Join AAMC for an evening under the stars as we shed light on the crucial need for mental health services in Anne Arundel County. In a county with 550,000 residents—the third most populous county in the state of Maryland—inpatient and outpatient mental health resources are severely limited. While some outpatient resources are available, they aren't growing at the pace needed, resulting in the transfer of dozens of residents to programs outside the county and the state for treatment every month. Anne Arundel Medical Center is planning to build a comprehensive mental health hospital on the Pathways campus on Riva Road. Set to open in late 2018, the first phase of this hospital will include a 16-bed inpatient mental health facility for adults, as well as an outpatient and partial psychiatric hospitalization program for both children and adults. These facilities and programs are desperately needed in this region. Last year, AAMC cared for more than 2,500 adult patients and more than 800 pediatric patients in our emergency departments suffering from depression, anxiety, bi-polar disease, suicidal thoughts and other mental illnesses. Crowns are amazing restorations—if you break your front tooth horseback riding or decay weakens a molar, a crown covers the damage and reinforces tooth structure in a way that looks natural. Whenever it’s time to get one, however, very few of us leap with joy. Many of us fail to appreciate just how far today’s gleaming crowns have come. In 200 A.D., the Etruscans, who lived in central Italy just north of Rome in what is now Tuscany, made crowns consisting of animal or human teeth held together by bands made of gold soldered together. Other cultures that used cosmetic dentistry included the ancient Egyptians, who hammered seashells into their gums as tooth replacements. During the 18th century, dentistry was actually performed by barbers and blacksmiths. Barbers routinely performed, along with shaving and cutting hair, tooth extractions usually without anesthetics. In fact, the all-familiar barber’s pole advertised a haircut and “dental surgery.” Paul Revere, America’s most famous messenger, was a highly skilled silversmith and placed advertisements in a Boston newspaper offering his services as a dentist able to make crowns and bridges. During this time, crowns were made crudely from cow bone, ivory tusks or human teeth (sometimes robbed from corpses buried in graveyards) and were installed using metal or wooden posts. Because the ill-fitting crowns never quite fit, they encouraged bacteria, decay and infection. Happily, much progress was made in the early 20th century. Novocaine was invented, dentistry’s first modern anesthetic, and the “lost-wax” technique was adapted for dentistry by W. H. Taggart. Using the techniques of ancient jewelers, Taggart discovered that warm wax could be used to create an impression of a tooth. The impression could be filled with heated materials such as gold or metals that when cooled became artificial teeth implanted into the mouth.

A digital wand is equipped with a tiny camera. Three-dimensional images and measurements of your mouth are sent immediately to a lab via computer.

In some ways, Taggart’s basic principle remains the same. You may be familiar with this scene: Your dentist gives you a tray of often bad-tasting putty and tells you to bite down for a few minutes. Then, a temporary crown is made for you and cemented in lightly while your impression is mailed off to a lab and processed. Weeks later, you return to the dentist who cements the final crown into place. Dr. Brian Valle of Valle Functional and Cosmetic Dentistry explains that, thanks to advanced digital technology, uncomfortable impressions are no longer necessary. The fitting, fabrication and installation of a crown has evolved from a messy process taking weeks into a smooth generally two-step procedure taking as little as days. It is a special kind of art involving close teamwork between dentist and an accomplished lab, he notes. Here’s what happens in his office. First he designs and prepares the tooth needing treatment to ensure the new crown will fit precisely next to the adjacent teeth. This is where skill is needed, says Valle. “It is an art. There must be precise shaping of the tooth for proper fit and function for the crown to be successful.” Then, with a digital wand—a hand piece equipped with a tiny camera—he scans your teeth and sends 3-dimensional images and measurements immediately to a lab via a computer. With this chairside digital oral scanner, “I can scan a tooth and in five minutes the lab I use on the West Coast can receive the information and begin making it immediately,” he explains. The advantages are many: “You don’t have a mess with impression material, the patient is more comfortable, and a scan is usually quicker and much more accurate. Since we use a lab out on the West Coast, traditionally the case would take three days to get there. Now, with the touch of a button, the lab receives the case in minutes.”


The artistry continues in the lab. Using sophisticated software and precise measurements, technicians manipulate 3-D imagery of the patient’s tooth and entire jaw, noting occlusion (bite), location, and the shade of tooth. They program a computerized milling machine to carve the crown from one block of material, usually a ceramic composite or zirconium, a high-grade porcelain. “In one step, they have everything they need to design the crown, for the right shape, size, color and bite,” says Valle. Once the tooth emerges, it is examined, hand sculpted to the doctor’s specific instructions and then painted by a technician who layers on shades of enamel using a very small brush. Staining, color and surface texture are applied to mimic the patient’s natural teeth. The finished masterpiece is mailed back to Dr. Valle, who cements the crown into place.

Many of these traditional instruments will become obsolete as new technologies evolve. Along with visual aids, dentists rely on digital imaging to match the color of a crown with adjacent teeth.

“Today’s digital tools and computer-operated milling machines dramatically improve the art and ensure a precision difficult to achieve using traditional methods,” says Valle, who opened his private practice in Millersville in 1989. Communication between the dentist and lab cannot be underestimated. That’s why Dr. Valle uses a lab in Seattle, Washington, which he visits often while helping other dentists advance their skills at the Kois Center, where he is a mentor. Meanwhile, he is constantly scanning the horizon for new and better technology. So, the next time you find yourself reclining in the dentist’s chair (invented in 1878) count yourself lucky—maybe, you get to experience and appreciate the “art of the crown.”

We use innovative treatments such as PROKERA®, which uses amniotic membrane tissue to heal damaged eye surfaces. PROKERA® is a biologic corneal bandage similar to a large contact lens. The lens is placed on the eye for only a few days. Once removed the patient will experience symptom relief and the cornea will be restored to a healthy state. The PROKERA® family of products are the only FDA cleared therapeutic devices that reduce inflammation and simultaneously promote regenerated healing of the ocular surface. Another breakthrough treatment that draws on special technology is LipiFlow®, which treats MGD using a Thermal Pulsation System. HORMONES By Meg Kabis, New Day Vitality Hormone Center Hormone replacement therapy (HRT) can be prescribed to women experiencing discomfort from menopause, such as hot flashes, fatigue and low libido. These days there are many types of HRT but questions persist about safety and options. Inaccurate reports from a large health study, the Women’s Health Initiative (WHI), conducted from 1995 to 2003, initially spread fear in women and physicians alike. However, those trials administered synthetic not bio-identical hormones. Upon further review, and in light of more current studies, medical consensus on hormone therapy is that it is, indeed, safe. “Experts do agree that the benefits of hormone therapy outweigh the risks for healthy women younger than age 60 or within 10 years of their last period,” according to Mayo Clinic: The Menopause Solution, published in 2016. In fact, a 2004 article in the Journal of the American Medical Association clarified that one arm of the WHI “definitively demonstrated that estrogen does not cause an increased risk for cancer. The increased risk was associated only with taking progestin—Provera® [a synthetic hormone], not estrogen.”

Unlike synthetic ones, bio-identical hormones duplicate the exact molecular structure of the human hormone. They also offer a longer lasting and safer delivery system. “Time-released pellet implants last about three months,” says Jennifer Musiol, C-RNP and medical director of New Day Vitality Hormone Center, an Arnold-based hormone replacement therapy center. Many physicians prescribe HRT with a one-size-fits-all approach: a daily dose taken by mouth, says Musiol. This is not the case with bioidentical hormone treatments. “Each woman experiences menopause in her own unique way. So, she should be treated in the same way,” she says. “Bio-identical hormone treatment is tailored to each woman’s unique needs and body chemistry.” Since medical experts say HRT benefits outweigh the risks for healthy women under 60, women have a real chance of improving their quality of life and their health through prescription HRT. With access to different types of hormones and hormone-delivery systems, it’s possible for women and their trusted healthcare professionals to map out a safe and successful plan for menopause symptom management.

Many medical journals now report the advantages of estrogen, like protecting the heart, brain and blood vessels, among other things, according to Biomedica (2000), a peer-reviewed research journal. A 2004 article in the New England Journal of Medicine states that “HRT provides significant coronary artery benefits.”

The year is 1973. War torn soldiers, after witnessing unspeakable horrors in Vietnam, return home to an indifferent, sometimes hostile America protesting U.S. involvement. Today, those politics don’t matter. But Vietnam veterans do—many hold on to trauma suppressed by an America that did not understand. Hospice of the Chesapeake volunteers, while tending to dying Vietnam veterans, know this. They hear the painful stories as they guide each person to a peaceful end. Silent for decades, only in their last hours can they break their selfimposed silence and share their grief. This revelation prompted Hospice of the Chesapeake Director of Volunteer Services Diane Sancilio to push to do more for veterans and help to organize an annual Welcome Home Vietnam Veterans Day Celebration for Marylanders. Now in its third year, the event builds on a vote by the U.S. Senate designating March 30, 2011 Welcome Home Vietnam Veterans Day, officially marking the 38th anniversary of the withdrawal of U.S. troops from Vietnam. In 2015, Maryland Senator John C. Astle (D-Anne Arundel) and Delegate C. T. Wilson (D-Charles) spearheaded a similar bill proclaiming March 30 a state holiday. The bill was signed into law by Maryland Governor Larry Hogan, who will be one of many distinguished speakers at this year’s event. Sen. Astle flew helicopters for the Marines during the war, winning two purple hearts. He returned to a different kind of battlefield. “Society did not want anything to do with us. I got spit on by a woman in L.A. at the airport. But I was proud of what I had done. I got back to West Virginia and suddenly friendships I had for years just ended. It wasn’t an easy time,” he says. He stayed on active duty, though, for five years. This gave him time to talk over his experiences with other soldiers. Many vets did not have that opportunity. He notes that unlike today, where U.S. forces generally leave and return as a unit, Vietnam soldiers left as individuals and returned home as individuals. Often cut off abruptly from deep friendships forged in combat, they did not receive psychological support and care when they needed it most. Welcome Back Vietnam Veterans Day is a way to reach out, at a fundamental level, to those veterans who are still alive and to make amends for their hostile reception, he explains. Russ Stewart, a Vietnam vet and longtime volunteer with Hospice of the Chesapeake, is grateful for a thank you, no longer how overdue.

A communications center specialist in the 586th Signal Company at An Khê (Camp Radcliff) Vietnam, he saw his fair share of combat. He recalls the long flight home and his disappointment upon landing. “The captain flew us over the Golden Gate Bridge and we all erupted into cheers and whoops as we flew by before tuning north to Ft. Lewis in Washington State. My thoughts were of relief and pride in accomplishment. This was short lived, however, by the reception of the public when I left Ft. Lewis to fly home. I clearly remember being spit at in the air terminal.” He says that Vietnam changed his life in good ways. It connected him to his father and other family members who fought in WWII. And, it made him a better person. “I came back from Vietnam more selfassured and accepting of responsibilities.” But people rarely praise or even notice Vietnam’s vets. “For years no one wanted to speak or acknowledge the war other than say Vietnam vets were ‘crazy,’ ‘baby killers,’ and I kept quiet. In the last six or eight years public attitude has been more accepting. I receive thanks from time to time when out wearing my Vietnam vet hat and it is appreciated.” For Stewart, Welcome Home Vietnam Veterans Day is very important. It is a “personal thank you from the public,” a milestone for Americans and for those who served,” he says. A renewed attention to Vietnam veterans can help forgotten heroes heal. It’s a tough world out there. Taking time out for ourselves is essential in this technologically fast-paced world. And with a bit of hard work, we will look good while we do it. Evolutions, an Annapolis-based fitness, health and wellness fixture for 15 years, was built to appeal to gentle souls who wish for physical and mental wellness. As a teenager in California Evolutions founder Erin Horst struggled with her weight. She experienced a eureka moment when she decided to quit dieting and just live her life. Miraculously, the weight started dropping. Her move to Atlanta proved to be pivotal. At an all-women’s gym she worked out with a personal trainer. She liked it so much that she became one herself. “I just loved it. It was a great environment. I was around people that I loved working with. I loved my clients.” The connection between work place harmony and personal satisfaction resounded with Horst. When she and her husband moved to Annapolis, she worked as a personal trainer at a Maryland-based fitness chain where the atmosphere was not positive. “I really felt unhappy in there because people who worked there complained about the management or just complained in general. I just decided to pull myself out of that environment and work at home.” Not intending to do anything more ambitious than train clients in her basement, she quickly changed her mind when her husband quit his job and started his own business, working from home too. “That was a little bit too much,” states Horst. In 2001, she opened her personal training and Pilates studio in a 2500-square-foot space on Housley Road. At this time, Pilates was creating seismic waves of interest in the fitness community and her studio was the only place in town to combine Pilates with personal training. “There was such a demand, quickly, for people not wanting to go to a personal training facility and then their gym to do their own workout, so through the clients pushing, pushing, pushing, we opened Evolutions as it is now.”

“People have worked here for a very long time, which has helped create a family and a special environment ... I built the business thinking of staff rather than customers, which is backwards for most businesses. I always thought that if my staff were happy, then the clients would be happy. As a former personal trainer, coming from different environments that I didn’t like, I kind of based my business on that in the beginning.” The journey hasn’t always been easy. The economic downturn in 2009–10 was a particular rough patch, with members dropping out or moving to the newly opened 24 Hour Fitness. Tough as it was, Horst weathered the storm and had an opportunity to take a good look at her business and evaluate what worked and what didn’t. “We started focusing much more on mindfulness and more than just your body, really trying to get that point across, that you can exercise till the cows come home, but if you’re not well inside, if you’re not well in your mind and your spirit, then it really doesn’t matter what your body looks like. Honestly, your body can’t be that healthy without those other two components.” At Evolutions, Horst encourages her clients to take time out from their fast-paced lives. “You can’t have mindfulness without a little quiet. It doesn’t have to mean a traditional meditation practice. It can be simply sitting quietly for five minutes and starting to just be aware of your mind, of the thoughts. However, I do believe that there has to be some silence in whatever practice you choose.” When asked about success, she is humble, “It’s intention … and gratitude. Most entrepreneurs don’t make it past one year. In 15 years, I have to count myself as successful for making it this long. When I mention deadlifts to someone his or her first reaction is fear. It has the word dead in it! Most people have no clue what a deadlift is. Those that do often say they avoid the lift because it will hurt their back. But what do you do if you drop your keys on the ground? You pick them up of course and that’s all a deadlift is. When programmed correctly and with proper form, deadlifts aren’t dangerous. Deadlifts done with poor form are. I have every one of my clients do this lift. A deadlift is simply picking something up correctly by learning how to “hinge” at the hips, brace the core and keep the item close to your midline. I’ve got a 3 and 1 year old, and with toys always strewn from one room to the next, I know a lot about picking things up. Bending over to pick something up is one of the most natural daily movements we do. Strengthening your hips, legs and back with the deadlift makes this everyday movement safer, helping you have a healthy and active life. Annapolis-based orthopedic surgeon and hip specialist Dr. Benjamin Petre agrees, “Strong hips are one of the keys to both movement and injury prevention. All limb control, positioning and performance begins at the origin of the limb, and for the leg this is the hips and core. Without good hip strength and stability, it won’t matter how good the remainder of the leg strength and stability is. Much of the power in the legs and even the arms comes from the hips and the core. Strong, healthy hips lead the way in an active lifestyle.”

As with every lift it’s best to work with a qualified coach to learn the movement. Proper technique is absolutely critical to ensure safety and effectiveness. I love the deadlift because it uses a ton of muscle, it’s relatively easy to learn, and because of how important strong hips, strong legs and a strong back are to all of life’s movements. TONS OF MUSCLE: Only the squat requires more muscle activation than the deadlift. It uses the muscles of the legs, hips, buttocks, lower back, abdomen, upper back and chest. This leads to major muscular and neuromuscular adaptations, increased insulin sensitivity, and healthy cardiovascular and metabolic changes. So if you want to lose fat, get lean and get strong, the deadlift is a great lift for you. IT’S EASY TO LEARN: The deadlift is not a complicated lift and with a good coach can usually be taught in less than hour. Few things are as natural as bending over and picking something up. Here is a very brief overview of the basics of a deadlift as taught by Mark Rippetoe in his seminal book Starting Strength: Basic Barbell Training: • Stand facing the bar, with feet hip width and barbell an inch or two away from shins over the middle of your feet (the top of your laces).


• Bend over at the hips, keeping knees straight and grab the bar just outside the feet. • Without moving the bar or lowering your hips, bring your knees forward until shins touch the bar, then raise your chest and tighten your entire body locking your spine into rigid extension. • This is your setup. The deadlift starts at the point of maximum tension—tension in the back, hamstrings, butt, arms, everything. • Once the setup is done you’ll take a deep breath and drag the bar up your legs as you stand up. At the top of the movement squeeze your glutes and fully extend the knees and hips. • Set the bar back down by reversing the movement without losing contact with the legs. The bar should end exactly where it started. That’s a deadlift. Anyone can do this lift at a level appropriate for his or her body. A lot of people who suffer from a sore back also spend

hours in poor positions (think sitting). When this becomes a daily routine the muscles in the back and hips get tight and weak. This is a perfect example of someone who should do the deadlift, not avoid it. The deadlift will strengthen the bones of your spine, hips and legs as well as all the muscles involved including the spinal erectors, hip extensors, glutes and hamstrings to name a few. It will increase your mobility and balance. Strong bones, strong muscles and our ability to produce and withstand force, like falling, will determine how we spend the last years of our lives. With my clients, my goal is to help them enjoy life right up until the end and to have the strength and mobility to do so. LG

Lindsay (from Severna Park) and Quinn (from Grasonville) first shared a friendship that would eventually blossom into much more. When planning their autumn nuptials a year in advance, several choices were pivotal in conveying the day’s spirit and ambiance. The venue selection of course. Also, engaging the services of planners Rachel Robinson and Jes Gignac of Intrinsic Events and selecting Intrigue Designs for the floral arrangements had a big impact. But Lindsay’s gown may have been the element that best encompassed the bride’s style and wit. She found her two-piece dress at a David Alexander trunk show held at Betsy Robinson’s Bridal Collection. “I didn’t want anything that I had seen before. My style is best described as ‘boho,’” she explains. “This was the first dress I tried on. It was so unique.” (The groom’s style was captured by Men’s Warehouse.) The headpiece, executed by Sarah Campbell of Intrigue, perfectly tied in the rustic and seasonal flavor of the day. Sherri Hoover of Body Wellness Salon and Spa fashioned the hairstyle, and makeup artist Amber Kwong created a natural, fresh look for a final touch. Guests feasted on a very extensive menu from Rouge Catering including passed hors d’oeuvres of petite lamb chops, sea scallops, and ahi tuna, an oyster bar, a plated dinner of beef tenderloin and Maryland lump crab cake, and desserts that included cupcakes, Berger Cookies, pumpkin cake, apple cider cake, mousse shots, and, of course, the wedding cake by Sugar Bakers. The bride (a pharmaceutical sales representative for AstraZeneca) and the groom (a full-time student going for his master’s in elementary education) shared one more magical moment when Quinn surprised Lindsay by coming out onto the dance floor at the reception in shoes that lit up the night! It looks like the whimsy, wit and joy will continue into their married life. LG
