INSPIRE 2018—A RARE OCCASION TO REDEFINE PATIENT CARE If you sense a celebratory tone in this issue of Urban Angel, it’s because we have reached a significant milestone in the life of this great hospital. Thanks to the immense generosity of our donors, we have not only met but exceeded our $210 million goal to complete our ambitious Inspire 2018 campaign. We are thrilled to be able to publicly announce this landmark success. At $235 million and counting, every additional dollar will support important priorities for your care, the care of your family and our shared community as we become Canada’s premier critical care hospital. When Peter Gilgan of Mattamy Homes announced his historic $30 million contribution to our campaign in the fall of 2014, it began the journey to the creation of a new 17-storey patient care tower designed to care for critically ill patients. His philanthropic leadership energized others to invest in St. Michael’s future through the revitalization and modernization of our facilities, including renovations that will double the size of our Slaight Family Emergency Department. It was a proud moment to join campaign Co-Chairs Tony Arrell, John Cassaday, Bill Downe and Honorary Co-Chair Margaret McCain as we celebrated the official groundbreaking at the corner of Queen and Victoria Streets last spring. We were also pleased that Health Minister Dr. Eric Hoskins could join us at this event, a great demonstration of support from the provincial government for which we are grateful. We also appreciate John Tory, who helped us in countless ways as a campaign Co-Chair before becoming Mayor of Toronto. As President of this hospital, I have never been more impressed by the unparalleled pace of innovation, spirit of discovery and culture of caring at St. Michael’s. When my family welcomed our first grandchild, Edith Howard, on November 26, 2015, we were fortunate enough to do so here. Everyone involved in bringing her into this world was kind, helpful and supportive. So when I say that you can expect the best in expert and compassionate care, I say it from personal experience.

UP IN SPACE—AT A COST Y
ou may think it’s glamorous to be an astronaut. Certainly when kids are surveyed, it’s always a top choice. But did you know that venturing into space may come at a cost? St. Michael’s Dr. Yeni Yücel, Director of our Human Eye Biobank for Research, is examining why, on their return from missions, astronauts have poorer vision than when they left. A condition called vision impairment and intracranial pressure has been found in half the male astronauts who have been into space. The weightlessness they experience may lead to pressure changes in the brain and spinal fluid and actually cause the eyeball to alter its shape. Astronauts like Commander Chris Hadfield routinely do eye tests during missions and record the results. Dr. Yücel and his team are collaborating with NASA to understand what happens in space and what they can do about it. And who knows what other things we can learn about Earth from space?

A HEALING TOUCH
S
t. Michael’s Neonatal Intensive Care Unit (NICU) has launched a program where carefully selected and trained volunteers cuddle infants when their parents can’t be present. Volunteers are encouraged to soothe, sing and read to the babies while holding them. They also help entertain siblings while their parents are caring for their newborn, and provide companionship to families during their time in the unit.
“HOLDING THE BABIES, SEEING THEIR LITTLE FACES REACT AND EYES OPEN IS VERY SPECIAL.” – Volunteer The program builds on the observed benefits of baby cuddling as well as research that has shown health improvements in infants resulting from a caregiver’s touch. According to Amanda Hignell, a social worker in the NICU, “Life doesn’t stop when you have a sick child and families can’t always be here—so our volunteers help and fill in wherever possible.” See news coverage of the program at stmichaelsfoundation.com/cuddle.

s you know, Toronto’s Urban Angel provides the same expert and compassionate care to everyone in need. Staff Physician, Family Medicine, Dr. Ashna Bowry is leading a new initiative that emphasizes that welcoming attitude. In December 2015, St. Michael’s opened a new clinic for Syrian refugees in our St. James Town Health Centre. Dr. Bowry says many of the patients have not received regular primary health care for several years or treatment for chronic conditions such as high blood pressure and diabetes. Some have suffered orthopedic injuries and traumatic injuries such as burns that are common to war zones. You can learn more about the clinic or make an appointment by calling 416.323.6400 ext. 5905. DOCTORS BACK TO SCHOOL
D
id you know that many children in what is considered Toronto’s inner city do not have OHIP cards or basic health insurance? That’s what St. Michael’s pediatrician Dr. Sloane Freeman discovered when she was asked to take part in a pilot project to bring health care directly to Sprucecourt Public School in Regent Park, basically in the backyard of the hospital. The pilot, created in partnership with the Toronto District School Board, received a tremendous response from the community, and the demand was so great in the first year that the clinic started to care for students from nine surrounding schools.
BOND, JAMES BOND
E
rnst Stavro Blofeld, the supervillain of Spectre, the latest James Bond film, may have his heart set on ruling the world, but according to St. Michael’s Dr. Michael Cusimano, he would never make it as a neurosurgeon. Towards the end of the film, Blofeld threatens to destroy our hero’s ability to recognize people. But the aim of his robotic drill is off: “In terms of today’s precision brain surgery, the villain was nowhere near the brain,” says Dr. Cusimano, a lifelong 007 fan. “Aiming to erase Bond’s memory of faces, Blofeld correctly identified the lateral fusiform gyrus as an area of the brain responsible for recognizing faces,” says Dr. Cusimano, “but in practice, the drill was placed in the wrong area, where it likely would have triggered a stroke or massive hemorrhage.” As a result, Bond would have been shaken—and stirred!

Now, through the St. Michael’s Model Schools Pediatric Health Initiative, children from 20 schools access the clinic, staffed by two pediatricians, a family doctor, a development specialist and a multilingual clinic coordinator. The team has now seen more than 600 children for appointments and follow up, and they are seeing roughly 100 new patients a year. With its success at Sprucecourt Public School, the team has moved into more space in Nelson Mandela Park Public School on Shuter Street, which will help enhance the program. The result will be healthier children and a brighter future for these young Canadians.
NO PAIN—GREAT GAIN! “Accidents can happen to anyone at any time,” says Charlie Atkinson, President of Hewlett Packard Enterprise for Canada. “Hunting was a way of life in Onaping Falls, now part of Sudbury, where I grew up. When my accident happened, I was barely 15 and out in the woods with my cousin. The odds of an accident are one in a million but that millionth time will change your life.”
W
hen we were in the woods, my cousin tripped and dropped his gun. The safety was supposed to be on but, as the gun fell, it hit a rock and a 12-gauge shotgun blast went through my right foot. Only the Kodiak boots I was wearing saved my foot. Following the accident, I had five corrective surgeries, but I remained in constant pain. I was also left with a shortened leg and a cane for a walking companion.
“MANY TIMES I COULDN’T WALK DISTANCES AND AT TIMES EVEN HAD TO USE A SCOOTER—A TOUGH TIME FOR SOMEONE WHO WAS AMBITIOUS AND ACTIVE.” A few years ago, Charlie decided it was time to do something about it. So he did some research to find out who was the best doctor to fix his problem. His search led him to Dr. Timothy Daniels at St. Michael’s. Dr. Daniels and his team performed reconstructive surgery on Charlie’s right foot, and after 30 years of pain and failed treatments, the pain was finally gone. However, after years of favouring his injured foot, his left had also deteriorated. Dr. Daniels repaired that as well. Now Charlie has no pain, no cane or scooter and a new outlook on life. Charlie’s research wasn’t wrong. This past fall, the prestigious journal Orthopedics This Week named the Top 16 Foot and Ankle Surgeons in —and St. Michael’s Dr. Timothy North America—and Daniels was one of them.
Head of Orthopedic Surgery at St. Michael’s, Dr. Daniels established our foot and ankle surgery program and is recognized as an international expert in this field. Dr. Daniels, who is St. Michael’s—and Canada’s—first Chair in Foot and Ankle Research, performs more foot surgeries than anyone else in Canada and more ankle replacements than most places in the world. Orthopedics This Week said that these 16 physicians “represent standout orthopedic surgeons dedicated to improving the daily lives of their patients” and their peers provided insight into why they were chosen: “Tim is one of the best thinkers in our profession. Every time he stands up and presents a paper, one has to stop and listen. There will be pearls of wisdom that one can use every day. As such, he is one of the most respected teachers in our time.” Says Charlie, “While some hospitals may be shinier and others have higher profile, none offered the patient experience I received at this hospital. St. Michael’s—and Dr. Tim class. Daniels—are world class.” Donors to our Inspire 2018 campaign are helping to build a new 16-bed orthopedic inpatient unit in the Peter Gilgan Patient Care Tower, which will connect with our existing 13-bed unit, enabling us to help even more patients like Charlie.

WORK NOW ADVANCING QUICKLY If you drive by on Queen Street you’ll see the frame of the tower climbing floor by floor to match and link into the 17-storey Cardinal Carter Wing. Less visible are the interior renovations to the Donnelly Wing, which will allow us to clear the Cardinal Carter lobby for the expanded Slaight Family Emergency Department—think of where the statue of St. Michael currently stands. The walk-in section of the Emergency will occupy the new Shuter Wing on the northeast corner of the campus—that’s right, the 1912 Shuter Wing will be demolished in 2018. The new two-storey base will accommodate future expansion upward.
Tim and Frances Price Medical Surgical Intensive Care Unit, $5 million Tim and Frances Price and colleagues through the Brookfield Partners Foundation have given to the largest ICU in the hospital, allowing us to create large, single rooms for optimal care of very ill patients.
Geoffrey H. Wood Foundation Respirology Unit, $5 million Named to honour the memory of this Companion of the Canadian Business Hall of Fame (1987), this gift will create a state-of-the-art inpatient floor for our respirology patients for the best in infection control and comfort, especially suited to our cystic fibrosis population. TARA BOURQUE’S BATTLE WITH CYSTIC FIBROSIS

T
ara Bourque remembers the panic she felt when she was told she needed a double-lung transplant to save her life. The social 17-year-old, actively dedicated to sports and friends, was fresh into her first year of university when her lung function began to rapidly deteriorate. Without warning, getting out of bed, showering and brushing her hair left her desperate for air. “I didn’t know what was happening. I was struggling to cross the street, let alone walk across the campus to the cafeteria from my residence.” Tara was just 14 months old when she was diagnosed with cystic fibrosis, the most common fatal genetic disease in Canada. But up until now, she had been spared the impact of the complications associated with the disorder. “I never really thought about it. I was always pretty healthy,” she says. One in every 3,600 children in Canada is born with the defective gene which produces a buildup of thick mucus that clings to the lungs and can lead to life-threatening infections.
“TARA WAS THE SICKEST CYSTIC FIBROSIS PATIENT I HAD EVER SEEN FOR THE FIRST TIME.” – DR. TULLIS When her symptoms worsened, Tara was taken to St. Michael’s Hospital to see Dr. Elizabeth Tullis, Director of the Adult Cystic Fibrosis Clinic, who is internationally renowned as a leading researcher on the disease. By that point, Tara’s weight had dropped to 78 pounds and her lung function had diminished. “Tara was the sickest CF patient I had ever seen for the first time,” recalls Dr. Tullis. After performing an initial assessment, she immediately paged for an oxygen tank and told Tara that she would need to spend the next few months in hospital. During her stay, the St. Michael’s team discovered that Tara was in urgent need of a double-lung transplant. “That was the first time a transplant had really crossed my mind,” she recalls. “To be told this was devastating. I was in complete denial.” Tara was warned that the risks of foregoing the procedure could be deadly, but she was still reluctant to proceed. Dr. Tullis started to panic. “I thought, ‘if I don’t convince Tara that this is important, she could die.’ My dilemma was: how do I tell her she needs surgery to survive without scaring her? How do I get her to trust me?”

PATIENT STORY Tara needed time to think, so she went home to Sarnia, Ontario, to spend the holidays with her family. “That was my way of taking control of the situation,” she says. But the festivities she used to love left her exhausted and unfulfilled. “Socializing, food, gifts—I didn’t enjoy a moment of it. I had no interest in talking to my friends or family,” remembers Tara, whose boyfriend had to carry her up and down the stairs. “That’s when I realized just how sick I was.”
“ST. MIKE’S WAS CONCERNED ABOUT ME FROM DAY ONE.” – TARA It was a wake-up call that impelled her to return to St. Michael’s to prepare for the surgery. “St. Mike’s was concerned about me from day one. They made it clear that it was critical to focus on my health,” says Tara, crediting Dr. Tullis’s persistence. “The staff was incredible. They were so thoughtful and kind. They really cared about me as a whole, not just my disease.” After just 11 days of waiting, Tara was told that a pair of lungs had arrived at Toronto General Hospital. Her outlook has transformed since the surgery.

I DON’T SWEAT THE SMALL STUFF ANYMORE.” – TARA Tara is one of 480 patients who belong to St. Michael’s renowned Adult Cystic Fibrosis Clinic, which is the largest of its kind in North America. “Tara is the best example of this amazing group of patients who, despite having a very busy life with all their treatments, still has time for a full life—a life that she was not ready to give up,” says Dr. Cecilia Chaparro, a respirologist with a cross appointment at both St. Michael’s and the Toronto Lung Transplant Program, who has followed Tara since her evaluation for transplant in October 2011.

Dr. Anne Stephenson, a respirologist and researcher at St Michael’s, has recently published the results of survival analysis showing that the median age of survival for Canadian cystic fibrosis patients has been on an upward trend—recently surpassing 50, which is the highest in the world, and 10 years more than the US median age of 40.

But she doesn’t want people to feel sorry for her. She doesn’t want to be treated differently. She never has. “Everybody has days when they feel down and they feel upset, but my main focus is not to be someone who asks ‘why me?’” Despite her declining lung capacity, Tara still finds the strength to play volleyball twice a week. “It’s so humbling to see these young people who are brave without knowing they’re brave,” says Dr. Tullis, referring to Tara as a dynamo. “You’d think that being born with a genetic disease and being sick all your life would make you grumpy or miserable, but it’s just the opposite. They’re funny. They’re tough. They’re determined. They have cystic fibrosis, but that’s not what defines them.” Tara remains focused on the future. She plans to graduate from college this spring. She plans to one day marry her longtime boyfriend. She plans to continue inspiring fellow cystic fibrosis fighters. “If we keep raising awareness and keep the funds coming for research, I have no doubt that we’re going to breathe easier and finally find a cure.”
In a period of eight months, Tara has lost 10 friends under the age of 25 to cystic fibrosis. She refers to every one of them as a warrior. “We’re a different breed of people, the way we think and do things,” she says. It’s what’s driven her to become a motivational speaker and fierce advocate for organ donation. “I say to every parent with a newly diagnosed child that this is the best time to be diagnosed with cystic fibrosis.” “She’s right,” says Dr. Tullis, who played an instrumental role in bringing a groundbreaking drug that treats a rare form of cystic fibrosis to Canada. “It’s an exciting time to be looking

SNEAK PEEK OF THE FUTURE: GEOFFREY H. WOOD FOUNDATION RESPIROLOGY UNIT The sixth floor of the new Peter Gilgan Patient Care Tower, to be built at the northeast corner of Queen and Victoria Streets in downtown Toronto, will house the Geoffrey H. Wood Foundation Respirology Unit. This modern facility will consist of 15 rooms designed for patients who have a variety of complex lung conditions. Since cystic fibrosis patients are at greater risk of acquiring infections, all inpatient rooms will be private with individual washrooms and showers. “This new unit will facilitate state-of-the-art care,” says Dr. Tullis.
AS HEAD OF ST. MICHAEL’S DIVISION OF RESPIROLOGY AND DIRECTOR OF THE ADULT CYSTIC FIBROSIS PROGRAM, DR. TULLIS HAS MADE GREAT STRIDES TO IMPROVE THE LIVES OF HER PATIENTS. With the generosity of St. Michael’s donors, Dr. Tullis was appointed the first Cystic Fibrosis Canada Chair in Adult Cystic Fibrosis Research last year. The five-year, $1 million chair supports research and innovation in care for adults like Tara. Dr. Tullis hopes to accelerate the pace of treatment and discovery in her field, with the ultimate goal of finding a cure. “It’s a complex disease that’s challenging from an intellectual point of view. It keeps me thinking. It stimulates me. It’s all about ‘how do we help this person live a longer, better life?’”

A ST. MICHAEL’S FIRST IN CRITICAL CARE

ime is of the essence for a patient suffering from massive blood loss. For St. Michael’s trauma care team, which treats patients from across the province who arrive by ambulance or helicopter, every effort must be made to control or stop the bleeding first. St. Michael’s blood lab is using an innovative test known as the rotational thromboelastometry or ROTEM® to help clinicians quickly determine the most appropriate clotting product. St. Michael’s is the first hospital in Canada to use this test in a clinical environment. Other facilities are looking to St. Michael’s for insights into how best to implement ROTEM® clinically. “ROTEM® has improved the way we care for patients suffering from massive bleeding. We no longer have to wait for multiple tests to be run or guess which clotting products may work,” says Dr. Sandro Rizoli, Trauma Director at St. Michael’s. Previously, the trauma team would have to rely on a “one size fits all” approach to stopping bleeding. ROTEM® allows physicians to be more targeted in their approach to stop bleeding and save lives while wasting fewer blood clot components.

onditions affecting kidneys such as diabetes and high blood pressure can cause organ scarring and failure. St. Michael’s nephrologist Dr. Darren Yuen explains, “If we could figure out who has a lot of scarring, we could treat these patients more aggressively.” Needle biopsy is the current “gold standard” for assessing kidney scarring. However, biopsies can create discomfort for the patient and only sample a small area of the kidney, making it difficult to get a complete picture of how much of the organ is affected. Dr. Yuen teamed up with Medical Imaging specialists Dr. Anish Kirpalani and Dr. General Leung to study the use of magnetic resonance imaging (MRI) to detect scarring in transplanted kidneys, which are also prone to scarring, without risk to the patient. “We’ve already tested this technique in more kidney transplant patients than anywhere else in the world,” says Dr. Kirpalani. He adds, “If we’re able to detect scarring more accurately, we may be able to better guide how kidney transplant patients are treated early on and improve their long-term outcomes.”

The Centre houses some of the best research scientists in the world who focus on urban health solutions. One important project under development at the Centre involves improving the health of Canada’s aboriginal and indigenous people. Under the direction of Dr. Janet Smylie, a recognized expert in the field of indigenous health and one of Canada’s first Métis doctors, the Centre will trial a new and innovative training program for front-line health-care providers. Ella was just 14 when she accepted a summer job working in St. Michael’s kitchen. “When I started delivering meals to patients, I really felt that this was a place I could be happy working for a long time,” says Ella, who graduated from the hospital’s nursing program in 1972. Since then, she’s been a Manager on the General Internal Medicine and Intensive Care Units, Director of the Heart and Vascular Program, received her MBA, became an Assistant Professor of Nursing at the University of Toronto, and capped off her career with a decade-long role as Executive Vice-President of Programs and Chief Executive, Nursing and Health Disciplines Executive.

There really is a magic about St. Michael’s. There’s a sense of purpose, a sense that one human being can always make a difference in another’s life. We all benefit from that. We all have that personal power. And it’s really, truly lovely.
In 1992, St. Michael’s was in a financial crisis along with the rest of the province. Our leaders were forced to make some really difficult decisions. As a result, 40% of management roles were eliminated, including mine. While some hospitals decided to lay off nurses and health professionals, St. Michael’s chose to keep the dollars at the bedside. For me, this was one of those life-changing moments. As tough as it was to lose my job, I respected that decision. I knew right then that I wanted to stay and work here, because this organization puts patients first. I reapplied at the next opportunity and became Manager of the Intensive Care Unit. That patientfirst philosophy has served as a guiding principle ever since. Whenever I’m faced with a difficult decision, I ask myself, “Will this enable the care of our patients?”

As a leader you have to listen to the people you work with. I’ve spent a lot of time walking through St. Michael’s and asking people, “What are you worrying about that I should be worrying about? How can I help?” We sometimes forget to ask these questions. If someone comes to me with a problem, I start by asking, “If I wasn’t here, what would you do to try to solve this problem?” Nine times out of 10 they have a better answer than I do. Really good leaders don’t direct, they facilitate. A leader’s number one job is to support their staff to reach their full potential.
I would like to be remembered as a transformational leader who always put people first; someone who valued every person who worked at St. Michael’s; someone who truly understood the strong link between caring for our physicians and staff and achieving our goal of high-quality patient care. In addition to this, I would want to be recognized as a health-care leader who advanced provincial health system integration in Cardiovascular, Neurosurgical, Trauma and Critical Care services.

I tell everyone that I wish I were 20. I think this is the best time to be working or to be a leader in health care. Now, more than ever, we are having explicit discussions about how to engage patients and families, to ensure safe transitions in care across the continuum. That excites me for the future of health care as a system, and it excites me for the future of St. Michael’s. Our initial reason for existing in 1892 has not changed. When the government agreed to redevelop St. Michael’s Hospital in the heart of the city, it was a public declaration that we are valued in the provincial health-care system and we are here to stay.
Be there. Be there for people when they need you. It all comes back to the philosophy of paying it forward. How do you want to be treated? It’s the golden rule, which was actually the nursing school’s motto: “Do unto others as you would have them do unto you.” It’s not that I’ve done anything fabulous; I’ve done what I would want others to do for the people I love.

THE BATTLE AGAINST HIV
Nothing grabs the public’s attention quite like the captivating lives of celebrities. So when actor Charlie Sheen began acting outlandishly in 2011, the story made headlines. At the time, he uttered his now-famous and perplexing statements about “winning” and having tiger’s blood coursing through his veins. He was suggesting that he was somehow immune to ill health.

In November 2015, Sheen made headlines again, this time for announcing that he was, after all, vulnerable to getting sick and had in fact contracted HIV. The announcement was a jarring reminder that this virus, which is deadly if not brought under control, continues to spread.

I HAVE A DIFFERENT CONSTITUTION… I GOT TIGER BLOOD, MAN.” – Charlie Sheen Sheen discovered he was infected only after developing symptoms that included difficulty swallowing and severe headaches. He was quickly put on powerful antiviral medication that reduced his viral load down to undetectable levels and brought his condition safely under control. Dr. Sean Rourke, a St. Michael’s neuropsychologist and international leader in HIV research, notes, “Sheen’s case has helped raise awareness about the importance of detecting infections early through regular testing—especially when people are at high risk—and, if necessary, receiving medical treatment to stay healthy and prevent the spread of this virus.”

“ENSURING THAT VULNERABLE PEOPLE LIVING WITH HIV ARE ACCESSING REGULAR HEALTH CARE IS VITAL TO ACHIEVING OPTIMAL OUTCOMES.” – Dr. Sean Rourke Overcoming the threat of HIV requires that everyone be educated about, and have access to, preventative and treatment options. That’s why St. Michael’s provides total HIV care and services through the Positive Care Program. Through his research, Dr. Rourke has also called attention to making sure HIV services reach our most vulnerable members of society, who are more likely to be living with HIV but less likely to be receiving the necessary care. “The good news is that people in care are doing well,” said Dr. Rourke. He added, “The bad news is that there still are some gaps. Ensuring that vulnerable people living with HIV are accessing regular health care is vital to achieving optimal outcomes.” Ontario is nearing an important milestone in its ambitious goal to end the AIDS epidemic. By 2020, it is expected that 90% of all people in this province living with HIV will know their HIV status. They will be receiving sustained antiretroviral drug therapy and have very low or undetectable levels of the virus, which prevents the virus from infecting others. To help us get closer to 100% success, Dr. Rourke says, “innovative and pragmatic solutions are needed to prevent all groups from falling between the cracks.” Asked once why he did not believe he was vulnerable like other people, Sheen said, “I just didn’t believe I was like everybody else. I thought I was unique.” His subsequent infection with the virus is a vivid reminder that, when it comes to HIV, we are all vulnerable. Nevertheless, with greater awareness and access to care we can beat this decades-old scourge.

As Dr. Mike Evans, Founder, Health Design Lab, Li Ka Shing Knowledge Institute, St. Michael’s, advises in 23 and ½ Hours, his video which has become an internet sensation, a 30-minute walk each day will make a huge impact on your health. Many perks of an outdoor fitness regime can’t be replicated in a climate-controlled gym. Breaking a sweat in local parks, ravines, hiking trails, beaches, waterways or even along neigbourhood streets with gardens and trees will leave you feeling more fit, energized and engaged. Exercise will improve your mood when undertaken in grassy, sandy, rocky or watery terrains. Natural settings have an overall positive effect on happiness, vitality and confidence while lowering tension, depression and fatigue. Exposure to sunlight allows your body to produce vitamin D3, important to bone health and metabolic function, as well as the production of feel-good endorphins. Another surprising physical benefit of external habitats comes from direct exposure to plants and trees. They emit airborne chemicals that may improve your immune system while decreasing heart rate, blood pressure and levels of the stress hormone cortisol. For example, one Japanese study reports that after a simple walk in a forest, almost all test subjects showed “significant differences” in immune cell activity in their blood.
John Muir once stated that in every walk with nature one receives far more than they seek. Dr. Beth Abramson reinforces this notion in her book, Heart Health for Canadians, with the simple but important message that going for a walk “is good for the heart.” Throughout her book, she stresses the importance of regular activity, which will help you maintain a healthy weight and avoid diabetes, high blood pressure and high cholesterol in the blood.

If you prefer combining exercise with social activity, go for a walk with a friend. There is also an endless array of meet-up groups, clubs and leagues, including in-the-park classes of every stripe which, in many cases, are free!

Potato Salad with Red Pepper Dressing Sure to be a new summertime favourite! A fiery fusion of jalapeño peppers, parsley and pink-skinned potatoes—it’s a mash made in heaven.
Ingredients 6 boiled Amarosa potatoes or fingerling potatoes, peeled 2 tomatoes, skinned, seeded and diced 1 tbsp. rocoto chiles or red jalapeño pepper, chopped 1 tbsp. Italian parsley, chopped 1-½ tsp. salt ½ tsp. pepper ½ cup olive oil Juice of 2 limes
Directions 1. Mash the potatoes and add all other ingredients. 2. Taste and adjust the seasoning to your preference. 3. Find a creative way to serve this new summer favourite.

1. Place the skewers into a baking dish and cover with water; let soak overnight.

Ingredients 2 cups uncooked red or regular quinoa 6 large sea scallops 1⁄8 cup vegetable oil 1 cup ripe fresh tomato, diced ¼ cup shallots, minced ¼ cup chives, snipped 1⁄8 cup sherry vinegar ¼ cup extra virgin olive oil Juice of half a lemon Kosher salt and pepper to taste Fresh garden herbs to garnish

Directions 1. Cook the quinoa. Bring 2 cups water or vegetable stock to a simmer. Add quinoa, cook until tender, usually about 10 minutes will do. Allow to cool. Season to your taste. 2. To prepare the scallops, heat the vegetable oil over medium-high heat in a skillet or fry pan.

3. Season the scallops with salt and place in the heated oil. Cook until golden, about 3 minutes. Alternatively, fire up the grill and cook for 2-3 minutes each side. 4. Turn the scallops and remove the pan from the heat to finish cooking in the hot pan. Remove scallops from the pan and squeeze lemon juice over them. 5. To prepare the dressing, mix the tomato, shallot and chives together. Add the olive oil, vinegar and season with salt and pepper.

EDible Garden SUMMER SALAD
Smash the salad status-quo with an edible garden that will leave your guests green with envy. Our unconventional quinoa crudités will quickly become a summertime classic. What you will need • Quinoa salad • Your favourite dip, like a flavoured hummus or salad dressing—green goddess works well here • Fresh berries (wild raspberries, strawberries, currants or gooseberries) • Tiny vegetables with the stems (radishes, carrots, beets, fennel, tiny turnips or other small vegetables and asparagus) • Edible flowers and leaves
For the Quinoa Salad Ingredients 2 cups uncooked quinoa 3-1⁄2 cups cold water Sea salt for seasoning 1 lemon juice and zest ¼ cup canola oil 1 cup toasted almonds (but any can be substituted, or can easily be left out for those with allergies) 2 tbsp. mixed chopped fresh herbs: lemon balm, mint, chive and parsley 1⁄2 tsp. mixed ground spices: use equal parts of cinnamon, cloves, star anise and nutmeg. Add more or less depending on your taste preference.

Making the Quinoa 1. Soak the quinoa for 30 minutes in cold water. Rinse thoroughly, and then drain through a fine-mesh strainer. 2. Place the rinsed quinoa in a 2-quart pot with 3-1⁄2 cups of cold water. Season with sea salt. 3. Bring to a boil. Cover the pot, let boil another minute, turn the heat down to a simmer and cook for 5 minutes. 4. Remove quinoa from heat and let sit covered for 5 minutes. When cool, fluff gently with a fork. The quinoa can be made ahead of time and refrigerated.
Building your veggie garden for guests 1. Just before serving, mix the quinoa, lemon juice and zest, olive oil, toasted nuts and herbs. Season with salt, fresh cracked black pepper and lemon zest to your taste. 2. Line a clean, shallow flower pot or garden-inspired serving dish with a layer of dip, to the depth of 1 inch. 3. Top evenly with the quinoa salad, to the depth of 1 inch. 4. Just before serving, add fresh, stem-on, cleaned little vegetables, pushing them into the quinoa and the dip so only the tops show. 5. 2⁄3 cup unsalted butter at room temperature
1. With a mixer, beat the softened butter until light and creamy. Add both sugars and molasses and beat until fluffy.
1⁄2 cup lightly packed dark brown sugar 1⁄2 cup granulated sugar 2 tsps. molasses 1 whole egg and 1 egg yolk 1-1⁄4 cups whole wheat flour 1⁄2 tsp. baking soda 1 tsp. kosher salt
2. Add the eggs and mix lightly until just combined, about 5 seconds. Remove the bowl from the mixer. 3. Mix together the whole wheat flour, baking soda and salt. Fold into the butter mixture.

4. Add the chocolates and walnuts and mix just to combine.

5. Chill the dough in the fridge for 30 minutes.

6. Preheat oven to 350°F. 7. Line a baking sheet with parchment paper. 8. Shape the cookies into balls using a scoop or a rounded table spoon. Regular size cookies are 2-3 tablespoons of dough. If you want to make smaller cookies, use 1 tablespoon. 9. Arrange the cookies 2 to 3 inches apart onto the lined baking sheet. 10. Bake the small cookies for 6 minutes, the regular size cookies for 12 minutes. Note: not all ovens are the same. The cookies will cook or brown faster in a convection oven. 11. The cookies are very delicate, so leave them to cool in the baking tray for 5 minutes before removing them to cool on a wire rack. 12. Hide them as your guests will devour them to the last crumb. The recipe will make approximately 30 regular size cookies.

We’re working really hard to preserve the vision of more and more people, thanks to generous donor support.”
Some four million Canadian adults have at least one of the leading eye diseases: age-related macular degeneration, glaucoma, cataracts or diabetic retinopathy. In the next 20 years, the number of Canadians with vision loss from the three major chronic eye diseases (the same list minus cataracts) will double. Vision loss costs Canada $15.8 billion a year—more than diabetes, cardiovascular disease and all cancers combined. Dr. Neeru Gupta is working to ensure more people retain their sight. Based on her discovery that the lymphatic system connects our eyes to the body, her team has created a high-tech imaging system to measure the impact of trial therapies on the transfer of fluid out of the eyes. The goal is to be able to reduce eye pressure as a potential treatment for glaucoma. A leader in her field, Dr. Gupta has received many honours and accolades, including being the first Canadian and the first woman appointed to the Board of Governors of the World Glaucoma Association. She is one of only 73 members of the highly prestigious Academia Ophthalmologica Internationalis, and serves as Chair of the International Glaucoma Guidelines Task Force on Glaucoma in collaboration with the World Health Organization. The international guidelines were just released at the most recent World Ophthalmology Congress this past February. “St. Michael’s and Canada are lucky to have such an international champion for

eye health,” says Dr. Arthur Slutsky, Vice-President of Research for St. Michael’s. “Dr. Gupta is dedicated to understanding and treating diseases that affect eye health and cause blindness.” As part of her efforts to save sight, Dr. Gupta was in Vancouver this past December for the World Diabetes Congress to introduce the world’s first eye health guide to help health-care professionals who work with patients with diabetes. One in every three patients with diabetes has diabetic retinopathy, which can cause severe vision loss and blindness. Dr. Gupta makes a special point of thanking the family of St. Michael’s donors who have supported her groundbreaking research over the years. “Donor funding is crucial for firststage research, which is too risky for granting agencies,” she says. “The generosity of our donors has pushed the boundaries on both research and treatment for eye disease.” Thanks to advances in eye care, including potential future treatments for glaucoma by Dr. Gupta, more and more Canadians will be able to enjoy the panorama of colour that we enjoy all spring and summer and fall.

Operating rooms are typically painted in greens, teals, blues or neutrals. Experiencing sadness or fatigue? Feeling sensitive to criticism and lacking confidence? Yellow could be the burst of happiness you need to feel a rise in your selfconfidence. Feeling timid and shy? Introducing the colour red into a space can increase energy levels, promoting togetherness. But be sure to use only in small doses as large amounts can create feelings of frustration and anger.
So, what are we doing with colour in our new spaces? Well, according to Isabelle Boba of Toronto’s Lux Design Inc., there are standard psychological values associated with each colour, the atmosphere they create, the emotions they evoke and their influence on mental health and recovery. She says that, while no exact colour palette can guarantee outcomes, you can define colour and design goals for specific spaces. So when designing a space, in the home or a hospital, designers must consider the psychology behind the colour.
Thexton says, “People in spaces with warm colours actually report the ambient temperature of the room to be warmer than it is. This is a great way to keep patients feeling warm but still allow the space to be cool enough for nurses and staff who are always on their feet.”

“When selecting colours and placements for the new patient care tower, we are utilizing colour contrast to assist anyone with low vision or depth perception challenges to navigate the environment. High colour contrast between adjacent materials can help prevent injury as it makes the environment appear crisp and well defined to the eye,” says Thexton.
Designing a public space or waiting room? Looking to reduce stress and lower anxiety in your home? Cool colours should be your palette of choice. Purple is known for fostering imagination and creativity, and lighter purple is the colour of relaxation and peace. Similarly, blue tones create a sense of calm and creativity— but Boba warns, “Beware of dark blues as they can have the opposite effect and induce feelings of sadness.” Tones

We don’t know what causes multiple sclerosis, says St. Michael’s neurologist Dr. Jiwon Oh. Our best understanding is that there is no single cause—it appears to originate in a complex interplay of genes and the environment. If I can contribute a small piece to solving the MS mystery—and provide tangible benefits to my patients through new knowledge or a new treatment—I’ll feel that I’ve done my job.

or Paul, it began with numbness in his thumb. Not really thinking anything of it, he would startle his friends by plunging it into a cup of hot coffee. But then the print began to blur when he read the morning paper. When it persisted and started to affect his work, he went to his doctor. Several tests later, he got a diagnosis that stopped him in his tracks: “The results indicate that you may have multiple sclerosis.”

WE ASKED DR. OH What should we know about MS? It’s a rapidly changing area of medicine. New discoveries are being made all the time. In fact, the number of treatments for relapsing-remitting MS has doubled in the last five years alone. Canada has one of the highest prevalences of MS in the world. This observation is likely in part due to the “Vitamin D theory”—the further away from the equator you are and the less sunshine you get, the higher the risk. To support this theory, we see much higher rates in Northern Europe and Southern Australia. The reason why this theory doesn’t explain it all, though, is because there are inconsistencies observed—for instance, among our northernmost people, the Inuit, the prevalence of MS is very low. It’s increasing here and all over the world. It’s now being found in places it was rarely seen in the past, like East Asia and the Middle East. This could be because of better ways to detect and diagnose MS, globalization, cultures mingling and a more common diet—but again we don’t know why yet. St. Michael’s is a leader in MS care and research. As one of the only facilities in Toronto to treat adults with multiple sclerosis, our MS Clinic is one of the largest of its kind in North America and follows approximately 7,000 patients a year. St. Michael’s is also well known as one of the largest centres for clinical trials and research. Recently, St. Michael’s spearheaded the clinical trial for a new drug now approved for use in relapsing-remitting MS: oral teriflunomide, shown to reduce relapse rates and the progression of the disease.
Are there misconceptions about MS? “Generally it begins in young people in their 20s and 30s, which creates a large public health burden because of their long-term disability. But I have patients of all ages,” says Dr. Oh. Referred by his doctor to a neurologist, Paul got treatment and is now managing his symptoms. “Living with MS is a challenge,” says Paul, “but I’m determined to stay as mobile and active as I can. Talking to my doctor, I know that new therapies are being developed all the time. Some days it’s hard to keep a positive attitude, but I’m doing my best!”

When people think of MS, usually they automatically think of people living with severe disability and requiring aids like wheelchairs, canes or walkers to remain mobile, but that’s a misconception. It’s an extremely unpredictable and variable disease: some people can have one attack and then nothing else, while others can suffer increasing debilitation. That’s why we need better predictors of the impact of the disease.
What is your research focus? Traditionally the brain was the focus for research. I’m looking at the spinal cord, which has been hard to examine because it’s only as wide as your thumb, is surrounded by fluid and moves with your breathing and heartbeat. In my research, I’m using MRI to better understand how tissue
damage in the spinal cord contributes to disability in MS patients. This approach is safer for patients because it doesn’t require taking blood or tissue, and unlike X-rays or CTs, there is no radiation—and it’s showing a lot of potential.
Why did you get involved in MS? My mother suffered a stroke at the age of 35 and I saw how she struggled physically to look after her family—that’s one reason I was drawn to work with multiple sclerosis, which can have a similarly restrictive impact on young people’s lives. As well, during my medical training I had mentors who were involved in the field, and you tend to be drawn to fields where you have mentors that you look up to.
How have donors helped? At St. Michael’s, we are incredibly fortunate to have donors who want to help us by funding explorations into new treatments and therapies. For example, Richard and Lynne Waugh have generously established the Waugh Family Chair in MS Research. The funds provided by this chair have enabled the recruitment of new talent and are helping to develop a universal clinical registry which will provide data for improved treatment and research. We have many remarkable champions who have provided great support, including John and Jocelyn Barford, Beth Malcolm— John’s sister, and Jon and Nancy Love. The generosity of all these wonderful people, through the Inspire 2018 campaign, has created the Love and Barford Multiple Sclerosis Research Fund, to advance highly promising translational and clinical research in MS. This support will make it possible to build on our existing knowledge and clinical expertise to broaden our investigations in translational research to develop new therapies. As well, the Elizabeth S. Barford MS Patient Comfort Fund, established by John’s and Beth’s father Ralph, is a remarkable benefit to our patients. Among other supports, it provides financial assistance to patients in need of things like modifications to patients’ cars and houses to enable them to cope with symptoms and continue to live as independently as possible. One patient, an older woman, lives in the country near Colborne, two hours east of Toronto. With her family at work or school, she could have been stuck at home all day. Now, with Barford Comfort Fund-supported modifications to her chair and van she has independence and freedom: “It’s wonderful that there are people who will help in this way.”
Most recently, the Eaton family, with a leading contribution from the John C. and Sally Horsfall Eaton Foundation and volunteer leadership from John David Eaton and Foundation Board member Gregory S. Belton, has generously made lead gifts to create the John David Eaton Chair for Multiple Sclerosis. This new chair will provide new resources to further basic science research in MS. We are really lucky to have such wonderful friends like these, as well as Scotiabank, the Multiple Sclerosis Society of Canada, Bayer Inc. and Hermès Canada who, through their support, are furthering our understanding of this often debilitating condition. Donor support, like that provided through our current fundraising campaign to advance research into new and better treatments, is critical because right now there is no cure for MS, and such support enables clinicians and scientists to do the necessary studies to get us closer to a cure.
WHAT IS MS? Multiple sclerosis (MS) is a disease of the central nervous system, which involves the brain and spinal cord. The disease attacks the protective covering of the nerves, necessary for the transmission of nerve impulses through nerve fibres, causing inflammation and damage. As a result, scar tissue can form, disrupting the transmission of nerve impulses and damaging nerve fibres. MS is unpredictable and can affect almost every neurological functional system. The ambulance lurches to a halt and the paramedics pound up the stairs. But tonight they are too late—another unlikely addict has succumbed to Toronto’s painkiller epidemic. Canada is awash in legal painkillers. Physicians are prescribing them to patients with chronic pain, a problem reported by up to 29% of Canadians. We are now the second-highest per capita users of prescription opioids in the world, behind the US. As Dr. Quastel explains, prescription opioid use can lead to addiction not just to manage the pain but for the euphoria it creates. Then there is the “escalator effect.” The body becomes increasingly tolerant to a given opioid, which leads people to either change to a sister drug or increase the volume of the drug to get the same effect. People also transition from oral consumption to nasal to, finally, injection, to increase potency. As they need more drugs to achieve the same feeling, they seek non-prescription sources.
I felt great—and I craved more of those same feelings. Over the course of several weeks, I stopped showering, brushing my teeth, and I slept in my clothes. The house looked as if a tornado had hit it. I had no appetite, and felt sick afterward when I did eat. I kept telling myself that I would stop tomorrow, but it was obvious, even to me, that I had lost the ability to make a conscious choice. – Rachel, detox patient testimonial A 2014 study led by St. Michael’s scientist Tara Gomes published in Addiction showed one of every eight deaths among young adults in Ontario is related to opioids, making the drugs a leading cause of premature death. Opioidrelated deaths went up by over 400% in the province over 19 years, from 127 deaths in 1991 to 550 deaths in 2010. In response to the problem, Ontario delisted Oxycontin and its tamper-resistant successor, OxyNEO, from its drug plan in 2012, and created a centralized prescription monitoring system. That, in turn, created another issue by driving people with addictions to other drugs, including heroin. And, alarmingly, heroin is increasingly being mixed with fentanyl, an opioid 40 times more toxic than heroin, leading to a surge in deaths across Canada and the United States.
I found myself biting the pills to get the rush into my system more quickly. At that point, I knew I had to get off this drug. – Nancy, quoted on globalnews.ca The Addictions Team offers both inpatient and outpatient services at St. Michael’s, seeing several hundred new patients a year. Often the team will be called in because an inpatient is unexpectedly showing signs and symptoms of withdrawal. “This can be a very dangerous moment for an undisclosed addicted inpatient: the body’s tolerance declines rapidly. If they then take what was their usual dose, it can be fatal,” says Dr. Quastel. “We work hard to be open and welcoming to new patients. We are not in the blame game: shame can kill.” Advocate Donna May of mumsDU (moms united and mandated to saving the lives of Drug Users) lost her daughter, Jac, to oxycodone. Married and with three children, Jac was prescribed Oxycontin following a fall down the stairs. This led to an addiction that ruled Jac’s life and eventually to her death at the age of 35. “The stigma that was put on her and me was enormous. We fought that every day,” said Donna to the Globe and Mail. Donna now travels the country to create awareness and change.

That’s something I’ll never get back. – Michael, quoted on globalnews.ca In addition to compassionate understanding, Donna and Dr. Quastel are keen to see the opioid-antidote naloxone made more accessible. Naloxone rapidly counteracts the effects of an opioid overdose, and wider distribution would save lives since the medication must be given as fast as possible to be effective. What would Dr. Quastel hope for to reduce opioid addiction and related overdose deaths? “It’s a complicated problem with no easy solution. I’d really like to see more MDs educated to better understand and care for patients with addictions disorders. Opioids are killing people, especially young people who could go on to have rewarding lives. We need to do more.”

We spoke to Peter Gilgan, Founder and CEO of Mattamy Homes, about why he supported Inspire 2018 and his commitment to health and wellness in the communities where he builds.

Why did you support Inspire 2018 through your remarkable gift of $30 million, the largest single gift in the history of St. Michael’s? Peter: I’ve received care at St. Michael’s on a number of occasions. As a builder, I was struck by the gap between the worn appearance of parts of the hospital with the truly exceptional care I received. I was also really impressed that everyone got the same level of care, regardless of who they were or what they did. And by the warmth of all the staff: they just really seemed to care. Through my foundation I’m looking to invest in organizations that are leading in what they do. St. Mike’s, under Dr. Bob Howard’s leadership, is committed to discovering better ways to provide care. What impressed you about the patient care tower itself? Peter: At Mattamy we understand the impact of design on customer enjoyment. In a hospital setting, it’s even more crucial that design supports function. I was impressed by the scope of the concept, by the thoroughness of the process, and by the difference these inpatient and ICU units and ORs will make on the patient and staff experience. I also appreciated the notion that St. Mike’s will become the top critical care hospital in Canada. We need that leadership.
What do you hope to be remembered for? By the world, by your children? Peter: I feel I’ve been really lucky in my career, and I want to give back in ways that make a difference. It’s been a lot of fun supporting sports centres, hospitals and other projects that will make this world a better place. Giving people a chance to be fitter and healthier. I believe I’m successful because my business is really committed to solid values. I want my kids to live their lives based on good values. Speaking of fitness, you are a monster cyclist. What is it about cycling that you find so appealing? Peter: It’s a zen thing. You, the bike, the scenery flashing by, the wind. You’re concentrating on the physical effort and it washes away all the trivial stuff and lets you really think. I try to get out at least twice a week in and around the city, or wherever I happen to be. It’s like a tonic and I don’t feel 100% if I miss a couple of sessions. Try it!

HOW AN AFTERNOON OF GARDENING LED TO A LONG ROAD TO RECOVERY Gardening has been described as “the purest of human pleasures.” Ironically, Tina Cusumano’s love for gardening would dramatically change the course of her day-to-day life for more than two years. “It was a spring day like any other,” she recalls, “and I spent the majority of it in and out of the yard cleaning, planting and watering.” As evening closed in, Tina began tidying up—and slipped. The pain and swelling to her left leg was instantaneous, and as the numbness set in, she knew something was terribly wrong.

Within minutes, an ambulance arrived, rushing Tina to the nearest Emergency. Her hands still muddied from her days work, Tina was whisked away by awaiting doctors. The next morning, as she lay in her hospital bed, Tina called for her doctor. “I couldn’t feel my foot,” she says, “and I knew something more serious was happening.” Concerned that her condition was worsening, Tina’s doctor arranged for an ambulance to take her to St. Michael’s—she had severe vascular damage. Tina’s arrival at St. Michael’s was met by a team of medical professionals ready to care for her. “It certainly does take a village,” she notes, referring to the number of staff dedicated to her recovery. Among those whose expertise led to Tina’s recovery included Dr. Daniel Whelan, orthopedic surgeon; Dr. Michael Ameli, vascular surgeon; Dr. James Mahoney, plastic surgeon; and many attentive nurses, physiotherapists and occupational therapists.
