The Art of Living Well

The days are getting longer, and optimism abounds with thoughts of an early spring.

W

e’ve been fortunate, being served a mild winter— as far as coastal regions are concerned—while the deserving mountain resorts of New England have received an abundance of snowfall, along with frigid temperatures. This year’s ski and riding season will be remembered for many years, while this one carries us into April—maybe even May; then it will be time to hike to Tuckerman’s Ravine for the final turns of the season. Now that our March issue is complete, we’ve accepted an invitation from representatives of General Motors, to participate in the GMC Winter Drive Program, to be held at Stowe Vermont. A couple of our staff members will load their gear into a 2019 GMC Sierra Denali and put the vehicle through the rigors of the harsh New England environment. During the week-long excursion, they’ll go over every detail the new truck has to offer and report back their impressions of how GMC is pushing luxury and technology into another class, and ahead of the competition. 10 MARCH 2019 | NEW ENGLAND NEWS + MEDIA

With the 2019 redesign of the Sierra Denali, and its industry-first, MultiPro Tailgate-offering six functions and positions for enhanced loading, the team will plow through the backwoods of the mountainous region, out onto the ice for some fishing, then it’s to the trails for snowmobiling, which offers some of the best views in the country. Boston Design Week will begin on Wednesday, March 27, with five kick-offs in five locations, under the tent of, Then and Now. Events are scheduled in the South End, Back Bay, Seaport, Fenway, and at Fort Port. Keeping with its commitment to be inclusive to all artists, the attractions being offered to the public will include; graphic design, fashion, interior design, technical design, historic preservation, architecture, and a few surprises. Three stand-alone conferences are planned this year, highlighting various design-related topics. MITX will hosts their annual Design Summit, which will bring together the industry’s leading experts who will discuss how design thinking and principals are being applied by organizations in the business of landscape. Mad*Pow, a 2-day Health Experience Design Conference, focuses on the changing view of the design for healthcare. While a program entitled, Interventions: Narrative is a one-day symposium, hosted by students of Northeastern’s SCOUT program, will bring together creative leaders who will explore the roles that designers play as storytellers, activists, and influencers. This year’s event will also look at stainability and high fashion at an open house at The School of Fashion Design at their Back Bay location, while Serenella presents a trunk show and book signing with author Cameron Silver; this event will be held at Marc Hall Design in the South End. The Boston Design Center and Innovation and Design Building will be a key venue this year. t’s a story created from a single strand, woven into the success of a fashion house started thirty years ago. The thread is only found in the arid heights of the Gobi Desert or from live flocks of goats bearing this rare material; it’s known as cashmere. During a trip to the challenging landscape of the Mongolian steppes, Eric Bompard learned of this natural fabric, which would become his main star in the creation of the fashion house bearing his name. ospitals are big business in the state of Massachusetts, and none are as significant as the Partner’s Healthcare network which includes the famed Massachusetts General Hospital (MGH). As active as an automobile plant, having patients move through a maze of medical specialties; its doctors, support staff and administrators have taken personal health to a new level of care, while leaving no 40 MARCH 2019 | NEW ENGLAND NEWS + MEDIA

stone unturned for future advancements, many of which are on the horizon. Recently the medical center announced a lofty—but necessary—plan to invest more than $1.0 billion in the institution so they may keep pace with its astounding reputation for universal medical expertise, and to accommodate the thousands upon thousands of patients who put their trust into this world-class hospital. It doesn’t come as a surprise to many the West End of Boston—bordering on

the flat of Beacon Hill—has undergone a remarkable transformation along Cambridge Street which intersects with Storrow Drive and only feet from the Charles River. In fact, with other development taking place along this stretch of roadway, it’s only natural that MGH makes better use of the old parking garages and cramped, low-level buildings in the area. With an estimated footprint of about one million square feet, plans for the

development of the two new towers show six floors of inpatient units—with an estimated 450 new beds—operating, procedural and intervention spaces, plus imaging, exam rooms, and infusion areas. As reported by the hospital, they will be eliminating 100-200 current beds, so that the net gain will be the difference between the two estimates. A primary focus on this new area will be a continuation of the hospital’s excellent reputation in cancer and cardiac care, as

well as providing general medical, thoracic and vascular surgical services. The new buildings will also accommodate dietary, pharmacy and clinical support services, and include a café and retail space on the street level which will likely improve the hospital’s curb appeal and integrate the buildings into the façades of existing structures along Cambridge Street. In addition to the ground up expansion, planned is a six-level subterranean parking garage having the capacity to hold more than 1000 automobiles. The new development comes at a time when the Longwood Medical corridor has witness huge expansion and updating by Brigham and Women’s Hospital, and projects currently underway by Beth Israel Deaconess and Boston’s Children’s Hospital. This area may be at a distance from the steps of MGH, but they are connected by a tight-knit medical community and can be found working together when necessary. MGH has submitted plans with the city within the last few weeks, but will likely face extensive examination by state and city officials as the project moves through the process of inspection and impact studies on traffic, site planning and the effects on patient care. The estimated date for completion of this massive undertaking is marked for 2026,

with a starting date sometime in 2020. The two-hundred-plus, year-old hospital, with its access to the finest medical schools and research companies at their calling, treats people from all parts of the globe, maintains a stream of patients from outside of the state’s borders, while providing stateof-the-art diagnosis and care for residents from P-Town to the Berkshires. As a teaching hospital, it isn’t surprising that MGH is approaching $5 billion in annual income; and, while this level of medicine comes at a higher than average cost, there is no indication that this major expansion will hinder the hospitals’ ability to treat those who are unable to find treatment elsewhere. As the wheels slowly turn on the approval phase of this project, (an estimated 18 months to complete) the hospital faces challenges on meeting the needs of its current patient load due to its popularity and reputation. From all reports, business leaders and the political class all appear to be in favor of the project and from a patient’s point of view, “What’s not to like?” The growth of Boston hinges on it being viewed as a leader in medical technology and healthcare. I

f you were to use media reports as a predictor of what the future holds for the public’s diet, and are curious about the direction of future consumption, you may be left with the lasting impression that the majority of Americans will continue to slug down Solo cups of craft beer, while eating out of food trucks at fairs and festivals. By contrast, a day doesn’t go by where broadcasts report that restaurants are having difficult times resulting from everincreasing food cost, interference with their overhead—due to increased wages for unskilled to semi-skilled workers— and liquor laws which have everyone paranoid about splitting a bottle of wine with dinner and getting behind the wheel of a car. For example, recently in Boston, some of the most notable restaurants, (Durgin Park and L’Espalier come to mind), have called it quits, with owners contributing their demise to the inability to keep pace with a dwindling customer base, increased regulation, sky-high rent, and a generation who view eating through a minimalist’s stomach. Indeed, these trends are observed in pockets around the country; still, the concept of fine dining is a staple in many of the premiere restaurants found in New York and Washington D.C. Holding the coveted status of being Michelin-rated, New York City’s most opulent dining rooms fill each night with appreciative patrons who view eating as more of an art form, custom, or even a reward for personal success, rather than a human necessity. The names and faces found in these favorite locations are quite familiar, and because of this, it has become exceedingly difficult for those without notoriety to find a seat. If you’re like most people, you’ve seen the enticements to shop at nontraditional grocery stores; Costco, BJ’s, and Walmart are aggressively battling for grocery dollars, and on top of this, all competitors are moving to non-GMO and organic products to compete with Amazon’s newest venture Whole Foods. Most troubling is even with these choices, it is a sorry sight when you divide your receipt by the number of bags purchased or realize the $125 of groceries—just bought—easily fits into two small plastic bags; that is, until they are outlawed and then everything will fit into one easily torn paper bag. Price remains a battleground for the local grocery stores, many who are developing same-day delivery options. Adding pressure to traditional business models is a company that has been around since 2014—now gaining in popularity—known as Thrive Market. The online market offers; organic, non-GMO, sustainable, free-range, pasture raised, paleo, vegan, glutenfree, ketogenic, cruelty-free, and paraben-free brands for less than everyone else. And, though they have a membership fee, ($59.95), they claim that customers save somewhere around 25-50% over a retail grocer, with a guarantee you’ll get the upfront payment back—sometimes in as little as two virtual shopping trips. Added to the claim of wholesale prices, if your order totals over $49.00, you earn free shipping. This concept will weight heavy on the brick and mortar shops and is going to change the game in food purchasing by cost-conscious shoppers. With transitioning as to how we consume and efforts to force the public into politically correct shopping lists, we’re sure to see much more arguing and finger-pointing from those who are outof-touch with reality, and those who are challenged to put food on the table for themselves and their families. ADDICTI N Who’s at risk for opioid abuse? BY ROB SAINT LAURENT, MED

merica’s opioid crisis hasn’t abated. As communities continue their search for solutions, the Centers for Disease Control and Prevention said in a 2018 press release that drug deaths in the US had continued to rise, driven by synthetic opioids— illicitly made fentanyl the likely culprit.1 In the news, the party many are pointing to as the progenitor of this crisis is facing yet another lawsuit, this time by the Massachusetts attorney general’s office. At least 35 states have accused members of America’s uber-wealthy Sackler family, along with their company Purdue Pharma, for making billions off the suffering of countless families with aggressive marketing of their drug OxyContin. The latest suit claims secretly recorded evidence that the Sacklers “viewed limits on opioids as an obstacle to greater profits.” Doctors were duped into believing OxyContin was weaker than morphine that led them to over-prescribe the drug, “even as a substitute for Tylenol.”2 At the same time, despite much criticism, the US Food and Drug Administration recently approved a new form of the opioid drug sufentanil, reported to be ten times more potent than fentanyl and up to a 1,000 times stronger than morphine.3 Though doctors today are more acutely aware of opioids’ risk, it’s sensible to consider who may be susceptible to the throes of addiction before accepting a prescription for pain relief.

short-lived, however, and this can lead to a strong craving for the next rush—a sign of early addiction. Over time, the brain becomes resistant to repeated doses, making less of the feel-good chemicals and prompting the user to ever-higher amounts. Because this can now be difficult to do legally, opioid users often turn to street heroin or stronger drugs like illicit fentanyl. Because opioids’ chemical structure resembles the body’s natural opioid endorphins, like dopamine, they can attach to and stimulate brain cells. The surge of pleasure is much greater than the natural smaller bursts of everyday healthy rewards, such as from eating and social interaction. In addition to the euphoria drugs produce, science now believes dopamine also reinforces pleasurable behavior. The neurotransmitter signals a change in the brain’s circuitry that makes it easier for us to repeat that activity and form a healthy (or unhealthy) habit. Any cues associated with that habit can also become imprinted, creating a “reflex” craving when the person encounters them. This phenomenon can last years after actual drug use has stopped. ADDICTION RISK FACTORS

PATH TO ADDICTION

Importantly, “anyone who takes opioids is at risk of developing an addiction,” states Mayo Clinic.4 “It’s impossible to predict who’s vulnerable to eventual dependence.” That said, below are important factors that can increase the risk of addiction.4

BEING TREATED LIKE THE KING AND QUEEN ­­YOU ARE BY ANDREW AARON, LICSW

pon checking into a five-star hotel, guests are quick to notice the royal treatment they receive. The attentive service provides a high level of comfort and makes guests feel special, even important. The price certainly reflects such extravagant accommodation but holds value long after. Similarly, we should treat our significant others as kings and queens. Maintaining the generous and loving attention, to the same level given to them during courtship, shows a woman that she is important. Simultaneously, men ought to also be treated with the same respect. When love relationships are successful, the key to that success is due to service. What we give and the attitude with which we give it, either contributes to a loving environment between romantic partners or fails miserably. Each one of us, in part, believes that we deserve the best; some call this entitlement, but it can also be called honoring the inner king or queen. Giving your spouse the royal treatment is not spoiling them so long the giving is mutual. This form of service is about sharing, and the more attentive we are to our partner, the more we are likely to understand what makes our king or queen happy.

Some partners are confused about service and mislabel it as subservient; however, being of service does not indicate a higher or lower position nor better or worse. On the contrary, being of service raises the level of the person providing the ceremonial effort. Giving, not only communicates importance but it also expresses the unspoken thought, “I am here to make your life better.” Treating partners like kings or queens embodies the feelings and values of honor, respect, reverence, loyalty, and adoration. Such treatment may appear to be too high of a standard for some, but it does beg the question, “Wouldn’t you enjoy being treated in this manner?” Living these values means the best of us is given to another; it has the greatest potential for evoking the best in our partner and others in our lives. A couple’s greatest capacity to love is generated when both partners simultaneously respect, honor, and adore each other. The superlatives found in one lover triggers the finest in the other; lifting the bond of love higher. Both, the kingdom of love and peace, and the space of security and harmony are created in a frantic, sometimes unsafe world. If not achieved, it leaves partners feeling vulnerable and less than a royal couple. It’s these factors which prevent us from

building the harmonious and loving royal kingdom that is found in all of us. SO WHY ARE WE UNABLE TO AIM HIGH?

The reality is that in relationships, the more intimately you know your partner, the more clearly you see his or her limitations and flaws. Compounding the issue is that the longer you and your partner have been together, the more hurt or frustration has been felt. If we base our love on the degree to which a partner “deserves” it, a pattern is established, and there is a creation of turmoil in which partners are treated like paupers or prisoners rather than royalty. In its purest form, love is unconditional. It’s when conditions are placed on love, such as, “I will love you when I think you are good enough,” the relationship will begin to devolve to a low state. Offering love and attention at the highest level possible requires effort, but it offers less pain than living in a poor-quality relationship and attempting to repair once it has fallen apart. Reciprocal and simultaneous royal treatment is the “secret” to a lasting, happy relationship. Go ahead, honor and revere your partner and give each other the fivestar treatment. This year presenters include Gish Jen, who is a second generation Chinese-American, born on Long Island, New York, and raised in Queens, Yonkers, and Scarsdale. She received a bachelor’s degree in English from Harvard University in 1977, and an MFA in fiction from the University of Iowa’s Writing Workshop in 1983. Jen has authored many short stories; her Birthmates was singled out by John Updike as

“one of the best American short stories of the century.” She has also written six novels; her fifth novel, World and Town, won the 2011 Massachusetts Book Prize in fiction and was nominated for the 2012 International IMPAC Dublin Literary Award. Her second work of non-fiction, The Girl at the Baggage Claim: Explaining the EastWest Culture Gap, is a provocative study of the differences in which Easterners and Westerners feel about self and society. The piece assesses how these differences impact one’s perception and the effect it has on how one speaks and acts. Jen received the Lannan Literary Award for fiction in 1999 and was honored in 2017 with the Legacy Award by the Museum of the Chinese in America in New York. Also presenting is Susan Wissler, the Executive Director of The Mount Edith Wharton’s estate in the Berkshires. Brought back from the brink of foreclosure, Wissler


is credited to turning the location into a cultural center for the arts. Boasting over 40,000 guests attending music and lecture series at The Mount each year, Wissler contributes this accomplishment to the assistance of Jean-Claude Lesage, husband of the late Claudine Lesage, who with him, completed— as co-authors

Wolk also worked as a senior editor with an educational publisher in Toronto before beginning a family and a writing career. In 1999, Random House published her first novel, and a short time later, she became a full-time English teacher at Sturgis Charter School in Hyannis. In 2004 she left teaching to become the Assistant Director at the Cape Cod Writers Center.

The First Million is the Hardest… BY ROBERT WRIGHT

T

hese were the words spoken to me—while I sat across the desk—by a customer, who had immense wealth and an equal amount of financial knowledge, all of which he leveraged to create even greater riches. At the time I had significant debt but was able to finance a delightful lifestyle. I had a beautiful boat, a Mercedes-Benz, and a lease on a Boston apartment; but, I discovered I lacked a plan for turning my static financial existence into a path designed to create long-lasting wealth. What this means is that I spent every dime I made and was keeping up appearances, I had ignored one of my life-long objectives—to become wealthy. Yes, I was told that creating wealth is a formidable task, even though many people earn that amount over their lifetime. I learned that making money isn’t the real challenge; instead, keeping your hands safely around the cash and investing it so that it will grow into millions is the real challenge. At that time I was learning all of this, I was enthralled with the discussion; attentive and optimistic, but I knew that if I had to raise even a measly $10,000—I couldn’t do it at the time. 54 MARCH 2019 | NEW ENGLAND NEWS + MEDIA

See, I was on the payment-plan for living a lifestyle. It’s a trap that is intended to snare those who earn a significant income but are locked into paying for “things” on a monthly schedule, which will, last a lifetime. Dates, dinners, and trips always fit nicely on a credit card, up and until the remorse sets in. None of these “investments” will be around when it is time to pay, either in full or worse, the monthly minimum. It would be delightful to say that I had an epiphany and suddenly discovered that all my money worries ended, that I had changed my stripes and went on to a life of success and leisure; but, it didn’t happen that way. There are few people—starting in a new financial direction—who can turn an idea into a million dollars overnight. In my time around multi-millionaires and quite a few billionaires, not one of them lays claim to a lightbulb going off in their heads, then suddenly selling the rights to a revolutionary product or service. To the contrary, each of them toughed it out, making changes to their lives so they could accomplish their financial objectives. Early on, I recognized that these people all shared a single characteristic. I learned that most wealthy people have unabashed respect for money, and they don’t believe that it should be spent without careful consideration, regardless of the size of the purchase or the amount they possess. Each of these people held the belief that money is to be invested so that it multiples or compounds. For evidence of this, be sure to read about Warren Buffett’s take on compounding.

Last month’s article mentioned that if you wish to achieve wealth, it is best to have a partner who either is cheaper than you or already respects money, because they have to cherish what they earn, and have learned valuable lessons, some of the same ones you’ll need to pay attention to. I recognize that of all my partners, the only one that looked out for me had an unparalleled reverence to saving and the procurement of money. It was not until we


met, that I realized the potential I had to reach my goals, all else was a smokescreen. LESSON 4. EAT LESS.

I’ve dined with very wealthy people, most of whom could afford a feast fit for royalty. Want to know what we ate? How about everything from pizza, burgers, and steaks, to all-you-can-eat-buffets; never in a five-star dining room, unless of course, someone else was paying for the meal. You’d be surprised at how often rich people eat for free, and it makes sense, they realize they can sell knowledge, often for the price of lunch or dinner. They can be found looking for free food as a means for cutting back on their monthly grocery expenses. It’s likely we could all lose a couple of pounds so that eating less isn’t the hardship it sounds like, at best, it will pay high dividends all around. Another suggestion to consider is if you enjoy that second cup of coffee at work, instead purchase a hot/cold thermos or an insulated travel mug. Untold benefits will result from this action. Perhaps you might have five to 10 cups of take-out coffee each week; if you multiply those numbers by 52 weeks, it could be $260-$520 servings annually. If you figure that each drink costs approximately $2.25, (which is at the lower end for most places), a fair estimate is that you will spend almost, $585-$1170 in twelve months. Moving on to lunch; with an average allowance of $5-$10 daily, and keeping with the same math as above, the money to be saved is about $1300-$2600, yearly. Using these readily and acceptable means for cutting expenses will yield somewhere between $1885-$3770 annually. Even extracting the cost of the coffee, mug, and a brown-bag lunch, you’re still ahead of the game by at least two to three grand. Not much consideration goes into the loss of capital due to overlooked daily habits such as grabbing coffee or lunch prepared by someone else. However, this is one of the most significant money drains that you have control over. Here is what you need to consider; if you were to invest the savings mentioned, into shares of Amazon during the last 1214 months, your current return on investment, (ROI) would be in the vicinity of 90-110%, and that is for a single year of equity. Yes, you would have doubled your money by doing relatively nothing. We’ll discuss more ideas like this later in the series.

Taking opioids for too long. Research shows that taking opioids for more than three days raises one’s chances of long-term use, increasing chances of addiction. Using opioids from just a Monday to Friday (five days) increases odds of being on them a year later.

Over time, the brain becomes resistant to repeated doses, making less of the feel-good chemicals and prompting the user to everhigher amounts.

Being female. Women are at particular risk for opioid addiction. They are more likely to experience chronic pain and be given opioids, and their prescriptions are generally at higher doses and for longer periods than men. Moreover, the Office on Women’s Health of the US Department of Health and Human Services explains that women may also be at increased risk for substance abuse for biological reasons. Less body water/higher body fat, leading to higher blood levels of drugs and alcohol; quicker metabolizing of medications; hormonal variations; and a potentially higher susceptibility to opioid cravings, for example.6

Being 18-64 years of age. The Substance Abuse and Mental Health Services Administration (SAMHSA) says those within this age group, both young and older, are at increased risk for opioid dependence or abuse.7 The Centers for Disease Control and Prevention say 18 to 25-year-olds, in particular, have the highest rate of chronic nonmedical opioid use.8 Being white. Non-Hispanic whites are at elevated risk for opioid abuse or dependence, as well as overdosing (overdose risk especially high in middle-aged non-Hispanic whites, 45-54 years).7 History of substance abuse. If a parent or other close relative was abusing drugs, chances are the next generation will be predisposed to that behavior. The tendency to addictive behavior also holds true from a person’s history of substance abuse. Cues to addictive behavior. Frequent association with the wrong crowd (high-risk people), or being in the wrong environment, can trigger abuse. Income, education, marital and employment status, and living situation. SAMHSA describes people with low income and less education, those not married, those unemployed, people without permanent housing, and those living in cities as being at higher risk for opioid abuse and overdose.7 Prone to risky behavior. Research by the CDC has shown, for example, that teens who abuse prescription drugs recreationally are more likely to engage in risky sexual behaviors.9 Stressful circumstances and a history of severe depression or anxiety. We know there is a link between personal stress

and initiating drug abuse. Similarly, those prone to depression or anxiety are more likely to resort to narcotics abuse. (Depression often occurs concurrently with opioid abuse.) Mental health issues. For example, the Mayo Clinic states prior problems with employers, family, and friends. History of legal problems or criminal activity. Women released from prison, for instance, are at increased risk for overdose and substance misuse.7

After studying patients at a chronic pain clinic, Dr. Lynn Webster, author of Avoiding Opioid Abuse While Managing Pain (2007), has devised a validated opioid risk assessment tool that “estimates risk of opioid-related aberrant behavior.”10 While not a substitute for medical advice and mainly for chronic pain, the online calculator can help identify who may be highly vulnerable to abuse and might instead benefit from other forms of pain relief. “NO ONE IS SAFE”

Mayo Clinic says opioids are safest for acute pain, such as after surgery or breaking a bone, and used for no more than three days at the lowest possible dose.4 Opioids “are not likely to be a safe and effective long-term treatment option,” however for chronic pain, the Mayo Clinic advises less-addictive drugs and non-drug options. For example, physical/exercise therapy can be a safer and more effective option for managing long-term pain, along with its many secondary benefits like weight loss and better health. Additionally, safe-keeping of all opioids currently being used is recommended, with proper disposal of unused pills. For those who recognize they have a problem, they advise talking to one’s prescribing doctor about gradually reducing medication and alternative pain therapies, and for a referral to an addiction specialist. The Mayo Clinic recognizes that anyone can become addicted to opioids and all of us play a role in combating this crisis.
