SYNERGY SERVES SULLIVAN From an afternoon rental in 2003, with a staff of 2 to a state of the art medical facility with a staff of 15. We have always tried to meet and even exceed the mental health and substance abuse needs of Sullivan County.
TO MEET THESE CHALLENGES: We are actually engaged with State, County, and Local agencies and providers forging a comprehensive plan to vigorously address these problems. Toward that goal we are happy to announce the following: 1. Participation in Opioid Task Force
BUT NOW WE ALL HAVE NEW CHALLENGES 1. The opioid crisis and other addictive disorders are growing at an alarming rate, too often with fatal consequences, too often to our young people. 2. Many of the Mentally Ill living in community residences are under served, and especially in need of long acting injectable medications. 3. There is a large and growing population of patients with treatment resistant depression where continued untreated depression often leads to dire consequences including suicide and substance abuse.
2. Full staff training and onsite capability for Narcan Administration. 3. The Bridge to Health Program with Catskill Regional Medical Center and Grover Hermann ERS. When patients who present for opioid detox, treatment, and overdoses they will, after ER Treatment and Stabilization be immediately transferred for follow-up treatments to the Synergy on call Medical Provider. 4. By January 2019 we will be doubling our clinical space by taking the vacant office next door to ours. 5. We will also be adding a full time, Nurse Practitioner, another Part Time License Clinical Social Worker, and additional clinical hours for our medical providers. 6. We will be expanding our substance abuse treatment capabilities with new groups, and two new Suboxone qualified clinicians. 7. We will be increasing our community outreach to the home bound mentally ill for –care, including injectable medication. 8. We have established and are expanding our Transcranial Magnetic Stimulation (TMS) Program; the greatest hope for our treatment resistant chronically ill depressed patients.
We are hopeful that these changes along with working with many others in the community will help to elevate the mental health of our county as never before. That is our goal and that is our mission.

A
s you walk into Dr. Mark Spina’s office for Advanced Physical Therapy and Chiropractic services, you are greeted by a wall full of positive messages that are meant to inspire and encourage a positive mental attitude. For 34 years, Dr. Spina has been helping those with pain not only in Liberty, but throughout Sullivan County. A native of Liberty and Neversink, Dr. Spina’s family has strong roots in the county. “My dad was also a chiropractor and he was a wonderful influence on me,” said Dr. Spina. “He also practiced here in Sullivan County.” Spina, now married with one son, attended grammar school in Liberty and went to high school at Tri-Valley and earned a degree at Life University’s Chiropractic Program in Georgia. His office can see anywhere from 10 to 30 patients a day who come for a variety of pain and ailments. Not only does Spina treat the immediate pain but also tries to get to the root of the problem by working to ensure that the body is in complete and total balance. “One of the primary things we do is the balance of the nervous system and make sure there's good communication throughout the body,” said Spina. “We talk about the importance of diet and rest and managing your stress.” Dr. Spina also talks to many of his patients about the importance of exercise and one’s mental attitude towards health and life. “If one can really balance those things properly, one should be healthy and the body should be able to function quite nicely,” he explained. The origin of the chiropractor pro-
fession started on September 18, 1895, where Daniel David Palmer gave someone an adjustment and according to Spina, that person was able to have their hearing returned to them. “As long as there is good communication from the brain—through the nerves and all the other parts of the body—we should be relatively happy and healthy,” said Spina. “Our proven strategies work to promote your overall health.” “We find that where interferences or subluxations occur because a bone is out of position, the nerve system can not commute with the rest of the body and will result in a dysfunction. That dysfunction can result in pain, numbness, spasms, tissue or organ comprises which will lead to symptoms,” added Spina. The medical definition of Subluxation is incomplete or partial dislocation, a condition—visible on x-ray films—in which the bony surfaces of a joint no longer face each other exactly but remain partially aligned. Spina is not only about fixing out of place bones or nerves but promotes a healthy lifestyle overall and to make a visit to a chiropractor a habit for a healthy life. “We set goals and what they want to achieve,” said Spina. “Seeing a CONTINUED ON PAGE 6H

The Community Health Champions support and guide the participants to reach their personal goals and to continue to sustain healthier habits. Within the Circle, other members often share the same goals and can work together to achieve health gains. A Community Health Champion recently reported that one of her Wellness Circle members has lost 20 pounds as a result of

These volunteers, from all walks of life in Sullivan County, have stepped forward and committed themselves to become certified Community Health Champions with Sullivan 180’s guidance and support. They complete training from the nationally recognized Talance online curriculum, and in-person orientation. There are several Community Health Champions who have already started Wellness Circles with their friends, peers, or coworkers. Each Wellness Circle participant chooses an area or areas of focus for improving his or her health. This could include losing weight, increasing physical activity, or reducing stress. Improving personal health behaviors can significantly improve one’s health and help in the management of or prevention of common chronic conditions such as diabetes or high blood pressure.

any of us in Sullivan County know that our county is ranked next to last in health outcomes in New York State, according to the Robert Wood Johnson Foundation (RWJF) county health rankings. Sullivan 180 was born out of a grassroots desire to do something as a community to improve the county’s ranking. Sullivan 180’s mission is to build a healthy, vibrant Sullivan County where everyone has a sense of purpose, connection to family and community and access to fresh foods and an active lifestyle. In an effort to support our residents in adopting healthier lifestyles, one of Sullivan 180’s first initiatives is to train and deploy volunteer Community Health Champions throughout the county. The Sullivan 180 Community Health Champions program is a new way that people within the community can get involved and improve their own health and that of their friends, co-workers and neighbors. he human eye works a lot like a camera, allowing us to focus on the larger picture of our surroundings or a smaller detail right in front of us. And having two eyes grants us incredible depth perception and protects us from perceived dangers.

It’s safe to say eyes are a pretty important aspect of a person’s life, and Dr. Maegan Sauer-Erlwein learned this pretty early on. “When I was younger I never really had my eyes checked. I of course did the school screenings, but that was it. When I was 16, I went to get my permit test and when they asked me to read the chart on the wall, I was squinting and I just couldn’t really see it,” stated Sauer-Erlwein. “My mom thought I was joking and said ‘Maegan cut it out, if you don’t read it you aren’t going to get your permit.’ I was eventually able to squint it out and pass but afterwards my mom felt so bad she took me to Walmart in Honesdale. That’s where I had my first eye exam.” Sauer-Erlwein admitted that she had always thought the blurriness was kind of normal. see that far. “I sat next to a friend of mine and would often copy her notes from the board because I couldn’t see them,” she said laughing. After her eye exam, the doctor put a pair of trial glasses on her. SauerErlwein said that when she walked out in the store she was astonished by what she could see. “All I could think was wow, you can see all the department signs and things hanging from the ceiling,” she stated. “The most amazing thing was when I walked outside and looked at the trees. I could see every individual leaf.” That was the turning point for Sauer-Erlwein and the moment she realized that just by giving a person a pair of glasses, you could change their life. You could change the way they see the world. And that’s just what she aims to do now that she opened up her very own Optometry office on Main Street in Jeffersonville, Eyes on Main St. Optometry.

Care is because the one thing that I’ve always been really passionate about is rural health care and accessibility to health care,� Sauer-Erlwein explained. “Studies show health care outcomes are poor in more rural areas because income is lower, sometimes education levels are lower. care in general. She completed all the data and analysis on what it means to be a rural Optometrist versus an urban Optometrist. She found that as an Optometrist working in a rural area, you need to be a specialist in nearly everything: Glaucoma, emaculate degeneration, dry eyes, lazy eyes, allergies, emergencies, and more. “As a rural Optometrist you need to be able to treat whatever walks through your door, because you’re the only one,� stated Sauer-Erlwein. “The next doctor or specialist might be an hour or more away, whereas when you’re in an urban setting you don’t treat as much because you have more specialists in a closer area that you can refer the patient to. I kind of knew that coming from a rural area but I wanted to actually document that.� So Sauer-Erlwein got the training
in Primary Eye Care so she could study and treat all of those areas. Walking around the Eyes on Main St. office, it’s clear Sauer-Erlwein takes what she does seriously and is knowledgeable and passionate in all aspects of her work. Influenza (Flu) Vaccine: What you need to know Information provided by the U.S. Department of Health and Human Services Centers for Disease Control and Prevention. Many Vaccine Information Statements are available in Spanish and other languages. See www.immunize.org/vis
Each year thousands of people in the United States die from flu, and many more are hospitalized. Flu vaccine can: • keep you from getting flu, • make flu less severe if you do get it, and • keep you from spreading flu to your family and other people.
1. Why get vaccinated? Influenza (“flu”) is a contagious disease that spreads around the United States every year, usually between October and May. Flu is caused by influenza viruses, and is spread mainly by coughing, sneezing, and close contact. Anyone can get flu. Flu strikes suddenly and can last several days. Symptoms vary by age, but can include: • fever/chills • sore throat • muscle aches • fatigue • cough • headache • runny or stuffy nose Flu can also lead to pneumonia and blood infections, and cause diarrhea and seizures in children. If you have a medical condition, such as heart or lung disease, flu can make it worse. Flu is more dangerous for some people. Infants and young children, people 65 years of age and older, pregnant women, and people with certain health conditions or a weakened immune system are at greatest risk.
2. Inactivated and recombinant flu vaccines A dose of flu vaccine is recommended every flu season. Children 6 months through 8 years of age may need two doses during the same flu season. Everyone else needs only one dose each flu season. Some inactivated flu vaccines contain a very small amount of a mercury-based preservative called thimerosal. Studies have not shown thimerosal in vaccines to be harmful, but flu vaccines that do not contain thimerosal are available. There is no live flu virus in flu shots. They cannot cause the flu. There are many flu viruses, and they are always changing. Each year a new flu vaccine is made to protect against three or four viruses that are likely to cause disease in the upcoming flu season. But even when the vaccine doesn’t exactly match these viruses, it may still provide some protection. Flu vaccine cannot prevent: • flu that is caused by a virus not covered by the vaccine, or • illnesses that look like flu but are not.
It takes about 2 weeks for protection to develop after vaccination, and protection lasts through the flu season. 3. Some people should not get this vaccine Tell the person who is giving you the vaccine: • If you have any severe, lifethreatening allergies. If you ever had a life-threatening allergic reaction after a dose of flu vaccine, or have a severe allergy to any part of this vaccine, you may be advised not to get vaccinated. Most, but not all, types of flu vaccine contain a small amount of egg protein. • If you ever had Guillain-Barré Syndrome (also called GBS). Some people with a history of GBS should not get this vaccine. This should be discussed with your doctor. • If you are not feeling well. It is usually okay to get flu vaccine when you have a mild illness, but you might be asked to come back when you feel better. 4. Risks of a vaccine reaction With any medicine, including vaccines, there is a chance of reactions. These are usually mild and go away on their own, but serious reactions are also possible. Most people who get a flu shot do not have any problems with it. Minor problems following a flu shot include: • soreness, redness, or swelling
where the shot was given • hoarseness • sore, red or itchy eyes • cough • fever • aches • headache • itching • fatigue If these problems occur, they usually begin soon after the shot and last 1 or 2 days. More serious problems following a flu shot can include the following: • There may be a small increased risk of Guillain-Barré Syndrome (GBS) after inactivated flu vaccine. This risk has been estimated at 1 or 2 additional cases per million people vaccinated. This is much lower than the risk of severe complications from flu, which can be prevented by flu vaccine. • Young children who get the flu shot along with pneumococcal vaccine (PCV13) and/or DTaP vaccine at the same time might be slightly more likely to have a seizure caused by fever. Ask your doctor for more information. Tell your doctor if a child who is getting flu vaccine has ever had a seizure. Problems that could happen after any injected vaccine: • People sometimes faint after a medical procedure, including vaccination. Sitting or lying down for about 15 minutes can help prevent fainting, and injuries caused by a fall. Tell your doctor if you feel dizzy, or
5. What if there is a serious reaction? What should I look for? • Look for anything that concerns you, such as signs of a severe allergic reaction, very high fever, or unusual behavior. Signs of a severe allergic reaction can include hives, swelling of the face and throat, difficulty breathing, a fast heartbeat, dizziness, and weakness. These would start a few minutes to a few hours after the vaccination. What should I do? • If you think it is a severe allergic reaction or other emergency that can’t wait, call 9-1-1 and get the person to the nearest hospital. Otherwise, call your doctor. • Reactions should be reported to the Vaccine Adverse Event Reporting System (VAERS). Your doctor should file this report, or you can do it yourself through the VAERS web site at

The National Vaccine Injury Compensation Program The National Vaccine Injury Compensation Program (VICP) is a federal program that was created to compensate people who may have been injured by certain vaccines. Persons who believe they may have been injured by a vaccine can learn about the program and about filing a claim by calling 1-800-338-2382 or visiting the VICP website at www.hrsa.gov/vaccinecompensation. There is a time limit to file a claim for compensation. 7. How can I learn more? • Ask your healthcare provider. He or she can give you the vaccine package insert or suggest other sources of information. • Call your local or state health department. • Contact the Centers for Disease Control and Prevention (CDC): - Call 1-800-232-4636 (1-800CDC-INFO) or - Visit CDC’s website at www.cdc.gov/flu.

n July 16, Sullivan Internal Medicine Group and Catskill Adult & Pediatric Medicine, together as Middletown Medical, merged their Monticello offices under one roof in the Thompson Square Mall, better known as the ShopRite Plaza, now offering Urgent Care. “Sullivan Internal Medicine Group and Catskill Adult & Pediatric Medicine are both high standing brands with a high patient loyalty,” stated Media Solstice Marketing & Communications Manager Caitlin Huebner, “but it was very disjointed. All the offices were in separate locations in Monticello although they were all Middletown Medical, so this brought everyone under the same roof." Having the offices of Middletown Medical in the same space allows easier accessibility for patients. There is a brand new CT scan as well as a laboratory on site so patients don’t have to be referred to another office or hospital, getting results much faster, and being located in the ShopRite Plaza allows patients that rely on trans-
portation to get grocery shopping done either before or after their doctors appointment. “Middletown Medical is also very unique in that, you can reserve your spot online or call ahead for Urgent Care,” said Huebner. “That way when you come in, wait time is minimal." Other specialties besides Urgent Care that are offered in the new location include: Pediatrics, Cardiology, Gastroenterology, Orthopedics, Pulmonary Medicine, and Endocrinology.
