling. “Upward of 40% of our cycle fill being produced by a single technician using the DOSIS machines is definitely significant when you are looking at the bottom line,” Corbell said. The pharmacy continues to grow, and while Corbell is confident that their three DOSIS robots will be able to keep up with that pace for another year or so, he said, “we are already considering purchasing another machine.”

Protecting Patient Outcomes Door-to-Door Meeting challenges in developing standardized shipping protocols that ensure medications maintain appropriate temperatures throughout transit
Business Challenge USSC works with a broad range of specialty medications used to treat chronic conditions ranging from anemia and psoriasis, to cancer and rheumatoid arthritis, to HIV/AIDS and infertility, among others. Many of these drug therapies, particularly biologics, are highly temperature sensitive and could lose efficacy if not kept within a specific temperature range.

Their objective is to give patients with chronic health conditions easier access to the complex drug therapies they need. Continually seeking innovative best practices, USSC turned to Temptime when investigating temperature monitoring solutions to help ensure the medications they deliver maintain their integrity throughout shipping.

According to Marlette Oelofsen, CSP, Pharmacist in Charge at USSC, the company ships medications from their Florida and Colorado locations to patients in all 50 states. When shipping from states with such different climates, it can be challenging to develop standardized shipping protocols that ensure medications maintain appropriate temperatures throughout transit. There’s literally hundreds of different courier routes to consider, she says, which can be further complicated by unexpected shipping and delivery delays. “Risk of compromising the integrity of the medications exists throughout the entire shipping process,” Ms. Oelofsen continues. “At pick up, for instance, packages might sit outside on the loading dock for a period of time before the courier arrives. They then typically go through UPS’s hub to the destination city. Temperatures within the courier’s delivery trucks and airplanes as well as at the shipping hub are uncertain and packages may be subjected to outdoor conditions at several points along the way—there is risk of exposure to extreme heat or cold at virtually every stage.”

The Solution USSC uses Temptime’s EDGE™ data loggers and TransTracker® CF visual temperature indicators to gain logistical insight into temperature controlled shipping methods and safeguard against the administration of medication exposed to potentially damaging temperatures. “As a pharmacist, patient safety is my primary concern,” says Ms. Oelofsen. “When we decided to incorporate temperature monitoring devices, we were looking not only for the best and most accurate devices but also for technology that was easy for the pharmacy to use and for our patients to understand.” They began by conducting extensive performance qualification (PQ) testing with their packouts using different types of data loggers and temperature trackers to monitor the temperature of test shipments throughout their service area. “We found that Temptime’s EDGE M-300 and S-400 programmable data logging devices were easy and quick to set up—and provided us with detailed yet easily comprehendible data,” Ms. Oelofsen explains. “The EDGE M-300 has a replaceable battery, so it can be reused indefinitely, and since the devices communicate via Bluetooth®, you can change settings and view data even through packing materials.” In addition, she says Temptime’s TransTracker CF temperature indicators are included in shipments to give patients a clear visual alert if the package has exceeded the temperature range during transit. They chose the TransTracker CF for its accuracy, also noting they were easier to use and interpret than others they tested. Some brands require indicators be
manually activated before use, adding an extra step for the packing team and potential risk that someone might forget to activate the device – leading to confusion or false positives. Other indicators lacked simplicity in design and were difficult for patients to understand, increasing phone calls to the pharmacy. After considering all options, USSC felt that TransTracker CF was the obvious best choice for them. “Without indicators, we would have no definitive way of knowing whether or not a package reached unsafe temperatures during transit – meaning any patient concerns about the temperature of their medication would result in a very costly reshipment for the pharmacy as well as delayed treatment for the patient.” Temperature data for PQ testing was recorded by the EDGE M-300 sensor every 10 minutes over a 36+ hour period. Data was uploaded to EDGEVue™ App to generate this graph. Note the 46ºF High Limit (red line) was exceeded for approximately 2.5 hours.
USSC’s Testing Methodology To simulate actual shipments as closely as possible, USSC places expired medications within their packouts during testing. They test a wide variety of scenarios with numerous packout sizes considering diff erent points of origin and destination as well as various shipping routes used by the pharmacy. Packing instructions are precisely followed and

shipments and other high-risk medicines. Considering the critical nature of the medications we dispense and the unpredictable risks associated with shipping, we feel that it is the right thing to do for our pharmacy and our patients.” USSC pharmacists counsel new patients on the importance of the temperature indicator and provide guidance on interpreting it as a part of the initial onboarding process. Additionally, literature is included with each shipment explaining how to read the indicator and what to do if it has triggered.

additionally include two EDGE data loggers that have been programmed to record temperature at predetermined intervals and provide alerts when particular temperature limits are exceeded. One EDGE device is placed inside the cooler by the medication to measure internal temperature. The second is placed ‘outside’ the package, either wedged in between the outside of the cooler and its surrounding corrugated box or in a padded envelope attached to the outside of the package to measure the external ambient temperature. Each test is replicated as a control for the fi rst, and tests are conducted several times a year to ensure the impacts of seasonal climate change have been considered.
The Result “Both physicians and patients have told us how much they appreciate the extra measures we take to give them peace of mind with at-a-glance confirmation that medications have remained viable throughout shipping,” says Ms. Oelofsen. “We now use TransTracker CF indicators with all refrigerated

Temptime’s EDGE devices have also proven extremely valuable to USSC’s quality control processes. “It’s been a challenging year, with extreme temperatures seen across the country. This winter was one of the coldest on record and we had several concerns.” This led USSC to conduct testing, using the EDGE devices to pinpoint exactly what happens with shipments during extreme cold. “We thought packages were being exposed to freezing temperatures at their point of origin in Colorado, but because of the information we received from our EDGE data loggers – we discovered the issue was happening at the courier’s hub in Kentucky,” she explains. “With that knowledge, we were able to make educated changes to the packout and eliminate the problem.” The result? USSC has successfully decreased the number of medication reshipments, delivering significant savings for the pharmacy, reducing drug waste and improving patient satisfaction. Performing the Test
The Pyrosate® kit is a rapid gel-clot test that contains a 2λ* endotoxin tube (PPC) matched to the sample (SPL) tube for each sensitivity. This feature is unique to the Pyrosate assay. The endotoxin tube (PPC) assures that the sample does not interfere with the test, ruling out false negatives. Pyrosate is formulated to eliminate false positives due to (1g3)-ß-D-glucans. This endotoxin specific reagent does not require additional blocking buffers.
Reconstitution Pyrosate® is reconstituted directly with the sample by adding 0.5 mL to the sample tube (SPL). After approximately 60 seconds of gentle mixing, 0.25 mL is transferred to the endotoxin tube (PPC). The lot-specific incubation time at 37±1°C is given on the Certificate of Compliance. Stability Pyrosate® is stable at room temperature and does not require refrigeration for shipping or storage. PRIMARY INTENDED OUTCOME Long-term care (LTC) pharmacies serve the medication needs of 9 million Americans, according to a 2014 report from the CDC. The typical LTC patient is living with more than one chronic condition and relies on as many as 10 different prescription drugs. For these generally elderly and often frail patients, prescription drugs are critical and STAT situations are common. Such potentially life-threatening, around-the-clock needs are why all LTC pharmacies are legally required to have an emergency backup system in place. Research conducted by Care Services LLC shows that anywhere from 2.5 million to 4 million claims per year are called in to a backup service and require a reliable partner to fulfill. MedCall, a leading provider of backup pharmacy services, is a member of the Care Services family of brands. THE CHALLENGE Providing medically complex patients with 24-hour access to a pharmacist seven days a week is challenging and can lead to inefficiencies and staff burnout. A ROBUST DELIVERY NETWORK With access to more than 3,000 couriers nationally, MedCall provides coverage for all active customers. The network covers over 85% of the nation’s zip codes and 94% of the national population base.
According to Vice President of Pharmacy Services Tom D’Andrea, Neil originally handled all of their backup needs on their own. “We had to find the backup pharmacies and arrange for delivery ourselves,” he recalled. Organizing these services in-house was, and continues to be, costly and highly inefficient, taking resources away from a pharmacy’s day-to-day responsibilities and ability to grow rapidly.
THE DECISION In 2011, Neil was approached by MedCall. “They offered to organize all backup services around the clock, as well as to consolidate billing. They also included services to manage deliveries,” D’Andrea said. At that time, MedCall was owned by a trusted LTC pharmacist from the North Carolina area, a factor that contributed to Neil’s choice to sign on with them. When MedCall was purchased by Care Services LLC in 2013, D’Andrea opted to continue with them because of the quality of the service Neil Medical had received so far, as well as in response to enhancements the company was able to make with their new ownership structure, including extensive delivery services.
THE RESULTS LTC performance standards include timely access to emergency medications. “Speed is definitely something facilities expect,” D’Andrea said. Most LTC facilities hold their LTC pharmacies to a four-hour compliance window to deliver STAT medications. MedCall’s Complete Delivery Solution meets those expectations; the majority of the drug requests they receive are fulfilled in under two hours. “You may not even be able to walk into a drugstore on your own and get a prescription filled that fast,” he said. “It’s amazing that we can now get medications delivered to an LTC facility in less than two hours.” In those instances when an obstacle arises and threatens to hold up access to a drug, MedCall still comes through. The company offers each client individualized account management and manages a 24 hour, seven days a week, in-house customer service desk. D’Andrea recalled a situation where a prescription was requested that required a physician’s signature. “They could not locate the doctor to get him to sign off,” he said. “MedCall phoned that practice maybe 12 times in two hours. They wound up having a PA sign and were able to get the prescription filled. They went above and beyond, and they stayed on it to make sure the patient got his medicine. That’s great customer service.”
