In this edition, we are spotlighting Breast Cancer Awareness month. “Breast Cancer: Attacking the very Bosom of a Women’s Femininity” is an excellent inspirational article written by breast cancer survivor Paula Flory of the YWCA of Princeton, NJ. Once you have read Paula’s inspiring words, then read the article about breast reconstruction followed by the uplifting message in the article entitled “It’s Time”. Breast cancer survivors, we honor YOU! Lastly, the Inaugural Edition is dedicated to my grandparents, Jacquelyn (Jackie) Cameron and Robert Cameron. The two of them moved to Somerset, New Jersey in 1970 with their four children and remained here until their passing. Jackie Cameron passed in 2002 after an outstanding career as an educator in the New Jersey School system for over 30 years. I am elated to continue the legacy of education as I use this powerful platform to Inform, Educate and Empower our Somerset County community.

Kelly McPadden, RT(R)(T), LMT, HC Kelly McPadden has trained as a radiologic technologist and has practiced for over twenty years. Early in her career she took additional studies in women’s imaging, specializing in mammography and is licensed through the ARRT. Her training at The Institute for Integrated Nutrition combines a mind/ body approach to health and nutrition. Kelly is also a New Jersey licensed massage therapist. She has a special interest in helping clients that are dealing with complicated pain issues, but specialize in injury recovery and prevention. Venita S. Welcome Venita S. Welcome is the President/Clinical Director of Christian Wellness Center of NJ (CWCNJ) which was established in 2003 as a non-denominational counseling center. The Center provides counseling for people residing in Central New Jersey and surrounding counties. Mrs. Welcome provides counseling services to address the needs of the community and faith-based organizations, for example, chronic illness, individual, couples, family counseling, and spiritual counseling. Depression, grief and loss, anxiety, addictions, chronic illness and intimate partner violence are a few of the topics that are addressed in counseling. Mrs. Welcome established CWCNJ while working for a domestic violence center. She saw women struggle with their faith and survival. She designed CWCNJ to be a safe haven that ensures confidentiality and a non-judgmental environment. She continues to be a visionary and organizer in creating innovative programs/workshops that address today’s issues. Non-traditional support groups are topic-focused and provide support and encouragement while building small group communities. One such support group that is being launched on October 6, 2013 is for people in the Somerset area diagnosed with pulmonary hypertension. Mrs. Welcome recognizes the importance of community and strives to establish relationships with small business owners working towards helping people overcome life challenges. She is constantly watching for ways to include or join up with people and organizations to offer her clientele one-stop services. There are many personal and work-related challenges that Mrs. Welcome experienced and continues to overcome, including being recently diagnosed with pulmonary hypertension and interstitial lung disease. In spite of personal challenges, she continues to strive to help people make healthy choices. She is currently pursuing a doctorate degree in Pastoral Community Counseling. Mrs. Welcome is licensed as a professional counselor by the NJ State Board of Marriage and Family Therapy. She holds certifications as a victim chaplain, relationship counselor and life coach. Mrs. Welcome enjoys jazz concerts, bicycle riding, reading, learning and exploring new things and spending time with her husband.

Health andMedical By Lawrence Davanzo, DO, FCCP, FAASM Dr. Lawrence Davanzo is Board Certified in Critical Care Medicine, Pulmonary Medicine and Sleep Medicine. He is a graduate of UMDNJ – School of Osteopathic Medicine, completed his residency and fellowship training at UMDNJ – Robert Wood Johnson Medical School. He has a fellowship with the American College of Chest Physicians and the American Academy of Sleep Medicine. He is a member of the American Thoracic Society, the American Lung Association and the NJ Sleep Society. He is the Clinical Assistant Professor of Medicine at Robert Wood Johnson Medical School. Dr. Davazo is owner of Brunswick Pulmonary & Sleep Medicine and the Medical Director for the Garden State Sleep Center. Lung Disease & Sleep Affects On Women

Chronic obstructive pulmonary disease (COPD), lung cancer, and asthma are three of the most common lung diseases in women. Chronic obstructive pulmonary disease More than twice as many women as men are now diagnosed with chronic bronchitis. The rate of emphysema among women has increased by 5 percent in recent years but has decreased among men. Researchers are trying to understand why. Cigarette smoking, a main cause of COPD, has increased among women. One theory is that cigarette smoke is more damaging to women than to men. Lung cancer In the United States, the mortality rate for lung cancer among women is higher than any other type of cancer. Tobacco use is the major cause of lung cancer. Asthma Women are more likely than men to have asthma. The percentage of women, especially young women, with asthma is rising in the United States. Researchers are not sure why. Many experts think that air pollution and allergens play a role in this increase. Breathing tobacco smoke also is linked to an increased risk of asthma. Treatment Complex pulmonary diseases, many of which do not have a cure, can be managed by reducing symptoms, such as shortness of breath, therefore improving quality of life. It is important to locate a fully functioning comprehensive Pulmonary Function Laboratory to assist in the diagnosis of all types of pulmonary diseases. Ensure that this laboratory is experienced and has certified respiratory therapists

to perform all types of testing. Having an extensive Pulmonary Hypertension program working with cardiologists, internists and family practitioners will help to identify these pulmonary patients early and start treatment as soon as possible. In addition, a Pulmonary Rehabilitation program for patients with chronic lung disease can help reduce shortness of breath, improve quality of life, and ultimately reduce healthcare costs by promoting wellness and prevention.

Women and Sleep Sleep is a basic human need, as important for good health as diet and exercise. When we sleep, our bodies rest but our brains are active. Although most people need seven to nine hours of sleep each night to function well the next day, the National Sleep Foundation (NSF) Women and Sleep Poll found that the average woman aged 30-60 sleeps only six hours and forty-one minutes during the workweek. A more recent NSF Sleep in America poll of all adults revealed that women are more likely than men to have difficulty falling and staying asleep and to experience more daytime sleepiness at least a few nights/days a week. Research has shown that too little sleep results in daytime sleepiness, increased accidents, problems concentrating, and poor performance on the job and in school, and possibly, increased sickness and weight gain.

When people lose a hormone called insulin they have a condition called diabetes and they treat this condition with replacing the hormone. The same is true when the thyroid hormone is diminished. The unfortunate fact is that each day women starting at age 35 are beginning to lose very important hormones and they are ignored. Conventional medicine is often not well versed on the important role that estrogen and progesterone play, not only in the way women feel, but also in that diminished hormones can negatively affect overall health. Estrogen, for example, has 400 functions in the female body, so when women decrease their hormones, they often become forgetful, lose energy, gain belly fat, become melancholy or depressed, see increased wrinkles and unwanted hair growth on their skin. Simple Steps to Reverse Diabetes Naturally

According to the Centers for Disease Control, 26 million Americans currently have diabetes and another 79 million have prediabetes. Once you understand what is happening in your body, it becomes easy to be committed to the lifestyle and behavior modifications required to get you back to optimal health and normal blood sugars. What is diabetes and prediabetes? Diabetes is the inappropriate regulation of blood sugar in the body. Normally the body digests food by breaking it down into fuel, glucose. Glucose then enters the blood stream with the goal of entering the cells. In response to glucose, the pancreas releases insulin, which binds to cells and allows the easy passage of glucose from the blood stream into the cell. The amount of insulin released by the pancreas is in direct proportion to the amount of glucose in the blood stream. When someone has diabetes this process is impaired and glucose has trouble getting out of the blood stream and into the cell. Pre-diabetes occurs when your body is having some difficulty regulating your blood glucose levels and without immediate action you will be diagnosed with diabetes in the future.

Diabetes diagnosis – emotions running wild The emotions that accompany diabetes or pre-diabetes diagnoses are often not considered by your doctor, but they are a crucial part of the treatment plan. Common emotions you might feel are panic, denial, confusion, frustration, anger and fear. These emotions are all born out of a lack of control over your health situation. I believe that the first step to effectively treating diabetes is to recognize and manage your emotions about the diagnosis so they will not sabotage your attempts to stick to your treatment plan and lifestyle changes. Medications are not the enemy The next step is to get your blood glucose levels back to normal ranges. High blood sugars need to be regulated to prevent irreversible damage to your body. This is handled in one of two ways: medications or lifestyle changes. Many are put on medications because their current lifestyles are contributing to high blood sugar levels. Lifestyle changes can reverse diabetes, but it requires working with an expert and allowing time to incorporate new behaviors into your everyday life. Normalizing blood sugars is the key to reversing diabetes. It might seem shocking for a naturopathic doctor to recommend medications but medications are not the enemy, high blood sugars are! When blood sugars are high, the glucose in the blood causes damage to blood vessels and organs. Can’t remember where you placed your keys? Having trouble recalling a friend’s name? Have you ever gone to another room and forgotten what you were there to get? Don’t worry - these moments happen to just about every woman with responsibilities. Most of the time, after retracing your path, you remember where you put your keys, your friends name pops into your head and something triggers why you went into the room down the hall. There is a big difference between a temporary loss of recall and the permanent, progressive, life altering disease known as dementia. Dementia is an illness that usually involves a progress of deterioration. Initial signs include changes in the way a person behaves and communicates. There are increased episodes of confusion as well as memory loss that negatively affects day to day functioning. Noted cognitive symptoms include impaired decision making, difficulty learning new skills and poor problem solving. Emotionally, there can be changes as well to include fear, insecurity, anger and often symptoms mimicking depression. Memory problems alone do not constitute a dementia diagnosis, rather there have to be multiple symptoms to meet criteria. There are over 100 types of dementia. There are progressive dementias which result in permanent brain deterioration and there are some causes of dementia or dementia like symptoms that can be reversed. The most common type of progressive dementia, Alzheimer’s Disease (AD), will be discussed as well as how it affects women. According to the 2012 Alzheimer’s Disease Facts and Figures produced by the Alzheimer’s Association, 60 to 80% of those with dementia are diagnosed with Alzheimer’s

disease; 65% of those with AD are women. A discussion on women and dementia cannot be had without mentioning the Shriver Report: A Study by Maria Shriver and the Alzheimer’s Association. Alarming statistics were produced by this report including that 10 million women either have Alzheimer’s or are caring for someone with it. The report suggests that because of AD women are, in fact a gender under siege due to the increase in occurrence as well as the fact that women are most likely to become a caregiver to someone with AD. There are women who have children, work and are caring for a parent(s) with the disease. This can add up to a very difficult scenario for what is called the sandwich generation. The Shriver report talks about how much we need to focus on the sixth largest killer of American women and how we need to bring attention to how the numbers are increasing to an epidemic proportion.

Dr. Makai earned her medical degree from Columbia University College of Physicians and Surgeons, where she won the Thomas F. Cook Prize for Excellence in Obstetrics and Gynecology. She completed her residency in Obstetrics and Gynecology at the combined program between Brigham and Women’s and Massachusetts General Hospitals in Boston, Mass. After residency, Dr. Makai completed a Fellowship in Gynecologic Endoscopy at Newton-Wellesley Hospital in Newton, Mass.

Fibroids

Fibroids are non-cancerous (benign) tumors of the uterus. While many reproductive-age women have uterine fibroids, less than half of these women are symptomatic. Still, hundreds of thousands of doctors’ visits each year are attributed to uterine fibroids and the problems they cause, and more than 600,000 hysterectomies are performed each year in the U.S. as a treatment for fibroids. Numerous studies have tried to determine who gets fibroid tumors and why. They tend to run in families, and black women are more likely to have fibroids than white women. In addition, black women may develop symptoms earlier than others, and grow more and larger fibroids than others. While there are some associations between foods and fibroid development, these tumors are not easily preventable or avoidable with lifestyle changes alone. Fibroids develop from the muscle cells in the uterus – each tumor growing from one muscle cell which starts to grow abnormally. For this reason, these tumors usually take a while to grow to a large size. They are hormonallysensitive tumors, and may continue to grow while a woman is menstruating, They tend to shrink at the time of menopause. Fibroids are not cancerous, and they are highly unlikely to turn into cancer. The size, number and location of uterine fibroids affect the symptoms women have as well as their treatment options. Common symptoms caused by fibroids include abnormal uterine bleeding, pain and pelvic pressure, bladder and bowel problems (if large enough), and fertility-associated

problems. Along with a pelvic exam, pelvic ultrasound, MRI and office hysteroscopy are common ways for doctors to exam fibroids. While many fibroids may never be noticed by a woman and warrant no treatment, there are many medical and surgical treatment options for women who do have symptoms. Those options range from hormonal treatments, to anti-bleeding medications, to pain control options . Many minimallyinvasive surgical treatments are available to women as well, and special consideration is given to women wishing to have children after fibroid treatment. No medical regimens are currently available to “cure” fibroids.

Hysteroscopy can be performed in the doctor’s office, where women can find out immediately if they have fibroids in the lining of the uterus. This information is

Before joining the agency, he served for 10 years as CEO of Morristown Neighborhood House Association, where he oversaw programs such as gang prevention, youth counseling, pre-schools and community outreach and secured government contracts, foundation grants and individual donations. He was also General Counsel for The Miracle Makers Inc., in Bedford-Stuyvesant, Brooklyn, New York. and directed programs for children and families living with HIV/ AIDS at Hope House of The Palm Beaches in Florida.

When there’s nowhere else to turn, Somerset Home gives homeless teens a home and a hopeful future. James doesn’t remember his mother. She died of a drug overdose when he was six. Relatives took him in, but when the little boy became a burden, he was sent to a series of foster homes. When he turned 18 and “aged out” of the child welfare system, he had nowhere to go, no support system and no skills or resources to live on his own. But unlike thousands of other traumatized teens in New Jersey, James was accepted in My Place, a residential transitional-living program run by Somerset Home for Temporarily Displaced Children. Counseling, part-time work and independent-living skills workshops have helped him heal and take the first steps toward independence. James is just one of the thousands of abused, neglected and homeless teens who have found a safe haven and support services in one of Somerset Home’s seven programs for young adults. Opened in 1973, the Bridgewater, N.J., nonprofit has helped thousands of teens from around the state put the past behind them and begin to build a brighter future. A lot has changed in four decades. Somerset Home started with one house for 12 youth. Today it has multiple buildings for 41 teenagers and young adults. And its services have also evolved as needs changed: an in-house school, HIV/ AIDS education, financial literacy workshops, a scholarship fund and permanent supportive-housing for young adults with nowhere to go…all designed to help residents heal emotionally and learn to live independently.

From the start, the agency has worked with local community groups and the N.J. Department of Children and Families to help teens deal with their difficult family situations, continue their education and learn the skills they need to move forward with their lives. Individual, group and family counseling provided by Somerset Home’s behavioral health department service agencies is a key component meeting the needs of clients and their families. One area in which the need is growing is street outreach. Funded by a federal grant, Somerset Home’s Street Smart outreach program began in 1998 and works with youth living on the street to find them safe housing and direct them to services for health care, counseling, education and jobs. Outreach workers also meet with school guidance counselors, police officers and other social service agencies to explain the services and ensure those who come in contact with teens living on the street or in other unsafe situations know Somerset Home’s outreach services are available.

These victims are ‘sold’ at truck stops, casinos, on internet sites and street corners. “We talk with them, offer blankets and toiletries, and over time build their trust and help get them to safety,” says Outreach Worker Tayeesha Ruff. Not everyone is ready to accept Somerset Home’s services, but for those who are, the result can be life-changing. As one client explained, “I had nowhere to go and no one to call. [The outreach workers] helped me when I needed it most and I will never forget them for what they did.” “For 40 years, Somerset Home has been giving homeless youth a home and a hopeful future, says Executive Director David Walker. And with the continued support of a caring community, we will be here for another 40 years to offer a safe haven to teens who have nowhere else to turn.” Somerset Home provides

In the past, outreach counselors focused on Somerset County, but a grant from the N.J. Department of Children and Families will allow them to expand into Essex, Bergen, Hudson, Middlesex and Union counties beginning this month. The grant is part of the state’s effort to combat the growing problem of human trafficking. According to the National Center for Missing and Exploited Children, in 2012 one in eight endangered runaways were likely sex trafficking victims, sold into sex slavery by pimps, organized crime members and even their parents. Many homeless and runaway youth trade sex for money, food, or shelter in order to survive. Others

The organization’s support groups, wig and prosthesis boutique, wellness program, and outreach education initiatives have been pivotal in the recovery process of women for 40+ years. Paula is a Stage III breast cancer survivor. She has a Masters in Counseling Psychology from NYU and multiple certifications in exercise physiology. Flooded with images of breathtaking women every day, we begin to believe that despite the fact that we may be intelligent, compassionate, interesting, and ambitious, we are not feminine or sexy unless we can fill a lacy cupped bra and have a full head of luscious hair. So when a woman learns that she has breast cancer, it is the rare kind of person who does not feel the sting of losing some or all of the parts of herself that make her feel like a “real” woman. Regaining Your Femininity Most women will experience sadness at having to accept their new body, and others a period of mourning. There are things that a woman can do and places she can go to ameliorate this sense of loss of femininity that often accompanies breast cancer.

Hair Loss If you go through chemotherapy, you will inevitably have to deal with hair loss. Cut your hair short before treatments so that you won’t lose large clumps as you brush your hair. Seek a wig that you love and that makes you feel pretty for later in your treatments. When your hair thins, take control and shave your head, or have your hairdresser or a friend shave it for you. For some women, this is empowering and a way to declare, “Hey cancer, I’m tougher than you are!” Wrapping your head in a gorgeous scarf is another way to feel feminine and stylish. You don’t need to buy “cancer” scarves. Experiment with different shapes and various patterns. When you wear a wig or scarf, you’ll be surprised at how cool and stylish you’ll look! Prosthetic Breasts If you’ve had a mastectomy without reconstruction, then

wearing prosthetics may help you feel more feminine. Mastectomy bras are like regular bras, only there is a pocket for a prosthetic. Look for bras that make you feel attractive and pretty. There is no need to stick to a utilitarian bra style just because you have lost a breast.

Remember, you are a uniquely sexy woman now. Own it and rock a lacy bra under your clothes. So what if you are the only one who knows what your undergarments look like? After all, the most important relationship we have in life is with oneself. Developing a Support Network Join a breast cancer support group that meets your needs. There are groups for the newly diagnosed, young women, different ethnicities, and different stages of breast cancer. You can even find groups geographically and in a variety of settings (from hospitals, to homebased groups, to retreats).

Recent studies have demonstrated that only about 33% of women diagnosed with breast cancer will receive breast reconstruction following removal of all or part of the breast. This number seems alarmingly low when one considers the advancements and benefits that breast reconstruction can provide. Advancements in breast surgery have occurred based on our improved understanding tumor biology as well as our ability to reconstruct a breast with very natural shape, contour, and volume. Some of the advancements include partial mastectomy instead of total mastectomy, oncoplastic surgery, nipple-sparing mastectomy, autologous reconstruction using perforator flaps, and prosthetic reconstruction with various shaped devices and biologic materials. Some of the benefits of breast reconstruction include improved body image, improved selfesteem, reassurance that a normal appearance can be achieved, and a willingness to educate and help other women who have to make many difficult decisions regarding their reconstructive options. Who are Candidates for Breast Reconstruction? All women that are diagnosed with breast cancer that will be having surgical removal or the tumor or breast (partial or total mastectomy), should be provided the opportunity to consult with a plastic surgeon. Not all women will need breast reconstruction because the majority of women with breast cancer will choose to have breast conservation in which the treatment is a lumpectomy (tumor excision with a 1-2 mm margin) and postoperative radiation therapy without the need for reconstruction. However, in women that will need a partial mastectomy (tumor excision

with 1 – 3 cm margin) or total mastectomy (complete removal of the breast tissue), most will be able to undergo either immediate reconstruction (at the same time as the partial or total mastectomy) or delayed reconstruction (following the partial or total mastectomy). The main advantage of immediate reconstruction is that a woman will go to sleep with her breast and wake up with a new breast. Candidates for immediate reconstruction typically include those with early to mid stage breast cancers that are generally healthy. Delayed reconstruction is usually considered in women that have very advanced cancers or who have significant health related conditions such as cardiac disease, refractory tobacco use, or morbid obesity. The good news is that breast reconstruction will not interfere or impede with breast cancer surveillance. The reconstructive options for immediate and delayed reconstruction are generally the same; however, in women that had had prior radiation therapy, the reconstructive options may have to be individually tailored. Partial Mastectomy and Oncoplastic Surgery Oncoplastic surgery is a relative new reconstructive option that consists of partial mastectomy and immediate or staged-immediate breast reshaping. Stagedimmediate breast reshaping is considered in some women when total removal of the breast cancer is uncertain immediately following tumor removal. In these cases, once the pathology confirms complete removal, the reshaping is scheduled. The options for breast reshaping include breast reduction, mastopexy, use of implants, or the use of flaps. Breast reduction is most often considered in women

with large or ptotic breasts that would otherwise be good candidates for this operation. Even though the breast cancer may be in only one breast, the reshaping procedures are typically performed on both breasts at the same time in order to achieve balance and symmetry. A required component of oncoplastic surgery is postoperative radiation therapy. The purpose of radiation is to destroy microscopic cancer cells potentially remaining in the breast. The goal of oncoplastic surgery is to perform the breast reshaping prior to the radiation therapy. This is because revising a breast after radiation is associated with more difficulty and can sometimes result in delayed healing and suboptimal contour. The advantages of these oncoplastic procedures are that patient satisfaction is very high because a mastectomy is avoided, the nipple areolar complex is usually preserved, and surgical outcomes are usually excellent. Nipple Sparing Mastectomy In most women with breast cancer that are having a mastectomy, a skin sparing (SSM) approach is used. With this technique, the nipple and the areola are removed with a small rim of adjacent breast skin. Nipple sparing mastectomy (NSM) is relatively novel approach to improving surgical and aesthetic outcomes following total mastectomy. With this technique, the nipple and the areola are not removed. Although NSM has aesthetic advantages, it is not recommended for all women having mastectomy. This approach is frequently performed in women having prophylactic mastectomy (no breast cancer) who have mild to moderate breast volume. Women with “D” cup or larger breasts are sometimes not

candidates because of the risk of nipple death or necrosis. NSM is sometimes recommended for women with breast cancer that have certain tumor and breast characteristics. The primary tumor characteristics are that the tumor be less than 2.5 cm in diameter and that it be at least 3 cm away from the nipple-areolar complex. These distances are recommended to reduce the risk that tumor cells would be left behind following the mastectomy because many of the sites where the tumors are located are ultimately connected to the nipple via a systems of ducts. Consultation with a breast and plastic surgeon is necessary to determine candidacy for NSM. It is important to keep in mind that whether the mastectomy is nipple sparing or skin sparing, excellent aesthetic outcomes are possible. Reconstruction following NSM can be completed using either autologous tissue or prosthetic devices. Autologous Breast Reconstruction Breast reconstruction using ones own body tissues has been available since the early 1980’s. The primary donor sites at that time were the back (latissimus dorsi flap) and the abdomen (TRAM flap). Both of these options are still in use today and utilize the muscle, fat, and skin in the area. The tissues are tunneled under a skin bridge to the breast pocket and shaped into a breast. These flaps can create a nicely shaped breast but occasionally result in adverse events related to the donor sites because the muscle has been removed. Recent advancements with breast reconstruction using ones own tissues include the use of more donor sites without the need to remove any of the muscle. The

In 2012, Dr Nahabedian chaired three national symposia that included the 4th Annual Abdominal Wall Reconstruction Conference, the Santa Fe Breast and Body Contouring Symposia, and the Challenging Complications in Plastic Surgery Conference. Dr. Nahabedian has also been a visiting professor 55 times over the past 8 years at various universities and hospitals around the country.

Considering Your Options Women considering these options should be seen in consultation with a board certified plastic surgeon.

reconstructions are performed using only skin and fat and are referred to as perforator flaps. It is the skin and fat that make up the important components of a reconstructed breast. The most common and popular perforator flap is the DIEP flap (deep inferior epigastric artery perforator) but other options include the SIEA (superficial inferior epigastric artery), SGAP (superior gluteal artery perforator), TAP (thoracodorsal artery perforator), IGAP (inferior gluteal artery perforator), and the PAP (profunda artery perforator). Although these perforator flaps are capable of producing excellent results, they are associated with more complexity because most require microsurgery. A small artery and vein associated with the flap has to be attached to a small artery and vein at the breast site using an operating microscope. These flaps can be performed for unilateral or bilateral breast reconstructions and typically require 4-6 hours and 6-10 hours respectively depending on the skill and experience of your plastic surgeon. The main risks associated with these flaps is that there is a 1-3% chance of failure and a 4-6% chance of a donor site abnormality such as a bulge or fluid collection. The main advantage is that the reconstructed breast is usually soft and supple, resembles the original breast, and lasts forever. Women are usually able to continue with there normal activities without restriction.

Prosthetic Reconstruction Breast reconstruction using prosthetic devices remains the most common method of reconstruction in the USA and is utilized in approximately 75% of women. This is because it is relatively simple, has the potential for excellent aesthetic outcomes, and allows women to resume their normal activities sooner than some of the more complex operations. Ideal candidates for prosthetic reconstruction include women that are generally healthy, have a body mass index less than 30, and have mild to moderate breast volume. These operations can be performed in 1 or 2 stages. With the 1-stage reconstruction, a permanent implant is inserted following the mastectomy. This usually requires close coordination between the breast and plastic surgeon to ensure that the mastectomy is performed accordingly. With the 2-stage reconstruction, a tissue expander is temporarily inserted to either maintain the space or to further stretch the remaining breast skin following the mastectomy. The tissue expander is partially inflated during the initial operation and further expanded in the office. Once complete, the tissue expander is removed and replaced with a permanent implant. The permanent implants come in a variety of shapes, sizes, and types. They can be round or contoured, saline or silicone gel filled, smooth or textured surface. Selection of the optimal device will depend on the woman’s body and breast characteristics as well as the experience of the plastic surgeon.

reconstructive process using prosthetic devices. These cadaveric skin materials have been demonstrated to possess regenerative properties and have been useful for improving surgical outcomes. A key point to remember with prosthetic devices is that they do not last forever. Most devices will last somewhere between 10 and 15 years so additional operations are typically required. The main adverse events associated with these devices include but are not limited to infection, capsular contracture, and rupture. It is also important to remember that silicone gel devices have been cleared by the FDA and deemed safe and effective for this purpose. Silicone gel implants are ideal for women that have had a mastectomy because the remain skin and fat of the breast is sparse and the silicone gel mimics the normal breast feel more closely that saline implants. With these recent advancements in breast cancer awareness and reconstruction, more women are now opting for breast reconstruction following mastectomy. The techniques have improved, as have the results. Breast reconstruction is one of the best ways to facilitate the

transition from being a breast cancer victim to a breast cancer survivor. Testimonial I developed an inverted nipple in 2006 but didn’t think much about it until one morning I woke up with my T-shirt dappled in blood stains in the chest area. A mammogram revealed abnormalities and was told I should see a surgeon as soon as possible. I began my search and knowing Georgetown had great reputation I called Dr. Shawna Willey office. I was in her office the next day. Six months later I developed breast cancer in the opposite beast, and after two lumpectomies Dr. Willey sent me to Dr. Nahabedian to discuss DIEP Flap surgery. At the time Dr. Nahabedian was the only surgeon in the area who had the knowledge and expertise to do DIEP Flap surgery. I underwent mastectomy by Dr.Willey and followed by immediate DIEP Flap by Dr. Nahabedian. I am forever grateful for the concern and care I received and count myself very lucky to have had Dr. Nahabedian care for me. It’s Time

N

Not understanding the “Why’s” isn’t always easy. At times, trying to figure out why things happen may be what is hindering us and holding us back. The time spent in looking back, keeps us from moving forward. One can never move to a better place if they are staying rooted in the negative experiences of the present and the past. There may, or may not, be a logical explanation as to why someone may be experiencing the challenges that they are facing. Our challenges can, and have in many cases, taken us to a place of pain, disappointment, hurt, and not being able to fully enjoy life. Many people are now faced with becoming full-time care givers, where there is no plan in place. Many people are feeling unsatisfied in their love relationships. Many people are concerned about their economic situation and providing for their families. Cancer has become so common among us, that it clearly has no regard for age, race, or one’s economic status. The challenges out there in living dayto-day are many, and sometimes the answers appear to be few. Have you ever heard the saying, “As long as you have a breath of air, you have hope”? It’s true, but you have to want to live in the now. Making the best of each moment regardless of how hard it may appear, pressing forward with an expectation that your situation will get better. Everything is not lost, it is not all hopeless. But in order to move to a place of having hope, from being in the place of being hopeless, you have to believe that your situation CAN change. That first step of belief is critical to moving forward; it’s the direction you want to be in. You must move to a place of overcoming. Quick answers and quick fixes don’t exist; it’s not what life is about. Many of the trials and tribulations that we face are experiences that we would

rather not go through if we had the choice. But there are lessons to be learned in every experience. Being unemployed and having to live on very little, teaches the lesson of being a good steward and manager of resources when the money does return. Having a failed relationship allows for the opportunity to be introspective and self-critical, so that we can evolve and be better partners in our next relationships. But you have to be open. You have to look for the lesson in every experience so that you can grow from it and move to a better place. Where you are in this moment does not define who you are and what you can achieve. Choose to not to give up, choose to press forward. Tough times don’t last, tough people do. Don’t allow yourself for a minute to believe that your circumstance can’t and won’t change. How do I know?? I know because I’ve had to make that journey first hand. I had to fight to figure out what a new normal was for me. Through the battle of breast cancer, and literally coming close to death as I fought for that next day, it was a pressing forward time. But I did it, and it wasn’t always happy moments. It was no easy thing to do, but I needed and wanted to be different and to be better. Words have tremendous power. Speak life into your circumstance, and seek the peace to know that it’s okay to take that step forward. Don’t look back, other than to see how far you’ve come. IT’S TIME!

“Planning for the 2nd Annual Youth Explosion is well underway,” says Gail Henderson, Founder of the Family Outreach of New Jersey (FORNJ) nonprofit organization. “The purpose of the Youth Explosion is to bring together our community, people from all walks of life, to support and encourage our youth while they showcase their talents as we raise funds to help those less fortunate.” All proceeds raised from the Youth Explosion are used to benefit underprivileged families of New Jersey. The Youth Explosion provides a positive outlet for children to gain confidence in their

abilities while being supported by family, friends and the community. This exciting event hopes to empower, encourage, and engage the community. Last year, over 400 people came out to support the children in this mega talent show and to watch 13 acts vie for a first place cash prize of $1,000. Between the talents, FORNJ provided entertainment from comedians to local singers to a well-known Brazilian pianist and percussionist to a flash mob dance number. Vendors and food were also onsite. The energy in the building was positively electric. Last year’s winners, The Finger Singers, practiced consistently

throughout the summer. They used sign language while dancing to songs by Michael Jackson and the Jackson Five to mesmerize the audience. Even children who may have been disappointed because they did not win are coming back this year to try again. What is extremely powerful is the peer support from the youth in the audience that truly shows these children care for one another and want to see each other do well. On October 26, 2013, the Youth Explosion will be held at the Ukrainian Cultural Center on Davidson Avenue in Somerset, New Jersey. Children aged 13-18 and 19-25 will compete to win

separate cash prizes of $1,000. Auditions were held in May and ended in August. What do these football analogies mean? Why should women care? Sports analogies are commonly used in business environments to build camaraderie among employees (the team), identify responsibilities (player positions) and communicate the strategy (game plan) to meet business objectives (Win!). They aim to connect everyone to the emotions generated in sporting competitions. They can be effective in engaging staff and creating the competitive spirit that we see on the football field. However, if the audience does not understand the sport, the message is not conveyed. This is the case for many women when football analogies are used in the conference room. Even some women who watch their alma maters play football or tune in for the NFL’s Super Bowl, do not understand the fundamentals of the sport. Therefore, they tune out when the analogies from the sport are used, laugh at inappropriate times or do not actively participate in the conversations. All of these actions are not conducive to building rapport with colleagues and getting positive attention from your superiors. Football is played between two teams with the object being to score the most points. Each team has three units: offense (move the ball to score), defense (stop the other team from scoring) and special teams (handle all kicking situations). Each team can only have 11 players on the field at one time. Since businesses focus on increasing revenue (scoring), we are going to review the offense. Offense The goal of the offense is to move the ball down the field to the defense’s end zone to score

Extra Point (1 point) – Kicking the ball from the opponent’s 2 yard line through the goal post after scoring a touchdown.

Field Goal (3 points) – Kicking the ball from anywhere on the field through the goal post. Understanding the offense provides clarity to these analogies. “We’re at first and 10.” - You have four attempts to gain 10 more yards and earn a new set of downs. You are at the beginning of a new assignment and can pace yourself to complete the next task. (Think of first downs as tasks, scoring as short-term goals and a victory as long-term goals.) “Its third and long.” - This is usually used when the offense has more than 8 yards to gain for a first down. The results of this attempt will decide the strategy

for the fourth down. This means that you are in a difficult situation, but you could pull it off. To be successful, your strategy must be one that gains a lot of yards fast because you do not have a lot of time or opportunities to reach the goal. So, your plan must sell more products immediately, not over the next five years. “Its fourth and inches (or goal).” – This is your final attempt to get a first down or score. You are close, but if you fail, the competition gets the ball. (The pressure is on!) Many times, teams elect to punt the ball in this situation because they want the other team to start their drive as far from their end zone as possible. So, if the coach elects not to punt, it demonstrates that he believes that his team can accomplish the goal during an intense situation. Don’t let him down! Community andEducation Presented By Yvette Long Yvette Long is the founder and director of Platinum Minds and the president of ASPIRE, a teen development company. She is a certificated psychologist, a former fashion model, certified teacher and guidance counselor. She holds a Bachelor of Arts degree in sociology from Thomas Edison State College and a Master’s degree in student guidance services from Montclair State University. Yvette’s skill set includes training, public speaking, executive management, event management, fundraising, volunteer management and project management. She is the proud recipient of a number of awards including the Boy Scouts of America Tribute to Women Award (recognized for excellence in working with boys) and the Model Citizen Award in Morristown, NJ. Inspired Approach To An Urban Problem

Platinum Minds is a nonprofit leadership development organization that puts on programs for academically motivated boys from select urban communities in New Jersey. The organization has a small staff and volunteer team who have assisted over 300 boys and assembled a small but growing core of Platinum Minds “scholars” who are smart, talented and passionate about making a difference. The organization is the brainchild of founder and director Yvette Long, A few years ago, Yvette read a news article describing deplorable environmental conditions in urban school districts. The article described a hostile educational environment, overcrowded classrooms, stressed teachers, and inadequate course work that should lead to success in our current economic environment. The conditions serve as a contributing factor leading to the influence of street gangs and alarming rates of incarceration among urban youth. To address this issue the concept of Platinum Minds began to take shape about six years ago. Platinum Minds provides life skills and educational services to a select group of boys in order to set them on a better path to a fulfilling future and instill in them a sense of confidence, leadership and integrity. The organization provides educational assistance, leadership training, summer camp opportunities, mentoring and community outreach initiatives. The non-profit works with school administrators, teachers and guidance counselors to identify boys willing to take part in the program. The staff works with parents to provide support for their son’s development. The Secondary School Scholarship

Initiative is central to Platinum Minds programs. Its purpose is to assist boy scholars with the application process for admission to top secondary schools where the scholars would be exposed to higher quality educational opportunities and new experiences. Of equal significance is the Platinum Minds Arete’ Leaders Initiative. The goal is to develop future community leaders by encouraging them to become community-conscious individuals. One method to achieve this is through the Frederick Douglas Reading Program, where secondary school students help primary school students develop reading and comprehension skills. This effort has been tremendously successful for the older boys in developing leadership skills and provides living, breathing examples of the advantages of integrity and scholarship for the impressionable younger students. The Platinum Minds Summer Camp Initiative is also viewed as an important element to develop well-rounded young men. For many of the scholars, it is a rare opportunity to learn

to communicate and form teambuilding skills with others from diverse backgrounds. The camp experience exposes students to activities and interests they may not otherwise develop. It serves as the foundation to develop strong bonds, networking opportunities, and life-long friendships. It has the added advantage of training the boys to think in terms of solutions for challenges they are likely to face in life and in the workplace. These camp experiences also stress the importance of nutrition and a healthy lifestyle. Rounding out the program initiatives is the effort where scholars are paired with committed mentors. Mentors dedicate their time and energy to modeling positive behavior while providing life-changing support and guidance. The Platinum Minds model works. To date, 100% of the students have gone on to college. Platinum Minds does not support students beyond high school but a few graduated scholars return to volunteer their time and offer their testimonies. News is spreading about Platinum Minds programs

- and more urban students are becoming interested in scholarship and leadership development as evidenced by the steady increased turnout at Platinum Minds symposiums held in urban areas such as Newark, Morristown and Asbury Park. The biggest challenge is fundraising. Money is needed to support the various initiatives, as well as other needs, such as transportation, meeting space accommodations, and supplies. Current funding is achieved through organized fundraiser efforts, generous donations, and a few small grants. In order to ensure long lasting sustainability Platinum Minds is seeking to attract philanthropists with the resources to underwrite the organization for two years with annual contributions of $150,000. Such funds would allow the organization to build capacity and focus on development rather than fundraising. Sweet Red Wines: More Than A New Fad

The most commonly asked question asked to me as a waiter about wine is to suggest a “sweet” red wine. There are a few different reasons for this. First, people today are trying to take advantage of the now known healthy benefits associated with drinking red wine. Secondly, the newest generation of wine drinkers has evolved to possess a higher craving and desire for the sweet. The problem with traditional red wine to this new clientele is that they find it far to “dry,” meaning a lack of sweetness. This has long just been the way it is, now it is becoming more of the way it use to be. When the customer base demands a product, the market responds. More and more sweeter red wines are being produced. Sweet reds are a great way to enjoy all of the red wines benefits, without the full tannic flavors of a Cabernet. What Makes Wine Sweet The sweetness of a wine, by definition, is the amount of residual sugar created throughout the wine’s fermentation process. This measures the amount of sugar that remained unfermented in the finished wine. Basically, if all of the grapes original natural sugar is converted during fermentation, the wine is considered “dry.” The amount of left-over sugar makes the wine sweeter, hence, less dry. A California Riesling has only 1 to 2 percent residual sugar, while dessert wines range anywhere from 5 to 30 percent. Sweetness vs. Fruitiness “Sweet” red wines are hard to find since sweetness is commonly considered a flaw. Most red wines do not get labeled sweet, but rather, are classified as “fruity.” Try not to ask for a sweet red wine unless you are looking for a dessert wine or a Port. You most likely are looking for a lighter red wine with a lot of fruit flavors. Beginners in the

wine tasting world may describe dry wines as sweet because they confuse fruitiness for sweetness. A wine is fruity when it possesses distinct aromas and fruit flavors. You smell the fruitiness with your nose, using you olfactory senses to tell your brain to taste something fruity. Sweetness, on the other hand, is a tangible impression on your tongue. If you have doubts, try holding your nose when you taste wine. You will find wine tasting noticeably less fruity, since your other senses have not predetermined what to expect. If the wine is truly sweet, you will still have the ability to taste it without the odor.

We expect that demand will continue to grow.” The people buying sweet red wines fulfill the exact demographic current marketers crave, Women and middle to high income 21-35 year-olds.
