The law prohibits the NYU Tandon School of Engineering from allowing any student to register for classes without documented proof of immunization against these diseases. Students must have the immunization form completed by the physician or health care provider who administered their vaccinations. Once completed, the Immunization Record should be submitted (via mail, fax or in-person) to: NYU Student Health Center Immunization Record Services 726 Broadway, Suite 336 New York, NY 10003 Phone: (212) 443-1199 Fax: (212) 443-1198 To expedite your registration, you should complete this process well in advance of your arrival at the NYU Tandon School of Engineering. For more information or to download the forms, visit: engineering.nyu.edu/grad/health_requirements
