STAY HEALTHY, SAN JOAQUIN!
If anything has been thrust into the spotlight this year it’s the healthcare industry. From every aspect we’re learning how to stay healthy, as well as how underlying illnesses, obesity, and other health factors can affect us in ways never imagined. In a way, it’s more important to be healthy now than ever before. We as a publication are invested in the health of our readers and the entire San Joaquin County community and that’s why we put out this annual go-to health guide for residents. From trends that focus on outside appearance to those that get to deep rooted issues we may not even know we have, our health magazine addresses many of the must-know and want-to-know topics our readers are clamoring to learn more about. And as the only comprehensive health and wellness guide in the area, we know just how important this magazine is for many. Within these pages we discover expert health advice from leading physicians including everything from how to ward off COVID-19 to how to boost your wellness with wine. From dietary trends to dental procedures, senior care to health in-utero, we cover health stories for everyone in the county—and that means men, too! Some of our best takeaways from this issue are how local hospitals are developing new ways to treat patients. From Kaiser to Dameron, St. Joseph’s Medical Center to San Joaquin General Hospital, our area’s hospitals are leading the charge in everything from weight loss to heart disease. Throughout this issue we hear the voices of leaders in healthcare, each working to make San Joaquin County a destination for quality healthcare. Sandwiched between Sacramento and the Bay Area, our care facilities don’t play second fiddle to anyone. Instead, the industry is booming right here in the Central Valley, making it possible to get quality care without leaving your neighborhood. So, if you’re ready to prioritize health or simply want to be inthe-know about what’s new in the health world, we welcome you to settle in and get ready to learn. After all, when it comes to health, knowledge is—quite literally—power. Stay healthy, San Joaquin,
LAST THOUGHTS
FOR ALL YOU DO, THANK YOU. We recognize that this is not an easy time for many, and we want to acknowledge healthcare workers on the front lines right now, doing all they can to care for those affected by COVID-19, which often means putting their own health on the line. If you’d like to help out, consider giving back to nurses, doctors, EMTs, and other healthcare workers by buying them a meal from a local restaurant, donating PPE equipment, or having your kids write thank you cards.

podiatry
BY NORA HESTON TARTE
SANDAL SEASON GET YOUR TOES PRETTY—AND HEALTHY—FOR SPRING It’s that time of year where the sandals come out and the toes make their debut! We all want to put our best foot forward, but that’s hard to do when the winter has given your feet a beating. In addition to getting a fresh pedicure and using a foot mask to soothe dry skin, there are some more in-depth approaches you can take to making your feet look and feel better. “The feet are a complex part of the body that are designed to support your weight and help you stand, walk, and run,” says Rajdeep K. Sahota, DPM, podiatrist and a primary care provider at San Joaquin County Clinics. “As with any other medical issue, prevention is key, so even if you have healthy feet, a podiatrist can help you learn about common issues to watch for and also discuss proper shoe gear and orthotics.” To tackle pain, discomfort, and unsightly bunions, corns, and structural deformities, talk to a specialist. “It is important to have these issues treated in a timely fashion to avoid more severe complications that may lead to pain and difficulty walking,” Dr. Sahota warns.
FOR THOSE WITH DIABETES
Those with diabetes need to be more diligent about footcare. Poor circulation makes it hard for the foot to heal on its own, plus numbness can lead diabetics to not notice foot problems before it’s too late. Check your feet daily and go to the doctor immediately with problems. Diabetics should not attempt at-home self-care without first consulting a podiatrist. ASK AN EXPERT:

CORNS AND CALLUSES Corns and calluses are thick, hardened layers of skin formed through friction and pressure, such as by wearing the wrong type of shoes or orthotics. Not only do they not look pretty, but they can become painful. TREATMENT “If painful, a podiatrist can shave down the thickened skin and provide recommendations for padding, shoe orthotics, and creams to prevent recurrence,” Dr. Sahota says. “If the corn/callus is caused by a deformity such as flatfoot or hammertoes, surgical intervention may be required.”
TRY THIS AT HOME:
• Change the type of shoes you are wearing or add orthotics into shoes • Moisturize regularly • Give yourself a fancy foot bath in an at-home foot spa • Use a pumice stone or similar device on affected areas
BUNIONS A bunion looks like a large bump on the big toe joint. “Bunions are formed when the bone or soft tissue of the big toe joint becomes misaligned and causes the toe to move toward the lesser digits,” Dr. Sahota explains. Ultimately, this is caused by persistent pressure on the joint or abnormal motion, which can be connected to physical activity or poor footwear choices.
TREATMENT Conservative treatments will be used first. Custom orthotics, anti-inflammatory medications, and even cortisone injections can alleviate pain caused by a bunion. Severe cases, or cases that cannot be managed with conservative treatment, may require surgical intervention. TRY THIS AT HOME:
“If mildly painful, wearing wider shoes and [a] toe spacer between the first and second toes and a good arch support can alleviate pain and help to slow the progression of the bunion,” Dr. Sahota says.
HAMMERTOE/ MALLET TOE Also referred to as a rotated toe, this references a toe that is bent at the joint. It is commonly caused by wearing high heels or shoes that are narrow at the front where the toes are. Dr. Sahota suggests avoiding prolonged wear of flip-flops and opting instead for sandals that have support at the back of the heel so as not to further progress a hammertoe deformity. TREATMENT Some hammertoes, specifically those that are rigid and painful, require surgical intervention to treat. TRY THIS AT HOME:
For flexible or less severe hammertoes, change footwear to roomier shoes with more room in the toe box and proper padding around the toes.

INGROWN TOENAILS Ingrown toenails have a variety of causes, including accidental and improper nail trimming, poor footwear choices, trauma, or even genetics. Regardless of the cause, ingrown toenails can lead to an infection that needs to be addressed, sometimes by a podiatrist. Prevention, when possible, is key. Avoid shoes with narrow toe boxes and be sure to always trim toenails straight across. If you have sharp corners on your toenail, smooth them with a nail file and trim toenails if you ever feel pressure on them from your shoes.
|
TREATMENT Redness, swelling, and/or drainage indicates an infection and should be addressed by a podiatrist. A podiatrist will be able to remove the ingrown portion of the nail and prescribe an antibiotic if needed. TRY THIS AT HOME:
• Epsom salt soaks • Antibiotic ointment • Bandaging the affected area

beauty BY NORA HESTON TARTE
THE SKINNY ON
FAT GRAFTING Do you wish your cheeks were plumper but your booty slimmer? What if your belly fat could help you go up a cup size? Well, it can. It’s called fat grafting, or fat transfer, a technique used to move fat from one part of your body to another. Let’s break it down. Fat transfer is a three-step process that starts with liposuction, removing fat from an area you don’t want it such as the abdomen, hips, or thighs. Then, the fat goes through a purifying process to remove damaged cells and other impurities. Lastly, the fat is reintroduced into the body into an area of your choosing— commonly the butt, boobs, hands, or face. Not everyone is an ideal candidate for this procedure. First, your body must have enough excess fat on it to be used. Doctors also want to manage expectations, ensuring your beauty goals are achievable with a fat transfer procedure. And, as always, health matters. Candidates for the procedure must be in generally good health and are typically nonsmokers. At McNemar Cosmetic Surgery in Tracy, the most popular fat transfer procedures are the following:
Brazilian Butt Lift This procedure results in rounder buttocks without the addition of synthetic implants.

2020 SJ HEALTH AND MEDICAL GUIDE
Breast Reconstruction – If you’ve had a full or partial mastectomy, fat transfer could be the best answer for reconstructing your breasts.
Cosmetic Enhancement for the Face/Hands If signs of aging are showing on your face and hands such as sagging skin or sallow cheeks, fat transfer can be used to give these parts of the body a youthful boost and a fuller look.
Lip augmentation Plump lips are in and you can give your kisser a boost with fat grafting. Recovery from a fat transfer procedure isn’t typically too strenuous but it does vary depending on both the patient and the procedure. Procedures are typically on an out-patient basis so you are able to go home the same day. Some of the above listed procedures can be done in office without general anesthesia, which means patients not only go home the same day, but also recover faster. Then, expect about a week for soreness and bruising to go down. After you heal, the incisions should be barely visible or not visible at all. However, this is still a cosmetic procedure and your body will need time to heal. Dr. McNemar recommends taking one week off of work to recover and waiting three to four weeks to resume most workout routines. SCHEDULE A CONSULTATION: MCNEMAR COSMETIC SURGERY 2160 W. Gum disease is one of the primary causes of tooth loss but proper brushing, flossing, and good nutritional habits can help reduce damage to teeth. Dr. Rafat Razi, a pediatric dentist in Tracy, says to diagnose gum disease, a dentist clinically evaluates the mouth for any bone loss. “[We also offer] preventive care to patients and parents [including] routine cleanings, periodic exams, x-rays, and fluoride treatments,” Dr. Razi says. Dr. Page Hudson, a dentist with Children’s Choice Dental Care in Stockton, says his office provides dental cleanings, dental sealants, fluoride treatments, and oral hygiene instruction to help combat the issue. “These services all help to strengthen the teeth. [They] can help reduce harmful bacteria in the mouth,” Dr. Hudson says.

mouth. The gums should be pink, firm, and relatively painless. An individual who is in pain should contact a dentist. Dr. Razi says there are two main types of gum disease. The first is gingivitis, which involves inflamed bleeding gums, and is reversible. The second is periodontitis, which is an advanced chronic form of gingivitis that involves bone loss and loss to the supporting tooth structures. One of the best ways to prevent gum disease is to remove plaque, a sticky coating of proteins and food byproducts that forms on the teeth. Brushing at least twice a day, flossing daily, and avoiding certain foods helps. “Chewy or sticky foods loaded with sugars and carbohydrates... sodas, energy drinks, sports drinks increase...risk of gum disease,” Dr. Hudson says.

HOW DENTISTS HELP
A dentist helps a patient avoid gum disease by removing plaque and identifying root causes. At appointments, Dr. Razi performs a risk assessment for tooth decay and gum disease. “Many factors can contribute to gum health, such as predisposing factors [like] root anatomy, tooth position, crowding…[and] environmental factors, like medications and stress,” she says. Once the risk factors are identified, it’s easier to suggest preventive measures. Hudson says to prevent gum disease, patients must stay vigilant. “It is only through constant daily oral hygiene, along with regular teeth cleanings by a dental professional, that you keep the numbers of bacteria down and prevent them from causing damage to the gums and bone,” Dr. Hudson says.

Dignity Health’s St. Joseph’s Medical Center’s premier spine and orthopedic services have expanded to offer the MAKO Total Knee Replacement Procedure, a total knee replacement surgery completed using a Mako Robotic-Arm that increases long-term comfort, reduces recovery times, and allows for more accuracy during surgery. This procedure benefits both those with arthritis and those who have suffered a serious injury to the knee joint. Like other total knee replacement surgeries, the joint is removed and replaced with an artificial joint. However, this approach has been proven to help those who undergo surgery return to their normal daily activities as quickly as possible by utilizing a smaller incision and increasing surgical technique and accuracy. the MAKO robotic arm, surgeons are able to be more precise, protecting both soft tissue and ligaments from damage during surgery. In a clinical study, MAKO patients surveyed six months after their procedure reported lower pain levels overall than those who underwent other joint replacement surgeries, as well as better satisfaction overall. MAKO surgeries require a series of steps: plan, assess, prepare, and insert. The first step—surgical planning—involves developing a 3D virtual model of the joint using a CT scan to help doctors plan the best approach for your procedure as well as create a custom-fit implant. During surgery, the MAKO arm sends information to the surgeon on range of motion capabilities to assure the best placement is chosen during the procedure. skin health BY NORA HESTON TARTE
NOT JUST SKIN DEEP BEAUTIFUL SKIN IS HEALTHY SKIN We’re not just talking about beauty, here. While healthy skin is certainly more luminous, hydrated, smooth skin is also indicative of overall health. “Our skin is the largest barrier against infection we have,” says Dr. Ruby Gill of Gill Aesthetic Institute in Lodi. “Keeping our skin healthy and hydrated helps keep this barrier durable.” When skin becomes dry, it cracks and those cracks can let in debris and bacteria leading to infection. There are a few steps people can take to ensure their skin stays healthy. Step one is establishing a legitimate skincare routine that involves moisturizer and sun protection. “Investing early in the health of your skin with a faithful skincare regimen will not only better protect it from harsh effects of the changing seasons, but also keep you looking and feeling your absolute best all year long.” Moisturize often using thirst-quenching products both in the morning and at night and include intensive hydrating products weekly or biweekly such as masks. You should also be using sunscreen everyday year round. Dr. Gill suggests grabbing one with zinc and titanium for the best protec20

tion. The words “broad spectrum” should also be on your sunscreen. When choosing any products, especially for the face, look for ingredients such as SPF, retinoids, vitamin C, AHAs, and hyaluronic acid to protect skin. “Our medications and skin products utilize the purist ingredients and the most potent actives to achieve unparalleled outcomes,” Dr. Gill says. The science behind the formulas goes like this: vitamin C repairs damaged skin cells, brightens skin, and evens skin tone; hyaluronic acid hydrates skin and reduces the appearance of fine lines and wrinkles; retinoids have powerful anti-aging properties; and AHAs exfoliate skin, alleviating dull complexions and promoting skin cell generation. What you don’t put on your skin is sometimes just as important as what you do put on your skin. Harsh soaps or makeup and chemical-heavy products can cause skin to chafe and crack. Dr. Gill specifically suggests staying away from products with DEA, parabens, propylene glycol, and formaldehyde, all of which can wreak havoc on both health and appearance, dulling skin’s natural glow. When you’re at a loss or are experiencing especially problematic skin, visit a doctor who can provide a visual analysis of the skin to uncover answers about how to treat your biggest complexion woes. pregnancy BY DEIRDRE CARLSON
DRINK ‘TILL IT’S PINK AND OTHER COMMON PREGNANCY MYTHS
When you’re pregnant, you’re likely to hear a lot of old wives’ tales, some of which can be amusing (carry high - it’s a girl, carry low - it’s a boy). Some, however, are not so amusing.
1
YOU CAN’T EXERCISE
This is a myth. Exercise is strongly recommended during pregnancy. It’s safe and better for you and your baby if you stay active during pregnancy. Most women can continue the same exercise they did before pregnancy if a doctor signs off on it. If you were not very active before you became pregnant, start with gentle exercise, such as walking, and build up.
2
IT’S OKAY TO DRINK A GLASS OF WINE WHEN YOU’RE PREGNANT
Thinking it’s ok to have a few drinks here or there while pregnant? Unfortunately, it’s not clear how much alcohol it takes to cause problems, so the best advice remains the same: women should avoid alcohol if pregnant or trying to become pregnant.
22
|
3
DON’T DYE YOUR HAIR
Thankfully you’re not condemned to nine months of bad hair days. Research shows it’s safe to color your hair in pregnancy. You’d need to use an extremely high dose of the chemicals far more than needed to color your hair - to cause harm.
4
YOU’RE EATING FOR TWO
Thinking of helping yourself to that extra scoop of ice cream? Yes, you’re eating for two, but that doesn’t mean two full adult size servings are necessary. Most women will only need an extra 200-300 calories per day to promote baby’s growth (on top of the 2,000 daily recommendation), assuming that you’re starting pregnancy at a normal weight.
5
YOU CAN’T FLY
Worried about booking a baby moon? Fear not. Get on the plane, have a good time, and revel in the freedom of a holiday while you can. When
2020 SJ HEALTH AND MEDICAL GUIDE
you fly, the air is dry. You’re also more likely to be sitting for a prolonged period of time. Just make sure to hydrate and get up and walk around every hour or so.
6
YOU CAN’T HAVE ANY COFFEE
You don’t need to rule out caffeine completely, but moderation is key here. Guidelines recommend that you limit yourself to no more than 200 mg of caffeine per day, which can be as little as one cup of filter coffee and a bar of chocolate. More than 60 percent of women who checked their average intake were over the limit. TALK TO A PROFESSIONAL: P. SICK SMARTS
“Washing your hands is extremely important.” Dr. Elyas Parsa, San Joaquin General Hospital
QUICK TIPS FOR STAYING HEALTHY
Whether it’s the common cold or a pandemic like COVID-19, there are ways to avoid getting sick and spreading illness onto others. Regardless of the type of illness, many of the precautions are the same. “As a primary care physician, I always emphasize that we need to do whatever it takes to boost our immune system,” says Dr. Elyas Parsa of San Joaquin General Hospital. Strong immune systems mixed with good hygiene habits can make all the difference. A strong immune system “likely will impact your reserve to recover from common illness faster and or have less severe symptoms when you do get sick,” Dr. Parsa says. GETTING CONTROL OF STRESS EATING HOW NOT TO FEED YOUR FEELINGS Sometimes it happens after getting an angry email from your boss, going through a break-up, or losing someone close to you. It starts with a handful of chips and the next thing you know you’re polishing off that pint of ice cream without even knowing what happened. This episode could be classified as stress or emotional eating. Although there is no formal definition of “stress eating”, on a basic level it is the practice of consuming food – usually comfort or junk foods – in response to feelings instead of actual hunger. Whatever emotions drive a person to overeat, the result is usually the same. Once a person realizes what they have done, they beat themselves up, feel bad, and overeat again, resulting in an unhealthy cycle. The good news is that if you are prone to falling into the trap of stress eating, you can take steps to regain control of healthy eating habits with tips from expert Jennifer Lombardi, MFT, Marriage and Family Therapist at Kaiser Permanente Stockton Medical Offices. Published by San Joaquin Magazine
BE AWARE OF STRESS LEVELS
Many of us also turn to food for comfort, stress relief, or to reward ourselves. When stress is chronic, as it so often is in our chaotic, fast-paced world, it is easier to fall into the trap of eating to feed your feelings. According to Jennifer Lombardi, MFT, it is concerning that “we pack our days full of ‘to do’ items and are often exhausted by the time we get home at night.” In her practice, Jennifer has witnessed younger and younger patients struggling with disordered eating.
FOCUS ON THE REAL ISSUE
We all know that food can become a crutch when we are stressed and it may be a symptom of unmet needs. Jennifer explains that it is important when treating her patients to identify what the motivation is to change it, or as she likes to say, the why behind the wish to make things different. She encourages her patients to
try to better understand their emotions and help them develop tolerance and coping skills instead of using food or another method as a numbing agent.
BE MINDFUL
According to Jennifer, “one of the most effective ways to take a step in the right direction is to engage in mindfulness.” She defines mindfulness as slowing down, paying attention, and being curious, (not critical) about habits. So, next time you feel the need to reach for that bag of chips or pint of ice cream, take a step back to slow down and identify your feelings in that moment. women’s wellness BY NORA HESTON TARTE
LOOK GOOD, FEEL GOOD: Beauty is certainly more than skin deep, but there is a connection between how we look and how we feel. Many women—and men—can get a confidence boost just by having a good hair day or taking care of their bodies. It’s more about feeling good about yourself than it is about conforming to traditional beauty constructs. If you’re feeling a little down lately, try one or two of these pick me ups. It’s amazing what a little TLC can do for your psyche.
HIT THE GYM (OR THE TRAIL)
Sweating does more than help you lose weight or tone your body. A gym sesh can have you feeling better instantly thanks to the release of endorphins. Plus, you’re likely to experience an energy boost. More than that, working out can make you feel strong, sexy, and accomplished. There is nothing more empowering than feeling like you can take on the world.
GET A MAKEOVER
We’re not talking Botox and plastic surgery (unless that’s what you want to do). Something as simple as a mini shopping spree or new haircut can make all the difference in how you feel about your outer appearance. And sometimes looking fierce translates to feeling fierce. So, pick up a new outfit before you ask for that raise or adopt a new makeup routine for date night. Whatever it is, if it helps you feel good then it isn’t silly.

SURROUND YOURSELF WITH SEXY
Feeling sexy has to do with all of your senses. While your outward appearance may be a big part of that, feeling relaxed, accomplished, and good in the spaces you are in can also help. Put some pizazz in your office with bright colors and #girlboss swag that inspires you or turn your bedroom into a cozy oasis designed to soothe the stress that can build-up over a long day.
FAKE IT TILL YOU MAKE IT Sometimes when you don’t feel confident, f e i g n i n g confidence can help. It sounds counterintuitive, but for some it truly works. Stop focusing on your flaws and instead draw attention to your most appealing attributes. Soon, others will be focusing on those, too, and that false act of confidence may become natural faster than you thought.

BEWARE OF BRITTLE BONES THE 411 ON OSTEOPOROSIS
BY DEIRDRE CARLSON
Osteoporosis is more than deteriorating and weak bones. The disease is responsible for an estimated 2 million broken bones per year and one in two women and one in four men over age 50 will break a bone due to osteoporosis. Breaking a bone is a serious complication of osteoporosis, and bone breaks are most likely to occur in the hip, spine, or wrist, but other bones can break too.
CAUSES OF OSTEOPOROSIS Although the exact cause of osteoporosis is unknown, the process starts early in life (mid-30s) when bone is continuously being replaced. When bones begin to lose calcium faster than they can replace it, the bones begin to thin. Osteoporosis by definition means “porous bone.” The good news is that the right diet, exercise, supplements, or medication can help prevent osteoporosis or lessen its effects.
WHO IS AFFECTED? Women are more susceptible than men. Of the estimated 10 million Americans with osteoporosis, about 8 million are women. A woman’s risk of breaking a hip is equal to her combined risk of breast, uterine, and ovarian cancer.
EATING THE RIGHT FOODS The two most important nutrients for bone health are calcium and vitamin D. Each day, we lose calcium through 28

2020 SJ HEALTH AND MEDICAL GUIDE
our skin, nails, hair, and sweat, but our bodies are unable produce new calcium. When we don’t get enough calcium for our body’s needs, our bones must provide it, leading to low bone density. Experts recommend a well-balanced diet with plenty of dairy, fish, fruits, and vegetables as well as an emphasis on foods high in calcium. The recommended daily intake (RDI) of calcium is 1,000 mg per day for most adults, though women over 50 and everyone over 70 should get 1,200 mg per day.
THE FOODS HIGH IN CALCIUM ARE: • Dairy products • Canned sardines and salmon • Beans and lentils • Green vegetables, including collard greens, turnip greens, kale, okra, Chi nese cabbage, dandelion greens, mustard greens, and broccoli The best foods for vitamin D are fatty fish such as salmon, mackerel, tuna, and sardines. Calcium and vitamin D are sometimes added to juices, breakfast foods, soy milk, rice milk, cereals, snacks, and breads. If you’re not getting enough calcium and vitamin D from food, you can also take supplements.
EXERCISE FOR BONE HEALTH Exercise not only prevents osteoporosis by strengthening bones but can also help maintain bone mass if you already have osteoporosis. Through exercise, you build and maintain your bones’ thickness.

VARICOSE VEINS NOT JUST A COSMETIC ISSUE Veins are blood vessels that return blood from organs in the body toward the heart, an imperative bodily function. OVERWHELMED by BAD NEWS? Hit the Pause Button BY DOUGLAS OAKLEY, KAISER PERMANENTE they can understand, and what ability they have of controlling the anxiety you may be inadvertently putting on them,” Dr. Balan said. “The idea is not to burden them with things they cannot control.” For younger kids, reminding them to eat right, wash their hands, and go to bed on time in most cases is better than explaining the worst-case-scenario horrors of the coronavirus.
STRESS AT WORK
Yener Balan, MD, Kaiser Permanente Northern California executive director of Behavioral Health, offers insight on how to maintain your mental health, talk to children, and support colleagues during times of unrelenting bad news. Just about every 5 minutes since the beginning of January, another round of frightening coronavirus news is released, often sensationalized, about the dangers of its spread around the globe. Hospitalizations, travel bans, school closures, shortages, the economy, and on and on. Bad news browsing can be addictive, raising heart rates and stealing sleep.
HIT THE PAUSE BUTTON “It’s important to recognize the moment between the stimulus — bad news — and the response — coping habits — and to pause and check in with yourself,” Dr. Balan said. “People need to stop for a half second and say, ‘Do I need to eat that entire bag of potato chips, 32

which I normally don’t do, or drink more than I normally do?’” Dr. Balan said many people are drawn to bad news, because it offers a short dopamine high that quickly fades. The next round of news gives another high and crash, creating a cycle of addiction that soon may have you losing sleep, changing your eating and drinking patterns, and responding differently to stress. “If I usually respond with a laugh when my colleague gives me a hard time about my funny hat, but now I am suddenly angry, that is a warning sign,” Dr. Balan said. “If you notice these kinds of changes, you are going to benefit if you take a few minutes and ask yourself, ‘Wait, how did I get here?’ You can also talk to a professional.”
Dr. Balan said employers and employees have a responsibility to look out for each other, especially if their work is impacted by the pandemic or other emergencies. People in the health care field, for example, may experience anxiety, fear, longer work hours, a barrage of changing strategies, and uncertainty, such as cancelled meetings or conferences. “If you notice someone in your office who is experiencing all or some of these things, at the very least you should say, ‘Hey, you’ve been working 12 hours every day here, take tomorrow off and we’ll help you get the work done,’” Dr. Balan said. In general, maintaining mental health during times that may seem apocalyptic requires an intentional awareness of how you are feeling and how you have changed, Dr. Balan said. “Don’t default from anything other than who you are,” Dr. Balan said. “If you are not as nice as you used to be or if you’re losing or gaining weight, that’s a warning sign. The sooner you are mindful of your choices and patterns, you will have a better chance of staying healthy.”
TALKING (OR NOT) TO CHILDREN What to say to children about bad news such as natural disasters or pandemics depends on their age, Dr. Balan said. “Be mindful of what they can tolerate, what

Part of the struggle is knowing when to seek care and when a cough is more than just a cough. The symptoms vary by age. Most children will experience a runny nose and cough that lasts one to two weeks. Then, the cough ramps up and children experience rapid coughing spells punctuated by a telltale whooping sound at the end. Infants, however, may never experience a cough. “Parents may describe episodes in which breathing briefly stops and the infant’s face turns red or purple,” Dr. Jain says. For older children and adults, the illness typically presents itself as a cough that lasts for several weeks It is likely to start with mild upper respiratory tract symptoms similar to common cold and progress to a cough with a “whooping” or gasping sound at the end. side, with an average sick time of six to ten weeks. “Approximately half of adolescents with pertussis cough for 10 weeks or longer,” Dr. Jain says. For many, whooping cough is an awful illness that they will recover from, but it can be dangerous especially for infants under six months of age. It is most dangerous in preterm and unimmunized infants. The stats are scary. Sixty-seven percent of infants will experience gagging, gasping, and/or apnea when they have whooping cough. Sudden death is also possible in infants. Complications among infants include pneumonia, pulmonary hypertension, and complications related to severe coughing spells such as conjunctival bleeding, hernia, severe coughing spells leading to hypoxia, and complications such as seizures, encephalopathy, apnea, and death. “More than two-thirds of infants with pertussis are hospitalized,” Dr. Jain says. Pertussis is highly contagious, as well, and even previously immunized people can contract the disease. This is in part due to how and when the vaccines are given. If you’re unsure, a test is available to check for the presence of antibodies in the bloodstream. If needed, additional boosters can be given. Limiting exposure to the general public as well as asking those who come in close contact with your baby to receive the booster, can help mitigate risk. You should also avoid people who are coughing, and if your child will be in daycare, ensure they have received the vaccine before enrollment.
PREVENTION • Stay up to date on immunizations • Get a Tetanus Booster (Tdap) • Cover your cough • Wash your hands

SAN JOAQUIN GENERAL HOSPITAL WINS A PERFORMANCE EXCELLENCE AWARD FOR CONGESTIVE HEART FAILURE CARE BY CREIGHTON YOUNNEL
San Joaquin General Hospital has won the California Public Hospital Association’s (CAPH) prestigious 2019 Quality Leaders award for Improving Heart Failure (HF) Outcomes at San Joaquin General Hospital. The Statewide award highlights the Hospital’s Primary Medicine Department’s Multidisciplinary Team-Based Approach in the CAPH Performance Excellence category. According to the American Heat Assn. reports that 5.7 million adults in the U.S. have heart failure costing the nation an estimated 30 billion annually. The congestive heart failure (CHF) rate in San Joaquin County 38

exceeds both the state and national rate. San Joaquin General Hospital, under the leadership of Moshen Saadat, DO, FACP Chairman of the Hospital’s Internal Medicine Dept., assembled a clinical team to identify and address congestive heart failure outcomes coming to San Joaquin General and SJ County Clinics. “Here at San Joaquin General we experienced the same escalating prevalence and challenges in HF management as exists nationally”, says Dr. Sadaat. “Despite having a “traditional specialty CHF clinic,” during a 12-month period of this study, our re-admission, length of stay when admitted, and number of ED visits for our HF patients increased, while utilization of the
clinic by our HF patients actually declined. We saw the opportunity to improve and we acted on it. Seven out of ten pivotal issues addressed by the 2017 ACC (American College of Cardiology) expert consensus paper for the optimization of HF involves addressing the challenges of care coordination, improving adherence, managing co-morbidities, integration of palliative care, and transition to hospice care. Our institutional goal was to create a multidisciplinary clinic that reduces HF related admissions and ED visits, to provide whole person care while specifically addressing HF and related co-morbidities. We centered the care around the patients’ needs. Our aim was to engage and more importantly empower our patients to improve their health and try to make care not only more accessible, but more convenient for our patients.” The new CHF Clinic assembled a Hospital and Clinic wide team consisting of the Hospital’s medical staff, pharmacy department, and nurse managers. “When we see patients in the heart failure clinic, we spend extra time focusing on the medications. We ensure our patients are on the most appropriate guideline-directed therapy and understand the importance of taking all of their medications appropriately” says Hospital Pharmacist Jared, Pharm. D, BCPSAQ ID. The redesigned multidisciplinary CHF Clinic has reduced HF Hospital and emergency visits. San Joaquin General Hospital’s team approach to HF management follows the Hospital’s mission of providing the optimal, compassionate care to the residents of San Joaquin County. San Joaquin General Hospital sees over 30,000 patients annually and also has 105,000 primary care visits annually. SAN JOAQUIN GENERAL HOSPITAL 500 W. A HEALTHY PREGNANCY DIET BY DIERDRE CARLSON
Congratulations! You are now eating for you and your baby. Good nutrition plays a key role in the health of both mother and baby. As a mom-to-be, you have higher nutrient needs than you did before conception. Yet the general principles of good nutrition—variety, balance, and moderation—still apply during pregnancy. Calorie needs increase during pregnancy, but “eating for two” only requires an additional 200 to 300 extra calories from nutrient-dense foods such as lean meats, low fat dairy, fruits, vegetables, and whole grain products. It will be important to carefully consider the foods you consume during your pregnancy.
KEY NUTRIENT GUIDELINES DURING PREGNANCY: Calcium: 1,000 milligrams per day Calcium is needed in the body to build strong bones and teeth. Calcium also allows the blood to clot normally, nerves to function properly, and the heart to beat normally. Dairy is the best source of calcium. Other sources of calcium are dark, leafy greens, fortified cereal, breads, fish, fortified orange juices, almonds, and sesame seeds.
40
|
FOLIC ACID: 400 MICROGRAMS PER DAY Folic acid is used to make the extra blood your body needs during pregnancy. Foods rich in folic acid include lentils, kidney beans, green leafy vegetables (spinach, romaine lettuce, kale, and broccoli), citrus fruits, nuts, and beans. Folic acid is also added as a supplement to certain foods such as fortified breads, cereal, pasta, rice, and flours.
2020 SJ HEALTH AND MEDICAL GUIDE
IRON: 27 TOTAL MILLIGRAMS OF IRON PER DAY Iron is an important part of red blood cells, which carry oxygen through the body. Good sources include whole grain products, lean beef and pork, dried fruit and beans, sardines, and green leafy vegetables.
VITAMIN D: 600 INTERNATIONAL UNITS OF VITAMIN D PER DAY Vitamin D works with calcium to help the baby’s bones and teeth develop. Good sources are milk fortified with vitamin D and fatty fish such as salmon. Protein: A woman weighing 150 pounds needs 75 grams of protein per day (each person is different. To estimate, use your pre-pregnancy weight and divide by 2). Protein is needed for energy and to build and repair different parts of your
body, especially brain, muscle, and blood. Choose a variety of protein-rich foods, which include seafood, lean meat and poultry, eggs, beans, and peas. Use labels on packaged food to determine how many grams of protein each food provides. FOR MORE INFORMATION: BHAGYA NAKKA, MD 1805 N. WHEN IT’S TIME FOR WISDOM
TEETH REMOVAL BY JESSICA ZIMMER What are wisdom teeth? Wisdom teeth are a third set of molars that typically come in between the ages of 17 and 25. For many patients, these teeth aren’t welcome. Wisdom teeth can remain trapped in the gums or develop too close to other teeth. Sometimes they impact other teeth and cause infections. A patient who is in their late teens or early twenties and experiencing pain in the back of their mouth should consult a dentist or oral or maxillofacial surgeon, a type of dentist that specializes in surgery of the face, mouth, and jaws.
WHAT EXTRACTION INVOLVES Extraction involves three steps: numbing the area, separating the tissue connecting the tooth to the jawbone, and removing the tooth. A dentist numbs the area where the wisdom teeth are erupting by administering an anesthetic. The dentist uses a local anesthetic to numb a specific spot to remove one or two teeth. They use a general anesthetic, which typically causes a patient to go to sleep, when removing several teeth. The extraction of each tooth usually takes only a few minutes and when the dentist removes multiple teeth, they may stitch up the gum afterward. If the dentist has to cut a tooth into smaller pieces to remove it, the process can take up to 20 minutes per tooth.

2020 SJ HEALTH AND MEDICAL GUIDE
HOW REMOVING WISDOM TEETH HELPS Removing wisdom teeth leaves room for the back teeth to grow and develop normally. Removal also gives a patient more room to clean the back teeth, which are more prone to gum disease and cavities. Removal of the upper wisdom teeth lessens the risk of sinus pain. This is because these wisdom teeth are located close to certain sinuses. When the upper wisdom teeth grow, they can put pressure on the sinuses.
KEEPING WISDOM TEETH When wisdom teeth come in properly, they do not need to be removed. A patient should not attempt to determine whether they need removal on their own. It is hard to see far back in the mouth to determine whether there is a problem. In addition, if the patient has an issue with gum disease already, they may not be able to feel or see it. A patient can learn more about which wisdom teeth they should get removed by getting dental x-rays. Before commiting to a procedure, always ask the dentist about different options regarding anesthetics, payment plans, and follow-up prescription painkillers.

MEETS MEDICAL TECHNOLOGY DAMERON HOSPITAL TAKES A DYNAMIC APPROACH TO WEIGHT LOSS BY FAITH LEWIS
geon Dr. Tarig Samarkandy, it is this approach of taking into account both the physical treatment and the need for mental and emotional support that gives Dameron patients the tools they need to achieve lasting success. “My role as a surgeon is to provide patients with a tool to help them reduce their overall caloric intake,” he says. “However, that is just one part of the solution. The rest of it has to do with helping patients with lifestyle changes and commitment to ensure their weight loss and health improvements are sustainable for the rest of their lives.” For patients who can show a commitment to these lifestyle changes and a motivation to reach weight loss goals, there are two possible options for weight loss surgery. Whether you choose a gastric bypass (where a small pouch is attached to the intestines to bypass the stomach) or gastric sleeve (where a portion of the stomach is removed), each surgery limits your food intake and reduces how many calories you consume to help you lose weight.
ATTEND A FREE SEMINAR
If you, like so many others, have fallen off the diet bandwagon and feel like you’re losing your battle to the bathroom scale, it might be time to step away from the diet food aisle and turn to the experts. At Dameron Hospital, the team of medical professionals recognizes how much of a struggle it can be to lose weight and they’re there to help. Dameron Hospital’s Bariatric Weight Loss Surgery Program is designed to offer a physical solution for weight loss, as well as a support system that will keep you motivated and on track. specific needs through weight-management education, behavior modification therapy, and the option of gastric bypass or gastric sleeve surgery. The team knows that weight loss will only be successful if long-term lifestyle changes are made and maintained. There is also a huge emphasis on building a support system with your medical team through monthly patient support groups. Here, patients have the opportunity to share their weight loss story, encourage and be encouraged by others, and develop relationships on their journey to weight loss. According to Bariatric Sur-

SAY ‘YES’ TO
RED WINE HEALTH BENEFITS OF RED WINE
BY NORA HESTON TARTE
We live in the perfect region for indulging in big glasses of spicy zinfandel and other bold reds. So, let’s stop talking ourselves out of enjoying our favorite sips. When had in moderation, red wine offers many health benefits—and white varietals aren’t without their own perks. While we can’t condone guzzling a bottle of pinot, we can give you a few reasons to say yes to that glass of vino with dinner.
WHOLE BODY HEALTH
The key to much research championing wine, especially reds, as healthy in moderation is resveratrol. Resveratrol is a polyphenol, a type of compound said to act like an antioxidant protecting the body against ailments such as cancer and heart disease. The compound was found in the skin of red grapes, 46

2020 SJ HEALTH AND MEDICAL GUIDE
which is why wine is one way to consume it. It is, however, important to mention that heavy alcohol consumption has a negative effect on the whole body so the key here is how much/ often you drink.
CARDIOVASCULAR HEALTH
A glass (five ounces) of red wine each day, or a few times a week, has been linked to good cardiovascular health, and recently white wine was also praised for its positive effects on blood pressure and lower risks of heart failure.
BRAIN HEALTH
What if we told you a single glass of vino per day could keep you from developing Alzheimer’s and other memory diseases? The flavanols in red wine support healthy blood cells
and good blood flow to the brain, preventing harmful plaque buildup and supporting cognitive function.
WEIGHT LOSS
Wine has a significant number of calories—about 120-150 per five-ounce pour—but that doesn’t change the fact that those who drink are less likely to suffer from obesity or even type 2 diabetes.
MOOD BOOSTER
While alcohol is a depressant, there is something to be said for having a glass when you’re in a funk. In uncomfortable or unenjoyable social situations, having a glass of vino was shown to increase the drinker’s mood substantially. This could be partially because wine has been linked to helping people relax or combat stress.
CHEERS TO A LONGER LIFE!
Moderate wine drinking can actually lead to a longer life, for all of the reasons above and more. Population studies have shown that those who drank in moderation—and not in excess—had longer average lifespans than those who didn’t drink alcohol at all. Part of the reasoning was linked to the lifestyles of those who drink in moderation being healthier overall.

Dental implants are a well-fitting, long-lasting alternative to crowns and bridgework. Individuals who want to protect weak and damaged teeth should talk to a dentist, oral surgeon, or maxillofacial surgeon about the advantages of this device. Benefits include more biting force, an easy cleaning routine, and no requirement to adjust or refit. According to the American Academy of Implant Dentistry, over 3 million people in the U.S. have dental implants. Every year, an additional

500,000 people receive dental implants. Like crowns and bridgework, dental implants can improve the appearance, alignment, and shape of teeth. These devices guard against breakage and wear. Another advantage of dental implants is that they are not attached to other teeth. If a tooth near a dental implant develops a cavity and needs repair, a dentist can address that concern without having to remove the implant.
WHAT IS A DENTAL IMPLANT? A dental implant is a titanium post inserted into the jawbone below the gum line. A dentist mounts dentures and bridges on top of the post. The implant remains stable because titanium is a metal that fuses with bone. Within a few weeks of surgery, bone cells grow and attach to the post. Whenever an individual chews, the movement applies pressure to the new bone surrounding the post. The pressure moves the root of the tooth slightly. This stimulates the bone tissue, which keeps it healthy. An individual considering dental implants should research these devices as soon as they learn their teeth are broken or weak. This is because a dental implant needs a certain amount of bone to remain steady. When time passes after an injury, a patient’s jawbone can start to lose density, making it more likely to fracture. Receiving an implant soon after an injury or diagnosis maximizes the chances of the bone being dense and healthy enough to anchor the implant.
WHAT’S NECESSARY FOR DENTAL IMPLANTS An individual considering dental implants must have healthy gums and enough bone to keep the implant in place. If an individual has receding gums, they can get gum grafts prior to surgery. Healing from the gum surgery takes additional time, but may be worth it in the long run. This is because the individual is likely to have needed a gum graft anyway and the dental implant will fit better than a denture or bridge alone. An individual with a thin, soft, or weak jawbone may need a bone graft before getting a dental implant. A patient with sinuses close to the jaw may need a sinus lift—a procedure to raise the floor of the sinuses—before receiving an implant. The procedure helps the individual develop more bone to anchor the implant.
TYPES OF DENTAL IMPLANTS There are two types of dental implants approved by the American Dental Association. The first is an endosteal implant, which is implanted directly into the jawbone. The second is a subperiosteal implant, a metal frame fitted onto the jawbone below the gum tissue. An endosteal implant requires a second surgery to connect the post to the implant. The dentist then attaches artificial teeth to the post one by one. The dentist can also group the teeth on a bridge or denture. The process for a subperiosteal implant requires the gums to heal before the second step. As the gums recover, the metal frame affixes to the jawbone. Posts stick out through the gums and a dentist can mount artificial teeth to the posts.

FORGETTING TO CLEAN YOUR TOOTHBRUSH HOLDER (OR REPLACE YOUR BRUSH)
Those toothbrush holders, both for regular toothbrushes and electric ones, need a wipe down every once in a while to clean off yeast and mold that grow there. We suggest soaking it in soap and hot water once a week. Oh, and switch out your bristles every three months.
LETTING YOUR FRIDGE FILTER EXPIRE
That water filter inside your fridge should be replaced every six months if you want to keep your drinking water clean. If you have a BRITA machine or something similar, the filter should be replaced about every two months.
SKIPPING LAUNDRY
Laundering your clothes is part of the routine, but what about those other household items that need a good cleaning? Items like towels, bath mats, and sheets should all go into the
wash routinely. We tend to forget just how many germs linger on these items, including dead skin cells, sweat, and bacteria. Throw all these extras in once a week to keep clean, and every three uses for your bath towel.
FORGETTING TO WIPE DOWN SURFACES
Simple spills and quick sweeps are good for the daily, but once a week you should spend a little extra time disinfecting spaces like countertops, floors, etc. Wipe down kitchen and bathroom surfaces using a disinfectant every week, including appliances, mirrors, and sinks. You should also dust all hard surfaces, vacuum, and mop just as often.
We know everything about this area.
