Iswear my knees didn’t used to creak when I got out of bed, and I’m pretty sure this morning I spotted five grey hairs instead of the resident four I had become accustomed to. The signs of aging, day by day, are getting harder to ignore, and I’m trying desperately to embrace them. Well, I’m trying desperately to ignore them but when that doesn’t work, embracing them seems the next best step.

NORA HESTON TARTEPlush, pillowy lips are all the rage in the beauty industry right now. While there’s no pressure to hop on the trend, if you’re curious about giving your pout a little more oomph,
it’s important to be informed about your options. And perhaps the biggest benefit to a procedure becoming trendy, is that your options suddenly become more robust.
First, manage expectations. In general
lip injections provide a bee-stung aesthetic for six to twelve months, with waning results over time. Semi-permanent injections can last up to two years. After that, you’ll need a fresh round of injections if you want to maintain the look.
Collagen fillers once reigned supreme, but their popularity has dipped. The look of HA fillers (below) have a reputation for being more natural looking, but collagen fillers last longer.
HYALURONIC ACID fillers are chosen by many because they are made of a substance that occurs naturally in the body. Well-known brands Restylane and Juvéderm both make HA fillers that trap water in the lips giving them a natural, plumped look. The use of ultra-fine needles also makes these injections precise, helping the surgeon manage results, including bigger lips and a reduction in wrinkles around the mouth. Surgeons can even shape the products using their hands. Overall, results may take a few days to show, and bruising, bleeding, and allergic reactions can occur. In general, you’ll need a refresh every six to twelve months.
FAT TRANSFER lip injections use fat from the abdomen, butt, or thighs. The fat is removed from the patient, treated, and then inserted into the lips. Size, fullness, texture, and symmetry can all be increased with this procedure, and in general the results last longer than dermal fillers. Like fillers, fat transfers can also reduce the appearance of fine lines and wrinkles around the mouth. Swelling, bruising, stiffness, and chapped lips, are all possible for the weeks following the procedure, and some may experience a temporary overdone look. In general, it can take a few procedures to get this one just right as the injectables are less precise than dermal fillers.
PERMANENT SILICONE LIP FILLERS offer one major advantage: no maintenance. If needed, the silicone implants can be removed. The downside is that some patients complain their lips do not feel as natural.
Protecting the Mouth from Gum Disease
STAY HEALTHY, BRUSH OFTEN
BY JESSICA ZIMMERGum disease, also known as periodontitis, is an infection of the soft gum tissue that surrounds the teeth. When left untreated, it can lead to pain, tooth loss, and further infection of the mouth. Generally, a patient can prevent gum disease by brushing and flossing after meals. Getting regular dental checkups and professional teeth cleanings also helps.
Signs of gum disease include swollen gums, gums that bleed easily, bad breath, loose teeth, gum recession (meaning the gums pulling away from the teeth), cavities, and a change in bite. Gum disease is worsened by genetics, smoking cigarettes, using chewing tobacco, alcohol and substance use, poor nutrition (especially a vitamin C deficiency), obesity, and hormonal changes like menopause. Gum disease usually begins with the development of plaque, a sticky, almost-invisible film

This method of self-care can be problematic for people who grind their teeth because chewing gum requires constant movement of the jaw and puts pressure on the teeth and these parts of the mouth may already be experiencing pain due to stress-related grinding.
Another method of preventing gum disease is brushing with prescription-strength fluoride toothpastes like Prevident. The best way, however, for a patient to determine which options will work for them is to consult a dentist.

Plaque that accumulates on the teeth and gums and hardens turns into tartar. Tartar can form quickly, within one or more days of failing to brush and floss. Tartar is composed of bacteria, calcium compounds, and other organic matter in the mouth. Tartar can range in color, from white and yellow to brown and black. A dentist or dental hygienist can remove tartar with scraping and ultrasonic tools.
Good gum health can help to prevent expensive dental, periodontal, and orthodontic care, such as gum grafts, crowns, veneers, and tooth removal. A person can improve gum health by chewing sugar-free gum and using alcohol-free mouthwash after eating. These behaviors change the pH level of the mouth, discouraging plaque from forming. Chewing

Let’s face it—if we don’t want the signs of aging to appear on our face, we’re going to need to do something about it. While there is nothing wrong with aging and it isn’t something we should shy away from, there are ways to do it more gracefully. So, if you want to look young for as long as possible without going under the knife, follow these tips from local dermatologist offices.
PREVENTION
Part of maintaining a youthful appearance is taking care of your skin. This includes plenty of sun protection. Always wear sunscreen and avoid harmful UV rays as much as possible. Hats, sunglasses, and other accessories can help when you’ll be outside during peak sunshine hours.
AT-HOME CARE
“For some individuals the use of products or facials with ingredients such as vitamin C, hyaluronic acid, glycolic acid, alpha hydroxy acid, and peptide serums can be beneficial,” says Nicole James from LUX Dermatology. Although, she does warn consulting a dermatologist is the best first step as no product is without risk.
Nicole also recommends topical retinoid medications, available both as prescriptions and over the counter, to slow the wrinkling process.
Avis Chiu, PA-C, from Golden State Dermatology says a daily routine focused on anti-aging products is important. She suggests using vitamins C and E, as well as ferulic serum in the morning and retinol in the evenings, in addition to daily sunscreen.
IN-OFFICE TREATMENTS
For a little extra help, seek the care of an esthetician or dermatologist. Hydrafacials and microneedling every six to eight weeks will help maintain your skin’s elasticity and fight wrinkling. Needle-less treatments using a laser can also help.
WHAT ABOUT BOTOX AND FILLERS?
If you want a little more without going under the knife, Botox and fillers are two options that can be preventative and/or transformative. Patients as young as 18 walk into Golden State Dermatology for preventative Botox. For the best results, Dr. Chiu recommends getting injections every three to four months while Nicole at LUX says most injections last six to nine months.

Take a Shot at Spring Allergies
IMMUNOLOGY TREATMENTS TO TREAT SEVERE SYMPTOMS
BY NORA HESTON TARTEAstaggering 81 million people in the United States were diagnosed with seasonal allergic rhinitis, or hay fever, in 2021. That equates to 26 percent of the adult population and 19 percent of children, according to the Asthma and Allergy Foundation of America. For most, over-thecounter remedies and even prescription medicines do the trick, however, some Americans simply need more.
While most allergy medicines address symptoms, immunology shots get to the root cause of allergies, effectively reducing the severity of reactions through a type of exposure therapy. Essentially, immunology shots contain small amounts of what a person is allergic to, usually pollen from trees, grasses, and weeds. By exposing the immune system to small doses of the culprits, it learns to react less aggressively to the stimulants.
While allergy shots are readily available (Allergy Immunology & Asthma Medical Group has eight locations through San Joa-
quin County and beyond), they are not for everyone. You may be a good candidate if your allergy symptoms are severe enough to interfere with daily life and over-the-counter remedies, prescription medicines, and a change in environment are not enough to relieve them.
If other remedies have failed you, and your reactions are severe (an aggressive runny or stuffy nose or allergy-induced asthma), then consult a professional about establishing a care plan that may include in-office allergy shots, also referred to as allergen immunotherapy injections.
The other candidate for these shots include those who experience life-threatening reactions to insect stings. According to professionals at Allergy and Immunology, most people will experience swelling, pain, redness, and even warmth at the site of
the sting for up to two days after an insect sting, but those that have hives, itching and swelling in areas other than the sting site, tightness in the chest, difficulty breathing, hoarse voice, swelling of the tongue, dizziness, and/or loss of consciousness due to a drop in blood pressure, are classified as severe reactions. And while less than 5% of people have this type of reaction, those who have it once have a 70% chance of experiencing it again.
For these types of reactions, it’s important to carry injectable epinephrine and antihistamine, but you may be able to reduce reactions by also exploring venom immunotherapy.

STRETCH It Out
IT’S GOOD FOR YOUR HEALTH
BY NORA HESTON TARTEStretching has a lot of benefits, and it can vary in intensity. Overall, it keeps muscles flexible, strong, and healthy. Sometimes, stretching is the majority of the workout (think yoga classes focused on holding poses for prolonged periods of time), and if you’re recovering from an illness or injury, it can be the first exercise you’re allowed to do when getting back into a routine.
Stretching is important because, regardless of age, it can improve flexibility and range of motion, which leads to improved physical performance in other areas of life and reduces risk of injury. While a well-rounded workout routine is most beneficial, including cardio, strength training, and stretching, this is undeniably an important part of any regimen.
On a more cellular level, stretching also helps to improve blood circulation and alleviate sore or tight muscles. If you stretch daily, you’ll quickly notice a difference in your daily movements both functional and athletic.
The American College of Sports Medicine specifically recommends static stretching and an active warm up two to three times each week. Each stretch should be held for 15-30 seconds and repeated two to four times.
If you’re looking to lose weight, stretching is more of a positive component to a more robust exercise regimen because stretching itself only burns a small number of calories. However, when you work stretching into other workouts such as yoga and pilates, you will quickly see an increase that can reach up to several hun-
dred calories burned in just one hour.
While it’s important to stretch often, stretching before or after a workout is most important. Stretching before an intense workout such as a run or weight lifting will warm up your muscles and make them more pliable. This, in turn, reduces your risk for injury. After a workout, stretching creates an opportunity for the body to cool down and for your heartbeat to return to its resting rate.
Scientifically, when you work out, lactic acid is released, and stretching stops lactic acid from building up, allowing for recovery and repair of the whole body. You will also notice less soreness if you stretch after a workout and, because muscles loosen during stretching, your risk of injury again is reduced.

BY NORA HESTON TARTESometimes taking care of your skin isn’t about putting creams and serums on your face; and even more important than a monthly facial is an annual skin exam.
The American Academy of Dermatology recommends that everyone get a full-body skin exam every year, and for those that have a history of skin cancer, screenings may be needed more often than that. During skin exams, dermatologists use tools to inspect marks on your skin from moles to dark spots. From this exam they are able to tell if any contain potentially cancerous properties or could be of concern later on.
“You should make an appointment if you notice any changes in your skin, such as a new mole or a mole changing in size, shape, or color,” says Jack Sharkey, PA-C, MMs, who works in the Lodi clinic of Golden State Dermatology.
Three types of appointments exist. The first is a visual skin exam, a good starting point. During the exam, a clinician looks for any suspicious moles or lesions on your skin. A dermoscopy is a magnified exam of the skin and can help to identify moles or lesions that are likely to be cancerous. Lastly, when needed, a biopsy is performed by removing a small piece of skin and examining it under a microscope to identify cancer
definitively.
While moles should always be checked, there are some signs that may more likely indicate cancer. These include spots that are bleeding or oozing and have not healed in two weeks. The other is moles that are changing over time (which is one reason self-checks are important, too).

Heal Your Skin From Within BEAUTIFUL INSIDE
AND OUT
BY NORA HESTON TARTESkin that looks beautiful on the outside is also healthy on the inside. While makeup may cover blemishes and hide uneven skin tones, it’s how you take care of your skin when the makeup is off that creates that under-glow. Keeping your skin healthy requires daily, weekly, and monthly care. If you wear makeup, be sure to start with makeup remover (and not a makeup wipe). Micellar water is one option that is affordable and accessible to most. Next, a daily (or twice daily) cleanser is a good second step. Choose something with gentle ingredients that will not irritate your skin and avoid daily use of an exfoliant. Lastly, moisturize. Keeping your skin hydrated reduces the appearance of fine lines and wrinkles and combats crepe-y looking skin. Always use an eye cream, as well. These three steps are a basic routine that can be repeated twice per day (morning and night). Adding in additional ingredients, however,
will give you more bang for your buck. Just don’t overdo it.
Try daily serums. Those rich in vitamin C will have a brightening effect, fighting signs of aging and diminishing dark spots. At night, choose a serum rich in hyaluronic acid to hydrate the skin and provide anti-aging properties. If you struggle with dry skin, add a hydrating spray into the routine and rub it in with your moisturizer.
Remember, it can take several weeks for results to show, but consistency is key. Using the same products and sticking to your routine allows skin to adjust and see its full potential. Weekly you may add in a few more products that are not suitable for daily use such as a sheet mask, mud mask, or exfoliant. It is best to discuss your concerns and products with an esthetician or other skin professional. Then, schedule monthly facials to back up the work you’re doing at home. These facials can include
anything from hydrotherapy to oxygenation. Switch up your monthly facials and experiment with new methods to learn what your skin responds to best, and, if your medi-spa offers it, dermaplaning is a great way to remove dead skin cells and unwanted peach fuzz.
HORMONE REPLACEMENT THERAPY AND
OTHER TREATMENTS
BY JESSICA ZIMMERHormone replacement therapy (HRT) is one method to treat negative symptoms associated with perimenopause, the beginning of the body’s transition to menopause, as well as menopause, the point at which the ovaries stop the production of estrogen. Utilizing HRT, especially for long periods of time, however, has risks, including strokes, heart attacks, blood clots, breast cancer, gallbladder disease, and dementia. These risks increase as patients age. HRT is not advisable for patients with certain chronic health conditions, including breast or uterine cancer. For these reasons, a patient should consult with a primary healthcare provider before starting HRT.
Symptoms of menopause include hot flashes, night sweats, brain fog, and disturbed sleep, as well as anxiety and depression related to these concerns. Other issues include joint pain, loss of bladder control, and vaginal dryness. In the past, HRT involved taking estrogen, a female sex hormone, and progesterone, aka the “pregnancy hormone,” which is also produced by male adrenal glands. Today, HRT may also include testosterone, which is produced by testicles or ovaries, to address low energy, low sexual drive, and brain fog.
A patient choosing HRT should use the lowest effective dose for the shortest time possible, and always under the care of a physician. Many changes associated with perimenopause and menopause can be addressed with lifestyle changes. These include avoiding alcohol and nicotine, which can worsen symptoms, layering blankets to allow the removal of covers during night sweats, and using lubricants to reduce vaginal dryness. Selective serotonin reuptake inhibitors (SSRIs) like Lexapro can help
alleviate anxiety, depression, and hot flashes. Non-medicinal therapeutic treatments like relaxation exercises and cognitive behavioral therapy (CBT) also benefit some.
HRT can reduce the severity of symptoms noted above. It may help improve bone strength and overall cardiovascular health, as well. Negative side effects of HRT include nausea, headaches, mood swings, acne, breast tenderness, and fluid retention. People using HRT to address menopause are usually encouraged to stop using it within 10 years.
A patient should work with their primary care physician and at times, their OB/GYN, to assess whether HRT is meeting their needs. It is important that a patient’s HRT prescriptions do not interact negatively with other prescriptions. Trans men, non-binary, and gender non-conforming people who were assigned female at birth may benefit from HRT. Trans women who lower their estrogen dose or stop transition-related HRT as they age can expect symptoms such as joint pain. These concerns may be mitigated if the organs present at birth continue to produce testosterone.
HRT comes in many forms, including pills, skin patches, sprays, gels, creams, suppositories, vaginal rings, and vaginal preparations. The two basic types of medication are systemic hormone therapy and low-dose vaginal products. Systemic hormone therapy comes in all forms except vaginal preparations. This type of treatment provides a high dose of estrogen, which is absorbed throughout the body. Lowdose vaginal products typically come in creams, pills, or vaginal rings. This type of treatment minimizes the amount of estrogen absorbed by the body while low-dose vaginal products primarily treat vaginal and urinary symptoms.
Treating Bruxism Get off the grind
BY JESSICA ZIMMERBruxism, the involuntary and habitual clenching and grinding of teeth, can lead to many medical problems, including muscle pain around the head and neck, headaches, temporomandibular joint disorder (TMJ), gum recession, and fractures in teeth and dental restorations like crowns. Reducing bruxism means reducing stress, says Dr. Maywell Inong, DDS, based in Stockton. “Because clenching and grinding can happen during the day or at night, some are unaware of their condition and the issues it creates,” says Inong.
Treating the condition can involve behavior management to reduce stress, as well as medication to relax muscles, says Dr. Manjeet Samra, DDS, also based in Stockton. One of the typical ways to reduce tooth wear is a bite guard at night, which you can get from an orthodontist with a 3D printer to make a custom guard in-house. Otherwise, the orthodontist’s
office sends the work to a lab.
According to Samra, an orthodontist can correct a bite with braces or Invisalign (transparent teeth aligners). Additional orthodontic treatment is also an option, but its effects on bruxism are not clear and most dental insurance plans cover only a portion of orthodontic treatment.
Young children often suffer from bruxism as well, however, the behavior may wane as they get older. If a child grinds their teeth, a parent should seek treatment to limit gum recession and prevent damage to the child’s adult teeth.
Bruxism tends to run in families. It is a good idea for a person suffering from bruxism to share their family history with their dentist and orthodontist. The use of certain antidepressants, smoking cigarettes, drinking caffeinated beverages or alcohol, and using recreational drugs can all increase the risk of bruxism.
A person who grinds their teeth often
should schedule a consultation with an orthodontist to hear about different options. A consultation usually involves taking pictures and x-rays to evaluate different courses of actions.
A person suffering from bruxism may have other sleep disorders as well, such as snoring and pauses in breathing, called sleep apnea. If you suspect one of these may be at play, consult a physician.

WHERE WE STAND WITH COVID-19
BY JESSICA ZIMMERAs of spring 2023, there have been considerable changes regarding the COVID-19 pandemic. In February 2023, California Governor Gavin Newsom declared the statewide COVID-19 state of emergency over; the frequency of new COVID-19 cases and deaths is much lower than it was in winter 2021-2022; children are not required to get the COVID-19 vaccine to attend school; and California students have not been required to wear masks in schools since March 2022, however California K-12 school or childcare facilities must allow any person to wear a mask if they choose to do so.
When it comes to where and when to wear a mask, the federal government dropped the requirement to wear masks on public transportation in April 2022.
The California Department of Public Health (CDPH), however, still encourages wearing a mask in indoor areas and while taking public transportation. CDPH recommends N95, KN95, and KN94 masks in particular.
Long gone are the complicated colorcoded systems that indicated risk. Instead, California counties, including San Joaquin County, established COVID-19 community levels (CCLs) like low, medium, and high regarding COVID-19 transmission. CCLs are based on hospitalization rates, hospital bed occupancy, and COVID-19 incidence during the preceding period of evaluation. At all CCLs, CDPH strongly recommends all persons stay up to date with COVID-19 vaccination, including all primary series doses and boosters.

1. People with prediabetes and diabetes may experience more severe and long-lasting effects from catching COVID.
2. Catching COVID-19 is likely to have more serious consequences for a person with liver or kidney problems. COVID-19 can increase the risk of blood clots, which makes dialysis difficult.
3. Smoking cigarettes or using alcohol worsens the symptoms of COVID-19 infection. Both can weaken the activity of the immune system proteins that aid in recovery.
4. Improving respiratory health assists the body in recovering from COVID-19. Strive to avoid indoor and outdoor air pollutants, get regular exercise, and schedule annual checkups.
5. Certain symptoms help to distinguish COVID-19 from seasonal allergies. These include fever, chills, muscle and body aches, nausea, and diarrhea. Symptoms of seasonal allergies that may also be indicators of COVID-19, another type of coronavirus, or the flu include runny nose, headaches, itchy eyes, and fatigue.

CBD VERSUS THC

It’s been nearly seven years since marijuana was legalized for recreational use in the state of California, yet many still seem mystified by its usage. Whether you’re weed-curious or a casual user looking to learn more, we’ve got the basics.
Q: ARE CBD AND THC LEGAL?
A: In 2016 the recreational use of marijuana was legalized in California. Technically it is still illegal for recreational use at the federal level, which is where some of the confusion comes in. Federally, however, CBD derived from hemp plants is legal, but is not legalized in all states.
Q: WHAT IS THE DIFFERENCE BETWEEN CBD AND THC?
A: Both CBD and THC are cannabinoids found in cannabis plants. They are often used to treat the same issues or illnesses but the biggest difference is that THC causes a person to feel high while CBD does not. Many associate CBD more with a body high, or extreme state of relaxation, versus a head high like THC.
Q: WHY USE CBD?
A: CBD can provide relief from physical and mental issues, including pain, nausea, anxiety, and depression. Of course, you should consult a doctor before using it to treat any diagnosed illnesses and learn how it may work with other medications or in their place. The proof, however, is mostly circumstantial. Scientific evidence doesn’t support CBD as a treatment for all of these ailments, but public opinion seems to.
Q: WHY USE THC?
A: THC treats many of the same ailments as CBD, plus others. In general, those who are looking for pain relief, improved sleep, reduced nausea and vomiting, improved appetite, and anxiety relief (although too much
THC can have the reverse effect), may benefit from THC. THC will provide the classic high feeling while also relieving these symptoms.
Q: WHAT ARE THE DIFFERENT TYPES OF THC?
A: THC can come from sativa or indica and deciding which is right for you depends on what you’re looking to get out of the experience. Sativa is associated with a head high that many use to reduce stress and anxiety while also increasing creativity and focus. Indica on the other hand provides full body sensations promoting deep relaxation and aiding in quality sleep. A hybrid is a mix of the two.
SAN JOAQUIN GENERAL HOSPITAL IS THE REGION’S ONLY DESIGNATED TRAUMA CENTER

San Joaquin General Hospital provides the only designated and verified level II Trauma Center in the county and surrounding area. As a Level II Trauma Center, the specially trained team of experts are ready to provide optimal care for the injured patient 24-hours a day.
There are five levels of trauma centers in the country ranging from a basic emergency room (level five) to a hospital that can provide complete care for every injury (level one), and SJGH has been a level two center since 2021. A level two trauma center is able to provide care for all injured patients.
As a level two trauma center, the team consists of dedicated emergency physicians,
trauma surgeons, and specially trained trauma nurses, intensive care nurses, emergency department nurses, and respiratory and radiology technicians. Surgical specialists in trauma, neurosurgery, vascular, thoracic, anesthesia, and orthopedic surgery are on-call at all times.
The main difference between a level one and level two trauma center is the hospital’s ability to care for the full scope of an injury from evaluation to rehabilitation. At a level two trauma center, the initial injury can be cared for, but there may be a need to send patients outside of the center for aftercare.
Prior to earning its level two distinction, SJGH was a level three trauma center, but
continued advancements and emphasis on emergency care and increasing staff.
SJGH has an overall survival rate of 96 percent and all staffed trauma surgeons are double board certified in general surgery and surgical critical care. Having a trauma center nearby should ease patient’s worries and provide peace of mind should the worst happen. Nationally, trauma is the number one killer for people ages 1-46, with a secondary peak after the age of 65. While the trauma center sees a variety of different patients each year, over 3,600 are trauma patients.

Medical Group welcomes Dr. Jeffrey Balduzzi to their practice. Dr. Jeffrey Balduzzi obtained his Medical Degree from the University of Virginia School of Medicine, in Charlottesville, VA. He then went on to complete residency training in Internal Medicine at NYU-Winthrop Hospital in Long Island, NY and subsequently Allergy and Immunology Fellowship at Wake Forest Baptist Medical Center in Winston-Salem, NC.
Dr. Balduzzi is Board Certified in Internal Medicine, as well as a BC/BE trained specialist in Adult and Pediatric Allergy, Asthma
and Immunologic Disorders. He is an active member of AAAAI, ACAAI and WAO. Dr. Balduzzi is an avid fan the Buffalo Bills, and follows both professional and colle giate sports actively.
