MEDICARE & STATE HEALTH INSURANCE ASSISTANCE PROGRAM (SHIP) Provides free, in-depth, one-on-one insurance counseling and assistance to Medicare beneficiaries. Call with questions or concerns about Medicare, Medicare benefits, or how to see if you’re eligible for the Medicare Savings Program. NATIONAL FAMILY CAREGIVER SUPPORT PROGRAM Provides services to help family caregivers care for their loved ones at home. Services are available to family members who provide in-home care for persons 60 and older or to grandparents age 55+ who serve as caregivers for children 18 and younger. DENTAL AND VISION ASSISTANCE PROGRAMS Medicare and Medicaid often do not pay for senior’s dental and vision needs. The AAA offers programs for adults 60 and older for dental and vision needs. TRANSPORTATION Through contracts with local organizations, the AAA offers transportation to medical appointments, nutrition sites and needs based errands. LONG-TERM CARE OMBUDSMAN People living in long-term care facilities, including nursing homes and assisted living facilities, require support and advocacy. Ombudsmen are resident advocates and ensure residents’ rights are respected. Ombudsmen provide information to individuals and families regarding facilities, quality care and are trained in problem resolution. INFORMATION & REFERRAL Provides older adults and their caregivers with timely information and referrals for area resources.

AAAs provide programs directly and through contracted providers. EVIDENCE BASED HEALTH & WELLNESS EDUCATION Health and fall-prevention classes and activities that encourage healthy living and wellness are offered to older adults in the region. Examples include N’Balance, Matter of Balance, Tai Chi for Arthritis, and Chronic Disease Self-Management. NUTRITION PROGRAM Includes congregate meals, home delivered meals, and nutrition education and counseling. Many of us procrastinate when it comes to writing a will. Many of us procrastinate when it comes to writing a will. Perhaps we don’t want to be reminded of our mortality. One can be done prior to its need arising, while the others are usually done after a traumatic incident has occurred and decisions for the person must be made by another party. The differences will be discussed in part as they pertain to Long Term Care Medicaid. A Power of Attorney, or POA, for medical decisions and financial transactions is a set of documents that will beTheonmakearepacitatedbecomeifsionstoyoursomeoneallowofchoosingmakedeci-foryouyoushouldinca-andnotabletodecisionsyourown.POA’smaydoneyearsin advance of being needed, and can be done thru an attorney or with forms available at office supply stores or online. With these documents, you may appoint someone with whom you’re confident, who will make choices on your behalf. For instance, if you discuss your preferences ahead of time with your POA, the appointee would know if you want life support if needed, or if you do not want to be put onto life support. Likewise, a financial Durable POA can pay bills out of your bank account for you, sell a vehicle, or even your home if required for long term assisted housing.ForColorado Medicaid, it is advisable in the financial Power of Attorney to include an additional paragraph that pertains to setting up trusts. Most documents have a general reference to trusts. However, Colorado Medicaid requires special verbiage in the document to work with the State. The

Did you know that a Power of Attorney appointment ends with the death of the person for whom the POA was established? Should the person pass away, the person’s POA, and their rights, end. It’s at this point that an executor of a Will begins.Ifaperson does not have Powers of Attorney set up and there’s a life-altering incident, such as a stroke, a court appointment for the person would be required. Conservators cover financial transactions and Guardianships take care of medical decisions. Generally speaking, a lawyer would be well-utilized in helping with the documents required for the courts. The Conservatorship/ Guardianship process can be cumbersome, so having an attorney who is familiar with the procedures is very helpful. Unfortunately, it’s alsoOverall,costly.the best advice would be to have Powers of Attorney set up while you’re healthy and able to make your own choices. HOMEMAKER/COMPANION –

INSURED – This refers to what hap pens if someone gets hurt on the job. If the company isn’t insured, then the claim gets filed against the home owner’s insurance. If the company is insured, then the claim gets filed against the company’s insurance.

CLASS B – A home care agency that provides only personal care services. An agency with a Class B license shall not provide any skilled healthcare service.

STATE REGISTRATION – The State of Colorado does not license homecare placement agencies. Placement agencies are registered with the state. Registration ensures that placement agencies are adhering to certain minimum standards such as background checks for caregivers and insurance requirements for agencies and caregivers.

Non-skilled care provided for clients with memory issues includes but not limited to establishing a routine, reminders for activities for daily living, rerouting if necessary and assistance with sleep issues or ‘sun downing’. By using creativity and caregiving skills, you can adapt routines and activities as needs change.

It might be a health condition that will surely change the patient’s life, such as dementia, diabetes, heart disease, Parkinson’s disease or a progressive vision loss. What’s next? “Steps taken now can affect their well-being as they move forward,” says David McKendry, Own er of Right at Home. After a diagnosis a patient often ex periences stages of grief, including shock, denial and anger. These emotions can lead to depression. Yet for the patient, spouse and or family members, re ceiving a diagnosis also can bring a sense of relief and reduce anxiety from worrying. Steps in facing the emotional challenges of a diagno sis are talking to family and friends for support, consult a counselor, and joining a support group - these help the patient cope and have a sense of control.Patients initially feel helpless and resigned to their fate, it’s much better to confront the diagnosis head on and focus on a plan of action. The goal is to move from “Why me?” to “What can I do?” Family members can take charge by learning all they can about the condition, discuss the prognosis and care plan with the health team, get organized financially (directives for financial and health matters), bring in experts (financial planners, elder law attorney’s, aging life care professionals), and support family caregivers to share the care tasks. Early on, consider whether the patient’s home will continue to meet their needs as those needs change. Though sometimes it is best to move to a skilled supportive living environment, most patients prefer to receive care in their own homes with a non-med ical in-home caregiver assistance like Right at Home. RE-Respite Care – Provided when the family needs time to rest or go out of town for a short time. Ideally, this service is provided for five consecutive days. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility, at no cost to the patient or the family.

HOSPICE (See definition in home health grid definitions)

Home Health Aide – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN.

GIP – General In-Patient Care –Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period.

CC-Continuous Care – Provided to prevent the individual from going to the hospital when a crisis occurs. Continuous care also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. PRICE DEPENDENT ON LEVELS OF CARE – Flexible levels of service geared to accommodate different needs over time. Rates may vary depending on the changing levels of care while one maintains the same physical residence. PUBLIC HOUSING (PH) – Affordable apartments for low-income families, the elderly and persons with disabilities.

INTERMEDIATE CARE – 24 hour inpatient care residents who need licensed nursing supervision and supportive care, but do not require continuous nursing care. Residents may receive full assistance with ADL’s and transfers. Residents receive meds from a nurse following a doctor’s orders and may receive outpatient rehab services. Facilities may use a nurse as a their consultant and the residents receive periodic assessments from a licensed practitioner.

REHABILITATION AND THERAPY –Many skilled nursing facilities offer restorative and rehabilitative physical and occupational therapy, and speech-language pathology services. Short-term rehabilitation services may be provided to individuals with diagnoses such as joint replacements, stroke, etc. Long-term rehab may also be provided for permanent residents of the facility who have chronic or progressive illnesses.

RESIDENTIAL HOME – Assisted living homes are private residential homes that typically have a small number of residents who live together and receive care from live-in caretakers. Adult family homes offer room and board for seniors who want a more private, home-like compage 121

MEDICAID WAIVER – An assisted living waiver may be used to transition individuals out of nursing facilities or to provide persons at risk of institutionalization with options for remaining in their communities. Assisted living waivers are designed to provide an alternative to more costly nursing home care.

Amenities are usually abundant; formal libraries, computer stations, swimming pools, gardens, and other social gathering spaces. Rates are monthly, and are typically based on size/ location of apartment.

SHORT-TERM CARE – Acute care provided on a short term basis. SKILLED CARE – 24 hour licensed nursing services, eight hours of which are RN covered. Patient may receive rehab services either inpatient or outpatient and may be fully dependent for ADL’s. Patients receive medications by an RN according to a licensed physicians orders.

WANDERGUARD - Offered at some facilities. A resident with memory loss or confusion wears a small discrete pendant. Each external door in the care home is fitted with a radio detector which detects when a pendant moves near an exit. The WanderGuard will sound an audible alarm, alerting the staff of a patient wandering.~

TRACH CARE – Refers to care administered to patients who have undergone a tracheotomy. A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions.

SECURED UNIT – A secured unit dedicated to Alzheimer’s/Dementia residents to prevent wandering outside the community without supervision.

SKILLED NURSING FACILITY –Facilities provide 24/7 care for residents and offer full assistance with ADL’s. Daily rates typically include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service, cable television and all personal/ nursing care. A PACE Day Center is a location where participants can receive prima ry care by meeting with a doctor or nurse in the clinic, receive rehabili tation treatments to improve strength and balance, enjoy recreational and social activities, participate in oneon-one or group therapy, receive spir itual support and enjoy a meal. The number of days a participant attends the day center will be discussed with the participant and determined through the assessment process.

By caring for older adults through this model, prevention of acute illness and conditions can often be prevented, thereby reducing the number of emergency room visits and hospital readmissions while allowing the individual to age in place in their own home. COLORADO’S WESTERN SLOPE

The Program for All-inclusive Care of the Elderly (PACE) has been pro viding wrap-around care to partici pants in Montrose and Delta Counties since 2008. PACE is a Medicare/ Medicaid managed care benefit for adults aged 55 or older who choose to live in their homes despite being certified by the state as nursing home eligible. The goal of the PACE program is help program participants remain as independent as possible while living in their own home and community.SeniorCommUnity

Care is the Western Slope’s PACE provider and manages the program with a staff of paid professionals of various disci plines. There are two PACE day cen ters and office locations in Montrose and Eckert. Participants must be able to live safely at home within a PACE site’s geographic area. The core of the PACE program is the interdisciplinary team. The team works to provide comprehensive med ical and social services to the PACE participants. This team includes a primary care physician, nurses, dieti cian, nursing aides, drivers, physical therapists, occupational therapists, recreational therapists and social workers. The interdisciplinary team meets daily and is responsible for de termining care needs, allocating re sources, coordination of all services, and development of a comprehensive care plan, which is developed for each individual participant.

The care plan addresses a set of functional issues that are relevant to the overall health of the older adult participant. These include cognition, emotional status, social activity, mobility, diagnoses, medications, nutrition and other activities of daily living. Payment for services and mon itoring of the quality of the services is authorized and directed by the interdisciplinary team.
