We are proud to have been ranked for the 25th consecutive year as one of the nation’s top mental health programs by U.S. News & World Report. We are also pleased to share that 2015 was another profitable year for the health system, thanks in large part to our fundraising initiatives. This year, we exceeded all expectations with our Care for Kids Benefit, raising more money than ever before. Our 1st Sheppard Pratt Stride was also extremely successful, allowing us to add a family-friendly walk that fosters employee collaboration to our annual philanthropic events.

Through his vision and ideas, the health system has changed the mental health landscape for the better, created Sheppard Pratt Health System out of the Sheppard and Enoch Pratt Hospital, and expanded our services beyond the increasingly short-term inpatient stay. Never one to shy away from a controversial issue when it impacts the quality of mental health care, Dr. Sharfstein has maintained a voice and viewpoint on issues such as stigma in mental illness and psychiatry, Scientology, the necessity of reimbursement, gun control, and ethics. This has allowed Sheppard Pratt to remain an advocate and to support the people we serve. It’s hard to believe that this will be the last annual report created under Dr. Sharfstein’s tenure. While his time as president and CEO is coming to a close, his contributions and the work he has done and

will continue to do for behavioral health care and the Sheppard Pratt Health System will live on long after he retires. Throughout this annual report, we pay tribute to Dr. Sharfstein’s legacy with a CEO Q&A and commentary from other Sheppard Pratt leaders.

WHAT ARE YOU MOST PROUD OF?
Thank You, Chairman! W. Byron Forbush II has left his successor, Fred Motz, with very large shoes to fill, as he retires as chairman of the board of trustees at Sheppard Pratt Health System this January 2016. A trustee for 41 years, he has helped to guide the health system from a single hospital in 1974 when he first arrived, to a hospital without walls. And for that, we could not be more grateful! Because of his leadership, we have grown into one of the nation’s largest providers of mental health, substance use, and special education services. During his tenure, Byron has been involved with multiple projects, all with the same goal: to tell the story of Sheppard Pratt and to destigmatize mental illness by bringing people together and onto our Towson campus. Some of his most memorable projects include: • Sending memorabilia to the National Institutes of Health to be included as the only psychiatric hospital in their collection
The quality of care we continue to provide despite challenges. We are a major success story in the behavioral health care sector, one of only a few remaining private psychiatric facilities in the nation. We have managed to sustain our excellent reputation even as we have faced numerous obstacles presented by a rapidly evolving mental health landscape. As I look back at our accomplishments over the last several decades, it’s hard not to take notice of our collective influence in mental health care. We transformed ourselves, creating Sheppard Pratt Health System out of the Sheppard and Enoch Pratt Hospital, and expanded our services beyond the increasingly short-term inpatient stay, among other accomplishments, to remain a national and local resource and to provide better mental health treatment and care to our patients.

While we will miss Byron’s spirit, dedication, and commitment, we will still be able to seek him out in the archives, as he will remain part of our family as chair emeritus of the board of trustees and archivist.

Improving Efficiency and Quality of Care OVER THE LAST FIVE YEARS, we have maintained a strong focus on improving our efficiency and quality of care through our Lean initiative, a health system-wide effort to empower employees to identify and solve problems themselves by eliminating waste. To date, 30 percent of our health system leadership has taken the Lean course, thus providing strong support for improvement activity within our health system. QUESTION FOR ANTONIO DEPAOLO, PH.D., DIRECTOR OF LEAN TRANSFORMATION
What defines a true leader? A true leader is one who can leave their position and the organization still continues to have the same success as before he or she left.
Using Lean to reduce aggression and violence on our Adult Psychotic Disorders Unit: This past year, our Adult Psychotic Disorders Unit, which treats patients with psychotic illnesses including schizophrenia, schizoaffective disorder, bipolar disorder, as well as neuropsychiatric disorders, participated in a Lean project to reduce aggression and violence on the unit.
The Problem: The Adult Psychotic Disorders Unit experienced increased aggression due to the health system’s need to serve highly unstable and potentially violent patients while using the least restrictive interventions. The combination of these two dynamic forces created a highly acute environment.
The Solution: Traditionally, when faced with increased aggression, the approach is to react to the crisis of the day. However, the team decided to take a proactive and preventive look at the processes and procedures in place to determine where

the opportunities were for improvement. Through a preventive lens, the investigation helped to pinpoint specific times of the day and specific locations on the unit when aggressive events took place. The data collected on severity, occurrence, and detection suggested that meal time, medication administration time, and free time were high-risk periods that were prone to aggression and were in need of improvement. The team implemented improvements in assessing risk upon patient admissions, ensuring medications were ready when needed, repositioning staff to improve patientto-patient interactions, and enhancing their approach to de-escalation when a patient becomes agitated. They also utilized the “daily huddle process” to create a “monitor and response” plan to ensure their standard practices are in place and working.
The Results: The results show promise, with a 60 percent reduction in aggressionrelated incidents over the last three months of FY2015.
Meeting a Need with our Adult Neuropsychiatric Unit THE MOST SIGNIFICANT new development in the hospital during the year was the reopening of our seven-bed Adult Neuropsychiatric Unit, which serves adults with complex psychiatric disorders associated with intellectual disability, autism, traumatic brain injury, and other conditions that lie at the interface of psychiatry and neurology. This new unit, led by Robert Wisner-Carlson, M.D., is the only inpatient unit of its kind in Maryland and one of only a handful in the country to specialize in treating patients with severe psychiatric and behavioral problems. Because some of the patients are nonverbal, diagnosing and treating mood disorders or other mental illnesses in this population can be difficult. Finding psychologists and nurses specially trained to treat this patient population’s needs is also a challenge. But meeting a need is the core of what we do at Sheppard Pratt.

What defines a true leader? True leaders, of course, lead the way; this requires thoroughly understanding the mission and work of the organization and having a vision about how to move the organization into the future. But true leaders also listen very well — to what’s happening in the world around them, to their formal advisors, to their staff at all levels, and especially to the people served by the organization. They allow their vision to be tempered, refined, and perhaps even reoriented by experience and by input from all kinds of sources, so that in the end the mission is honored and the core values sustained, but the organization is transformed into something new and fresh and ready for tomorrow.

Breaking New Ground AFTER A $4 MILLION RENOVATION, the Steven S. Sharfstein, M.D. Integrated Health Care Center was completed and unveiled in May of this year. The Integrated Health Care Center has allowed Mosaic Community Services to expand its community-based behavioral health care programs and services to even more Baltimore City residents, combining a range of services, including mental health care, addiction treatment, and primary health services, in one facility. People with mental health problems, on average, die 25 years earlier than the general population largely due to treatable medical conditions. Because of this statistic, Mosaic Community Services has made great strides in moving toward a more integrated care model so that people with mental health and addiction issues can access medical services as well. Mosaic is the only mental health center in the state of Maryland that has developed this type of “all inclusive care” center within a behavioral health setting.

Why was this integrated care center named after Dr. Sharfstein? I can’t think of anyone who deserves this more. Steve has had a long-standing commitment to community mental health services and has been incredibly supportive of creating both a continuum of care and an integrated care model.

A Pilot Program for Integrated Care Through our community hospital contract management, we have initiated a pilot for integrated care at North Pavilion, located adjacent to our Towson campus at Greater Baltimore Medical Associates (GBMA) Family Care Associates (FCA) primary care, patientcentered medical home. In June 2015, we began offering psychiatric consultation to the FCA physicians to provide evaluations for referred patients.
WHAT DOES THE FUTURE LOOK LIKE FOR SHEPPARD PRATT HEALTH SYSTEM? We will continue to try to help people near where they live. We will work to become even more of an integrated health care system. Above all else, we will continue to provide the best, most compassionate care to our patients, treating each patient with the dignity and respect they deserve.

DBT Infiltrates Programming at Mann RTC
QUESTION FOR TOM GRAHAM, DIRECTOR OF RESIDENTIAL PROGRAMS AND RELATED SCHOOLS
What has been the most significant accomplishment at the health system over the past 30 years? The movement from being an inpatient psychiatric hospital to also providing a full array of community-based services throughout much of the state of Maryland has been critical to meeting the full range of needs for our patients, residents, and students. It will be even more critical in the future as we move to reimbursement for community success and stability.

Dialectical Behavior Therapy (DBT) remained a focus for this fiscal year across the entire health system. Leading the way in the effort to integrate DBT into all of our programming was The Berkeley & Eleanor Mann Residential Treatment Center (Mann RTC). In January, Mann RTC implemented DBT multi-family groups in an effort to enhance family involvement in the program. It began offering a four-group session series for all parents, family members, and/or guardians of youth in its program, and focused on teaching participants DBT skills that they could then practice in the home environment and with their children upon discharge.
DBT skills are also integrated into programming at The Jefferson Residential Program & Day School.

In June, the Mann RTC created a DBT Honors program in which eight to 10 residents applied to learn advanced DBT programming. Members of this honors group were singled out for their commitment to treatment and to using their DBT skills both in the program and in the community. As a reward for their commitment, the members of this group are able to participate in specialized outings and groups available only to them. Finally, in close collaboration with the marketing department, Mann RTC is piloting an Instagram page, managed by staff, as a way to visually model DBT skills in action. The staff post pictures and videos of themselves using DBT skills in their everyday lives so that followers, including other staff, residents, and their families, can see them modeling the effective use of these skills. The Forbush School at Glyndon Grows Perhaps the biggest highlight during this past school year was the planning and executing of the merger of three of our day school programs. The Forbush School at Glyndon, The Hannah More School, and The Forbush Therapeutic Preschool at Towson all merged to form a larger school, The Forbush School at Glyndon, with a Glyndon Campus and a Hannah More Campus. The two campuses are a little less than three miles apart, and the programs offer nursery school through grade 8, as well as the autism program on the Glyndon Campus and high school, grades 9-12, on the Hannah More Campus.
Providing Real World, Working Experience for Our Students Another large initiative taken on by the day schools that provide education for students 14 years and older was the development of transition plans beyond classroom education, to provide solid experience in the real world of work. Under the guidance of Vera Roth, CTE (Career & Technology Education) director, all programs began providing transition services, and leveraged and cultivated relationships with local businesses in order to facilitate on-the-job training for our students.

What has been the most significant accomplishment at the health system over the past 30 years? In 1985, there was one school, The Forbush School, serving approximately 125 students, 95 of whom were inpatient students and about 25-30 who were day students. Today, there are 650-700 residential and day students served in 12 residential and day schools across Maryland. During this decade, Sheppard Pratt became the single largest provider of private, nonpublic special education services in Maryland (only to be surpassed this last year by the High Roads Schools).

While our health system website has continued to evolve into a resource for those not only looking for help, but also searching for information on behavioral health and addiction, it is our social media presence that has truly grown. This past year, we utilized social media as a way to provide valuable information to our community in an accessible and friendly manner. We experienced 30 percent growth on Facebook and 35 percent growth on Twitter. We also added an Instagram account for the Mann RTC to visually capture DBT skills in action. Furthermore, to showcase our experts and share expertise with our community members, we produced and published nearly 20 videos on our YouTube channel.

WHAT IS YOUR HOPE FOR THE FUTURE? My hope is that we will see a future where mental illness and addiction are no longer stigmatized, where people will feel as comfortable asking for help with a mental disorder as they do with any type of physical disorder. We try to counter this stigma by reaching out to the community, sharing patient stories and experiences, actively engaging with the public and the media as an expert, and ensuring that we are a resource for all people.

A history of steady growth
DEAR FRIENDS: Did you know that charitable giving can improve mood, enhance physical health, and increase life expectancy? Studies have shown that being philanthropic has a way of amplifying several different happiness chemicals in the brain. We are so grateful that our donors choose to bless us with their generosity and hope that they too can experience the mental and physical benefits of being philanthropic. Because of our donors, Sheppard Pratt Health System raised $24.5 million, with $6.8 million in private donations and $17.7 million in government awards last fiscal year. Initiatives receiving significant support included charity care, integrated health, research, special education, substance use treatment, supported housing, telepsychiatry, and workforce development. Of special note, we received a $2 million donation from Mildred Kern, the largest individual donation to the health system since our doors opened in 1891. For more than a century, the largest donation to mental health and the health system came from Enoch Pratt, who donated $1.6 million in 1896.

Mildred’s donation will support child and adolescent programs with the intent to improve opportunities for children in need. Our 2015 Care for Kids Benefit was another record-breaker and raised nearly $315,000 for our Care for Kids Fund. The event at the Four Seasons Hotel Baltimore featured gorgeous views, delicious food, a program highlighting our residential treatment centers, and fabulous disco music. Tickets sold out before the invitations were mailed, so be sure to save the date for our 2016 gala on Saturday, April 2. Additionally, we created another fundraising event in 2014 with the 1st Sheppard Pratt Stride, a familyfriendly, one-mile walk. The Stride was more of a success than we could have imagined, as it allowed us to connect with more than 500 new donors. Additionally, almost 90 percent of our participants were employeerelated. We raised more than $43,000 for the Patient Care Fund through the 1st Stride and are looking forward to turning this into an annual event. We also welcomed seven new members to our Gatehouse Society, which recognizes in perpetuity those who have made extraordinary contributions in support of Sheppard Pratt. Donors are eligible to join once they have made cumulative gifts of $10,000 or more. Within the Gatehouse Society, we had 13 members who moved to a new membership level representing an increased philanthropic commitment. On the following pages, we acknowledge the many donors who
have contributed to Sheppard Pratt Health System during the past fiscal year. We are very thankful for the impact that our loyal and generous supporters make in helping us to provide compassionate care to the people we serve. Most sincerely,

What do you think has been Dr. Sharfstein’s most significant contribution to SPHS? Some would say he has led us to stay ahead of the curve. Others would say he has transformed and reinvented Sheppard Pratt to be a “hospital without walls.” In addition to all of the above, I say that he has stayed true to Moses Sheppard’s founding ideas — “to meet a need” — each area of SPHS growth has been “to meet a need.”

What three words would you use to describe Dr. Sharfstein? Larger – he is recognized as a national expert. Than – he has a brilliant mind, able to analyze complex issues, comparing and contrasting all input and potential solutions. Life – at the same time, he is grounded and connected to real life issues and leads with humility and humor.

What have you learned from working with Dr. Sharfstein? Some short sentences to sum up what I have learned from working with him: • Be prepared for all meetings, especially board committee meetings. • When presenting an idea, memo, or document, be “crisp.” • Don’t jump to conclusions, or do a knee-jerk reaction; instead consider other options, solutions, or even reasons, and always get the other person’s point of view. • If he is waving his wrist, wrap up what you are doing — you are taking too long. • If he gets up to leave a meeting and you traveled with him, you better get up and leave also! • Get a flu shot!
