Prevent spread of illness in your family this season

The holidays can be a magical time of year for families — unless everyone’s passing around a cold or flu bug.

“There are a few simple things you can do to minimize the spread of illness during the winter,” said Craig Webb, MD, FAAP, pediatrician with Kaiser Permanente. Webb joined the Kaiser Permanente Parkside Medical offices, 215 S. Parkside Drive, in September.

First, get a flu shot. “I strongly encourage all parents and children down to 6 months of age to get immunized,” he said.

Second, pay attention to personal hygiene. “As we enter cold and flu season, the best thing you can do for yourself and others is wash your hands,” Dr. Webb said. “Teach your young children to cough and sneeze into their elbows, rather than hands.

It’s also important to make sure children are appropriately dressed and prepared for whatever temperatures might occur that day.”

If your family does catch a cold, do what you can to stop it from spreading. “If you’re ill, stay home from school or work to keep others safe,” Dr. Webb said.

For most kids, a trip to the doctor’s office is not necessary, though it’s important to know the signs of a dangerous cold or flu. “Most children can receive home health care if they have a runny nose lasting less than 10 days or a cough that interferes a bit but doesn’t cause them to stop moving,” Dr. Webb said. “Fevers up to 103 degrees are OK, as long as the child is over 6 months in age.”

Take your child to the pediatrician if his fever is 100.4 degrees or higher and he is under 3 months old or hits 101 degrees or higher at 3– 6 months old. “If a child is having difficulty breathing, that something your pediatrician definitely needs to take a look at.”

Holiday safety doesn’t end with avoiding the flu. “As we head into the Thanksgiving and Christmas

seasons, we want to focus on safety in the house, especially in the kitchen,” Dr. Webb said. “It can be a dangerous place for children, especially the very small ones.” Dr. Webb recommended placing one adult in charge of keeping undistracted eyes on kids in the kitchen during holiday gatherings.

“Most importantly, keep your knives and heavy items and dangerous utensils out of reach.”

Dr. Webb, a Colorado Springs native, was born on the Air Force Academy and recently returned to the city after retiring as a colonel from the U.S. Army. “The setting of Colorado Springs is fantastic for raising children,” Webb said. “We have 350 days of sunshine and a real emphasis on being outside and enjoying the mountains,” he said. “I also believe while we are a nice-sized city, we have a small-town feel, and that goes far when it comes to nourishing and mentoring our children as they grow.”

Dr. Webb joins Kaiser Permanente in the midst of its expansion across

southern Colorado, including the Parkside Medical offices, which opened in January. “In this facility, you can see a doctor, visit the lab, go to the pharmacy; it’s great, especially from a pediatric standpoint. Mom doesn’t have to run all over town, saving families time and money,” he said.

A large portion of Dr. Webb’s patients are under age 2 and visit him for routine checkups. He also sees older children and teenagers for annual school and sports physicals, which emphasize Kaiser’s focus on preventative medicine, exercise and nutrition, and works with children who have chronic diseases, from asthma to ADHD. “We’re well positioned to handle anything a child might be dealing with,” he said. “We also have great working relationships with a variety of pediatric specialists in both Colorado Springs and Denver for any special cases and needed referrals.”

Survivor Profile: Calcium Score Scan discovers heart blockage

If Marty Townsend could give any piece of advice, it would be not to put all your eggs in one basket — so to speak. “If something is wrong, don’t rely on the results of one test,” he said. “I relied on just one test and it could have killed me.”

The first time Townsend experienced chest pain was in October 2011; he was walking into the Pepsi Center in Denver to watch a Colorado Avalanche hockey game. “I suddenly felt a powerful constriction in my chest,” he said.

As soon as he got home, Townsend scheduled an appointment with his primary physician, who suggested he undergo a chest stress test. The test showed no abnormalities and healthy heart chambers. Over the next three years, Townsend’s chest pains only intensified, to the point he had to stop mid workout. He was playing racquetball three days a week and alternating cardio and weight exercise.

“I was diagnosed with Parkinson’s disease, and part of the therapy is you have to exercise, so I was working out about two hours a day,” Townsend

said. “But, I’ve never been a couch potato.” The pain got so bad, he decided to get it checked out again.

Townsend had another stress echo test, which he couldn’t even complete due to chest pain. “The ultrasound indicated no blockages or problems,” he said. “Fortunately, my cardiologist had told me about the Calcium Score Scan.” Townsend had the test done in December 2014.

As soon as his doctor received the results, he called Townsend and told him to stop exercising immediately and take a few Nitroglycerin tablets.

He scheduled Townsend for an angiogram and possible angioplasty the following week. Though he has no family history of heart disease and has always been active, the camera used in the angiogram found a 95-percent blockage in one of Townsend’s arteries – the cause of his chest pain. Christian Simpfendorfer, MD, a cardiologist at Penrose-St. Francis, inserted a stent into the affected artery, and Townsend was cleared to get back out on the racquetball court the next month. ✚

NAVIGATING HOME HEALTHCARE:

Understanding private vs. agency-based care for older adults

When caring for aging parents, many adult children are unsure where to turn for in-home care. The myriad of choices can be Dorothy Silvanic overwhelming.

When broken down, there are three key options: private hire, domestic referral agencies and full-service companies.

PRIVATE HIRE CAREGIVER

When it comes to hiring a private caregiver, family members are responsible for all steps of the hiring process. They must advertise, interview and conduct background checks.

Once a caregiver is selected, the older adult or family member must negotiate schedule, salary and benefits. A written contract, signed by both parties, clearly outlining all of the duties and expectations, is imperative. Liability normally falls on the client’s lap when it comes to payroll taxes, social security, unemployment taxes and workers compensation cost.

It is important to get a rider on your homeowner’s insurance policy to protect your assets if anything should happen to the private hire in your home. A private hire might be cost effective, but safety risks are greater, especially when an older adult lives alone or has impaired judgment. The risk is greatly reduced when a family member is able to supervise the caregiver. Professional geriatric care managers are also excellent supervisors when family is not available.

DOMESTIC REFERRAL AGENCIES

Domestic referral agencies are essentially registries. A client calls for service and a caregiver is placed in a home for a placement fee and sometimes an ongoing management fee. The referral agencies do not employ the caregiver. The client becomes the employer, and similar to a private hire, is responsible for payroll taxes, social security, and unemployment insurance and workers compensation.

Some referral agencies have trust accounts set up, in which the client pays the agency and the agency then pays the caregiver, and sometimes sets

up a tax service for the client, but the client is still considered the employer.

Since referral agencies are not employers, they do not supervise the caregiver. This increases the safety risk. Like a private hire, it is important that a family members or geriatric care manager supervise and oversee care.

FULL SERVICE AGENCIES

Full service agencies employ caregivers. They hire, screen, train and supervise the employees and place them in the older adult’s home. As the employer, the full service agency is responsible for all payroll taxes, social security, worker compensation, and unemployment insurance. A quality full service agency should be able to produce a copy of its liability insurance and carry some sort of bond.

Supervision is done by the agency, with some agencies providing care managers who directly oversee client cases, lowering the risk to the consumer. The cost of a full service agency is normally higher than a private duty hire or a domestic referral agency, but ultimately, the risks are lower.

When choosing homecare for yourself or an older adult, remember to take into consideration the risk level of the older adult and the familial involvement.

Educate yourself on what service is best for your situation and then interview several potential individuals or agencies. Finally, think ahead, don’t wait for a crisis to begin searching for the best care for you or your loved one. ✚ Dorothy Silvanic is a 25-year veteran of the U.S. Air Force, serving primarily as an Aircraft Maintenance Officer with one assignment at the USAF Academy as a Group AOC.
