Tidbits® of Sheridan and Johnson Counties Tidbits Presents the

HEALTH PAGE TO YOUR GOOD HEALTH By Keith Roach, M.D..

Nonalcoholic Fatty Liver Is Increasingly Common --DEAR DR. ROACH: I am 59 years old and am having problems with my lower abdomen. After colonoscopy and ultrasound, my doctor called to say I have a fatty liver. When asked what that means, I was told to lose weight. I weigh 170 pounds, and am 5 feet, 6 inches tall. Research online suggests that alcohol is an issue, but I don’t drink. I may have a drink or a beer when I go out, but most times I am the designated driver, so I don’t drink. The only medication I take is estradiolnorethindrone 0.5-0.1; I take one daily. Diabetes does not run in my family, and I have not been tested for it. Can you give me information on “fatty liver” and treatment? -- J.A. ANSWER: Non-alcoholic fatty liver disease is an increasingly common problem; in fact, it is the most common liver disorder in industrialized countries. Recent studies have suggested that 30 percent to 46 percent of people in the U.S. have NAFLD. Most people are diagnosed in their 40s or 50s. Obesity, particularly abdominal obesity (as opposed to fat around the hips and thighs) is common in fatty liver disease. People with suspected NAFLD should have other possible causes evaluated, especially viral hepatitis (types B and C), hemochromatosis and autoimmune hepatitis. Simple fatty liver is a benign condition; however, some people will develop the more serious condition nonalcoholic steatohepatitis (NASH). Seventy percent of people with NASH are obese, and up to 75 percent have Type 2 diabetes. NASH without treatment can lead to fibrosis and even cirrhosis of the liver. Your body mass index is 27, so you are considered overweight, not obese. Still, I would recommend gradually losing 10 pounds or so. I also would recommend that you continue to drink very moderately or not at all. Exercise also can help. *** DEAR DR. ROACH: I am 77, female and in great health except for being diagnosed with achalasia. I know it is rare, and I was happy to find out why my digestive system had discomfort. Do yoga twists or shoulder stands worsen this? It seems worse when I do these. -- E.H. ANSWER: Achalasia is a disease of abnormal movement in the esophagus, the muscular tube that carries food from the back of your throat to the stomach. It is caused by damage to the nerves in the esophagus, possibly from destruction by the body’s immune system. Because of the nerve damage, the food gets stuck in the esophagus, leading to difficulty swallowing and vomiting of undigested food. Both solids and liquids are affected, as opposed to esophageal cancer, in which the problem is mostly solids. Gravity helps us swallow, so being upside down will make gravity work against you, and I would expect the condition to worsen with yoga positions that turn the esophagus upside down. I wouldn’t recommend those types of positions, especially since they may cause or worsen vomiting. Treatment of achalasia should be done by an expert in the condition, and may include dilation of the esophagus, injection with Botox into the muscle or surgery. *** Dr. Roach regrets that he is unable to answer individual letters, but will incorporate them in the column whenever possible. Readers may email questions to ToYourGoodHealth@ med.cornell.edu. To view and order health pamphlets, visit www.rbmamall.com, or write to Good Health, 628 Virginia Drive Orlando, FL 32803. TRIVIA ABOUT TRIVIA • In ancient Greece, it was common to find statues of the god Hermes (messenger of Zeus) or the goddess Hecate (goddess of childbirth, wilderness, and land of the dead) at the street corners. • Hermes’ name originated with the Greek word ‘herma’ meaning a stone cairn used to mark roads and pathways. The Greeks thought that the spirit of Hermes would reside inside these piles of stones, protecting and guiding travelers. • Hecate’s job was to ensure that people didn’t take the wrong road, so pillars were erected at crossroads to please her. • The cairns and pillars built for Hecate and Hermes were also considered to be altars, so passersby could pay homage to the two gods as they traveled, hoping for a safe journey. Travelers customarily added stones to the cairns to win the favor of the gods. • Of course, any time roads and pathways meet, people also meet, and they pass the time gabbing. With so many cairns and pillars handy, they could chat with each other and worship the gods at the same time. • Altars and statues of Hermes and Hecate were so plentiful that Hecate became known as Trioditis, which is Greek for ‘one who is worshipped where three roads meet.’ • When the Romans began to adopt Greek gods as their own, they re-named Trioditis, giving her a new name that meant ‘three roads’ in Latin. • Eventually Trioditis’ new Roman name entered our language meaning ‘useless or obscure knowledge’ such as those petty details exchanged where three roads meet. Her name was Trivia, and that how a word that means ‘three roads’ came to mean ‘things of little importance.’ (continued on last page)

Visiting Dog Ruins Holidays --DEAR PAW’S CORNER: My sister-in-law insists on bringing her yappy, ill-tempered lap dog, “Mitsi,” to our house every Christmas through New Year’s. Mitsi gets along with no one but her owner, frightens our two cats and barks at everyone in the house during dinner, nonstop. I love having my brother and his wife over, but Mitsi is making things hellish. How can I politely ask her to leave Mitsi in a kennel during visits? -- Barked Out in Birmingham, Alabama DEAR BARKED OFF: I don’t know that there’s a way to tell your sister-in-law to leave her dog at home that would be taken well. Mitsi clearly is a full member of their family and travels with her. Can a compromise be reached instead? Ask her if Mitsi can be placed in a separate room during mealtimes while they are visiting. She’ll probably still bark, but at least it won’t be right in your ear. You also need to discuss how Mitsi is stressing out the cats. Sometimes people think it’s funny when a visiting dog chases the family pets around, but it’s not fun for those pets. It’s like having a schoolyard bully walk right into your home. Try to keep the discussion civil. Ultimately, you’ll have to decide what level of compromise to ask for in order to keep peace in your house, and you’ll have to be firm. Your sister-in-law takes her relationship with her dog seriously; she should respect your concerns as well. That discussion probably won’t be comfortable, but if Mitsi’s barking and bullying are making the holidays a living hell, you need to say something. Send your questions or tips to ask@pawscorner.com. (45 kg) • Approx. number of cows consumed by McDonalds per day: 2,500 • Percent of U.S. cattle purchased by McDonalds: 1 • Number of cows a typical American will eat in a lifetime: 350 • The Latin word ‘caput’ meaning ‘head’ is the origin of the word ‘capital’ meaning ‘a principle sum of money.’ That word went into the Anglo-Norman language as ‘catel’ and the Old French language as ‘chatel.’ That evolved into the word ‘cattle’ because a person measured wealth in terms of how many head of livestock they owned. The Old English word ‘feoh’ also meant cattle or property, and survives as our word ‘fee.’ • Cattle were domesticated some 10,000 years ago, descended from a species of wild ox called aurochs which is now extinct. DNA studies have shown that it’s likely the 1.4 billion cattle that now inhabit the earth are descendants of a single herd of auroch. • Christopher Columbus brought cattle with him on his second journey in 1493. Texas longhorns descended from the cattle explorer Coronado brought with him in the year 1540. • Cows have nearly panoramic vision, being able to see 300 degrees from side to side. This helps them spot predators as they graze. Their sense of smell is highly developed and it’s been estimated they can catch a whiff of a predator at a distance of six miles (9.6 km). • Cows have 32 teeth, but they are all located on the bottom part of the jaw. The top part of the mouth is a hard platform of skin which serves as a grinding pad. • Cattle are ruminants, meaning they have a digestive system that allows them to digest difficult foods by regurgitating and re-chewing the cud. The cud is then re-swallowed and further digested by specialized microorganisms in the four different compartments of the stomach, called a rumen. They can digest things such as almond shells and shredded newspaper.
