RESOURCES FOR AGING WELL
Welcome to the October 2021-22 Seniors Blue Book for Northern Colorado. As we publish this annual edition, we are making great strides in our recovery from a pandemic. We are collectively breathing a cautious sigh of relief as our community reopens and continue to be hopeful as we watch the numbers of vaccinated people grow. While we navigate our new normal, the Seniors Blue Book is here to help. We have worked diligently to bring you the most up to date and accurate information. But, the one thing we can count on is change, so we encourage you to do your homework and research options. As a tried and true ‘Authoritative Publication’ our entire mission is to provide our readership with everything they need to navigate the resources they need – and share resources they might not even realize exists. This guide is divided into three colored tabbed sections (Housing, Health at
Home and Community Resources). Check out our Housing and Health at Home comparison grids which offers you an array of shopping amenity columns for easy shopping. We have some fabulous educational articles written by local experts to help you better understand relevant topics. We are also pleased to share a 4-Page insert of contact and service information supplied to you by the Larimer County Office on Aging! You can check that out on pages 12-15 Finally, be sure to check out our new website www.SeniorsBlueBook. com. There you will be able to access local resources, read educational articles and watch videos, as well as view our full event calendars. And, you can ‘Live Chat’ with us 24/7! As always, we love to hear from you with any suggestions, questions or feedback. Here’s to healthy aging and a much better 2021-22 season! Sincerely Yours, Vicky Evans, Regional Director

Guardianship and Conservatorship: What You Need As we age, issues of guardianship and conservatorship may come up. to Know If at some point an individual is no longer able to make financial or medical decisions a court may designate a guardian, conservator, or both.
The court’s decision to appoint a guardian or conservator removes the individual’s rights to make decisions due to the individual’s inability to manage safely and effectively on their own. Those entrusted with these roles take over making decisions for the protected person. Colorado separates guardianship, appointing someone to make health and welfare decisions, and conservatorship, appointing someone to manage finances and assets. Separating these roles allows for an individual to continue making health and welfare decisions when able, while appointing someone to take over financial decisions. The opposite could occur, depending on an individual’s situation and ability. When both a guardian and conservator are needed the court may appoint two different people required to work together making decisions in the best interest of the protected person. A conservator works to ensure the protected person’s assets are used wisely for housing, medical, and other needs. Allowing bills to be paid and bank accounts managed. The guardian manages medical needs, support, care, welfare, housing, etc.
24 NORTHERN COLORADO – OCTOBER 2021-2022
Leaving the decision of who takes on these critical roles to the court may not be in your best interest. By naming individuals within powers of attorney you allow yourself a say in the decision. A financial power of attorney allows you to name a conservator, and indicate which financial decisions they are allowed to make for you. As your situation and needs change this document may need updating to fit your circumstances. A medical power of attorney names someone to act as guardian, making health and welfare decisions when you are unable. When appointing a guardian or conservator the court takes several factors into consideration, including those named within powers of attorney. Ensuring someone appointed guardian or conservator will act in your best interest is crucial. You want someone who will manage your assets and medical care wisely. Having powers of attorney in place as part of comprehensive estate planning can give you peace of mind knowing someone you trust will take care of you. ~ Editor’s Note: This article was submitted by Andrew P. A Checklist for When a Spouse or Parent Passes When you lose a spouse, partner, or parent, the grief can be overwhelming. In the midst of that grief, life goes on. There are arrangements to be made, things to be taken care of – and in recognition of this reality, here is a checklist that you may find useful at such a time. First, gather documents. Ask for help from other family members if you need it. Start by gathering the following. A will, a trust, or other estate documents. If none of these exist, you could face a longer legal process when settling the person’s estate. n A Social Security card/number. Generally, the person’s Social Security number will be retired shortly following the death. If you are uncertain, consider checking with the Social Security office. n
Then, gather these additional highly important items. ny account statements A Deeds/titles to real estate n Car titles or lease agreements n Storage space keys/account records n Any bills due or records of credit card statements n Any social media platform information, if applicable n n
Last, but not least, look for a computer file or printout with digital account passwords. Prior to their loved one’s passing, some family members may try to centralize all this informa30 NORTHERN COLORADO – OCTOBER 2021-2022
tion or state where it can be found. In addition, see if the person left a letter of instructions. A letter of instructions is not a legal document; it’s a letter that provides additional and more-personal information regarding an estate. Keeping Your Brain Healthy Many people expect to live to age 90 and beyond. The chances of developing Alzheimer’s (the most common type of dementia) or other types of dementia increase as we get older. Many of my patients ask me, “How do you keep your brain healthy?” It would be nice if there were a simple supplement or medication to prevent dementia, but unfortunately, this is not the case. The good news is there are things you can do to improve your overall health and well-being as you get older, and these things also help keep your brain healthy. First, it’s important to take care of your physical health. Avoid tobacco or too much alcohol. For people over 65, that means no more than one drink per day. Some medications, including over the counter, can harm the brain, so review your medications with your doctor or pharmacist at least once a year. If you have high blood pressure, diabetes, or high cholesterol, work with your health care team to keep these conditions under control. Enjoying a healthy diet with plenty of fresh fruits and vegetables is also helpful. Second, get plenty of physical activity – this is one of the best things you can do for your brain health. With more physical activity, you’ll feel better, sleep better, have
32 NORTHERN COLORADO – OCTOBER 2021-2022
less stress, and have more energy. Choose an activity that you enjoy and invite a friend or family member to join you. If you are just starting
out, go slow and gradually increase, with a goal to get to at least 30 minutes of moderate activity (like a brisk walk), at least five days a week. Third, learning new things helps form new connections in your brain. Dusting off an old musical instrument, learning a new language, or solving puzzles are examples of fun activities to try. Finally, it’s essential to stay socially connected. Spend time nurturing friendships and family relationships. The same things that make life enjoyable are also the things that help to keep your brain healthy. ~ Editor’s Note: This article submitted by Tracy Lippard, MD. Pre-Planning And WHAT IS PRE-PLANNING? Pre-Payment Pre-planning is the process of drawing up end-of-life plans before they are needed. It involves gathering information and making decisions about your wishes. Working together with a funeral home, you can plan, organize, and record your requests. You can choose to pay for arrangements in advance, or your plans can be held on file at the funeral home to be carried out when they are needed and invoiced to your family at time of need.

WHY PRE-PLAN? While everyone who makes the decision to pre-plan has their own reasons for doing so, the greatest motivator is often the desire to protect family by ensuring they won’t have to face the difficult task of making arrangements at the time of a loss. Making your wishes clear ahead of time can be a great comfort to your loved ones.

There’s No Place Like a 2020 demonstrated the Safe Home sentiment that most of us feel:
Half Page Blue Book Aug 2020.indd 1
There’s no place like home. Most Americans hope to live at home as long as possible, but many need to address areas of the home that are unsafe or inaccessible. There can be many obstacles that affect home safety. These can include steps at the home’s entry, narrow doorways, levels in the home, and unsafe bathrooms. Northern Colorado’s hot real estate market makes it difficult to find a reasonably priced home without barriers. Northern Colorado is home to several companies that can help people of all ages adapt and modify their homes. Some offer construction

services to install grab bars, widen doorways, and create barrier-free bathrooms. Others install equipment such as stair lifts, ramps, wheelchairs lifts, and ceiling-mounted lifts. Be sure to ask about complimentary home evaluations. The company will visit your home to discuss potential challenges that exist now and possibly in the future, providing ideas of modifications as well as associated costs. ~

An ombudsman is an advocate for residents of nursing homes, board and care homes, and assisted living facilities who is trained to resolve problems. Under the federal Older Americans Act, every state is required to have an ombudsman program that addresses residents’ complaints and advocates for improvements in the longterm care system. While ombudsmen do not have direct authority to require action by a facility, they have the responsibility to negotiate on a resident’s behalf and to work with other state agencies for effective enforcement. of Credit is still relatively new and consumers should be interested!
When you have a Line of Credit, you have money available to you to use — but you only accrue interest on the money you withdraw. This means the line of credit can act as an excellent back up source of funds or can be used for retirement. This line of credit has other perks too. Not only are you not paying interest or monthly loan payments, but your untouched line of credit will grow in value. And with home values at record highs, you are guaranteed to lock in the value of your
home now, even if the value was to fall later. The normal Reverse Mortgage qualification requirements still apply: age 62 or over, loan based on primary residence which meet HUD’s guidelines, and borrower must have the financial capability to continue to pay homeowners insurance, property taxes etc… ~ Editor’s Note: This article was submitted by Jan Jordan with Mutual of Omaha, Reverse Mortgage, NMLS 543249. HEALTH AT HOME - HOME HEALTH AGENCIES
Tips for Selecting a Home When the time comes to choose Healthcare Agency home health care, a lot of tough decisions become part of the equation. Researching home care providers and taking the steps to hire the right agency that best meets your needs, can help ensure that your loved ones are safe and well cared for in the hands of an in-home caregiver. Assess your needs The availability of inhome help ranges from companion care, ensuring social interaction eliminating isolation, assisting with light housekeeping and errands, all the way to skilled nursing for individuals with debilitating health conditions. If you’re unsure of the best fit for your loved one, a Care Manager will work with your physician to determine what type of care best meets your needs. Work with a reputable agency By working with a reputable agency, you hand off a greater share of these duties, enabling you to focus on your role as a family member, rather than a family caregiver. Ask whether the agency meets local certification requirements Requirements for home care agency certifications vary by state. However, inquiring about an agency’s State Certification status as well as higher standard certifications can help you gauge their legitimacy and ensure
84 NORTHERN COLORADO – OCTOBER 2021-2022
they meet regulatory requirements. Ask to meet potential caregivers You probably want to make sure the caregiver that you choose is not only well trained, but that they are also compatible with both you and your loved one. Meeting your Care-Team is essential in building trust. Talk about a care plan When you’re paying for someone to provide inhome care, you’ll likely want to be sure that they’re addressing the tasks they’ve been hired to perform. Discussing a plan of care with the home care agency at the time of your assessment and asking how the agency monitors progress and completion of care plans is critical. A plan for care maintains the level of care and outlines each task safeguarding the completed and supervision of your care. Choosing an agency to hire can be a time-consuming process, but when armed with the right questions, you can help put your nerves at ease when choosing who to trust with the care of your loved one. ~

Stand-Alone, Hospice Inpatient Care Center Being Added When a person makes the difficult decision to no longer continue curafor Northern Colorado tive measures for a terminal illness and seeks the help of a hospice care provider, most of the time they can receive all the care they need in the comfort of their own home or the care community where they live. However, there are times when a person needs a higher level of care to keep them comfortable. More support than what can be provided in a home setting. Many times, this higher level of care is provided in a hospital or other acute care setting. While this can be an adequate way to manage a patient’s pain and symptoms, it is not the best setting for complete care of the patient’s emotional and spiritual needs and, not the best setting for family and loved ones to help and be involved. These are the times when an inpatient care center or “hospice house” provides the medical support needed to manage acute pain and other symptoms while offering a comfortable, home-like setting. To meet the needs of patients who require inpatient level hospice care, and future growth of the northern

Colorado community, a 12-bed inpatient care center designed to care for hospice patients with the most acute end-of-life needs is being added to the Pathways campus at 305 Carpenter Road between Fort Collins and Loveland. In this stand-alone facility, each of the 12 rooms will be designed to accommodate groups of visitors as well as family members who wish to stay overnight with a patient. There will be access to outdoor garden spaces where patients will be able to enjoy the outdoors from the comfort of their bed. Also included will be a chapel, a large, shared communal kitchen, areas for quiet reflection and gardens and walkways throughout the site blending indoor and outdoor spaces. ~ Editor’s Note: This article was submitted by Evan Hyatt, Director of Marketing and Communications for Pathways. HOSPICE CARE
Four Common Myths Myth #1: Hospice is a place. About Hospice Fact: “Hospice is not a place but rather, a philosophy of care,” Dr. Payne said. “Hospice comes to you wherever you call home, surrounding you and your caregivers with the support you need at the end of life.” Myth #2: Hospice is giving up. Fact: “Hospice is not giving up,” Dr. Payne said. “It is enhanced care at the end of life, to maximize quality of life at the end of life.” Myth #3: Hospice is only for those who are actively dying. Fact: “Hospice is for any person with chronic illness whose life expectancy is six months or less,” Dr. Payne said. “The true benefit of electing hospice care is seen when hospice is brought in sooner rather than later.” Myth #4: Can patients actually improve? Fact: This one is actually true! “To remain on hospice, a patient needs to have a life expectancy of six months or less should the disease process run its normal course,” Dr. Payne said. “Many times, we see hospice care come into a patient’s life and wrap their arms around a patient and family, getting them the right care at the right time, and
92 NORTHERN COLORADO – OCTOBER 2021-2022
the patient actually improves. The disease process may still be there, but perhaps the patient has learned to manage it differently, they now have the resources they need. Some patients do not continue to decline but rather, improve. Big takeaway Hospice care is generally an appropriate option for those who are terminally ill with approximately six months or less to live. While there are generally some misconceptions about hospice care, first and foremost, it is about maximizing a patient’s quality of life at the end of life. In addition, hospice care can provide support, resources and information to you and your family during this difficult time, including bereavement counseling. “Should the patient continue to decline, they have everything they need to do this with comfort and dignity on hospice care. Should the patient improve, they can either choose to revoke their hospice benefit or the hospice may say the patient no longer qualifies for hospice care and they graduate.” ~ Editor’s Note: This article was submitted by Banner Health. GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period. CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team. 24/7 ADMISSIONS – If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice,24/7, evenings weekends, and holidays.
AROMATHERAPY – Uses plant materials and aromatic plant oils, including essential oils, and other aromatic compounds for the purpose of altering one’s mood, cognitive, psychological or physical well-being. This is not reimbursed by Medicare. GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Many provide Grief Groups that meet and address issues of grief in a supportive group setting.

Hospice Care Definitions
INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients. IN-PATIENT CARE – These hospices offer full hospice services in their on-campus unit or building. The patient moves into the inpatient care facility to receive their hospice services rather than in the patients’ home setting. When Alzheimer’s Affects Understanding the challenges, Your Loved One diagnosis and options available
Alzheimer’s disease can be one of the most emotional and devastating diagnoses that effects an individual and their family. As a progressive brain disorder, the disease depletes memory and thinking skills, making even daily life tasks difficult. Estimates vary, but experts suggest that more than 5 million Americans may have some form of Alzheimer’s. It is currently ranked as the sixth leading cause of death in the United States. Although extensive global research continues to explore the causes and treatments, unfortunately no cure currently exists. Alzheimer’s disease damages the hippocampus, the part of the brain that controls the formation of memories. Vital nerve cell connections to the brain also begin to deplete which prevent the brain from sending messages to the body’s muscles and organs. Over time, as nerve cells perish, other parts of the brain are altered. Simple activities like preparing a small meal, spending time outdoors or driving a car become more difficult for your loved one. As the disease progresses, your loved one can get lost easily, misplace items and even become confused doing the most basic of tasks. As the dis-
96 NORTHERN COLORADO – OCTOBER 2021-2022
ease advances, some personalities can develop into a worried, angry, or even violent state. Treatments for Alzheimer’s disease can include a variety of options and modalities. Working with professionals such as an Occupational Therapist, Neurologist and Geriatrician can help greatly. Cognition-enhancing medication can also prove helpful for some people. Research that is helping loved ones and caregivers establish a better quality of life with Alzheimer’s disease is on-going but keeping your loved one safe is often the greatest challenge. Oasis Senior Advisors of Northern Colorado can provide you with support and guidance when you feel it’s time for your loved one to move to a memory care or assisted living community. Caring for the Whole Person Through Providing care at any level should Holistic Care and go beyond simply treating a perIntegrative Wellness son’s ailment or condition. While the phrase “health and wellness” may lead some to focus on physical aspects, it really refers to the well-being of the whole person – mind, body, and spirit. Wellness infers that a person’s whole being is treated and cared for.
In recent years, the holistic approach has become more popular. One of the reasons for the increasing popularity is that when used in conjunction with traditional western medicine, the holistic care approach can benefit individuals, provide a deeper level of healing, and improve their overall well-being. It is common to think that holistic care refers to alternative or complementary therapies such as aromatherapy or massage therapy, but that is only part of this person-centered care approach. The idea behind holistic care is that it treats the whole person, not just the condition and symptoms. Holistic care is a different way of thinking about and approaching care and wellness. A vital part is personalized education and involvement in the process. Without

the power of positive thinking and learning about and following treatment remedies, holistic care is not as effective. Due to the nature of holistic care, it can promote healthier living habits. As a more natural approach that may include significant lifestyle changes and focus on healing the whole person, holistic care is a great way to refresh your lifestyle and get on a path that leads to improved health and wellness. It is important for each of us to get to know our minds, bodies, and spirits as well as what heals us and helps us to feel safe and at home. “The holistic approach is important because it embodies and aligns with person-centered care and building community. We know it can be challenging to find balance and meaning in life. ~ Editor’s Note: This article was submitted by Hope Carwile, an Innovations Specialist for Vivage & WellAge Senior Living. The Final Season I’ve often heard the phrase, “Getting old isn’t for sissies”, from my mom and her friends. When someone reaches their end-of-life season, they frequently need extra help with things like cooking, cleaning, personal care, household chores, and more. It’s exhausting, and it’s hard. When this final season begins, it puts extra stress on family who may be providing the care themselves or trying to find quality caregivers to help. Elderly people may move in with a relative or find placement in a facility or a home where assistance is provided. Even though this final season of

life can be a beautiful time for the whole family to enjoy, it is often overshadowed by the pressures of daily caregiving. If this is happening for you, there is no one-size-fits-all solution. tor said we could be in “COVIDmode” for the next two years. What? I thought to myself, please don’t say that out loud. Don’t send it out into the universe! Deep down, I knew she was right. I also knew COVID would become part of the health care fabric of diseases we need to manage and treat for the older adults we serve. As we have walked the COVID journey, we saw our residents, staff, and families become strong and then stronger. Their resiliency and dogged determination were an inspiration. Everyone was in the trench fighting together. Then the light came at the end of the tunnel…. vaccines. What a blessing. As the COVID positive numbers declined, so did everyone’s energy. Their enthusiasm for the fight had waned. People were tired. Residents. Staff. Families. They were emotionally worn out. Our resiliency meter was low. Some had experienced trauma. COVID has brought trauma. According to the National Council for Behavioral Health, 70% of adults have experienced some kind of traumatic event. COVID is not even in this equation, yet. For those of us in senior care, there is a high likelihood that many of our residents and staff could
be affected by trauma. Would it be safe to say, we should approach each thinking there may be a response from something that occurred in their life? We think so. We think COVID has added to our mental health and resiliency, and not always in the best way. Should we become a Trauma-Informed organization? We answered with a resounding, YES! The Trauma Informed Care Project of Orchard Place/Child Guidance Center defines Trauma-Informed Care as an organizational structure and treatment framework that involves understanding, recognizing, and responding to the effects of all types of trauma; emphasizing physical, psychological, and emotional safety for both consumers and providers, and helps survivors rebuild a sense of control and empowerment. Realizing there may be trauma, recognizing the signs and symptoms, responding with trauma informed policies/practices, and resisting re-traumatization are the key principles to a successful trauma-informed effort. ~

They will be a resource and a sounding board for you, as you navigate the decision-making process of finding a senior living community that meets your needs. The best news is…they will perform these services for you at no cost to you! Many folks turn to the internet or friends to get advice on what communities they should consider. Well-meaning friends who don’t know the entire situation and can send you on a bit of a wild goose chase. The internet can take up tons of time as you search, not “really” knowing what you are looking for. RESOURCES FOR AGING WELL
Early in her 50”s something seemed “off” with my wife. She would often become forgetful. It would take me several years to realize this “forgetfulness” wasn’t going to go away. After a diagnosis of early-onset dementia how could I know then that I was in for a fifteen year journey of exhaustion, confusion, hopelessness and hard suffering while losing my companion to this disease. As it became unsafe for her to live at home and I was far beyond exhaustion, I was surprised to find her “happy” when we placed her in a Senior Memory Care Com-
munity. During the journey I would say to the kids, “there ought to be someone to help us with this”. Years later I discovered Senior Placement Services. It was too late for my journey but it’s not too late for yours! The service is FREE! Don’t wait for a crisis; plan ahead! They will help you navigate the overwhelming, complex process to “find the right fit” for your loved one. ~ Editor’s Note: This article was submitted by Walt and Britt Roberson of Harmony Senior Referrals. MEMORY CARE ASSISTED LIVING COMMUNITIES
Where would our residents be and what would they be doing if they did not have dementia? How would they be living their lives? What would their hobbies be? Who would they be spending time with? These are these questions we ask our residents, their families and ourselves. Our goal is to create communities that are reflective of the folks that live in them and to enable them to become active participants in their own lives once again.

HOSPICE CARE DEFINITIONS
Hospice Care Definitions continued from page 95
as providing an alternative to those who can’t take medication or prefer not to, massage therapy has become essential in hospice care. This is not reimbursed by Medicare. MUSIC THERAPY – Music Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals”. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare. PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their agency. PALLIATIVE CARE – Specialized medical care for people with serious illness. It is focused on providing patients with relief from the symptoms, pain and stress of a serious illness – whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment.
132 NORTHERN COLORADO – OCTOBER 2021-2022
PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria. PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare. VA CONTRACT – If the patient is not a Medicare Recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~
