Welcome: Director of Medical Services In addition to being an Anaesthetist with an interest in pain management, Dr John Monagle is the Director of Medical Services for SJGHC’s South East Melbourne hospitals. He reflects on 2020 and the Covid-19 pandemic. It’s tough to even know where to begin when trying to sum up 2020. While it’s been difficult, sad, frustrating and challenging, it has also opened our eyes, as a healthcare provider, to the exceptional capacity our caregivers have to go above and beyond and respond to crisis. We always knew it to be true, but Covid-19 has tested us and proven our commitment in a way we could never have previously imagined. Like you, we have adapted to a no-contact world, adopting telehealth and providing care, without the human touch where possible, to keep ourselves safe and protected. As a whole, the Victorian health care system has been stretched and pulled in directions it’s never seen before. While working closely with the DHHS, St John of God Health Care (SJGHC) South East Melbourne hospitals were able to collaborate and support the public system throughout the pandemic. We accepted public lists to ease the pressure placed on the public hospitals. Despite the difficult circumstances, this was a welcome move for many public patients who were able to experience private hospital care including robotic surgery and private maternity care. As the months rolled by and Victorian Covid-19 cases increased, we received 30 aged care residents from a facility located in Kilsyth. 24 of the residents tested positive to Covid-19. They were provided with care and medical treatment in a locked down ward on the 4th floor of our Berwick Hospital. Caregivers from throughout the hospital, including nurses, physiotherapists, doctors, pastoral practitioners and PCAs, came together to provide aged care during a pandemic- something they had never done before. They did it with compassion, humour and respect and worked harder than ever, facing the hardship of losing 10 of the residents during this period. In order to move recovered patients out of the Covid unit, the old Berwick Hospital at Gibb St was repurposed and recommissioned within 10 days to become a makeshift aged care facility. It was a safe place for the residents to transfer to, where they were cared for by a team of professionals. This team involved caregivers from many St John of God Hospitals, including our SEM hospitals, Bendigo, Geelong, Accord (disability services) and Ballarat.

Many of these selfless individuals stayed in hotels, away from their homes and families, for up to 6 weeks while they worked long shifts dressed in full PPE. Now that Victoria is free of Covid-19 cases, we are starting to see the aftermath of the pandemic- the toll which has been taken on small business, those out of work. and for all of us during lockdown. Mental health care is revealing itself as a post-Covid health crisis. This is just one reason we are pleased to announce the repurposing and redevelopment of our Gibb St hospital, which will re-open in 2022 as a mental health facility. Located right in the heart of south east Melbourne, the Langmore Centre will offer both inpatient and outpatient mental health care, drug and alcohol services, with the addition of a mother baby unit. As 2020 draws to a welcome close, we will take a long list of learnings with us into a new year. We will use our experiences to work smarter and more efficiently with humanity and compassion. We will look to continue the relationships which were formed in a time of crisis, building on a stronger health care network for all Victorians. We will value each day, each person, each life, each encounter and never take human interaction for granted. St John of God Frankston Rehabilitation Hospital

Technology transforms recovery for stroke patients St John of God Frankston Rehabilitation Hospital is using cutting edge technology to transform the fine motor skills of Parkinson’s and Stroke patients. The hospital was one of the first early adopters in Victoria to use the Tyromotion Amadeo - a world class device that sets the standard in robotic hand and finger rehabilitation. The robotic therapy system is designed to improve patient outcomes in grip, range, strength and overall movement.

Amie Malcolm explains that clinicians are able to manipulate the intensity and complexity of the robotic movements. She adds that the patient uses computer aided games that get increasingly more challenging and the movements can be measured with pin-point accuracy and real time reporting of results.

Amie Malcolm, senior occupational therapist and clinical lead for movement disorders at St John of God Frankston Rehabilitation Hospital, says that the hospital is experiencing quite dramatic results.

The hospital acquired the Amadeo in January 2020 and is now using the second generation of the Pablo system at a combined investment of $281,000. The technology is available for in-patients and out-patients as a part of their rehabilitation program.

“This technology is providing positive rehabilitation outcomes for our patients with Parkinson’s and Stroke. The ability to customise the therapy to work on specific finger movements as well as measure and evaluate patient progression sets this technology apart from traditional methods of therapy,” said Amie. The robotic technology uses sensors that can detect even the smallest flicker of movement. The continuous repetitions help the brain to recognise and send the neuro-signals required for the hand to move its fingers. The clinicians at St John of God Frankston use the Amadeo in tandem with the Tyromotion Pablo system which focuses on shoulder, arm and elbow movement. Together, these systems allow patients with upper limb immobility to master greater movement and improve neuro-plasticity.

“We are using this technology on a range of patients with an acquired brain injury. But we’re also achieving results with patients who have balance and more general mobility issues. It’s a highly motivating form of rehabilitation for the patient as they can see their results in real-time on the screen. “People who completed rehab some years ago can still benefit from the Amadeo and Pablo systems and we are seeing that in our outpatient clinics from people who have returned to rehab to gain enhanced mobility,” said Amie. St John of God Frankston combines the robotic rehab system with sensory retraining as part of the SENSe Implement Research Project which aims to improve upper limb sensory loss as part of a broader neurorehabilitation approach. “We are one of only a few hospitals chosen to participate in the SENSe program. The loss of sensation is sometimes overlooked in rehabilitation therapy so it is very rewarding from a clinical perspective to provide a more holistic approach to help patients regain independence following stroke.”

Interventional Cardiologist Dr. Michael Leung is an Interventional Cardiologist and a Specialist in both Cardiac CT and Cardiac MRI, joining Melbourne Heart care in 2002. Michael undertook a two year Fellowship in coronary intervention at Monash Health gaining experience in a broad range of percutaneous interventions. In 2007, Michael was awards a PhD from Monash University for his research into human coronary microvascular haemodynamics and aortic biomechanics. In 2008, Michael completed a two year Post-doctoral fellowship at Royal Adelaide Hospital in Cardiac CT and Cardiac MR imaging. Michael has a public appointment at Monash Heart, Monash Health. Dr Jeff Alison Cardiologist Dr Jeff Alison has special expertise in all facets of Cardiac Rhythm Management having more than 30 years’ clinical experience in this subspecialty (often referred to as “Electrophysiology and Pacing”). He consults and advises on: -management of ambulatory and hospitalised patients with heart rhythm disorders. -performs interventional procedures -electrophysiology studies, -catheter ablation, device therapy with pacemakers and implantable defibrillators and cardiac resynchronization therapy for heart failure (“biventricular pacing”).

Dr David Adam Cardiologist-Cardiac Rhythm Management Dr Adam trained at Monash University in Melbourne and completed his residency and general cardiology training at Monash Heart. He then spent two years completing a fellowship in cardiac electrophysiology in British Columbia, Canada, before returning to Melbourne in 2006 to commence specialist practice. He has a public hospital appointment as a cardiologist and electrophysiologist at Monash Heart. Commencement of Electrophysiology Studies at SJOG Berwick St John of God are focused on continually expanding our health services to accommodate the growing needs and demands of our local community. It is with much excitement that we announce the commencement of Electrophysiology studies at St John of God Berwick. Electrophysiology studies are invasive procedures that involve the placement of catheter electrodes at various intra-cardiac sites. The aim is to investigate the electrical aspects of abnormal heart function for diagnostic, therapeutic and/or prognostic purposes. Indications for the referral of patients to have electrophysiology procedures are; Atrial Fibrillation, Atrial Flutter, Sick Sinus Syndrome, Supraventricular Tachycardia, Atrioventricular Nodal Re-entry Tachycardia, Ventricular Tachycardia, and Syncope. St John of God Pinelodge Clinic

The Langmore Centre Mental health hospital coming to Berwick in 2022 2021 will see the finalisation of plans and the commencement of the redevlelopment of Berwick’s Langmore Centre. The Langmore Centre, formerly the old St John of God Berwick Hospital located at Gibb Street, will be refurbished and repurposed to meet the everincreasing demand for mental health care in South East Melbourne. This project will produce a contemporary mental health facility in a location which is placed to work collaboratively with the Berwick Hospital and to provide care for patients closer to home in the heart of Melbourne’s south eastern corridor. Melbourne is in desperate need of mental health beds and services for patients closer to home.” The Centre will not only provide services for mental health, addiction, day therapy and consulting, but also the introduction of a Perinatal Mental Health Unit. “The Perinatal Unit will add significant support for the new families in our community. The City of Casey alone saw 5,314 births in financial year 19-20 and children aged 0-4 years were the largest cohort with 31,100 in 2020.* With so many young, growing families it’s important that we offer the support systems they need closer to home,” says Linda. PET/ CT coming to I-MED Radiology Casey in December 2020 I-MED Radiology are always striving to be at the forefront of technology. That is why they are proud to announce the introduction of a PET/CT service at the Casey clinic this coming December. Positron Emission Tomography/Computed Tomography (PET/CT) scans are used to help diagnose a variety of diseases including many types of cancers. Using radiopharmaceuticals, radiologists use PET to diagnose, stage and monitor a variety of malignancies, and to assess the metabolic activity of a variety of disease processes. It is also useful in assessing cardiac function and neurodegenerative disease. The “metabolic” image is fused with a CT image to allow high anatomical detail. Meet our reporting radiologist: Dr Raymond Shafik-Eid M.B.B.S MMed FRANZCR FAANMS Dr Raymond Shafik-Eid is a dual-trained radiologist in both diagnostic radiology and nuclear medicine. After completing his radiology training at St Vincent’s Hospital in Melbourne, Dr Shafik-Eid gained his Fellowship in Nuclear Medicine and PET/CT at Monash Medical Centre.

Dr Shafik-Eid’s interests include oncologic imaging (particularly prostate cancer). He has extensive experience in prostate PET/CT, general diagnostic radiology (including mammography and MRI), and general interventional procedures (especially MSK intervention). As well as private practice, Dr Shafik-Eid holds an esteemed position at St Vincent’s Hospital in Melbourne as a PET/CT specialist. He is also actively involved in teaching radiology and nuclear medicine Fellows as an honorary senior lecturer at the University of Melbourne. Dr. Shafik-Eid is the clinic director at I-MED Radiology Berwick. Benefits of PET Scans PET scan plays a very important role in cancer diagnostic staging. It alters clinical management by adequately staging the cancer status and detecting unknown occult metastases which are unrecognised in conventional imaging. In addition, PET scan also provides an important role in the treatment response evaluation and as a prognostic marker. At present, the Medicareapproved indications for PET in few cancers e.g lung, breast, oesophageal, colorectal, gynaecological, melanoma, head and neck cancer,

sarcoma and primary brain cancer, and lymphoma. PET scan service is an essential component in cancer care. The new PET service at SJOG Berwick will provide stateof the art cancer services to local residents in Casey, Cardinia and regional Gippsland. Our patients now are able to consolidate all their cancer investigations and treatments in one centre. They do not need to travel miles away to hospitals e.g. Frankston or Moorabbin for their PET scan, which they have been doing for the last few decades.
