: You’ve been a dermatologist since 1997. How have your patients become smarter about sun protection? “MORE OFTEN, I’ve noticed families using umbrellas or renting umbrellas at the beach, and I’m seeing an increased use of sun-protection clothing, especially for children. Ten years ago, I remember making my kids wear sun shirts when no one else was. Now, when we’re on vacation, there are so many kids wearing them. Also, more people seem to be using sunless tanning creams or getting professional spray tans—their look and scent has greatly improved over the years. Still, there has only been a very slight decrease in tanning bed use—unfortunately, 10 million Americans still use them. Some states have banned the use of tanning beds for anyone under 18, but this is not the case in Georgia. Now they need parental consent in Savannah, but they can still tan.”

1. CCHD SCREENING: “As part of the newborn hospital stay, all infants now receive a noninvasive screen for oxygen level. It’s the same red flashlight-looking device that doctors place on your finger at visits. This simple screen can reduce the chance of missing critical congenital heart disease in newborns.” 2. BABY FRIENDLY: “This is a certification sought after by birthing hospitals and physicians. As the benefits of breastfeeding became increasingly understood, Baby Friendly USA has become an important leader in benchmarking breastfeeding friendliness.” 3. SAFE TO SLEEP: “Many parents remember the ‘back to sleep’ campaign—having infants sleep on their backs reduced SIDS significantly. We now know that’s only one of many factors that contribute to SIDS, so ‘back to sleep’ was expanded to ‘safe to sleep.’ Other factors that reduce SIDS include sleeping on a firm surface and avoiding sharing a bed with parents.”

4. BACKWARD UNTIL AGE TWO: “Until just recently, most parents knew the car seat guideline for newborns was backwards until age one (or 20 pounds). Those guidelines have been updated, and it’s now recommended that infants face backwards in their car seat until at least age two—unless they have outgrown the maximum height and weight per the manufacturer.” 5. YES TO PACIFIERS: “Pacifiers have been shown to be another important factor in reducing SIDS. There is a balancing act, though, because while many pediatricians recognize the benefit, they will also recommend waiting to introduce a pacifier until a consistent latch has been established with breastfeeding.” 6. NO TO COUGH MEDICINES: “As their safety and efficacy has been studied and questioned over the years, these have fallen out of recommendation. Most products are now no longer available over the counter, and prescribing them has become much less common, especially in younger children.”

: You’ve been a physical therapist for 16 years. What are some of the biggest changes you’ve seen in physical therapy?

Entry level therapists are now graduating with a Doctorate of Physical Therapy. Second, therapists are incorporating new technology like lasers, dry needling, spinal manipulation, telehealth and robotics in treatment. Also, 0ne of the biggest changes is in referral orders from physicians. It is now common to receive physician orders that simply prescribe, “evaluate and treat.” This means physicians are no longer dictating specifically what the therapists should do with a patient, but instead leaving judgment and discretion to the PT. Lastly, direct access is now recognized in 43 states. This directive allows patients to access physical therapy diagnosis and therapy without a physician referral.

“THREE YEARS AGO, a two-year-old girl was in my office for a speech evaluation. She was grunting and screaming and reaching for a toy on the shelf. Her parents said that’s how she communicated. She would get so frustrated that her face would turn bright red and she’d cry and scream until somebody figured out what she wanted. I spent several sessions playing with her, trying to elicit sounds, with absolutely no progress. I went to my boss and said I had no idea what I was going to do because all that this little girl would do was scream. She told me to keep trying and that I would eventually break through if I remained patient. Several months passed. Then, the unforgettable moment of her first word: “bubble.” It still gives me chills thinking about it. Now, this little girl is in elementary school and she wants to be a meteorologist. She tells me funny stories and asks very thought-provoking questions. At a recent session, I told her that three years ago, she could not say a single word. She leaned in close and said, ‘I know, and you helped me.’”
