Annapolis Doctor Braves Political Unrest to Train Doctors in Egypt Dr. Marcia V. Ormsby did not let recent revolution and political turmoil prevent her from doing what she is led to do: teaching the art and techniques of plastic/ cosmetic surgery to those in need. Last year, she hosted two Egyptian surgeons from a military hospital in Cairo, Egypt. They observed her at the Annapolis Aesthetic Surgery Center, which she founded in 1988. During their visit, the surgeons learned about specialized surgical techniques used by American doctors; one of the surgeons stayed six months. After the doctors returned to Cairo, the military hospital’s program director invited Dr. Ormsby to fly to Egypt and teach his team. She packed her lecture notes and instruments and, though concerned, made the journey last September. For two weeks, with military escorts and a translator, Dr. Ormsby traveled through the unstable capitol still ravaged by recent revolution. Though under heavy surveillance, Dr. Ormsby was well treated. The medical team made the most of her expertise. Sometimes she saw fifty patients a day, performing various surgical procedures and lecturing. “Little did I know that almost all my instruments would be needed as the ones provided were mostly inadequate for the surgeries I was expected to perform,” she reflects. She returned safely and with a much greater appreciation for American hospitals and with a respect for her hard-working Egyptian colleagues. It is not just her volunteer travels that set Dr. Ormsby apart. It is also her ability to see the beauty within every human being. This is no mere cliché. It is the philosophy that grounds Dr. Ormsby’s life’s work. Her goal is to help each patient reconnect with their own inner beauty and to reveal their own unique essence. “I love being part of their inner

and outer transformation. To see a person who may have been unhappy, open up and bloom like a rose is the best reward for my efforts,” reflects Dr. Ormsby. Dr. Ormsby has specialized in skin rejuvenation and surgery of the face, breast, and body for over twentyfive years. She is also a gardener and artist; her own paintings and drawings adorn her office along with other works of art. It is this innate sensitivity to form and proportion that impassioned her to specialize in cosmetic surgery, which she also believes is an art form. Dr. Ormsby’s approach to medicine includes a holistic perspective. She truly cares about each person’s well being. In fact, she often visits her patients at home the day after a surgery. “Yes, I make house calls. I visit to ensure my patients are doing well and are following instructions. Once I found a patient in bed with her puppy. I was concerned about infection. She followed me back to my office for a thorough cleaning and new garment,” she recalls. During every visit to her office, Dr. Ormsby meets her patients with enthusiasm and an attentive ear and offers extraordinary care. After all, this doctor truly cares.

12 LOOKING GOOD
Welcome to the February/March issue of Looking Good! Do you see that group picture right above? Together we put together our Cover and Inside Spread featuring “the little black dress.” We actually did this shoot at the Westfield Mall and found that this most conventional setting sparked an unprecedented wild time. Read on to see the drama that unfolds inside. Here you’ll find a visual narrative with a cinematic flair and many reincarnations of Coco Chanel’s ingenious invention in the 1920s, the practical Little Black Dress, that did not show stains and could be worn right into the evening. (No need to go home to change!) What the photo does not show, regrettably, is the little eighty-year-old lady in dark glasses who sat right down on a nearby sofa and observed for a good half hour our male model’s bared torso. Seriously, she could not take her eyes off of him. The real fun will come February 19th, when we celebrate the launch of this issue in support of the Wellness House. Indulge in a libation and show off your own little black dress and “grey tie.” We’ll all be there to welcome you and hope our eighty-year-old fan will show up as well! COVER TREND THE LITTLE BLACK DRESS In 1926, French fashion designer Coco Chanel introduced the now legendary Little Black Dress. A recent incarnation of the LBD appears on our cover and inside this issue. Part of Chanel’s revolutionary vision was to adopt elements of menswear into her designs, and to introduce comfort and freedom into women’s daily wardrobe. Today, almost ninety years later, it is still with us as is Coco Chanel. She has given us a garment each woman inhabits with her self. Black, in its fullness and its void, channels the deepest desires of the body and mind that it clothes.

Overcoming Emotional Eating
What Is Emotional Eating Anyway? Emotional eating refers to consuming food and beverages for reasons other than true hunger. Eating emotionally is usually a response to stress or is a way to avoid dealing with anxiety-provoking issues. Most people who are emotional eaters don’t eat the “good for you” stuff like fruits and vegetables. Instead, they turn to comfort foods, ones high in fats and sugar. As a result, emotional eating often leads to weight gain.
Why Are Some People Emotional Eaters? In some cases, emotional eating is a learned response. When the going gets tough, you reach for a cookie or a brownie instead of confronting the problem. Doing this makes you feel good, at least temporarily. You feel anxious or stressed, so you again snack on comfort food to avoid having to directly deal with something that’s bothering. There are physiological reasons why emotional eating is so rewarding. Foods high in carbohydrates boost levels of a brain
chemical called serotonin. Serotonin is linked with feelings of well being and comfort—just what you need when you’re feeling anxious. However, keep in mind that eating and snacking when you’re stressed can be a distraction, so you can delay having to deal with a problem or conflict. People have different ways of coping with stress. Some people go out and take a walk or shoot a game of basketball. Others turn to food to feel better. Unfortunately, eating in response to stress doesn’t solve the problem. It only creates a new one— weight gain.
Ways to Cope Journaling
To get a grip on emotional eating, you first have to be aware you’re doing it. Start by keeping a food journal for three weeks. In your journal, write down when you ate, what you ate, and how much.
Rate Your Hunger Level
Most important, before you devour the pint of Ben & Jerry’s, rate your level of hunger on a scale of one to five. Jot down how you felt at the time too. Were you feeling calm and happy or stressed and frazzled? Do this religiously and you’ll begin to notice patterns. If your rating is not at least three out of five on the hunger scale, do something else instead.
Adopt Simple Alternative Stress Relief Techniques
Deep breathing, visualization, self-hypnosis, and meditation can all help you deal with stress, anxiety, and uncertainty. Next time you’re tempted to have a second helping of dinner when you’re not hungry, take a short walk instead. If you have a dog, grab a leash and do both of you a favor. Walking is a healthy way to clear your head and one
that doesn’t involve food. When you repeat this behavior numerous times, it will become a habit. When the going gets tough, you’ll automatically take a walk or meditate rather than reach for a candy bar.
Get Plenty of Sleep
Make sure you’re getting at least seven hours of uninterrupted sleep. Skimping on sleep increases levels of a stress hormone called cortisol and cortisol drives cravings for sugary foods.
Seek Help If Necessary
If you can’t gain control over emotional eating by yourself, ask for help. A counselor can help you identify what’s triggering your emotional eating and teach you more constructive techniques for dealing with stress.
The Bottom Line You can break the cycle of emotional eating and find healthier and more constructive ways to deal with stress. When you do, you’ll be healthier and feel better about yourself.

Saying GOODBYE to the Knife Four of Today’s Most Effective Non-invasive Cosmetic Procedures
By Kymberly Taylor Have you ever had a friend tell you about their recent “injectables” or “fillers” and nodded, pretending to know what they were talking about? Or, consulted with an esthetician and plastic surgeon, only to stumble out of the office in a daze? I am not ashamed to admit that it took me months to figure out the difference between “fillers” and “injectables.” Folks “in the know” conversed in a private language, a language I had never heard of. The jargon can be intimidating, especially when you are trying to get straight answers.
28 LOOKING GOOD
For those suffering from jargon overdose, we summarize here, in a very general and non-technical fashion, today’s top treatments. Basically, these procedures do four things: lift and contour; fill and plump; erase wrinkles and frown lines temporarily; or renew, tighten, and refresh skin damaged by sun, age, acne, and scars. No single treatment can do all four things simultaneously. However, a skilled plastic surgeon may combine these procedures, producing what many are calling today “a non-surgical facelift.” The following terminology may help you have a more in-depth conversation with your practitioner. However, it is not meant for a self-diagnosis or a replacement for the most important thing: finding the right plastic surgeon or dermatologist for you, one who is highly skilled, educated, and an “injection expert.” Do your homework! Do not surrender your skin to an amateur. Then, let your doc work his or her magic. The best doctors do not follow formulas or even the
latest trends. They choose fillers and injectables as an artist would choose a brush. Each product or substance provides a different effect and is used in different areas for different reasons. REDUCE WRINKES WITH INJECTABLES Reduces lines caused by repetitive muscle motions, such as those on upper region of the face. See results in two to five days. Lasts for three to five months. In general, injectables are known as “neuromodulators” or “neurotoxins.” They cannot treat skin that is sun damaged or skin sagging due to gravity. Rather, they specifically treat lines caused by repetitive motion, such as those on your forehead, between your brows, and around the eyes (crow’s feet). When used in small amounts, neuromodulators reduce the activity of muscle contraction. Botox and Dysport are both FDA approved for cosmetic use.
February/March 2015
29
BOTOX: Botox, the brand name of a toxin produced by the bacterium Clostridium botulinum, blocks signals from the nerves to the muscles. The injected muscle can no longer contract, which causes the wrinkles to relax and soften. Many people worry because they are injecting poison into their system, the same poison that causes botulism. Keep in mind that the FDA approved its usage in the late 1980s upon the discovery that small amounts of Botox could stop ailments like blepharospasm (uncontrolled blinking) and strabismus (lazy eye). Botox was approved for cosmetic use in 2002. DYSPORT: Dysport is another brand name for a drug containing the Clostridium botulinum toxin. The jury is still out on the major differences between Dysport and Botox. Some claim that Dysport works faster and is less expensive. However, others claim that the cost difference is minimal. What doctors do agree upon, however, is that both Botox and Dysport do basically the same thing: paralyze the muscles under the skin for a short time, blocking the nerves and reducing the appearance of wrinkles. Check in with your doctor if you have more questions.
VOLUMIZE AND PLUMP UP SKIN WITH FILLERS Increases the volume of tissue underneath lines and folds, lasts between four and twelve months. Response time varies from instantaneous to several months. Fillers are used to increase the volume of tissue underneath lines and folds. Usually administered with a series of small needle pricks to the lower half of the face, fillers are commonly used on the creases that run from the nose to the mouth (nasolabial folds). They can also be used to enhance cheekbones, lips, and the area under the eyes. Fillers are used to treat lines that develop in tissues that are normally at rest, versus neuromodulators, which treat tissues controlled by muscle use. Results typically last between four and twelve months depending on the severity of the crease, the type of filler used, and area of injection. Keep in mind that different fillers are made and approved by the FDA to do different things in different areas of the face. Some wrinkle fillers plump and lift cheeks, jawlines, and temples; others are best at filling out thin lips and plumping sagging hands. The following are the most prevalent fillers, whose names are tossed casually around the waiting room. VOLUMA: Best known for adding volume to cheeks and the midface. Works instantly. The filler Voluma is currently called “the liquid face lift” and has generated quite a bit of buzz. It is believed to be the most sculptural filler on the market and used primarily to add volume to your cheek area. It is said to work instantly, and may give you contour and a subtle lift for up to two years with regular treatments. 30 LOOKING GOOD
SCULPTRA AESTHETIC: Best for lower face, temples, and chin. Results occur slowly, over time. May take up to three injections over several months. Sculptra works by replacing lost collagen and fills in shallow to deep facial folds and wrinkles. It is recommended that you have three injectable sessions over a few months period. Subtle results occur over time. Sculptra can be used in areas of the lower face, temples, and chin to produce collagen in a slow, natural appearing way. RESTYLANE®: Corrects smile lines and wrinkles around the mouth and plumps lips. One to four injections are recommended per year. PERLANE: Corrects moderate to severe facial folds and wrinkles, such as nasolabial folds. One to four injections are recommended per year. The fillers Restylane and Perlane are made by the same company. Perlane differs from Restylane primarily in the size of the particles and the depth of the required injection. Perlane, used for deeper wrinkles, has larger particles that are injected deeper into the skin than Restylane. Perlane is not approved for use in lip enhancement, but Restylane is approved for this procedure.
LIFTING AND TIGHTENING EYEBROWS, NECK, AND JOWELS WITH ULTHERAPY Sometimes called “the mini-lift.” Tightens and lifts skin under the eyes, neck, and chin using ultrasound technology. Does not affect outermost layer of the skin, so there is no peeling or scabbing. For most, full results are seen at around the sixth month. Administered with an ultrasound wand, Ultherapy harnesses heat produced by precisely targeted sonic waves to startle your skin’s collagen into rebuilding, which in turn tightens and lifts skin and underlying tissues. It can be somewhat painful and some swelling following treatment may occur. Most likely, your doctor will administer some kind of pain relief. As new collagen is produced you will see the tightening of the skin occur, which results in a natural lift of skin and tissue. The first FDA approval was granted in 2009 for a noninvasive eyebrow lift, followed by approval for lax tissue on the neck and submental (under chin) in 2012.
LIFT, FIRM, AND CORRECT SUN DAMAGE WITH FRACTIONAL LASER TREATMENT Corrects sun damage, fine lines, and scarring and improves skin appearance and texture using laser beams. Downtime can range from two days to two weeks, depending upon the type of laser technology and machine used. Patients can expect redness and numerous tiny scabs as skin peels. Lasers have come a long way and continue to be perfected. The two types of lasers most commonly used in laser resurfacing are carbon dioxide (CO2) and erbium. Each laser vaporizes skin cells damaged at the surface-level.
CO2 LASER RESURFACING: This method of fractional laser treatment has been used for years for different skin issues, including wrinkles, scars, warts, enlarged oil glands on the nose, and other conditions. The newest version of CO2 laser resurfacing (fractionated CO2) uses very short pulsed light energy (known as ultrapulse) or continuous light beams that are delivered in a scanning pattern to remove thin layers of skin with minimal heat damage. Recovery takes up to two weeks. ERBIUM LASER RESURFACING: Erbium laser resurfacing is gentler than CO2 lasers and designed to remove surface-level and moderately deep lines and wrinkles on the face, hands, neck, or chest. One of the benefits of erbium laser resurfacing is minimal burning of surrounding tissue. This laser causes fewer side effects—such as swelling, bruising, and redness—so your recovery time should be faster than with CO2 laser resurfacing. In some cases, recovery may only take one week. Ask your doctor how long recovery is likely to take for you. The different types of fractional resurfacing use different wavelengths of laser light to treat the skin. They are all similar. The major difference is between providers. Some practitioners are more aggressive, using higher settings and multiple passes over the face, producing more dramatic results, but also more redness and swelling. The best person to ask about how red you will be is your doctor, because it depends a lot on what type of treatment he or she has planned for you. If you still have trouble digesting the terms, technologies, and treatments, and even if you do not, keep things simple and safe: Consult a highly experienced licensed plastic surgeon or dermatologist and ask them to recommend a custom plan for you. After all, he or she has probably devoted over half of their life to researching and practicing cosmetic surgery and to keeping up with advances in the field. This is precisely why they are compensated so much and why they earn their money.

Before you begin your research, keep in mind that by the time you read this, the information may have changed yet again. Something old may come back into vogue. Something new may fall from favor. As technology advances, we are witnessing discoveries that are restoring youthful appearance and improving skin. It is safe to say this is only the beginning as the team of science and medicine is making it possible for everyone, if they wish, to improve his or her appearance by removing years of time from the face without undergoing invasive procedures. And, remember, you are not just choosing a doctor, but an artist who knows how to wield his or her brush. Take your time and be selective when choosing a doctor. After all, you are the painting. Since her rise to fame as creative director of the entire Alexander McQueen brand in 2010 and, in 2011, receiving global recognition for her design of Duchess of Cambridge Catherine Middleton’s wedding dress, the eyes of the fashion world have been on Sarah Burton. This year, Burton, who worked alongside Lee Alexander McQueen for fourteen years prior to his death in 1994, has come forth with a menswear collection able to do the almost impossible: break radically with McQueen’s signature look while at the same time paying it a subtle homage. For inspiration, Burton headed to the Far East, immersing herself in the fantastical yet disciplined Kabuki mask, with counterpoints of black and red, that she artfully redirected into abstract whirls of color. Her radical departure seems to rebuff completely last Fall’s nostalgic look, which many believe is vintage McQueen. Or, does it really? Gone was the McQueen’s familiar symmetry, colors in lockstep advancing across an immaculate premeditated field. Yet, what rises from this collection to make it stellar is the ingenious order that has always beat at the heart of the McQueen label, and of the UK itself. Yes, boldly patterned shirts and baggy shorts exuded a hip-hop defiance, but the default position of McQueen menswear shines through—an elongated, aristocratic elegance, exemplified here by long coats belted high across the chest. What is most exciting for the mercurial world of fashion design, where, in but a season, high-art can swiftly plummet to the depths of despair, is that, with this collection, Burton is able to push through the anxiety of influence hovering over all great artists, especially those with very recent masters.

FIVE REASONS YOU CAN’T AFFORD TO SCHEDULE EXERCISE OUT OF YOUR LIFE
By Wanda Marie
If you’re busier than ever these days, you’re admittedly not alone. Even so, a hectic schedule is not an excuse to cut workouts from your calendar. In fact, evidence suggests you might be shooting yourself in the foot by eliminating your trips to the gym. These are just some of the reasons why you need to keep workouts on your plate.
Regular exercise extends your life expectancy.
According to the American Heart Association, for each hour of exercise you get, you can gain two hours of life expectancy. This phenomenon is due largely to the numerous physiological effects exercise has, such as keeping cholesterol and stress hormones under control. You’ll get back the time you put into your workouts and then some if you commit.
You will be more productive.
As Forbes contributor Jennifer Cohen describes, numerous studies have tied exercise to better cognitive status and results. The endorphins (often referred to as feel-good chemicals) released when you exercise rev your memory into high gear, for example, and focus can be elevated for up to two hours after a workout session. In fact, these results are significant enough that researchers have pointed toward regular physical exercise as a drug-alternative treatment for attention deficit hyperactivity disorder (ADHD), as James Hamblin of The Atlantic reports. The combination of good concentration and information retention can mean you end up working more efficiently, which can translate into more free time for what you truly enjoy. 38 LOOKING GOOD
You won’t just live life—you’ll actually enjoy it. Endorphins produced when you exercise don’t just elevate your focus and memory. They also affect your mood, reducing anxiety and stress. In particular, levels of oxytocin, the so-called “cuddle” or “love” hormone, tend to go up. Researchers point to this hormone as being especially critical for social well being, connecting it to the ability to trust and connect with other people.
Your energy will improve and your sex life.
Many people dismiss exercise because they’re so tired from meeting all the legitimate responsibilities they have. As the Mayo Clinic asserts, however, exercise results in high levels of oxygen and nutrients being delivered to your tissues, improving your cardiovascular efficiency. It also is connected to falling asleep quickly and entering a deeper sleep. The result often is a boost in energy, which can affect everything from your work to the pleasure you get during sexual encounters.
You’ll have another reason to be more confident. Exercise is always a journey even for professionals, the entire idea being to continually challenge yourself. With this in mind, maybe you aren’t ready to thrust an Olympic bar laden with plates over your head and do a full squat, but you can bet your getting-in-shape booty you can benefit from the five non-weighted half squats you just tried out and survived. It’s more than possible to take the “I did it” from your workouts, even as a beginner, and translate that into an “I can” for any other area of your life. Not only that, but the compliments you’ll likely get about your improved energy, productivity, mood, and physique can do wonders for your self-esteem, too. The modern world makes it easy to write exercise out of daily routines, but the reality is that regular physical activity delivers fabulous returns. If you have been putting off getting into a program, now is the moment to take charge.

Summer Wedding in Full Bloom Darrien & Jennifer Bean | June 21, 2014 Darrien and Jennifer Bean joined their lives together on June 21, 2014, surrounded by friends, wild grasses, and the blooms of summer. They met two years earlier on Plenty of Fish, an online dating site. It seems fitting that the couple chose to get married on the Eastern Shore under a simple, bold sky and arbor festooned with long-stemmed red roses. Their courtship, which lasted one year and 11 months, was filled with peaceful moments, casual moments, and passion. Jen, 31, a speech pathologist from Glen Burnie and Darrien, 30, a computer engineer living in Severn, thought hard before committing to a specific theme or color scheme. They wanted their wedding to express their personalities and it did. The couple rejected the pastel palette so common in warm-weather nuptials. “We wanted our wedding to be elegant with strong colors, so we chose black, red, and silver despite 42 LOOKING GOOD

discouragement for those not being “summer colors,” they recall. And bold it was, featuring the classic color of true love: red. The bride’s hair flowed; she wore a diamond necklace; a gold and gem-encrusted belt circled her waist. She carried deep red roses and red hibiscus; the bridesmaids’ brilliant red gowns were accentuated by white bouquets. On the tables, red orchids, snapdragon, and snow white hydrangeas cascaded from extra tall slender vases. Even the cake was especially romantic. The usual stiff couple that is usually at the tip top was replaced by a groom swooping up his bride. The best moment in one’s wedding is always hard to pinpoint. But, for Jen, it was easy: “The moment I said, ‘I do,’ and sealed it with a kiss. Then, it was party time!” she recalls. And, on this quintessential summer day, with flowers in full bloom, the bride of course got her way.

Reflections o n Tu r n i n g B a c k t h e C l o c k By Mary Ann Treger
Aging gracefully is a noble goal but one that isn’t coming easy to this sixty-ish baby boomer. If only my brain would age in sync with my body, I wouldn’t register shock every time I look in the mirror and catch the reflection of a stranger. I thought when I was old enough to collect Social Security, I’d be at peace with wrinkles and jowls, but I’m not. Call me superficial. Call me insecure. Call me honest. Fifteen years ago I had a facelift that turned back the clock by ten years. Before going “under the knife” I met with several plastic surgeons but many of their recommendations were so extreme I feared I’d wind up with a permanent walkingthrough-a-wind-tunnel look and never look like “me.” I chose Dr. Fleury in Chevy Chase, a doctor with a conservative approach. But the minute my stitches healed, Mother Nature weighed in and the aging process started all over again. Now, over a decade later, I need a tweaking. While I love my original doctor, like me, he got old. I wanted a younger version, someone up to speed on the latest techniques. I asked my in-theknow Annapolis friends for references from Baltimore to DC and then, as with my facelift, I picked a few to interview. Before zeroing in on one doctor, I always tell my friends to meet with at least three. And, don’t be afraid to ask questions. A lot of questions. I’ll never know why women ask more probing questions of potential housekeepers who may scrub their floors than doctors who may operate on their face. To me, no question is off the table. Have you ever had any serious complications? And, one touchy question: Have you ever been sued by unhappy patients? I scrutinize the doctor’s medical training, too. What medical school did you attend and where did you complete your residency? Do you have experience in general surgery, otolaryngology, head, and neck surgery? Are you a member of any significant medical organizations? I always save my favorite question for last: If I were your mother (or sister, depending on the age of the doctor) what would you do to make me look younger? Fortunately our region is a mecca for experienced doctors who are erasing years from women (and men!) via surgery, lasers, or injectables. While there are many good doctors to choose from, the one important thing that can’t be defined by research and medical schools is chemistry. Am I comfortable with this doctor? Is this doctor listening, really listening or placating me? Do I feel a sense of trust?
I check out the vibes coming from the office staff, too. Do they seem proud to be working for this doctor? Are they working well as a team? Do they treat everyone with a warm, understanding attitude? A chilly front-desk administrator can tell you a lot about how a place is run. One doctor I visited and felt chemistry with was Dr. Sandel in Annapolis. “I need a tweaking,” I blurt out while yanking at the excess skin on my neck. “Your neck isn’t bad enough for surgery,” he replied. Huh? A surgeon not recommending surgery, I think to myself. Instead, he came up with a three-part, non-surgical plan: an injectable filler to fill out the hollowness in my cheeks, Botox above my brow for a mini brow lift, and a laser treatment to tighten my neck. While I’ve used Botox for years, I’ve been nervous about using a lot of fillers. I’ve seen too many women overdo them and wind up looking like blowfish. Because of the trust I felt, I gave Dr. Sandel the green light. With a few easy pricks, several years were erased. I don’t look “done.” Now, months later I’m still happy with my new, younger me. This approach is not unusual. These days, more and more women are choosing fillers versus going under the knife. While surgery is often still necessary to achieve the desired results, non-invasive anti-aging procedures are in vogue for good reason. They are quick, easy, and usually provide immediate results. And, there are so many fillers now on the market that target different areas of the face or different needs. As for “Botox parties,” where the lure is to sip complimentary wine and chat with friends while getting a discounted injection, I avoid them. While I love getting a bargain when shopping for cars or clothes, when someone is about to approach my face with a needle, I couldn’t care less about a free glass of Cabernet. Aging is an interesting journey for all of us. Even though I’ve embraced the latest anti-aging opportunities from surgery to lasers to injectables, I still believe in aging gracefully. As far as I’m concerned, these techniques are simply good grooming, no different from brushing my teeth. The bottom line is, all I want to do is look my best—for my age—and that is why I’ll keep pursuing today’s smorgasbord of options to turn back that clock, tastefully, of course.
